Pengisian Family Folder Untuk Optimalisasi Kegiatan Home Visit Pasien Diabetes Mellitus Tipe II

The existence of health services that are accessible, good quality, and answer the needs of community can improve public health. One of the programs provided by health center to resolve patient problems is the Home Visit. During the home visit, various accurate and up-to-date data will be obtained regarding the health condition of patients and family members, environmental conditions around the house, and relationships between family members. One of the supporting instruments that can be used for home visit as an effort to optimize health services is the availability of medical records in the form of a family folder. Community service activities carried out in the form of home visits to one patient with Type II Diabetes Mellitus in the working area of the Jeulingke Health Centre. Home visit activities to the preparation of reports are supervised by supervisors from the Family Medicine Department of the Faculty of Medicine, Syiah Kuala University, Banda Aceh. Home visits were carried out twice in one family. The aims are to evaluate treatment, obtain more information about the health condition of patient and family, patient-family relationship, living conditions to the environmental conditions around the patient and family members who live with the patient. Home visit activities are complemented by completing the family assessment tools which consists of a genogram, family life cycle, family map, family APGAR, family SCREEM and family life line.

Epidemiologic correlates of malnutrition among under-three children in the rural community of Northern India.

Worldwide malnutrition is identified as a major health and nutrition problem. Undernutrition contributes to an estimated 45% of child deaths globally. The prevalence of underweight among children in India is among the highest in the world. Our children also bear a tremendous double burden of malnutrition. To find out the prevalence and determinants of malnutrition among six months to three-year-old children in the rural community of Northern India. A community-based cross-sectional study conducted among children aged six months to three years in a rural area of Ludhiana district, Methods and Material: A total of 662 children in the age group of six months to three years from a population of 30,000 were identified and included in the study. All the relevant information regarding these children was collected from family folders. Socioeconomic status was assessed using the modified Udai Pareek scale (MUP). The data collected was entered in MS Excel and was analyzed using SPSS version 26 and WHO Anthro Survey Analyzer. Out of 662 children, 16% were underweight. Almost 50% of the children in the two-three years category were underweight. The prevalence of stunting in the study population was 20.7% and that of overweight was 4.8%. The prevalence of underweight was higher in children of low socioeconomic status than in children from upper socioeconomic status (P = 0.000). There was a significant association between birth order and increasing cases of underweight (P = 0.000). The causes of malnutrition in children are complex and involve multiple factors. There is a need for vigorous monitoring for early detection of malnutrition for children aged two-three years. The improvement of maternal education will improve the nutritional status of the child.

Early Postnatal Care Utilization and Associated Factors Among Women Who Give Birth in the Last Six Weeks in Hosanna Town, Southern Ethiopia, 2022.

Background: The early postnatal period is defined as the first 48 h to 7 days after delivery. The early postnatal visit is especially the most critical time for the survival of mothers and newborns, particularly through early detection and management of postpartum complications. Despite the benefits, most mothers and newborns do not receive early postnatal care services from healthcare providers during the critical first few days after delivery. Objectives: This study is aimed at assessing the prevalence of early postnatal care utilization and associated factors among mothers who gave birth within the last 6 weeks in Hosanna town, Southern Ethiopia, from April 20 to May 30, 2022. Method: A community-based cross-sectional study was conducted in Hadiya Zone, Hosanna town, Southern Ethiopia. A simple random sample technique was used to recruit 403 mothers who had given birth in the previous 6 weeks from a family folder. Data was collected through face-to-face interviews using a standardized questionnaire. Binary logistic regression was used to assess the association between outcomes and explanatory variables, and the strength of the association was interpreted using an odds ratio with a 95% confidence interval. In our study, p values of 0.05 were considered statistically significant. Results: The prevalence of early postnatal care utilization among mothers who gave birth within 1 week of the study area was 25.8% (95% CI: 21.7-30.0). No formal and primary educational level of husband (AOR = 0.05, 95% CI: [0.02, 0.16]), antenatal care follow-up (AOR = 2.13, 95% CI: [1.11, 4.1]), length of hospital stay before discharge (≥24 h) (AOR = 0.3, 95% CI: [0.16, 0.55]), and information about early postnatal care utilization (AOR = 3.08, 95% CI: [1.72, 5.52]) were factors significantly associated with early postnatal care utilization. Conclusion: In comparison to World Health Organization standards, the study's overall prevalence of early postnatal care utilization was low. Early postnatal care use was significantly associated with antenatal care follow-up, the husband's educational level, knowledge of early postnatal care use, and length of stay at the health institution following birth. As a result, the strength of health facilities is to improve service provision, information education, and communication.

The initial complaint was that it felt like itching accompanied by the appearance of reddish spots, and over time, pimples appeared between the fingers. Complaints felt worse in the last week until the patient felt itchy all day and complaints of itching felt worse at night until the patient had difficulty getting to sleep because he kept scratching the part of the body that felt itchy. Based on the anamnesis of complaints obtained after contact with the patient's older brother who experienced similar complaints. A physical examination was carried out and the examination results were within normal limits. Primary data was obtained through anamnesis and physical examination by conducting home visits, filling in family folders, and filling in patient files. The assessment is carried out based on a holistic diagnosis at the beginning, process and end of the visit quantitatively and qualitatively. Upaya Pengelolaan Pasien Diabetes Mellitus Tipe 2 Dengan Underweight Dalam Pendekatan Kedokteran Keluarga

The patient came with complaints that the body often felt weak even though the patient did not do strenuous activities. This complaint has worsened in the last 1 week, causing the patient to be discouraged from doing activities and disrupting his daily activities Patient also complained that his weight was decreasing. Another complaint that was felt was that the hands often felt numb. History of diabetes mellitus in the last 10 years, but the patient admitted that he did not routinely take medicine to control his blood sugar. The KGDS examination obtained in the patient was 324 mg/dl. The patient's BMI was 18.2 kg/m2 and classified as underweight. Primary data was obtained through autoanamnesa and physical examination by conducting home visits, filling out family folders, and filling out patient files. Assessment was carried out based on holistic diagnosis at the beginning, process, and end of the visit quantitatively and qualitatively. Interventions include education about the causes of diabetes mellitus, education about lifestyle modification and the importance of taking medication regularly to control blood sugar levels and explaining complications that may arise from the patient's disease so that patients make preventive efforts.

A 17-month-old female patient was brought by her family to the emergency room at the Lhoksukon Health Center with complaints of weakness for the last 2 days. Weakness was felt to be burdensome since the patient had a fever on the 5th day. Fever is felt up and down throughout the day. The fever is accompanied by chills, sweating, and restlessness. On the first day of fever, the patient was given fever medicine by the patient's mother at home, but the fever did not subside. The patient also experiences nausea and vomiting. Nausea appears sometimes when eating. Vomiting is in the form of food that is eaten but the vomit volume is not much. The patient's appetite also decreases. Primary data were obtained through alloanamnesa, physical examination, supporting examinations, as well as making home visits, filling out family folders, and filling out patient files. The assessment was carried out based on the initial holistic diagnosis, process and end of the visit both quantitatively and qualitatively. Interventions carried out include education about the causes of typhoid fever to their families, education about hygiene and handling of the disease and explaining complications that may arise from the patient's illness so that they can make efforts to prevent and treat it if it recurs.

Studi Kasus Tatalaksana Skabies Melalui Pendekatan Kedokteran Keluarga Pada Pasien Anak Laki-Laki Usia 10 Tahun

Scabies is a parasitic infectious disease (Sarcoptes scabiei var. hominis) which is still a public health problem in the world, including Indonesia. Scabies can be transmitted through direct and indirect contact with the greatest prevalence in children, densely populated populations, poor sanitation and hygiene. Symptoms and signs of scabies include papules, vesicles, pustules or nodules accompanied by itching at night with a predilection for a thin epidermal layer. This study is a case report that aims to examine the accuracy in diagnosing and providing pharmacological and non-pharmacological (communication, information and education) treatment of scabies patients. Primary data were obtained through anamnesis (by autoanamnesis and alloanamnesis with the patient's family members) and physical examination by conducting home visits, filling out the family folder, and filling out patient files. The assessment was carried out based on the initial holistic diagnosis, process and end of the visit both quantitatively and qualitatively. In the study, a 10-year-old boy patient was found at the Pediatric Polyclinic at the Dewantara Health Center with complaints of itching all over the body, especially between the fingers, since the last 8 months. Complaints of itching worsen at night so that it disturbs the patient's sleep. The patient has two siblings who live in Dayah with the same complaint and occasionally stay at home with the patient. The patient's mother also had the same complaint. On physical examination, HR: 88x/I, RR: 18x/I, temperature: 36.5°C and nodules were found in the left palmar region of the manus, and canaliculi in the form of white straight lines with papular tips. The Effect of Community Health Information System on Health Care Services Utilization in Rural Ethiopia.

In Ethiopia, the community health information system (CHIS) is implemented at the health post (hp) level with the aim of improving service delivery and use. We conducted a national level assessment of CHIS utilization and explored the associations of CHIS utilization with use of antenatal care (ANC), postnatal care (PNC), institutional delivery and child immunization in rural Ethiopia. We conducted a cross-sectional study measuring community-based health service use and HP based CHIS assessment from March to May 2019. Data were collected from 343 HPs and 2,864 women who delivered in the last five years, and multistage sampling was used to select the study subjects. We used descriptive statistics for CHIS implementation and service utilization and multilevel logistic regression to investigate the association of CHIS implementation with maternal and child health care services use. Fifty five percent of the HPs were implementing CHIS. These HPs were using a paper-based household data collection tool called family folder (FF). Of the HPs, one third implemented lot quality assurance sampling (LQAS) based data quality check and 60.4% documented and followed execution of decisions. Overall, among the eligible women, 40% used ANC, close to 50% of currently married women used ANC services; 28% of women that fall in the high wealth index category used PNC within 48 hours after delivery; and 86.1% of women who had at least a high school education delivered at a health facility. Implementation of CHIS and family folder utilization and conducting LQAS based data quality check in the HPs were significantly associated with increased odds of ANC, delivery, and vaccination services use. We found that better implementation of CHIS was associated with better maternal and child health service use which implies that increasing utilization of CHIS at HPs will improve mother and child health service use.

Weakness is felt suddenly throughout the body and feels continuously. Complaints of weakness are also accompanied by a feeling of dizziness in the head. The patient also complained of frequent urination at night, urinating more than 10 times in one night. The patient also constantly feels thirsty, but his appetite has decreased since the last 2 years or so. The patient feels a loss of weight. In addition, the patient also complained of a wound on his left leg which had not healed since 1 year ago. Primary data were obtained through anamnesis and physical examination by conducting home visits, filling out family folders, and filling out patient files. The assessment was carried out based on the initial holistic diagnosis, process and end of the visit both quantitatively and qualitatively. Patients and families are given education about diabetes mellitus including the symptoms and complications that will arise, education about the patient's motivation to take medication regularly as well as educating and teaching patients about foot care and diabetes exercise. An approach with family doctor intervention is able to improve habits that are influential in the treatment of diabetes mellitus.

Perancangan Ulang Map Rekam Medis Di Puskesmas Glugur Darat Medan 2022

Medical record folder is a folder that is used to protect the forms in it to facilitate storage, especially patient medical record forms. The medical record folder at the Glugur Darat Health Center in Medan is currently in the form of a landscape and uses pink cardboard which is easy to tear and does not last long. The purpose of this study was to analyze and design a medical record folder in terms of three aspects, namely the physical aspect of the form, the anatomical aspect of the form, and the aspect of form content at the Glugur Darat Health Center in Medan. This research method uses descriptive, qualitative methods with saturated sampling technique, which was carried out at the Glugur Darat Health Center Medan in July-September 2022. The subjects in this study were Medical Record Officers, Filing Officers and Registration Officers at the Glugur Darat Health Center Medan. The result of this study is the design of a medical record folder at the Glugur Darat Health Center in Medan which is viewed from three aspects, namely the physical aspect of the form, a medical record folder made of thin cardboard is easy to tear and not durable. In the anatomical aspect, the form was found to only contain the name of the Family Folder, the name of the Puskesmas and the logo of the Puskesmas in the Heading section. In the aspect of the contents of the form found in the medical record folder at the Glugur Darat Health Center Medan, there is no loading in the medical record folder with the words Confidential.

Household food insecurity and coping strategies among rural households in Kedida Gamela District, Kembata-Tembaro zone, Southern Ethiopia: mixed-methods concurrent triangulation design

BackgroundHousehold food insecurity is a state in which household members experienced limited or uncertain physical and economic access to safe, plenty, and healthy food to meet the dietary needs for a fruitful, healthy, and active life. Food insecurity continues to be a major development and public health problem across the globe, having adverse consequences. This study was done to assess household food insecurity and to explore coping strategies in Kedida Gamela District, Southern Ethiopia.MethodA cross-sectional study complemented with the qualitative inquiry was carried-out from August to November 2020. Multistage sampling was used to select study subjects. A total sample of 597 households was selected randomly using up to date family folder list in the district as a sampling frame. For the qualitative study, 16 food-insecure households were selected randomly from food in secured households. Quantitative data were entered using Ep-Data 3.1 and exported to SPSS 20 for analysis. Bivariate analysis was carried out to see the crude association between each independent variable and outcome variable. P-value < 0.05 and 95%CI for adjusted odds ratios (AOR) were used to declare the significance of the associations. The qualitative data were analyzed using thematic analysis.ResultThe findings of this study showed that 76% of the households were food insecure. Being female-headed households [AOR: 2.82:CI(1.10, 7.24)], absence of formal education [(AOR: 9.75:CI (3.7, 11.31)], lack of engagement in non-farm farm activities [(AOR; 3.30: CI (1.86, 5.96)], absence of credit service [AOR:4.04; C I (2.11,7.73)], presence of dependent household members [AOR: 3.47;(2.91,6.34)], poorest wealth status [AOR;9.86:CI (3.72, 15.85)] were factors significantly associated with food insecurity of the households. Food insecure households employed different coping strategies with the respective level of food insecurity.ConclusionThe findings of this study indicated that household food insecurity was higher in the study area. Moreover, sex, educational status, wealth status of the households; engagement of households in off/non-farm farm activities, credit service, and active and inactive labor force were significantly associated with household food insecurity.Food insecure households practice different coping strategies with respective food insecurity levels from the less severe strategy of eating inedible, fewer-quality foods to the most severe of migrating and begging for food.Planning and exhaustively implementing sustainable food security programs should get due attention.

TINJAUAN PELAKSANAAN PENGELOLAAN DOKUMEN REKAM MEDIS DI BAGIAN FILING DI PUSKESMAS WONOSARI 1

Introduction: The implementation of the management of medical record documents at the Wonosari 1 Health Center there are still obstacles including in the medical record document storage section there is still a misfile because the medical record document after being used is not directly stored on the storage rack and there is still a buildup of medical record documents outside the medical record document storage rack and not yet there is a tracer.&#x0D; Objectives: The purpose of this study was to determine the implementation of medical record document management in the filing section at the Wonosari 1 Health Center.Methods: This research is a descriptive study with a cross sectional approach. The subjects of this research are filing officers and registration officers. The objects of this research are medical record documents, Standard Operating Procedures (SPO) and filing facilities. Result: The results showed that the implementation of storage and alignment of medical record documents was in accordance with Standard Operating Procedures (SPO), namely family folder storage and centralization, namely outpatient medical record documents, inpatient care, one family emergency room stored in one folder and the alignment based on Straight Numerical filing (SNF).Conclusion: the implementation of storage and alignment of medical record documents still found misfiles. this is because after used documents are not directly stored on the storage rack and the tracer aids are not used at the time of retrieval and return of medical record documents.

PENATA LAKSANAAN PENYIMPANAN REKAM MEDIS DI PUSKESMAS PISANGAN

The implementation of Community Service activities at the Pisangan Health Center aims to assist the management of the health center's medical record storage, especially the shrinkage of medical records, carried out according to plan and running smoothly. This activity was carried out in collaboration with the Pisangan Health Center. Clinics, as healthcare facilities, require organized administration and effective management of medical records to ensure the production of quality information. This community service activity was conducted at Panacea Clinic with the objective of providing counseling to medical record officers on implementing a family folder system for medical record storage. The direct method was employed for this service, involving on-site visits to deliver educational materials and demonstrate proper and accurate medical record storage practices. The outcome of this community service activity was an improvement in the officers' understanding of the family folder storage system. Medical records serve as archives that document all activities within a healthcare facility over a specific period. These records contain information and documents related to patient identity, examinations, treatments, interventions, and other services provided. Medical records play a vital role for both patients and healthcare professionals, necessitating complete and timely record completion by all parties involved.

Management of medical records and health information is one of the elements in the assessment of Public Health Center accreditation. Medical records play an important role in collecting accurate and comprehensive data for targeted health policies. Community service activities at the Cibeureum Health Center are carried out using consultation, training, and mentoring methods. Concordance of Mother-Child (6-23 Months) Dietary Diversity and Its Associated Factors in Kucha District, Gamo Zone, Southern Ethiopia: A Community-Based Cross-Sectional Study.

Meeting minimum standards of dietary quality in mothers and children is a challenge in many developing countries including Ethiopia. Emerging evidence suggests that maternal and child dietary diversity is associated, but little is known about the associated factors of concordance of mother-child dietary diversity in Ethiopia and none is documented in the study area. This study examines the concordance between mother-child (6–23 months) dyads dietary diversity and the associated factors in Kucha District, Gamo Zone, Southern Ethiopia. A community-based cross-sectional study was conducted among 791 mother-child (6–23 months) pairs from 11 selected kebeles on March 6 to April 13, 2017. Multistage cluster sampling technique was used to select the study subjects. The sampling frame was obtained from the family folder of health posts in each kebele. The mother-child pairs were selected by the simple random sampling method. The 7 food groups of the World Health Organization (WHO) for children and the 10 food groups of FANTA/FAO 2016 for mothers were used to analyze the dietary diversity. Cohen's kappa statistics was calculated to see the strength of concordance. The multivariable logistic regression model was fitted to determine factors affecting mother-child dietary diversity concordance. A good concordance was noted between mother-child dietary diversity scores (Kappa = 0.43). Only 56 (7.1%) mothers were negative deviants, and 133 (16.8%) mothers were positive deviants in dietary diversity consumption. Rural residence (AOR = 3.49; 95% CI: 1.90–6.41), having no formal education (AOR = 1.8; 95% CI: 1.08–3.05), not owning milking cow (AOR = 1.7; 95% CI: 1.10–2.56), children with low dietary diversity (AOR = 8.23; 95% CI: 5.17–13.08), and mothers with low dietary diversity (AOR = 0.46; 95% CI: 0.29–0.74) were found to be factors associated with mother-child dietary diversity concordance. An increase in the percentage of children reaching the minimum dietary diversity was greater with a successive increase in maternal dietary diversity. Despite interesting similarities between mothers and children dietary consumption, more than three-quarters of concordants did not achieve the recommended dietary diversity score (were low concordants). Interventions targeting on rural women's access to high school education, home-based milking cow rearing, and promoting nutrition-sensitive agriculture to meet the dietary requirements of mothers and children in a sustainable manner and public health efforts to improve child nutrition may be strengthened by promoting maternal dietary diversity due to its potential effect on the entire family.
