The Effectiveness of Cabbage Leaf Compresses and Aloe Vera Against Breast Engorgement Scales in Breastfeeding Mothers

Previous research reported the reason a mother stops breastfeeding her baby in the first month post partum is because of breast engorgement. This has a big impact on exclusive breastfeeding status. Engorgement in the breasts, whatever the cause, must be treated immediately. Providing non-pharmacological therapy must be attempted as initial prevention because the side effects are minimal. The aim of this research is to analyze the effectiveness of cabbage leaf compresses on the scale of breast engorgement in breastfeeding mothers. This research is a type of intervention research with a quasi-experimental design in the work area of the Bandung community health Center, Serang Regency, Banten Province in December 2023. The sample used was 30 breastfeeding mothers. The data was interview, anamnesa, and collected by observational form. The bivariate analysis used in this study was the Mann Whitney U test. The results of the Mann-Whitney test showed a p value of 0.049 < 0.05 for the aloe vera compress, the results of the cabbage leaf compress intervention analysis showed a p value of 0.042 < 0.05, which means there was difference in mean scale breast engorgement before and after applying aloe vera and cabbage leaf compresses.

The Association of Maternal Pre-Pregnancy Obesity on Breastfeeding: A Pilot Prospective Cohort Study

Objective: This prospective cohort study was conducted to investigate the association of mothers who are obese according to body mass index (BMI) in the pre-pregnancy period on their infant’s exclusive breastfeeding status six months after delivery.
 Material and Method: The study sample consisted of 44 obese and 44 non-obese mother-infant dyads in Izmir, Turkey. The data were collected during the 1st week, 1st, 3rd, and 6th months after delivery using the Individual Identification Form, LaTCH Tool, and Breastfeeding Status Follow-up Form. Independent t-test, Chi-square and Cochran’s Q tests, and Cox’s regression analysis were used.
 Results: A significant difference was found between the exclusive breastfeeding status of the infants of obese and non-obese participants in all follow-up weeks. In the Cox regression model, the only significant explanatory variable on exclusive breastfeeding in all follow-ups was prepregnancy BMI of the participants. The infants of obese participants were exclusively breastfed by 2.25 times less in the 1st week, 4.50 times less in the 1st month, 2.20 times less in the 3rd month, and 1.69 times less in the 6th month than the infants of non-obese participants.
 Conclusions: It was determined that maternal obesity statistically decreased the exclusive breastfeeding status. It can be recommended to monitor women of the reproductive age for obesity during pre-pregnancy follow-ups and to implement nursing interventions such as health education and counseling to reduce obesity.

DETERMINANTS OF STUNTING IN SEMARANG CITY

Background : Stunting is not only a problem of physical growth disorders, but also causes children to become sick easily, brain and intelligence development also occurs. So, stunting is a big threat to the quality of human resources in Indonesia. The purpose of writing of this literature review is to collect determinants of Stunting in Semarang.
 Objective : This research uses a literature revies from Google Scholar library sources, which is presented descriptively according to the problem formulation.
 Result : Review from some of articles, found that maternal education level, maternal occupation, child order, birth length, child rearing patterns, exclusive breastfeeding status, family income, housing conditions, nutritional intake, and history of KEK are risk factors determining the incidence of Stunting in the Semarang. 
 Conclusions : Stunting prevention programs must be improved, by screening and monitoring the growth and development of babies and toddlers to prevent Stunting from an early age. Public is also advised to be more diligent in carrying out examinations at health services to monitor children’s growth and development.

CLASSIFICATION OF TODDLER’S NUTRITIONAL STATUS USING THE ROUGH SET ALGORITHM

The health and nutrition of children at the age of five are very important aspects in the children’s growth and development. An assessment of the nutritional status of toddlers that is commonly used is anthropometry. This study aims to obtain the decision rules used to classify toddlers into nutritional status groups using the rough set algorithm and determine the level of classification accuracy of the resulting decision rules. The index used in this study is the weight-for-age index. Attributes used in this study were the mother’s education level, mother’s level of knowledge, the status of exclusive breastfeeding, history of illness in the last month, and nutritional status of toddlers. The results of the analysis show that there are 21 decision rules. In this study, the resulting decision rules experience inconsistencies. The selection of decision rules that experience inconsistencies is based on each decision rule’s highest strength value. The rough set algorithm can be used for the classification process with an accuracy rate of 86.36%.

Determinants of Stunting in Toddlers In Gatak Subdistrict, Sukoharjo Regency

<span>Indonesia faces nutritional problems that have an impact on the quality of human resources. One of the nutritional problems is the incidence of stunting experienced by more than half of stunting toddlers in the world who come from ASIA (55%). The highest number of stunting cases is based on e-PPGBM data for 2020-2021 in Sukoharjo Regency in Gatak District. In Gatak District, from 2020 to 2021, there has been an increase in stunting cases in toddlers. In 2020 in Gatak District, the number of stunted toddlers was 414 toddlers while in 2021 the number of stunted toddlers increased to 463 toddlers. </span><span>This research is a quantitative research with a cross-sectional approach. The bound variables in this study are the incidence of stunting in toddlers, while the free variables in the study are maternal knowledge about nutrition, exclusive breastfeeding status, parental income, BBLR history and child care. The research sample was taken with a sampling quota with a total sample of 96 respondents who were toddlers in Gatak District, Sukoharjo Regency in 2021. The analysis used by researchers to determine the relationship of each variable using chi square analysis. The value of P is said to be related if the Value of P is <0.05. Based on the frequency distribution is mostly on productive age, higher education level, mostly employed, and income level is largely less than the UMR. There is a significant relationship between maternal knowledge, exclusive breastfeeding status, BBLR history and parenting with stunting events (p value = 0.000 < 0.05), while parental income has nothing to do with stunting events (p value = 0.067 > 0.05).</span>

STATUS OF EXCLUSIVE BREASTFEEDING AND COMMUNITY-BASED TOTAL SANITATION (STBM) ON STUNTING EVENTS IN PEAT AND THE KAPUAS RIVER

Abstract
 
 Background: Stunting occurs due to a lack of micro and macro nutrients which are not sufficient both in quality and quantity, and also due to the presence of chronic diseases, especially when still in the womb until the age of 2 years. The prevalence of stunting can also be caused by exclusive breastfeeding status. Giving breast milk for less than 4 months has a risk of 7.325 times greater for disrupting its development compared to toddlers more than 4 months getting breast milk.
 The study aimed to determine the status of exclusive breastfeeding and Community-Based Total Sanitation (STBM) with the incidence of stunting in peat soils and the banks of the Kapuas River. This type of research is an analytical design that uses a case-control design to compare the peat area with the Kapuas watershed in terms of stunting related to the risk of stunting factors. This study used a retrospective approach with a total sample of 50 case groups and 50 control groups. The data obtained will be analyzed using univariate and bivariate methods using Logistic Regression.
 The results showed that there was a significant relationship (p='<0.05) between exclusive breastfeeding and the incidence of stunting in peat soils and riverbanks. Toddlers who are not exclusively breastfed for 6 months on peat soils and riverbanks have 7.5 times and 3.3 times chance that stunting will occur. There is no significant relationship between Community-Based Total Sanitation and the incidence of stunting on peat soils and riverbanks (p='>0.05). However, toddlers who do not do STBM have the opportunity to experience stunting.
 Conclusion. There is a significant relationship between exclusive breastfeeding status and the incidence of stunting. There is no significant relationship between STBM status and the incidence of stunting.

Association of labor epidural analgesia use with exclusive breastfeeding up to six months: a online-based cross sectional survey in Jiaxing, China

BackgroundThe impact of labor epidural analgesia (LEA) on breastfeeding remains controversial. The aim of this study was to assess the relationship between LEA use and exclusive breastfeeding (EBF) up to 6 months.MethodsThis was a cross-sectional survey on healthy mothers who had vaginal delivery with infants aged 7-12 months from seven maternal health WeChat groups in Jiaxing, China. Data including EBF status up to 6 months, maternal sociodemographic characteristics, LEA use in labor, breastfeeding supports during hospitalization and reasons for stopping EBF were collected using online self-administered questionnaires in October 2021. A multivariable logistic regression model was used to determine the potential association of LEA use with EBF up to 6 months by the adjusted odds ratio (AOR) and 95% confidence interval (CI).ResultsOf a total of 537 surveyed mothers, 408 (76.0%) delivered with LEA and 398 (74.1%) exclusively breastfed their infants until 6 months. All mothers delivered in the hospitals with active breastfeeding policies. There was no statistical difference in the rate of EBF up to 6 months between mothers with and without LEA (73.8% versus 75.2%, P = 0.748). Multivariable logistic regression analysis indicated that only increased maternal age (AOR = 0.906, 95% CI 0.854-0.961, P = 0.001) and perceived insufficient breast milk (AOR = 0.129, 95% CI 0.082-0.204, P < 0.001) were associated with lower odds of EBF up to 6 months. The top three reasons for non-EBF were no or insufficient breast milk (41.7%), inability to breastfeed infants after return to work (27.3%), and maternal related factors (24.4%).ConclusionsLEA does not affect EBF up to 6 months. Other factors such as health education and breastfeeding-friendly hospital strategies may be much more important to breastfeeding outcomes compared to LEA use.

Low Latch Score: A Red Flag Sign and an Educational Tool for Intervention Among Mothers to Promote Exclusive Breastfeeding

Introduction LATCH score is used by the health-care professionals (HCPs) to identify mothers who require assistance in breastfeeding. We conducted this study to assess if the low LATCH score can be an indicator to alert the HCPs for intervention to achieve exclusive breastfeeding (EBF) till 6 weeks and also used as an educational tool for mothers to understand and rectify their technique of breastfeeding. Methods In this prospective cohort study, LATCH score was assessed after delivery within 24 h, at 48 h, and at discharge. After first assessment, mothers were informed and counselled about LATCH scoring tool and their confidence was assessed using Likert scale at each time point. Follow-up was done at 6 weeks either by person at the time of immunization or through telephone to know the status of EBF and weight of the infant. Results A total of 251 mothers and neonates were enrolled. Prevalence of EBF was found to be 76% at 6 weeks. Overall, the median LATCH Score at 24 h was found to be 6 (2,10) and at 48 h was 7 (3,10) whereas the median LATCH score at 48 h for those delivered vaginally was 7 (4,10) and those delivered by LSCS was 6.5 (3,9) ( P &lt; .0001). LATCH score of ≥6 at 48 h had sensitivity and specificity of 80% (confidence interval [CI] 68%, 89%) and 69% (CI 62%, 76%) respectively with area under the curve 0.81 (CI 0.74, 0.87) to predict EBF at 6 weeks. We found statistically significant change in mother’s confidence level compared from 24 h to 48 h with better LATCH score and EBF by 6 weeks after informing them about LATCH score. Conclusion A LATCH score of &lt;6 at 48 h can be a significant indicator (Red Flag sign) to intervene and promote exclusive breastfeeding. LATCH scoring itself may be used as educational tool to improve the technique of breastfeeding and thus promote EBF.

Linear growth of infants aged 0-6 months in breastfeeding mothers who consume Moringa oleifera leaf extract capsules: randomized controlled doubleblind design

The prevalence of stunting in children in Indonesia is still high. However, linear growth and infant growth rates are relatively rarely published. This study aimed to analyse the effect of linear growth and growth rate of infants aged 0-6 months exclusively breastfed on mothers who consumed Moringa leaf extract and Moringa leaf powder supplements. The design of this study was a randomized controlled Double-Blind Design. Subjects were divided into 35 intervention groups and 35 control groups. The intervention group was given Moringa extract capsules (3.2 g/day) and breastfeeding education, while the control group was given Moringa leaf powder capsules (3.2 g/day) and breastfeeding education for about 3 months. Infant growth was followed until the age of 6 months, the variables of infant growth were expressed in weight for age, body length for age and growth based on the nutritional status of the infant expressed in the Z score of weight for age (W/A), Z score of body length for age (BL/A), Z score of body weight for body length (BW/BL) and Z score of body mass index for age (BMI/A). Statistical analysis with paired T-test, Kolmogorov-Smirnov, and Exat Fisher Test. The results showed that the growth of body weight, body length, and BL/A were not different between the intervention group and the control group (p&gt;0.05). Especially for the Z score of BW/A, BW/BL, BMI/A found a difference between the intervention and control groups with a p-value &lt;0.05. The conclusion of the research is the growth rate of weight and length of infants aged 0-6 months were influenced by birth weight, infant morbidity, and exclusive breastfeeding status. The difference in the average deviation of the Z score value from the four anthropometric indices (BW/A, BW/BL, and BMI/A) roled in the administration of Moringa extract.

Pathways linking social support, self-efficacy, and exclusive breastfeeding among women in northern Uganda

ABSTRACT Despite improvements in infant feeding practices over the past two decades, the prevalence of exclusive breastfeeding (EBF) is below global targets. Social support can create an enabling environment for recommended infant feeding practices such as EBF, but the types of social support most important for sustained EBF and their potential mechanisms of action have not been thoroughly characterized. We therefore aimed to assess the relationship between EBF-specific social support, EBF self-efficacy, and EBF at 1 and 3 months among postpartum women in northern Uganda. Women (n = 238, 36.2% living with HIV) were recruited during pregnancy. EBF, social support, and EBF self-efficacy were assessed at 1 and 3 months postpartum. Path analysis was used to assess relationships between these factors. Most mothers exclusively breastfed to 1 (80.8%) and 3 months postpartum (62.9%). EBF-specific, but not general, social support differed by EBF status. EBF-specific social support was associated with higher odds of EBF, which was almost fully mediated by EBF self-efficacy. That is, there was evidence that social support primarily influences EBF through its association with self-efficacy. In sum, EBF-specific social support and self-efficacy likely promote EBF and are modifiable factors that can be intervened upon.

FACTORS ASSOCIATED WITH ACUTE RESPIRATORY TRACT INFECTION PNEUMONIA IN TODDLERS

Abstract Background: Most of the deaths of children under five in the world occur due to respiratory tract infections, especially in the lower respiratory tract. Pneumonia is one of the biggest causes of infant mortality in the world. Objective: The purpose of this study was to determine several factors that influence acute respiratory tract infections with pneumonia in children under five. Methods: Case control research design was used with statistical analysis of Chi Square test bivariate and multivariate logistic regression analysis which had a significance of 5%. Data collection using simple random sampling technique. Results: The variables that have the greatest significant value are immunization status and exclusive breastfeeding status. Toddlers with incomplete immunization have a 2.876-fold risk of being infected with pneumonia compared to toddlers who are fully immunized. Breastfeeding that is not exclusive is at risk of infection with pneumonia 3.111 times that of toddlers who are given exclusive breastfeeding. Conclusion: Exclusive breastfeeding status and immunization status are the most influential variables on acute respiratory tract infection with pneumonia. The association of breastfeeding and other factors on respiratory virus positivity and severity in hospitalized children.

Acute respiratory infections (ARIs) are the most prevalent diseases in children under 5 years old, and viruses are the leading cause. ARIs arise due to numerous factors, including age, contact with siblings or other children in daycare centers, and environmental pollution. Breastfeeding reportedly confers protection against ARIs through bioactive components related to mucous epithelial immunity. This study aimed to evaluate the frequency and severity of viral ARIs in hospitalized children, together with the status and duration of exclusive breastfeeding (EBF) and other associated factors. It comprised an epidemiological surveillance study to investigate respiratory viruses in hospitalized children, in which demographic and clinical data were collected. Overall, 279 patients were included, 190 (68%) had positive viral results, and 132 (47%) were exclusively breastfed. In an adjusted analysis, it was observed that older children, the parents' educational level, and the presence of chronic disease were significantly related to EBF for more than 6 months. No significant differences were observed in viral positivity and disease severity concerning EBF. Whereas the EBF status was associated with a positive rate of virus detection, the significance did not remain after adjustment, and it was not considered a protective factor against ARIs. On the other hand, young age and exposure to tobacco were confirmed as risk factors of frequency and severity, respectively. Such confounding factors can impact the analysis and should be considered in future studies.

The effect of hospital-based breastfeeding group education given early perinatal period on breastfeeding self-efficacy and breastfeeding status

AimThis study was conducted to examine the effect of hospital-based group breastfeeding education provided to mothers before discharge from the hospital on mothers’ self-efficacy and on the increase of breastfeeding rates. MethodsThis randomized controlled educational research was conducted in the hospital is located in the West of Turkey. Ninety mothers who had healthy and term babies, and an intent to breastfeed their babies participated in the study. The questionnaire forms were collected by the researcher using face to face interview technique. Chi-square, dependent and independent groups t-test and ANOVA analysis were used along with descriptive statistics in analyzing the data. ResultsCompared to the control group, the scores obtained from the Breastfeeding Self-Efficacy Scale were higher in the education group in all the follow-ups. Similarly, status of exclusive breastfeeding was higher in the education group compared to the control group. In the twelfth week, the rate of exclusive breastfeeding was 80.0% in the education group, while it was 47.5% in the control group. ConclusionThese results show that breastfeeding group education and follow-up counseling given before discharge from the hospital were effective. This is evidence in favor of increasing the use of group education.This RCT was registered NCT04515862 with www.clinicaltrials.gov on 2020.

Extent, duration and predictors of exclusive breastfeeding in a longitudinal study: adjusting for missing data using an accelerated failure time model and multiple imputation

&#x0D; Background: The World Health Organization recommends at least 6 months of exclusive breastfeeding (EBF). Longitudinal studies facilitate estimation of EBF duration, but often suffer from loss to follow-up and missing information. The study estimates the prevalence of EBF, duration and predictors of EBF duration while adjusting for missing data using multiple imputation (MI).&#x0D; Methods: A longitudinal study was conducted on all women giving birth between September 2009-February 2010 in selected hospitals (N=2119). Data on EBF and socio-demographic and other characteristics were collected at birth, and at 2, 6, 12 and 24 months. Information on EBF status and duration was missing for 29%. To deal with missing data, we generated multiple datasets using logistic regression-based MI to impute missing EBF practice, and an accelerated failure time (AFT) model to impute missing duration of EBF. The latter model also identified factors associated with EBF duration.&#x0D; Results: The observed 64% of women practicing EBF (95%CI; 62%-66%) was adjusted, after imputation, to 62% (95%CI; 60%-65%). After imputation, the estimated median time of EBF among women practicing EBF was 4.9 months. Predictors of EBF duration were stated intention to breastfeed, religious observance, and giving formula milk while in hospital.&#x0D; Conclusion: Adjusting estimates of EBF practice and duration using MI is feasible and potentially important. Using an AFT model for EBF duration enables the execution of MI in such studies and allows direct interpretation of the impact of various factors on EBF duration. &#x0D;

Does COVID-19 Policy Affect Initiation and Duration of Exclusive Breastfeeding? A Single-center Retrospective Study

PurposeIt assessed the effect of a COVID-19 policy that involved maternal-neonatal separation on early initiation and duration of exclusive breastfeeding (EBF) at two weeks, three, and six months postpartum during the peak of the COVID-19 pandemic in 2020.Patients and MethodsThis retrospective study included consecutive mother-newborn dyads with deliveries between mid-March and the end of December 2020 who agreed to participate and were eligible according to the following criteria: full-term pregnancy, singleton birth, and healthy newborn. Participants were called at least twice during the study period to inquire about exclusive breastfeeding status at two weeks, three, and six months postpartum. Multivariate logistic regression analysis was carried out to identify associated factors with EBF.ResultsA total of 454 eligible mother-infant dyads were included. Mean maternal age (in years), gestational age (in weeks), birth weight (in kilograms), and 5-min Apgar score was, respectively, 32±5.4 years, 39±1.3 weeks, 3.1±0.42 kg, and 9.24±0.6. Most of the mothers (86.1%) had no known chronic diseases and had a normal spontaneous vaginal delivery (NSVD, 91.4%). Approximately 44.7% of the sample initiated breastfeeding either directly or with expressed breastmilk during their hospital stay. Prevalence of EBF during the first two weeks, three, and six months postpartum was, respectively, 30%, 19.8%, and 31.7%. EBF was associated with birth weight, hospital breastfeeding and EIBF at early postpartum period, while EBF at six months was associated with parity and method of delivery.ConclusionDuring the peak of the COVID-19 pandemic, the prevalence of breastfeeding initiation and EBF in the first six months postpartum were low among Saudi mothers. However, multicenter, prospective, cohort studies with adjustment for known confounding factors are required to explore the impact of infection control policies on breastfeeding. Meanwhile, these policies should support early and safe breastfeeding practices, especially with new and evolving information regarding the current pandemic.

AIM: This study aimed to determine the relationship between social-informational support, emotional, instrumental, and family assessment support on exclusive breastfeeding behavior at Talise Health Center, Palu City. MATERIALS AND METHODS: The research method was quantitative, with a cross-sectional study design. The population was 70 mothers who have babies aged 7–24 months and live in the Talise Health Center working area. The sampling technique was total sampling. Data analysis was the chi square test with a significance level of p &lt;0.05 using SPSS. RESULTS: Univariate analysis showed that the dominant age of the mother was 29–37 years (75.7%), bachelor degree education level (85.7%), multiparous parity status (68.6%), and exclusive breastfeeding status (68.6%). The results showed that there was a significant relationship between social-information support (p = 0.042), instrumental (p = 0.013), emotional (p = 0.038), and assessment (p = 0.020) on the behavior of exclusive breastfeeding. CONCLUSION: Lack of social support from the family, including informational, instrumental, emotional, and assessment, would hinder exclusive breastfeeding behavior and impact on the failure of exclusive breastfeeding.

Application of Cart Method in Analyzing Factors Affecting Nutritional Status of Children Aged 6-23 Months

This study aims to analyze factors that affect the nutritional status of children aged 6-23 months. The data used were acquired from a survey conducted at mother and child health services (‘posyandu’), daycares, and households in four districts in Padang City, West Sumatera. In this study, variables that are hypothesized to contribute to nutritional status are sex, birth weight, family income, number of family dependents, exclusive breastfeeding status, mother's employment status, education level, knowledge and nutritional behavior. Data analysis was done by using tree structure analysis namely CART method. It is showed that variables which significantly affect the nutritional status of children aged 6-23 months are mother’s education level, knowledge and nutritional behavior. Children with low mother’s education level have higher prevalence of severely underweight and underweight when mother’s knowledge is poor or enough. in the group of children whose mothers have higher level of education, the variable that has a significant influence on child’s nutritional status is mother's nutritional behavior. In the group of children whose mothers have higher level of education, the variable that has a significant influence on child’s nutritional status is mother's nutritional behavior, the prevalence of overweight is likely to be high in children whose mothers nutritional behaviour is not good. Therefore, intervention from related parties is needed to implement programs for improving mother's knowledge about nutrition that will lead to better child’s nutritional status.

HUBUNGAN FAKTOR PENGASUHAN DENGAN STUNTING DI KECAMATAN UBUD GIANYAR BALI

Introduction: The quality of a child is also determined by the health status of the child, especially in the toddler period. This period is a golden period for child development, so it is very important to ensure that the child's growth runs optimally, including ensuring that the nutritional status of toddlers is in a good category. The problem of short toddlers illustrates the existence of chronic nutritional problems that can be affected by the condition of the mother during pregnancy, the fetus, infants and toddlers, including diseases suffered during toddlers. This study aimed to analyze the relationship between parenting factors and stunting.&#x0D; Methods: Design of this study was analytic observational with cross sectional approach. The sampling technique used was purposive sampling by taking samples from two areas of stunting loci in Ubud District with a sample size of 92 people. The data collected are primary and secondary data. Data analysis used Chi Square and Fisher's Exact test.&#x0D; Results and Discussion: The results showed that most of the children were in nonstunting status with a stunting rate of 25%. The results of the bivariate analysis between the independent variables and stunting showed the following results: exclusive breastfeeding status (p = 0.01), active posyandu visits (p = 0.022) and diarrhea exposure (p = 1.00).&#x0D; Conclusion: There is a relationship between exclusive breastfeeding status and active posyandu visits with stunting in toddler.&#x0D; &#x0D; Keywords: toddlers, posyandu visits, stunting, exclusive breastfeeding, child infections
