Imaging and Hematologic Findings in Thrombosis and Thrombocytopenia after ChAdOx1 nCoV-19 (AstraZeneca) Vaccination.

This case series reports six patients (4 men; median age 38 years; interquartile range 26-48) presenting with vaccine-induced thrombocytopenia and thrombosis beginning 3 to 26 days after receiving the first dose of the ChAdOx1 nCoV-19 (AstraZeneca) vaccine. The patients were admitted to a general hospital between 9 to 31 days after the first dose. All patients had strongly detected anti-platelet factor 4 antibodies and severe thrombosis. Laboratory features included thrombocytopenia and elevated D-Dimer levels. Thrombotic events were predominantly venous; two patients had arterial or mixed arterial/venous thrombosis. All patients recovered after receiving intravenous immunoglobulin and non-heparin based anticoagulation.

A randomised clinical trial comparing the patient comfort and efficacy of three different graduated compression stockings in the prevention of postoperative deep vein thrombosis.

To compare the comfort levels of patients regarding the use of three different graduated compression stockings and to analyse the efficacies of the graduated compression stockings in relation to patient comfort and compliance in prevention of postoperative deep vein thrombosis. Graduated compression stockings are very important with other prophylaxis methods in postoperative deep vein thrombosis prophylaxis. In meta-analyses and systematic review studies, it was reported that knee-length and thigh-length graduated compression stockings had similar efficacies. However, there is no randomised study in literature regarding the patient problems and levels of comfort with the use of graduated compression stockings of different sizes and pressures. A randomised clinical trial design. A total of 219 patients were randomised into three groups (n=73 in each group). Group I was given low-pressure, knee-length graduated compression stockings, group II was given low-pressure, thigh-length graduated compression stockings and group III was given moderate-pressure, knee-length graduated compression stockings. The level of patients comfort regarding the graduated compression stockings and occurrence of deep vein thrombosis were examined. The vast majority of the patients (79·5%) in group III and 52·1% of the patients in group II stated experiencing problems during the use of the graduated compression stockings (p<0·001). The graduated compression stockings were reported by the patients as being very comfortable in the group I (p<0·001). No findings of thrombosis were observed in any of the groups. The low-pressure, knee-length graduated compression stockings are as effective as the other graduated compression stockings of different pressures and sizes in the postoperative deep vein thrombosis prophylaxis, and the patients have fewer problems while using these graduated compression stockings with a high satisfaction. The combined use of pharmacological, mechanical and physical methods and patient education is effective in the prevention of postoperative deep vein thrombosis. The use of low-pressure, knee-length graduated compression stockings in clinical practice may be recommended, as the patients have fewer problems while using these graduated compression stockings with a high satisfaction.

Relationship between magnetic resonance imaging and histopathologic findings in thrombosis in the oral and maxillofacial region: a case report

Histopathologic changes are observed during the clinical course of thrombosis, and the evaluation of such changes by magnetic resonance imaging (MRI) might enhance the accuracy of qualitative diagnosis of the disease. The relationship between histopathologic and MRI findings in the chronic phase of deep venous thrombosis (DVT) that developed in the masseter muscle of a 50-year-old Japanese female patient is described. Two regions with different MRI signal intensities were identified, and a 3-layer structure was observed by microscopy. The distinct MRI regions correlated with the microscopic structure, suggesting that MRI can be used for qualitative imaging of a maxillofacial thrombus.

The use of recombinant activated factor VII in the treatment of massive pulmonary hemorrhage in a preterm infant

Pulmonary hemorrhage is a rare but well-known complication in preterm infants. We present a case of massive pulmonary hemorrhage in a 9-day-old male infant, successfully treated with intravenous recombinant activated factor VII (rFVIIa) (NovoSeven; Novo Nordisk). The infant was diagnosed with sepsis-related disseminated intravascular coagulation and required ventilator support for respiratory distress syndrome and blood transfusions due to active bleeding from endotracheal tube. After administration of the second dose of rFVIIa (120 microg/kg per dose, every 2 h), the active bleeding subsided dramatically and a significant improvement in the oxygenation index was seen 8 h after the third dose of rFVIIa treatment. There were also significant improvements in the prothrombin time, International Normalized Ratio, activated partial thromboplastin time and plasma fibrinogen levels after the third dose of rFVIIa treatment. The infant was discharged on day 82 of life and there was no finding of thrombosis during the hospitalization period. At month 18 of follow-up, there was no morbidity related to the pulmonary and central nervous systems. This case suggests that rFVIIa is effective as an alternative therapy in controlling massive pulmonary hemorrhage of preterm infants.

Non-Invasive Contrast Enhanced Electron Beam Tomography: A Three Dimension Imaging Adjunctive Test To Detect Renal Vascular and A-V Fistula Graft Abnormalities.

Electron beam tomography (EBCT) is a relatively new technology with several potential advantages over traditional CT such as fast acquisition time, less exposure to radiation, less sedation and volume of dye, and better 3D visualization. A retrospective study was done to assess the use of 3D EBCT to detect renal vascular abnormalities and patency of arteriovenous-fistula (A-VF) and arteriovenous-graft (A-VG). EBCT was done in 7 children (age 3–18 year, mean age 10.8 years). All had hypertension (> 95%) and high rennin levels. Four of the patients (group I) had normal renal function. One patient (group II) was 20 days post enblock renal transplantation with ascites, hypertension and renal dysfunction. Two patients (group III) had end-stage renal disease, hypertension, and AV-F and AV-G. One of these patients had SLE and difficult IV access. In group I, 2/4 had left ventricular hypertrophy, eye and CNS changes. MAG-3 renal scan (RSC) and renal ultrasound (RUS) were normal in 2/4 group I patients. In group I, 2/4 EBCT showed unilateral renal artery stenosis: one of these children had decreased velocity in one kidney by RUS and the other child had possible renal artery stenosis by RSC. One child in group I with abnormal RUS suggestive of RAS did not have RAS by EBCT and traditional angiography. In group II, patient with transplant, EBCT showed normal renal vasculature and ureterovesicle anastomoses without RAS or leak respectively. In group III, the SLE patient had thrombosis of the inferior vena cava and renal veins and the other patient had patent A-VF with dilated vessels contrary to US finding of thrombosis. In conclusion, EBCT with its non invasive, maximum intensity projection and 3D images, less radiation and acquisition time is a useful adjunct test to detect RAS and AV-F, AV-G and transplant vascular thrombosis/stenosis in children.

We describe an 11-year-old girl investigated after spontaneous thrombosis of a venous angioma. The case is discussed in relation to the diagnostic difficulties and rarity of the event. In the presence of headache with nausea and left hemiparesis, two CT investigations were done with and without contrast administration. The scans disclosed an intraparenchymal haematoma with nodular enhancement from a deep location to the surface, suggesting a possible vascular origin. MR scans disclosed thrombosis of the drainage vein of a vascular abnormality. Angiography confirmed the finding of thrombosis of a venous collector in a venous angioma.

Thrombosis of the deep venous drainage of the brain in adults. Analysis of seven cases with review of the literature.

To characterize the clinical, radiographic, and pathologic findings in thrombosis of the deep venous drainage of the brain. To highlight clinical and radiographic findings that may lead to the diagnosis of disease and distinguish it from dural sinus thrombosis. To review the published literature on this disorder. Retrospective review of the medical and radiographic records of seven patients from three institutions over the past 10 years. Review of the English language literature from 1971 to the present. All seven patients had risk factors for cerebral venous thrombosis. Five patients presented with a short, rapidly progressing course characterized by headache, nausea and vomiting, and decline in level of consciousness. All five patients died or were rendered severely disabled. Computed tomographic scans, magnetic resonance imaging, and magnetic resonance angiography showed findings associated with deep cerebral vein thrombosis in three of four, in five of five, and in three of three patients, respectively. Transfemoral catheter angiography was diagnostic in two of two patients. Twenty-one reported cases of deep cerebral venous thrombosis were identified in the literature. When the two populations are combined and compared with large series of patients with dural sinus thrombosis, patients with deep venous system thrombosis are more commonly women, tend to present with a more rapidly declining time course, altered consciousness, and long tract signs. Death or long-term sequelae are far more likely to occur in internal cerebral vein thrombosis than with dural sinus thrombosis. Unenhanced computed tomography can demonstrate findings that are strongly suggestive of the diagnosis. Magnetic resonance imaging and magnetic resonance angiography are confirmatory. Angiography may still be necessary when the diagnosis is not clear.

Review of computed tomography findings in thrombosis of the major abdominal venous pathways

Computed tomography is commonly the first diagnostic modality to detect venous thrombosis. The relatively constant location and anatomic relations of the major abdominal veins allow for their proper recognition and differentiation from other tubular structures. Whether the occlusion is secondary to blood clot or tumor extension, the vessels demonstrate a characteristic appearance. The presence of adjacent tumor mass is the single most important factor to differentiate the two types of thrombi. Invaluable aid is provided by computed tomography in the management of this clinical situation. Uncommonly, the diagnosis remains unclear and venography must be done. Selected examples of major abdominal venous thrombosis are presented.
