Blouse-hook injury in breastfeeding children: Canalicular laceration to corneal tear

An ocular injury in a kid is very common, especially in <5 years of age, which ranges from eye adnexa to globe injury. In breastfeeding kids, there are chances of eyelid avulsion with involvement of the canaliculus. In our case series, we report two cases of eyelid laceration and one case of corneal tear due to the use of a safety pin as a blouse hook. Our first case of a 2.5-year-old kid presented with multiple laceration of the eyelid and was managed by primary repair due to the unavailability of mini-monoka stent. The second child was managed well by the mini-monoka stent and had given excellent anatomical as well as functional outcome on 6 months of follow-up. Our third case was corneal tear which was managed well with good visual outcome without cataract formation on 6 months of follow-up.

Replantation of avulsed eyelid followed by use of bioengineered dermal substitute

Eyelid injuries secondary to dog bites are common, particularly ones involving the lacrimal system. Full thickness eyelid avulsion is less often seen after dog bites. We report an unusual case of a young female who underwent replantation of full thickness avulsed eyelid tissue after a dog bite. The skin of the grafted eyelid necrosed postoperatively and was replaced with bioengineered skin substitute resulting in good functional and cosmetic outcomes.

Reconstruction of a Traumatic Large Upper Eyelid Avulsion with Tarsoconjunctival Graft and Rotational Flap: A Surgeon’s Preference

Introduction: Large, full-thickness upper eyelid defects management presents a thorough and challenging approach due to the multilaminar character of the eyelid and its dynamic function. The Cutler-Beard flap method has been the mainstay management, but posterior lamellar graft usage with recent modifications may reduce complications. Case Presentation: A 43-year-old man presented to the emergency ward after a traffic accident left him with a full-thickness laceration on his left upper eyelid and an approximately 60% avulsion on the one-third medial of the eyelid. Traumatic ptosis was also found. After considering the large size of the affected area and the probability of faster wound healing, a free graft from the tarsus and conjunctival eyelid of his fellow eye was harvested to replace the posterior lamellar of the affected eye and closed the defect with a rotational skin flap from the remaining of the left upper eyelid. Conclusions: This technique could be one of the alternatives to the more common cutler-beard flap with mild complications. However, temporary visual obstruction cannot be avoided due to the traumatic ptosis complication of this patient.

Traction Applying Technique on Bicanalicular Crawford Silicone Tube to Overcome the Notching Deformity After Lacrimal Canalicular Repair.

Lower eyelid avulsion injury with lower canalicular laceration generally occur just medial to the punctum with insufficient skin remnant for repair causing tension on repair margins. The inevitable blinking force, along with the tension widens the repair margin, resulting in an aesthetically challenging notch at the medial lower lid. The authors attempted to minimize this notching deformity with a traction applying technique on bicanalicular silicone tube.Fifteen patients were enrolled and divided into 2 groups: the experimental group with 10 patients which received the traction technique, and the control group with 5 patients which the traction technique was omitted. Each end of the bicanalicular silicone tube was intubated through both puncta and the lacerated canaliculus. No canalicular anastomosis was performed. The tube ends were retrieved through the nostril, followed by medial canthal tendon, orbicularis oculi muscle, and skin repair. The tube ends were pulled to create a tension on the loop, until upper and lower puncta apposed each other, creating a dimple in the medial canthal area. The tube ends were tied and anchored at internal valve of the nostril to maintain the traction tension. Medial lower lid notching (>2 mm width), epiphora, and conjunctival injection were evaluated and compared in the 2 groups.Patients with traction technique showed symmetric medial canthal angle and minimized lower lid notching deformity when compared with the control group (P = 0.025). No conjunctival injections and epiphora were observed in either group. Symmetric and aesthetically satisfactory results were achieved by traction applying technique on bicanalicular silicone tube.

Eyelid avulsion managed with composite autograft in a 6-year-old child

The management of total eyelid avulsion is an arduous task with unpredictable functional and cosmetic result. We present a case of total lower eyelid avulsion in a 6 year old child following injury with bicycle. The reconstruction was done within 6 hours of injury. Retrieval of tissue from the incident site and its preservation permitted us to use it as a composite autograft. The child has recovered well with minor sequelae.

Eyelid avulsion due to bull horn injury: A rare presentation

A 52-year-old female presented with a history of bull horn injury to the right eye. Examination revealed an avulsed flap of right lower eyelid starting from a point just lateral to the lacrimal punctum up to the lateral canthus without any other ocular or orbital damage. After a thorough assessment and cleaning, lid flap was sutured in layers. Postoperative antibiotic prophylaxis was given. Eyelid wound healed without complications.

Eyelid Avulsion from Monkey Bite

A 19-day-old infant was bitten by a macaque monkey in western Singapore, resulting in a full thickness laceration of his upper eyelid, which was successfully repaired with good structural, functional and aesthetic outcome. Periorbital injuries of children are of concern in view of the risk of visual impairment. In addition, monkey bite inoculations carry the risk of transmitting multiple bacterial, Rabies and Herpes B viral infections. The acute management and surgical repair, as well as learning points on management of Simian injuries are discussed.

We present a unique case in which a dog bite resulted in total full-thickness upper eyelid avulsion in a young Caucasian female. The eyelid was replanted using meticulous microsurgical techniques reanastamosing the arterial supply. To our knowledge, this is the first reported case of a successful microvascular replantation of a completely avulsed upper eyelid. The details of the case and the photographic clinical improvement are described and discussed.

Eyelid Avulsion

Traumatic eyelid avulsion is reported infrequently. We cared for an 18-year-old man who lost > 75% of his right upper eyelid. The tissue was recovered 10 h following the initial trauma. Successful surgical reimplantation resulted in a functional upper eyelid with extensive madarosis. An experimental study was undertaken to attempt to determine the optimal method for preservation of avulsed eyelid tissue. Experimental surgery was performed to simulate repair of eyelid avulsion. The postoperative changes in experimentally reimplanted eyelids parallel those noted in the clinical example of eyelid avulsion. This clinical example and experimental study suggested the following management principles for handling avulsed eyelid tissue: eyelid tissues should not be immersed in saline; avulsed eyelid tissues should be stored at 4 degrees C; if reimplantation time is < 6 h, storage in a cold moist environment is adequate.

Operative Complications of the Transconjunctival Inferior Fornix Approach

The transconjunctival inferior fornix incision provides access to the floor, rim, lateral, and inferior medial walls of the orbit. Complications of this surgical approach to the orbit are known to be rare but heretofore have not been clearly defined. Over an 8-year period, in an estimated 1200 cases, the authors have encountered cicatricial entropion, lower eyelid retraction, canthal dehiscence, lower eyelid avulsion, canalicular laceration, buttonhole laceration of the lower eyelid, conjunctival chemosis, and lacrimal sac laceration. Attention to anatomic landmarks and sound surgical execution will prevent these complications in most patients.
