Magnetic resonance imaging of a stream of bubbles injected into liquid suspensions

Magnetic resonance imaging (MRI) is a powerful tool for characterizing opaque multiphase flows non-invasively. However, MRI has often (i) had low temporal resolution and thus not captured transient dynamics, (ii) only provided 2D slice images of 3D flows and (iii) been limited to flows in narrow (∼30 mm) tubes with significant wall effects. Here, we apply multi-band echo planar imaging (MB-EPI) with a custom-built radiofrequency coil in a full-body MRI scanner to provide fully 3D images of the dynamics of a stream of bubbles rising through a dense suspension with 151 ms resolution in a 178 mm diameter system. Image processing demonstrates that bubble rise and coalescence dynamics vary significantly with (a) initial spacing between bubbles and (b) particle volume fraction. The ability to image bubble dynamics in dense suspensions as well as in full 3D provides future opportunities to characterize complex, non-axisymmetric, multiphase flows.

Disentangling the impact of gluteofemoral versus visceral fat accumulation on cardiometabolic health using sex-stratified Mendelian randomization

Background and aimsIndividuals with a higher abdominal adipose tissue accumulation are at higher risk of developing cardiometabolic diseases. For a given body mass index (BMI), women typically present lower abdominal adipose tissue accumulation compared to men. Whether abdominal adiposity is a causal driver of cardiometabolic risk, or a mere marker of ectopic fat deposition is debated. MethodsWe investigated the sex-specific and sex-combined impact of height and BMI-adjusted gluteofemoral adipose tissue (GFATadj) adjusted abdominal subcutaneous adipose tissue (ASATadj) and adjusted visceral adipose tissue (VATadj) on cardiometabolic traits and diseases using Mendelian randomization. ResultsLeveraging genome-wide summary statistics on GFATadj, ASATadj and VATadj from 39,076 UK Biobank participants with full-body magnetic resonance imaging available, we found that GFATadj is associated with a more favourable cardiometabolic risk profile including lower low density lipoprotein (LDL) cholesterol, triglycerides, fasting glucose, fasting insulin, liver enzyme levels and blood pressure as well as higher high density lipoprotein (HDL) cholesterol levels. GFATadj also is negatively associated with ischemic stroke, coronary artery disease, type 2 diabetes, and non-alcoholic fatty liver disease (NAFLD). ASATadj is not associated with cardiometabolic traits and diseases, whereas VATadj is associated with liver fat accumulation but not with NAFLD or other cardiometabolic traits or diseases. Although the absolute effect sizes of GFATadj on LDL cholesterol were more pronounced in women compared to men, most associations did not differ by sex. ConclusionsThe inability of subcutaneous fat depots to efficiently store energy substrates could be the causal factor underlying the association of visceral lipid deposition with cardiometabolic health.

Energy Expenditure, Body Composition, and Skeletal Muscle Oxidative Capacity in Patients with Myotonic Dystrophy Type 1.

Myotonic dystrophy type 1 (DM1) patients are at risk for metabolic abnormalities and commonly experience overweight and obesity. Possibly, weight issues result from lowered resting energy expenditure (EE) and impaired muscle oxidative metabolism. This study aims to assess EE, body composition, and muscle oxidative capacity in patients with DM1 compared to age-, sex- and BMI-matched controls. A prospective case control study was conducted including 15 DM1 patients and 15 matched controls. Participants underwent state-of-the-art methodologies including 24 h whole room calorimetry, doubly labeled water and accelerometer analysis under 15-days of free-living conditions, muscle biopsy, full body magnetic resonance imaging (MRI), dual-energy x-ray absorptiometry (DEXA), computed tomography (CT) upper leg, and cardiopulmonary exercise testing. Fat ratio determined by full body MRI was significantly higher in DM1 patients (56 [49-62] %) compared to healthy controls (44 [37-52] % ; p = 0.027). Resting EE did not differ between groups (1948 [1742-2146] vs (2001 [1853-2425>] kcal/24 h, respectively; p = 0.466). In contrast, total EE was 23% lower in DM1 patients (2162 [1794-2494] vs 2814 [2424-3310] kcal/24 h; p = 0.027). Also, DM1 patients had 63% less steps (3090 [2263-5063] vs 8283 [6855-11485] steps/24 h; p = 0.003) and a significantly lower VO2 peak (22 [17-24] vs 33 [26-39] mL/min/kg; p = 0.003) compared to the healthy controls. Muscle biopsy citrate synthase activity did not differ between groups (15.4 [13.3-20.0] vs 20.1 [16.6-25.8] μM/g/min, respectively; p = 0.449). Resting EE does not differ between DM1 patients and healthy, matched controls when assessed under standardized circumstances. However, under free living conditions, total EE is substantially reduced in DM1 patients due to a lower physical activity level. The sedentary lifestyle of DM1 patients seems responsible for the undesirable changes in body composition and aerobic capacity.

Sacral neuromodulation: Rechargeable versus non-rechargeable device. What would the patient preferences be in France?

Sacral neuromodulation (SNM) is a minimally invasive technique that provides effective treatment for the management of refractory overactive bladder (OAB), non-obstructive urinary retention (NOUR), and fecal incontinence (FI). This study assessed patient preferences between the currently available non-rechargeable SNM device and a new, full-body magnetic resonance imaging (MRI)-safe, smaller, rechargeable device. An online cross-sectional survey was conducted among French OAB, NOUR, FI patients, recruited via a market research vendor. To assess their preferences, patients were asked to indicate their level of agreement with 10 statements regarding the size of the device, its rechargeability, and the role of MRI using a 6-item Likert scale. A descriptive statistical analysis was performed. In all, 95 patients (68% women), mean age 50 years, were included in the study: 51% were treated for OAB; 44% received an oral treatment and 28% had SNM. Overall, 71% of the 95patients indicated a preference for the new device; 75% considered that recharging the device would not impact their lifestyle; 74% believed that the smaller size of the rechargeable device would facilitate their choice to be treated with SNM; 80% found full-body MRI compatibility important. Most patients may prefer the new rechargeable SNM device over the current "standard". Compatibility with full-body MRI and the smaller device size seemed the key features of the newer device that would influence their choice of being treated with SNM. Future national and international recommendations should consider a shared decision-making process between the physician and the patient.

CONGENITAL MUSCULAR DYSTROPHIES: EP.69 LAST STRONG: LAMA2 and SELENON to study trial readiness, outcome measures and natural history

SELENON (SEPN1)-related myopathy (SELENON-RM) is a rare congenital myopathy characterized by slowly progressive proximal muscle weakness, early onset rigidity of the spine and respiratory insufficiency. Patients with mutations in LAMA2 gene causing merosin-deficient congenital muscular dystrophy (LAMA2-MD) have a similar clinical phenotype, with a heterogenous disease spectrum ranging from a severe, early-onset congenital (complete laminin α2 deficiency) to a mild, childhood-onset limb-girdle type muscular dystrophy (partial laminin α2 deficiency). For both SELENON-RM and LAMA2-MD, no curative treatment options exist, yet promising preclinical studies are ongoing. Currently, there is a paucity on natural history data. Appropriate clinical and functional outcome measures need to be selected to reach trial readiness. We are performing a natural history study in Dutch-speaking patients diagnosed with SELENON-RM (n=10, mean age=18 (3-50) years, F=3, M=7) and LAMA2-MD (n=20, mean age=19 (3-50) years, F=15; M=5). Patients have four visits over a period of 1.5 year, with an interval of six months. At all visits, they undergo standard neurological examination, hand-held dynamometry (age≥5 years), functional measurements, questionnaires (patient report and/or parent proxy; age≥2 years), muscle ultrasound including ultrasound of the diaphragm, pulmonary function tests (spirometry, maximal inspiratory and expiratory pressure, sniff nasal inspiratory pressure; age≥5 years), and accelerometry for eight days; at visit one and three, they undergo cardiac evaluation (electrocardiogram, echocardiography), X-ray of the spine (age≥2 years), Dual-energy X-ray absorptiometry (DEXA-)scan (age≥2 years) and full body magnetic resonance imaging (MRI) (age≥10 years). Hereby we aim to describe the natural history of patients diagnosed with SELENON-RM or LAMA2-MD and to select outcome measures for future clinical trials. Our natural history study is an essential step to reach trial readiness.

Magnetic resonance imaging interactions with a sacral neuromodulation system.

AimsSacral neuromodulation (SNM) has successfully treated patients with functional urinary and/or bowel disorders for more than two decades. Historically, patients with the InterStim system (Medtronic) were contraindicated for Magnetic Resonance Imaging (MRI) scans. In 2012, Medtronic obtained Food and Drug Administration (FDA) approval for allowing 1.5 Tesla (T) MRI head scans. In September 2019, the Axonics System (Axonics) received FDA approval for 1.5 T full‐body MR Conditional labeling and then 3 T full‐body MR Conditional labeling in July 2020. In August 2020, Medtronic received 1.5 and 3 T full‐body MR Conditional labeling from the FDA for their new SNM systems (InterStim II and Micro devices with SureScanTM leads). With the advancements in MRI technology and availability of full‐body MRI eligible SNM systems, it is important for physicians to better understand MRI safety for these systems.MethodsThis paper explains the fundamentals of MRI physics, its interactions with active implantable medical devices (AIMDs), the subsequent potential safety hazards with emphasis on radio frequency (RF)‐related safety, and the risks associated with “Off‐label” scans, including abandoned and broken leads.ResultsMRI guidelines provided by the AIMD device manufacturer should be followed to ensure MRI scan safety and avoid any unnecessary risk to patients.ConclusionsMRI guidelines provided by the device manufacturer are the best resource for guidance for performing safe MRI scanning. Specific conditions should be fully understood and generalizations on MRI safety claims based on partial analysis or case studies should be avoided.

New Technologies and Applications in Sacral Neuromodulation: An Update

Recently rechargeable devices have been introduced for sacral neuromodulation (SNM) with conditional safety for full-body magnetic resonance imaging (MRI). Currently a recharge-free SNM device represents the standard implant; however, it is only approved for MRI head scans. As further new technologies with broader MRI capabilities are emerging, the advantages as well as disadvantages of both rechargeable versus recharge-free devices will be briefly discussed in this commentary from the perspective of patients, healthcare professionals, and providers.

Unexpected Fat Distribution in Adolescents With Narcolepsy.

Narcolepsy type 1 is a chronic sleep disorder with significantly higher BMI reported in more than 50% of adolescent patients, putting them at a higher risk for metabolic syndrome in adulthood. Although well-documented, the body fat distribution and mechanisms behind weight gain in narcolepsy are still not fully understood but may be related to the loss of orexin associated with the disease. Orexin has been linked to the regulation of brown adipose tissue (BAT), a metabolically active fat involved in energy homeostasis. Previous studies have used BMI and waist circumference to characterize adipose tissue increases in narcolepsy but none have investigated its specific distribution. Here, we examine adipose tissue distribution in 19 adolescent patients with narcolepsy type 1 and compare them to 17 of their healthy peers using full body magnetic resonance imaging (MRI). In line with previous findings we saw that the narcolepsy patients had more overall fat than the healthy controls, but contrary to our expectations there were no group differences in supraclavicular BAT, suggesting that orexin may have no effect at all on BAT, at least under thermoneutral conditions. Also, in line with previous reports, we observed that patients had more total abdominal adipose tissue (TAAT), however, we found that they had a lower ratio between visceral adipose tissue (VAT) and TAAT indicating a relative increase of subcutaneous abdominal adipose tissue (ASAT). This relationship between VAT and ASAT has been associated with a lower risk for metabolic disease. We conclude that while weight gain in adolescents with narcolepsy matches that of central obesity, the lower VAT ratio may suggest a lower risk of developing metabolic disease.

Designing High-Accuracy Permanent Magnets for Low-Power Magnetic Resonance Imaging

We present a highly scalable and versatile family of dipole magnet designs suitable for high-resolution magnetic resonance imaging (MRI), constructed entirely from cylindrical permanent magnet rods of two distinct radii. The multiplicity, $N$ , of the main set of rods (which dominate the generated field) can vary from 10 to 26. For each such $N$ , we present several designs that produce magnetic fields uniform to better than 100 parts per million (ppm) over spherical regions of radius half that of the bore. This design family includes peak fields ranging from 0.15 to 0.50 T, adjustable by changing $N$ , with the best accuracy obtained for $N=14$ with a peak field of 0.32 T. With optional correction rods that start out in a zero field configuration, these designs can be tuned to compensate for manufacturing errors simply by adjusting the rotations of these rods. Most of the 100 ppm accuracy radius can thereby be recovered, even in realistic imperfect configurations. We specifically present dimensions for full-body MRI with a standard bore (clearance) diameter of 70 cm, but this family of designs can be scaled uniformly to produce high-uniformity magnetic fields for applications at any length scale.

Assessing the use of the anatomical method for the estimation of sub-adult stature in Black South Africans

Stature estimation is rarely attempted in sub-adults due to the general lack of available standards as a result of the dearth of sufficiently large sub-adult skeletal collections with known demographic information. To overcome this problem sub-adult research mainly relies on modern imaging modalities. In the current study Magnetic Resonance Imaging (MRI) scans were used to assess the use of the anatomical method for stature estimation in sub-adults. A total of 53 Black South African sub-adult males (n=24) and females (n=29) aged between 10 and 17 years participated in the study by voluntarily completing a full-body MRI scan. A stadiometer was used to measure living stature prior to all MRI scans. Skeletal elements that contribute directly to stature were measured from the MRI scans using OsiriX and summed to compute the total skeletal height. Total skeletal height was calculated using the diaphyseal, maximum and physiological long bone lengths and correlated to living stature using Pearson’s correlations. Subsequently least squares regression equations were generated for the estimation of sub-adult stature. Results indicated strong, statistically significant positive correlations between living stature and total skeletal heights in sub-adult males, females and a combined sex sample. The regression equations were characterized by small standard error of estimates which are comparable to that reported for Black South African adults. Based on these results the anatomical method can be used to accurately describe living stature in Black South African sub-adults. This method is therefore encouraged as it will add valuable information when dealing with unknown sub-adult skeletal remains.

Nanoparticle Biokinetics in Mice and Nonhuman Primates.

Despite the preponderance of iron oxide nanoparticles (NPs) designed for theranostic applications, widespread clinical translation of these NPs lags behind. A better understanding of how NP pharmacokinetics vary between small and large animal models is needed to rapidly customize NPs for optimal performance in humans. Here we use noninvasive magnetic resonance imaging (MRI) to track iron oxide NPs through a large number of organ systems in vivo to investigate NP biokinetics in both mice and nonhuman primates. We demonstrate that pharmacokinetics are similar between mice and macaques in the blood, liver, spleen, and muscle, but differ in the kidneys, brain, and bone marrow. Our study also demonstrates that full-body MRI is practical, rapid, and cost-effective for tracking NPs noninvasively with high spatiotemporal resolution. Our techniques using a nonhuman primate model may provide a platform for testing a range of NP formulations.

The accuracy of the anatomical method for stature estimation in Black South African females

The anatomical method is considered the most accurate stature estimation method, but investigation has shown that it continuously underestimates stature. This underestimation is believed to be related to the use of universal soft tissue correction factors. Therefore, the aim of this study was to assess the accuracy of the soft tissue correction factors in a living population of Black South African females and to subsequently calculate a new soft tissue correction factor, specific for stature estimation in this population group. Thirty Black South African adult females voluntarily participated in this study and underwent a full body Magnetic Resonance Imaging (MRI) scan. Living stature was measured with a stadiometer and total skeletal height (TSH) was calculated from the MRI measurements. Stature was estimated from the TSH of each participant using Fully’s (1956) [17], Raxter et al.’s (2006) [38] and Bidmos and Manger’s (2012) [5] methods. Results indicated strong, statistically significant positive correlations between living and estimated statures, however, paired t-tests revealed that living stature was significantly underestimated using Fully’s and Raxter et al.’s methods, while the method by Bidmos and Manger significantly overestimated stature. A lack of statistically significant correlations between soft tissue correction factors and the total skeletal height was found. Likewise, an absence of statistically significant correlations between age and the estimation error, with and without age adjustments were also observed. A new soft tissue correction factor, specific for stature estimation in Black South African females was calculated. The newly proposed regression equation presented improved stature estimation accuracies for this population group.

Anesthetic Considerations and Perioperative Management of Spinal Cord Stimulators: Literature Review and Initial Recommendations

Background: Patients with implanted spinal cord stimulators (SCS) present to the anesthesia care team for management at many different points along the care continuum. Currently, the literature is sparse on the perioperative management. What is available is confusing; monopolar electrocautery is contraindicated but often used, full body magnetic resonance imaging (MRI) is safe with particular systems but with other manufactures only head and specific extremities exams are safe. Moreover, there are anesthetizing locations outside of the operating room where implanted SCS can interact with surrounding medical equipment and pose significant risk to patient and device. Objectives: The objective of this review is to present relevant known literature about the safe management of SCS in the perioperative period and to begin to develop recommendations. Study Design: A review of current literature and each manufacturers’ labeling was performed to assess risk of interference and patient harm between SCS and technology used in and around typical anesthetizing locations. Methods: A systematic search of the literature was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) statement. A computerized search was conducted for English articles in print up to April 2016 via PubMed www.ncbi.nlm.nih.gov/pubmed; EMBASE www.embase. com; and Cochrane Library www.thecochranelibrary.com. Search terms included “spinal cord stimulator AND MRI,” “spinal cord stimulator AND ECG,” “spinal cord stimulator AND implanted cardiac device,” “spinal cord stimulator AND electrocautery,” and “spinal cord stimulator AND obstetrics.” In addition, a search of Google and Google Scholar was performed. Websites of SCS manufactures were reviewed. Results: Generalized recommendations include turning the amplitude of the SCS to the lowest possible setting and turning off prior to any procedure. Monopolar electrocautery is contraindicated but is still often utilized; placing grounding pads as far away from the device can reduce the risk to device and patient. Bipolar cautery is favored. Implanted cardiac devices can interfere with SCS, but risks can be minimized. Neuraxial anesthesia can be attempted in a patient with implanted SCS, provided the device is not in the expected path. MRI labeling differences present the biggest difference among SCS manufactures. Medtronic’s SureScan SCS, Boston Scientific’s Precision system, St. Jude’s Proclaim, and Stimwave’s Freedome SCS are full body MRI compatible under specific conditions, while other manufacturers have labeling that restricts exams of the trunk and certain extremities. Limitations: This review was intended to be a comprehensive, cumulative review of recommendations for perioperative SCS management; however, the limitations of a review of this nature is the complete reliance on previously published research and the availability of these studies using the methods outlined. Conclusions: SCS is being used earlier in the treatment algorithm for patients with chronic pain. The anesthesia care team needs working knowledge of where the device resides in the neuraxial space and what risks different medical technologies pose to the patient and device. This understanding will lead to appropriate perioperative management which can reduce risk and improve patient outcomes. Kappa statistic to measure agreement beyond chance in free-response assessments

BackgroundThe usual kappa statistic requires that all observations be enumerated. However, in free-response assessments, only positive (or abnormal) findings are notified, but negative (or normal) findings are not. This situation occurs frequently in imaging or other diagnostic studies. We propose here a kappa statistic that is suitable for free-response assessments.MethodWe derived the equivalent of Cohen’s kappa statistic for two raters under the assumption that the number of possible findings for any given patient is very large, as well as a formula for sampling variance that is applicable to independent observations (for clustered observations, a bootstrap procedure is proposed). The proposed statistic was applied to a real-life dataset, and compared with the common practice of collapsing observations within a finite number of regions of interest.ResultsThe free-response kappa is computed from the total numbers of discordant (b and c) and concordant positive (d) observations made in all patients, as 2d/(b + c + 2d). In 84 full-body magnetic resonance imaging procedures in children that were evaluated by 2 independent raters, the free-response kappa statistic was 0.820. Aggregation of results within regions of interest resulted in overestimation of agreement beyond chance.ConclusionsThe free-response kappa provides an estimate of agreement beyond chance in situations where only positive findings are reported by raters.

Virtopsy in conjoined ischiopagus twins

To propose a multidisciplinary protocol for postmortem disclosure of complex fetal malformations, comparing ultrasound, computed tomography (CT), magnetic resonance imaging (MRI), and autopsy in a case of conjoined ischiopagus twins. A screening second-trimester ultrasound diagnosed ischiopagus twins at 20 gestational weeks in a 31-year-old woman without any previous ultrasound examination. The couple decided for pregnancy termination. The formalin-fixed fetuses underwent full-body CT, MRI, and autopsy. ultrasound accurately diagnosed ischiopagus twins. CT was very accurate in the description of bone components. MRI allowed better visualization of the visceral organs than CT. Only autopsy could disclose the aspect of the two gastrointestinal tracts and the external genitalia. Prenatal ultrasound represents the standard diagnostic exam for conjoined twins. CT-MRI virtual autopsy (virtopsy) may be an option if the couple refuses to authorize necropsy or may be useful to plan a minimally invasive autopsy preserving the external phenotype.

Stature estimation from the femur and tibia in Black South African sub-adults

Stature estimation can play a role in the positive identification of unknown individuals and as such it is routinely assessed during the examination of adult remains. Unfortunately, this is not a standard procedure when dealing with sub-adult remains due to the general lack of standard procedures for the estimation of sub-adult stature. The aim of this study was therefore to derive regression equations for the estimation of stature in black South African sub-adults. Fifty nine black South African sub-adult males and females, aged 10–17 years, voluntarily participated in the study by undergoing a full body Magnetic Resonance Imaging (MRI) scan. Living stature was measured with a stadiometer and the maximum and diaphyseal lengths of the femur and tibia were measured from the MRI scans using the image processing software OsiriX. Pearson’s correlation coefficients and linear least square regression analyses were used to assess the correlations between living stature and the measurements and to generate sub-adult stature estimation equations for males, females and a combined sex sample. Measurements of the femur, tibia and the combined measures thereof showed strong statistically significant positive correlations with living stature, while the obtained regression equations were characterized by low standard error of estimates. The strong correlations and low standard error of estimates are comparable to stature estimation models reported for Black South African adults and therefore these variables can be considered good estimators of sub-adult stature which will contribute valuable information to the biological profile of unidentified sub-adult skeletal remains.

Shape estimation of subcutaneous adipose tissue using an articulated statistical shape model

The quantification of adipose tissue can give insights into obesity and the associated diseases. The segmentation of this tissue is however difficult, particularly due to the complex geometry and the local appearance similarities between the subcutaneous and visceral types. Shape priors can be used to regularise the segmentation of geometries with small variation in shape. However, human bodies are articulated and substantially different across subjects. In this paper, a novel method is proposed for the segmentation of the subcutaneous fat layer in full-body magnetic resonance imaging scans. The proposed method is based on a statistical shape model of the whole body surface, which is learned from geometric scans. The body model is factorised into pose and shape deformations, which allows a compact parametrisation of large variations in human shape. The proposed method is applied in the segmentation of five magnetic resonance imaging data-sets. The experiments show that the proposed model can be used to effectively segment the subcutaneous fat geometry in subjects with different body mass indices. The incorporation of the statistical model in the algorithm regularises the segmentation, and establishes correspondences between the subcutaneous fat layer of the geometries across subjects. The registration of the fat layer with a common geometry could facilitate the statistical analysis of the shape distribution across the different geometries.

A multiscale and multiphysics model of strain development in a 1.5 T MRI magnet designed with 36 filament composite MgB2 superconducting wire

High temperature superconductors such as MgB2 focus on conduction cooling of electromagnets that eliminates the use of liquid helium. With the recent advances in the strain sustainability of MgB2, a full body 1.5 T conduction cooled magnetic resonance imaging (MRI) magnet shows promise. In this article, a 36 filament MgB2 superconducting wire is considered for a 1.5 T full-body MRI system and is analyzed in terms of strain development. In order to facilitate analysis, this composite wire is homogenized and the orthotropic wire material properties are employed to solve for strain development using a 2D-axisymmetric finite element analysis (FEA) model of the entire set of MRI magnet. The entire multiscale multiphysics analysis is considered from the wire to the magnet bundles addressing winding, cooling and electromagnetic excitation. The FEA solution is verified with proven analytical equations and acceptable agreement is reported. The results show a maximum mechanical strain development of 0.06% that is within the failure criteria of −0.6% to 0.4% (−0.3% to 0.2% for design) for the 36 filament MgB2 wire. Therefore, the study indicates the safe operation of the conduction cooled MgB2 based MRI magnet as far as strain development is concerned.
