Comparative Assessment of Clinical and Quality of Life Outcomes in Mandibular Angle Fractures Treated with Standard and Three-Dimensional Mini-Plates.

To undertake a multidimensional comparative assessment of the standard and three dimensional (3D) mini-plates in managing isolated mandibular angle fractures (MAFs). Thirty-six subjects were divided equally into two groups. Group A underwent fixation with a standard 2mm miniplate, while group B with 2mm 3D mini-plates. Evaluations were done preoperatively (T0), postoperatively at one week (T1), one month (T2) and three months (T3). Maximal inter-incisal mouth opening (MIO) and mean bite force (MBF) at the central incisors, right and left molars were calculated. Postoperative complications and Quality of life (QoL) outcomes were evaluated using the short form Oral Health Impact Profile (OHIP-14). Mean operative time was almost similar for both groups. Although mean MIO improved significantly from T1 to T3 in both groups, intergroup comparison of MIO was not significant. The MBF values were significantly higher in group B on the right and left molars at T2 and T3. Although significant improvement in OHIP-14 scores was seen from T2 to T3 in both the groups, intergroup comparison of OHIP was not significant. 3D plates demonstrated similar clinical and QoL outcomes compared to the standard mini-plates.

Impact of orthognathic surgery on quality of life and comparison of patients’ postoperative experience after single- and double-jaw surgery: a longitudinal study

To evaluate the impact of orthognathic surgery on quality of life (QoL) and to compare single- and double-jaw surgeries in terms of ratio and patient perceptions of the postoperative period. A prospective, longitudinal observational study was conducted. The short form Oral Health Impact Profile (OHIP-14) and the Orthognathic Quality of Life Questionnaire (OQLQ) were applied preoperatively and 6 months postoperatively to evaluate oral health-related QoL (OHRQoL). Additionally, patient perceptions of the immediate postoperative period were assessed at the first and fourth week after surgery. One hundred consecutive patients were recruited and assigned to the single-jaw group (n=24) or the double-jaw group (n=76) according to the characteristics of each facial or occlusal deformity. The questionnaires showed lower scores for both groups after surgery, indicating significant benefits to OHRQoL. The whole sample OHIP-14 mean total scores decreased from 10.5 to 2.8 (P < .001, d=1.35), whereas OQLQ showed a decrease from 48.4 to 11.6 (P < .001, d=1.75). Orthognathic surgery can improve OHRQoL, and long-term benefits outweigh the risks and discomfort associated with the treatment.

Influence of Orthognathic Surgery on Oral Health and Quality of Life

Orthognathic surgery is a common treatment modality for moderate to severe dentofacial deformities. This study aimed to determine the early postoperative changes in quality of life (QoL) after orthognathic surgery.Twenty patients were evaluated preoperatively (T0), postoperatively after 1 month (T1), and postoperatively after 3 months (T2). Short Form Oral Health Impact Profile and Orthognathic Quality of life Questionnaire were used as generic oral health-related and condition-specific QoL measures.Oral Health Impact Profile -14 and Orthognathic Quality of life Questionnaire scores significantly decreased from T0-T2. Physical pain, psychological disability, social disability, and handicap scores significantly decreased during this interval. Facial esthetic, function, awareness of facial deformity, and social aspects of deformity were also found to significantly improve. No significant difference was found between age groups; female patients had significantly greater preoperative Oral Health Impact Profil-14 scores than did male patients.Orthognathic surgery not only enhances the skeletal and facial relations, but also has significant positive effects on the patients' QoL, oral health, and psychological status.

Impact of surgical orthodontic treatment on quality of life in Chinese young adults with class III malocclusion: a longitudinal study

BackgroundThe quality of life in Class III malocclusion patients are worse than those without the disorder and previous studies have shown that surgical orthodontic treatment has a different effect on these patients compared with mild or moderate malocclusion. This study aimed to investigate the changes in quality of life in patients with Class III malocclusion during surgical orthodontic treatment in Chinese young adults.MethodsThe 14-item Short Form Oral Health Impact Profile (OHIP-14), and the 22-item Orthognathic Quality of Life Questionnaire (OQLQ) were used to assess the effect of surgical orthodontic treatment on the quality of life in 21 patients with Class III malocclusion at pre-treatment (T0), pre-surgical orthodontic treatment (6 to 8 months, T1) and post-surgical orthodontic treatment (6 to 8 months after surgery, T2), and 24 healthy individuals were included as controls. The comparisons in numerical variables between patients and controls were performed using Mann-Whitney U test. The scores of the two questionnaires between T0, T1, T2 and controls (Tc) were compared using generalized estimating equation.ResultsAccording to OHIP-14 questionnaire, the mean scores in T0 and T1 were higher than those in T2 and Tc (P < 0.001), and a significant decrease was observed after post-surgical orthodontic treatment (P < 0.001), which achieved a level similar to the control group (P > 0.05). As to OQLQ questionnaire, the mean scores of all domains showed a significant increase between T0 and T1 except for awareness of dentofacial aesthetics (P > 0.05) and social aspects of dentofacial deformity (P > 0.05), followed by a significant decrease between T1 and T2.ConclusionSurgical orthodontic treatment may improve quality of life in patients with Class III malocclusion, but pre-surgical orthodontic treatment may have an adverse effect on quality of life.

Assessing the Quality of Life in Patients With Dentofacial Deformities Before and After Orthognathic Surgery

This study aimed to assess the effect of health-related quality of life (QoL) among patients withdentofacial deformities who underwent orthognathic surgery compared with a control group without dentofacial deformities by use of generic oral health and condition-specific approaches. In this prospective study, 2 questionnaires were administered to 85 patients (31 male and 54 female patients) who were evaluated before undergoing orthognathic surgery. The Short Form Oral Health Impact Profile Questionnaire (OHIP-14) and the Orthognathic Quality ofLife Questionnaire (OQLQ) were administered before and 5 to 7months after orthognathic surgery. The control group comprised 96 young university student volunteers without dentofacial deformities. The questionnaires were collected 5 to 7months after surgery. The preoperative scores of the patients and the control group were contrasted separately. The respondents' postoperative OHIP-14 and OQLQ scores were significantly lower (P<.001 for total scores). The preoperative OQLQ scores for all domains were significantly higher among the patients than among the controls (P<.001 for total scores), whereas the total scores and 3 subscale scores of the OHIP-14 in the functional and psychological domains were significantly higher among the patients than among the controls (P<.05 for total scores). The preoperative and postoperative OQLQ total scores were remarkably different between male and female patients (P<.05). The postoperative OQLQ total scores were considerably higher in older patients than in younger patients (P<.05). All patients in the Class III group who underwent double-jaw surgery showed remarkable changes after surgery (P<.001 for total scores). Patients with dentofacial deformities had a poorer QoL compared with the healthy population, especially in functional and psychological aspects.Orthognathic surgery had a significant positive impact on QoL. Patients with Class III malocclusion who underwent double-jaw surgery seemingly benefitted the most after surgery.

Assessment of changes in quality of life in patients with Dentofacial deformities after orthognathic surgery—A study in Nepalese population

ObjectiveTo assess the changes in quality of life of Nepalese patients with Dentofacial deformities after orthognathic surgery using generic health related, short form oral health impact profile and condition specific quality of life questionnaires. MethodsFourteen patients who underwent orthognathic surgery were assessed pre-operatively (P0) and eight to twelve months post-operatively. Short form health survey (SF-36), short form oral health impact profile (OHIP-14) and orthognathic quality of life questionnaires (OQLQ) was used to assess generic health related quality of life, generic oral health related quality of life and condition specific quality of life respectively. ResultsThere was significant (P<0.05) improvement in role limitation due to physical health, role limitation due to emotional well being, energy/fatigue and social function after surgery. There was significant decrease (P<0.05) in five out of seven domain score of OHIP-14 as well as overall OHIP-14 score. Similarly all domain scores of OQLQ also showed a significant (P<0.05) decrease in their score suggesting improvement of quality of life post orthognathic surgery. ConclusionSignificant improvement in QOL occurs after orthognathic surgery when carried out in patients with dentofacial deformities.

Quality of life in patients undergoing orthognathic surgery - A two-centered Swedish study.

Surgical corrections of dentofacial deformities have both physical and psychological impact on quality of life (QoL). The objectives of the present study were to evaluate the impact of oral health related problems on QoL before and after a combination of orthodontic treatment and orthognathic surgery. Additionally, the study aimed to identify correlations between different dentofacial patterns and possible improvements due to treatment. In a prospective study, we evaluated fifty patients before start of treatment, 6 weeks and 6 months postoperatively. The questionnaires used were: OHIP-14 (Short Form Oral Health Impact Profile), a condition-specific QOL approach (Orthognathic Quality of Life Questionnaires; OQLQ) and a social-demographic questionnaire. There was a statistically significant improvement in the OHIP domains from baseline to 6 months follow-up and for the OQLQ, the improvement was significant both at 6 weeks and 6 months in relation to the baseline data. Significant improvement of quality of life over time is proved by both OHIP-14 and OQLQ in the present study. Socio-demographic and holistic considerations are important when evaluating treatment outcome after combined orthodontic and orthognatic surgery. However, longer follow-up would be beneficial.

Oral Health Related Quality of Life Among Young Adults

To assess the oral health-related quality of life (OHRQoL) of young adults in Hong Kong and its relationships with oral health status. A survey was conducted in a representative sample of Hong Kong adults aged 18 years. OHRQoL was measured by the short form Oral Health Impact Profile (OHIP-14, Chinese version). Clinical examinations were taken to assess the oral health status using DMFT index and Community Periodontal Index (CPI) according to WHO criteria. Information on socioeconomic status, self-perceived oral health status and treatment need was also collected. Totally 324 young adults (male: 177 and female: 147) were examined. The mean and median OHIP-14 score were 6.3 (SD = 5.8, range = 0–26) and 5, respectively. Half of the subjects (50.6 %) reported some negative impacts. Good internal consistency (Cronbach’s alpha = 0.87) and construct validity in relation to perceived oral health status, perceived dental treatment need and perceived oral health impact on daily life were established. Neither socio-demographic background nor oral health status showed a significant association with OHIP-14 score (p > 0.05). The findings indicated that the impact of oral health on quality of life appeared to be low in the young Hong Kong adults. Chinese version of OHIP-14 had good reliability and validity in measuring OHRQoL of young Chinese adults. Neither socio-demographic background nor oral health status had an influence on their OHRQoL in this low impact population.

Oral health-related quality of life and periodontal and dental health status in Iranian hemodialysis patients.

The aim of this study was to evaluate oral health status and oral health related quality of life (OHRQoL) in Iranian patients undergoing hemodialysis. In this cross-sectional study 145 (95 dentate and 50 edentulous) patients undergoing hemodialysis participated. Demographic information, laboratory findings and dental health status was recorded by a standard form. Oral hygiene status was obtained by simplifed oral hygiene index (OHI-S) and oral health was evaluated by decay missing flling teeth (DMFT) index, plaque index (PLI) and periodontal disease index. Oral health related quality of life was determined by means of short form oral health impact profle (OHIP-14) and general oral health assessment index (GOHAI). The mean (± SD) DMFT, PLI and PDI were 15.47 ± 7.85, 2.03 ± 0.95, 4.09 ± 1.31 respectively. OHI-S was good in 7 (7.4%), fair 25 (26.6%) and poor in 38 (26.6%) of patients. The mean OHIP-14 and GOHAI scores were 31.32 ± 12.53 and 29.07 ± 8.5 respectively. The self-perceived oral health of 58 (40%) was good, 49(33.8%) was fair and 38 (26.2%) was poor. Patients with higher scores for the questionnaires had significantly worst self-rated oral health. Besides, there was a significant positive correlation between dental and periodontal variables with OHIP-14 and GOHAI scores. Hemodialysis patients had a poor oral hygiene and periodontal status, weak attitudes and negligence toward oral health but they were satisfed of their oral health condition and their OHRQoL was approximately good. Therefor it should be communications between nephrologists and oral health care professionals for promoting the oral health status of the patients. The findings high light the need of comprehensive oral examinations including periodontal therapy, restorative treatment, preventive dental treatment and follow-up care in the hemodialysis patients.

Trigeminal Neurosensory Deficit and Patient Reported Outcome Measures: The Effect on Quality of Life

ObjectivesTo investigate the effect of persistent neurosensory disturbance of the lingual nerve (LN) or inferior alveolar nerve (IAN) on general health and oral health- related quality of life (QoL).MethodsThe study design was a case-control study. Patients with persistent neurosensory deficit of LN or IAN after lower third molar surgery (for 12 months or more) were the study group. The control group was an age and gender matched sample of patients who had dental extractions or lower third molar surgeries without trigeminal neurosensory deficit. The outcome variables were the general health and oral health-related QoL. General health-related QoL was assessed using the 36-item Short Form Health Survey (SF-36) and oral health-related QoL using the 14-item Short Form Oral Health Impact Profile (OHIP-14). Differences in SF-36 scores and OHIP-14 scores between the groups were compared.ResultsForty-eight subjects (24 cases and 24 controls) were recruited. When compared to the control group, patients with neurosensory deficits had poorer Mental-Health Component Scores (MCS) (p = 0.005), General Health (p = 0.023), Vitality (p = 0.048), Social Functioning (p = 0.003), Role-emotion (p = 0.008) and Mental Health (p = 0.022). The OHIP-14 scores were also significantly worse in this patients with neurosensory deficits compared with the control group (p = 0.002). When compared within the study group, older patient with neurosensory deficit was found to correlate with worse Physical Health Component Scores (PCS) (p = 0.02) and OHIP-14 scores (p = 0.02), while more severe visualized analog scaling rating of numbness was correlated with a worse PCS (p = 0.034).ConclusionsPatients with persistent LN or IAN deficit after lower third molar surgery have poorer health-related QoL and poorer oral health-related QoL than those without such deficits.

Quality of life and psychological well‐being among endodontic patients: a case‐control study

Much is already known about the consequences of endodontic disease from clinicians' perspectives; a significant omission is an understanding from patients' perspectives. This study aimed to determine oral health-related quality of life (OHQoL) and psychological distress among subjects referred for endodontic care compared to patients in periodontal maintenance. This was a case-control study involving 200 patients; 100 patients requiring endodontic treatment and 100 control subjects (periodontal maintenance patients). OHQoL was assessed using the short form Oral Health Impact Profile measure (OHIP-14) and psychological well-being using the short form of the General Health Questionnaire (GHQ-12). Variations in OHIP-14 and GHQ-12 scores between the 'case' and 'control' group were determined, and the magnitude of such differences through effect size (ES) calculations. There were significant differences in OHIP-14 summary scores between the case and control groups (p < 0.001) and significant differences across all seven domain scores (p < 0.05). The ES was moderate (0.63) with respect to summary OHIP-14 scores. There were also significant differences in GHQ-12 scores between the case and control groups (p < 0.05), but the ES was small (0.36). OHQoL and psychological well-being is compromised among patients seeking endodontic treatment, and to a greater magnitude than patients in periodontal maintenance.

Translation and Cross-Cultural Adaptation of the Manchester Orofacial Pain Disability Scale

ABSTRACTObjectivesThe purpose of the present study was to translate and perform a cross-cultural adaptation of Manchester Orofacial Pain Disability Scale to the Portuguese language.Material and MethodsA synthesis of two independent translations done by bilingual translators whose mother tongue was the Portuguese language began the process of translation. From the synthesis of the translated version and totally blind to the original version, two different non-native English language teachers without dental knowledge translated the questionnaire back to English. The pre-final version was done by an Expert committee: the researchers, two other non-native English language teachers and one native English language speaker. The new questionnaire was then piloted among 8 patients from the target setting that were interviewed to probe it on their perceived meaning of each question. The Manchester Orofacial Pain Disability Scale (MOPDS) thus translated was called Brasil-MOPDS and was validated in 50 patients with Orofacial pain from TMJ and Occlusion clinic ambulatory of São Paulo University School of Dentistry. The Brasil-MOPDS was administered twice by an interviewer (15 - 20 day interval) and once by a second independent interviewer. The Brazilian version of the short form oral health impact profile (OHIP-14) questionnaire and the visual analogue pain scale (VAS) were applied on the same day.ResultsInternal consistency (Cronbach's α = 0.9), inter-observer (ICC = 0.92) and intra-observer (ICC = 0.98) correlations presented high scores. Validity of Brasil-MOPDS compared to OHIP-14 (r = 0.85) and VAS (r = 0.75) shown high correlations.ConclusionsBrasil-MOPDS was successfully translated and adapted to be applied to Brazilian patients, with satisfactory internal and external reliability.

Change in quality of life after combined orthodontic-surgical treatment of dentofacial deformities

To determine changes in quality of life (QoL) after orthodontic-surgical treatment in patients with dentofacial deformities, using generic health, generic oral health, and condition-specific QoL approaches. Patients were evaluated at baseline (T(0)), postoperative 6 weeks (T(1)), postoperative 6 months (T(2)), and after orthodontic treatment (T(3)). Generic health-related QoL, generic oral health-related QoL, and condition-specific QoL were assessed using the 36-item Short Form Health Survey (SF-36), the 14-item Short Form Oral Health Impact Profile (OHIP-14), and the 22-item Orthognathic Quality of Life Questionnaire (OQLQ), respectively. There were significant changes in Physical-Health Component Scores (PCS) (P < .001) and Mental-Health Component Scores (MCS) (P < .01) of SF-36, in OHIP-14 scores (P < .001), and in OQLQ scores (P < .001) during the trajectory of treatment. A transient decrease in PCS was found at T(1) (P < .001) and an increase in MCS was found only at T(3) (P < .05). There was a significant decrease in OHIP-14 scores at T(2) and T(3) (P < .001) and in OQLQ scores at all time points compared with T(0) (P < .05). Significant changes in QoL occurred after orthodontic-surgical treatment. A comprehensive assessment of QoL using generic health, generic oral health, and condition-specific approaches proved to be useful in determining the trajectory of changes.

Generic health-related QOL was assessed using the 36-item Short Form Health Survey (SF-36), generic oral health-related QOL was assessed by the 14-item Short Form Oral Health Impact Profile (OHIP-14), and condition-specific QOL was assessed by the 22-item Orthognathic Quality of Life Questionnaire (OQLQ). There was a significant reduction (deterioration) in SF-36 summary physical (P < .01) and mental health scores (P < .001) at 6 weeks after surgery but no significant change in overall OHIP-14 or OQLQ scores. At 6 months after surgery, SF-36 summary scores returned to baseline levels and significant reduction (improvements) in OHIP-14 (P < .001) and OQLQ mean scores (P < .001) were observed. Significant changes in QOL occurred following orthognathic surgery. A marked but transient deterioration in many aspects related to general well being was noted in the early postoperative period and significant improvement was documented by 6 months. A comprehensive assessment of QOL using generic health, generic oral health, and condition-specific approaches proved useful in determining such changes.

Quality of life in patients with dentofacial deformity: a comparison of measurement approaches

The aim of this study was to examine the effect of dentofacial deformity on quality of life (QOL) as assessed by generic health, generic oral health and condition-specific approaches. A case-control study was performed involving 154 subjects: 76 with dentofacial deformity (non-cleft) and 76 without dentofacial deformity. Generic health-related QOL was assessed employing the 36-item Short Form Health Survey (SF-36), generic oral health-related QOL was assessed by the 14-item short form Oral Health Impact Profile (OHIP-14) and condition-specific QOL was assessed by the 22-item Orthognathic Quality of Life Questionnaire (OQLQ). No significant difference in SF-36 scores between case and control groups was observed ( P > 0.05). There were significant differences in overall OHIP-14 scores ( P < 0.001) and overall OQLQ scores ( P < 0.001) between case and control groups. The results suggest that dentofacial deformity affects individuals in many aspects of their lives, and that generic oral health and condition-specific approaches to assessing QOL are able to discriminate patients with dentofacial deformities from those without, and thus have value in determining the impact of dentofacial deformities on QOL.

A preliminary investigation into tooth care, dental attendance and oral health related quality of life in adult stroke survivors in Tayside, Scotland

The aim of this study was to investigate patterns of oral care, dental attendance and oral health-related quality of life among adults who had suffered a stroke. Stroke is the most common cause of adult disability in the UK. Seventy per cent of strokes occur in adults over 65 years of age. A mild stroke may leave little residual disability but in cases of moderate or severe stroke the disability may be significant and may impact on oral health and function. A cross-sectional survey was conducted among adults surviving 1 year after stroke, between January and July 2001. A medical screening was carried out which included an assessment of disability and handicap using the modified Rankin scale. A structured interview was conducted to identify normal patterns of oral care and dental attendance and to elicit if since suffering a stroke any changes had occurred or were likely to occur. The Short Form Oral Health Impact Profile (OHIP-14) was administered prior to an oral examination. Analysis used SPSS 11.0 for Windows. Parametric and nonparametric tests were undertaken (t-tests and chi-squared tests with Yates correction where appropriate). Forty-one adults were recruited into the study comprising 21 female and 20 male. They ranged in age from 50 to 87 years and the mean age was 69 years (SD = 9.8). Forty per cent of participants experienced moderate disability or greater following their stroke. Thirty-seven per cent had difficulty with tooth cleaning. The most frequently reported problem was being unable to use one hand properly as a result of the stroke. There was a significant association between the degree of disability following stroke and difficulty with tooth cleaning (P = 0.015). Disability as a result of the stroke was cited as the main reason for reported or projected attendance pattern change. The most frequently experienced OHIP-14 dimension was functional limitation (39%). Individuals who have been left disabled after a stroke may require help with or advice on oral care and information on how to access dental services in a setting appropriate to their disability. Further research is needed to identify the dental needs of adults with stroke and to identify appropriate interventions to meet these needs.

Job characteristics and the subjective oral health of Australian workers

Abstract: The way in which work is structured and organised is associated with the health and well‐being of workers.Objectives: To examine the associations between hours worked, job security, skill maintenance and work and home interference and subjective oral health; and to compare findings for different occupational groups.Methods: Data were collected in 1999 from a random stratified sample of households in all Australian States and Territories using a telephone interview and a questionnaire survey. Subjective oral health was evaluated with the short form Oral Health Impact Profile (OHIP‐14), which assesses the adverse impact of oral conditions on quality of life.Results: Data were obtained for 2,347 dentate adults in the workforce. In the 12 months preceding the survey, 51.9% had experienced oral pain and 31.0% reported psychological discomfort from dental problems. Males, young adults, Australian‐born workers, and those in upper‐white collar occupations reported lower mean OHIP‐14 scores (ANOVA p<0.001). Having controlled for the effects sex, age, country of birth and socio‐economic factors in a linear multiple regression analysis, hours worked, skill maintenance and work and home interference were significantly associated with OHIP‐14 scores for all workers. While part‐time work was associated with higher OHIP‐14 among upper white‐collar workers, working >40 hours a week was associated with higher OHIP‐14 scores for other workers.Conclusions: Aspects of the work environment are associated with the subjective oral health of workers. Because these contexts are subject to only limited control by individual workers, their influence is a public health issue.
