Human papillomavirus genotyping in high-grade vaginal intraepithelial neoplasia: Amulticentric Italian study.

This study aimed to analyze the human papillomavirus (HPV) genotype distribution in a large cohort of high-grade vaginal intraepithelial neoplasia (VaIN) (vaginal HSIL, VaIN2/3) patients from two Italian referral centers. We included all patients with histologically confirmed VaIN2/3 from the Department of Surgical Sciences, Sant'Anna Hospital, University of Torino, Torino, Italy, and Ospedale Maggiore della Carità, Novara, Italy, between 2003 and 2022. After the histological evaluation of formalin-fixed paraffin-embedded samples, we performed HPV genotyping with VisionArray HPV Chip 1.0. We detected HPV DNA in 94.4% of VaIN2/3 (168/178), with HPV 16 as the most prevalent genotype, accounting for 51.8% of all infections, 41.2% of VaIN2and 77.6% of VaIN3 cases. Other frequent genotypes were HPV 58 (8.3%, 10.9% of VaIN2and 2.0% of VaIN3), HPV 73 (5.4%, 5.0% of VaIN2and 6.1% of VaIN3), and HPV 31 (5.4%, 6.7% of VaIN2and 2.0% of VaIN3). 73.2% of VaIN2/3 had a single HPV genotype infection and 26.8% a multiple infection (20.8% a double infection, 4.8% a triple infection, and 1.2% a quadruple infection). Single infection was more frequently present in VaIN3 than VaIN2 (81.6% vs. 69.8%). 69.1% of single infections and 73.3% of multiple infections had one or more genotypes covered by nine-valent HPV vaccine. HPV vaccination is expected to have a large impact on reducing the incidence of vaginal intraepithelial neoplasia.

Trend of HPV Molecular Epidemiology in the Post-Vaccine Era: A 10-Year Study.

Cervical cancer, a major health concern among women worldwide, is closely linked to human papillomavirus (HPV) infection. This study explores the evolving landscape of HPV molecular epidemiology in Taiwan over a decade (2010-2020), where prophylactic HPV vaccination has been implemented since 2007. Analyzing data from 40,561 vaginal swab samples, with 42.0% testing positive for HPV, we reveal shifting trends in HPV genotype distribution and infection patterns. The 12 high-risk genotypes, in order of decreasing percentage, were HPV 52, 58, 16, 18, 51, 56, 39, 59, 33, 31, 45, and 35. The predominant genotypes were HPV 52, 58, and 16, accounting for over 70% of cases annually. The proportions of high-risk and non-high-risk HPV infections varied across age groups. High-risk infections predominated in sexually active individuals aged 30-50 and were mixed-type infections. The composition of high-risk HPV genotypes was generally stable over time; however, HPV31, 33, 39, and 51 significantly decreased over the decade. Of the strains, HPV31 and 33 are shielded by the nonavalent HPV vaccine. However, no reduction was noted for the other seven genotypes. This study offers valuable insights into the post-vaccine HPV epidemiology. Future investigations should delve into HPV vaccines' effects and their implications for cervical cancer prevention strategies. These findings underscore the need for continued surveillance and research to guide effective public health interventions targeting HPV-associated diseases.

The vaginal microbiota among the different status of human papillomavirus infection and bacterial vaginosis.

Although human papillomavirus (HPV) infection plays a decisive role in causing tumors, its infection is insufficient for independently promoting cancer development and other co-factors facilitate the carcinogenic process. The objective of this study was to demonstrate the association between vaginal microbiota and high-risk human papillomavirus (HR-HPV) infection in women with and without bacterial vaginosis (BV). The study included 1015 women aged 21-64 who participated in cervical cancer screening in two areas of China from 2018 to 2019. Women were collected cervical exfoliated cell specimens and reproductive tract secretions samples for HR-HPV, BV and microbial composition testing. From the non-BV& HPV- group (414 HPV-negative women without BV) to the non-BV& HPV+group (108 HPV-positive women without BV), to the BV& HPV-group (330 HPV-negative women with BV) and then to the BV& HPV+group (163 HPV positive-women with BV), microbial diversity increased. The relative abundance of 12 genera, including Gardnerella, Prevotella, and Sneathia increased, while Lactobacillus declined. Correlation networks of these genera and host characteristics were disrupted in the non-BV& HPV+group, and the network trended more disordered in the BV& HPV+group. Besides, multiple HPV infection, certain HPV genotype infection and cervical intraepithelial neoplasia (CIN) status were associated with some microbes and higher microbial diversity. HPV shifted the composition and diversity of vaginal microbiota, and BV further reinforced the trend. The relative abundance of 12 genera increased and 1 genus decreased on account of BV and HPV infection, and some genera including Lactobacillus, Prevotella, and Sneathia were associated with some specific HPV genotypes infection and CIN.

The diversity of vaginal microbiome in women infected with single HPV and multiple genotype HPV infections in China.

The human papillomavirus (HPV) is the leading cause of cervical cancer globally. However, its microbial composition and association with the types of HPV infection remain elusive. This study was designed to characterize the vaginal microbiota of 53 HPV-infected and 16 normal women (control group) by using high-throughput sequencing with the Illumina platform. In this study, the five leading phyla were Firmicutes (73.9%), Actinobacteriota (12.8%), Proteobacteria (6.2%), Fusobacteria (3.5%), and Bacteroidota (3.1%). We found that single HPV genotype-positive women had higher α-microbial diversity compared with HPV-negative and multiple HPV-positive women. In women with a single HPV genotype infection, the HPV-16 infection had significantly higher α-diversity than other genotype infections. In multiple HPV genotype-positive women, the highest α-diversity was found in women positive for HR-HR HPV genotype infection, compared with other infections. Furthermore, in single- and multiple-genotype infections, the abundance of s_unclassified_g_Lactobacillus decreased whereas the abundance of s_Gardnerella_vaginalis increased compared with control. Additionally, s_unclassified_f_Rhizobiaceae and s_sneathia_sanguinegens were only found in HPV-infected women. This study showed that the type of HPV infection was associated with the composition of the vaginal microbiota. Further studies on HPV genotypes and vaginal microbiota are necessary to uncover more mysteries of their association and provide a promising therapeutic target as well as low-cost future therapeutic strategies.

Prevalent characteristics of human papillomavirus infection in 29,508 women in Jinshan District, Shanghai

ObjectiveThis study aimed to investigate the prevalence characteristics of human papillomavirus (HPV) infections in 29,508 female cases in Jinshan District, Shanghai. Materials and methodsThe current research represents a retrospective study that included 29,508 women who received cervical screening in the Jinshan Branch of Shanghai Sixth People's Hospital from 2010 to 2020. The overall prevalence, distribution types, age-specific prevalence and annual trends were analysed. ResultsThe overall HPV infection rate among the patients was 21.97%. They were primarily high-risk HPV (HR-HPV) infections (20.30%) and single HPV infections (15.91%). A general decline in HPV and HR-HPV prevalence was observed with time from 33.52% to 25.45% in 2011 to 21.47% and 20.18% in 2020. The most common HPV genotypes were HPV52, HPV16, HPV58, HPV51, HPV53 and HPV68. The infection rates of HPV genotypes, including HPV6, HPV11, HPV16, HPV18, HPV31, HPV33, HPV35, HPV39, HPV42, HPV43, HPV66 and HPV68, showed overall declines across different years. The age-specific prevalence of total HPV and HR-HPV infections formed an approximate U-shaped curve with two peaks, one in the ≥55 age group (29.75%, 28.43%) and the other in the <25 age group (22.93%, 20.85%). Both total HPV and HR-HPV infection rates decreased to their lowest in the 25–34-year age groups. ConclusionsThe prevalence of HPV infections showed a downward tendency with time. Single HPV genotype infections and HR-HPV infections were predominantly detected. The after prevalent characteristics of HPV can help to guide HPV vaccinations and cervical cancer screenings: 1) non-HPV16/HPV18 H R-HPV genotypes were prevalent;2) non-vaccine-covered HPV53, HPV51 and HPV68 were also prevalent; 3) women above the age of 55 years had the highest HPV and HR-HPV infection rates.

Human papillomavirus (HPV) genotype prevalence and impact of COVID-19 on the HPV prevention program in Duhok city.

Introduction and aimsHuman papillomavirus (HPV) is a sexually transmitted virus that can cause cervical cancer. This study aimed to investigate the prevalence of HPV infection, the prevalent HPV genotypes in women and men with recurrent genital infections, and the impact of the coronavirus disease 2019 (COVID-19) pandemic on the HPV prevention program.Materials and methodsThis cross-sectional study was conducted in Duhok city, in the Kurdistan Region of Iraq, between January 2018 and September 2020. We recruited patients from an infectious disease clinic, who were married, were older than 18 years, and agreed to participate in this study. A reverse hybridisation-based assay was used to identify the HPV genotypes prevalent in these patients.ResultsAmong the patients in the study, 20.9% (67/320) tested positive for HPV infection. The HPV prevalence in females was 18.8% (52/276), which was lower than that in males (34.1%,15/44) (p = 0.21). Thirty-six patients (11.3%) were infected with a single HPV genotype, seventeen (5.3%) with two HPV genotypes, eight (2.5%) with three HPV genotypes, and the remaining six (1.8%) with four or more HPV genotypes. The most common genotypes detected among the patients were HPV-6 (7.2%), −11 (3.8%), and −16 (3.4%). The prevalence of all HPV genotype infections was highest and lowest in the 18–25- and 36–45-year age groups, respectively (X2 = 8.24; p = 0.041). The number of referred patients substantially reduced from 12 patients per month to 1 patient per month during the COVID-19 pandemic.ConclusionHPV infection was common in the study population. The most common genotypes were HPV 6, 11, and 16, against which vaccines are available. Further population-based studies are needed to investigate the prevalence of such an infection.

Evaluation of the characteristics of multiple human papillomavirus (HPV) infections identified using the BD Onclarity HPV assay and comparison with those of single HPV infection.

BackgroundHuman papillomavirus (HPV) infection is a major cause of cervical cancer and associated precursor lesions. Multiple HPV genotype infections have been reported. However, their clinicopathological characteristics still remain elusive.MethodsFor this study, 814 consecutive patients who had undergone colposcopy and HPV genotyping test using BD Onclarity HPV assay were retrospectively selected. Clinicopathological parameters of multiple HPV infections were compared with those of single HPV infection.ResultsMultiple HPV infections were found in 110 out of 814 cases (13.5%). Multiple HPV infections were associated with a significantly higher incidence of high-grade intraepithelial lesions (HSILs) compared with single HPV infection. Other high-risk HPV genotypes, in addition to HPV 16, were found more frequently in the multiple HPV infections group; these included HPV 51, 52, 33/58, 56/59/66, and 35/39/68. No specific coinfection pattern was not identified. Additionally, the number of HPV genotypes in multiple HPV infections was not associated with the progression to HSIL or squamous cell carcinoma.ConclusionsMultiple HPV infections have distinct clinicopathological characteristics (compared with single HPV infection). As their biological behavior is uncertain, close and frequent follow-up is warranted.

Cervical cancer screening in high-altitude areas in China: A large cross-section study of 25,173 women in northern Tibet.

BackgroundCervical cancer has become a worldwide concern owing to its high incidence and mortality rates. To date, high-altitude areas of Tibet have not benefited from any large-scale cervical cancer screening programs. Therefore, we initiated a screening program to investigate the prevalence of human papilloma virus (HPV) and HPV genotype distribution to reveal cervical cancer and its precursor which lead to morbidity among women in the city of Nagqu in northern Tib3et.MethodsA total of 25,173 women were recruited to undergo HPV genotype tests between June and December 2019. Women infected with HPV 16 and/or 18 underwent colposcopy and histological examination. Women with other high-risk HPV type (hr-HPV) underwent cytological tests to determine whether to conduct further colposcopy and histological examination for diagnosis. HPV prevalence was calculated in the total population and further stratified according to various parameters, such as age group, area location (altitude level), and single or mixed infection status. The HPV genotype distribution was also investigated accordingly. Cervical lesions revealed by further colposcopic findings were also analyzed; high-grade and malignant lesion morbidities were calculated in total and in each county. Most data were collected and analyzed using descriptive and consistency check statistical methods, and a risk factor investigation for HPV infection was performed using logistic regression models.ResultsThe total HPV infection rate among women in Nagqu was 13.42%. Of the 25,173 women in the study, 999 (3.97%) were HPV 16/18 positive, 2,379 (9.45%) were other hr-HPV-positive, and 21,795 (86.58%) were HPV-negative. The five most common HPV genotypes, accounting for more than 60% of all HPV infections in Nagqu people, were HPV 16, 58, 31, 18, and 52. Tibetan women younger than 20 years and older than 60 years were the two age groups with the highest rates of HPV infection, 26.7% and 19.8%, respectively. Among the HPV-positive women, 2,656 (78.33%) were infected with a single strain and 732 (21.67%) were infected with multiple strains (more than two genotypes). HPV prevalence increased in high-altitude areas (positive rate highest in Nyima with an altitude of 5,000 m, 23.9%) and decreased in relatively low-altitude areas (positive rate lowest in Lhari with an altitude of 4,000 m, 6.6%). Multiple analyses showed that age, parity, age at first delivery, and altitude of residence were independent factors facilitating HPV infection in Tibetan women. High-grade and malignant cervical lesions revealed by histological findings were different among living locations, with the highest rates in Xainza, Baingoin, and Nyainrong, these being 2.019%, 1.820%, and 1.116%, respectively, among women in these areas.ConclusionOur survey provides an overall perspective on HPV genotype infection and cervical lesions in women in northern Tibet. The data not only provide useful information for the treatment of cervical lesions but also has great value in terms of the primary and secondary prevention measures that can be taken for women living in these regions.Clinical Trial Registration www.chictr.org.cn, indentifier ChiCTR2000035061.

Burden and Associated Genotype Patterns of High-Risk Human Papilloma Virus Infection and Cervical Cytology Abnormalities among Women in Central India.

Background The epidemiology of human papilloma virus (HPV) infection and the pattern of HPV genotype distribution are much-needed parameters to assess the risk of cervical cancer among females. However, due to less availability of data on HPV burden and its genotypes from various geographical regions in India makes cervical cancer screening modalities and vaccination strategies difficult to implement. Objective The present study was conducted to identify the various genotypes particularly high-risk HPV types in premalignant or malignant cervical lesions. Methods The study was a hospital-based cross-sectional study wherein 295 symptomatic women were screened by Pap smear and multiplex real-time PCR was performed for HPV genotypes identification in women with abnormal cervical cytology. Results Out of 295 women, 237 (80.3%), 45 (15.3%), and 13 (4.4%) women had normal Pap smear, squamous cell carcinoma and precancerous cytology, respectively. Among these 58 women having abnormal cervical cytology, HPV was detected in 48 (81.0%) participants. Most common HPV genotypes in our study were HPV 16 (n = 29; 60.4%) followed by mixed infections; i.e., more than one type of HPV was detected (n = 10, 20.8%). HPV 18 was detected only in 6.25%, whereas other high-risk HPV genotypes were found to be 12.5%. Conclusion HPV positivity was >80% in women having abnormal Pap smear. The prevalence of HPV 18 was found to be much less in Central India, compared to other parts of country. HPV 16 was the most common genotype followed by mixed HPV genotype infections. It is evident from our study that symptomatic women even if having normal Pap smear should be screened for HPV and followed up with periodic Pap smears for detecting any change in cervical cytology, thus preventing cervical cancer in women.

Prevalence and distribution of human papillomavirus (HPV) in Luoyang city of Henan province during 2015\u20132021 and the genetic variability of HPV16 and 52

BackgroundPersistent high-risk Human papillomavirus (HPV) subtypes infection has been implicated as a causative of cervical cancer. Distribution and genotypes of HPV infection among females and their variations would assist in the formulation of preventive strategy for cervical cancer. The purpose of the present study is to investigate the prevalence of HPV among females in central China.MethodsThe distribution and genotypes of HPV among 9943 females attending the gynecological examinations in central of China during 2015–2021 were investigated. HPV genotypes were detected using a commercial kit. Nucleotides sequences of L1, E6 and E7 genes in HPV16 or HPV52 positive samples collected in 2021 were amplified by polymerase chain reaction (PCR). Variations of L1, E6 and E7 in HPV16 and HPV52 were gained by sequencing and compared with the reference sequence. Sublineages of HPV16 and HPV52 were determined by the construction of phylogenetic tree based on L1 gene.ResultsThe overall prevalence of HPV infection was 22.81%, with the infection rate of high-risk human papillomavirus (HR-HPV) was 19.02% and low-risk human papillomavirus (LR-HPV) was 6.40%. The most top five genotypes of HPV infection were HPV16 (7.49%), HPV52 (3.04%), HPV58 (2.36%), HPV18 (1.65%) and HPV51 (1.61%). Plots of the age-infection rate showed that the single HPV, multiple HPV, HR-HPV, LR-HPV infection revealed the same tendency with two peaks of HPV infection were observed among females aged ≤ 20 year-old and 60–65 year-old. The predominant sublineage of HPV16 was A1 and B2 for HPV52. For HPV16, The most prevalent mutations were T266A (27/27) and N181T (7/27) for L1, D32E for E6 and S63F for E7 in HPV16. For HPV52, all of the nucleotide changes were synonymous mutation in L1 (except L5S) and E7 genes. The K93R mutation was observed in most HPV52 E6 protein.ConclusionsThe present study provides basic information about the distribution, genotypes and variations of HPV among females population in Henan province, which would assist in the formulation of preventive strategies and improvements of diagnostic probe and vaccine for HPV in this region.

Epidemiological Characteristics and Genotypes of Human Papillomavirus Infection in Mianyang

Objective: To investigate the epidemiological characteristics and genotypes of human papillomavirus (HPV) infection in Mianyang. Methods: The cervical samples of 27,040 patients, who visited the Department of Gynecology of The Third Hospital of Mianyang from January 2018 to January 2020, were collected. Results: The HPV-positive infection rate was 21.40% (5,787/27,040); the single HPV infection rate was 72.04% (4,169/5,787); the double HPV infection rate was 19.73% (1,142/5,787); the triple and above HPV infection rate was 8.22% (476/5,787); the top five high-risk HPV subtypes with the highest infection rates were HPV52, 16, 58, 53, and 51, while the top five low-risk HPV subtypes with the highest infection rates were HPV81, 42, 43, 6, and 11; there were significant differences in the HPV-positive infection rate, high-risk infection rate, low-risk infection rate, and multiple infection rate among different age groups (P &lt; 0.05). Conclusion: The HPV infection rate in Mianyang was 21.4%, in which the majority of the cases were single infection; the high-risk HPV subtypes were HPV52, 16, 58, 53, and 51; the HPV-positive infection rate, high-risk infection rate, low-risk infection rate, and multiple infection rate were high in the middle but low at both ends in the context of age distribution; the top three age groups with the highest infection rates were 45-49, 40-44, and 30-34.

Prevalence and genotype distribution of human papillomavirus among 29263 women from the Longgang community of Shenzhen.

Human papillomavirus (HPV) vaccination and screening have been demonstrated to be effective methods for controlling cervical cancer. To provide evidence for effective targeted interventions, we investigated the HPV prevalence and genotypic distribution in women of different age groups in the Longgang community of Shenzhen, China. A total of 29263 women were enrolled in the Longgang District Central Hospital from January 2018 to October 2020. Cervical specimens were collected at enrolment. We used a polymerase chain reaction diagnostic kit to detect the genotypes of HPV. The prevalence of overall, high-risk, probable high-risk (PHR) and low-risk HPV infection was 8.2% (95% confidence interval [CI] 7.9 to 8.5), 6.9% (95% CI 6.6 to 7.2), 0.9% (95% CI 0.8 to 1.1) and 1.6% (95% CI 1.5 to 1.8), respectively. High-risk HPV genotype infections accounted for 84.4% (95% CI 82.8 to 85.8) of the overall HPV infections. The five most predominant genotypes were HPV-52, HPV-16, HPV-58, HPV-53 and HPV-51. Our study found that the prevalence of HPV infection increased with age and women 55-59y of age presented the highest HPV prevalence. As a PHR subtype, HPV-53 has a higher infection rate in women. HPV-52, HPV-16 and HPV-58 are common infection genotypes.

Prevalence and Genotypes of Human Papillomavirus among Men in Yunnan Province, China.

Human papillomavirus (HPV) infection is one of the most commonly reported sexually transmitted infections. Persistent infection is associated with the development of cervical cancer. Men may serve as a medium for HPV transmission among women, and much remains unknown about the prevalence and genotypes of HPV infection among men in Yunnan Province. This study was designed to investigate these parameters among men in this region. Clinical information was collected from January 2015 to May 2020, and 369 men who wished to undergo tests for HPV were included in this study. HPV screening was performed using the HPV GenoArray Test Kit, and the anatomical site of sample collection was the urethra. The mean age was 36.15 ± 9.08 years. Our results revealed that the prevalence of HPV infection among men was 23.85% in Yunnan Province, and there was no association between age and HPV prevalence in men. There were 72 (81.81%) individuals positive for a single HPV type, and 16 (18.19%) positive for multiple HPV types. The most prevalent oncogenic HPV types were found to be 52, 51, and 16. The most prevalent types of HPV infection were similar between men and women in this region, though the prevalence of HPV in men was relatively high.

Where do we Stand on the Molecular Diagnostics for the Detection of Human Papillomavirus? A Comprehensive Review

Human Papillomavirus (HPV) infection plays a vital role in the development of various cancers and imposes a burden on the global health. Detection of HPV infection is a challenge to the healthcare industry as they cannot be cultured and their fascinating life cycle limits the accuracy of serological methods. Hence, molecular assays are the gold standard for the accurate viral detection and genotyping. Currently, various methodologies are available with great sensitivity and specificity and are even clinically validated as screening tools. However, there are only a few that are approved by the US Food and Drug Administration and much more are suitable only for research. At present, the molecular methods available mainly depend on signal amplification, nucleic acid amplification and hybridisation assays for detecting and genotyping HPV infection, all with their own advantages and drawbacks. There is further scope for the development of more reliable, cost-effective diagnostic techniques that can harness the power of next genome sequencing, mutation analysis and may further improve the chances of early detection and prevention of HPV related cancers. This review summarises the present knowledge on currently available molecular diagnostic tests for the detection and genotyping of HPV infection and their possible utility in a clinical setting.

The Contrast Research of Human Papillomavirus Infection Genotypes in Tissues of Cervical Squamous Cell Carcinoma and Cervical Adenocarcinoma

The large sample data of HPV genotypes on cervical squamous cell carcinoma (CSCC) and cervical adenocarcinoma (CAC) tissues are rarely reported in China. This study is aimed to investigate the clinical value of distribution of different kinds of genotypes of human papillomavirus (HPV) within regional (mainly in Jiangsu Province in China) patients of both CSCC and CAC. We collected tissue samples of from 1044 women with CSC (826 cases) and CAC (218 cases) in 29 hospitals of 6 Provinces in China from November 1978 to December 2017. HPV DNA was extracted and 23 genotypes of HPV were detected through the combination of gene-chip and polymerase chain reaction (PCR) technology to analyze distribution of HPV infection types among subjects of CSCC and CAC. Among 1044 cases of CSCC and CAC, 901 were found HPV positive making total HPV detection rate of 86.30% (901/1044). Total detection rate of CSCC was 91.53% (756/826) in which 16, 18, 58, 33, 52, 31 were the most common types where detection frequencies of 16 and 18 types were 56.84% (544/957) and 9.93% (95/957), respectively. Total detection rate of CAC was 66.51% (145/218) in which 16, 18, 31, 33, 52, 58 were the most common types where detection frequencies of 16 and 18 types were 35.29% (84/238) and 32.35% (77/238), respectively. The HPV detection rates were different in female CSCC and CAC tissues of region studied. The 16, 18, 31, 33, 52 and 58 types are the most common genotypes found in two sorts of cervical carcinoma tissues. The detection rate of 16 types was higher than 18 types in CSCC and were very close in CAC tissues. Conducting HPV genotypes detection of CSCC and CAC will help doctors evaluating onset risk of cervical carcinoma and tracking HPV infected patients with high cancerogenic risk. The early detection will play important role in prevention and treatment of female CSCC and CAC in our nation.

HPV Infection Downregulates the Expression of Autophagy-Related Genes in Condylomata Acuminata

Background: Condylomata acuminata are benign anogenital warts caused by human papillomavirus (HPV) infection with a high recurrence rate. Autophagy plays an important role in maintaining internal environmental stability. However, the role of autophagy regulation in the anogenital warts caused by HPV infection remains unknown. Objective: A multigroup case-control study was designed to identify the autophagy gene fingerprint involved in anogenital warts arising from infections with different HPV genotypes. Methods: Human autophagy PCR arrays were performed on the initial 18 participants grouped by their different HPV genotypes for gene expression-profiling analysis. The negative control was skin samples collected during plastic surgery on the chest from a group of individuals who showed none of the clinical symptoms or evidence of HPV infection. Real-time polymerase chain reaction (qPCR) was performed to validate the microarray results in another 24 individuals. Results: Out of 84 genes involved in autophagy, different autophagic responses were found among the 29 genes that encode autophagy machinery components, and expression levels of 13 of these genes were downregulated. Finally, we verified that the expression levels of 2 key genes that participate in the formation of autophagosomes, ATG3 and ­BECLIN1, were downregulated in the HPV infection groups independently of genotype compared with the control group. Conclusions: These findings showed that HPV infection downregulated the expression of ATGs in CA. Additionally, there were no differences in the expression of ATGs between the different HPV genotype infection groups. This study provided new insights into the autophagic response to HPV infection.

Effectiveness of physically ablative and pharmacological treatments for anal condyloma in HIV-infected men.

BackgroundThere is limited information on the effectiveness of available treatments for anal condyloma acuminata in HIV-1-infected men.AimTo provide data on the effectiveness of electrosurgical excision, infrared coagulation and pharmacological (imiquimod) treatments for anal condyloma acuminata (peri-anal and/or intra-anal) in HIV-1-infected men based on authors’ practice.MethodsSingle-center, retrospective descriptive analysis of HIV-1-infected men, 18 years or older treated for anal condyloma acuminata. Standard treatments were offered: electrosurgery excision, infrared coagulation and topical imiquimod. Effectiveness was evaluated by the recurrence rate at 1 year after treatment. Recurrence was defined as any anal condyloma acuminata diagnosed after 3 months of condyloma-free survival post-treatment. Anal cytology and human-papillomavirus-infection (HPV) was assessed.ResultsBetween January 2005 and May 2009, 101 men were treated for anal condyloma acuminata: 65 (64%) with electrosurgery, 27 (27%) with infrared coagulation and 9 (9%) with imiquimod. At 1 year after treatment, the cumulative recurrence rate was 8% (4/65, 95%CI: 2–15%) with electrosurgery excision, 11% (3/27, 95%CI: 4–28%) with infrared coagulation and 11% (1/9, 95%CI: 2–44%) with imiquimod treatment. No predictive factors were associated with recurrence.Anal HPV-6 or HPV-11 was detectable in 98 (97%) patients and all had high-risk HPV genotypes, and 89 (88%) patients had abnormal anal canal cytology. Limitations: this was a retrospective descriptive analysis; limited to a single center; it cannot know if the recurrence is related to new infection.ConclusionRecurrence of anal condyloma after any treatment was common. Abnormal anal cytology and high-risk HPV-infection were highly prevalent in this population, therefore at high-risk of anal cancer, and warrants careful follow-up.

GENOTIPE HPV DAN POLA INFEKSINYA TERKAIT JENIS HISTOPATOLOGI KANKER LEHER RAHIM (HPV Genotype and HPV Infection Pattern Related to the Histopathological Type of Cervical Cancer)

Uterine cervical cancer is the number one cause of female cancer in Indonesia. The most common cause is a persistent infection with ahigh risk of HPV (Human Papilloma Virus). A multiple genotype HPV infection with high risk HPV increases the poor prognosis of cervicalcancer. Non-squamous type of cervical cancer has a poorer prognosis than the squamous type. The aim of this study was to know theassociation between HPV genotype and HPV infection pattern with histopathological type of cervical cancer in patients of the Dr. SoetomoHospital, Surabaya. This study was an analytical observational-cross sectional research. The samples consisted of 44 uterine cervicalswabs from the Oncology Out-Patient Clinic, proven and diagnosed as the related cervical cancer in October-November 2012. PCR resultsshowed 33 HPV positive, seven (7) HPV negative and 4 invalid. The Fisher's exact test was only done for the 33 positive HPV samples.The positive HPV samples consisted of 24 squamouse and nine (9) non-squamous types of uterine cervical cancer. A 90.9% (30/33) highrisk HPV genotypes was found in 72.7% (24/30) squamous and 18.2% (6/30) non-squamous types. Multiple genotype HPV infection9.1% (3/33) occurred in 3% (1/3) squamous and 6.1% (2/3) non-squamous types. There was a significant association between HPVgenotype and histopathological type of the uterine cervical cancer (p=0.015). There was no association between HPV infection patternand histopathological type of uterine cervical cancer (p=0.174). Negative HPV and low risk HPV genotypes can be found in uterine cervicalcancer patients of the Dr. Soetomo Hospital, Surabaya. Multiple genotype HPV infections have a 2.86 times greater risk in developingnonsquamous type with a poor prognosis.

The Risk of Human Papillomavirus Infection for Spontaneous Abortion, Spontaneous Preterm Birth, and Pregnancy Rate of Assisted Reproductive Technologies: A Systematic Review and Meta-Analysis

Aims: To estimate the impact of human papillomavirus (HPV) infection on spontaneous abortion, spontaneous preterm birth (sPTB), pregnancy rate of females undergoing assisted reproductive technologies (ART), and spontaneous abortion of ART pregnancy. Methods: PubMed, Medline, Embase, and the Cochrane Library were searched until December 16, 2016. The OR or relative risk (RR) with its corresponding 95% CI were selected as effect size. Subgroup analysis of HPV genotype infection (high-risk HPV [HR-HPV] or indiscriminate genotype) was performed. Statistical analysis was conducted using STATA 12.0. Results: Eighteen studies were included. Eight studies revealed no significant association between HPV infection and spontaneous abortion (OR 1.40, 95% CI 0.56–3.50). However, subgroup analysis showed indiscriminate genotype HPV infection increased the ratio of spontaneous abortion with OR of 2.24 (95% CI 1.37–3.65), while HR-HPV infection had no significant effect (OR 0.65, 95% CI 0.21–1.98). The results indicated that HR-HPV infection was a risk for sPTB with a pooled OR of 2.84 (95% CI 1.95–4.14). HPV infection was found to be independent of the ART-based clinical pregnancy rate (RR 1.04, 95% CI 0.64–1.70) and spontaneous abortion of ART pregnancy (RR 1.47, 95% CI 0.86–2.50). Conclusions: Indiscriminate HPV genotype infection can increase the risk of spontaneous abortion and HR-HPV infection was a risk factor for sPTB. However, there was not enough evidence to indicate the association between HPV infection and pregnancy rate of ART, and spontaneous abortion of ART pregnancy. Different genotypes of HPV infection may play a discrepant role in adverse pregnancy outcomes.

Genotyping of high-risk anal human papillomavirus (HPV): ion torrent-next generation sequencing vs. linear array

BackgroundOur next generation sequencing (NGS)-based human papillomavirus (HPV) genotyping assay showed a high degree of concordance with the Roche Linear Array (LA) with as little as 1.25 ng formalin-fixed paraffin-embedded-derived genomic DNA in head and neck and cervical cancer samples. This sensitive genotyping assay uses barcoded HPV PCR broad-spectrum general primers 5+/6+ (BSGP)5+/6+ applicable to population studies, but it’s diagnostic performance has not been tested in cases with multiple concurrent HPV infections.MethodsWe conducted a cross-sectional study to compare the positive and negative predictive value (PPV and NPV), sensitivity and specificity of the NGS assay to detect HPV genotype infections as compared to the LA. DNA was previously extracted from ten anal swab samples from men who have sex with men in Nigeria enrolled on the TRUST/RV368 cohort study. Two-sample tests of proportions were used to examine differences in the diagnostic performance of the NGS assay to detect high vs. low-risk HPV type-specific infections.ResultsIn total there were 94 type-specific infections detected in 10 samples with a median of 9.5, range (9 to 10) per sample. Using the LA as the gold standard, 84.4% (95% CI: 75.2–91.2) of the same anal type-specific infections detected on the NGS assay had been detected by LA. The PPV and sensitivity differed significantly for high risk (PPV: 90%, 95% CI: 79.5–96.2; sensitivity: 93.1%, 95% CI: 83.3–98.1) as compared to low risk HPV (PPV: 73%, 95% CI: 54.1–87.7; sensitivity: 61.1, 95% CI: 43.5–76.9) (all p < 0.05). The NPV for all types was 92.5% (95% CI: 88.4–95.4). The NPV and specificity were similar for high and low risk HPVs (all p > 0.05). The NGS assay detected 10 HPV genotypes that were not among the 37 genotypes found on LA (30, 32, 43, 44, 74, 86, 87, 90, 91, 114).ConclusionsThe NGS assay accurately detects multiple HPV infections in individual clinical specimens with limited sample volume and has extended coverage compared to LA.
