Combined use of stannous fluoride-containing mouth rinse and toothpaste prevents enamel erosion in vitro

ObjectiveTo compare the protective effect of commercial stannous-containing mouth rinses on enamel erosion in a simulated 5-day in vitro cycling model.Materials and methods81 human enamel specimens were embedded in resin blocks and divided into nine groups as follows; group 1: stannous fluoride (1000SnF2) toothpaste; groups 2,3, and 4 were the same as group 1 plus Elmex®, PerioMed™, and Meridol®, respectively, group 5: stannous fluoride (1450SnF2) toothpaste, groups 6, 7, and 8 were the same as group 5 plus Elmex®, PerioMed™, and Meridol®, respectively, group 9: negative control. An erosive challenge was induced with a 1 min hydrochloric acid (0.01 M, pH 2.2) treatment 3 times per day. Each cycle included immersing in the toothpaste slurry twice for two minutes and a one-minute rinse. The enamel slabs were immersed in artificial saliva between each erosive cycle and incubated overnight at 37 °C. Surface hardness loss and enamel loss were determined by Knoop surface hardness and non-contact profilometry, respectively. Finally, enamel surfaces were analyzed by scanning electron microscopy and X-ray energy dispersive spectroscopy (SEM/EDS).ResultsAll three mouth rinses had similar protective effects against erosion when using adjunct with 1000 SnF2 toothpaste (p > 0.05). With 1450 SnF2 toothpaste, Elmex® presented significantly lower surface hardness loss than Meridol® (p < 0.05). The combined use of Elmex® or PerioMed™ with toothpaste provided significantly better erosion protection than toothpaste alone, either 1000 or 1450 SnF2. In addition, 1000SnF2 toothpaste adjunct with mouth rinse is comparable to 1450 SnF2 toothpaste alone in preventing enamel erosion.ConclusionAll three mouth rinses reduced enamel erosion. The additional use of a high concentration stannous containing mouth rinse with 1450 SnF2 toothpaste increases the protective effect against enamel erosion in vitro.Clinical significanceTo date, no standard protocol for preventing dental erosion is available. There are three stannous-containing mouth rinses on the market; however, no study compared their efficacy or indicated whether using adjuncts with anti-erosion toothpaste provides additional benefits. This study found that adding stannous mouth rinse to twice-daily toothpaste increases erosion protection.

Evaluation of antibacterial properties of fluoride-containing mouth rinses differing in their acidic compound using a Streptococcus mutans biofilm.

This in vitro study assessed the antibacterial effect on Streptococcus mutans biofilms of mouth rinses with 700 ppm F- (derived from NaF) that differed only in their acid compounds (malic (A), citric (B), tartaric (C), fumaric (D), hydrochloric (E), phosphoric (F), and lactic (G) acid) used to adjust pH. S. mutans (ATCC 25175) was grown for 22 h at 37°C, harvested, resuspended in simulated body fluid and biofilm formation followed for 24 h at 37°C. Thereafter, biofilms were treated with experimental rinses for 30 s, and placed in TAM48 isothermal microcalorimeter at 37°C for 72 h. Applying Gompertz growth model parameters lag time and growth rate were determined from heatflow curves; additionally, reduction of active biofilms was calculated. Moreover, samples were live/dead stained and analyzed by confocal scanning microscopy. All mouth rinses were showing statistically significant lag time and reduction of active biofilm (p<0.05, A 19.1+/-2.3h and 58.5+/-7.7%, B 15.5+/-1.1h and 41.9+/-5.3%, C 17.6+/-1.9h and 53.1+/-7.5%, D 18.4+/-2.4h and 55.8+/-8.8%, E 20.2+/-3.3h and 61.5+/-10.0%, F 20.2+/-3.0h and 61.6+/-9.3%, and G 18.3+/-2.5h and 55.3+/-8.9%). Interestingly, there were no differences found between the treated groups (p>0.05, A 0.064+/-0.004 1/h, B 0.063+/-0.005 1/h, C 0.065+/-0.004 1/h, D 0.067+/-0.004 1/h, E 0.066+/-0.006 1/h, F 0.067+/-0.004 1/h, G 0.066+/-0.006 1/h) for the maximum growth rate. Vitality staining supported these findings.. The present investigation demonstrates that the type of acid compounds used to produce the rinses did not show any negative effect on the antimicrobial properties of the tested products as all of them exhibited a similar efficacy against S. mutans biofilms.

Fluoride bioavailability on demineralized enamel by commercial mouth rinses.

The ability of mouth rinses, available in the international market, to form reaction products on demineralized enamel (bioavailability test) was evaluated in vitro. Nine mouth rinses purchased in Chile were evaluated; eight formulated with NaF (one containing 100 µg F/mL and seven containing 226) and one with Na2FPO3 (226 µg F/mL as ion F). Demineralized enamel slabs (n=15 per mouth rinse) were sectioned; one half was subjected to the assigned mouth rinse treatment for 10 min and the other half was used to obtain baseline data. Loosely bound and firmly bound fluoride formed on enamel were determined with an ion-specific electrode and the values were expressed in µg F/cm2. The concentration of fluoride and the pH of the mouth rinses were previously determined. Concentrations of loosely bound and firmly bound fluoride formed on enamel were independently analyzed by ANOVA and Tukey's test (α=5%). The loosely bound and firmly bound fluoride concentrations (µg F/cm2) formed ranged from 3.2 to 36.2 and 0.4 to 1.7, respectively. Loosely bound fluoride formed on enamel was significantly more effective in discriminating the effect of different commercial mouth rinses than firmly bound fluoride. Mouth rinses with 226 ppm F as NaF and low pH presented significantly greater bioavailability of fluoride on enamel than those with higher pH or lower NaF concentration. The mouth rinse with Na2FPO3 showed low reactivity. Although further studies are necessary, the findings showed that commercial fluoride-containing mouth rinses have important variations in enamel fluoride bioavailability, which may result in differences on anticaries efficacy.

White Spot Lesions: Fixed Orthodontic Treatment Scars - A Review

White spot lesions (WSLs) are one of the common iatrogenic effects seen in patients undergoing orthodontic treatment with fixed appliances. Fixed orthodontic appliances act as food traps leading to prolonged plaque accumulation. The acidic by products of the bacteria in plaque are responsible for the subsequent enamel demineralization and formation of white spot lesions. These orthodontic scars lead to poor esthetics, patient dissatisfaction and legal complications. This concern raises the need for assessing the saliva, oral hygiene status and caries rate before beginning of treatment and initiating preventive measures. Orthodontists must take up the active responsibility to educate patients about the importance of maintaining good dietary compliance and excellent oral hygiene regime. A review of literature suggests that topical fluoride agents such as fluoridated toothpaste, fluoride-containing mouth rinse, gel and varnish prevent or minimize the formation of WSLs during orthodontic treatment.

Development of resistance of mutans streptococci and Porphyromonas gingivalis to chlorhexidine digluconate and amine fluoride/stannous fluoride-containing mouthrinses, in vitro.

The aim if this study was to determine the minimal inhibitory concentrations of chlorhexidine digluconate and an amine fluoride/stannous fluoride-containing mouthrinse against Porphyromonas gingivalis and mutans streptococci during an experimental long-term subinhibitory exposition. Five P. gingivalis strains and four mutans streptococci were subcultivated for 20-30 passages in subinhibitory concentrations of chlorhexidine digluconate or an amine fluoride/stannous fluoride-containing mouthrinse. Pre-passaging minimal inhibitory concentrations for chlorhexidine ranged from 0.5 to 2 mg/l for mutans streptococci and from 2 to 4 mg/l for the P. gingivalis isolates. For the amine fluoride/stannous fluoride-containing mouthrinse minimal inhibitory values from 0.125 to 0.25% for the mutans streptococci and from 0.063 to 0.125% for the P. gingivalis isolates were determined. Two- to fourfold increased minimal inhibitory concentrations against chlorhexidine were detected for two of the five P. gingivalis isolates, whereas no increase in minimal inhibitory concentrations was found for the mutans streptococci after repeated passaging through subinhibitory concentrations. Repeated exposure to subinhibitory concentrations of the amine fluoride/stannous fluoride-containing mouthrinse did not alter the minimally inhibitory concentrations of the bacterial isolates tested. Chlorhexidine and the amine fluoride/stannous fluoride-containing mouthrinse are effective inhibitory agents against the oral bacterial isolates tested. No general development of resistance against chlorhexidine or the amine fluoride/stannous fluoride-containing mouthrinse was detected. However, some strains showed potential to develop resistance against chlorhexidine after prolonged exposure. The use of chlorhexidine should be limited to short periods of time. The amine fluoride/stannous fluoride-containing mouthrinse appears to have the potential to be used on a long-term basis.

Fluoride toothpastes and fluoride mouthrinses

To provide a brief commentary review of fluoride-containing toothpastes and mouthrinses with emphasis on their use at home. Toothpastes and mouthrinses are just two of many ways of providing fluoride for the prevention of dental caries. The first investigations into incorporating fluoride into toothpastes and mouthrinses were reported in the middle 1940s. Unlike water fluoridation (which is 'automatic fluoridation'), fluoride-containing toothpastes and fluoridecontaining mouthrinses are, primarily, for home use and need to be purchased by the individual. By the 1960s, research indicated that fluoride could be successfully incorporated into toothpastes and clinical trials demonstrated their effectiveness. By the end of the 1970s, almost all toothpastes contained fluoride. The widespread use of fluoride- containing toothpastes is thought to be the main reason for much improved oral health in many countries. Of the many fluoride compounds investigated, sodium fluoride, with a compatible abrasive, is the most popular, although amine fluorides are used widely in Europe. The situation is similar for mouthrinses. Concentrations of fluoride (F), commonly found, are 1500 ppm (1500 μg F/g) for toothpastes and 225 ppm (225 μg F/ml) for mouthrinse. Several systematic reviews have concluded that fluoride-containing toothpastes and mouthrinses are effective, and that there is added benefit from their use with other fluoride delivery methods such as water fluoridation. Guidelines for the appropriate use of fluoride toothpastes and mouthrinses are available in many countries. Fluoride toothpastes and mouthrinses have been developed and extensive testing has demonstrated that they are effective and their use should be encouraged.

In vitro antimicrobial effects of two antihalitosis mouth rinses on oral pathogens and human tongue microbiota

The aim of the study was to compare the antimicrobial activity of a mouth rinse containing chlorhexidine and cetylpyridinium chloride (MR1) with a stannous fluoride-based mouth rinse (MR2) in vitro. Samples of the tongues from 10 subjects with and 10 subjects without halitosis were inoculated on blood agar plates. The agar was perforated, and the cylindrical holes were filled either with mouth rinse MR1 or with mouth rinse MR2. After incubation, inhibition zones of the whole tongue microbiota and Fusobacterium nucleatum were measured. In addition, MR1 and MR2 were applied in a short interval killing test (SIKT) on four oral pathogens Porphyromonas gingivalis, Prevotella intermedia, F. nucleatum and Aggregatibacter actinomycetemcomitans. Total viable cell counts were made after two minutes of incubation with increasing concentrations of MR1 and MR2. MR1 showed a significantly higher in vitro antimicrobial activity against the whole tongue microbiota and F. nucleatum than MR2 in both groups of subjects. In the SIK test, MR1 showed a significantly greater killing capacity than MR2. The results show that a mouth rinse with low concentrations of chlorhexidine and 0.05% cetylpyridinium chloride appears to be more effective in inhibiting growth of the human tongue microbiota in vitro than a fluoride/stannous fluoride-containing mouth rinse. This in vitro observation supports the use of chlorhexidine and cetylpyridinium chloride in the treatment of oral halitosis.

The pH Levels of Different Methamphetamine Drug Samples on the Street Market in Cape Town.

The purpose of this study was to determine the pH levels of 29 different samples of methamphetamine on the street market in Cape Town. The sample was dissolved in water and the pH of each sample determined. The pH levels varied from 3.02 to 7.03 with an average of 5.0. Seventy-two percent (21) of the samples had a pH level below the saliva “critical pH point of 5.6” and therefore should cause significant damage to enamel, especially in hyposalivation subjects without a saliva flow. However, about 26% of the samples had a pH level close to the neutral point and should cause minor damage to enamel. To lessen enamel damage, subjects should exercise good oral hygiene practice, rinse with a fluoride-containing mouth rinse, drink artificially sweetened drinks, and eat cheese. It is concluded that most of the methamphetamine samples have a low enough pH to cause direct damage to enamel especially in hyposalivation subjects.

Effects of different fluoride recharging protocols on fluoride ion release from various orthodontic adhesives

ObjectiveThe purpose of this study was to find the most effective fluoride recharging protocol for orthodontic adhesives. MethodsFive orthodontic adhesives were used: a non-fluoride-releasing composite, a fluoride-releasing composite, a polyacid-modified composite (compomer), and two resin-modified glass-ionomer cements (RMGICs). Each specimen was placed into deionized water (DW) and the initial fluoride ion release was measured for 2 months. Each specimen was then subjected to four different treatments to simulate a fluoride recharge: 1000ppm NaF solution, acidulated phosphate fluoride gel (APF), fluoride-containing dentifrice and DW (control). After topical fluoride treatment, each specimen was submitted to fluoride re-release tests. ResultsFluoride-containing adhesives initially showed higher rates of fluoride ion release, but significantly declined to lower levels. The overall cumulative fluoride ion release during the initial period was RMGICs>compomer>fluoride-containing composite>non-fluoride-releasing composite. After topical fluoride treatment, the amount of fluoride ion re-released was proportional to the amount of fluoride ion previously released from the adhesives. However, the amount of fluoride ions released only lasted for 2 days and then returned to the levels before fluoride application. The overall cumulative fluoride ion re-release according to the fluoride treatments was APF and NaF solution>dentifrice. ConclusionThis study suggests that using the combination of RMGICs and a fluoride-containing mouth rinse solution is the most effective protocol for long-term fluoride re-release from orthodontic adhesives, given the difficulty of routine use of APF at home, although all topical fluoride treatments can recharge fluoride ion in adhesives.

Efficacy of a Tin/Fluoride Rinse

Concentrated tin- and fluoride-containing mouthrinses are effective erosion inhibitors in enamel and dentin. To test whether this is also true for solutions with lower concentrations, we conducted a randomized double-blind three-cell crossover in situ study with extra-orally performed erosive impacts (citric acid, 6 x 5 min/day) and an intra-oral rinsing protocol (1 x 30 sec/day) in 24 volunteers. The mouthrinses were a placebo, a NaF (500 ppm F-), and an amine fluoride (AmF)/NaF/SnCl2 mouthrinse (500 ppm F-, 800 ppm Sn2+). Compared with the placebo, the NaF mouthrinse reduced substance loss by 19% in enamel and 23% in dentin (p ≤ 0.01 each); the AmF/NaF/SnCl2 mouthrinse reduced this parameter by 67% in enamel and 47% in dentin (p ≤ 0.001 each). AmF/NaF/SnCl2 was significantly more effective than NaF in both tissues (p ≤ 0.01). The mouthrinse containing Sn and F exhibited good efficacy, even under severe erosive conditions.

Tin and fluoride as anti-erosive agents in enamel and dentine in vitro

Objective. To investigate the efficacy of an experimental tin- and fluoride-containing mouth rinse on progression of erosion in enamel and dentine in vitro. Material and methods. Human enamel and dentine specimens were subjected to a cyclic demineralization and remineralization procedure for 10 days, with six 5-min demineralization periods per day. Erosive demineralization was performed with 0.05 M citric acid (pH 2.3). Except in the negative control group, the specimens were treated for 2 min with mouth rinses after the first and sixth demineralizations. An experimental tin-containing fluoride mouth rinse [125 mg/kg F– (amine fluoride), 375 mg/kg F– (NaF), 800 mg/kg Sn2+ (SnCl2)] and an experimental sodium fluoride mouth rinse (500 mg/kg F–) were used (both pH 4.5). A commercially available, tin-containing mouth rinse served as a positive control (pH 4.2, 409 mg/kg Sn2+, 250 mg/kg F–). Tissue loss was determined profilometrically. Results. The highest tissue loss was found in the negative control group, in both enamel and dentine. In enamel, the NaF solution showed almost no effect. Both tin-containing solutions significantly reduced tissue loss (positive control: 65%; 800 mg/kg Sn2+: 78%; both p ≤ 0.001 compared to negative control). In dentine all mouth rinses significantly reduced tissue loss (positive control: 43%; 800 mg/kg Sn2+: 53%; NaF: 40%; all p ≤ 0.001 compared to negative control). Conclusions. In enamel, the efficacy of mouth rinses depended on the compound used; tin-containing preparations were notably effective. In dentine, however, reduction of substance loss was nearly the same in all treatment groups.

A group of Midwestern university students needs to improve their oral hygiene and sugar/pop consumption habits

Poor oral hygiene and sugar/pop consumption practices are detrimental to one's overall health. College women were hypothesized to have better oral hygiene habits and to consume less sugar/pop than men and that the students' habits would be different from those the students had before college. These habits of students at a Midwestern university were evaluated by sex. The volunteers included 105 men and 91 women. Three quarters of the students reported brushing their teeth at least the recommended twice daily, with women brushing their teeth more often. About a third of the students flossed at least the recommended once daily. Not quite a third of the students reported brushing and flossing their teeth more often than they did before college. More than a third reported using mouth rinses 4 or more times weekly, with 13% reporting using a fluoride-containing mouth rinse. More than 60% reported using fluoride-containing toothpaste. Slightly more than a third reported drinking fluoridated water in their younger years. A larger percentage of women than men reported that diet pop was their pop of choice. More than two thirds of the students that drank pop indicated that regular pop was their favorite. Most of the students reported consuming sugary foods more than once daily, but they indicated that most of these sugars were not sticky. Few differences were observed in oral hygiene and sugar/pop consumption habits of these college students by sex. Nutritionists and other health professionals should work cooperatively in helping individuals improve their oral hygiene and sugar/pop consumption habits.

In vitro efficacy of experimental tin- and fluoride-containing mouth rinses as anti-erosive agents in enamel

Objectives The aim of this in vitro study was to investigate the efficacy of various experimental tin- and fluoride-containing mouth rinses with stepwise reduced concentrations of the active agents on erosion progression in enamel. Methods Human enamel specimens were subjected to a cyclic demineralisation and remineralisation procedure for 10 days with 6 demineralisation periods per day, 5 min each. Erosive demineralisation was performed with 0.05 M citric acid (pH 2.3). Except in the control groups, the specimens were treated for 2 min with experimental mouth rinses after the first and sixth demineralisations. The tin concentrations ranged between 800 and 2800 ppm, and fluoride concentrations of 500 and 250 ppm were used. All preparations were adjusted to pH 4.5. As positive control, a commercially available, tin-containing mouth rinse was used (pH 4.2, 409 ppm Sn 2+, 250 ppm F −). Tissue loss was determined profilometrically. Results and conclusion As expected, the highest tissue loss was found in the negative control group. All experimental mouth rinses were able to reduce tissue loss significantly ( p ≤ 0.001). The best reduction was achieved by the 2800 ppm Sn 2+, 500 ppm F − solution (80%). The lowest reduction was achieved by the 800 ppm Sn 2+, 250 ppm F − solution (54%). Amongst the 500 ppm F − solutions, in the Sn 2+ concentration range of 2800–800 ppm, only small differences in efficacy were observed, meaning that the tin concentration can probably be reduced without losing efficacy. This factor is particularly important if one regards the possible clinical applicability of such mouth rinses.

Influence of 0.05% sodium fluoride solutions on microhardness of resin-modified glass ionomer cements

This study aimed to evaluate the influence of fluoride-containing mouthrinse solutions (Fluorgard and Oral B) on the superficial microhardness of two resin-modified glass ionomer cements (Vitremer and Fuji II LC). Fifteen discs-shaped specimens of each glass ionomer cement (Ø10 mm; 2 mm thick) were prepared, thereby forming two groups. After 24-hour storage in artificial saliva, the microhardness was measure and the data were recorded. Next, each group was divided into three subgroups (n = 5), according to the solution to be immersed in. Control specimens were kept in artificial saliva along the whole experiment. The test specimens were kept in mouthrinse solution for 30 days. Vickers surface microhardness was analyzed at predetermined evaluation periods: 24 h, 48 h, 7, 14, 21 and 30 days after specimens' preparation. Data were subjected to three-way ANOVA and to Tukey test (p<0.05). A better behavior of Fuji II LC was observed and Fluorgard affected most the characteristics of the tested materials. It may be concluded that fluoride-containing solutions influenced the tested characteristics of materials, mainly of Vitremer.

Fluoride-containing mouthrinses in three European countries

The caries-preventive effect of fluoride is well known. The widespread use of fluoride-containing toothpastes and mouthrinses has been recommended to reduce dental caries. Many kinds of fluoride containing toothpastes are available in Japan. However, only two kinds of mouthrinses which contain fluoride are comically available. The aim of this study was to investigate the current status of fluoride-containing mouthrinses on the market in three European countries. Fluoride-containing mouthrinses were purchased from retail outlets in Athens, Amsterdam and Geneva. The labels of ingredients were studied to determine the type and concentration of fluoride. Most of the mouthrinses contained sodium fluoride (90%); some contained stannous fluoride (5%) or potassium fluoride (7%). The concentrations of fluoride ranged from 100 ppm to 500 ppm.

Dentine hypersensitivity: a randomised, double‐blind placebo‐controlled study of the efficacy of a fluoride‐sensitive teeth mouthrinse

Potassium and fluoride salts have been used in the treatment of dentine hypersensitivity (DH). The primary aim of this study was to compare a fluoride-containing mouthrinse product with a placebo rinse for the treatment of DH. A secondary aim was to compare effects on plaque index (PI) and gingivitis index (GI). The study was a double-blind, randomised, two-treatment parallel design involving 91 subjects with DH. At screening, an air evaporative stimulus (AES) was used for selection. Approximately 4 weeks later, at the baseline visit, DH was recorded, using a response-based visual analogue scale (VAS) from upper and lower incisors, canines, premolars and first molars in response to AES and from two to six teeth in response to a cold water stimulus (CWS). PI and GI were also recorded. Subjects were then allocated the test or placebo rinse. On days 28 and 56, VAS scores were again recorded for AES and CWS, with PI and GI recorded at day 56. Both groups improved symptoms to day 28 and again to day 56. The majority of the data favoured the test rinse as more effective but there was no clear evidence for a statistically significant difference between test and placebo. PI at lingual sites approached significance for the test rinse. The placebo response has again been shown to play a significant role in DH clinical trials and narrows the range over which to detect treatment differences. Study designs to minimise or prevent the placebo response in DH seem worthy of consideration.

Effect of Fluoride-containing Mouthrinses on the Translucence of Resin Modified Glass Ionomer Cements

The aim of this in vitro study was to assess the effect of different fluoride-containing mouthrinses on the translucence of resin-modified glass ionomer cements, as a function of the restorative material used, the fluoride-containing solution employed and the time of immersion. Disks were prepared (10 mm × 2 mm) with the modified glass ionomer cements Vitremer (3M) and Fuji II LC (GC Co.), and immersed in three fluoride-containing solutions: Fluordent Reach (Johnson & Johnson), Fluorgard (Colgate-Palmolive) and Oral B (Gillette do Brasil Ltda). Translucence was measured with electrophoresis equipment (JOUAN) after different immersion times. The results obtained were submitted to ANOVA and Tukey's test, and a statistically significant decrease was observed in the translucence of the materials after immersion in the fluoride-containing solutions. Fuji II LC demonstrated the lowest alteration in translucence, independently of the solution employed. Fluorgard was the fluoride-containing mouthrinse that promoted the highest alteration. It can be concluded that the fluoride-containing mouthrinses influenced the translucence of the resin-modified glass ionomer cements.
