THE ROLE OF ALTERED CAST IMPRESSION TECHNIQUE IN ACHIEVING ACCURATE PROSTHETIC FIT: A CASE REPORT

In dental practice, posterior bilateral edentulous spaces are most commonly encountered situations. Removable prosthesis generally is tooth or tissue borne or in combination. But the distal extension cases obtain their support from both the teeth and tissue adjacent to ensure equal distribution of forces on the teeth and the soft tissue. In this case report altered cast technique for fabrication of removable partial dentures has been described in detail. It is best alternative for impression making especially in distal extension cases for its better tissue adaptability, better support, uniform stress distribution, and stability of the prosthesis. Thus, increases patient comfort and function.

Quality of Communication between Dental Technicians and Dentists during the Fabrication of Removable Partial Dentures in Casablanca, Morocco

Introduction: Communication between the dentist and the dental laboratory technician plays an essential role in the success of prosthetic treatment. The purpose of this study was to describe the quality of communication between dentist and dental technicians, and to highlight the different constraints encountered by the laboratory technician when making removable partial prostheses. Materials and Methods: This is a cross-sectional study conducted among 94 dental technician working in the private sector in Casablanca. To collect data an auto administrated questionnaire was distributed to the dental technicians. This tool included five sections : Identification, communication methods, prosthetic design, communication difficulties and the added value of digitizing the prosthetic chain. Data were statistically processed using SPSS 20 software. Results: A total of 94 dental technicians working in Casablanca were interviewed. All these laboratory technicians (100%) used the laboratory form as their main means of communication. Additional means of communication were used to complete missing information (phone, photo sharing, e-mail). 72.3% of dental technicians designed the metal framework for cast partial dentures themselves and 67% of prosthetists said that digitizing the prosthetic chain could improve dentist - dental laboratory technician communication. Conclusion: Effective communication between dentist and dental technician is essential for successful prosthetic treatment and patient satisfaction

Fabrication of Distal Extension Removable Partial Denture with Surveyed Crown and Altered Cast Technique: A Case Report

Support and stability for a Removable Partial Denture (RPD) are difficult to achieve in a distal extension edentulous ridge and are even more compromised in reduced dentition. The combination of the altered cast technique and milled crown in removable partial denture fabrication would greatly enhance support by redistributing the load more effectively. However, it is not routinely practiced due to the complexity of the treatment and the lack of emphasis in dental curriculum. In the present case of a 54-year-old male, despite several attempts at wearing the denture, the patient was unable to tolerate it. The atrophic mandibular ridge and poor inclination of the abutment tooth seemed to be complicating factors. Therefore, the prosthesis required some modifications. The present case report highlights the Applegate altered cast technique, which can be deployed with a milled crown, including the clinical and laboratory stages involved. This might help to overcome the problems encountered with distal end saddle dentures, making them more tolerable to patients.

CAD-CAM for removable partial denture fabrication after marginal mandibulectomy based on a well-adjusted pre-existing denture.

This clinical report summarizes a workflow that introduces two approaches for fabricating a metal framework removable partial denture for a marginal mandibulectomy patient. First, the pre-existing denture was adjusted according to the piezographic technique to determine the optimal configuration for adhering to the available denture space. Second, computer-aided design-computer-aided manufacturing (CAD-CAM) technology was used to fabricate a new removable partial denture by digitally scanning the oral condition and the well-adjusted denture and then using the obtained data to design and fabricate the new denture. These two approaches can help to reduce chair time not only for maxillofacial prosthetic treatment but also for other denture cases.

Dental Technicians' Perception of the Quality of Dentists' Communication on the Fabrication of Removable Partial Dentures: A Cross-Sectional Study in Saudi Arabia.

Effective and clear communication between the dentist and dental technician plays a vital role in rendering quality prostheses for patients. When fabricating a removable dental prosthesis, it is uncertain if the information received by the dental laboratory technician is clear and sufficient. This investigation aimed to assess dental technicians' perceptions of the quality of dentists' communication on the fabrication of removable partial dentures (RPDs) in Saudi Arabia. After obtaining the institutional review board (IRB) approval from Riyadh Elm University, Riyadh, Saudi Arabia, a cross-sectional survey on a convenient sample of 115 dental technicians registered with the Saudi Commission for Health Specialties was conducted in January 2022. The voluntary participation of 94 technicians fabricating RPDs was included. A 19-item online questionnaire was developed, including quality of written instruction, selection of impression trays, and impression materials for RPD fabrication, shared through Google Docs. Descriptive statistics were tabulated, and responses were displayed as a percentage of the total. Of the 94 study subjects, 35% had less than five years of experience, 44% stated that they routinely receive work authorizations with clear instructions, 13% always used digital technology to fabricate prostheses, and 58% reported difficulty with communicated work authorization by dentists having less than five years of experience. Thirty-three respondents (35.1%) reported that 75% or more of the fabricated partial dentures were cast framework partials. Thirty-three respondents (35.1%) indicated that the master casts received for partial framework construction were usually accurate. Tooth alterations, however, were reported as usually adequate by only 28 respondents (29.8%). For creating the artificial gingiva portion of cast partials, 56 respondents (59.6%) preferred heat-cured acrylic resin. Furthermore, 40 respondents (42.6%) said that 75% or more of the requested partials were entirely made out of acrylic resin. Regarding case design discussions, 26 respondents (27.7%) always engaged with dentists, while 39 (41.5%) did so occasionally. The obtained assessments pointed to the fact that dental technicians expressed a perception of inadequacy regarding the work authorizations provided by dentists for the fabrication of RPDs, where they seemingly felt that the instructions conveyed by the dentists were not sufficiently comprehensive or clear.

Fit accuracy in the rest region of RPDs fabricated by digital technologies and conventional lost-wax casting: a systematic review and meta-analysis

BackgroundDigital technologies have recently been introduced into the fabrication of removable partial dentures (RPDs). However, it is still unclear whether the digitally fabricated RPDs fit better than conventionally cast ones in the rest region. The aim of this study was to evaluate the fit accuracy in the rest region of RPDs fabricated by digital technologies and compare it to those made by conventional lost-wax (CLW) technique.MethodsA comprehensive search was conducted in Cochrane Library, PubMed, EMbase, Web of Science and SpringerLink. Studies published up to August 2022 were collected. Two authors analyzed the studies independently and assessed the risk of bias on the modified methodological index for non-randomized studies (MINORS) scale. The mean values of gap distance between rests and corresponding rest seats of each study were extracted as outcome. A random-effects model at a significance level of P < 0.05 was used in the global comparison and subgroup analysis was carried out.ResultsOverall, 11 articles out of 1214 complied with the inclusion criteria and were selected, including 2 randomized controlled trials (RCTs), 1 non-randomized clinical trial and 8 in vitro studies. Quantitative data from Meta-analysis revealed that fit accuracy in the rest region of RPDs fabricated with CLW showed no statistically significant difference with digital techniques (SMD = 0.33, 95%CI (-0.18, 0.83), P = 0.21). Subgroup analysis revealed a significantly better fit accuracy of CLW-fabricated RPDs in the rest region than either additive manufacturing (AM) groups or indirect groups (P = 0.03, P = 0.00), in which wax or resin patterns are milled or printed before conventional casting. While milled RPDs fit significantly better than cast ones in the rest region (P = 0.00). With digital relief and heat treatment, hybrid manufactured (HM) clasps obtained better fit accuracy in the rest region (P < 0.05). In addition, finishing and polishing procedure had no significant influence in the fit accuracy in all groups (P = 0.83).ConclusionsRPDs fabricated by digital technologies exhibit comparable fit accuracy in rest region with those made by CLW. Digital technologies may be a promising alternative to CLW for the fabrication of RPDs and additional studies are recommended to provide stronger evidence.Trial registrationCRD42020201313.

A new proposal for improving the accuracy of intraoral scanning for partially edentulous residual ridge.

Purpose This study investigated the usefulness of a newly proposed intraoral scanning method, using markers that can be used directly in the oral cavity, in order to improve the accuracy of impression taking of the residual ridge for fabrication of removable partial dentures.Methods An intraoral scanner was used to scan a dental model of a partially edentulous mandibular arch (Kennedy Class I). As markers, pieces of dried pasta were used. The scanning operation was performed under three conditions. In Condition 1, scanning was performed on the remaining teeth and the residual ridge without markers. In Condition 2, scanning of the remaining teeth and residual ridge was performed with markers. In Condition 3, the markers were removed from the model used in Condition 2, and the residual ridge was scanned again. The scanning data of each condition was superimposed on the control data, and the shape error was calculated and compared among the conditions.Results There was a significant difference in trueness of the residual ridge before and after marker application. The application of markers improved the trueness, while maintaining precision. Re-scanning after removing the marker did not affect trueness between before and after re-scanning and the re-scanned region showed shape continuity with the surrounding region.Conclusions The present method using markers that can be used in the oral cavity was effective in improving the accuracy of impression taking at the residual ridge.

Fabrication of milled removable partial dentures using a custom plate with prefabricated artificial teeth.

Although digital removable partial dentures have been previously described, there have been no reports on how to fabricate them in one piece. This study proposes a new method for fabricating patient-specific digital removable partial dentures using a custom plate. First, a gypsum model was scanned using a laboratory scanner and a removable partial denture was designed using computer-aided design (CAD) software based on standard tessellation language data. The metal clasp was fabricated from Ti-6Al-4V using a 3D printer. For custom plate fabrication, a resin plate frame was designed using computer-aided design (CAD) software and fabricated using a 3D printer. An artificial tooth and metal clasp were fixed on the base surface of the frame, an auto-polymerizing resin was poured into the frame for the denture base, and the artificial tooth and metal clasp were packed to form a custom plate. The plate was cut using a milling machine. Subsequently, the support attached to the denture was removed and polished for complete fabrication of the denture. Our novel removable partial denture fabrication method is more efficient than the conventional method. The obtained removable partial dentures demonstrated satisfactory accuracy.

Conformity of removable partial denture designs from three laboratories to a set of design principles

Purpose. The purpose of this study was to assess and to compare the technical quality of removable partial dentures (RPDs) servicing both public and private sectors in relation to biomechanically acceptable principles. A convenience sample of work carried out in three commercial dental laboratories servicing both the private and government sectors was used to provide a descriptive study of the laboratory stage of fabrication of RPDs over a 3-month period. Prescription sheets, master casts and completed dentures of 114 cases were evaluated and procedural and design-related information data were captured for each case. The results were disappointing in that not one of the RPDs evaluated conformed to commonly accepted principles. Not one dentist prescribed the design of the denture. Only one cast was surveyed to determine the path of insertion and any undercuts for the placement of clasps, yet 119 clasps were placed. Although 81 rests were used (in only 25 of the dentures), only 11 of those rests were pre-prepared on the teeth. Only 8 of the 95 acrylic-based dentures had any rests, making the remainder entirely mucosa-borne and therefore potentially iatrogenic.The study suggests that principles of RPD design taught during undergraduate training are not being adequately practised in both private and public sectors; and if practised the RPDs are not designed, nor constructed to requirements guided by design principles. Further training of dental technicians and clinicians is clearly indicated, and consideration should be given to a system of clinical audit to identify and prevent the malpractice observed in this study. In addition, ethical and legal guidelines emphasising the roles of clinicians and technicians in the prescription and design of RPDs in South Africa should be developed; and the mandatory comprehensive completion of laboratory work authorisation forms by dental clinicians should be considered.

A Digital Workflow for the Fabrication of a Milled Removable Partial Denture.

Complete dentures fabricated with the additive or subtractive method have been widely used and proven to be clinically acceptable. However, fabrication of removable partial dentures (RPDs) using computer-aided design and computer-aided manufacturing is limited by its technique sensitivity as the pink resin, which encases part of metal framework, cannot be fabricated digitally. This article introduces a digital workflow to fabricate an RPD with the subtractive method. A complex structure of the offset metal framework and denture base with teeth sockets was milled with this technique. Artificial teeth were milled with a resin disk according to the computer-aided design data, resulting in the customized occlusal surface. This digital technique can be an alternative to the analog fabrication method as the RPD was fabricated digitally, keeping the original structures and reducing resin shrinkage on the intaglio surface.

Novel fully digital workflow for removable partial denture fabrication

Background: Digital dentistry technology based on computer-aided design and computer-aided manufacturing (CAD/CAM) has been widely applied in the field of prosthodontics, ranging from digital techniques for impression processes, jaw registration, artificial tooth arrangement and base design, along with base and denture fabrication. However, conventional analog workflows are still used and digital workflows have not completely replaced analog workflows. Purpose: Knowing the latest complete digi-tal workflow in the manufacture of removable partial dentures (RPD). Methods: Full-arch digital impressions of the partially edentulous jaw were made with an intraoral scanner or by conventional methods. The denture framework, artificial teeth, and denture base were designed by commercially available CAD software. Each of the denture components including connectors, clasps, and artificial teeth and the denture bases were fabricated separately by the CAM machine, and then assembled using an adhesive material. Conclusion: RPDs were successfully fabricated using a fully digital workflow and no clinical complications were reported.

Influence of the Commercial Mouthwashes on the Corrosion Behaviour of Dental Alloy

Dental alloys are widely used in fabrication of removable partial dentures, particularly as a dental framework. The present study aimed at studying the effect of the commercial mouthwashes on the corrosion behavior of the Co−Cr dental casting alloy. Corrosion behavior was investigated in alcohol-free and alcohol-containing mouthwashes added to artificial saliva solutions at room temperature by electrochemical tests recorded the open circuit potential (OCP) and current density, after the surface analysis of the samples was performed. During the early 250s, the OCP values showed significant changes, with the exception of one test group containing alcohol, where potential decreased continuously during the test, which also recorded the highest current density, different from the other groups. It was observed that the presence of alcohol increased the current density and the corrosion process in this alloy and the mouthwashes had more aggressive action in specimens submitted to solutions with alcohol in the composition.

Current Trends in Removable Partial Prosthodontics Teaching in Undergraduate Dental Colleges of Sindh, Pakistan

Objective: To explore current trends in removable partial prosthodontics teaching in undergraduate dental colleges of Sindh, Pakistan.&#x0D; Study Design: Descriptive Cross-sectional.&#x0D; Place and Duration of the Study: Department of prosthodontics in dental colleges of Sindh in September 2019.&#x0D; Methodology: A descriptive cross-sectional study was done in which a questionnaire was sent by email to head of Prosthodontics department of 16 dental institutes of Sindh, Pakistan. The survey included different questions regarding current trends in removable partial prosthodontics education in undergraduate dental colleges.&#x0D; Results: Out of sixteen questionnaires, fourteen were completely filled and returned. Majority (69.2%) of the institutes teach fabrication of removable partial dentures (RPDs) in third year of dental college. Students of only one dental college fabricate 10-12 RPDs in their clinical rotation. All dental colleges teach acrylic RPD construction in their preclinical years. Faculty in 12 (85.7%) dental colleges always give clinical demonstrations to students before their clinical rotations. Eleven out of the fourteen colleges teach cast partial dentures (CPDs) theoretically, but no CPDs are fabricated by the students during their clinical rotation. About 70% dental schools do not have in-house facility to fabricate cast partial dentures.&#x0D; Conclusion: This study describes current trends of removable prosthodontics teaching in undergraduate colleges of Sindh. A large majority of undergraduate dental students are not fabricating the minimum number of partial dentures as per PMC requirement. Although theoretically cast partial dentures are taught in lectures but students do not construct them in their undergraduate years. Efforts should therefore be made to improve teaching practices and clinical skills of undergraduates, for them to be able to treat partially dentate patients after graduation.

An update on indirect prosthodontic materials and their manufacturing techniques

Innovations in the fabrication of removable partial dentures depend not only on the development of new materials, but also on the availability of manufacturing techniques that can be applied to a dental environment. Many of these new materials have limited clinical evaluations, hence it can be difficult for the general dental practitioner to confidently determine which materials to use. The introduction of any new material into clinical practice often requires practitioners to go through a learning curve to make the most of the material and employ it most appropriately. This article provides an update on the materials available for removable partial dentures and discusses the advantages and disadvantages to enable the GDP to make an evidence-based decision. CPD/Clinical Relevance: It is important that clinicians are aware of the alternative materials to conventional acrylic and cobalt chrome.

Cast-free fabrication of a digital removable partial denture with a polyetheretherketone framework

Digital workflows for removable partial dentures have enabled new machinable framework materials, including polyetheretherketone (PEEK). In addition, all denture components can be designed and manufactured digitally. Nonetheless, physical casts are still required for assembly of the components. The adoption of a cast-free fabrication procedure may enhance benefits, including a reduced number of procedures, faster processing, and reduced material waste. The purpose of this technical report was to demonstrate a workflow for the fabrication of a removable partial denture with a PEEK framework, milled teeth, and milled flanges, which uses intraoral scanning and no physical casts.

Digital technology for removable partial denture realization. Case report

The conventional fabrication of removable partial dentures is a complex process, which involves a lot of clinical and technological steps. The design of the elements must be performed in order to maintain the homeostasis and to rehabilitate, in optimal conditions, the functions of the stomatognathic system. Digital technology-Computer-aided design/computer-aided manufacturing (CAD/CAM) method for partial denture fabrication brings a lot of advantages: high accuracy of the denture, increases the strength and durability, avoid changes of the volume and, time working economy. This clinical report aims to illustrate the stages of getting a removable prosthesis, using the computerized method and to emphasize that digital prosthesis can be effective and accurate, eliminating or replacing steps that can lead to complications.
