Stable polyethylene inlay fixation and low polyethylene wear rate in fixed-bearing total knee arthroplasty at five to six years' follow-up.

Micromotion of the polyethylene (PE) inlay may contribute to backside PE wear in addition to articulate wear of total knee arthroplasty (TKA). Using radiostereometric analysis (RSA) with tantalum beads in the PE inlay, we evaluated PE micromotion and its relationship to PE wear. A total of 23 patients with a mean age of 83 years (77 to 91), were available from a RSA study on cemented TKA with Maxim tibial components (Zimmer Biomet). PE inlay migration, PE wear, tibial component migration, and the anatomical knee axis were evaluated on weightbearing stereoradiographs. PE inlay wear was measured as the deepest penetration of the femoral component into the PE inlay. At mean six years' follow-up, the PE wear rate was 0.08 mm/year (95% confidence interval 0.06 to 0.09 mm/year). PE inlay external rotation was below the precision limit and did not influence PE wear. Varus knee alignment did not influence PE wear (p = 0.874), but increased tibial component total translation (p = 0.041). The PE inlay was well fixed and there was no relationship between PE stability and PE wear. The PE wear rate was low and similar in the medial and lateral compartments. Varus knee alignment did not influence PE wear.

Meta-analysis of factors influencing anterior knee pain after total knee arthroplasty.

Total knee arthroplasty (TKA) is a mature procedure recommended for correcting knee osteoarthritis deformity, relieving pain, and restoring normal biomechanics. Although TKA is a successful and cost-effective procedure, patient dissatisfaction is as high as 50%. Knee pain after TKA is a significant cause of patient dissatisfaction; the most common location for residual pain is the anterior region. Between 4% and 40% of patients have anterior knee pain (AKP). To investigate the effect of various TKA procedures on postoperative AKP. We searched PubMed, EMBASE, and Cochrane from January 2000 to September 2022. Randomized controlled trials with one intervention in the experimental group and no corresponding intervention (or other interventions) in the control group were collected. Two researchers independently read the title and abstract of the studies, preliminarily screened the articles, and read the full text in detail according to the selection criteria. Conflicts were resolved by consultation with a third researcher. And relevant data from the included studies were extracted and analyzed using Review Manager 5.4 software. There were 25 randomized controlled trials; 13 were comparative studies with or without patellar resurfacing. The meta-analysis showed no significant difference between the experimental and control groups (P = 0.61). Six studies were comparative studies of circumpatellar denervation vs non-denervation, divided into three subgroups for meta-analysis. The two-subgroup meta-analysis showed no significant difference between the experimental and the control groups (P = 0.31, P = 0.50). One subgroup meta-analysis showed a significant difference between the experimental and control groups (P = 0.001). Two studies compared fixed-bearing TKA and mobile-bearing TKA; the results meta-analysis showed no significant difference between the experimental and control groups (P = 0.630). Two studies compared lateral retinacular release vs non-release; the meta-analysis showed a significant difference between the experimental and control groups (P = 0.002); two other studies compared other factors. Patellar resurfacing, mobile-bearing TKA, and fixed-bearing TKA do not reduce the incidence of AKP. Lateral retinacular release can reduce AKP; however, whether circumpatellar denervation can reduce AKP is controversial.

Aseptic loosening of the option stemmed tibial tray in the Zimmer NexGen LPS total knee arthroplasty system

BackgroundWe investigated the relationship between the backside deformation of polyethylene (PE) tibial inserts and aseptic loosening of the Option stemmed tibial tray used with Zimmer NexGen posterior-stabilised (PS) devices. We hypothesized that explanted inserts used in PS designs would exhibit greater extents of PE backside deformation than those used in equivalent cruciate retaining (CR) designs and that PE inserts retrieved from total knee arthroplasties (TKAs) revised for aseptic tibial tray loosening would exhibit greater extents of backside deformation than TKAs revised for other reasons. MethodsA total of 73 explanted fixed-bearing TKAs (42 CR and 31 PS) were examined. PE components underwent geometric examination with a coordinate measuring machine using validated techniques. Multiple regression modelling was used to identify variables associated with revision secondary to aseptic loosing and to determine factors associated with increased PE backside deformation. ResultsPE inserts retrieved from TKAs with aseptic loosening had significantly greater backside deformation than those retrieved from TKAs revised for other reasons (p < 0.001). Greater PE backside deformation was significantly associated with larger tray/insert clearance heights (p < 0.001), thinner inserts (p < 0.001) and PS TKAs (p = 0.001). ConclusionPE backside deformation was significantly greater in the PS TKAs. This may provide one explanation for the increased rate of aseptic loosening reported with the Option tibial tray used with the Legacy Posterior Stabilised (LPS) system.

Wider translations and rotations in posterior-stabilised mobile-bearing total knee arthroplasty compared to fixed-bearing both implanted with mechanical alignment: a dynamic RSA study

PurposeThe purpose of this study was to investigate the in vivo kinematics of the same femoral design mechanically aligned posterior-stabilised (PS) total knee arthroplasty (TKA) with either fixed-bearing (FB) or mobile-bearing (MB) inlay, implanted by the same surgeon, using model-based dynamic radiostereometric analysis (RSA). The hypothesis of the present study was that the MB design would show wider axial rotation than the FB design, without affecting the clinical outcomes.Materials and methodsA cohort of 21 non-randomised patients (21 DePuy Attune PS-FB) was evaluated by dynamic RSA analysis at a minimum 9-month follow-up, while performing differently demanding daily living activities such as sit to stand (STS) and deep knee lunge (DKL). Kinematic data were compared with those of a cohort of 22 patients implanted with the same prosthetic design but with MB inlay. Anterior–posterior (AP) translations, varus–valgus (VV) and internal–external (IE) rotations of the femoral component with respect to the tibial baseplate were investigated. Translation of medial and lateral compartment was analysed using the low point method according to Freeman et al. Questionnaires to calculate objective and subjective clinical scores were administered preoperatively and during follow-up visit by the same investigator.ResultsThe FB TKA design showed lower AP translation during STS (6.8 ± 3.3 mm in FB vs 9.9 ± 3.7 mm in MB, p = 0.006*), lower VV rotation (1.9 ± 0.8° in FB vs 5.3 ± 3.3° in MB, p = 0.005) and lower IE rotation (2.8 ± 1.1° in FB vs 9.5 ± 4.3° in MB, p = 0.001) during DKL than the mobile-bearing TKA design. Posterior-stabilised FB group showed significant lower translation of the low point of the medial compartment than the MB group (p = 0.008). The percentage of patients performing medial pivot in the FB group was higher compared to MB group in the examined motor tasks. No significant differences in post-operative range of motion (117° ± 16° for FB group and 124° ± 13° for MB group) and in clinical outcomes emerged between the two cohort.ConclusionsThe FB and MB designs differed in AP translations, VV rotations and IE rotations of the femoral component with respect to the tibial component in STS and DKL. Furthermore, FB cohort reported a significant higher percentage of medial pivot with respect to MB cohort. Despite this, no differences in clinical outcomes were detected between groups. Both designs showed stable kinematics and represent a viable option in primary TKA.Level of evidenceProspective cohort study, II.

Joint awareness after fixed and mobile-bearing total knee arthroplasty with minimum 12 years of follow-up: A propensity matched-pair analysis

BackgroundMany comparative articles studied mobile-bearing (MB) and fixed-bearing (FB) total knee arthroplasties (TKAs). Meta-analyses found no difference in survival or biomechanical outcome. This study aimed to compare long-term clinical results between fixed-bearing (FB) and mobile-bearing (MB) total knee arthroplasty (TKA) as well as patients’ adaptation to their artificial joints. MethodTKAs performed with the same surgical protocol divided into categories according to the insert design preferred. 70 MB design TKAs were compared with 70 FB design TKAs utilizing propensity matching for parameters; gender, age, body mass index, coronal plane deformity, range of motion (ROM) and appropriateness criteria. Forgotten Joint Score-12 (FJS-12) was used to assess patients’ ability to forget their artificial joints in daily life. ResultsPatients had a mean follow-up of 15.6 (±2.2) years. No difference was observed between groups for post-operative ROM, WOMAC, Knee Society Knee and Function Scores. The FJS-12 in the MB and FB groups were 66.1 and 72.8, respectively (P = 0.026). There was no significant difference in survival between both designs. ConclusionThis study suggests that in TKA, joint awareness is higher in MB compared to FB design. FJS-12 appears to be a sensitive measuring tool when comparing two designs and should be implemented in long-term follow-up.

NO DIFFERENCE BETWEEN RP AND FB IN TKA AFTER TWO YEARS IN MULTIPLE PROMS: A PROSPECTIVE, MULTICENTRE, NON-RANDOMIZED STUDY

Total Knee Arthroplasty (TKA) is an effective surgery performed for knee osteoarthritis. Despite this success up to 20% of patients are less than fully satisfied. Rotating platform (RP) bearings have demonstrated significant reduction in rates of wear in laboratory studies but thus far have failed to demonstrate a clinical difference compared to fixed bearing (FB) arthroplasty in patients. This may be due to studies limited by small sample size and single centre design. Additionally, no studies have utilised modern patient-reported outcome measures (PROMs) such as PKIP to investigate differences between these bearings.A non-randomised, prospective, multi-centre study was conducted across centres in Australia, United Kingdom and the United States from November 2012 to May 2015. Knee prosthesis included were the RP and FB models of the PFC Sigma Knee (Depuy, Synthes) and the ATTUNE Knee (Depuy, Synthes). Data was collected on pre-arthroplasty interventions, demographics and PROMs. PROMs included were the KOOS, OKS, EQ53DL and PKIP. Participants were followed-up for 2 years.A total of 1,718 patients were recruited across 27 centres. 959 participants underwent a FB TKA, 759 participants underwent a RP TKA. No significant difference was found between RP and FB in change from baseline at 2 years across any parameter in all PROMs investigated at 2 years. Body Mass Index pre-operatively was the biggest indicator of change from baseline at 2 years.Our data demonstrates similar improvements in the short-term in both RP and TKA across all PROMs. Patients reported similar scores in satisfaction, confidence, mobility and stability in the PKIP PROM for both RP and FB groups. We have not found any evidence to suggest that RP TKA is disadvantageous at 2 years, and this is reassuring for any surgeon looking to utilise this bearing type in practice.Our data does not demonstrate a clinical advantage in RP TKA over FB TKA at 2 years in any PROM.

Validation of a novel finite-element model for evaluating patellofemoral forces and stress during squatting after posterior-stabilized total knee arthroplasty

IntroductionSeveral studies have documented the relationship between patellofemoral pain and patient dissatisfaction after total knee arthroplasty (TKA). However, few computer simulations have been designed to evaluate the patellofemoral joint during flexion. The aim of this study was to validate a new computational simulation, driven by forces and moments, and to analyze patellofemoral reaction forces and stress under squat loading conditions after TKA implantation. HypothesisThis computational simulation of a squat using a model driven by forces and moments is comparable to in vitro and in silico data from the literature. Material and methodsWe developed a finite element model of the lower limb after implantation of a fixed-bearing posterior-stabilized TKA. To simulate squat loading conditions when standing on both legs, an initial load of 130N was applied to the center of the femoral head. Quadriceps force, patellofemoral contact force and Von Mises stress on the patellar implant, tibiofemoral contact forces and pressure on the tibial insert, and post-cam contact force were evaluated from 0° to 100° of knee flexion. ResultsQuadriceps force increased during flexion, up to 6 times the applied load. Von Mises stress on patellar implant increased up to 16MPa at 100° flexion. Tibiofemoral contact forces increased up to 415 N medially and 339 N laterally, with 64% distributed medially on the tibial insert. Post-cam contact started slightly before 70° of flexion. DiscussionIn this simulation, tibiofemoral, patellofemoral and post-cam contact forces, and pressure distribution on the tibial insert were consistent with various published studies. This agreement suggests that computational simulation driven by forces and moments can reproduce squat loading conditions during knee flexion after TKA, without experimental kinematic data used to drive the simulation. ConclusionThis study represents an initial step towards validating tibiofemoral and patellofemoral mechanical behavior under squat conditions, from this computational simulation driven by forces and moments. This model will help us better understand the influence of various implantation techniques on patellofemoral forces and stress during flexion. Level of evidenceIV, biomechanical computational study.

Mobile-Bearing has no Benefit Over Fixed-Bearing Total Knee Arthroplasty in Joint Awareness and Crepitus: A Randomized Controlled Trial

BackgroundGiven higher component conformity, rotational availability of polyethylene insert, and more physiologic patellofemoral tracking, mobile-bearing (MB) total knee arthroplasty (TKA) is supposed to offer advantages in joint perception and crepitus. The purpose of this study was to investigate whether MB TKA offers superior outcomes over fixed-bearing (FB) TKA in terms of joint awareness and crepitus. MethodsThis prospective, randomized, controlled trial included 49 FB and 49 MB TKAs that shared the same posterior-stabilized femoral component. Primary outcomes included joint awareness assessed with the Forgotten Joint Score and crepitus graded according to severity and sound at 3 years postoperatively. Secondary outcomes were the range of motion, the Knee Society Score, Western Ontario and McMaster Universities Osteoarthritis Index, component position, and joint line level on radiographs. ResultsThe Forgotten Joint Score showed no significant difference between the FB (67 points; range, 0-100) and MB groups (63 points; range, 13-100) (P = .440). For crepitus, no significant differences were found in the overall incidence rate (FB group, 18%; MB group, 12%; P = .386) and grade (grade 1, 6 versus 5; grade 2, 2 versus 1; grade 3, 1 versus 0, respectively). There were no significant differences in range of motion, Knee Society Score, and Western Ontario and McMaster Universities Osteoarthritis Index, as well as radiographic outcomes between the two groups (all, P > .05). ConclusionMB TKA offers no benefits in terms of joint awareness and crepitus compared with FB TKA at mean 3 years postoperatively. The theoretical advantages of MB TKA have yet to be demonstrated in practice, leaving the selection of bearing type to the surgeon’s judgment. Level of EvidenceLevel II.

Optimal Conformity Design of Tibial Insert Component Based on ISO Standard Wear Test Using Finite Element Analysis and Surrogate Model

Total knee replacement is a standard surgical treatment used to treat osteoarthritis in the knee. The implant is complicated, requiring expensive designs and testing as well as a surgical intervention. This research proposes a technique concerning the optimal conformity design of the symmetric polyethylene tibial insert component for fixed-bearing total knee arthroplasty. The Latin Hypercube Sampling (LHS) design of the experiment was used to create 30 cases of the varied tibial insert conformity that influenced the total knee replacement wear volume. The combination of finite element analysis and a surrogate model was performed to predict wear volume according to the standard of ISO-14243:2014 wear test and to determine the optimal conformity. In the first step, the results could predict wear volume between 5.50 to 72.92 mm3/106 cycle. The Kriging method of a surrogate model has then created the increased design based on the efficient global optimization (EGO) method with improving data 10 design points. The result revealed that the optimum design of tibial insert conformity in a coronal and sagittal plane was 0.70 and 0.59, respectively, with a minimizing wear volume of 3.07 mm3/106 cycle. The verification results revealed that the area surface scrape and wear volume are similar to those predicted by the experiment. The wear behavior on the tibial insert surface was asymmetry of both sides. From this study it can be concluded that the optimal conformity design of the tibial insert component can be by using a finite element and surrogate model combined with the design of conformity to the minimized wear volume.

Tibial tray debonding from the cement mantle is associated with deformation of the backside of polyethylene tibial inserts

The aim of this study was to investigate whether wear and backside deformation of polyethylene (PE) tibial inserts may influence the cement cover of tibial trays of explanted total knee arthroplasties (TKAs). At our retrieval centre, we measured changes in the wear and deformation of PE inserts using coordinate measuring machines and light microscopy. The amount of cement cover on the backside of tibial trays was quantified as a percentage of the total surface. The study involved data from the explanted fixed-bearing components of four widely used contemporary designs of TKA (Attune, NexGen, Press Fit Condylar (PFC), and Triathlon), revised for any indication, and we compared them with components that used previous generations of PE. Regression modelling was used to identify variables related to the amount of cement cover on the retrieved trays. A total of 114 explanted fixed-bearing TKAs were examined. This included 76 used with contemporary PE inserts which were compared with 15 used with older generation PEs. The Attune and NexGen (central locking) trays were found to have significantly less cement cover than Triathlon and PFC trays (peripheral locking group) (p = 0.001). The median planicity values of the PE inserts used with central locking trays were significantly greater than of those with peripheral locking inserts (205 vs 85 microns; p < 0.001). Attune and NexGen inserts had a characteristic pattern of backside deformation, with the outer edges of the PE deviating inferiorly, leaving the PE margins as the primary areas of articulation. Explanted TKAs with central locking mechanisms were significantly more likely to debond from the cement mantle. The PE inserts of these designs showed characteristic patterns of deformation, which appeared to relate to the manufacturing process and may be exacerbated in vivo. This pattern of deformation was associated with PE wear occurring at the outer edges of the articulation, potentially increasing the frictional torque generated at this interface. Dislocation of the polyethylene insert following fixed-bearing total knee arthroplasty: A case report

Dislocation of the polyethylene liner is a rare complication of fixed-bearing total knee arthroplasty with only a few cases described in the literature. This case report describes a 62 year old gentleman with previous total knee arthroplasty (TKA) using a cemented Physica posterior stabilised implant who experienced an anterior dislocation of the polyethylene liner after a fall causing hyperflexion. This is to the best of our knowledge the first case in literature that describes sudden dislocation of the polyethylene liner following traumatic injury.

Mobile-bearing versus fixed-bearing total knee arthroplasty: a meta-analysis of randomized controlled trials.

ObjectiveThe purpose of this study was to perform a meta-analysis comparing mobile-bearing with fixed-bearing total knee arthroplasty (TKA) in terms of all-cause revision rates, aspetic loosening, knee functional scores, range of motion and radiographic lucent lines and osteolysis.MethodsPubMed, Cochrane Library, Google Scholar and Web of Science were searched up to January 2020. Randomized controlled trials that compared primary mobile-bearing with fixed-bearing TKA, reporting at least one of the outcomes of interest, at a minimum follow-up of 12 months were included. All outcomes of interest were pooled at short-term (< 5 years), mid-term (5 to 9 years) and long-term (> = 10 years) follow-up intervals.ResultsA total of 70 eligible articles were included in the qualitative and statistical analyses. There was no difference between mobile-bearing or fixed-bearing TKA at short-term, mid-term and long-term follow-ups in all outcome measures including all-cause revision rate, aseptic loosening, oxford knee score, knee society score, Hospital for Special Surgery score, maximum knee flexion, radiographic lucent lines and radiographic osteolysis.ConclusionThe current level of evidence demonstrated that both mobile-bearing and fixed-bearing designs achieved excellent outcomes, yet it does not prove the theoretical advantages of the mobile-bearing insert over its fixed-bearing counterpart. Gradual-radius femoral component with s-curve post-cam provides stable kinematics at mid-flexion after total knee arthroplasty

BackgroundMid-flexion instability is the one of the reasons for patient dissatisfaction after total knee arthroplasty (TKA). The purposes of this study were to evaluate in vivo knee kinematics and clinical outcomes using a novel TKA design with a gradual femoral radius component and s-curve post-cam, which are intended to prevent the instability initiated by sudden reductions in the femoral radius observed with conventional components. MethodsWe used radiographic-based, image-matching techniques to analyze femorotibial anteroposterior translation, axial rotation, and anterior/posterior cam-post contact during two dynamic movements, squatting and stair climbing, in 20 knees that had undergone posterior-stabilized fixed-bearing TKA with an improved sagittal profiles of the femoral component and post-cam mechanism. We also evaluated patient-reported outcomes assessed by the 2011 Knee Society Score (KSS 2011). ResultsSquatting and stair climbing produced a similar trend in anteroposterior translation and a relatively small standard deviation at mid-flexion. Although the rotation angles varied widely during squatting and stair climbing, the femoral component was consistently externally rotated. Anterior/posterior cam-post contact during squatting and stair climbing were observed in 0/17 knees and 0/0 knees, respectively. The “Symptoms”, “Satisfaction”, and “Functional activities” subscales of the KSS 2011 were significantly (P < 0.05) improved postoperatively compared to preoperatively (“Symptoms”, 10 to 21; “Satisfaction”, 15 to 26; “Functional activities”, 25 to 71). ConclusionA gradual femoral radius component with an s-curve post-cam provided stable kinematics and favorable clinical results during squatting and stair climbing at 1 year after surgery.

A cemented mobile-bearing total knee prosthesis prevents peri-prosthetic bone mineral density loss around the femoral component: a consecutive follow-up at a mean of 11years.

Research has shown that a cemented mobile-bearing component has a favorable effect on the bone mineral density (BMD) of the distal femur at 2years after total knee arthroplasty (TKA). This study was performed to determine whether the advantage on BMD of a cemented mobile-bearing TKA over a conventional cemented fixed-bearing TKA changes with time. This report is an update of a matched cohort study initiated in 2004 and for which the 2-year results have been published. Twenty-eight knees that were treated with a fixed-bearing posterior stabilized (PS) prosthesis and 28 matched knees from a database of 76 knees that were treated with a mobile-bearing PS prosthesis in the same period were investigated. All knees underwent dual-energy X-ray absorptiometry (DEXA) scans around the femoral component preoperatively, 2weeks postoperatively, 5years postoperatively, and annually thereafter. Eighteen knees with a cemented mobile-bearing PS prosthesis and 20 knees with a cemented fixed-bearing PS prosthesis were investigated for more than 6years. The mean follow-up period was 11years. The range of motion, Knee Society Score, BMD of the lumbar spine, and follow-up period were not significantly different preoperatively and postoperatively in the two groups. In the fixed-bearing group, the BMD of the anterior part of the femoral condyle decreased postoperatively. In the mobile-bearing group, the BMD of the posterior part of the femoral condyle increased postoperatively. The postoperative change in the BMD at 5years and the latest follow-up period was statistically significant in the two groups. This DEXA study revealed that a cemented mobile-bearing component had a favorable effect on the BMD of the distal femur after TKA even at a mean of 11years postoperatively. Effect of Coronal Alignment on 10-Year Survivorship of a Single Contemporary Total Knee Arthroplasty.

Debate remains regarding the utility of mechanical axis alignment as a predictor of durability after total knee arthroplasty (TKA). Our study aimed to assess the effects of coronal alignment on implant durability, clinical outcomes, and radiographic results with a single fixed-bearing TKA design. All patients undergoing primary cemented TKA of a single design (Stryker Triathlon) from 2005–2007 with >10 years of follow-up and available pre-operative and post-operative hip–knee–ankle radiographs were included (n = 89). Radiographs were measured to determine coronal alignment and assessed for loosening. Mean preoperative mechanical axis alignment was −6° ± 6.7° (varus, range, −16°–23°), while mean post-operative alignment was −1° ± 2.7° (varus, range, −3°–15°). The aligned group was defined as knees with a post-operative mechanical axis of 0° ± 3° (n = 73) and the outlier group as those outside this range (n = 16). No patients underwent revision. Ten-year survivorship free from any reoperation was 99% and 100% in the aligned and outlier groups, respectively (p = 0.64). Knee Society scores improved significantly in both groups (p < 0.001) and did not differ at final follow-up (p = 0.15). No knees demonstrated radiographic evidence of loosening. Post-operative mechanical axis alignment within 3° of neutral was not associated with improved implant durability, clinical outcomes, or radiographic results at 10 years following primary TKA.

Long-Term (Up to 27 Years) Prospective, Randomized Study of Mobile-Bearing and Fixed-Bearing Total Knee Arthroplasties in Patients

BackgroundOur study determined long-term (up to 27 years) results of fixed-bearing vs mobile-bearing total knee arthroplasties (TKAs) in patients <60 years with osteoarthritis. MethodsThis study included 291 patients (582 knees; mean age 58 ± 5 years), who received a mobile-bearing TKA in one knee and a fixed-bearing TKA in the other. The mean duration of follow-up was 26.3 y (range 24-27). ResultsAt the latest follow-up, the mean Knee Society knee scores (91 ± 9 vs 89 ± 11 points, P = .383), Western Ontario and McMaster Universities Osteoarthritis Index (35 ± 7 vs 37 ± 6 points, P = .165), range of knee motion (128° ± 13° vs 125° ± 15°, P = .898), and University of California, Los Angeles activity score (6 ± 4 vs 6 ± 4 points, P = 1.000) were below the level of clinical significance between the 2 groups. Revision of mobile-bearing and fixed-bearing TKA occurred in 16 (5.5%) and 20 knees (6.9%), respectively. The rate of survival at 27 years for mobile-bearing and fixed-bearing TKA was 94.5% (95% confidence interval 89-100) and 93.1% (95% confidence interval 88-98), respectively, and no significant differences were observed between the groups. Osteolysis was identified in 4 knees (1.4%) in each group. ConclusionThere were no significant differences in functional outcomes, rate of loosening, osteolysis, or survivorship between the 2 groups.

Mobile Bearing versus Fixed Bearing for Total Knee Arthroplasty: Meta-analysis of Randomized Controlled Trials at Minimum 10-Year Follow-up.

This meta-analysis aimed to compare the clinical and radiographic outcomes between mobile-bearing total knee arthroplasty (MB-TKA) and fixed-bearing total knee arthroplasty (FB-TKA) at a minimum 10-year follow-up. PubMed, EMBASE, and Cochrane databases were searched. All included articles were evaluated by two trained reviewers according to the guidelines of the Cochrane Collaboration Handbook for potential risk, and the Consolidated Standards on Reporting Trials (CONSORT) checklist and scoring system was also used to assess the methodological quality of each study. The extracted data included function scores, range of motion (ROM) of the knee, incidence of adverse events or revision, survivorship analysis, and radiographic outcomes. Seven randomized controlled trials (RCTs) were included in this meta-analysis, and all RCTs had a follow-up period longer than 10 years. This meta-analysis shows no significant difference between the two groups with respect to the Keen Society Score (KSS; p = 0.38), KSS function score (p = 0.30), the Western Ontario and McMaster Universities Osteoarthritis index (WOMAC; p = 0.59), ROM (p = 0.71), radiolucent line (p = 0.45), femoral and tibial component positions in the coronal plane (p = 0.55 and 0.35, respectively), revision incidence (p = 0.77), and survivorship rates (p = 0.39). Meanwhile, it showed a slight difference between the two groups in the tibial component position in the sagittal plane (p = 0.003). According to this meta-analysis, the current best available evidence suggests no significant difference between the MB-TKA and FB-TKA groups with respect to the clinical outcomes, radiographic outcomes, revision, and survivorship at a minimum 10-year follow-up. Superior Mid- to Long-Term Clinical Outcomes of Mobile-Bearing Total Knee Arthroplasty Compared to Fixed-Bearing: A Meta-Analysis Based on a Minimum of 5 Years of Study.

As more patients undergo total knee arthroplasty (TKA) each year, and the average age of patients gets younger, the patients are generally more active requiring a greater physiological demand and increasing range of motion on the prosthesis than the previous patients. However, there is no consensus on the optimal TKA tibial bearing design. We performed this systematic review to compare the clinical differences between mobile and fixed bearing constructs used in contemporary TKA. We searched PubMed, EMBASE, and Cochrane Library databases, identifying 515 total publications, including 17 randomized controlled trials (RCTs). A meta-analysis was performed, while the quality and bias of the evidence were rated according to the Grading of Recommendations Assessment Development and Evaluation (GRADE) guidelines and the Cochrane Database questionnaire. The meta-analysis was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. Seventeen studies were included, with a total of 1505 knees receiving a mobile bearing TKA and 1550 knees receiving a fixed bearing TKA. The meta-analysis compared clinical outcomes between mobile bearing (MB)-TKA and fixed bearing (FB)-TKA using postoperative Knee Society Score, postoperative ROM, and survivorship and showed that there was a distinct difference in Knee Society Score between the mobile-bearing and fixed-bearing groups (overall standardized mean difference = 1.38; 95% confidence interval (CI): 0.50-2.25; p = 0.002; I2 = 60%). Patients treated with mobile-bearing prostheses were more likely to report good or excellent range of motion results (overall standardized mean difference = 2.06; 95% CI: 0.65-3.47; p = 0.004). No difference in implant survivorship or reoperation rate were identified. The fixed-bearing and mobile-bearing TKA designs are both capable of producing excellent long-term results with excellent clinical outcomes if properly implanted; however, the mobile-bearing TKA have superiority in mid- to long-term clinical results. Peri-prosthetic bone mineral density after simultaneous bilateral total knee arthroplasty under oral bisphosphonate therapy — A comparison between mobile- and fixed-bearing prostheses

BackgroundPeri-prosthetic bone mineral density (BMD) decreases after total knee arthroplasty (TKA). We aimed to specifically compare peri-prosthetic BMD changes between mobile- and fixed-bearing prostheses in patients undergoing oral bisphosphonate therapy, hypothesizing that mobile-bearing components would have a favorable effect on postoperative peri-prosthetic BMD. MethodsThis prospective cohort study investigated 30 patients who underwent simultaneous bilateral TKA for primary knee osteoarthritis between December 2007 and September 2012. All patients underwent mobile-bearing TKA in one knee and fixed-bearing TKA in the other and received oral alendronate therapy at a dosage of 35 mg/week. Peri-prosthetic and lumbar spine BMDs were measured using dual X-ray absorptiometry scans, with peri-prosthetic BMD changes being compared between the two prostheses in each patient at six months and one, two, three, and five years post-operation. ResultsClinical results did not significantly differ between two prostheses. Relative change of lumbar spine BMD was significantly greater at five years post-operation than at one year post-operation (p = 0.01), and was significantly correlated at five years post-operation with peri-prosthetic BMD in the central femur (r = 0.39, p = 0.002), posterior femur (r = 0.39, p = 0.002), and medial tibia (r = 0.42, p = 0.007). ConclusionsThere was no difference in peri-prosthetic BMD changes between two prostheses in patients undergoing oral bisphosphonate therapy. Our results suggest that the influence of oral bisphosphonate therapy might offset the influence of prosthetic design. Thus, oral bisphosphonate therapy may be more effective than prosthetic design selection in preventing post-TKA peri-prosthetic BMD loss.Level of evidence: II

Mobile-bearing insert reduced patellar contact force at knee flexion during posterior stabilized total knee arthroplasty

BackgroundThis study aimed to compare tibial rotation and patellar contact force between mobile- and fixed-bearing total knee arthroplasty from extension to flexion by using a navigation system and patellar contact force sensor on the same patients' knees. MethodsThirty-one consecutive patients who had undergone a primary posterior stabilized total knee arthroplasty were included. Patellar contact forces on the medial and lateral sides were measured at each flexion angle, and tibial rotation was assessed during 30–90°, and 90–120° knee flexion. The patellar contact force and tibial rotation were measured twice with the mobile- and fixed-platform trial components and compared between the two groups. FindingsThe patellar contact force was significantly lower with mobile than with fixed-bearing total knee arthroplasty on the medial side at 120° flexion (P = .0138) and lateral side at 60°, 90°, and 120° flexion (P = .0346, P = .0127, and P = .0376). There were no significant differences in tibial rotation between the mobile- and fixed-bearing inserts during both 30–90° and 90–120° knee flexion. InterpretationPatellar contact force was significantly lower, especially on the lateral side in mobile than in fixed-bearing total knee arthroplasty, whereas no significant difference in tibial internal rotation was found between the two inserts. Mobile-bearing inserts might reduce the patellar contact force by the bearing rotation, rather than natural tibial rotation during posterior stabilized total knee arthroplasty.
