Family Therapy for Schizophrenia: Co‐Occurring Psychotic and Substance Use Disorders

People with schizophrenia have a high prevalence of co-occurring substance use disorders, which is related to a worse course of psychiatric illness, more frequent relapses, and increased depression and suicide, compared with those with schizophrenia alone. The Family Intervention for Dual Diagnosis (FIDD) program, which includes psychoeducation and goal setting and focuses on building communication and problem-solving skills within the family, can aid in decreasing the stress related to having a close relationship with someone with a dual disorder. Here, the case of a young man with psychosis and marijuana dependence is examined. This article describes how the FIDD intervention helped him with his problematic substance use, as well as to build skills within his relationship for increased empathy and reduced interpersonal stress.

Family therapy for schizophrenia: cultural challenges and implementation barriers in the South African context

Family therapy is an effective, evidence based intervention for schizophrenia. This literature review explores the impact of culture on family therapy as a treatment model for schizophrenia and examines how cultural beliefs impact on access to care. Although there is a good deal of evidence to suggest that certain principles of family therapy such as empathy and psycho-education are universal, there is a paucity of literature about the role of culture in designing family interventions for people living with schizophrenia in a culturally diverse setting such as South Africa. It is well acknowledged that cultural ideologies influence families' belief systems of schizophrenia, expected expressed emotion, and levels of stigma in relation to mental illness. Additionally, in adapting models designed for first-world settings, consideration needs to be given to aspects such as language, educational level and accessibility of mental health care facilities. Family therapists are increasingly recognising the need for the study and implementation of evidence based culture-relevant and culture-responsive therapeutic techniques. These techniques need to be cost-effective and will require training, supervision, staff support, and management input in order to become generally available.

Family Therapy for Schizophrenia in the South African Context: Challenges and Pathways to Implementation

Schizophrenia is a chronic psychiatric disorder that affects 1% of the world's population. Family interventions whereby the patients and their relatives can attend psychiatric therapy sessions are effective at preventing relapse of symptoms. In a country such as South Africa where there has been a shift from institutionalisation to community care, family therapy treatment models are an important option to explore. Although there is a paucity of research on family therapy for schizophrenia in the South African context, we found a number of studies conducted in both developed and developing countries. Problems with adherence to medication, lack of psychoeducation and low expressed emotion (EE) were identified as challenges to effective family therapy models. A country such as South Africa has additional challenges of stigmatisation of mental illness as well as cultural perceptions of illness that may influence barriers to mental health care. These should be considered when designing family therapy interventions. We suggest further research endeavour to explore the applicability of family therapy models for people living with schizophrenia in South Africa. EE in relation to different cultural groups in South Africa should be considered.

Systemic Family Therapy in Schizophrenia: A Randomized Clinical Trial of Effectiveness

Background: Although evidence suggests the effectiveness of cognitive and behaviorally oriented family therapies for schizophrenia, no specific randomized study has been carried out on Systemic Family Therapy (SFT). The purpose of this longitudinal prospective study was to evaluate the clinical effectiveness of SFT in the treatment of patients suffering from schizophrenia, as compared with a control case sample composed of patients undergoing routine psychiatric treatment. The study included a 2-year follow-up period and was conducted following the Milan School model. Methods: The case sample in the study was made up of 20 patients treated with SFT associated with routine psychiatric treatment, and 20 patients undergoing routine psychiatric treatment alone. Both groups of patients were re-evaluated at the end of treatment (T1) and 12 months after the end of treatment (T2) to assess the clinical course of the illness. Results: At the end of the follow-up period, we noticed an improved clinical course and a better pharmacological compliance in the group of patients treated with SFT. In the experimental case sample, 3 subjects (15%) had relapsed, compared to 13 patients (65%) in the control group (p = 0.03). No significant difference was found in the 2nd year of follow-up. Conclusions: Our study provides an innovative contribution with regard to the evaluation of the clinical effectiveness of SFT in the treatment of schizophrenic patients. The clinical results were significant, and we thus feel justified in continuing to use this type of therapy in integrated interventions involving schizophrenic patients.

Long-Term Outcome of Family Therapy in Schizophrenia

Introduction: Cognitive-behavioral family interventions in schizophrenia have proven to be effective in reducing relapse and readmission rates, and also appear to offer other benefits to patients and their families, at least in the short-term. Of particular interest to mental health services is ascertaining how long family interventions should last to maintain their benefits over the course of time. Objective: To determine whether or not the benefits of a family intervention in schizophrenia, conducted in the clinical practice framework, are sustainable over a five-year period. Method: A follow-up over a five-year period with a representative sample of patients and their families who, five years ago, participated in a twelve-month long, cognitivebehavioral family intervention. Results: 53.5% of patients had at least one relapse, and 16.9% followed a continuous course. Family therapy of schizophrenia.

Family therapy of schizophrenia has long been conceived and practised under etiological premises. Familial disturbances as pathological regression/fixation (psychoanalytical) and individuation-impairing family dynamics (systemic) were addressed directly in the hope of "curing" the disorder. The efforts to prove the viability of the concepts and/or the efficacy of the therapeutic approach were largely unsuccessful. Newer strategies of family therapy of schizophrenia are both more precise in their theoretical assumptions and more performing in the pursuit of their therapeutic goals. We analyse the basis of modern family therapy in the "Expressed-Emotions (EE)"--research and propose a newer, more adequate understanding of the EE phenomenon. From our own studies and from a general review of relevant studies we derive an understanding of the rationale of family work and family therapy of schizophrenia. We discuss the results of a meta-analysis on the active ingredients and the conditions of efficacy of family interventions.

Collaboration with families for the rehabilitation of schizophrenic patients and the concept of expressed emotion.

Recent interest in community care of schizophrenic patients has led to publication of a number of studies that deal with patients' rehabilitation in their families. Joint efforts of psychologists, psychiatrists and social workers have produced a number of techniques for collaboration with families of schizophrenic patients, including techniques based on the concept of expressed emotion. This concept is presented with reference to its spectrum consistency in time, generalizability to nonfamily setups, relationship to relapse, prevalence in disorders other than schizophrenia and usefulness in comparison to other concepts in family therapy of schizophrenia. Caution is urged in not incriminating the family in causing relapse, after the quiet abandonment of older theories that blamed families for initiating the illness of schizophrenic members.

Family therapy and schizophrenia.

The early concepts of family influences in the development of schizophrenic illness have been reviewed. The empirical research which these concepts generated has failed in large measure to confirm many of these hypotheses, but has demonstrated reliable and valid differences between families of schizophrenics and those of patient and non-patient controls. However, these empirical findings have had little impact to date on family therapy as it is practised, and the failure to confirm theoretical concepts from psychoanalysis, systems, and communication theories has not led to a re-examination of family therapy and its theoretical basis. The author suggests that the empirical research on family influences in schizophrenia and particularly those factors which precipitate and perpetuate schizophrenic symptomatology can be applied to evaluation of schizophrenics and their families, specific family therapy with families of schizophrenics, and particularly suggest that evaluative research on the effectiveness of family therapy in schizophrenia will be necessary for family therapy to become more than a highly fascinating and experimental technique in the total management of schizophrenic patients.
