The complications of extracorporeal shockwave lithotripsy (ESWL) occur in a few patients. In addition, serious renal trauma is extremely rare. we report a severe complication of kidney trauma after receiving twice ESWL.

Stenting versus non-stenting before extracorporeal shock wave lithotripsy for proximal ureteric stones: A prospective interventional study

Background: Extracorporeal shock wave lithotripsy (ESWL) has become the first-line treatment for stones in the upper urinary tract measuring <2cm. It is the minor invasive treatment for calculi of the upper urinary tract. Aims and Objectives: This study aims to identify the effect of the presence and absence of a ureteric stent on the outcome of ESWL in patients with proximal ureteric stones. Materials and Methods: This was a prospective cohort study done on 300 patients and wasdivided into two equal groups of 150 each. Group A patients were selected for ESWL with DJ stent, and Group B patients werefor ESWL without DJ stent. Non-contrast CT KUB was done as a functional study along with plainX-ray KUB and ultrasound before the intervention. All treatments were done with Direx Compact XL (electromagnetic generator) device as an outpatient procedure. Patients were followed up radiographically at 15 days, 1 month, 2 months, and in 3 months. Hematuria, steinstrasse, fever, ureteric colic, lower urinary tract symptoms (LUTS), stone clearance, number of ESWL sessions, and secondary procedures were recorded for each group. DJ stent was taken out when the stone disappeared or at the end of 3 months. Variables were compared with Chi-square test. Results: The overall stone-free rate at 3 months was 90.6% (272/300). Clearance after the first sitting, second sitting, and the third sitting wasnot significant between the two groups(P>0.05).There was no significant difference between the two groups in regard to clearance according to the size of the stones. A total of 9.3%(28/300) of patients did not have a successful outcome and were termed as ESWL failures. Post- ESWL complications were significant between the both groups in terms of hematuria and ureteric colic. All the ureteric colic patients had a stone size larger than 13 mm, P=0.0005, which was more in the non-stented (Group B) patients. Most of the lower urinary tract symptoms were seen in stented (Group A) patients, P<0.0001 significant between the two groups, and dysuria was also significant between the two groups with P=0.0033. Conclusion: Pre-ESWL ureteral stenting provides no additional benefit over no stent in situ, although ureteral stents are associated with more irritative symptoms.

The use of an artificial neural network in the evaluation of the extracorporeal shockwave lithotripsy as a treatment of choice for urinary lithiasis

ObjectiveArtificial neural networks (ANNs) are widely applied in medicine, since they substantially increase the sensitivity and specificity of the diagnosis, classification, and the prognosis of a medical condition. In this study, we constructed an ANN to evaluate several parameters of extracorporeal shockwave lithotripsy (ESWL), such as the outcome and safety of the procedure. MethodsPatients with urinary lithiasis suitable for ESWL treatment were enrolled. An ANN was designed using MATLAB. Medical data were collected from all patients and 12 nodes were used as inputs. Conventional statistical analysis was also performed. ResultsFinally, 716 patients were included in our study. Univariate analysis revealed that diabetes and hydronephrosis were positively correlated with ESWL complications. Regarding efficacy, univariate analysis revealed that stone location, stone size, the number and density of shockwaves delivered, and the presence of a stent in the ureter were independent factors of the ESWL outcome. This was further confirmed when adjusted for sex and age in a multivariate analysis. The performance of the ANN at the end of the training state reached 98.72%. The four basic ratios (sensitivity, specificity, positive predictive value, and negative predictive value) were calculated for both training and evaluation data sets. The performance of the ANN at the end of the evaluation state was 81.43%. ConclusionOur ANN achieved high score in predicting the outcome and the side effects of the ESWL treatment for urinary stones.

Success of extracorporeal shock wave lithotripsy based on CT texture analysis.

The aims of the study were to evaluate whether computerised tomography texture analysis (CTTA) based on non-contrast computed tomography (NCCT) has predictive value for the success of extracorporeal-shockwave lithotripsy (ESWL) in upper urinary tract stones (UUTS). This study included 156 of 356 patients undergoing ESWL for UUTS sized 0.5-2cm from 2015 to 2019. Patients with congenital kidney anomalies, radiolucent stones, multiple stones, treated for upper urinary tract stones previously and lower pole stones were excluded from study. The number of ESWL sessions of the patients was as follows: 78 (50%) patients had 1 session, 30 (19.2%) patients had 2 sessions and 48 (30.8%) patients had >2 sessions. First- and second-order CTTA properties of patients' UUTS were evaluated using texture analysis software (LIFEx Software). Other clinical features such as Hounsfield Unit (HU), initial stone size, body-mass index (BMI) and skin to stone distance (SSD) was recorded. The patients were divided into two groups according to ESWL success. Cases with residual stones larger than 4mm were considered failed cases. BMI, the standard deviation of HU, SSD, skewness, kurtosis, entropy and all second-order statistics were found to be statistically different between the two groups except for correlation (P<.05). Multivariate analysis showed longer SSD and four new parameters of CTTA (kurtosis, entropy, dissimilarity and energy by the distribution of pixel grey levels in the UUTS) to be significant predictors for unsuccessful ESWL outcomes. SSD and second-order CTTA properties (dissimilarity and energy) had an area under ROC curve of 0.802, 0.850 and 0.824 at a 95% confidence interval. ESWL success rate in all patients was 76.9%. CTTA can help select patients who will undergo ESWL for upper urinary tract stones. Thus, we can reduce treatment costs and ESWL complications by preventing unnecessary ESWL applications.

Steinstrasseafter Extracorporeal Shockwave Lithotripsy: Aetiologyand Management

Background: Renal stone is the most common disease managed by an urologist. Extracorporeal shock wave lithotripsy (ESWL) is one of main modality of management of renal stone. Steinstrasse one of complication of ESWL is not an uncommon event. Steinstrasse may be managed with conservative management, ESWL or ureteroscopy (URS) or open surgery. We describe our experience of management of steinstrasse patients in IGMC Shimla.Subjects and Methods:Extracorporeal shockwave lithotripsy (ESWL) is given to patients presenting with renal and upper ureteric stones. This study included diagnosis and management of 7 patients developing steinstrasse after ESWL.Results:All steinstrasse developed in patients of renal stone and size more than 2cm. Four patients had pain after ESWL and 3 patients were asymptomatic. Four patients had pre-ESWL DJ stenting in view of large stone, 2 patients did not require DJ stenting and one patient required DJ stenting after persistent pain. Five patients required URS at 3 weeks after an incomplete response to conservative treatment. One patient received ESWL for fragments in ureters, and 1 patient didn't require intervention.Conclusion: Stone size and site were the significant factors predicting steinstrasse formation. URS and ESWL are very effective interventions for patients not responding to conservative management

Early complications of extracorporeal shockwave lithotripsy in the records of the Department of Paediatrics, Nephrology and Allergology of the Military Institute of Medicine - preliminary results.

The aim of the study was to analyse the frequency and type of early complications of extracorporeal shock wave lithotripsy (ESWL) and to assess the safety of the procedure among paediatric patients. The study is a retrospective analysis of all ESWL procedures carried out due to urolithiasis in 2009-2015 in the Department of Paediatrics, Nephrology and Allergology of the Military Institute of Medicine. Medical records of 170 children were analysed: 94 girls (55.3%) and 76 boys (44.7%) aged 1 to 18 years. Out of the 272 procedures performed, 247 were included in the study. Among 233 effective ESWL treatments, complications were noted in 35 (15.02%) procedures: among 10 girls (40%) and 15 boys (60%). In 25 cases (10.73%) surgical intervention was necessary due to urinary obstruction caused by a fragment of a disintegrated stone. Urinary tract infection was present among 3 patients (1.29%) who required surgical treatment. Nonsurgical complications included 7 urinary tract infections (3%) and 3 cases of severe abdominal pain (1.29%). Complications such as perirenal haematoma, subcapsular haematoma or ureteral perforation were not observed. The vast majority of complications observed in the study were associated with urinary obstruction caused by partial blockage of the ureter with kidney stone fragments (“steinstrasse”). Despite the complications that were documented, ESWL should be considered a safe procedure.

Efficacy and safety of extracorporeal shock wave lithotripsy for chronic pancreatitis

Introduction: There is still uncertainty regarding the efficacy and optimal modalities of extracorporeal shock wave lithotripsy (ESWL) in the treatment of chronic pancreatitis. The aims of the present study were to assess the safety and the efficacy of ESWL, either alone or followed by therapeutic endoscopic retrograde cholangiopancreatography (adjuvant ERCP) and to determine predictive factors of efficacy, in a real-life setting.Patients and methods: This study included all consecutive patients who underwent an ESWL in a single University Hospital between 2001 and 2012. The indication for ESWL was obstructive stone(s) of the main pancreatic duct resulting in either painful chronic pancreatitis or recurrent acute pancreatitis. Success was defined by resolution of pain, no analgesic treatment, no acute pancreatitis and no surgical treatment for chronic pancreatitis 6 months after the ESWL.Results: One hundred and forty-six patients were studied; 6/146 (4%) had a complication of ESWL. Among the 132 patients in whom follow-up was completed, 91 (69%) had an adjuvant ERCP. After 6 months of follow-up, 100/132 (76%) patients achieved success. In multivariate analysis, the single significant predictive factor of the success of the ESWL treatment was chronic pain (p = 0.03). Patients who had chronic pain and needed opioid treatment had less chance of success than patients without chronic pain (OR 95%CI 0.31 [0.07–1.14]). We found no difference in the success rates between patients who underwent adjuvant ERCP and those who had ESWL only (p = 0.93).Conclusion: This study shows that the ESWL is a safe and effective treatment for patients with chronic pancreatitis and obstructive stones within the main pancreatic duct. Systematic association with therapeutic ERCP appears to provide no additional benefit and is therefore not recommended.

Diagnostic and prognostic role of computed tomography in extracorporeal shock wave lithotripsy complications.

Purpose:To evaluate the role of multidetector computed tomography (MDCT) in recognizing the complications of extracorporeal shock wave lithotripsy (ESWL) and providing a prognostic grading system for the therapeutic approach.Materials and Methods:A total of 43 patients who underwent ESWL because of urinary stone disease were assessed by 320-row MDCT examination before and after ESWL. Pre-ESWL CT unenhanced scans were performed for diagnosing stone disease. Post-ESWL CT scans were acquired before and after intravenous injection of contrast medium searching for peri-renal fluid collection or hyper-density, pyelic or ureteral wall thickening, blood clots in the urinary tract, peri- or intra-renal hematoma or abscess, active bleeding. A severity grading system of ESWL complications was established.Results:Patients were affected by renal (n = 36) or ureteral (n = 7) lithiasis. Post-ESWL CT examination detected small fluid collections and hyper-density of peri-renal fat tissue in 35/43 patients (81%), pyelic or ureteral wall thickening in 2/43 (4%), blood clots in the urinary tract in 9/43 (21%), renal abscesses or hematomas with a diameter of <2 cm in 10/43 (23%), large retroperitoneal collections in 3/43 (7%), active bleeding from renal vessels in 1/43 (2%). Mild complications were found in 30 cases; moderate in 9; severe in 4. The therapeutic choice was represented by clinical follow-up (n = 20), clinical and CT follow-up (n = 10), ureteral stenting (n = 9), drainage of large retroperitoneal collections (n = 3), and arterial embolization (n = 1).Conclusion:MDCT plays a crucial role in the diagnosis of urolithiasis and follow-up of patients treated with ESWL recognizing its complications and providing therapeutic and prognostic indications.

Outcomes of extra corporeal shock wave lithotripsy in renal and ureteral calculi.

Since the introduction in early 1980s, Extracorporeal Shockwave Lithotripsy (ESWL) became the accepted first line treatment modality for renal and upper ureteric stones. It is simple, safe and effective noninvasive procedure which can be performed without anaesthesia in outpatient basis. The objective of this study was to determine the efficacy of ESWL to achieve complete stone clearance in the patients with different sizes of renal and upper ureteric stones. The aim of this study was to assess the outcome, efficacy and complications of ESWL in the treatment for renal and ureteric stones in terms of the site and the size of the stone in the patients presented at Dhulikhel Hospital Kathmandu University Hospital. In this prospective study a total of 430 (214 renal and 216 ureteric) cases of urinary stone disease in 257 male and 173 female patients treated by ESWL at Dhulikhel Hospital, Kathmandu University Teaching Hospital during time period of May 2010 to June 2012 were included. Data of patients with renal and ureteric stones were evaluated for stone site, size, and number of sessions. Data were analyzed using spss 13.0. Out of 430 cases, the overall stone free rate in after 1st session was 341 (79.3%) at one month and in three months follow up (3 sessions) it was increased up to 414 (96.3%). In 16 (3.7%) patients treatment was failed. Average size of the stone was 12.24 (SD± 3.65) mm. Stone free rate was 154 (72%) in the case of renal and 187 (86.6%) in the case of ureteric stones in first session. In three months follow up (three sessions) it was 204 (95.4%) and 210 (97.2%) respectively for renal and ureteric stones. In relation to size the stone free rate in <10 mm, 10-15 mm and > 15 mm was 97%, 97% and 90%. ESWL is the first line preferred choice for renal and upper ureteric stones which provides the maximum stone free rate in the case of stone size smaller than 1.5 cm.

Thoracic spinal cord epidural hematoma after extracorporeal shock wave lithotripsy

Extracorporeal shock wave lithotripsy (ESWL) is a well-established, safe, and effective therapeutic alternative to surgical treatment for urolithiasis. Complications of ESWL do occur in a small number of patients, and when they do, they typically involve the kidney. Formation of a spinal epidural hematoma is an extremely rare complication after ESWL for kidney stone removal. The authors present the case of a 61-year-old man in whom a large spinal epidural hematoma developed after ESWL. They discuss the possible pathogenic mechanism of such a complication.

Life-Threatening Complication after Extracorporeal Shock Wave Lithotripsy for a Renal Stone: A Hepatic Subcapsular Hematoma

Extracorporeal shock wave lithotripsy (ESWL) has revolutionized the management of urolithiasis since it was first introduced in 1980. ESWL is a well-established, safe and effective therapeutic alternative to surgical treatment for urolithiasis. Complications of ESWL do occur in a small number of patients, and when they do, they typically involve the kidney. We present a case of a young female patient who developed a huge hepatic subcapsular hematoma accompanied by hypovolemic shock after ESWL for a 9 mm stone in the right kidney. The hematoma measured 13×6 cm. Conservative care with no surgical intervention was chosen because there was no evidence of active bleeding on the computed tomography. After conservative therapy, the hematoma was gradually absorbed and the patient was discharged.

Kidney Rupture After Extracorporeal Shockwave Lithotripsy: Report of a Case

Background: Complications of extracorporeal shockwave lithotripsy (ESWL) occur in a small number of patients, although serious injury is rare. Objective: To report the serious complication of kidney rupture during ESWL. Case Report: A 65-year-old man was transferred to the Emergency Department (ED) with right flank pain. He had undergone ESWL for the right renal stone at a regional hospital 2 days earlier. Flank pain developed immediately after ESWL and was not spontaneously relieved. Computed tomography scan performed at the regional hospital showed an extensive right perinephric hematoma. When the patient arrived at the ED, his vital signs were unstable but were stabilized with fluid resuscitation and transfusion. Conservative care with no nephrectomy was chosen because there was no evidence of active bleeding on Doppler ultrasound examination. He was uneventfully discharged on the 31st hospital day without further complications. Conclusion: Although it is rare, patients may present with kidney rupture or hypotension after ESWL.

A Death Due to Perirenal Hematoma Complicating Extracorporeal Shockwave Lithotripsy

Perirenal hematoma is an occasional complication of extracorporeal shockwave lithotripsy (ESWL) which does not usually require treatment. A 79-year-old woman died 23 h after ESWL. Forensic autopsy was performed to determine whether medical treatment contributed to her death. The cause of death was hemorrhagic shock due to massive hematoma from a ruptured small vein in the perirenal adipose capsule. No injury to other organs was found and the patient had neither coagulation abnormality nor venous disease. Perirenal hematoma can easily be diagnosed with abdominal sonography, if pain or symptoms of anemia develop. Doctors must be aware of the possibilities of severe renal hematomas after ESWL.

Metodología y límites de la ureteroscopia ambulatoria

To demonstrate the efficacy of ambulatory ureteroscopy under sedation-analgesia as diagnostic and/or therapeutic procedure for the upper urinary tract diseases, neoplasias, stenosis, stones.We analyze the results of a series of diagnostic and/or therapeutic procedures in 1243 patients divided into five groups: 1. Work up for filling defects with positive cytology and hematuria, 36 cases. 2. Treatment of urinary stones, 1135 cases. 3. Treatment and follow-up of upper urinary tract tumors, 19 cases. 4. Foreign body extraction, 27 cases. 5. Dilation/ section of ureteral stenosis, 26 cases. We perform the procedure under local anesthesia (urethral xylocaine gel), sedation with midazolam and analgesia with remifentanil perfusion (0.08-0.20 mcg kg/minute); the operation is subdivided into three steps: access to the ureter, progression/ureteral examination, and diagnostic and/or therapeutic actions.Fifteen percent of the patients presented pain or intolerance at the start or during the procedure, and they progress to general anesthesia. 10% of the cases have a hospital admission longer than six hours. Ureteroscopy was effective as a diagnostic procedure in 30/36 cases. Ureteroscopy was indicated as elective treatment in 833 cases of ureteral stones (54 lumbar; 248 iliac; 531 pelvic), achieving good results in 93% of the patients; it was indicated for failures or complications of extracorporeal shockwave lithotripsy in 302 cases (73 lumbar; 83 iliac; 146 pelvic), with good results in 98% of the patients. Ureteroscopy was used to treat conservatively low-grade ureteral tumors of the pelvic ureter in 12 cases, or as palliative therapy (7 cases). From the 26 cases of ureteral stenosis, 8 underwent ureterotomy and 18 balloon dilation, leaving a ureteral catheter for 4-6 weeks, obtaining good results in 21 cases.Ureteroscopy is an effective technique in 94% of the cases, and it can be performed as an outpatient procedure for diagnosis or treatment of urinary stones and ureteral stenosis.

Clinical Experience of Extracorporeal Shock Wave Lithotripsy (ESWL) using a Dornier Compact S®

Purpose: We analyzed the efficacy and safety of extracorporeal shock wave lithotripsy (ESWL) for treating patient with urinary tract calculi with using a Dornier Compact S lithotriptor. Materials and Methods: We retrospectively reviewed the records of all the patient who had urinary calculi and who were treated by ESWL between August 1, 1996 and August 1, 2001. The location and sizes of the stones, the number of sessions, the success rate, the causes of failure and the complications of ESWL were analyzed. The definition of successful treatment was no calcification on the X-ray (plain film, KUB) or residual fragments ≤3mm in size. Results: Of the 470 cases, there were 157 (33.4%) and 313 (66.6%) cases of renal stones and ureteral stones, respectively. The total success rate was 94.3% (445/470), with success rates of 97.4%, 90.8%, 82.1% and 88.9% for stone sized 31mm, respectively. Conclusions: The results showed that ESWL is a highly effective and minimally invasive treatment modality as the 1st therapeutic option for urinary stones. The role of ureteral stent placement in the prevention of Steinstrasse.

Steinstrasse constitutes a potentially serious complication of extracorporeal shock-wave lithotripsy (SWL). Ureteral stent placement has been used to prevent Steinstrasse after fragmentation of larger stones. However, particularly more recently, its preventive efficacy has been questioned. The aim of this study therefore was to analyze the role of ureteral stent placement in the prevention of Steinstrasse. We analyzed data of 1087 patients who had been treated with a Wolf Piezolith 2300 in the General Infirmary in Leeds/UK for stones ranging from 10 to 95 mm in diameter. The incidence of Steinstrasse was 6.3%. The likelihood was significantly correlated with the stone size and was significantly less in patients with stones >20 mm if a stent had been inserted prior to SWL. Moreover, in these patients, the risk of acute clinical symptoms in the event of Steinstrasse was greatly reduced, and the treatment could be continued safely in the majority of cases (86%). Treatments of the Steinstrasse itself with SWL resulted in its clearance in most of the cases (78%). It was always possible to clear even extended persistent Steinstrasse by laser lithotripsy. These results provide a clear indication for the pre-SWL insertion of a ureteral stent in patients with stones >20 mm in diameter. In the event of Steinstrasse, SWL of the collection should be tried before more invasive endourologic procedures are considered.

Splenic rupture and abscess after extracorporeal shock wave lithotripsy

Extracorporeal shock wave lithotripsy (ESWL) is the first choice for the treatment of most urinary stones. Complications of ESWL can be separated into two groups: those related to the administration of the shock waves and those related to fragmentation and elimination of the stone’s particles. We report a rare case of splenic trauma followed by abscess after ESWL.

Cholecystolithiasis: Three years experience with ultrasound-guided electromagnetic shock-wave lithotripsy

In a three-year period, 207 patients with cholecystolithiasis were referred to an interdisciplinary team at our hospital to evaluate the possibility of non-surgical treatment. Fifty-eight patients were found eligible for extracorporeal shock-wave lithotripsy (ESWL); 50 of them underwent ESWL on an outpatient basis with adjuvant bile acid therapy. Thirty patients (60%) had single stones, 14 (28%) had two or three stones, and 6 (12%) had five-ten stones. The mean total stone volume of all patients was 2,5 cm3, equivalent to a single stone of 1.7 cm diameter; 6 patients (12%) had slightly calcified stones. Successful stone fragmentation (fragments 4 mm, ESWL was considered a failure and elective cholecystectomy was eventually performed or recommended. All patients received adjuvant oral bile acid therapy. The stone clearance rates after 3, 6, 12 and 18 months were 29%, 50%, 83% and 95% in patients with single stones and 22%, 40%, 64% and 88%, respectively, in all patients. The duration of treatment in 32 patients who have cleared all their gallstone fragments to date, was 8.5 months (range: 0.4–20.4 months); all of these patients are also free of their initial symptoms. Sideeffects and complications of ESWL included biliary colic in 17 patients (34%), mild acute pancreatitis in 1 patient (2%) and acute cholecystitis requiring emergency cholecystectomy in another patient (2%). The electromagnetic device and the protocol we used is well suited for ambulatory treatment of patients with a limited stone volume. A fragment size <- 4 mm was usually achieved with 2 treatment sessions and appears to be a reasonable endpoint of ESWL. Although laparoscopic cholecystectomy is now a routine procedure in our hospital, we believe that ESWL remains a valid treatment option in selected patients.

Complications of renal extracorporeal shock wave lithotripsy reviewed

Renal extracorporeal shock wave lithotripsy (ESWL) is a noninvasive and effective method for the treatment of a variety of urinary tract calculi with significantly lower morbidity than both surgical procedures for stone removal (pyelolithotomy/ureterolithotomy) and percutaneous nephrostolithotomy. Though significant, severe complications are rare, ESWL therapy can result in a wide spectrum of tissue injury. This article serves to review the currently known complications of ESWL, which radiologists may encounter, providing an understanding of the potential effects of shock waves on body organs and the resultant functional and morphologic changes that may result.
