Extra-follicular Variant of Adenomatoid Odontogenic Tumor: A Diagnostic Enigma

Adenomatoid odontogenic tumor (AOT) is an uncommon, slow-growing, noninvasive odontogenic tumor mostly in the anterior maxilla with three well-recognized clinico-pathological variants: follicular, extra-follicular, and peripheral. Extra-follicular variant presents as a well-defined, unilocular, radiolucency in between, above, or superimposed on the roots of an erupted tooth. A 19 years female reported with the chief complaint of a loose tooth in the right front region of the upper jaw for 6 months, associated with firm swelling without pain or discharge. On orthopantomogram and cone-beam computed tomography, the lesion appeared as a single, localized, well-defined, roughly oval unilocular radiolucency with flecks of radiopacity integrating radicular and cervical third of 13. Complete surgical enucleation followed by histopathological examination revealed the lesion as AOT. The extra-follicular AOT can cause diagnostic dilemmas and is often misdiagnosed as an odontogenic cyst.

Analysis of 55 cases of adenomatoid odontogenic tumor in an Indian population and review of literature.

Background This study reviews the demographic, clinical and radiographic features of adenomatoid odontogenic tumor(AOT) diagnosed in an Indian population over 50 years and also evaluate and compare follicular AOT(F-AOT) and extra-follicular AOT(EF-AOT).Material and Methods 55 diagnosed cases of AOT from 1971-2020 were studied retrospectively. The data regarding the age, sex, location, variant of AOT, duration, clinical features, radiographic appearance, treatment and recurrence were collected and analysed.Results Of the 722 odontogenic tumors diagnosed, 7.6% were AOTs with higher prevalence of extra-follicular (67.3%) than follicular (32.7%) variant. All the tumors were intraosseous with a marked predilection for maxilla over mandible, ratio 2:1. The patients mean age was 19.8 years with slightly higher female predilection (male:female ratio - 1:1.5). The anterior region (76.4%) was more frequently affected and entire quadrant was involved in 21.8% cases. Clinically, asymptomatic, slow-growing swelling was seen in 81.8% cases with duration of 15 days to 10 years. Radiographically, AOT appeared as well-corticated radiolucent lesion. Canine was the most commonly impacted tooth. Recurrence was seen in 3 cases.Conclusions Interestingly, in this series extra-follicular was twice more common than follicular AOT. Few cases involved the entire quadrant or crossed the midline of either jaws. Maxillary extrafollicular adenomatoid odontogenic tumor: root resorption and involvement of the maxillary sinus and nasal cavity

Adenomatoid odontogenic tumor (AOT) is a benign, slow-growing lesion and it is considered the fourth most common odontogenic tumor. AOT affects more frequently the young subjects with a predilection for the second and third decades of life. We present here the case of a 36-year-old man affected by large extrafollicular AOT which caused roots resorption of multiple teeth. The lesion involved the entire right maxillary bone and extended into the right maxillary sinus up to the orbital floor and the nasal cavity. Intra-oral surgical excision of the AOT and functional endoscopic sinus surgery led to complete clinical healing in absence of local recurrences.

ADENOMATOID ODONTOGENIC TUMOR: AN UNUSUAL PRESENTATION

The adenomatoid odontogenic tumor (AOT) has a prevalence of 3% to 7% among odontogenic tumors and 3 described variants: Follicular, extrafollicular, and peripheral (or gingival). We reported a case of extrafollicular AOT in a 31-year-old melanoderm woman. An increase in volume was observed in the lower left third of the face associated with bulging of the buccal and lingual cortices in the region of elements 34 to 32. The imaging revealed a well-defined unilocular radiolucent area with sclerotic margin, involving periapices of elements 34, 33, and 32, rupture of the vestibular bone cortex in the region, and mild root resorption of units 33 and 32. Histopathologic examination revealed presence of fibrovascular capsule and epithelial cells forming structures such as ductus and solid patter. The treatment was an excisional biopsy. Follow-up of 7 and 15 days with satisfactory postoperative period was performed. Provision will be made every 6 months.

Extrafollicular AOT presenting as a gingival overgrowth: A rare case report

Adenomatoid odontogenic tumor (AOT) is an uncommon, benign, asymptomatic, slow-growing tumor with marked predilection for occurrence in the anterior maxilla. It represents 3% of all odontogenic tumors. There are three clinicopathological variants of AOT, namely, follicular, extrafollicular, and peripheral. This paper describes in detail a rare case of an extrafollicular adenomatoid odontogenic tumor occurring in mandible and presenting as gingival growth.

Case reports of a typical extrafollicular adenomatoid odontogenic tumor of maxilla

Adenomatoid odontogenic tumor is a rare benign odontogenic tumor comprising of 3% to 7% of all Odontogenic tumors. It is usually asymptomatic, well circumscribed, slow growing tumor associated often with impacted canine and discovered on routine radiographs. Microscopically this lesion shows a variety of patterns forming sheets, strands, nest or rosette-like structures with ductal pattern with central space surrounded by single layer of cuboidal to columnar epithelial cells with a rim of eosinophilic coagulum which is the characteristic feature of these tumors. In this article we present two extrafollicular cases of this rare tumor occurring in a male and a female patient in the maxillary anterior region with both symptomatic and asymptomatic presentation. Both the tumors were not associated with any impacted tooth. Adenomatiod Odontogenic tumor was confirmed based on histopathology.

Expression of CK14 and vimentin in adenomatoid odontogenic tumor and dentigerous cyst.

Background:Origin of adenomatoid odontogenic tumor (AOT) has long been a controversy, and the issue of it being a neoplasm or hamartoma was a subject of debate for a long time. Earlier it was grouped under a mixed group of odontogenic tumors considering the varying degrees of inductive changes. Recently, the WHO classification states that the presence of hard tissue within AOT was not due to induction but was rather a metaplastically produced mineralization and hence the tumor was reclassified under a group of tumors arising from odontogenic epithelium. This study is an attempt to identify if both epithelial (cytokeratin 14 [CK14]) and mesenchymal (vimentin) markers are expressed in the follicular and extrafollicular variants of AOT and to compare the expression with dentigerous cyst (DC) as this cyst is known to arise from reduced enamel epithelium which expressed CK14. This is done to possibly relate the origin of AOT with reduced enamel epithelium.Aims and Objectives:To study, analyze and correlate the expression of CK14 and vimentin in AOT and DC.Materials and Methods:Retrospective study on paraffin embedded tissues. Sixteen cases of AOT and 15 cases of DC were retrieved from the departmental archives and subjected to CK14 and vimentin immunostaining.Statistical Methods:Measures of central tendency was used to analyze the results.Results and Observations:Ninety percent of cases of follicular AOT (FAOT) and 100% cases of extra-follicular AOTs (EAOTs) showed positivity for CK14 and all cases of DC showed positivity for CK14. Vimentin was positive in 44% and negative in 56% cases of both FAOT and EAOT taken together.Conclusion:The CK14 expression profile in AOT and DC supports its odontogenic epithelial specific nature. The possible role of reduced enamel epithelium and dental lamina in histogenesis of AOT and DC is strongly evident by their CK14 expression pattern.

A rare case of extrafollicular adenomatoid odontogenic tumor in the posterior mandible and ramus treated by decompression

We present a rare case of an extrafollicular adenomatoid odontogenic tumor (AOT) occurring in the posterior mandible and ramus area of a young adult female. The AOT's location, radiographic appearance, and its relationship to adjacent teeth were all unlike the usual presentations. We also present the first report in the English literature on management of AOT by decompression of the lesion as the lone modality of treatment.

Interradicular adenomatoid odontogenic tumor of mandible: A case report

The present case report is of extrafollicular adenomatoid odontogenic tumor (AOT) in the mandibular anterior region, in a 20-year-old female patient. She reported with a painless swelling in the mandibular anterior region to our department of oral and maxillofacial surgery. Fine needle aspiration yielded no fluid. Periapical, panoramic radiograph showed circumscribed radiolucent area with fine calcifications. The lesion was totally enucleated and incisors, canine and premolars of that quadrant were removed. The rarity of AOT may be associated with its slowly growing pattern and symptomless behavior. Therefore, it should be distinguished from more common lesions of odontogenic origin in routine dental examinations.

Adenomatoid odontogenic tumor—Report of a posterior mandibular case with the presence of ghost cells

Adenomatoid odontogenic tumor (AOT) occurs more frequently in the maxillary anterior region. Recurrence of AOT after surgical excision is very rare. In this case report, we describe a left posterior mandibular AOT case that exhibited three recurrences after surgical removal. A 25-year-old female patient was first seen by an oral surgeon in a regional hospital with the chief complaint of a swelling at the left mandibular molar region. The tumor was removed by enucleation. Five years later, the tumor recurred at the same region. The patient was referred to our hospital for treatment, and the recurrent tumor was widely excised. The pathological report was an extrafollicular AOT. Eleven years later, the tumor recurred again. It was removed using segmental mandibulectomy with titanium plate reconstruction. The pathological report was also an extrafollicular AOT. Nine months later, the mandibular bone defect was further reconstructed by a segment of iliac bone graft fixed by a new titanium plate and screws. Twenty-eight years after the initial surgery, the tumor recurred again in the alveolar mucosa covering the graft bone. It was further excised, and the pathological report was a peripheral AOT with the presence of amyloid materials and globules of ghost cells. Our AOT case recurred thrice after two conservative excisions and one more radical surgical resection. The multiple recurrences of the tumor indicate the importance of initial complete resection of the tumor. In addition, our case may represent the first reported AOT with the presence of ghost cells in the tumor tissue.

ADENOMATOID ODONTOGENIC TUMOUR – AN UNUSUAL EXTRAFOLLICULAR VARIANT OF THE POSTERIOR MANDIBLE

Adenomatoid odontogenic tumor (AOT) is a relatively uncommon distinct odontogenic neoplasm. Although this lesion was formerly considered to be the variant of ameloblastoma, its clinical features and biologic behavior indicate, that it is a separate entity. It is a tumor of odontogenic epithelium with duct like structures and with varying degrees of inductive change in the stroma. It is a benign and slow growing tumor which is usually located in the anterior region of the maxilla without pain, and represents 3% of all odontogenic tumors. Most adenomatoid odontogenic tumors (AOTs) occur intra-osseously. They surround the crowns and are attached to the necks of unerrupted teeth in a true follicular relationship, whereas the extrafollicular type has no relation with an impacted tooth, and the peripheral variant is attached to the gingival structures. The aim of this paper is to present a rare case of an extrafollicular AOT occurring in the posterior aspect of the mandible which is distinct.

An Unusual Site of Adenomatoid Odontogenic Tumor Presenting as Periapical (Radicular) Cyst: A Rare Case Report

The adenomatoid odontogenic tumor is a rare odontogenic tumor often misdiagnosed as an odontogenic cyst, constituting only 3% of all odontogenic tumors. Though odontogenic in origin but it appeared as duct like structures, often interspersed throughout the lesion which gives a glandular, i.e. Adenomatoid appearance. Here we are presenting a rare case report of an unusual site of extrafollicular adenomatoid odontogenic tumor in the mandible w.r.t 32, 33, 34 and 35 mimicking periapical disease clinical and radiographically. However, diagnosis of adenomatoid odontogenic tumor should be considered when the clinician is presented with a corticated radiolucency in the anterior lower jaw, especially in teens and young adults.

Adenomatoid Odontogenic Tumor (AOT) Originating in a Unicystic Ameloblastoma: A Case Report

The follicular variant of the adenomatoid odontogenic tumor (AOT) is thought to originate from the reduced enamel epithelium of the dental follicle. The origin of the extra-follicular variant however, remains less clear. This paper presents a case of an extra-follicular AOT, which we believe originated from the epithelial lining of a unicystic ameloblastoma, and reviews the literature. The available evidence seems to indicate that some extra-follicular AOTs might arise as secondary phenomena within pre-existing odontogenic cysts or cystic tumors.
