Can Angiography Be Planned Based on Specific CT Signs of Ongoing Soft Tissue Bleeding in COVID-19 Patients Receiving Anticoagulants?

Background. Anticoagulant therapy is recommended as basic medical treatment for COVID-19 patients with risk factors. At the same time, it is accompanied by a large number of hemorrhagic complications. Massive bleeding into abdominal and chest walls soft tissues as well as retroperitoneal space significantly worsens the course of underlying disease.Objective: to assess the prognostic value of specific CT signs and their correlation with angiography data for the diagnosis of ongoing bleeding into soft tissues in COVID-19 patients.Material and methods. A retrospective study was conducted, in which multi-slice computed tomography (MSCT) and angiography data were assessed and compared in 60 COVID-19 patients with spontaneous bleeding into chest and abdominal wall soft tissues as well as retroperitoneal space. A retrospective analysis of MSCT was carried out to identify symptoms specific to coagulopathic bleeding into soft tissues: contrast agent extravasation, hematocrit effect, and signal flare phenomenon.Results. Contrast agent extravasation identified by MSCT was confirmed in 57 (95%) patients. More often (39 (68.4%) patients), extravasation was detected in MSCT arterial phase. In MSCT venous phase it was noted in 17 (29.8%) cases, in delayed phase – in 1 (1.8%) patient. With native MSCT, hematoma contained clots and fluid level (hematocrit effect) in 43 (71.7%) patients, in 17 (28.3%) patients it had a homogeneous structure. With intravenous contrast, in 39 (90.7%) cases with fluid level the signal flare phenomenon was established, which was more often detected in the arterial phase (33 (84.6%) cases). Extravasation was detected on angiography in 27 (45%) patients; they underwent therapeutic embolization. In another 15 patients preventive embolization was performed. When extravasation into hematoma without fluid level on MSCT (13 cases), extravasation on angiography was detected in 4 (30.8%) patients. In the presence of extravasation into hematoma with fluid level, but without signal flare phenomenon (4 patients) it was noted on angiography in 1 case. In the presence of all three signs (extravasation, hematocrit effect, and signal flare phenomenon – 39 patients), the incidence of extravasation on angiography was the highest – 56.4% (p = 0.028), and patients more often required therapeutic embolization.Conclusion. Detection of extravasation, fluid level and signal flare phenomenon on MSCT in the structure of spontaneous hematoma in COVID-19 patients corresponded to ongoing bleeding and more often required embolization than in isolated extravasation.

Impacted upper third molar surgical removal leads to pseudoaneurysm: A case report highlighting surgical risks

AbstractAimThe rate of excessive bleeding in the post‐operative period after third molar extraction is low (less than 0.6%), and even lower in the case of upper molars. We report the case of excessive life‐threatening bleeding 2 weeks after an upper third molar extraction that had an intraoperative complication with a displacement of the molar into the maxillary sinus.Materials and methodsAn initial CT scan showed only postsurgical changes of tooth extraction, and a small contrast agent extravasation (measuring 5 mm). An angiogram was done to locate the origin of the bleeding, and a pseudoaneurysm was located in the right superior alveolar artery. The patient was treated by embolization with coils, with no subsequent episodes of bleeding.ResultsPseudoaneurysms of branches of the external carotid artery system are rare, and even more rare after dental or oral surgery (only five cases published to date). Endovascular treatment is the treatment of choice, as it is minimally invasive as well as being selective.ConclusionThe rate of traumatic pseudoaneurysm of branches of the ECA is low; however, in order to avoid severe consequences, a correct diagnosis as early as possible is essential for a proper management.

Endovascular Treatment of Vascular Injuries in the Craniocervical Region With a Graft Stent: A Single-Center Experience.

We aimed to evaluate the efficacy and safety of graft stent implantation in the endovascular treatment of vascular injuries in the craniocervical area. This study was carried out through the retrospective screening of eight (two females and six males) patients' records. Patients who used graft stents as an endovascular method were included in the study. The mean age of the patients was 43.6 years (with a range of 15-69 years). Due to different mechanisms, the patients had vascular injuries in the craniocervical region, and graft stent implantation was performed between 2010 and 2022. We evaluated patient demographics, admission symptoms, trauma mechanisms, angiographic findings, treatment modalities and materials, patient outcomes, and follow-up periods. Due to iatrogenesis for four patients, penetrating trauma for three patients, and blunt trauma for one patient, injuries were present in the right internal carotid artery {ICA} (n=1), left ICA (n=1), left common carotid artery {CCA} (n=3), right CCA (n=1), right vertebral artery (n=1), and left vertebral artery (n=1). Angiographically, pseudoaneurysm was detected in seven patients, and vascular rupture was detected in one patient using contrast agent extravasation. All patients who underwent the endovascular method had technical success. Since bleeding from the external carotid artery branches was seen in two patients, these branches were embolized with coils. No radiologically or neurologically pathological findings were recorded during the follow-up period (with a range of one week to 12 years). Graft stent use in the endovascular treatment of craniocervical vascular injuries is an operable, safe, and promising option, especially in patients with pseudoaneurysms and active bleeding.

Spontaneous resolution of new coronary artery aneurysm following guideline-directed medical therapy after drug-eluting stent implantation

Here, we present a case of a 56-year-old man with acute myocardial infarction. The patient underwent percutaneous coronary intervention (PCI) at the left main bifurcation and mid-left anterior descending artery using drugeluting stents. Four months after the PCI, the patient was readmitted for cardiac arrest. Coronary angiography (CAG) revealed stent thrombosis in the left main-toproximal left anterior descending artery and in-stent restenosis in the left main-toproximal left circumflex artery. We performed balloon angioplasty at the left main to mid-left anterior descending artery and left main to proximal left circumflex artery stents; subsequently, blood flow was fully restored. However, contrast agent extravasation was observed outside the mid-portion of the left main artery to the proximal left anterior descending artery stent, indicating the presence of a coronary artery aneurysm (CAA) outside the stent. After guideline-directed medical therapy with dual antiplatelet agents and high-intensity statins, follow-up CAG revealed near-resolution of the CAA, absence of stenotic lesions, and good blood flow.

A modified esophagogastric reconstruction method after laparoscopic proximal gastrectomy: A technical note with video vignette

TechniqueAlthough the double flap technique effectively reduces the incidence of postoperative reflux esophagitis and anastomotic leakage after laparoscopic gastrectomy, its clinical application is restricted because the procedure is technical complex. We devised a modified esophagogastric reconstructive method which we termed the "arch-bridge-type" reconstruction. This reconstruction method was performed for a 71-year-old man, who was admitted to our hospital with the diagnosis of cT2N0 upper gastric cancer. The present study reported the surgical details and accompanied with the video. ResultsThe patient underwent surgery successfully without switching to open surgery. The total operation time was 203 min, the time for making the “arch-bridge” was 16 min, and the time for esophagogastric anastomosis under laparoscopy was 23 min. No surgery-related complications occurred. The postoperative hospital stay was 10 days. The upper GI radiography demonstrated that the anastomosis was not narrow and no extravasation of contrast agent was observed. The gastroscopy found no reflux esophagitis and anastomotic stenosis 1 year after surgery. ConclusionThe "arch-bridge-type" reconstruction method is safe and time saving. It has advantages in simplifying the procedure of conventional double flap technique and reducing postoperative complications after proximal gastrectomy.

Pelvic ring fractures with concomitant large hematomas: diagnostic investigation with arteriography and eventual embolization in 157 trauma patients, with or without contrast extravasation at emergency CT.

This study aims to evaluate the diagnostic value of pelvic arteriography in patients with pelvic ring fractures and associated large hematomas, in both cases of positive or negative findings of contrast agent extravasation at emergency CT; in those patients with positive DSA subsequently treated with embolization, correlations with clinical-radiological parameters were investigated. In this single-center retrospective study, patients with acute blunt pelvic trauma showing at CT pelvic ring fractures with associated large (> 3cm) hematoma, with or without signs of arterial bleeding, were investigated with DSA. Technical success was considered radiographic bleeding control with disappearance of angiographic bleeding; clinical success was defined as clinical bleeding control hemodynamically stable, before applying other surgical maneuvers. Pelvic ring fractures were evaluated according to Tile classification system. One hundred and fifty-seven patients, mean age 54years, were analyzed. 70.7% had polytrauma; 14.6% patients assumed antiplatelets and/or anticoagulation therapy. False-negative and false-positive rates at CT were 29.6% and 27.1%, respectively. Polytrauma and B3/C1 Tile pattern fractures were significantly associated with bleeding signs at DSA. Seventy-two patients required embolization: 52.8% showed direct signs of DSA bleeding; among these, technical and clinical successes were 88.8% and 81.9%, respectively. In this study, patients with pelvic ring fractures and concomitant hematomas > 3cm, with or without contrast extravasation at CT, have been examined in depth with DSA focusing on both direct and indirect angiographic signs of bleeding, finding polytrauma and Tile fracture patterns B3/C1 predictive factors for arterial hemorrhage detection at DSA despite negative CT findings.

Evaluation of the Jichi Medical University diverticular hemorrhage score in the clinical management of acute diverticular bleeding with emergency or elective endoscopy: A pilot study.

Emergency endoscopic hemostasis for colonic diverticular bleeding is effective in preventing serious consequences. However, the low identification rate of the bleeding source makes the procedure burdensome for both patients and providers. We aimed to establish an efficient and safe emergency endoscopy system. We prospectively evaluated the usefulness of a scoring system (Jichi Medical University diverticular hemorrhage score: JD score) based on our experiences with past cases. The JD score was determined using four criteria: CT evidence of contrast agent extravasation, 3 points; oral anticoagulant (any type) use, 2 points; C-reactive protein ≥1 mg/dL, 1 point; and comorbidity index ≥3, 1 point. Based on the JD score, patients with acute diverticular bleeding who underwent emergency or elective endoscopy were grouped into JD ≥3 or JD <3 groups, respectively. The primary and secondary endpoints were the bleeding source identification rate and clinical outcomes. The JD ≥3 and JD <3 groups included 35 and 47 patients, respectively. The rate of bleeding source identification, followed by the hemostatic procedure, was significantly higher in the JD ≥3 group than in the JD <3 group (77% vs. 23%, p <0.001), with a higher JD score associated with a higher bleeding source identification rate. No significant difference was observed between the groups in terms of clinical outcomes, except for a higher incidence of rebleeding at one-month post-discharge and a higher number of patients requiring interventional radiology in the JD ≥3 group than in the JD <3 group. Subgroup analysis showed that successful identification of the bleeding source and hemostasis contributed to a shorter hospital stay. We established a safe and efficient endoscopic scoring system for treating colonic diverticular bleeding. The higher the JD score, the higher the bleeding source identification, leading to a successful hemostatic procedure. Elective endoscopy was possible in the JD <3 group when vital signs were stable.

Transcranial focused ultrasound-induced blood‒brain barrier opening in mice without shaving hairs

Acoustic coupling through hairs remains a challenge to performing transcranial-focused ultrasound procedures. Here, we demonstrated that this challenge could be addressed by using oil as the coupling medium, leveraging oil's high affinity to hairs due to their inherent hydrophobicity. We compared focused ultrasound-induced blood–brain barrier opening (FUS-BBBO) outcomes in mice under three coupling conditions: oil with hairs (“oil + hairs”), ultrasound gel with hair shaving (“ultrasound gel + no hair”), and ultrasound gel with hairs (“ultrasound gel + hairs”). The quality of the coupling was evaluated by {mathrm{T}}_{2}-weighted magnetic resonance imaging (MRI) and passive cavitation detection (PCD). The outcome of FUS-BBBO was assessed by MRI contrast agent extravasation using in vivo {mathrm{T}}_{1}-weighted contrast-enhanced MRI. It was also evaluated by ex vivo fluorescence imaging of the mouse brain after intravenous injection of a model drug, Evans blue. The results showed that “oil + hairs” consistently achieved high-quality acoustic coupling without trapping air bubbles. The FUS-BBBO outcome was not significantly different between the “oil + hairs” and the “ultrasound gel + no hair” groups. These two groups had significantly higher levels of BBB opening than the “ultrasound gel + hairs” group. This study demonstrated that oil could be a coupling medium for transcranial FUS procedures without shaving hairs.

T1 Mapping for Differentiating of Haemorrhagic Transformation from Extravasation of Iodine Contrast Agents: A Phantom Study.

To evaluate T1 mapping values in different concentrations of iodine and mixed blood and to simulate the application of T1 mapping in differentiating iodine contrast extravasation and haemorrhage transformation after revascularisation in acute ischemic stroke. A phantom-based experimental study. Place and Duration of the Study: Department of Radiology, the Second Affiliated Hospital of Soochow University, China, from October 2020 to December 2021. Fresh blood,pure iodine, blood-iodine mixtures (75/25, 50/50, and 25/75 ratios), and diluted iodine (at a concentration of 2.1 mmol I/L) were scanned in a phantom on 3-T MR T1 mapping imaging. A total of 10 layers in the middle section of tubes were scanned. The mean value of T1 mapping and 95% confidence interval for the investigated sample compositions were calculated and compared by ANOVA. The mean values (95% CI) for fresh blood, [2/3] blood +[1/3] iodine, [1/2] blood+[1/2] iodine, [1/3] blood+[2/3] iodine, and pure iodine were 2108.69 ± 1966.68-2250.71(ms), 1991.72 ± 1763.22-2220.21(ms), 1811.62 ± 1614.79-2008.45(ms), 1624.39 ± 1442.41-1806.37(ms), 1294.68 ± 1172.92-1416.44(ms), respectively. The differences between the T1 mapping values of all compositions were significant (p <0.01), except for fresh blood and the sample consisting of 67% blood. The mean value on T1 mapping (95% CI) was 1294.68 ± 1172.92-1416.44 (ms) in the samples only with diluted iodine, which was significantly different from other investigated samples (p <0.01). The intra-class correlation coefficient between the two times drawing of radiologist A was excellent (ICC=0.913, p<0.01), and between radiologists A and B was 0.99. Iodine contrast extravasation in a phantom setting might be distinguished from haemorrhagic transformation using T1 mapping. Dual-energy Computed Tomography (DECT) predicts the efficacy of contrast medium extravasation and secondary cerebral hemorrhage after stent thrombectomy in acute ischemic cerebral infarction

ABSTRACT To prospective research the efficacy of dual-energy computed tomography (DECT) in predicting contrast medium extravasation and secondary cerebral hemorrhage after stent thrombectomy in acute ischemic cerebral infarction. Ninety-two patients with acute ischemic stroke who underwent intra-arterial thrombolysis in our hospital from December 2019 to January 2022 have opted as the study subjects. DECT was performed immediately after stent thrombectomy. Images were generated through the image workstation and routine diagnosis was performed 24 hours after the operation. To analyze the diagnostic value of To analyze the diagnostic value of DECT, and to explore the diagnostic status of lesions with hemorrhagic transformation or increased hemorrhage and their correlation with iodine concentration. (1) 68 situations were confirmed, 56 positive and 12 negative with detection rates of 10.71% for hemorrhage, 75.00% for contrast agent extravasation, and 14.29% for extravasation combined with hemorrhage; (2) DECT diagnosed 8 cases of postoperative bleeding and 44 cases of extravasation of contrast media and 4 cases of extravasation of contrast media with hemorrhage ; The accuracy of DECT in diagnosing postoperative hemorrhage was 96.43%. The accuracy of diagnosis of extravasation was 96.43%. (3) The mean iodine concentration of lesions with increased hemorrhage or hemorrhagic transformation was higher compared to those without; (4) There was a correlation between hemorrhagic transformation or increased hemorrhage and iodine concentration. Dual-energy CT (DECT) can accurately distinguish the extravasation of contrast agent and secondary cerebral hemorrhage, and can predict the increased bleeding and bleeding transformation, with good diagnostic value and good predictive efficacy.

Integrated Care to Reduce Contrast Agent Extravasation in CT Enhancement Examinations

To explore the effectiveness of integrated care in reducing contrast agent extravasation during CT enhancement examinations. A total of 7,247 patients undergoing CT enhancement examinations in our department between August 2022 and May 2023 were selected. The 3,615 patients treated between August 2022 and December 2022 served as the control group, while the 3,152 patients treated from January 2022 to May 2022 were the research group. The research group received integrated care, while the control group received conventional care. The incidence and severity of iodine contrast extravasation and patient satisfaction with nursing care in the two groups were compared. The incidence of iodine contrast extravasation in the research group (0.27%) was lower than in the control group (0.94%), and the nursing satisfaction rate in the research group (99%) was higher than in the control group (90%). The differences were statistically significant (P &lt; 0.05). The use of integrated care in CT enhancement examinations can effectively reduce the incidence of iodine contrast extravasation and improve patient satisfaction with nursing care.

MRI findings of iatrogenic extravasation of gadolinium-based contrast agents in patients with cancer.

Magnetic resonance imaging (MRI) findings after subcutaneous extravasation of gadolinium-based contrast agent (GBCA) have not been investigated in detail. To present the MRI findings of iatrogenic extravasation and to evaluate the characteristic findings. In this retrospective study of 16,039 patients with cancer, 11 patients had significant extravasation of macrocyclic GBCA, and 7 of the 11 had MRI of the injection site. Characteristic MRI findings as well as symptoms and changes over time were evaluated. The forearms or antecubital fossa felt cold in all seven cases, and 3 (43%) patients felt pain at the injection sites. Fat-suppressed T1-weighted images showed a mosaic pattern of the extravasate with mixed high and low signal in the subcutaneous tissue in 7 (100%) cases. Contrast enhancement of the fascia toward the proximal portion was observed in 3 (43%) cases. There was no subfascial deep extension. On T2-weighted images, GBCAs were observed as low-signal fluid collection on the fascia, with linear spread to the dermis and subcutaneous tissue. Four patients underwent daily MRI scans, all of whom had their contrast disappeared within three days. Macrocyclic GBCA disappears from subcutaneous tissue quickly after extravasation and is unlikely to cause serious sequelae.

Blush in Lung Contusions Is Not Rare and Has a High Risk of Mortality in Patients With Blunt Chest Trauma.

BackgroundPatients with blunt chest trauma have a high mortality rate. The assessment of blush in hepatic and splenic trauma is important for determining the need for emergency hemostatic interventions. However, the frequency and importance of blush in lung contusions are unknown. Therefore, this study aimed to evaluate the frequency of blush in patients with lung contusions and elucidate the relationship between blush and the clinical outcomes of patients with blunt chest trauma.Materials and MethodsIn this retrospective observational study, we enrolled patients with an injury severity score of 16 or higher and a chest abbreviated injury scale (AIS) score of 3 or higher who were admitted to the emergency department of Hokkaido University Hospital from January 1, 2003, to December 31, 2016. Blush was defined as active extravasation of an intravascular contrast agent recognized on contrast-enhanced computed tomography. The date of trauma, trauma severity, treatments, and outcomes were obtained from the patients’ electronic medical records.ResultsDuring the study period, 83 patients had severe lung contusions and 13 had blush. In-hospital mortality of patients with blush was significantly higher than that of patients without blush (53 vs. 10%, P < 0.001). Patients with blush required thoracic drainage more frequently (100 vs. 71%, P < 0.001) and support through mechanical ventilation more often (100 vs. 64%, P < 0.001) and for a longer duration (median duration, 0 vs. 25 days, P = 0.001) than patients without blush.ConclusionsOur study revealed that blush in lung contusions was not rare and was associated with a high risk of mortality in patients with severe blunt chest trauma. Clinicians should not hesitate to intervene if blush is detected in a lung contusion of a patient with blunt chest trauma.

Safety and efficacy of transcatheter arterial embolization for management of refractory hematuria of prostatic origin

ObjectiveTo estimate the safety and efficacy of transcatheter arterial embolization (TAE) in the treatment of refractory hematuria of prostatic origin (RHPO). MethodsThis retrospective study included 23 patients who underwent TAE for RHPO between May 2013 and August 2021. Technical and clinical success rates were calculated, and arteriogram findings and complications were detected. ResultsEmbolization was performed 24 times in 23 patients. Technical success was achieved in 24/24 (100%) embolization procedures. Contrast agent extravasation was detected during 2 of the 24 angiographic procedures. Bilateral embolization was performed in 23 (95.8%) of the 24 procedures. The clinical success rate was 21/23 (91.3%), and hematuria stopped 1–4 days after TAE. No serious complications were observed. ConclusionTAE is a safe and effective minimally invasive technique for treating patients with RHPO.

Purpose: Description of a clinical case of surgical treatment of nontraumatic rupture of the renal calyx fornix, with extravasation of contrast agent into the perirenal tissue. Material and methods: Provided surgical treatment to a patient with spontaneous rupture of the calyx fornix on the background of ureteral obstruction caused by a stone. Results: Considering the concomitant pathology in the form of obstructive pyelonephritis, it was decided to drain the upper urinary tract with an external stent. After 7 days, the patient underwent replacement of the external stent with the internal one. The patient underwent antibacterial, anti-inflammatory therapy, with a positive clinical, laboratory effect. Retrograde pyelography was performed 7 days later. Violations of the integrity of the cavity system of the left kidney were not revealed. The external stent was replaced with an internal one. After 2 months, ureterolitho-extraction was performed on the left. Conclusion: From the data of the world literature and our own experience, we can conclude that spontaneous, non-traumatic rupture of the fornix is an extremely rare, urgent urological disease. We assume that the development of this process is similar to the Burhave syndrome, since smooth muscle fibers are present in the esophagus as well as in the esophagus. In order to provide immediate assistance to such patients, it is necessary to urgently drain the upper urinary tract. Timely diagnosis of this condition requires rapid tactics in the form of immediate ultrasound, radiological diagnostic methods, as well as computed tomography. The operational manual on the removal of stones in this case should be postponed for 2–3 weeks.

Recurrent umbilical varix rupture with hemoperitoneum: a case report and review of literature

BackgroundNon-traumatic hemoperitoneum was a rare event with the risk of sudden death. Spontaneous rupture of hepatocellular carcinoma is the most intuitive diagnosis when hemoperitoneum occurs in cirrhotic patients who are not regularly followed up. However, other etiologies of hemoperitoneum, such as intra-abdominal varix rupture, should be kept in mind.Case presentationA 44-year-old man with alcoholic liver cirrhosis, Child–Pugh B was sent to our emergency department (ED) because of recurrent abdominal pain and hypovolemic shock. He had similar symptoms one month ago and was diagnosed as hepatocellular carcinoma (HCC) rupture with hemoperitoneum, therefore he underwent trans-arterial embolization (TAE). However, the follow-up magnetic resonance imaging (MRI) showed less possibility of hepatocellular carcinoma. Contrast enhanced abdominal computed tomography (CT) showed possible umbilical vein contrast agent extravasation. Exploratory laparotomy confirmed the diagnosis of rupture umbilical varix with hemoperitoneum.ConclusionAlthough umbilical varix rupture is a rare cause of hemoperitoneum, it should be kept in mind in cirrhotic patients with unexplained hemoperitoneum.

The Effect of Enhanced Multislice Spiral Computed Tomography Scan in Nursing

Purpose. Side effects such as local pain and tissue swelling will affect the effect of enhanced multislice spiral computed tomography (CT) scan. The aim of this study was to explore the effect of imaging nursing in enhanced CT scan for heart patients. Method. We selected 86 heart patients to receive enhanced multislice spiral CT scan as the research objects and randomly divided them into the test group and the control group. Imaging nursing and traditional nursing intervention was taken to compare the nursing effects of the two groups. Results. The incidence of unpleasant mood in the test group was 9.3%, which was significantly lower than that in the control group. The nursing satisfaction of the test group was 97.7%, which was much higher than the 79.1% of the control group. At the same time, the visual analogue scale (VAS) score of the test group and the incidence of contrast agent extravasation were significantly lower than those of the control group. Conclusion. The use of imaging nursing for heart patients with enhanced multislice spiral CT scan can reduce the patient’s unhealthy mood and improve nursing satisfaction, and the nursing effect is ideal.

Computed tomography and endovascular occlusion in diagnosis and treatment of spontaneous bleeding into soft tissues of abdominal, chest wall and retroperitoneal space in patients with COVID-19

To investigate the results of therapeutic and prophylactic endovascular hemostasis of spontaneous bleeding into soft tissues of abdominal, chest wall and retroperitoneal space in patients with COVID-19. We retrospectively studied 35 patients with COVID-19 complicated by spontaneous bleeding into soft tissues of abdominal, chest wall and retroperitoneal space. According to CT data, the volume of hematoma was 1193.4±706.1 ml. In all patients, CT signs of ongoing bleeding were detected. Moreover, contrast agent extravasation in all phases of examination was established in 15 patients. In other ones, extravasation was detected in late phases or study phase was not identified. All patients underwent angiography. Ongoing bleeding was detected in 12 (34.3%) patients (group 1). They underwent embolization of the target vessel. In 23 patients, bleeding was not established during angiography. Of these, 13 ones underwent prophylactic embolization (group 2). No embolization was carried out in 10 patients (group 3). All groups differed in hematoma localization and COVID-19 severity. Fourteen (40%) patients died in postoperative period. Mortality was similar in all groups. The most common cause of death was progressive respiratory failure following pneumonia. The last one was established by autopsy in 10 (71.4%) patients. Angiography confirmed MR signs of contrast agent extravasation in 34.3% of patients. In case of extravasation in all CT phases, ongoing bleeding was confirmed in 66.7% of patients. Endovascular embolization is effective for arterial bleeding into soft tissues. However, large-scale studies are needed to assess the effect of this technique on survival.

Background: Peptic ulcer perforation is a complication of peptic ulcer disease frequently encountered in the emergency departments. Although there are many treatment options ranging from nonoperative treatment to wide resections, surgery is the first treatment option in peptic ulcer perforations. In this article, we aimed to present the feasibility of nonoperative treatment in appropriate and selected cases. Materials and Methods: The data of the patients who were not operated but provided with medical treatment in our clinic between January 2016 and July 2018 were evaluated retrospectively. Results: Three of the patients were male, one of them was female, and the mean age was 58 years (range, 35-79). On physical examination, there were no signs of acute abdomen, only tenderness was observed in the right upper quadrant and epigastric region. Standing direct abdominal radiography revealed free air under the diaphragm in three patients. The diagnosis was made with tomographic findings in one patient. No contrast agent extravasation was observed in any patient. The patients were examined intermittently by a specialist surgeon, oral intake was permitted on the 4th day of hospitalization, and they were discharged on the 7th day on average. Conclusion: Peptic ulcer perforation is a condition, which is still common and requires emergency surgery in case of acute abdomen development. Although the time elapsed after perforation is a factor affecting mortality and morbidity, nonoperative treatment is a method that can be successfully applied under strict physical examination and laboratory follow-up in appropriate and selected patients without diffuse peritonitis and extravasation on CT with water-soluble contrast agent.
