Longitudinal associations among experiences of sexual assault, posttraumatic stress disorder symptoms, and heavy drinking in young adults.

Prior research with young adults has demonstrated clear associations between experiences of sexual assault, symptoms of posttraumatic stress disorder (PTSD), and alcohol use, but most studies have been cross-sectional or have not considered multiple theoretical pathways to understand these associations. Using six waves of data from a longitudinal cohort sample of 1,719 young adults, we examined associations among experiences of past-year sexual assault (i.e., rape, unwanted sexual touching, and physical intimidation in a sexual way), PTSD symptoms, and the frequency of binge drinking over time, allowing for the exploration of symptom-induced, interpersonal risk, and substance-induced pathways for male and female participants. For both male, βs = 2.84 to 6.55, and female participants, βs = 2.96 to 10.1, higher prior levels of PTSD symptoms were associated with larger increases in binge drinking over time. For female participants, higher prior levels of sexual assault were associated with larger increases in PTSD symptoms over time, βs = 3.48 to 4.25, whereas for male participants, higher prior levels of past-year binge drinking were associated with decreases in PTSD symptoms over time, βs = -2.75 to -0.53. Continued efforts are needed to prevent sexual assault among young adults and address PTSD symptoms among those who experience sexual assault. Interventions that target binge drinking are also needed for individuals who experience PTSD symptoms, especially young adults, to address potentially hazardous drinking before problems escalate and become chronic.

Investigating the Influence of Architectural Environment in Rehabilitation Facilities on the Psychological Well-being of Military Personnel with PTSD symptoms

This article delves into a crucial aspect of this challenge: how the design and layout of physical spaces affect the mental health and overall quality of life of military personnel displaying symptoms of Post-Traumatic Stress Disorder (PTSD). Veterans who have encountered traumatic and life-threatening situations during their service often grapple with emotions like fear, helplessness, and distress. This group is particularly vulnerable to the effects of PTSD, with a significant portion experiencing these symptoms. Recent studies shifted in focus towards understanding and mitigating triggers for PTSD symptoms underscores the importance of investigating how architectural and urban design elements impact the well-being of these individuals. A review of current literature reveals that while progress has been made in understanding the psychological well-being of veterans and the design of rehabilitation centers, much of this research remains fragmented. Existing data mostly pertains to indirect and general guidelines for architectural design and planning. International literature offers a more extensive body of work, encompassing a wide array of aspects related to the creation of stress-reducing environments. It is imperative to consider how various architectural and urban design elements either exacerbate or alleviate PTSD symptoms. Hence, understanding potential connections between these elements and symptom triggers holds critical significance for veterans' well-being. The primary focus of this study involved conducting semi-structured interviews with veterans experiencing PTSD symptoms to identify and formulate recommendations for the design of the interior and exterior environments within rehabilitation facilities. The questionnaire included questions about architectural features that had either positively or negatively affected the veterans' mental well-being. Notably, most of the recommendations derived from previous research align with the preferences expressed by the veterans, considering specific nuances related to the psychological states of individuals following combat experiences. The results underscore the importance of creating a supportive and empathetic social environment that respects the need for privacy (private spaces), fosters transparency and comprehension of the design logic (open spaces), adheres to insulation and acoustic requirements, and incorporates specific structural and interior features in facilities of this kind. Although the study featured a relatively small sample size, the detailed examination of veterans' preferences and their enthusiastic responses validate the relevance of these recommendations. The imperfections found in existing facilities highlight the ongoing necessity for further exploration, additional interviews, and refined proposals for the renovation and reorganization of rehabilitation institutions serving veterans with PTSD symptoms. In summary, this article investigates how architectural environments impact the mental well-being of military personnel with PTSD symptoms. It takes a unique approach by incorporating veterans' own experiences to create recommendations and approaches for designing rehabilitation centers aimed at providing more effective and compassionate support for veterans. This research contributes to an expanding body of knowledge seeking to enhance the well-being and recovery of veterans with PTSD, ultimately aiming for a more comprehensive understanding of the role of architectural design in mental health support.

Intergenerational trauma and war-induced PTSD in Kosovo: insights from the Albanian ethnic group.

War has profound and deep-rooted ramifications for individuals and societies. War-induced post-traumatic stress disorder (PTSD) is highly prevalent in Kosovo. This study aimed to obtain insights into the prevalence of perceived PTSD symptoms and their relation to the traumatic experiences of two generations: parents (survivors of the Kosovo War) and youth (children born after the Kosovo War), with an emphasis on the Albanian ethnic group. These experiences were then compared to understand intergenerational trauma. The study also aimed to identify the factors affecting PTSD prevalence, the role of social support, and the participants' experience with mental health services. A total of 237 Kosovar Albanians (121 parents, 116 youth) from all seven districts of Kosovo were included in this study. Study variables were measured using the PTSD Checklist, the Life Events Checklist, Criterion A, and the Multidimensional Scale of Perceived Social Support. The results revealed that the youth had significantly higher levels of perceived PTSD symptoms and lower levels of perceived support than their parents. Youth whose parents had PTSD were more prone to experiencing PTSD symptoms than those whose parents did not have PTSD. These youth also experienced significantly more traumatic situations, such as exposure to sudden violent death or accidental death, assault with a weapon, sexual assault, and captivity. Participants with perceived PTSD and lower perceived social support needed mental health interventions significantly more than those without PTSD symptoms. The findings emphasize the importance of addressing the intergenerational nature of PTSD and identifying factors affecting its prevalence, including social support and access to mental health services. The study underscores the need for a comprehensive approach to examine the complex and diverse nature of PTSD and its impact on individuals, families, and communities, especially in conflict-prone or conflict-affected societies.

The Impact of Turkey and Syria Earthquakes on University Students: Posttraumatic Stress Disorder Symptoms, Meaning in Life, and Social Support.

(1) Background: Earthquakes are natural disasters that often result in significant loss of life and property. The objective of this study is to explore the frequency of symptoms associated with posttraumatic stress disorder (PTSD), levels of meaning in life (ML), and perceived social support (SS) among university students in the aftermath of the earthquake that occurred in Turkey and Syria in 2023. (2) Methods: This study utilizes a cross-sectional correlation design to achieve its objectives among 603 university students from two public universities using an online survey (via Google Forms). The survey was launched one month after the earthquake in Turkey and Syria and concluded on 6 April 2023. (3) Results: The results indicate that a total of 158 university students, constituting 26.20% of the sample, reported extreme symptoms of PTSD. The results also indicate that 184 students (30.51), and 140 students (23.22%) reported low levels of ML and SS, respectively. Female students were significantly more vulnerable to experiencing PTSD symptoms, as well as difficulties in finding ML and SS. Finally, results revealed that students who were pursuing postgraduate studies had a greater likelihood of exhibiting symptoms indicative of PTSD. (4) Conclusions: It is recommended that universities provide support services and resources for students experiencing PTSD symptoms.

Traumatic stress symptoms and PTSD risk in children served by Children's Advocacy Centers.

Children who experience maltreatment are at high risk for posttraumatic stress disorder (PTSD). Children's Advocacy Centers (CACs) can facilitate access to treatment following maltreatment allegations. We describe PTSD symptoms and intervention decision-making for children served by CACs. Children served by CACs in a single state were screened for PTSD symptoms using a structured mental health screening/referral protocol. CAC staff used an electronic form that provided guidance for decision-making. We examined descriptive statistics for PTSD symptoms and risk and tested associations between child characteristics and symptoms. We described CAC staff's delivery of brief interventions and referral decisions and tested associations with child characteristics and symptoms. Two thousand and three hundred fifty children completed screening between 2018 and 2020. Almost half (45.5%) exhibited traumatic stress symptoms suggesting high probability of PTSD at the time of their CAC visit. Children who identified as female or transgender male and older children were more likely to be at high risk for PTSD. Brief interventions were delivered to 66% of children, and most were referred to evidence-based trauma treatment (53.1%) or community mental health services (39.0%). Categorization as moderate or high PTSD risk was associated with a higher likelihood of brief intervention delivery and referral to trauma treatment. Many children served by CACs are likely to meet criteria for PTSD at their initial visit. CAC staff demonstrated the ability to deliver brief interventions and make referrals to mental health treatment. Use of structured screening/referral protocols may improve early identification and treatment access for children experiencing PTSD symptoms.

Trauma and Coping in Battling the COVID-19 Pandemic Among Malaysian Healthcare Workers

Introduction: Prolonged COVID-19 pandemic with high morbidity and mortality may cause traumatic events to Healthcare Workers (HCW), resulting in Post-Traumatic Stress Disorder (PTSD) symptoms. Hence, this study aims to determine the prevalence of PTSD symptoms and its association with coping strategies among HCW in managing COVID-19 pandemic at Klang Valley Public Hospitals in Malaysia. Methods: A cross-sectional study with total of 424 eligible respondents were recruited through stratified random sampling. Data was collected from 6th May until 6th June 2021 using a self-administered online questionnaire adopted from MPCL-5 and Brief COPE instruments. IBM Statistical Package for Social Sciences Version 26 was used to analyse data. Result: 25% of the respondents demonstrated PTSD symptoms. Respondents who are single (aOR=3.319, 95% CI: 1.912, 5.762, p-value <0.001) and had history of positive COVID-19 (aOR= 2.563, 95% CI:1.058, 6.209, p-value=0.037) were more likely to experience PTSD symptoms. Frequently coping with self-blaming (aOR= 7.804, 95% CI: 3.467, 17.568, p-value < 0.001), behavioural disengagement (aOR= 7.262, 95% CI: 1.973, 26.723, p-value =0.003), humour (aOR= 5.303, 95% CI: 1.754, 16.039, p-value =0.003), venting emotion (aOR= 3.287, 95% CI: 1.521, 7.105, p-value =0.002) and less planning (aOR= 2.006, 95% CI:1.154, 3.487 p-value =0.014) are significant predictors for PTSD symptoms. Conclusion: One in four HCW managing COVID-19 in Klang Valley public hospitals experienced PTSD symptoms. Therefore, urgent interventional program targeting HCW who are single with history of positive COVID-19 is beneficial to prevent PTSD. Maladaptive coping strategies like self-blaming, venting emotion, humour and behavioural disengagement should be replaced with more adaptive coping strategies like planning, self-compassion, self-care and self-reflection.

Rates and correlates of psychological distress and PTSD among persons with physical disabilities in Cambodia

BackgroundCompared to the general population, persons with disabilities are at increased risk of poor mental health. The aim of this study was to determine the rates and correlates of psychological distress and post-traumatic stress disorder (PTSD) among persons with physical disabilities in Cambodia.MethodsFrom July to December 2021 data were collected as part of a mental health screening programme for persons with physical disabilities who access prosthetic and orthotic services. Psychological distress was measured using the Kessler-10 (K-10) and PTSD using the PC-PTSD-5. Bivariate and multiple linear regression analyses were conducted to identify factors associated with levels of psychological distress and PTSD among this population.ResultsOur study found a high prevalence of psychological distress and PTSD in this patient cohort. Of the 213 participants, 31.5% were likely to be experiencing mild to moderate psychological distress indicative of a mental health disorder, with 13.6% likely to have a severe mental health disorder. Sixty-five percent of patients reported experiencing PTSD symptoms, with forty-six percent meeting the criteria for probable PTSD. Psychological distress was associated with pathological worry, rumination, and facets of mindfulness. Rumination and pathological worry were found to be significant predictors of psychological distress. PTSD symptoms were associated with pathological worry but not facets of mindfulness or rumination. Facets of mindfulness and pathological worry were found to be significant predictors of PTSD.ConclusionIntegration of mental health services within the disability sector is required to address psychological distress and PTSD symptoms among people with physical disabilities in Cambodia. Health system interventions, such as screening, referral, and the training of health providers, need to be strengthened. Further studies focussing on the psychosocial determinants of mental health of persons with disabilities in Cambodia are required.

Causal discovery replicates symptomatic and functional interrelations of posttraumatic stress across five patient populations.

Approximately half of individuals with posttraumatic stress disorder (PTSD) may meet criteria for other psychiatric disorders, and PTSD symptoms are associated with diminished health and psychosocial functioning. However, few studies examine the longitudinal progression of PTSD symptoms concurrent with related symptom domains and functional outcomes, such that may neglect important longitudinal patterns of symptom progression beyond PTSD specifically. Therefore, we used longitudinal causal discovery analysis to examine the longitudinal interrelations among PTSD symptoms, depressive symptoms, substance abuse, and various other domains of functioning in five longitudinal cohorts representing veterans (n = 241), civilians seeking treatment for anxiety disorders (n = 79), civilian women seeking treatment for post-traumatic stress and substance abuse (n = 116), active duty military members assessed 0-90 days following TBI (n = 243), and civilians with a history of TBI (n = 43). The analyses revealed consistent, directed associations from PTSD symptoms to depressive symptoms, independent longitudinal trajectories of substance use problems, and cascading indirect relations from PTSD symptoms to social functioning through depression as well as direct relations from PTSD symptoms to TBI outcomes. Our findings suggest PTSD symptoms primarily drive depressive symptoms over time, tend to show independence from substance use symptoms, and may cascade into impairment in other domains. The results have implications for refining conceptualization of PTSD co-morbidity and can inform prognostic and treatment hypotheses about individuals experiencing PTSD symptoms along with co-occurring distress or impairment.

Childhood psychological maltreatment, sense of self, and PTSD symptoms in emerging adulthood

AbstractThis study outlines the relationships among childhood psychological maltreatment (CPM), an understudied form of maltreatment, sense of self (SOS), a developmental construct negatively impacted by trauma, and post‐traumatic stress disorder (PTSD) symptoms in emerging adults. Results from regression analysis on a sample of emerging adults (N = 358) indicated CPM to be the greatest predictor of a weaker SOS, even when considered alongside other types of child maltreatment. Among those exposed to CPM, SOS was a significant predictor of PTSD symptoms, explaining 34.1% of the variance in the outcome. Counseling interventions that focus on SOS during emerging adulthood are recommended as one important consideration for survivors of CPM who experience PTSD symptoms. Specific counseling practice and research implications are included.

Differences in the diagnosis and treatment decisions for children in care compared to their peers: An experimental study on post-traumatic stress disorder.

Despite evidence of high rates of diagnosable mental health difficulties in children in care, there remains ongoing debate around the appropriateness of traditional diagnoses and treatments. The aim of this study was to quantitatively explore whether mental health diagnosis and treatment decision-making differed when a young person was identified as being in care, specifically focused on post-traumatic stress disorder (PTSD). PTSD is a trauma-specific mental health disorder with rates substantially higher in children in care versus their peers. Participants were 270 UK mental health professionals who completed an online survey. Participants were randomized to receive one of two vignettes, which were identical in their description of a teenage boy experiencing PTSD symptoms, except in one he was in foster care and in the other he lived with his mother. Participants were asked to select a primary diagnosis, treatment approach, and potential secondary diagnosis. Professionals were twice as likely to choose a primary diagnosis of PTSD and a National Institute for Clinical Excellence (NICE)-recommended PTSD treatment when randomized to the mother vignette versus the foster carer vignette. Selecting PTSD as the primary diagnosis made clinicians three times more likely to select a NICE-recommended treatment for PTSD. Developmental trauma was the most common 'diagnosis' for both groups, although this led to different treatment decisions. In the context of PTSD, we found children in care face diagnosis and treatment decision-making biases. Practice implications are discussed.

Analysis of Symptom Characteristics, Influencing Factors, and Their Predictive Value for Posttraumatic Stress Disorder in Nurses: A Cross-Sectional Questionnaire Study.

Nurses often face a variety of nursing-related stresses, making them more prone to symptoms of posttraumatic stress disorder (PTSD). We aimed to explore symptom characteristics, influencing factors, and their predictive value for PTSD in nurses, so as to prevent the occurrence of PTSD in nurses. This was a cross-sectional study conducted in two tertiary hospitals in Yangzhou. A total of 1290 valid questionnaires were received in our study, and 190 nurses (14.7%) were positive for PTSD symptoms. The results show that individuals with higher scores on the Perceived Stress Scale-10 (PSS-10), Patient Health Questionnaire-15 (PHQ-15), Generalized Anxiety Disorder Scale-7 (GAD-7), and maladaptive cognitive emotion regulation strategies questionnaire (maladaptive CERS) were more likely to experience PTSD symptoms, whereas those with lower scores on the Perceived Social Support Scale (PSSS) were more likely to experience PTSD symptoms. Compared with the PSS-10, PHQ-15, and PSSS, GAD-7 and maladaptive CERS had higher predictive value. This study provided the optimal threshold of relevant factors that may have a positive effect on the prevention of PTSD symptoms. This has guiding implications for active prevention and intervention in some institutions.

Posttraumatic Stress Disorder and Relationship Satisfaction among Firefighters: The Role of Emotion Regulation Difficulties

ABSTRACT Firefighters are exposed to potentially traumatic events throughout their careers, placing them at heightened risk for the development of posttraumatic stress disorder (PTSD). Individuals experiencing PTSD symptoms often experience interpersonal problems and relationship stress, and this may be due to emotion regulation difficulties. The current study examined the association between PTSD symptoms, couple relationship satisfaction, and emotion regulation difficulties among firefighters. Participants were comprised of 188 firefighters (M age = 41.32, SD = 9.25, 97.3% male) who completed an online questionnaire. Results indicated that PTSD symptom severity was negatively associated with relationship satisfaction and positively associated with emotion regulation difficulties. Additionally, there was a significant negative indirect effect of PTSD symptom severity on relationship satisfaction through heightened emotion regulation difficulties. Negative alterations in cognition and mood were especially relevant to emotion regulation difficulties and relationship satisfaction. Findings highlight the importance of understanding associations between PTSD and interpersonal functioning among firefighters. Emotion regulation difficulties may offer a clinically relevant transdiagnostic factor for targeting PTSD symptoms and relationship functioning among firefighters.

Most research evaluating eye movement desensitization and reprocessing (EMDR) therapy has taken place in community settings, leaving the impact for service users within inpatient environments less clear. This systematic review sought to identify, summarize, and critically evaluate studies that investigated the impact of EMDR on symptoms of posttraumatic stress disorder (PTSD) within adult inpatient mental health settings. Seven databases were systematically searched to identify published and unpublished articles eligible for inclusion. Eleven studies, published between 1995 and 2020, were included within this review. All studies showed that EMDR improved PTSD symptoms, across a variety of study populations. However, these findings are limited by the (a) preponderance of case study designs (preventing causal attribution); (b) provision of synchronous treatments; (c) poor methodological quality; and (d) high heterogeneity across studies. Prior research has shown EMDR’s effectiveness and safety for vulnerable populations. While the evidence is beginning to support its use with those experiencing PTSD symptoms within adult inpatient settings, it is premature to strongly recommend it as a routine intervention. Future research within this area is recommended.

Traumatic life experiences and post-traumatic stress symptoms in middle-aged and older adults with and without autistic traits.

Research with younger adults has begun to explore associations between autism/autistic traits and vulnerability to Post Traumatic Stress Disorder (PTSD). Large scale studies and/or examination of age-effects have not been conducted. Adults aged 50years+ from the PROTECT study (n=20,220) completed items about current and childhood socio-communicative difficulties characteristic of autism. Approximately 1% (n=251) endorsed high autistic traits, henceforth the Autism Spectrum Traits (AST) group. Differences between the AST and an age-and sex-matched "Comparison Older Adults" (COA; n=9179) group were explored for lifetime traumatic experiences and current symptoms of PTSD, depression, and anxiety. Almost 30% of the AST group, compared to less than 8% of the COA, reported severe trauma in childhood/adulthood, including emotional, physical or sexual abuse. Elevated current PTSD symptoms were reported by AST compared to COA. An interaction was observed between autistic traits and trauma severity; the effect of level of trauma on PTSD symptoms was significantly greater for AST versus COA participants. This interaction remained significant when controlling for current depression and anxiety symptoms. The findings suggest that high autistic traits may increase the likelihood of experiencing trauma across the lifespan, and the impact of severe trauma on PTSD symptoms. Older adults with high (vs. low) autistic traits may be at greater risk of experiencing PTSD symptoms in latter life. Future research should test whether the pattern of results is similar for diagnosed autistic adults.

Resolving the paradox of posttraumatic growth and event centrality in trauma survivors.

When a traumatic experience is central to an individual's identity and worldview, it can result in either severe posttraumatic stress disorder (PTSD) symptoms, perceived posttraumatic growth (PTG), or, paradoxically, both. To resolve this apparent paradox, we used network analytic methods to estimate the relations among components of event centrality (EC), PTSD symptoms, and PTG in 1,136 undergraduates who had experienced trauma. Participants completed surveys on their experiences with traumatic events as well as the degree to which they experienced PTSD symptoms, components of EC, and components of PTG. We performed network analysis to examine EC, PTSD, and PTG and identify which components of EC were most conducive to its associations with PTSD versus those with PTG. We found that the components of EC most associated with PTSD, the extent to which trauma serves as a script for the future, were markedly distinct from the components associated with PTG, the extent to which trauma is seen as a turning point in one's life. The combined findings suggest that EC may be a catalyst for subsequent positive or negative effects contingent upon how an individual interprets the centrality of their traumatic experience.

Effects of Indirect Experience of Client Violence on Social Workers' Posttraumatic Stress Disorder.

Objective This study aims to empirically determine if indirect exposure to client violence has significant negative effects on social workers’ posttraumatic stress disorder (PTSD) the same way direct victimization does. Methods Using a sample of 1,359 social workers drawn from the data collected by the Seoul Association of Social Workers, this study employs descriptive statistics to examine the prevalence of indirect experiences with client violence, and utilizes a series of hierarchical regression analyses to demonstrate the potential impact of indirect exposure to client violence on PTSD. To assess the severity of PTSD symptoms in participants, the Korean version of the Impact of Events Scale-Revised (IES-R-K) was employed. Results A descriptive analysis shows that 12.4% of the sample indirectly experienced client violence by witnessing it or hearing about a violent incident, whereas 6.0% were directly victimized. Hierarchical regression analyses indicate that direct experience (B=4.548, p<0.05) and indirect experience (B=7.297, p<0.001) of client violence have a significant association with the scores on IES-R-K. An investigation of the interaction terms between experiences of client violence and violence-prevention training illustrates that such training significantly moderates the influence on the scores on IES-R-K from indirect exposure to client violence (B=-8.639, p<0.01). Conclusion Social workers who are indirectly exposed to client violence experience PTSD symptoms comparable to their colleagues who were directly victimized. Further, violence-prevention training has greater ameliorative effects with regard to indirect experience of client violence than for direct victimization.
