A case of full mouth rehabilitation of patient who lost occlusal support using attachment

患者は15 年ほど，歯科医院を受診しておらず，奥歯のかみ合わせがないことを主訴に，全顎的な歯科治療を希望して来院した．口腔内所見では，臼歯部は残根状態で咬合支持が喪失しており，閉口時に反対咬合所見を呈していた．医療面接，口腔内診察・検査，エックス線検査，摂取可能食品質問表から，仮性反対咬合と咬合支持の喪失による咀嚼および審美障害と診断した．治療方針として，診察・検査，診断後，不良な口腔状態を改善し，治療用義歯を装着して下顎位の安定化を図ってから，最終補綴装置の製作に移行する治療計画を立案した．患者の了承を得た後，歯冠修復処置，磁性アタッチメントと歯冠外アタッチメントを用いてフルマウスリハビリテーションを行った．咬合支持を喪失した患者にフルマウスリハビリテーションを行った結果，仮性反対咬合の改善が認められ，審美的回復が得られた．初診時の咀嚼スコアは48.2 であったが，最終補綴装置装着後の咀嚼スコアは94.6 となり，咀嚼機能および発音機能の回復が確認でき，患者のクオリティ・オブ・ライフの向上に寄与できた．【顎咬合誌 39 （1･2）:35-44，2019】

Multidisciplinary Approach for Full-Mouth Rehabilitation of an Adult Patient with Collapsed Occlusal Plane and Several Missing Teeth: A Clinical Report.

In the full-mouth rehabilitation of a patient with severely collapsed occlusion, a multidisciplinary approach including prosthodontics, orthodontics, endodontics, and surgery could provide better esthetic and functional results. Orthodontic treatment aiming for stable occlusion that meets the standard requirement of functional occlusion, enables the maintenance of occlusal function, and guarantees long-term stability on the basis of periodontal health was attempted. The essential element of this protocol is careful treatment planning based on the optimal sequence pursuing functional occlusion.

Gradual occlusal attrition is a normal process of aging. However, severe attrition causes pathogenic pulp, occlusal disharmony, functional disorder and esthetic problems. Alteration of vertical dimension should be considered for space regaining for tooth restoration, esthetic improvement or correction of occlusal relationship. Vertical dimension should be determined within the range of minimal invasive process satisfying patient's esthetic requirements and operator's functional goal. And patient's adaptation to newly determined vertical dimension should be assessed simultaneously. Deep overbite is not a simple problem of overbite, instead it is an usually complicated problem with anterior-posterior occlusal relationship. Considering these facts, appropriate restoration of edentulous part as well as improvement of anterior-posterior relationship should be performed to solve this fundamental problems. In this study, a 67 year-old male patient with many worn teeth and loss of posterior teeth was treated with removable partial denture at edentulous mandibular area to increase vertical dimension and fixed prostheses at dentulous maxillary and mandibular area. With these treatments, we attained a satisfactory result in functional and esthetic aspects as a report case. (J Korean Acad Prosthodont 2016;54:65-71)
