Visual Analysis of Research Hotspots in Geriatric Nursing Education in China and Abroad.

Background This study aimed to conduct a literature search to determine research hotspots in the field of gerontological care education in China and abroad. A knowledge of the focus of research conducted abroad may assist Chinese educators in determining the shape of gerontological care education in the future. Methods The “Web of Science Core Collection” and “CNKI” databases were searched for literature on gerontological care education published from 2010 to 2020. CiteSpace software was used to display the knowledge map of co-occurrence of keywords, and an evolution trend map of research hotspots in recent 10 years was constructed. Results From 2010 to 2020, the focus of foreign gerontological nursing education research was on the training of gerontological nursing personnel; the development of elderly care services; and education and training in dealing with patients with senile delirium and dementia. The focus of gerontological nursing education research in China was primary care education, training of senior elderly nursing personnel, talent training mode reform, training of nursing staff, and career development paths for geriatric nursing staff. Conclusions Foreign geriatric nursing education research focuses on cultivating talents, mental health services for the elderly, innovating the mode of pension, and the care of patients with disorders such as dementia and delirium, while in China, the emphasis remains on gerontological nursing personnel training. Attention to research developments in other countries may assist Chinese educators to promote the development of geriatric nursing education in our country.

Advancing Gerontological Nursing Science in Climate Change.

Older adults have unique health risks related to climate change. This commentary addresses the health impacts of climate change for older adults, identifies gaps in gerontological nursing research, and highlights areas for research to address the significant gap in nursing science. Climate risks of extreme weather events, such as heat, rain, flooding, and wildfires, as well as poor air quality, vector-borne diseases, interruptions of services, and treatment plans all place older adults at risk of experiencing greater morbidity and early mortality. Despite these risks, there is a gap in nursing research related to climate change and aging. Nurse scientists can address this gap with an interdisciplinary approach. There are climate resources and theoretical frameworks to support scientific inquiry. Funding sources must be made available to assure rigorous scholarship of climate-related health impacts for older adults. Improving practice and informing policy development: The impact of gerontological nursing research

Nurses have always approached the clinical care challenges they encounter with a combination of critical observation and a profound concern for human suffering. Beginning in earnest in the early 1980s, nurse researchers have pursued the answers to questions about the wellbeing of older adults, particularly those suffering from serious, chronic conditions. This paper offers three exemplars of nursing research domains that illustrate the creativity and commitment of nurse researchers seeking to understand and improve pervasive clinical problems experienced by older adults and to demonstrate the profound influence these activities have had on the evolution of the science and the quality of care for older adults. The impact of this work is reflected in practice protocols, institutional policies, government oversight, and improved outcomes for patients.

Defining Usual Care in Clinical Trials.

Pragmatic trials occur within the complexity of real-world care delivery, and when effective, contribute to more rapid translation into practice because of their greater generalizability. Research with older adults is complex when participants have chronic conditions and multiple comorbidities. Often pragmatic trials introduce a novel intervention and try to determine whether it offers a benefit beyond the usual or routine care provided. Researchers commonly focus attention on describing the intervention, yet the comparator condition of usual or routine care can be anything but standard, reducing the effect size of the intervention and introducing threats to the overall validity of the study. The current article describes clinical trial guidelines, then illustrates the complexity of characterizing usual care for interventions addressing type 2 diabetes. The authors provide recommendations for improving description of usual care and discuss implications for gerontological nursing research. [Research in Gerontological Nursing, 13(3), 125-129.].

Progress and Changes in Gerontological Nursing Research in the Midwest: A Review

Gerontological nursing has a rich research history. The current article reports trends in gerontological nursing research found through review of abstracts of presentations at the Midwest Nursing Research Society annual conference at 10-year increments over the 40-year existence of the organization. Researchers studied a wide range of topics, from quality of life to use of technology, with a variety of older adult populations represented. Designs, theoretical frameworks, and other methods mirrored trends in nursing research from 1977 to 2017. Gerontological nursing researchers had a strong and sustained presence in the Midwest, as evidenced by these abstracts. [Res Gerontol Nurs. 2018; 11(5):231-237.].

A comparative review of gerontological nursing citation data.

Gerontological nursing is a highly interdisciplinary specialty. English-language bibliometric research is scarce. Study gerontological nursing literature to identify journals useful for nurses and researchers working with older adults, help librarians assist gerontological nursing authors to find publication outlets and make library collection decisions. Using a combination of methods, the authors identified cited journals in the Journal of Gerontological Nursing (JGN) and Geriatric Nursing (GN) from 2008 to 2010 using part of the Nursing and Allied Health Resources (NAHRS) Sectionprotocol. A list was generated and compared to the results of an earlier unpublished NAHRS study and an earlier review article. A second list was developed following set criteria of bibliometric indicators and journal lists. Zone 1 showed 28 journals, and Zone 2 had 1472 titles. Zone 1 journals were examined further based on the inclusion of certain bibliometrics and journal lists. Providing insight into the complementary use comparative review and citation mapping, this study reports an increase in gerontological nursing research from 2008 to 2010 across a diverse group of highly cited journals. A list of new journal titles is presented to aid collection development and suggestions of places to publish gerontological research.

Lifelong Health and Health Services Use: A New Focus for Gerontological Nursing Research and Practice.

It is commonly believed that older adults are often ill and therefore high users of health services. A pilot study involving adults 60 and older living in the Canadian province of Alberta was conducted to (a) raise interest in the concepts of lifelong health and health services use; (b) develop a lifelong health data collection tool; (c) obtain information about lifelong and recent health services use, and self-perceived lifelong and current health; and (d) reveal links (if any) between perceived health and lifelong health services use. A questionnaire was developed, pilot tested, and posted for completion by 100 volunteers. Most older adults reported good or very good current and lifelong health. Comorbidities and low finances were associated with higher health services use. These findings suggest community-dwelling older adults may be healthy currently and throughout their lives. Gerontological nursing research, practice, and advocacy are needed because myths about aging must be addressed to refocus attention on the importance of lifelong health promotion for older adults. Evaluation of the Nurses Caring for Older Adults Young Scholars Program.

There is a "perfect storm" brewing in nursing. We are faced with a growing number of older patients, while at the same time nurses with expertise in gerontological nursing are aging and retiring. This critical shortage is most evident for nurses with research-intensive preparation needed to replenish actual and anticipated nurse faculty vacancies across the United States, especially those in underrepresented minority groups. We describe one solution to this problem; the Nurses Caring for Older Adults Young Scholars Program (YSP) that selects promising, ethnically diverse students and offers them a 1- to 3-year mentorship experience with the focus on students continuing to PhD studies on completion of their basic nursing studies. The YSP has mentored 15 prelicensure students with an identified interest in gerontological nursing research, with 8 young scholars (53%) going on to pursue doctoral studies. Program elements are described as well as philosophical and practical challenges of program implementation. Formative evaluations including student and faculty perceptions of the program as well as summative evaluation including admission success rate, student products, and progression in the doctoral program are discussed. Students indicate that establishing a strong mentor relationship with opportunities to participate in their mentor's research activities leading to the generation of a commitment to a research topic is the strongest factor in young scholars following through with enrollment into a doctoral program. A synergistic outcome of the YSP was the development of a critical mass of students interested in pursuing PhD studies that further extended the impact of the program.

Developing and proposing the ethno-cultural gerontological nursing model.

By 2050, for the first time in U.S. history, almost half of elders will be from ethnic minority groups. To meet the needs of this rapidly diversifying population, nurses need to be able to marry transcultural nursing knowledge with gerontological nursing knowledge. The purpose of this article is to propose a new theoretical model for explaining health outcomes and health responses for older individuals in unique ethno-cultural groups and to discuss implications and applications of the model to transcultural gerontological nursing practice and research. The discussion will include (1) an overview of currently available theoretical knowledge in the area, (2) a description of the theory development process, (3) presentation of the proposed ethno-cultural gerontological nursing theoretical model, and (4) discussion of how this model can enhance nursing's contributions to reducing health disparities. This model is presented not as a finished product but as a basis for future discussion and refinement.

Engaging Students in Gerontological Research: The STARR Program

There is a critical need to prepare undergraduate and graduate nursing students to take active roles in gerontological nursing research. Nursing educators are faced with multiple challenges such as the demands of the tripartite role (teaching, scholarship, and service), and managing large numbers of students. These issues make it difficult to actively engage nursing research. Students Advancing Reminiscence Research (STARR) is and example of an innovative program that fosters student engagement in research using available resources while enhancing the faculty advisor's program of reminiscence research. The impetus for creating the STARR program is described in this article, and implications and recommendations for nursing education, practice, and policy are presented.

Barriers to Depression Care for Black Older Adults

Late-life depression is a public health problem in the United States. Untreated, depression contributes to poorer health outcomes and increased mortality among older adults. Specifically, Black older adults are at higher risk for misdiagnosis, undertreatment, and more severe depressive symptomatology than other groups. Barriers to identification and treatment of depression in Black older adults include lack of access to quality mental health care, the stigma of mental illness, mistrust of mental health providers, and poor provider-client communication. Recommendations for gerontological nursing practice, education, and research to improve the care of depressed Black older adults are discussed. Implications for policy development are presented.

Measuring Gerontological Research Intensity in Schools of Nursing

Because of the urgent need for more gerontological nursing research, it is critical for more schools of nursing to develop research-intensive environments within which faculty can develop programs of gerontological nursing research. Unfortunately, many schools have limited resources for developing the needed infrastructures and for acquiring the required expertise in gerontology and research. Many also lack the expertise and methods for assessing their current status so they may increase their gerontological nursing research intensity. To help schools assess their status and gaps in the essentials required for a research-intensive environment and gerontological nursing research, an instrument was developed to measure benchmarks of research intensity; it is described in this article. Initial results of pilot testing of the instrument, the revised instrument, and plans for future research are presented. Uses of the instrument for assessing and strengthening a school's research infrastructure and culture and for assisting with the development of faculty programs of gerontological nursing research are also discussed.
