Detection and intelligent optimization of blood glucose signals based on MXene-assisted sensors

Two-dimensional materials exhibit unique properties that make them ideal for electronic sensing applications, particularly in the field of biosensors. In this study, a Ti3C2Tx MXene/graphene composite was investigated for its potential use in non-invasive blood glucose detection. The strong electrostatic interaction between MXene and graphene nanosheets effectively reduces the self-stacking of both components, resulting in the formation of a porous interlayer structure with enlarged interlayer spacing. This unique structure creates an efficient nanoscale transport channel, which increases the number of active sites and shortens ion diffusion paths. Consequently, the electrochemical properties of the composite membrane are enhanced, making it a promising candidate for sensing element applications. As the human blood glucose level is primarily determined by the glucose concentration in the blood, the concentration of glucose solution serves as the foundation for noninvasive blood glucose concentration detection. In this work, a prediction model was established using the partial least squares fitting method to correlate the electrochemical fingerprinting data with the glucose aqueous solution concentration, and the reliability of the model was verified. To address the limitations of the partial least squares method, which lacks nonlinear fitting capabilities, an RBF (radial basis function) network model was employed. Finally, a genetic algorithm was utilized to perform global optimization of the training parameters of the RBF network, and the quantitative relationship between the NIR spectral data and glucose concentration was validated. The findings of this study provide a theoretical basis for the development of equipment and the improvement of measurement accuracy in the field of human noninvasive glucose detection.

Effects of Garlic on Glucose Parameters and Lipid Profile: A Systematic Review and Meta-Analysis on Randomized Controlled Trials.

(1) Background: The effect of garlic on glucose and lipid metabolism in humans remains controversial. The aim of this study was to investigate the effects of garlic on blood lipid levels and glucose levels in humans through a systematic review and meta-analysis. (2) Methods: We extensively searched four databases, including PubMed, Web of Science, Embase, and the Cochrane Library, up to February 2024. To assess the collective impact of garlic and its supplements on fasting blood glucose (FBG), glycosylated hemoglobin (HbA1c), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), and triglycerides (TG), an analysis was conducted using a random effects model. Subgroup analyses were performed when I2 < 50%. (3) Result: We found that the garlic intervention was effective in controlling FBG (mean difference = -7.01; 95% CI: -8.53, -5.49, p < 0.001), HbA1c (mean deviation = -0.66; 95% CI: -0.76, -0.55, p < 0.001, I2 = 62.9%), TC (mean difference = -14.17; 95% CI: -19.31, -9.03, p < 0.001), and LDL-C (mean difference = -8.20; 95% CI: -15.58, -0.81, p = 0.03); moreover, it also increased the level of HDL-C in humans (mean difference = 2.06; 95% CI: 1.54, 2.59; p < 0.001). Nonetheless, the intervention involving garlic did not yield a substantial impact on triglyceride (TG) levels. (4) Conclusion: The intervention of garlic is beneficial to control blood glucose and blood lipids in humans.

Measurement Analysis of Non-Invasive Blood Glucose On Sensor Coplanar Waveguide Loaded Square Ring Resonator with Interdigital Coupling Capacitor

This study presents the experimental results of a system with a sensor structure detecting human blood glucose levels. A microwave-based sensor is used for non-invasive blood glucose monitoring. The sensor design uses an asymmetrically loaded CPW structure as a square ring resonator with an interdigital coupling capacitor on the ground side. Simulated with a load of artificial finger tissue made from gelatin, modeled in four layers. The first layer is the skin is the outermost tissue, the next layer is fat, blood and bone. Each layer of tissue has a certain thickness size; skin (0.3mm), fat (0.2mm), blood (1.5mm), and bone (4mm). The measurement simulation is used, HFSS as modeling simulation and VNA as a measurement of the physical representation of the design results with parametric optimization methods. To verify the correlation and the expected sensitivity, media with different dielectrics were mounted on the surface of the sensor resonator with blood glucose levels of 1mg/dl, 72mg/dl, 126mg/dl, 162mg/dl and 216mg/dl. Reflection factor S11 was observed based on dielectric constant blood glucose levels (dB) fluctuations. Analysis of the data on the graph between the independent variables, namely blood glucose concentration and the dependent variable levels of S11 has an “R” correlation value of 0.97. The sensitivity level of the sensor on the S11 reflection factor with HFSS simulation averages 73.36mdB/mgdl-1 and VNA reaches 82.39mdB/mgdl-1. The results are interesting for developing a more optimal glucose sensor system.

Effect of Seaweed Rice Intervention on Fasting Blood Glucose Levels and Antioxidant Activity in Prediabetics and Diabetics Adults

This study aimed to analyze the effect of seaweed rice on Fasting Blood Glucose (FBG) and Superoxide Dismutase (SOD) levels in humans measured using finger prick method. This research was a quasi-experimental study with pre-posttest design. A total of 34 adult subjects were divided into control and intervention groups. The intervention group received 250 g/day of seaweed rice made by extrusion method at 8 AM for 42 days, while the control group received none. The results showed that the intervention had no significant effect on either FBG or SOD levels.

A single atom cobalt anchored MXene bifunctional platform for rapid, label-free and high-throughput biomarker analysis and tissue imaging

Few of single-atom materials have been served as platform to analyze small molecules for surface assisted laser desorption/ionization mass spectrometry (SALDI-MS). Herein, a novel single Co atom-anchored MXene (Co–N–Ti3C2) is prepared to achieve enhanced SALDI-MS and mass spectrometry imaging (MSI) performance for the first time. The Co–N–Ti3C2 films were prepared by a simple in situ self-assembly strategy to generate an efficient SALDI-MS platform. Compared to typical inorganic/organic matrices, Co–N–Ti3C2 films exhibit superior performance in small molecules detection with ultra-high sensitivity (LOD at amol level), excellent repeatability (CV <4%), clean background and wide analyte coverage, enabling accurate quantitative analysis of various low-concentration metabolites from 1 μL biofluid in seconds. Its usage efficiently enhanced SALDI-MS detection of various small-molecule biomarkers such as amino acids, succinic acid, itaconic acid, arachidonic acid, citrulline, prostaglandin E2, creatinine, uric acid, glutamine, D-mannose, cholesterol and inositol in positive ion mode. The blood glucose level in humans was successfully determined from a linearity concentration range (0.25–10 mM). Notably, the Co–N–Ti3C2 assisted SALDI-MSI enables study the spatial distribution of small molecules covering the range central to metabolomics at a high resolution on a tissue section. Furthermore, Co–N–Ti3C2 platform revealed a specific peak profile that distinguishes osteoarthritis (OA) from rheumatoid arthritis (RA) tissue. Density functional theory theoretical investigation revealed that single Co atoms anchored on Ti3C2 could highly enhanced the ionization ability of metabolites, resulting in high-sensitivity and heterogeneous metabolome coverage.

Effect of cooking and storage temperature on resistant starch in commonly consumed Indian wheat products and its effect upon blood glucose level

Background/objectivesThe health benefits provided by resistant starch have been well documented; however, few studies are available on the resistant starch content of wheat products in India. Moreover, few studies have examined the in vivo efficacy of resistant starch in wheat products in improving glucose levels. This study was conducted to evaluate the effect of cooking and storage temperature on the formation of resistant starch in Indian wheat products and its effect on blood glucose levels in humans and rats.MethodsWheat products were prepared by common cooking methods including roasting (Chapati), boiling (Dalia), Shallow frying (Paratha), and Deep frying (Poori). They were then stored at different temperatures including freshly prepared within 1 h (T1), stored for 24 h at room temperature (20-22°C) (T2), kept at 4°C for 24 h (T3) and reheated after storing at 4°C for 24 h (T4). The products were then analyzed for proximate composition (moisture, crude protein, crude fat, ash crude fibre, and carbohydrates). The effect of different cooking methods and storage temperatures on Resistant, non-resistant and total starch, total dietary fibre (soluble and insoluble), in vitro starch digestion rate (rapidly and slowly digestible starch), amylose and amylopectin content were analysed using standard operating procedures. The effect of products found to have higher resistant starch was studied on the post prandial blood glucose response of 10 healthy individuals using change in by analysing their glycemic index and glycemic load of wheat products. Further, the effect of resistant starch rich chapati on the blood glucose level of rats was also studied. Tukey’s test in factorial CRD was used to assess the effect of cooking and temperature on various parameters.ResultsThe amount of resistant starch was found to be high in dalia (boiling, 7.74%), followed by parantha (shallow frying, 4.94%), chapati (roasting, 2.77%) and poori (deep frying 2.47%). Under different storage temperatures, it was found high in products stored at 4°C (T3), followed by products stored at room temperature (T2), reheated products (T4) and lesser in freshly prepared products (T1). The glycemic index and glycemic load were found low in chapati (43, 32.3) and dalia (41.1, 28.6) stored at 4°C (T3) compared to others. The resistant starch content found in chapati stored at T3 was found to be more effective at reducing blood glucose levels in rats from 291.0 mg/100 mL to 225.2 mg/100 mL in 28 days of study compared to freshly prepared chapati (T1) and stored at room temperature (T2).ConclusionCooking methods including boiling, roasting and shallow frying increased the amount of resistant starch in foods, but cooking methods such as deep frying decreased the amount of resistant starch in food. Products stored at 4°C and at room temperature for 24 h increased the amount of resistant starch whereas the products that were freshly cooked and reheated decreased the amount of resistant starch in foods. At 4°C the stored products have a high amount of insoluble dietary fibre, slowly digestible starch, high amylose and low glycemic index. They take time to digest, meaning that they slowly increase blood glucose levels. The effect of insoluble dietary fibre and resistant starch in the inhibition of glucose diffusion in the small intestine is suggested to be due to the absorption or inclusion of the smaller sugar molecules. In vivo research showed that fibre and resistant starch in the digestive system of rats acts as the main factors in slowing glucose absorption and reducing a rise in blood glucose levels by promoting glycogen synthesis and inhibition of gluconeogenesis.

Impact of metformin and Dysosmobacter welbionis on diet-induced obesity and diabetes: from clinical observation to preclinical intervention

Aims/hypothesisWe aimed to investigate the association between the abundance of Dysosmobacter welbionis, a commensal gut bacterium, and metabolic health in human participants with obesity and diabetes, and the influence of metformin treatment and prebiotic intervention.MethodsMetabolic variables were assessed and faecal samples were collected from 106 participants in a randomised controlled intervention with a prebiotic stratified by metformin treatment (Food4Gut trial). The abundance of D. welbionis was measured by quantitative PCR and correlated with metabolic markers. The in vitro effect of metformin on D. welbionis growth was evaluated and an in vivo study was performed in mice to investigate the effects of metformin and D. welbionis J115T supplementation, either alone or in combination, on metabolic variables.ResultsD. welbionis abundance was unaffected by prebiotic treatment but was significantly higher in metformin-treated participants. Responders to prebiotic treatment had higher baseline D. welbionis levels than non-responders. D. welbionis was negatively correlated with aspartate aminotransferase (AST) and alanine aminotransferase (ALT) levels and fasting blood glucose levels in humans with obesity and type 2 diabetes. In vitro, metformin had no direct effect on D. welbionis growth. In mice, D. welbionis J115T treatment reduced body weight gain and liver weight, and improved glucose tolerance to a better level than metformin, but did not have synergistic effects with metformin.Conclusions/interpretationD. welbionis abundance is influenced by metformin treatment and associated with prebiotic response, liver health and glucose metabolism in humans with obesity and diabetes. Kombucha tea as an anti-hyperglycemic agent in humans with diabetes - a randomized controlled pilot investigation.

Kombucha is a popular fermented tea that has attracted considerable attention due, in part, to its suggested health benefits. Previous results from animal models led us to hypothesize kombucha may reduce blood sugar levels in humans with diabetes. The objective of this pilot clinical study was to evaluate kombucha for its anti-hyperglycemic activities in adults with diabetes mellitus type II. The study was organized as a prospective randomized double-blinded crossover study at a single-center urban hospital system. Participants (n = 12) were instructed to consume either a kombucha product or a placebo control (each 240 mL) for 4 weeks. After an 8-week washout period, participants consumed the alternate product. Fasting blood glucose levels were self-determined at baseline and at 1 and 4 weeks during each treatment period. Secondary health outcomes, including overall health, insulin requirement, gut health, skin health, mental health, and vulvovaginal health were measured by questionnaire at the same time points. The kombucha microbiota was assessed by selective culturing and 16S rRNA gene (bacteria) and ITS (fungi) sequencing. Fermentation end products were assessed by HPLC. Statistical significance of changes in fasting blood glucose was determined using paired, two-tailed student's t-tests. Kombucha lowered average fasting blood glucose levels at 4 weeks compared to baseline (164 vs. 116 mg/dL, p = 0.035), whereas the placebo did not (162 vs. 141 mg/dL, p = 0.078). The kombucha microbiota, as assessed by cultural enumeration, was mainly comprised of lactic acid bacteria, acetic acid bacteria, and yeast, with each group present at about 106 colony forming units (CFU)/mL. Likewise, 16S rRNA gene sequencing confirmed that lactic acid and acetic acid bacteria were the most abundant bacteria, and ITS sequencing showed Dekkera was the most abundant yeast. The primary fermentation end products were lactic and acetic acids, both less than 1%. Ethanol was present at 1.5%. Although this pilot study was limited by a small sample size, kombucha was associated with reduced blood glucose levels in humans with diabetes. Larger follow-up studies are warranted. An enzyme-free glucose sensing device based on TiO2 nanorod array photoelectric catalysis

A simple one-step hydrothermal method is used to prepare an enzyme-free photoelectric combined glucose sensor based on TiO2NRs/FTO with low cost, sample two-electrode, and excellent detection. Under 380 nm light (0.5 mW cm−2) irradiation and a positive voltage, holes are accumulated on TiO2NRs surface, catalyzing glucose and forming a photocurrent without the need for an enzyme, such as Glucose oxidase (GOx). The designed sensor exhibits high sensitivity (about 0.96 μA mM−1 cm−2, without GOx) and excellent linear relationship in the glucose concentration range of 5–15 mML−1. The prepared glucose sensor performs better with a sensitivity of 1.48 μA mM−1 cm−2 when a certain amount of GOx is mixed in the detected solution. In addition, the sensor has excellent anti-interference resistance to non-reducing chitosan and reducing ascorbic acid with short response time (less than 5 s); thus, it can be used in quick detection with a double electrode system. This sensing device has the advantages of simple fabrication, easy storage, and reusability; therefore, it can be very promising in the portable and rapid monitoring of human blood glucose levels.

Allulose for the attenuation of postprandial blood glucose levels in healthy humans: A systematic review and meta-analysis.

D-Allulose is a rare sugar that exists in nature. It is a food ingredient with nearly zero calories (<0.4 kcal/g) and has many physiological functionalities such as attenuation of postprandial blood glucose levels, attenuation of postprandial fat mass accumulation, and anti-aging property. This study focused on the postprandial blood glucose changes in healthy humans by a systematic review and meta-analysis. They were chosen because of its importance to a prevention from diabetes. The study objective was to examine acute blood glucose concentrations of healthy humans after the meal with and without allulose. The study collected all D-allulose related studies from various databases. A forest plot of the comparison between an allulose intake group and the control group showed both 5g and 10g intake groups have the significantly smaller area under the curve of postprandial blood glucose levels. It means that D-Allulose attenuates postprandial blood glucose concentrations in healthy humans. As the result, D-Allulose is a valuable blood glucose management tool for healthy humans and diabetes patients. Allulose Diet enables reduction of sucrose intake through Sugar Reformulation in the future diet.

Highly Sensitive and Flexible Copper Oxide/Graphene Non‐Enzymatic Glucose Sensor by Laser Direct Writing

AbstractAccurate and convenient detection of human blood glucose levels is an effective method for early diagnosis of diabetes and prevention of complications. The flexible and wearable electrochemical glucose sensor with low cost, fast responsiveness, good stability, reliability, and high sensitivity has attracted much attention in monitoring glucose concentration. The preparation of a conductive layer with catalytic activity on a flexible substrate is the key to making a wearable glucose sensor. Here, graphene composite materials sintered with copper oxide (CuO) nanoparticles are successfully prepared on a polyimide film by laser direct writing method and fabricated a flexible non‐enzymatic glucose sensor using laser‐engraved graphene (LEG) as a conductive electrode. The CuO/LEG sensor exhibits a high sensitivity of 619.43 μA mm−1 cm−2 in 0–3 mm glucose and 462.96 μA mm−1 cm−2 in 0–8 mm glucose. In addition, the CuO/LEG sensor shows good reproducibility, high anti‐interference capability, and long‐term stability. It also presents good bending stability, which can maintain 82.40% initial current after 100 times bending. Moreover, the CuO/LEG sensor has an obvious step‐ampere response in the detection of sweat samples, indicating the great potential of wearable sweat sensors.

Analyzing the Human Blood Glucose Level Trend (Up or Down) by Using Optical Polarization

Large amount of people having diabetics in the world, and it is increasing very rapidly. Diabetics should to check their blood glucose level at least four times a day. When controls are not done regularly, risk of hypoglycemia and hyperglycemia may arise. There are ongoing researches for developing non-invasive methods as an alternative to finger piercing, which is the most commonly used and painful method for patients. In this study, although a physical glucometer design has not been completed yet, it has been demonstrated which wavelength ranges and which parameters can be used for non-invasive device. Most importantly an empirical relationship has been established between the level of blood sugar and de-polarization disturbance seen in circularly polarized light. The proposed equation can estimate the blood glucose level based on de-polarization information with an average error rate of 1%.

Principles of Construction of Hybrid Microsystems for Biomedical Applications

The paper analyzes the existing non-invasive methods and tools for measuring and monitoring heart rate (HR), glucose saturation and human blood glucose level (BGL), shows ways of improving them to increase accuracy and expand the number of parameters obtained by these methods, which became the basis for creating a hybrid microsystem device for biomedical applications.&#x0D; A block diagram of the microsystem and a prototype of software and hardware tools for continuous monitoring of heart rate, BGL, saturation level and other blood parameters by photoplethysmography (PPG) have been developed. The proposed algorithms and tools provide primary processing of signals from optical sensors, calculation of pulse wave parameters, data transmission to mobile devices and a remote server, the possibility of calibration during operation based on research results.

Synthesis and characterization of CuO nanostructures on ZnO nanotubes for non-enzymatic detection of glucose

A non-enzymatic glucose sensing material is investigated on the basis of nanocomposites. Etched zinc oxide (ZnO) nanotubes have been synthesized as base and then dip-coated with various cupric oxide (CuO) nanostructures. Different concentrations of CuO precursor solution formed several sizes and shapes of nanostructures, including nanoparticles and nanoleaves. The fabricated CuO/ZnO nanotubes electrode has high sensitivity of 3.2 mAmM−1cm−2 and superior anti-interference ability towards glucose sensing. It also has good linearity in the range of human pre-meal blood glucose level, which has potential for diabetes monitoring.

Reduced graphene oxide supported MXene based metal oxide ternary composite electrodes for non-enzymatic glucose sensor applications

Diagnosis and monitoring of glucose level in human blood has become a prime necessity to avoid health risk and to cater this, a sensor’s performance with wide linearity range and high sensitivity is required. This work reports the use of ternary composite viz. MG–Cu2O (rGO supported MXene sheet with Cu2O) for non-enzymatic sensing of glucose. It has been prepared by co-precipitation method and characterized with X-ray powder diffraction, Ultraviolet–visible absorption spectroscopy (UV–Vis), Raman spectroscopy, Field emission scanning electron microscopy, High resolution transmission electron microscopy and Selected area diffraction. These analyses show a cubic structure with spherical shaped Cu2O grown on the MG sheet. Further, the electrocatalytic activity was carried out with MG–Cu2O sensing element by cyclic voltammetry and chronoamperometry technique and compared with M–Cu2O (MXene with Cu2O) composite without graphene oxide. Of these, MG–Cu2O composite was having the high defect density with lower crystalline size of Cu2O, which might enhance the conductivity thereby increasing the electrocatalytic activity towards the oxidation of glucose as compared to M–Cu2O. The prepared MG–Cu2O composite shows a sensitivity of 126.6 µAmM−1 cm−2 with a wide linear range of 0.01to 30 mM, good selectivity, good stability over 30 days and shows a low Relative Standard Deviation (RSD) of 1.7% value towards the sensing of glucose level in human serum. Thus, the aforementioned finding indicates that the prepared sensing electrode is a well suitable candidate for the sensing of glucose level for real time applications.

TonEBP in Myeloid Cells Promotes Obesity-Induced Insulin Resistance and Inflammation Through Adipose Tissue Remodeling.

The phenotypic and functional plasticity of adipose tissue macrophages (ATMs) during obesity plays a crucial role in orchestration of adipose and systemic inflammation. Tonicity-responsive enhancer binding protein (TonEBP) (also called NFAT5) is a stress protein that mediates cellular responses to a range of metabolic insults. Here, we show that myeloid cell-specific TonEBP depletion reduced inflammation and insulin resistance in mice with high-fat diet-induced obesity but did not affect adiposity. This phenotype was associated with a reduced accumulation and a reduced proinflammatory phenotype of metabolically activated macrophages, decreased expression of inflammatory factors related to insulin resistance, and enhanced insulin sensitivity. TonEBP expression was elevated in the ATMs of obese mice, and Sp1 was identified as a central regulator of TonEBP induction. TonEBP depletion in macrophages decreased induction of insulin resistance-related genes and promoted induction of insulin sensitivity-related genes under obesity-mimicking conditions and thereby improved insulin signaling and glucose uptake in adipocytes. mRNA expression of TonEBP in peripheral blood mononuclear cells was positively correlated with blood glucose levels in mice and humans. These findings suggest that TonEBP in macrophages promotes obesity-associated systemic insulin resistance and inflammation, and downregulation of TonEBP may induce a healthy metabolic state during obesity.

Effect of taurine on glycaemic, lipid and inflammatory profile in individuals with type 2 diabetes: study protocol of a randomised trial.

Type 2 diabetes mellitus (T2DM) is characterised by chronic hyperglycaemia. Despite the efficacy of conventional pharmacotherapy, some individuals do not reach glycaemic goals and require adjuvant therapies. Taurine, a semi-essential amino acid, decreases blood glucose and cholesterol levels in rodents and humans. However, glycated hemoglobin (HbA1c) has not been evaluated in randomised controlled trials after taurine treatment for more than 12 weeks. This study aims to evaluate the effect of taurine administration on glycaemic, lipid, inflammatory, anthropometric and dietary parameters in individuals with T2DM. A randomised, double-blind, placebo-controlled clinical trial will be conducted at the Clinical Research Center of a tertiary public hospital. Participants with T2DM (n 94) will be recruited and randomised to receive 3 g of taurine or placebo, twice/day, orally, for 12 weeks. Blood samples will be collected before and after 12 weeks of treatment, when HbA1c, fasting glucose, insulin, albuminuria, creatinine, total cholesterol and fractions, triglycerides, C-reactive protein, TNF-α, IL 1, 4, 5, 6, 10 and 13 will be evaluated. Anthropometric parameters and 24-hour food recall will also be evaluated. The study will evaluate the effect of taurine treatment on biochemical and anthropometric parameters in individuals with T2DM. These results will guide the decision-making to indicate taurine treatment as an adjunct in individuals with T2DM who have not reached their glycaemic goal.

Reversal of the renal hyperglycemic memory in diabetic kidney disease by targeting sustained tubular p21 expression

A major obstacle in diabetes is the metabolic or hyperglycemic memory, which lacks specific therapies. Here we show that glucose-mediated changes in gene expression largely persist in diabetic kidney disease (DKD) despite reversing hyperglycemia. The senescence-associated cyclin-dependent kinase inhibitor p21 (Cdkn1a) was the top hit among genes persistently induced by hyperglycemia and was associated with induction of the p53-p21 pathway. Persistent p21 induction was confirmed in various animal models, human samples and in vitro models. Tubular and urinary p21-levels were associated with DKD severity and remained elevated despite improved blood glucose levels in humans. Mechanistically, sustained tubular p21 expression in DKD is linked to demethylation of its promoter and reduced DNMT1 expression. Two disease resolving agents, protease activated protein C (3K3A-aPC) and parmodulin-2, reversed sustained tubular p21 expression, tubular senescence, and DKD. Thus, p21-dependent tubular senescence is a pathway contributing to the hyperglycemic memory, which can be therapeutically targeted.

A randomized, double-blind, placebo-controlled trial to evaluate the hypoglycemic efficacy of the mcIRBP-19-containing Momordica charantia L. fruit extracts in the type 2 diabetic subjects.

BackgroundThe fruits of Momordica charantia L., also named as bitter gourd or bitter melon in popular, is a common tropical vegetable that is traditionally used to reduce blood glucose. A peptide derived from bitter gourd, Momordica charantia insulin receptor binding peptid-19 (mcIRBP-19), had been demonstrated to possess an insulin-like effect in vitro and in the animal studies. However, the benefit of the mcIRBP-19-containing bitter gourd extracts (mcIRBP-19-BGE) for lowering blood glucose levels in humans is unknown.ObjectiveThis aim of this study was to evaluate the hypoglycemic efficacy of mcIRBP-19-BGE in subjects with type 2 diabetes who had taken antidiabetic medications but failed to achieve the treatment goal. Whether glucose lowering efficacy of mcIRBP-19-BGE could be demonstrated when the antidiabetic medications were ineffective was also studied.DesignSubjects were randomly assigned to two groups: mcIRBP-19-BGE treatment group (N = 20) and placebo group (N = 20), and were orally administered 600 mg/day investigational product or placebo for 3 months. Subjects whose hemoglobin A1c (HbA1c) continued declining before the trial initiation with the antidiabetic drugs were excluded from the subset analysis to further investigate the efficacy for those who failed to respond to the antidiabetic medications.ResultsThe oral administration of mcIRBP-19-BGE decreased with a borderline significance at fasting blood glucose (FBG; P = 0.057) and HbA1c (P = 0.060). The subgroup analysis (N = 29) showed that mcIRBP-19-BGE had a significant effect on reducing FBG (from 172.5 ± 32.6 mg/dL to 159.4 ± 18.3 mg/dL, P = 0.041) and HbA1c (from 8.0 ± 0.7% to 7.5 ± 0.8%, P = 0.010).ConclusionAll of these results demonstrate that mcIRBP-19-BGE possesses a hypoglycemic effect, and can have a significant reduction in FBG and HbA1c when the antidiabetic drugs are ineffective.
