Giant umbilical cord in a normal preterm infant: a case report and review of the literature

BackgroundGiant umbilical cord, defined as a cord diameter of more than 5 cm, is an extremely rare malformation. There are few case reports of giant umbilical cord often associated with patent urachus duct or cystic malformation. These cases are usually managed by surgical excision and repair of patent urachus or cyst resection.Case presentationWe report the case of a 1-day-old Iranian boy with giant umbilical cord detected postnatally. The pregnancy course was uneventful, except for preterm premature rupture of the membrane and preterm delivery. There was no relevant family history. The patient was delivered by vaginal delivery with a good Apgar score. On clinical examination, the umbilical cord was very thick (about 6 cm in diameter), and huge fluctuating Wharton’s jelly was observed. Other organs were normal. During the hospital stay, the patient did not develop any complications except borderline hyperbilirubinemia, which improved with conventional phototherapy. Since the umbilical cord had no discharge and was dried, the newborn was discharged with advice for cord drying care.ConclusionThe newborn was well, and the dried umbilical stump was detached after 32 days, leaving a granulomatous structure without discharge. The patient was followed up for 4.5 months and had no problems except delayed separation of the umbilical cord.

Giant umbilical cord with impending heart failure—Prognostic significance of prenatal ultrasound

Detection of giant umbilical cord prenatally should lead to vigilant monitoring particularly at term as fetal demise is a possibility, as noted in the present case because of altered hemodynamics and related vascular complications such as compromised fetal circulation. A case of prenatally diagnosed, giant umbilical cord with fetoplacental insufficiency and impending heart failure is reported. An 18-year-old primigravida, presented at 34 weeks of gestational age with complaints of acute onset abdominal pain. Fetal umbilical artery obstetric Doppler examination demonstrated reversal of diastolic flow. 2D echocardiography demonstrated dilated all 4 chambers of the heart with reduced contractility suggesting impending heart failure. Subsequently, fetal demise was noted after 2 days, and labour was induced followed by vaginal delivery of the dead fetus. A giant umbilical cord was seen at delivery with absent knots or strictures. This case report deserves a special mention as there were no earlier reported cases of giant umbilical cord in literature presenting with impending heart failure and fetal demise. We have presented the detailed clinical course of the fetus with giant umbilical cord along with a review of the related literature.

A giant umbilical cord: Benign finding or surgical emergency?

Congenital urachal anomalies are usually asymptomatic. However, infection or obstruction of urachal remnant may result in serious complications. The giant umbilical cords with suspected internal communications could contain remnants and require surgical exploration. With timely recognition and surgical intervention, the outcome is generally good.

Proteus syndrome (PS) is characterized by the progressive, segmental, or patchy overgrowth of the skin, and other tissues. This is the first case report of recurrent severe insulin-independent hypoglycemia in an infant with PS. Somatic p.E17K of AKT1 mutation was confirmed. The patient also had a giant umbilical cord, which has not yet been reported in PS.

Giant Cystic Umbilical Cord Associated with Patent Urachus and Intrauterine Fetal Demise

True cystic structures within the umbilical cord are rare, and when they persist into the second and third trimester, they are often associated with an abnormal karyotype or other developmental abnormalities. Clinically significant pseudocysts resulting from massive edema of the umbilical cord have been associated with a congenitally patent urachus. We present a case of intrauterine fetal demise at 28 weeks' gestation in which cystic dilatation of the umbilical cord was diagnosed prenatally by ultrasound imaging. At autopsy, a congenitally patent urachus was contiguous with the massively edematous umbilical cord.

Giant Umbilical Cord Edema Caused by Retrograde Micturition through an Open Patent Urachus

A giant umbilical cord is a rare finding in mature newborns and originates from different developmental etiologies. We report on a case of a mature female newborn presenting a 50 × 8-cm giant umbilical cord without further malformations. Antenatal sonographic findings of a diffuse giant umbilical cord, elevated creatinine levels of 1.3 mg/dL in umbilical cord edema, gross and histopathological findings of allantoic remnants, and umbilical urinary discharge lead to the diagnosis of a patent urachus with retrograde micturition into the umbilical cord. Postnatal surgical repair was required. In antenatal sonography, cystic and diffuse changes should be considered in the differential diagnosis of a giant umbilical cord. In cases of diffuse enlargement, elevated umbilical creatinine can support the diagnosis of a patent urachus with open leakage into the Wharton's jelly. Appropriate surgical management is required.

The giant umbilical cord: an unusual presentation of a patent urachus

Umbilical cord anomalies remain a frequent newborn nursery consultation for the pediatric surgeon. The authors report on a giant umbilical cord associated with a patent urachus. Although it is an uncommon anomaly, operative exploration must be carried out to repair the associated urachal remnant.

Giant umbilical cord associated with cord hemangioma

A 6-h-old girl was transferred to the Pediatric Surgery Department of Sisli Children's Hospital due to a giant umbilical cord. Radiologic and laboratory studies were within normal limits. The umbilical mass excised through an intraumbilical incision and the umbilicus reconstructed. Pathologic investigation demostrated an umbilical cord hemangioma located near the umbilical end, compressing the cord structures externally. The remaining cord consisted of loose, edematous stroma similar to Wharton's jelly. The patient was discharged from the hospital on the 5th postoperative day and remains healthy 1 year later. Umbilical cord hemangioma should be considered in the etiology of a giant umbilical cord.

Giant umbilical cord associated with patent urachus. An external clue to internal anomaly.

A giant, swollen, umbilical cord may be associated with a patent urachus. When the obvious physical finding is noted, the presence of this rare hidden internal abnormality must be sought. Two cases of the association are presented. Awareness of this association presents a simple external clue to aid in diagnosis of the internal anomaly.
