Giant Coronary Artery Pseudoaneurysm after Drug-Eluting Stent Implantation

Formation of coronary artery pseudoaneurysm after percutaneous coronary intervention is rarely reported. A close review of available literature indicates a trend of growing incidence in recent times. Here, we present a case of coronary pseudoaneurysm developing in the left circumflex artery within 50 days of sirolimus-eluting stent implantation in a 47-year-old man who was treated for triple-vessel coronary artery disease. Initially, the patient refused any further treatment. However, considering the rapid growth of pseudoaneurysm and increased severity of symptoms at subsequent follow-up, we decided to treat giant pseudoaneurysm with embolization coils. The procedure was successful and the patient remained asymptomatic thereafter.

Iatrogenic Giant Coronary Artery Pseudoaneurysm With “Daughter Aneurysm” Formation

Coronary pseudoaneurysms rarely occur spontaneously; rather, they are more commonly seen as a complication of coronary intervention. We present a case of a giant right coronary artery pseudoaneurysm with partial thrombosis after arterial perforation during percutaneous intervention for acute myocardial infarction and formation of a "daughter aneurysm" due to a contained rupture 12 years later. Right coronary pseudoaneurysm repair and coronary artery bypass grafting were eventually performed 16 years after the acute event. Cardiac magnetic resonance imaging, coronary computed tomography angiography, and autopsy findings are shown.

Pulmonary Stenosis Caused by Extrinsic Compression of an Aortic Pseudoaneurysm of a Composite Aortic Graft

Pulmonic stenosis and stenosis of the right ventricular outflow tract related to extrinsic compression have been described in patients with tumors, in a patient with a pericardial cyst, and in patients with vascular abnormalities as an unruptured sinus of Valsalva aneurysm, a giant coronary artery pseudoaneurysm and an aortic arch aneurysm. Composite graft replacement of the ascending aorta and aortic valve with reimplantation of the coronary arteries has some inherent complications. Our case report describes a patient with a pericomposite graft aneurysm presenting as a stenosis of the pulmonary artery, detected by Doppler echocardiography.
