Long-term results of a rotationally unconstrained fixed-bearing total knee prosthesis.

Introduced in the market in 1990 by Ceraver (France), the posterior-stabilised (PS) Hermes prosthesis has limited literature regarding long-term survivability. The purpose of the study is to evaluate the survival and functional outcomes of the prosthesis. A retrospective case series was performed including 164 patients (176 knees) having undergone total knee arthroplasty with the Hermes prosthesis between 1997 and 2000 with a follow-up period of 18years. Kaplan-Meier analysis showed a survival rate of 99.4% (95% CI. 96.0-100.0%) at 18.4years with one revision. At final follow-up, the International Knee Society (IKS) functional score was 93.2 ± 15.6 and IKS knee score was 99.1 ± 2.5. The Hermes PS model is a low conformity prosthesis that offers reliable durability that is comparable to other popular designs while minimizing rotational constraints and having an approachable learning curve for new users.

In vivo comparison of rotating platform and fixed bearing knee replacements during lunge and pivot activities

BackgroundThe objective of this study was to discover whether notable differences in mobile and fixed-bearing kinematics occur during activity that promotes tibial rotation, and to compare these results with normal healthy kinematics. We hypothesized that rotating-platform knee replacements would exhibit greater rotation of the tibia relative to the fixed-bearing knee replacements. Materials and MethodsThe in vivo motion of the tibia relative to the femur was measured in subjects with posterior stabilized fixed-bearing (FB) and rotating-platform (RP) total knee arthroplasties using a high-speed stereo radiography system during a lunge and gait with a change in direction (pivot). ResultsThe in vivo internal/external (IE) rotation and anterior/posterior translation of the tibia relative to the femur was similar between mobile and fixed-bearing total knee prostheses during two activities of daily living that included an activity that challenged tibial IE rotation. Measurements of IE rotation in participants with RP had higher variability and significantly greater range between maximum internal and external rotation compared with FB participants. The greater amount of variability of RP was not unlike the healthy knee. ConclusionThe pattern of IE rotation and AP translation for both RP and FB designs were similar to healthy kinematics but with less IE rotation. The RP implants more closely replicated the asymmetrical posterior condylar translation and range of IE rotation of the healthy knee during activity that challenged tibial IE rotation.

Long-Term Comparison of Fixed-Bearing and Mobile-Bearing Total Knee Replacements in Patients Younger Than Fifty-one Years of Age with Osteoarthritis

There is limited information comparing the results of fixed-bearing total knee replacement and mobile-bearing total knee replacement in patients with osteoarthritis who are younger than fifty-one years and who have a fixed-bearing implant in one knee and a mobile-bearing implant in the other. The purpose of this study was to compare our long-term clinical and radiographic results of fixed-bearing total knee replacement and mobile-bearing total knee replacement in a group of patients from this population. We prospectively compared the results of 108 patients with osteoarthritis who were younger than fifty-one years (mean age, forty-five years) who had received a fixed-bearing prosthesis in one knee and a rotating platform mobile-bearing prosthesis in the other. The mean follow-up was 16.8 years (range, fifteen to eighteen years). The patients were assessed clinically and radiographically. Knee motion and function were assessed as a primary outcome. Patients were assessed with questionnaires, and each knee was assessed separately. Although there was significant improvement in both groups of knees, there was no significant difference between the groups (i.e., fixed-bearing and mobile-bearing knees) with regard to the mean postoperative knee motion (126° and 128°, respectively; p = 0.79), the mean Knee Society knee clinical score (95 and 94 points, respectively; p = 0.79), or the Knee Society knee functional score (84 and 85 points, respectively; p = 0.19) at the latest follow-up. In the fixed-bearing group, one knee was revised because of infection, two for aseptic loosening of the tibial component, and two because of wear of the tibial polyethylene insert. In the rotating platform mobile-bearing group, two knees were revised because of instability and one because of infection. The Kaplan-Meier survivorship for revision at 16.8 years of follow-up was 95% (95% confidence interval, 91 to 100) for the fixed-bearing prosthesis and 97% (95% confidence interval, 93 to 100) for the rotating platform mobile-bearing prosthesis. Long-term results of both fixed and mobile-bearing total knee arthroplasties were encouraging in patients who were younger than fifty-one years of age with osteoarthritis. However, we found no superiority of the mobile-bearing total knee prosthesis over the fixed-bearing total knee prosthesis.

Kinematics and early migration in single-radius mobile- and fixed-bearing total knee prostheses

BackgroundThe mobile-bearing variant of a single-radius design is assumed to provide more freedom of motion compared to the fixed-bearing variant because the insert does not restrict the natural movements of the femoral component. This would reduce the contact stresses and wear which in turn may have a positive effect on the fixation of the prosthesis to the bone and thereby decreases the risk for loosening. The aim of this study was to evaluate early migration of the tibial component and kinematics of a mobile-bearing and fixed-bearing total knee prosthesis of the same single-radius design. MethodsTwenty Triathlon single-radius posterior-stabilized knee prostheses were implanted (9 mobile-bearing and 11 fixed-bearing). Fluoroscopy and roentgen stereophotogrammetric analysis (RSA) were performed 6 and 12months post-operatively. FindingsThe 1year post-operative RSA results showed considerable early migrations in 3 out of 9 mobile-bearing patients and 1 out of 11 fixed-bearing patients. The range of knee flexion was the same for the mobile-bearing and fixed-bearing group. The mobile insert was following the femoral component during motion. InterpretationDespite the mobile insert following the femoral component during motion, and therefore performing as intended, no kinematic advantages of the mobile-bearing total knee prosthesis were seen. The fixed-bearing knee performed as good as the mobile-bearing knee and maybe even slightly better based on less irregular kinematics and less early migrations.

Wear-pattern analysis in retrieved tibial inserts of mobile-bearing and fixed-bearing total knee prostheses

Components from 73 failed knee replacements (TKRs) consisting of rotating-platform, mobile-bearing and fixed-bearing implants were examined to assess the patterns of wear. The patterns were divided into low-grade (burnishing, abrasion and cold flow) and high-grade (scratching, pitting/metal embedding and delamination) to assess the severity of the wear of polyethylene. The rotating-platform group had a higher incidence of low-grade wear on the upper surface compared with the fixed-bearing group. By contrast, high-grade wear comprising scratching, pitting and third-body embedding was seen on the lower surface. Linear regression analysis showed a significant correlation of the wear scores between the upper and lower surfaces of the tibial insert (R(2) = 0.29, p = 0.04) for the rotating-platform group, but no significant correlation was found for the fixed-bearing counterpart. This suggests that high-grade wear patterns on the upper surface are reduced with the rotating-platform design. However, the incidence of burnishing, pitting/third-body embedding and scratching wear patterns on the lower surface was higher compared with that in the fixed-bearing knee.

Is there any superiority in the clinical outcome of mobile-bearing knee prosthesis designs compared to fixed-bearing total knee prosthesis designs in the treatment of osteoarthritis of the knee joint?

To substantiate the validity and relevance of the mobile-bearing knee prosthesis design compared to the fixed-bearing design, concerning the clinical outcome, this review was conducted. Articles published in 6 major orthopaedic journals were screened. Only randomized controlled trials, which investigate the clinical outcome, were included. The clinical outcome parameters of each study were analysed. Despite the numerous quantities of publications in orthopaedic literature, we could conclude, that only a few of them are randomized controlled trials. Although better kinematics of mobile-bearing knee prosthesis designs compared to fixed-bearing knee prosthesis designs are reported, no superiority of one of the bearing designs concerning clinical outcome could be revealed. Because no superiority of one of the designs concerning revision rate, survival and outcome can be found, the cheaper one should be the one to be recommended. For this reason, we advise that further research, comparing the costs and cost-benefit of mobile-bearing compared to fixed-bearing knee prosthesis designs, should be performed.

Effect of conformity and contact stress on wear in fixed-bearing total knee prostheses

Ultra high molecular weight polyethylene (PE) remains the primary bearing surface of choice in total knee replacements (TKR). Wear is controlled by levels of cross-shear motion and contact stress. The aim of this study was to compare the wear of fixed-bearing total knee replacements with curved and flat inserts and to test the hypothesis that the flat inserts which give higher contact stresses and smaller contact areas would lead to lower levels of surface wear. A low-conforming, high contact stress knee with a low-medium level of cross shear resulted in significantly lower wear rates in comparison to a standard cruciate sacrificing fixed-bearing knee. The low wear solution found in the knee simulator was supported by fundamental studies of wear as a function of pressure and cross shear in the pin on plate system. Current designs of fixed-bearing knees do not offer this low wear solution due to their medium cross shear, moderate conformity and medium contact stress.

Bilateral total knee arthroplasty: One mobile-bearing and one fixed-bearing.

We compared the early results of mobile-bearing knee prosthesis with fixed-bearing knee prosthesis in 16 patients who had one-stage, sequential, bilateral replacements. In each patient, a Low Contact Stress (LCS, Depuy) rotating-platform prosthesis was inserted in one side, and an Anatomic Modular Knee (AMK, Depuy) posterior-stabilised prosthesis was inserted in the other side. The same surgical routines were adopted for both sides in each patient. There were significant improvements in the Knee Society knee score and functional score, as well as the Oxford Knee score after both mobile-bearing and fixed-bearing knee replacements (p<0.001). However, we could not find any significant difference between the clinical results of the two prostheses. The authors early experience with the mobile-bearing total knee prosthesis was as favourable as the medium-term experience of the fixed-bearing total knee prosthesis in this prospective, match-pair study.
