Giant Right Ventricular Fibroma: Prenatal Diagnosis and Partial Resection in Early Infancy.

Congenital cardiac fibromas are very rare and prenatal diagnosis has been reported in just a few cases. We describe a four-month-old infant presenting a symptomatic giant right ventricular fibroma discovered during prenatal scanning at 33 weeks of gestation, which was confirmed after delivery on echocardiogram and cardiac magnetic resonance imaging. Due to progressive hemodynamic deterioration, partial surgical resection was performed and the patient recovered uneventfully. We report the successful management during early infancy of a giant cardiac fibroma prenatally diagnosed.

Surgical excision of a giant cardiac fibroma in an asymptomatic child

Cardiac fibroma is a rare primary tumor of the heart. Patients can be asymptomatic or present with palpitations, cardiac murmur, syncope, arrhythmias, symptoms of congestive heart failure, or sudden death. We report a case of successful surgical excision of a giant cardiac fibroma in an asymptomatic child.

Giant Cardiac Fibroma in a Completely Asymptomatic Teenager

We report the case of a 19 years old “healthy” and asymptomatic patient accessing the Emergency Room after a car accident trauma. A routine electrocardiogram documented an intraventricular conduction disturbance. The subsequent transthoracic echocardiogram showed the presence of a voluminous heterogeneous intracardiac mass (10 × 10 × 8 cm), localized in the medium-apical cavity of the right ventricle and extended to the outflow tract. A mass debulking intervention was performed and the intraoperative biopsy samples allowed the diagnosis of cardiac fibroma. Because of dimension, intracardiac infiltration and relations, radical surgery was not an option; the patient was candidate for heart transplantation. This unique case highlights the questionable classification of cardiac fibromas as benign and the possibility of a delayed diagnosis because of late clinical presentation.

Successful surgical treatment for recurrent cardiac fibroma 21 years after resection.

Cardiac fibromas are rare benign tumors usually seen in the pediatric population. Generally, long-term survival after surgical resection is favorable, and recurrence of fibroma has been hardly reported. Herein, we report a case of a 34-year-old woman who presented with ventricular tachycardia 21 years after resection of a cardiac fibroma and was found to have a recurrent giant cardiac fibroma. We performed a complete resection of the recurrent fibroma. At the 2-year follow-up, she remains asymptomatic with no evidence of ventricular tachycardia or recurrence of fibroma.

Long-term conservative management of a giant cardiac fibroma

A giant cardiac fi broma was discovered during evaluation for a soft systolic murmur in an asymptomatic 2-week-old girl. Echocardiography and magnetic resonance imaging showed a large intraventricular solid mass developed at the expense of the left ventricular lateral wall. Tumour progression resulted in failure to thrive and ventricular arrhythmia between 2 and 18 months of age. At that time, complete resection seemed unfeasible and conservative management with heart failure and antiarrhythmic medications was chosen. All drugs were discontinued when the patient was 5 years old. Since that time, the mass is stable and the patient is strictly asymptomatic. Conservative strategy seems to be acceptable in selected cases but close follow-up is mandatory

Third-Degree Atrioventricular Block in an Adult With a Giant Cardiac Fibroma

A 48-year-old man was admitted with exertional dyspnea. An ECG on admission revealed third-degree atrioventricular block (Figure 1). Transthoracic echocardiography demonstrated a giant mass, 6.4×6.5×7 cm, located in the basal interventricular septum (Figure 2A and 2B and Movie I in the online-only Data Supplement).
