Atypical cytological presentation of granular cell tumour: Tumour-associated fibrosis may affect fine-needle aspiration cytology accuracy.

The cytological features of granular cell tumour (GCT) are generally quite typical but, in some cases, the fine needle aspiration cytology (FNAC) diagnosis of GCT may be difficult or impossible because of unusual sites of onset or equivocal cytological features. In this report, two GCTs with atypical FNAC features are described in order to investigate the causes and provide possible diagnostic tips. From a series of nine histologically proven GCTs, two inconclusive FNAC cases were retrieved. Smears were poorly cellular showing isolated naked nuclei, anisonucleosis, granular chromatin and occasional small nucleoli. The background was finely granular in one case. Histological controls of these cases revealed marked fibrosis. Tumour-associated fibrosis in GCT is variable and does not seem to influence clinical behaviour but it influences the harvest and the integrity of granular cells collected by FNAC. When GCT smears are poorly cellular, attention should be paid to the granular background and to the few granular cells, if any, as they might be the only features to suggest a GCT.

Granular cell tumor of the breast: correlations between imaging and pathology findings.

ObjectiveTo review the imaging features of granular cell tumors of the breast (on mammography, ultrasound, and magnetic resonance imaging), establishing a pathological correlation, in order to familiarize radiologists with this entity and make them aware of the differential diagnoses, other than malignancy, of lesions with spiculated margins.Materials and MethodsWe reviewed the medical records (from a clinical-pathology database and picture archiving and communication system) of five patients with a pathologically confirmed diagnosis of granular cell tumor of the breast, treated at the Portuguese Oncology Institute of Lisbon, in the city of Lisbon, Portugal, between January 2012 and December 2018.ResultsAll five tumors exhibited imaging features highly suggestive of malignancy (BI-RADS 5 lesions), namely spiculated margins, significant depth, and posterior acoustic shadowing (on ultrasound). One tumor showed a kinetic curve indicative of washout on magnetic resonance imaging, two were adherent to the pectoralis muscle, and one was accompanied by skin retraction. Pathology provided the definitive diagnosis in all cases.ConclusionGranular cell tumors of the breast pose a diagnostic challenge because they can present with clinical and imaging features mimicking malignancy, and the diagnosis is therefore provided by pathology. Radiologists should be familiarized with this entity, so they can be aware of the fact that breast lesions with spiculated margins can be indicative of diagnoses other than malignancy.

Clinical and imaging features of granular cell tumor of the neurohypophysis: A retrospective analysis.

The aim of this study was to explore x-ray computed tomography (CT) and magnetic resonance imaging (MRI) characteristics specific to granular cell tumors (GCTs) of the neurohypophysis.The clinical and imaging data of 6 patients with operation-proven GCTs of the neurohypophysis were analyzed retrospectively.There were 6 patients with GCTs of the neurohypophysis confirmed pathologically. Five of the tumors were purely suprasellar lesions distinct from the intrasellar pituitary gland. One tumor was both intra- and suprasellar. Solid tumors were hyperattenuated and enhanced homogeneously on CT, while showing isointensity on T1WI and heterogeneous hypointensity on T2WI, with homogeneous (4/5) or inhomogeneous (1/5) enhancement. The cystic tumor showed hypointensity on T1WI and hyperintensity on T2WI, with ring enhancement. “Star-like crack sign” was seen in 2 larger solid cases. Calcification was absent in all cases.Well-circumscribed intra and/or suprasellar masses, hyperattenuated and homogeneously enhancing on CT, hypointensity on T2WI, rarely seen calcification, should arouse suspicion of GCTs of the neurohypophysis.

The granular cell tumor is an uncommon benign neoplasm with a strong predilection for the oral cavity, with a greater number of cases affecting the tongue. It typically presents as a solitary nodule, asymptomatic, sessile base, with slow growth (months or years) and variable color. Despite its uncertain etiology, studies suggest a neural origin for the lesion. The aim of this article is to present two clinical cases whose main complaint was a submucosal nodules in the dorsum of the tongue, with slow evolution and healthy surface. In both cases, definitive diagnosis was made after histological and immunohistochemical analysis and the treatment was made with conservative surgical excision. It is also discussed clinical, histological and immunohistochemical features of granular cell tumor.

Canine eyelid granular cell tumor: a report of eight cases

To describe a previously unreported neoplasm of the medial canthus and eyelid in dogs. Clinical and pathologic features of granular cell tumors in the dog were reviewed. Granular cell tumors, arising from the medial canthal eyelid of eight dogs, were identified from the archives of the Comparative Ocular Pathology Laboratory of Wisconsin (COPLOW). The affected dogs ranged in age from 5 to 12 years (mean of 9.25 years). Follow-up information was available for seven of the eight cases. The clinical presentation included swollen hyperemic lids (4/8), ulcerated skin overlying the mass (2/8), and red conjunctiva (7/8). All eight of the cases had firm masses extending from the palpebral conjunctiva to the eyelid margin at the medial canthus. Histologically, the tissue was composed of a highly collagenous neoplastic growth. The neoplastic cells were oval to strap-like cells with an oval bland appearing nucleus and abundant amounts of granular cytoplasm with very distinct cell boundaries. These granular cells were embedded in a dense collagen matrix. A PAS stain faintly highlighted the granular appearance of the neoplastic cells, which is a defining characteristic of this tumor. There was no recurrence in the seven cases available for follow-up. Canine granular cell tumors of the medial canthus present clinically and histologically as a benign neoplasm. Granular cell tumors have a characteristic histological appearance. Granular cell tumors should be on the differential list for nodules of the medial canthus in dogs.

Endosonographic features of esophageal granular cell tumors using a high-frequency catheter probe

Objective. Submucosal tumors (SMTs) are occasionally found in the esophagus during upper endoscopy. Granular cell tumors (GCTs) are reported to be the second most common esophageal mesenchymal tumors, after leiomyomas. Endoscopic ultrasonography (EUS) is an effective tool for predicting the histologic characteristics of SMTs by providing an accurate image of the layering structure of the esophagus, but it is hard to differentiate GCTs from submucosal leiomyomas accurately with conventional EUS. The aim of the present study was to characterize the EUS features of GCTs compared with those of submucosal leiomyomas using a high-frequency catheter probe EUS. Material and methods. A total of 41 patients with GCTs or submucosal leiomyomas were included. All of the patients underwent EUS before histologic confirmation by endoscopic resection or biopsy. Results. There were 14 GCTs in 12 patients and 30 leiomyomas in 29 patients. GCTs had a white-to-yellow surface color more frequently than leiomyomas. In comparison with the surrounding normal proper muscle layer, the echogenicity of the leiomyomas was similar to that of the surrounding muscle layer, but more than half of the GCTs were hyperechoic compared to the surrounding muscle layer. Unclear borders were observed more frequently in GCTs than in leiomyomas. The presence of at least two of these three features in a given tumor had a sensitivity of 85.7%, a specificity of 96.7%, and an accuracy of 93.2% for predicting GCTs. Conclusions. High-frequency probe EUS is helpful for differentiating esophageal GCTs from submucosal leiomyomas.

Characterization of Sonographic and Mammographic Features of Granular Cell Tumors of the Breast and Estimation of Their Incidence

The purpose of this series was to review the spectrum of sonographic and mammographic features of granular cell tumors (GCTs) of the breast and to compare their frequency relative to breast carcinoma. Ten cases of a breast GCT diagnosed during the last 13 years were analyzed for the imaging characteristics and clinical features. Sonographic images were reviewed for location, size, echogenicity, margins, height-width ratio, and sound transmission. Mammograms were reviewed for tumor size, location, margin characteristics, spiculations, and calcifications. The frequency of GCTs was compared with that of breast carcinoma during the same study period. Of 10 GCT cases, 9 tumors occurred in female patients, and 1 occurred in a male patient. The mean patient age was 51.8 years, and the mean lesion size was 1.57 cm. All 7 tumors visualized on sonography were hypoechoic masses. Posterior enhancement was noted in 3 of 7 cases, and posterior shadowing was noted in 2 of 7 cases. Two of 7 did not show any posterior enhancement or shadowing. Two of 7 masses were taller than wide. Of 8 tumors visualized on mammography, 5 were spiculated, and 3 were well circumscribed. Calcifications were not visualized in any tumor. During the same period, 1482 cases of breast carcinoma were diagnosed, making the frequency of GCTs of the breast about 6.7 per thousand breast carcinomas. Granular cell tumors of the breast are rare neoplasms with a relative frequency of 6.7 per thousand breast carcinomas in our series, which was higher than reported in literature. Spiculations are a common imaging feature and mimic carcinoma when present.

Tumoral Fibrosis and Lack of Circumscription in Granular Cell Tumor of the Breast

Granular cell tumors are benign neoplasms of putative Schwann cell origin that most commonly occur in the tongue. While granular cell tumors in the breast are uncommon, the occurrence of this tumor at this site warrants special consideration, since the mammographic, gross, and microscopic appearance can mimic carcinoma. We describe the case of a 61-year-old white woman with a granular cell tumor of the left breast. Our case and a review of the international literature show that desmoplasia and lack of circumscription are common features of granular cell tumor of the breast.

Two interesting epulis, a congenital epulis (granular cell tumor) and extremely enlarged ossifying epulis were reported.The first case was a congenital epulis found in a newborn. Firm, slightly reddish mass (18×11×10 mm) had formed on the lower gums. The lump-shaped tumor was extirpated under the diagnosis of congenital epulis. The histopathologic findings revealed typical features of granular cell tumor composed of large polygonal cells possessing eosinophilic cytoplasmic granules and polarized nuclei. Immunohistochemically, proteins such as carcinoembriogenic antigen, a-fetoprotein, vimentin, desmin, lysozyme and keratins were fully negative but positive for S-100 protein stain. From the results above, this tumor was mostly suspected to be neurogcnic in its origin.The second case was a extremely giant ossifying epulis. A peclunculted large and strongly firm, slowly growing painless tumor was found on the left side of the upper jaw of a 33-year female. The hard tumor had enlarged beyond the teeth to reach the median line on the palate. The extirpated tumor was composed of proliferating dense fibrous tissue and abundant ossifying spicules with mild cell infiltration. Fibroblasts seemed young with large hyperchromatic nuclei, but no osteo-blast could be observed. The histopathologic features indicated that osteofibrous epulis a type of ossifying epulis, was most suitable for this tumor.Adding with the report of two cases above, some clinical and histopathologic characteristics of epulis were discussed with guidance of the published studies.

Granular cell tumor of the ascending colon: A case report

A granular cell tumor (granular cell myoblastoma) of the ascending colon in a 48-year old male is reported. The tumor was detected by barium enema study as a sessile polyp, and colonofiberscopy revealed submucosal tumor. It was removed by endoscopic polypectomy. Macroscopic examination showed the characteristic features of granular cell tumor. The avidin-biotin-peroxidase complex (ABC) method for detection of S-100 protein demonstrated that the cytoplasm of tumor cells and the pleomorphic nuclei were strongly stained with anti-S-100 protein serum, which supports the concept of the Schwann cell origin of granular cell tumor.
