Facial fat necrosis after autologous fat transfer possibly associated with SARS-CoV-2 vaccine.

A 52-year-old female patient developed facial fat necrosis presenting with cutaneous induration three weeks after minimal access cranial suspension (MACS) lift with autologous fat grafting from the abdomen. Given that the patient received the Moderna SARS-CoV-2 vaccine one week after surgery, we hypothesize that the former predisposed her to tissue ischemia leading to fat necrosis. Histological findings after biopsy were consistent with fat necrosis, which included marked dermal fibrosis with areas of focal fat necrosis, lipophages, multinucleated giant cells, and siderophages. It is our hope that documenting this rare development in literature may serve as encouragement for adverse effect reporting after the SARS-CoV-2 vaccine administration and may boost inspection and monitoring of other health consequences by regulating agencies.

Subcutaneous fat necrosis (SCFN) is a rare fat tissue inflammation of the newborn that generally occurs in term or postterm newborns exposed to perinatal stressing factors such as cord prolapse, perinatal asphyxia, therapeutic hypothermia, meconium aspiration, and sepsis. Although most cases of SCFN resolve spontaneously, SCFN may be associated with symptomatic hypercalcemia presenting as lethargy, hypotonia, irritability, vomiting, polyuria, polydipsia, constipation, and dehydration. We describe a case of newborn affected with the disease, its presentation, clinical course, and treatment.

Severe Unusual Presentation of Fat Necrosis in Neonatal Age: A Case Report

Fat necrosis in newborns is estimated to be a rare and self-healing (limited) condition that affects full term or post term newborns in the first few weeks of life. The etiology is not clear and even then this disorder is linked with hypothermia, thrombocytopenia, anemia, gestational diabetes, neonatal hypoxia, and obstetric trauma, preeclampsia and maternal exposure to cocaine or calcium channel blockers usage at the time of pregnancy. Hypercalcemia is considered the most serious, albeit rare, complication appearing a maximum six months after skin lesions.

Pericardial Fat Necrosis Presenting as Acute Pleuritic Chest Pain

A 58-year-old man presented to the Emergency Department with a 4-day history of acute and progressive left pleuritic chest pain. The chest pain appeared suddenly, awakening the patient in the middle of the night. There was no history of trauma or associated dyspnea. Pain was not entirely relieved by non-steroidal anti-inflammatory drugs (NSAIDS). Prior medical history was relevant for diabetes, hypertension, and dyslipidemia. There were no risk factors for pulmonary embolus. On examination, the patient was cooperative and alert. Vitals signs were normal. Physical examination revealed decreased air entry in the left lower lung. Calves were non-tender and not swollen. The initial chest X-ray study showed a rise of the left hemidiaphragm and small left pleural effusion (Figure 1). Laboratory values were normal, with negative cardiac enzyme levels and D-dimers. The electrocardiogram (ECG) showed a normal sinus rhythm with a right bundle branch block, the exact same result as an ECG done 5 years prior (Figure 2). The patient’s pain was relieved by morphine. Probability for pulmonary embolus was intermediate at ventilation-perfusion scintigraphy, which demonstrated a ventilation-perfusion mismatch in the lateral basal segment of the left lower lobe. Intravenous heparin was started. Due to the result of lung scintigraphy, a pulmonary computed tomography (CT) angiography was requested. CT angiography did not show any signs of pulmonary embolism, although distal vascular evaluation at both lung bases was limited due to respiratory artifacts.

Axillary lump: an unusual presentation of fat necrosis in the breast

The clinical presentation of an axillary lump, in majority of cases, raises suspicion of an enlarged lymph node due to malignant causes. In this case report, we established a diagnosis of an axillary lump caused by fat necrosis. We present this case report with review of the literature to familiarize clinicians with this condition.

Ultrasound appearance of scrotal fat necrosis in prepubertal boys

Scrotal fat necrosis is a rare cause of scrotal pain in prepubertal boys. Despite its characteristic clinical presentation, it is an under-recognized entity. Specific scrotal US imaging features of this condition have not been previously reported. To describe the US imaging features of scrotal fat necrosis. Between January 1999 and November 2006 we identified all boys with idiopathic scrotal fat necrosis presenting for ultrasound imaging. All the children were reviewed by the urology service during the initial presentation and assessment with clinical confirmation of the diagnosis of scrotal fat necrosis. The sonographic appearances in each child were reviewed. Over the course of 6 years we identified ten children who had the classic clinical diagnosis of idiopathic scrotal fat necrosis. The scrotal fat was characteristically hyperechoic, with posterior shadowing. This was located inferior to the testes which were displaced superiorly. The testes and epididymi were normal in all children. There are specific imaging features of scrotal fat necrosis that when combined with the characteristic clinical presentation can confirm the diagnosis of scrotal fat necrosis.

Subcutaneous fat necrosis presenting as a large mass

Subcutaneous fat necrosis, though a rare phenomenon, can occur in patients of all ages. In newborns, it is an uncommon, self-limited panniculitis of traumatic origin, while in adults it is most commonly encountered among women presenting to breast clinics following a trivial or unnoticed injury sustained long ago. An acute presentation of fat necrosis as a large mass, however, is quite unusual and that may confuse the observer. History of trauma gives a clue to the diagnosis. We report an interesting case of traumatic subcutaneous fat necrosis because of massage therapy in a 66-year-old lady.

Background: Cutaneous pancreatic fat necrosis is a pathognomonic sign for pancreatic disease and usually presents as subcutaneous nodules in the pretibial region. Objective: A case of cutaneous pancreatic fat necrosis is presented in which the clinical presentation of diffuse erythema was unusual. This disease is discussed and its possible etiologies are reviewed. Methods: A MEDLINE search for cases of cutaneous pancreatic fat necrosis presenting as diffuse erythema without nodules was conducted. Results: Diffuse erythema is an unusual presentation of cutaneous pancreatic fat necrosis. Conclusion: This may be the first case of cutaneous pancreatic fat necrosis presenting as diffuse erythema.

Bilateral Breast Masses in a Man

Other than gynecomastia, bilateral breast masses in a male patient is an unusual occurrence. We present a case of fat necrosis presenting in a male veteran as bilateral palpable breast masses.

We report an unusual case in which the coalescence of fat necrosis foci produced a palpable abdominal mass. Hence, focal fat necrosis should be included in the differential diagnosis of abdominal masses in patients with a history of pancreatitis.

Fat necrosis of the breast: characteristic xeromammographic appearance.

Eight cases of fat necrosis of the breast were examined by xeromammography and confirmed pathologically. They showed a spectrum of mammographic appearances, from mimicking other benign diseases to suggesting a malignant lesion. Three of the eight cases showed the benign ring-like calcification associated with fat necrosis; this is a higher percentage than reported in previous series. In our experience this radiological presentation of fat necrosis is not infrequent and biopsy of these lesions is not necessary unless other clinical or radiological signs suggest malignancy.
