A deep learning reconstruction framework for low dose phase contrast computed tomography via inter-contrast enhancement

Phase contrast computed tomography (PCCT) offers excellent imaging contrast on soft tissue while it generate absorption, phase and dark-field contrast tomographic images. It has shown a great potential in clinical diagnosis. However, existing PCCT methods require high radiation doses. Reducing tube current is a universal low dose approach while it will introduce quantum noise in projections. In this paper, we report a deep learning (DL) framework for low dose PCCT based on inter-contrast enhancement. It utilizes the multi-contrast feature of PCCT and the varying effects of noise on each contrast. The missing structure in the contrasts that are more affected by noise can be recovered by those that are less affected. Considering the grating-based PCCT as example, the proposed framework is validated with experiments and a dramatic quality improvement of multi-contrast tomographic images has been obtained. This study shows potential of DL techniques in the field of low dose PCCT.

Towards virtual histology with X-ray grating interferometry

Breast cancer is the most common type of cancer worldwide. Diagnosing breast cancer relies on clinical examination, imaging and biopsy. A core-needle biopsy enables a morphological and biochemical characterization of the cancer and is considered the gold standard for breast cancer diagnosis. A histopathological examination uses high-resolution microscopes with outstanding contrast in the 2D plane, but the spatial resolution in the third, Z-direction, is reduced. In the present paper, we propose two high-resolution table-top systems for phase-contrast X-ray tomography of soft-tissue samples. The first system implements a classical Talbot–Lau interferometer and allows to perform ex-vivo imaging of human breast samples with a voxel size of 5.57 μm. The second system with a comparable voxel size relies on a Sigray MAAST X-ray source with structured anode. For the first time, we demonstrate the applicability of the latter to perform X-ray imaging of human breast specimens with ductal carcinoma in-situ. We assessed image quality of both setups and compared it to histology. We showed that both setups made it possible to target internal features of breast specimens with better resolution and contrast than previously achieved, demonstrating that grating-based phase-contrast X-ray CT could be a complementary tool for clinical histopathology.

Learning from Projection to Reconstruction: A Deep Learning Reconstruction Framework for Sparse-View Phase Contrast Computed Tomography via Dual-Domain Enhancement

Phase contrast computed tomography (PCCT) provides an effective non-destructive testing tool for weak absorption objects. Limited by the phase stepping principle and radiation dose requirement, sparse-view sampling is usually performed in PCCT, introducing severe artifacts in reconstruction. In this paper, we report a dual-domain (i.e., the projection sinogram domain and image domain) enhancement framework based on deep learning (DL) for PCCT with sparse-view projections. It consists of two convolutional neural networks (CNN) in dual domains and the phase contrast Radon inversion layer (PCRIL) to connect them. PCRIL can achieve PCCT reconstruction, and it allows the gradients to backpropagate from the image domain to the projection sinogram domain while training. Therefore, parameters of CNNs in dual domains are updated simultaneously. It could overcome the limitations that the enhancement in the image domain causes blurred images and the enhancement in the projection sinogram domain introduces unpredictable artifacts. Considering the grating-based PCCT as an example, the proposed framework is validated and demonstrated with experiments of the simulated datasets and experimental datasets. This work can generate high-quality PCCT images with given incomplete projections and has the potential to push the applications of PCCT techniques in the field of composite imaging and biomedical imaging.

In-situ deformation imaging of articular cartilage using grating-based phase-contrast X-ray CT at a synchrotron light source

Quantification of deformation behavior of articular cartilage (AC) is crucial for understanding its mechanical function and response to mechanical stimuli. Here, we explored whether grating-based phase-contrast X-ray CT using monochromatic synchrotron light (grating-based msPXCT) enables in-situ quantification of local deformation in AC. Grating-based msPXCT of a porcine AC sample during axial compression test was conducted using a Talbot grating interferometer. Local displacements and strains were computed using a digital volume correlation method. The magnitude of axial strain decreased from the upper to middle sample zones and reached almost constant over the middle-lower zone, consistent with the depth-dependent density increase with compression. Thus, grating-based msPXCT may be suitable for quantitative analysis of AC deformation.

Single spectrum three-material decomposition with grating-based x-ray phase-contrast CT

Grating-based x-ray phase-contrast imaging provides three simultaneous image channels originating from a single image acquisition. While the phase signal provides direct access to the electron density in tomography, there is additional information on sub-resolutional structural information which is called dark-field signal in analogy to optical microscopy. The additional availability of the conventional attenuation image qualifies the method for implementation into existing diagnostic routines. The simultaneous access to the attenuation coefficient and the electron density allows for quantitative two-material discrimination as demonstrated lately for measurements at a quasi-monochromatic compact synchrotron source. Here, we investigate the transfer of the method to conventional polychromatic x-ray sources and the additional inclusion of the dark-field signal for three-material decomposition. We evaluate the future potential of grating-based x-ray phase-contrast CT for quantitative three-material discrimination for the specific case of early stroke diagnosis at conventional polychromatic x-ray sources. Compared to conventional CT, the method has the potential to discriminate coagulated blood directly from contrast agent extravasation within a single CT acquisition. Additionally, the dark-field information allows for the clear identification of hydroxyapatite clusters due to their micro-structure despite a similar attenuation as the applied contrast agent. This information on materials with sub-resolutional microstructures is considered to comprise advantages relevant for various pathologies.

Inner Focus Iterative Reconstruction Method with the Interlaced Phase Stepping Scanning for Grating-based Phase Contrast Tomography.

The potential clinical application of grating-based phase contrast computed tomography (GPCCT) requires moderate scanning time to reduce the radiation dose, which are not met by traditional GPCCT phase stepping (PS) method. Previous studies have proposed the interlaced scanning method to reduce the scanning time. However, due to the projection number demanded by the analysis reconstruction algorithm, the projection and scanning time cannot be further reduced. In this paper, we proposed an iterative algorithm based on the interlaced PS scanning for GPCCT, which was capable in reducing the motion artifacts during reconstruction as the same as the inner focus (IF) method we raised before. Furthermore, the iterative procedure is expected to introduce some machine learning method and allows a lower radiation dose while maintaining the image quality. Our proposed method mainly consists of three steps: 1) Interlaced data acquisition, 2) Phase retrieval, 3) Inner focus iterative reconstruction. Through changing the virtual rotation center and merging high resolution regions, images without severe boundary blurring can be reconstructed with fast scan speed. The experiment result indicates that our method can reconstruct GPCCT data with interlaced PS scanning.

Assessment of intraductal carcinoma in situ (DCIS) using grating-based X-ray phase-contrast CT at conventional X-ray sources: An experimental ex-vivo study.

BackgroundThe extent of intraductal carcinoma in situ (DCIS) is commonly underestimated due to the discontinuous growth and lack of microcalcifications. Specimen radiography has been established to reduce the rate of re-excision. However, the predictive value for margin assessment with conventional specimen radiography for DCIS is low. In this study we assessed the potential of grating-based phase-contrast computed tomography (GBPC-CT) at conventional X-ray sources for specimen tomography of DCIS containing samples.Materials and methodsGBPC-CT was performed on four ex-vivo breast specimens containing DCIS and invasive carcinoma of non-specific type. Phase-contrast and absorption-based datasets were manually matched with corresponding histological slices as the standard of reference.ResultsMatching of CT images and histology was successful. GBPC-CT showed an improved soft tissue contrast compared to absorption-based images revealing more histological details in the same sections. Non-calcifying DCIS exceeding the invasive tumor could be correlated to areas of dilated bright ducts around the tumor.ConclusionsGBPC-CT imaging at conventional X-ray sources offers improved depiction quality for the imaging of breast tissue samples compared to absorption-based imaging, allows the identification of diagnostically relevant tissue details, and provides full three-dimensional assessment of sample margins.

Direct quantitative material decomposition employing grating-based X-ray phase-contrast CT

Dual-energy CT has opened up a new level of quantitative X-ray imaging for many diagnostic applications. The energy dependence of the X-ray attenuation is the key to quantitative material decomposition of the volume under investigation. This material decomposition allows the calculation of virtual native images in contrast enhanced angiography, virtual monoenergetic images for beam-hardening artifact reduction and quantitative material maps, among others. These visualizations have been proven beneficial for various diagnostic questions. Here, we demonstrate a new method of ‘virtual dual-energy CT’ employing grating-based phase-contrast for quantitative material decomposition. Analogue to the measurement at two different energies, the applied phase-contrast measurement approach yields dual information in form of a phase-shift and an attenuation image. Based on these two image channels, all known dual-energy applications can be demonstrated with our technique. While still in a preclinical state, the method features the important advantages of direct access to the electron density via the phase image, simultaneous availability of the conventional attenuation image at the full energy spectrum and therefore inherently registered image channels. The transfer of this signal extraction approach to phase-contrast data multiplies the diagnostic information gained within a single CT acquisition. The method is demonstrated with a phantom consisting of exemplary solid and fluid materials as well as a chicken heart with an iodine filled tube simulating a vessel. For this first demonstration all measurements have been conducted at a compact laser-undulator synchrotron X-ray source with a tunable X-ray energy and a narrow spectral bandwidth, to validate the quantitativeness of the processing approach.

High resolution laboratory grating-based X-ray phase-contrast CT

The conventional form of computed tomography using X-ray attenuation without any contrast agents is of limited use for the characterization of soft tissue in many fields of medical and biological studies. Grating-based phase-contrast computed tomography (gbPC-CT) is a promising alternative imaging method solving the low soft tissue contrast without the need of any contrast agent. While highly sensitive measurements are possible using conventional X-ray sources the spatial resolution does often not fulfill the requirements for specific imaging tasks, such as visualization of pathologies. The focus of this study is the increase in spatial resolution without loss of sensitivity. To overcome this limitation a super-resolution reconstruction based on sub-pixel shifts involving a deconvolution of the image data during each iteration is applied. In our study we achieve an effective pixel size of 28 μm with a conventional rotating anode tube and a photon-counting detector. We also demonstrate that the method can upgrade existing setups to measure tomographies with higher resolution. The results show the increase in resolution at high sensitivity and with the ability to make quantitative measurements. The combination of sparse sampling and statistical iterative reconstruction may be used to reduce the total measurement time. In conclusion, we present high-quality and high-resolution tomographic images of biological samples to demonstrate the experimental feasibility of super-resolution reconstruction.

Accurate effective atomic number determination with polychromatic grating-based phase-contrast computed tomography.

The demand for quantitative medical imaging is increasing in the ongoing digitalization. Conventional computed tomography (CT) is energy-dependent and therefore of limited comparability. In contrast, dual-energy CT (DECT) allows for the determination of absolute image contrast quantities, namely the electron density and the effective atomic number, and is already established in clinical radiology and radiation therapy. Grating-based phase-contrast computed tomography (GBPC-CT) is an experimental X-ray technique that also allows for the measurement of the electron density and the effective atomic number. However, the determination of both quantities is challenging when dealing with polychromatic GBPC-CT setups. In this paper, we present how to calculate the effective atomic numbers with a polychromatic, laboratory GBPC-CT setup operating between 35 and 50\,kVp. First, we investigated the accuracy of the measurement of the attenuation coefficients and electron densities. For this, we performed a calibration using the concept of effective energy. With the reliable experimental quantitative values, we were able to evaluate the effective atomic numbers of the investigated materials using a method previously shown with monochromatic X-ray radiation. In detail, we first calculated the ratio of the electron density and attenuation coefficient, which were experimentally determined with our polychromatic GBPC-CT setup. Second, we compared this ratio with tabulated total attenuation cross sections from literature values to determine the effective atomic numbers. Thus, we were able to calculate two physical absolute quantities -- the electron density and effective atomic number -- that are in general independent of the specific experimental conditions like the X-ray beam spectrum or the setup design.

Tilted grating phase-contrast computed tomography using statistical iterative reconstruction

Grating-based phase-contrast computed tomography (GBPC-CT) enables increased soft tissue differentiation, but often suffers from streak artifacts when performing high-sensitivity GBPC-CT of biomedical samples. Current GBPC-CT setups consist of one-dimensional gratings and hence allow to measure only the differential phase-contrast (DPC) signal perpendicular to the direction of the grating lines. Having access to the full two-dimensional DPC signal can strongly reduce streak artefacts showing up as characteristic horizontal lines in the reconstructed images. GBPC-CT with gratings tilted by 45° around the optical axis, combining opposed projections, and reconstructing with filtered backprojection is one method to retrieve the full three-dimensional DPC signal. This approach improves the quality of the tomographic data as already demonstrated at a synchrotron facility. However, additional processing and interpolation is necessary, and the approach fails when dealing with cone-beam geometry setups. In this work, we employ the tilted grating configuration with a laboratory GBPC-CT setup with cone-beam geometry and use statistical iterative reconstruction (SIR) with a forward model accounting for diagonal grating alignment. Our results show a strong reduction of streak artefacts and significant increase in image quality. In contrast to the prior approach our proposed method can be used in a laboratory environment due to its cone-beam compatibility.

Diagnosis of breast cancer based on microcalcifications using grating-based phase contrast CT.

Microcalcifications are an important feature in the diagnosis of breast cancer, especially in the early stages. In this paper, a CT-based method is proposed to potentially distinguish benign and malignant breast diseases based on the distributions of microcalcifications using grating-based phase-contrast imaging on a conventional X-ray tube. The method presented based on the ratio of dark-field signals to attenuation signals in CT images is compared with the existing method based on the ratio in projections, and the threshold for the classification of microcalcifications in the two types of breast diseases is obtained using our approach. The experiment was operated on paraffin-fixed specimens that originated from 20 female patients ranging from 27-65years old. Compared with the method based on projection images (AUC = 0.87), the proposed method is more effective (AUC = 0.95) to distinguish the two types of diseases. The discrimination threshold of microcalcifications for the classification of diseases in CT images is found to be 3.78 based on the Youden index. The proposed method can be further developed to improve the early diagnosis and diagnostic accuracy and reduce the clinical misdiagnosis rate of breast cancer. • Microcalcifications are of special importance to indicate early breast cancer. • Grating-based phase-contrast imaging can improve the diagnosis of breast cancers. • The method described here can better classify benign and malignant breast diseases.

Qualitative and Quantitative Imaging Evaluation of Renal Cell Carcinoma Subtypes with Grating-based X-ray Phase-contrast CT

Current clinical imaging methods face limitations in the detection and correct characterization of different subtypes of renal cell carcinoma (RCC), while these are important for therapy and prognosis. The present study evaluates the potential of grating-based X-ray phase-contrast computed tomography (gbPC-CT) for visualization and characterization of human RCC subtypes. The imaging results for 23 ex vivo formalin-fixed human kidney specimens obtained with phase-contrast CT were compared to the results of the absorption-based CT (gbCT), clinical CT and a 3T MRI and validated using histology. Regions of interest were placed on each specimen for quantitative evaluation. Qualitative and quantitative gbPC-CT imaging could significantly discriminate between normal kidney cortex (54 ± 4 HUp) and clear cell (42 ± 10), papillary (43 ± 6) and chromophobe RCCs (39 ± 7), p < 0.05 respectively. The sensitivity for detection of tumor areas was 100%, 50% and 40% for gbPC-CT, gbCT and clinical CT, respectively. RCC architecture like fibrous strands, pseudocapsules, necrosis or hyalinization was depicted clearly in gbPC-CT and was not equally well visualized in gbCT, clinical CT and MRI. The results show that gbPC-CT enables improved discrimination of normal kidney parenchyma and tumorous tissues as well as different soft-tissue components of RCCs without the use of contrast media.

Fully 3D geometrical calibration foranX-ray grating-based imaging system.

The Precision of geometric parameters is the key requirement for grating-based phase-contrast CT and attenuation-based CT in cone-beam geometry. By extending our fan-beam geometric calibration work via Locally Linear Embedding (LLE) into the cone-beam case, here we calibrate the geometric parameters of our in-house phase-contrast CT system with attenuation projection data. Numerical and experimental studies show that the LLE-based calibration method is feasible to calibrate 8 parameters of the cone-beam geometry that improves reconstruction quality significantly.

Bilateral filtering using the full noise covariance matrix applied to x-ray phase-contrast computed tomography

The purpose of this work is to develop an image-based de-noising algorithm that exploits complementary information and noise statistics from multi-modal images, as they emerge in x-ray tomography techniques, for instance grating-based phase-contrast CT and spectral CT.Among the noise reduction methods, image-based de-noising is one popular approach and the so-called bilateral filter is a well known algorithm for edge-preserving filtering. We developed a generalization of the bilateral filter for the case where the imaging system provides two or more perfectly aligned images. The proposed generalization is statistically motivated and takes the full second order noise statistics of these images into account. In particular, it includes a noise correlation between the images and spatial noise correlation within the same image.The novel generalized three-dimensional bilateral filter is applied to the attenuation and phase images created with filtered backprojection reconstructions from grating-based phase-contrast tomography. In comparison to established bilateral filters, we obtain improved noise reduction and at the same time a better preservation of edges in the images on the examples of a simulated soft-tissue phantom, a human cerebellum and a human artery sample. The applied full noise covariance is determined via cross-correlation of the image noise.The filter results yield an improved feature recovery based on enhanced noise suppression and edge preservation as shown here on the example of attenuation and phase images captured with grating-based phase-contrast computed tomography. This is supported by quantitative image analysis. Without being bound to phase-contrast imaging, this generalized filter is applicable to any kind of noise-afflicted image data with or without noise correlation. Therefore, it can be utilized in various imaging applications and fields.

AHA classification of coronary and carotid atherosclerotic plaques by grating-based phase-contrast computed tomography.

To evaluate the potential of grating-based phase-contrast computed-tomography (gb-PCCT) to classify human carotid and coronary atherosclerotic plaques according to modified American Heart Association (AHA) criteria. Experiments were carried out at a laboratory-based set-up consisting of X-ray tube (40kVp), grating-interferometer and detector. Eighteen human carotid and coronary artery specimens were examined. Histopathology served as the standard of reference. Vessel cross-sections were classified as AHA lesion type I/II, III, IV/V, VI, VII or VIII plaques by two independent reviewers blinded to histopathology. Conservative measurements of diagnostic accuracies for the detection and differentiation of plaque types were evaluated. A total of 127 corresponding gb-PCCT/histopathology sections were analyzed. Based on histopathology, lesion type I/II was present in 12 (9.5%), III in 18 (14.2%), IV/V in 38 (29.9%), VI in 16 (12.6%), VII in 34 (26.8%) and VIII in 9 (7.0%) cross-sections. Sensitivity, specificity and positive and negative predictive value were ≥0.88 for most analyzed plaque types with a good level of agreement (Cohen's kappa = 0.90). Overall, results were better in carotid (kappa = 0.97) than in coronary arteries (kappa = 0.85). Inter-observer agreement was high with kappa = 0.85, p < 0.0001. These results indicate that gb-PCCT can reliably classify atherosclerotic plaques according to modified AHA criteria with excellent agreement to histopathology. • Different atherosclerotic plaque types display distinct morphological features in phase-contrast CT. • Phase-contrast CT can detect and differentiate AHA plaque types. • Calcifications caused streak artefacts and reduced sensitivity in type VI lesions. • Overall agreement was higher in carotid than in coronary arteries.

Simulated cystic renal lesions: quantitative X-ray phase-contrast CT--an in vitro phantom study.

To determine if grating-based x-ray phase-contrast computed tomography (CT) can allow differentiation of simulated simple, protein-rich, hemorrhagic, and enhancing cystic renal lesions in an in vitro phantom. An in vitro phantom specifically designed to simulate simple, protein-rich, hemorrhagic, and enhancing renal cysts was scanned with an experimental grating-based phase-contrast CT setup consisting of a Talbot-Lau interferometer with a rotating anode x-ray tube and a single photon counting detector. Various combinations of serum and saline (100% and 0% to 0% and 100%), blood and saline, blood and serum (100% and 0% to 6.25% and 93.75% for both), and an iodinated contrast agent and saline (7.6-1.6 mg per milliliter of saline) were used to reproduce the chemical composition of the different types of cysts. A thickened solution of an iodinated contrast agent calibrated with a clinical multidetector CT scanner served as contrast agent-enhanced renal parenchyma (195 HU at 80 kVp, 400 mAs and 98 HU at 140 kVp, 200 mAs). Standard attenuation- and phase-contrast images were reconstructed from the raw projection data. Quantitative values for attenuation and phase contrast and image noise were determined. Contrast-to-noise ratios were calculated. Simulated lesions were assessed for visual differentiability by means of pairwise comparison of the attenuation- and phase-contrast images and both images simultaneously. Attenuation-contrast imaging showed large differences in Hounsfield units with increasing concentrations of iodine (118.9 HU for 1.6 mg/mL vs 331.4 HU for 7.6 mg/mL). Values for phase-contrast imaging were substantially distinguishable for saline, serum, and blood (7.9, 23.7, and 52.8 HU, respectively). Both imaging modalities combined allowed differentiation of all four types of simulated cysts (100% saline, 100% serum, 100% blood, and 1.6-7.6 mg of iodine per milliliter of saline) with one imaging acquisition. Grating-based phase-contrast CT allows differentiation of simulated simple, protein-rich, hemorrhagic, and enhancing renal cysts in an in vitro phantom through simultaneous assessment of their distinct attenuation- and phase-contrast signal.

Imaging Liver Lesions Using Grating-Based Phase-Contrast Computed Tomography with Bi-Lateral Filter Post-Processing

X-ray phase-contrast imaging shows improved soft-tissue contrast compared to standard absorption-based X-ray imaging. Especially the grating-based method seems to be one promising candidate for clinical implementation due to its extendibility to standard laboratory X-ray sources. Therefore the purpose of our study was to evaluate the potential of grating-based phase-contrast computed tomography in combination with a novel bi-lateral denoising method for imaging of focal liver lesions in an ex vivo feasibility study. Our study shows that grating-based phase-contrast CT (PCCT) significantly increases the soft-tissue contrast in the ex vivo liver specimens. Combining the information of both signals – absorption and phase-contrast – the bi-lateral filtering leads to an improvement of lesion detectability and higher contrast-to-noise ratios. The normal and the pathological tissue can be clearly delineated and even internal structures of the pathological tissue can be visualized, being invisible in the absorption-based CT alone. Histopathology confirmed the presence of the corresponding findings in the analyzed tissue. The results give strong evidence for a sufficiently high contrast for different liver lesions using non-contrast-enhanced PCCT. Thus, ex vivo imaging of liver lesions is possible with a polychromatic X-ray source and at a spatial resolution of ∼100 µm. The post-processing with the novel bi-lateral denoising method improves the image quality by combining the information from the absorption and the phase-contrast images.

SU-C-144-06: Detectability Index and Its Variation in Differential Phase Contrast CT Compared with the Conventional CT - A Channelized Hotelling Observer Study

Purpose: As an emerging imaging method for preclinical and eventually clinical applications, the grating-based x-ray differential phase contrast (DPC) CT has drawn increasing interest. It has been hoped that the potential detectability index d' in the DPC-CT can be significantly larger than its counterpart in the conventional attenuation-based CT. In this study, with the channelized Hotelling (CHO) model observer, we investigate the detectability d' and its variation in the DPC-CT and compare it with that of the conventional CT. Methods: The CHO study of the DPC-CT's detectability index d' and its comparison with that of the conventional CT are carried out under the framework of signal and background exactly known (SKE/BKE). Using the Gabor channel template, we investigate the detectability index d' and its variation over dimension of the target to be detected and the detector cells used for data acquisition, while the x-ray exposure is kept constant between the DPC-CT and conventional CT over all the cases. In addition, the internal noise, through which the CHO model deals with the psychological uncertainty existing in a human observer, is assumed identical in both the DPC-CT and conventional CT. Results: Preliminary data show that, the DPC-CT outperforms the conventional CT significantly in detectability while both the object to be detected and the cell of detector used for data acquisition are relatively small. However, the DPC-CT's dominance in the detectability diminishes with increasing dimension of either object or detector cell, and virtually disappears while the dimension of object or detector cell approaches a threshold, respectively. Conclusion: Based on the CHO study, the potential imaging performance of the grating-based DPC-CT in comparison to the conventional CT can be further understood and appreciated, providing insightful guidelines on the design and performance optimization of DPC-CT as a new imaging technology.
