A Rare Presentation of Breast Filariasis: A Case Report

Background: Filariasis is a common tropical disease but filariasis in breast is uncommon. Mostly, presentation is with typical symptoms of inflammation and lump. Atypical presentation is very rare and needs to be reported. Case Presentation: We report a case of breast filariasis with its atypical presentation and challenges faced for diagnosis. Patient was successfully treated with DEC (Di-ethyl-carbamazine). Conclusion: A high index of suspicion is required for its diagnosis in endemic zones even with atypical presentation, only then can correct treatment for the disease be given and prevention in progression and spread of disease achieved. This case report urges clinicians to keep a fair degree of clinical suspicion for parasitic infections as a cause of breast lump, especially in endemic countries and when presentation is atypical.

BackgroundFilariasis is a tropical vector-borne disease seen predominantly in endemic areas, and the most common presentation is a lymphatic disorder in the form of lymphangiectasia. Filariasis is a tropical disease that, despite innumerable geopolitical measures, continues to be rampant in India. Breast filariasis, although uncommon, is not unknown. It can masquerade as breast carcinoma or breast abscess, with consequent delay in diagnosis. Real-time ultrasonography is a noninvasive method that can help with the diagnosis, as the adult worm shows classical rigorous body movements known as the ‘filarial dance’.

Microfilaria in breast masquerading as a breast lump: A report of two cases

Filariasis is one of the common health problems in India. Extranodal filariasis occurs rarely, and the breast is one of the uncommon sites. Here, we report two cases of breast filariasis in the middle-aged females presented with a painless lump in the left breast. These cases were clinically mistaken as a malignancy. Cytology smears showed numerous microfilariae in both coiled and uncoiled forms along with benign ductal cells. Thus, the diagnoses of filariasis breast probably arising from intramammary lymph nodes were made in both cases. The patients were treated conservatively, and the recovery was uneventful. Thus, fine-needle aspiration cytology played an important role to avoid unnecessary surgical intervention in these patients.

Breast filariasis presenting as breast abscess: a rare entity from an endemic region

A thirty two year female presented with painless progressive lump in right breast with intermittent high grade fever for 10 days. No other symptoms were present. She was non diabetic, non-hypertensive with insignificant family and menstrual history. On examination there was a tender freely mobile lump of approximately 5×4 cm occupying lower inner quadrant having firm consistency and a centre fluctuant area of 1 cm, the overlying skin was erythematous. A single non tender mobile lymph node was present in the right axilla. Contralateral breast was clinically normal. High resolution sonography revealed cystic lesion from 3-6 O’ clock position and axillary lymphadenopathy. The fine needle aspiration cytology showed microfilariae. Hence, a diagnosis of breast filariasis was made. Fluid aspirated from the cavity came out to be sterile. Patient improved symptomatically on di-ethyl-carbazine citrate (DEC) therapy (6 mg/kg for 2 weeks) and the lump too regressed in size.

Filariasis is a benign parasitic disease that is unusual in western countries, especially filariasis of the breast. However, cases of breast filariasis are increasing in our environment due to greater immigration and tourism to endemic areas. We report a case of breast filariasis to describe the diagnostic and therapeutic management of this disease.

Authors present a case of breast lump from an endemic region which turned out to be a filarial lump on histopathology report. Typical clinical manifestations include a subclinical syndrome, Acute adenolymphangitis (ADL), Filarial fever (characterized by fever without associated adenitis) and Tropical pulmonary eosinophilia (TPE). Unusual manifestations include a subcutaneous nodule, filariasis of breast, lymphadenovarix in the axilla and others. Massive hemorrhagic pericardial effusion due to Filariasis is rare. Breast Filariasis - A Fine Needle Aspiration Cytology Report

Abstract Filariasis is a major health problem in tropical countries. The disease is endemic in large areas of India. Lymphatic filariasis in human is commonly caused by Wuchereria bancrofti and Brugia malayi . Extranodal filariasis is a rare entity and the breast is an uncommon site for filariasis. We report a case of female who presented with lump in the left breast. Fine needle aspiration cytology of lump revealed numerous adult filarial worms.

Filariasis of breast: An unusual presentation

Lymphatic filariasis involving the female breast is unusual but not rare, especially where filariasis is endemic. In this report, we present a 38-year-old married woman presenting with a painless mass in the left breast of 4 months duration and slightly painful masses in both axillae for similar duration. Examination revealed a small, discrete, firm, non-tender mass in lower inner quadrant of left breast with associated bilateral, tender axillary lymphadenopathy. Mammography suggested a well-defined mass lesion in the left breast without any architectural distortion or micro calcifications. Ultrasonography (USG) of the breast suggested hypoechoic lesion in infero-medial quadrant of left breast and bilateral axillary lymphadenopathy. USG-guided fine needle aspiration cytology (FNAC) from the breast lump showed non-specific findings like eosinophilic infiltrate and occasional granuloma, while that from the enlarged lymph nodes in the axilla revealed inflammatory cells, giant cells, and epithelioid cells. Histopathological examination of the lumpectomy specimen revealed adult microfilaria with characteristic morphology of Wuchereria bancrofti.

Breast filariasis: Real‐time sonographic imaging of the filarial dance

A 50-year-old woman presented with a palpable tender nodule in the upper quadrant of the breast that was clinically thought to be a fibroadenoma, with mammographic findings of a well-defined lobulated density. Sonographic examination revealed a "filarial dance sign" within the cystic lesion, which is diagnostic of lymphatic breast filariasis. In endemic areas, sonographic examination is recommended to search for this pathognomonic real-time diagnostic feature and, if present, to initiate treatment without delay.

Over 120 million have already been affected by it and over 40 million of them are seriously incapacitated or disfigured by the disease. One third of the people infected with the disease live in India, one third are in Africa and most of the remainder in South Asia, the Pacific and the Americas [1]. Lymphatic filariasis in humans is commonly caused by Wuchereria bancrofti and Brugia malayi. The breast is an uncommon site of involvement. Breast filariasis has been reported in the Indian subcontinent where the organism is endemic. CASE REPORT A 55 year old woman presented with pain and swelling of five months duration in her lef t breast. An ill defined lump was palpable in the central part of the breast. No lymph nodes were palpable in either axilla. She was referred to us for X-Ray mammography and ultrasound of the breast. Mammography was done and standard craniocaudal and mediolateral oblique views were obtained (GE Senographe 700 T, France). Tortuous band like op acity was noted in the central and upper outer part of the left breast (Fig. 1 ­a and b). No calcifications were seen. Ultrasound was done subsequently , using a high frequency transducer of 11-14 MHz (GE Voluson 730 Expert, Austria). It showed cystic lesions in the central and outer part of the left breast (Fig. 2). Within these cysts, vigorously moving clusters of small echogenic structures were seen (Figs.3 and 4). These movements are characteristic of live adult worms of W. bancrofti and a provisional diagnosis of breast filariasis was made. Fine needle aspiration was done and cytologic examination confirmed the diagnosis by showing the presence of adult worms of W . bancrof ti.

Filariasis of the Breast in a Pregnant Woman Diagnosed by Fine‐needle Aspiration Cytology: A Case Report

Background: Despite increased immigration to the United States from endemic areas, the diagnosis of microfilariasis in this country remains infrequent. This disease may occasionally present as a breast mass, in the absence of other clinical findings. Case: We report an unusual case of Wuchereria bancrofti diagnosed in a pregnant woman by breast fine-needle aspiration (FNA) and discuss the clinical implications of filariasis in pregnancy. Conclusion: FNA is safe and reliable in pregnancy. Infants of mothers with breast filariasis should be monitored.

Breast filariasis diagnosed by needle aspiration cytology

A case of nodular lesion of the breast in a 37 year old female caused by filariasis is described. The case is instructive since the diagnosis was made by fine-needle aspiration cytology.
