Evaluation of the KTP 532 laser in aesthetic facial surgery

This study investigated the ability of the KTP 532 laser to reduce postoperative swelling and bruising in aesthetic facial surgery. Thirty consecutive patients receiving a laser-assist procedure (following skin incision with a scalpel, dissection and hemostasis were accomplished primarily with the laser) were compared to 16 patients who had similar technical operations but relied on scissor/scalpel dissection and electrocautery for hemostasis. Evaluations were performed by comparison of serial photographs after surgery and by patient self-assessment. The clinical observations demonstrated a noticeable reduction in postoperative swelling, bruising, and discomfort in the patients whose procedure was performed with the aid of the laser.

Combined Otolaryngology Spring Meetings of the American Academy of Facial Plastic and Reconstructive Surgery, 1995

Fifty-five scientific papers were presented at the Academy's spring meeting in Palm Desert, Calif, on May 3 and 4, 1995. The conference was directed by Roger Crumley, MD, and also included four panel discussions and a variety of workshops. A broad spectrum of topics in facial aesthetic surgery was presented. In addition, Jon Turk, MD, presented the recent experience in Croatia of the Face-to-Face team sponsored by the American Academy of Facial Plastic and Reconstructive Surgery. Dr Turk showed photographs of the successful results obtained from reconstruction of difficult congenital craniofacial defects. Brian Deutsch, MD, began the scientific session with a presentation on Secondary Healing of Mohs Defects of the Forehead, Temple, and Eyelids. Hydrogen peroxide and antimicrobial ointment were used. Hypopigmentation and telangiectasias after healing were encountered. Acceptable results were obtained with forehead and temple defects, and excellent results were obtained with eyelid defects. The Role of Orbital Fat Preservation in Facial Aesthetic Surgery: A New Concept

Preservation of lower orbital fat in facial rejuvenation is a new concept for preventing the "operated" appearance that may follow fat removal in conventional blepharoplasty. By creating a narrower, more shallow orbit, the surgeon can create a truly youthful eyelid-cheek complex. Orbital fat advanced over the complete inferior orbital rim hides the bony framework that becomes apparent with normal aging. This technique is used routinely in isolated blepharoplasty and with composite rhytidectomy. It accompanies repositioning of the orbicularis oculi muscle, cheek fat, and facial platysma muscle. It is a procedure that can correct postoperative problems caused by excess orbital fat removal, lower eyelid retraction, and contour problems resulting from malar augmentation. The orbital fat is one of the many deep "pieces" of the human face that must be preserved and repositioned for optimal facial rejuvenation.

Bicoronal forehead lift.

Although aesthetic surgery on the upper third of the face has received relatively little attention in recent literature, it is an extremely important component of facial aesthetic surgery and often yields results that are quite dramatic. Periorbital rejuvenation, either alone or in conjunction with other aesthetic surgical procedures, offers facial enhancement to younger patients. In addition, older patients may benefit from a "forehead lift" performed in conjunction with lower facial surgery. This benefit is derived because lower facial surgery may create facial disharmony by yielding a more youthful lower face while leaving a more mature upper face. In these cases a forehead lift serves to unify the patient's facial appearance. The coronal browlifting technique described here is a personal interpretation and modification of periorbital rejuvenation techniques. This approach represents an ideal aesthetic procedure: It is a technically straightforward operation that yields results which are predictable, natural appearing, and long lasting with minimal morbidity.

The Anatomic Data Sheet in Plastic Surgery

A graphic and written anatomic data sheet (ADS) for facial aesthetic surgery and body contour surgery is presented as a tool for: (1) analyzing surgical maneuvers and their effect on final outcome; (2) recording individual anatomic variations; (3) communicating data between office personnel and colleagues; (4) aiding in the treatment of the postsurgical patient; (5) use as a worksheet for the surgeon; and (6) standardizing, evaluating, and comparing surgical techniques. The ADS takes account of the common aesthetic facial and body contour procedures performed in plastic surgery. A supplemental rubber stamp for use on the office chart, with graphics identical to those of the ADS, identifies areas of treatment concern before and after surgery.

Lower eyelid blepharoplasty.

Lower eyelid blepharoplasty is a frequently performed procedure to reverse partially gravitational aging effects on eyelid tissues. Careful planning, proper execution, and appropriate management of potential postoperative complications will generally result in a happy and satisfied patient. However, in any surgical procedure that is primarily aesthetic in nature, the surgeon needs to be constantly aware of factors that may compromise the functional integrity of the eyelid. A patient with altered visual function or ocular discomfort is no longer primarily concerned about the final aesthetic appearance of even the most perfectly executed, technically correct blepharoplasty procedure. To describe the preoperative evaluation, surgical technique, and postoperative complications of lower eyelid blepharoplasty. Specific attention will be given to lower eyelid retraction and ectropion following blepharoplasty. Successful lower eyelid blepharoplasty surgery can be a source of both patient and physician satisfaction. As long as patient expectations are consistent with realistic surgical goals, lower eyelid blepharoplasty is an important aspect of overall facial aesthetic surgery.

The authors present a series of 74 patients that underwent injections of a biphasic copolymer (Bioplastique) to improve the facial contours or to fill deep creases and folds. The followup period was ten months. This preliminary report could be useful in defining the possibilities of the new product.
