Pulmonary sequestration is a congenital pulmonary malformation in which non-functioning lung tissue receives systemic arterial blood supply and does not communicate with the adjacent tracheobronchial tree. Extralobar pulmonary sequestration presenting with torsion: a case report and review of literature

Posterior mediastinal masses have widely variant presentations and a broad differential diagnosis. We describe an adolescent boy presenting with a posterior mediastinal mass, associated with abdominal pain and pleural effusion. Radiologic, operative, and pathologic findings are reviewed. This patient was found to have torsion of an extralobar pulmonary sequestration, of which only 2 other cases have been reported in the literature. Although radiologic findings of a systemic arterial supply to the mass may be absent, with patients presenting with this constellation of symptoms, a high index of suspicion of a sequestration should be maintained.

Extralobar pulmonary sequestration presenting as hemothorax

Symptomatic extralobar pulmonary sequestration (EPS) is extremely rare. Herein, we report two male patients (3 and 16 years of age) with EPS presenting as hemothorax. Thoracotomic resections of the sequestrated lungs were uneventful. Pathologic examinations revealed hemothorax caused by circulatory disorders within the EPSs.

Intrapericardial Extralobar Pulmonary Sequestration Presenting as a Prenatal Intrathoracic Mass

An intrathoracic mass, which persisted during the remaining pregnancy, was first seen during routine ultrasound examination performed at 20 weeks gestation. After birth, the child was asymptomatic. Echocardiography showed the mass to be located intrapericardially. The mass was electively resected via sternotomy 3 weeks after the birth. Microscopic examination showed normal lung tissue surrounded by pleura corresponding to the diagnosis of extralobar pulmonary sequestration. To the authors’ knowledge, this is the earliest described detection of such a lesion. Furthermore, this article reports the unique finding of a feeding vessel from the right pulmonary artery.

Intra-abdominal extralobar pulmonary sequestration presenting antenatally as a suprarenal mass

Extralobar pulmonary sequestration is a rare pulmonary parenchymal anomaly which rarely may be present in an intra-abdominal location. The authors report a case of intra-abdominal extralobar pulmonary sequestration which presented to us as an antenatally diagnosed suprarenal mass and was worked up as such. The diagnosis was revealed only at laparotomy. Intra-abdominal extralobar pulmonary sequestration should also be kept in differential diagnosis in cases of masses in the suprarenal location, especially on the left side.

Extralobar Pulmonary Sequestration Presenting as a Mediastinal Malignancy

Pulmonary sequestration is an uncommon congenital anomaly usually diagnosed in childhood. It frequently presents as a lower lobe mass with symptoms from vascular shunting, anatomic impingements, or associated anatomic defects. This case report describes an adult with asymptomatic extralobar sequestration involving the mediastinum and left upper lobe. The unusual location and radiographic appearance, suggestive of malignancy, led to prompt surgical exploration. We review the literature on this topic, focusing on anatomic variability and difficulties with preoperative diagnosis.(Ann Thorac Surg 1997;63:1169–71)

Extralobar Pulmonary Sequestration Presenting as an Anterior Mediastinal Tumor in an Adult

Extralobar pulmonary sequestration (EPS) is a rather uncommon congenital anomaly. Most patients are diagnosed in their early life or during the first decade because of the early appearance of symptoms, including feeding difficulty, cyanosis, and dyspnea, or because of symptoms arising from the associated congenital abnormalities. Extralobar pulmonary sequestration is more often found between the lower lobe and the diaphragm and is usually associated with other congenital abnormalities, but the case reported herein differs in these respects. In this report, an incidental finding of EPS is described in a 30-year-old Chinese man, where EPS presents as an anterior mediastinal mass roentgenographically. The mass was attached to the right suprahilar region by a fibrovascular pedicle which contained a small elastic artery, veins, and nerve bundles.
