Incomplete duplex renal system with severe obstruction at the uretero-ureteral junction and minor obstruction at the common stem-vesical junction: Challenging diagnosis and conclusive laparoscopic treatment

Twin obstructive megaureters associated with duplex renal system is a rare malformation, particularly when a giant ureter dilation is present. We report an original case, initially misinterpreted, of a right incomplete duplex renal system with huge disproportion between the upper pole giant megaureter and the lower pole moderate megaureter, with severe obstruction at the uretero-ureteral junction and minor obstruction at the uretero-vesical junction. Uromagnetic resonance imaging allowed conclusive diagnosis and the laparoscopic approach was effective for definitive, successful treatment.

Diagnostic Value of Real-Time Ultrasound in Congenital Segmental Giant Megaureter: A Case Report

: Congenital segmental giant megaureter (CSGM), which refers to segmental cystic dilatation of the upper collecting system, is extremely rare in the pediatric population. Depending on the etiology, the condition could be categorized as primary or secondary megaureter with three subtypes including refluxing, obstructed, nonrefluxing and nonobstructed types. We report a case in which the CSGM presented as a giant abdominal cystic mass with associated right-sided hydronephrosis that was diagnosed by abdominal ultrasound. Cystic mass and the tubular structure at the caudal side of the cystic mass were demonstrated on abdominal ultrasound. We identified a connection between the cystic mass and the lumen of the tubular structure. Therefore, the cystic mass was reported as CSGM on abdominal ultrasound, especially categorized as the nonrefluxing and nonobstructed type. The percutaneous nephrostomy was performed to relieve hydronephrosis, followed by excision of the dilated segment. CSGM is an extremely rare presentation of megaureter, requiring variable imaging modalities and an appropriate intervention. Especially, careful real-time ultrasound was critical to identify the CSGM promptly and essential for the individual appropriate management.

Bilateral Primary Obstructive Giant Megaureter in an Adult Dog

A rare case of bilateral, primary, obstructive, giant megaureter was found during necropsy examination of an 11-year-old female German shepherd dog. On ultrasound examination and at necropsy examination, both ureters were tortuous and extensively dilated with diameter ranging from 1.86 to 4.8cm. Both vesicoureteral junctions were obstructed by uroliths. A diagnosis of giant megaureter was established using human parameters since these values are not recognized in animals. The classification of obstructive and primary megaureter was determined because the obstruction was due to uroliths at the vesicoureteral junctions.

Single System Ectopic Congenital Giant Megaureter Associated with Ipsilateral Multicystic Dysplastic Kidney

Congenital anomalies of the kidney and urinary tract (CAKUT) is a disease spectrum of a wide range of structural and functional anomalies. In this report, we have presented a rare case of CAKUT displaying the wide spectrum of renal and urinary tract anomalies with an unusual association like single system ectopic giant megaureter with the ipsilateral multicystic dysplastic kidney.

An extremely rare clinical case of giant megaureter in an adult patient is described. Megaureter was revealed in examination of the abdominal cavity and retroperitoneal space during surgery for gynecological pathology. Surgical treatment was successful, resection of megaureter with plastic reconstruction of the left ureter with anastomosis end-to-end and end-to-side on introduced ureteral stents into the lumen was performed. Postoperative examination revealed the presence of megaureter on the right side and doubling of both kidneys.

Congenital giant megaureter associated with ipsilateral multicystic dysplastic kidney in newborn

Congenital giant megaureter presents as abdominal mass and impose diagnostic difficulties. It can be associated with other upper urinary tract anomalies. A female newborn with antenatal diagnosis of polycystic kidneys was admitted at birth due to lower abdominal mass. Ultrasound and CT scans diagnosed a multiloculated cystic lesion in the mid and lower abdomen along with right side multicystic kidney. At laparotomy, an extaperitoneal, lobulated cystic swelling was found due to rightside giant megaureter. Its lower end was of normal caliber and orthotopic. End cutaneous ureterostomy was done. Intravenous urogram and isotope renograms showed nonfunctioning right kidney. She also had grade II vesicoureteral reflux on left side. Child suffered urinary infection twice. At 9m age, right nephroureterectomy was done. Histopathologic examination was consistent with cystic renal dysplasia and dilated ureter. This is the first case report of giant megaureter associated with ipsilateral multicystic dysplastic kidney in newborn.

Congenital Segmental Giant Megaureter Presenting as Cystic Abdominal Mass in a Child

A six years old male child presented with gradually progressive distension of abdomen since one year of his age. Imaging suggested it as a large mesenteric cyst. Further evaluation and intra-operative findings suggested it to be bilateral congenital megaureter with giant one on the left side which was causing gross abdominal distension. Excision of left megaureteric segment, psoas hitch and ureteroneocystostomy was performed. Excision, tapering and uretereroneocystostomy of right ureter was performed. Congenital giant megaureter should be considered as one of differential diagnosis in children presenting as abdominal mass.

Congenital giant megaureter with duplex kidney presenting as abdominal lump in a neonate

A neonate with unilateral complete duplex system with congenital giant megaureter of the upper moiety presenting as abdominal lump is reported. A left upper moiety nephroureterectomy was performed. Such an anomaly with this presentation has not been reported in neonates.

Preoperative diagnosis of congenital segmental giant megaureter presenting as a fetal abdominal mass

We describe a case of congenital segmental giant megaureter in a boy that presented as a fetal abdominal mass. He also had bilateral undescended testes, bilateral vesicoureteral reflux, and segmental aniridia. He presented with hypoglycemia in the neonatal period that resolved. Postnatal magnetic resonance imaging, voiding cystourethrography and radionuclide imaging established the diagnosis, and a ureteroureterostomy was performed at 12 months.

Giant Obstructive Megaureter Causing Contralateral Ureteral Obstruction and Hydronephrosis: A First-time Report

We report for the first time a "giant megaureter" causing symptomatic compression of the contralateral ureter in a 15-year-old girl. Extrinsic compression of the ureter by a contralateral giant hydronephrotic kidney, although rare, has been reported most commonly because of congenital pelviureteric junction obstruction. In this case, the possible etiology of megaureter, the differential diagnosis, and the clinical implications are discussed.

Congenital giant megaureter: First neonatal case

The authors report the first neonatal presentation of a congenital giant megaureter (CGM), a very rare unilateral urinary anomaly, which is defined as “a ureter whose lumen is congenitally, focally and segmentally dilated to more than 10 times the normal diameter, in presence of normal bladder volume and function.” The definition of CGM is purely descriptive; it can be secondary to several different diseases, and in the series of 21 CGM (12 in duplex system) reported by Chen-Ju Huang, the morphology of the distal ureter varied from a near normal caliber to a ureteral stenosis, a ureterocele or a complete ureteral atresia. Congenital segmental giant megaureter

A case of congenital segmental giant megaureter is reported. The patient presented with an asymptomatic abdominal mass resulting from the megaureter. A mildly hydronephrotic ipsilateral kidney was palpable as a separate mass. The ureter proximal and distal to the dilated segment showed normal peristaltic activity. Treatment consisted of excision of the dilated segment, with end-to-end ureteric anastomosis.

Congenital giant megaureter

We have seen 21 patients with unilateral congenital giant megaureter. Age ranged from 2 months to 8 years. This condition is characterized by giant focal segmental ureteral dilatation producing an elongated and distorted ureter; distally there may be congenital ureteral stenosis or atresia, or a ureter of approximately normal caliber. They usually have a dysplastic and disorganized muscular coat, lined with a columnar epithelial mucosa rather than the usual transitional epithelium. The associated kidney or moiety was hypoplastic, dysplastic, or the site of comparatively mild hydronephrosis associated with atrophic renal parenchyma. Function and volume of the bladder was normal. Twelve of 21 patients had a duplex collecting system on the affected side. One patient had four pelves and proximal ureters opening into a single lower ureter, which was the site of a giant cyst. One of eight simple giant megaureters was suspected of being associated with a solitary kidney. Depending on whether the giant segmental ureteral dilatation was multiple or single, these cases have presented either with marked protruberance of the mid and lower abdomen, which was distended by transverse elongated cysts, or with a round cyst mass in the lower abdomen. Nineteen patients treated by excision of the giant megaureter and the associated kidney or moiety were free of symptoms postoperatively. One 4-month-old infant with multiple congenital abnormalities and urinary tract infections died soon after operation. Another 4-month-old infant with suspected solitary kidney was treated by ureterostomy and was discharged in a critical condition without follow-up.
