Lessons from the European and Israel National MDS Registry

On behalf of the Israeli MDS working group and EUMDS Registry/ XIII Eurasian Hematology Oncology Congress (EHOC) 2022. The myelodysplastic syndromes (MDS) are a group of clonal stem cell diseases with cytopenias and a tendency to transform to leukemia. Despite the progress, there is still lack of real world data about the disease. In 2008, top European experts launched the EUMDS Registry. In this project, including 19 countries and 150 sites, epidemiological, clinical and lab data on newly diagnosed MDS patients, are collected and analyzed. As of today, 3300 patients have been recruited. In 2012, the Israel MDS group joined the project. We have contributed data on 360 patients, # 4 in the contributors. This project led to more than 100 abstracts in international meetings and 40 publications in first line journals. We will mention some of these studies. de Swart et al. summarized data on the first 1000 patients, validated the IPSS-R prognostic classification, showing its superiority on IPSS (de Swart L. BJH 2015). Another study, focusing on Quality of life (QoL) of LR-MDS patients revealed that these patients suffer from depression, anxiety, pain, discomfort and mobility difficulties, compared to controls (Stauder R, Leukemia 2018) The effects of Erythroid Stimulating Agents (ESAs,) was evaluated: Garelius et al. showed that ESA administration delays RBC transfusion dependency (Garelius HK. J Intern Med 2017). Since in most countries ESA is given to transfusion-dependent MDS patients, it might change the paradigm. Recently, we demonstrated that ESA treatment, is associated with improved outcomes and overall survival (Garelius HK. EHA 2022; submitted). Since prognostic factors are often determined at disease presentation, it was important to develop dynamic parameters. We showed that a rapid decline > 25% in the platelet count within 6 months is an adverse prognostic marker (Itzykson R. Bl Adv 2018) A study focusing on the mutational status was presented at ASH 2021 showing that LR-MDS patients can be grouped into 3 clusters, with a correlation to the clinical status (Malcovati L. ASH 2021) The Israeli group, independently and as a part of the EUMDS, was involved in several projects. Oster et al. suggested a non-invasive calculator to assess the probability of or excluding MDS diagnosis, based on patient characteristics, avoiding BM examination (Oster HS. Bl Adv 2021). We also investigated the correlation between Hb level and QoL. This correlation was found to be partial, and the decrease in QoL was not linear. This suggests that other factors other than Hb might play a role in determining QoL (Haring Y. ASH 2021). We recently presented data on lymphoid aggregates in BM biopsies, suggesting a possible association with poor prognosis (Book-Rabinowitz. Int MDS Symposium, Toronto 2021; submitted). In summary , the EUMDS registry is a platform of a scientific project, and an example of international collaboration. Such projects, especially in relatively uncommon diseases, allow collection of enough data to allow meaningful conclusions that might change paradigm and improve patient care. We call all participants of this meeting to join us and improve the quality of this wonderful important project.

Adaptation of the Health Literacy Survey Questionnaire (HLS19-Q) for Russian-Speaking Populations-International Collaboration across Germany, Israel, Kazakhstan, Russia, and the USA.

The Russian language is the eighth most spoken language in the world. Russian speakers reside in Russia, across the former Soviet Union republics, and comprise one of the largest populations of international migrants. However, little is known about their health literacy (HL) and there is limited research on HL instruments in the Russian language. The purpose of this study was to adapt the Health Literacy Questionnaire (HLS19-Q) developed within the Health Literacy Survey 2019–2021 (HLS19) to the Russian language to study HL in Russian-speaking populations in Germany, Israel, Kazakhstan, Russia, and the USA. The HLS19-Q was translated either from English or from a national language to Russian in four countries first and then critically reviewed by three Russian-speaking experts for consensus. The HLS19 protocol and “team approach” method were used for linguistic and cultural adaptation. The most challenging was the adaptation of HLS19-Q questions to each country’s healthcare system while general HL questions were flexible and adaptable to specific contexts across all countries. This study provides recommendations for the linguistic and cultural adaptation of HLS19-Q into different languages and can serve as an example of international collaboration towards this end.

Connectivism for writing pedagogy: Strategic networked approaches to promote international collaborations and intercultural learning

Learning today is supported and made complex by ubiquitous technologies. To prepare students for global workforce and intercultural exchange, writing pedagogy must incorporate strategic networked learning approaches. In this article, we present three examples of international collaborations and intercultural learning by the means of connectivism—a learning theory for the digital age. Through assessing student projects and reviewing student and instructor feedback, we demonstrate the uses and implications of connectivism for writing pedagogy.

Celebrating the 60th Anniversary of the International Commission on Trichinellosis: An early example of Franco-German collaboration to study the Trichinellosis outbreak of Emersleben (1883)

Two prominent physicians, Grancher and Brouardel, were sent by the French Health authorities to study the severe ongoing outbreak of trichinellosis in Emersleben, Germany (1883). They produced a highly accurate description of the outbreak and confirmed that they had never observed such cases in France. Grancher performed two necropsies and very precisely described the histological lesions provoked by the larvae entering and transforming muscular fibers. They also had the opportunity to meet Virchow in Berlin and they emphasized his extreme kindness. This French medical mission and cooperation with famous German scientists such as Virchow is probably one of the first examples of international collaboration on trichinellosis. It prefigured the international collaborative efforts made by the International Commission on Trichinellosis which was created in 1958 and is still active.

Challenges and insights of early phase oncology drug development in the Asia-Pacific region.

Early phase clinical trials form the very foundation of a drug's clinical development. Traditionally, these trials were focused exclusively on dose optimization and toxicities with a broad aim across tumour types in the hope of achieving a signal of response. No real efforts were made to pair patients with best therapies. Moreover, early phase clinical trials have traditionally been performed in Western populations. Asia is the world's largest continent in terms of population size, and is rich in ethnic, cultural and socio-economic diversities. There are also distinct regulatory and methods of healthcare provision between countries within the region. These qualities form a 'double-edge sword' when it comes to drug development. However, the rapidly growing population which inevitability leads to increasing patients' numbers, coupled with improving economies and accessibility to quality healthcare put Asia in a prime position to play a more dominant role in early phase oncology drug development. At the same time, there is also increasing recognition of differences in disease epidemiology between different geographical regions, as well as potential pharmacogenomic differences between ethnicities. In this review article, the challenges faced, and opportunities foreseen for early phase oncology drug development in Asia will be discussed and illustrated with practical examples of international collaborations formed in recent years.

Intensive Chemotherapy for Childhood Acute Lymphoblastic Leukemia: Results of the Randomized Intercontinental Trial ALL IC-BFM 2002

From 2002 to 2007, the International Berlin-Frankfurt-Münster Study Group conducted a prospective randomized clinical trial (ALL IC-BFM 2002) for the management of childhood acute lymphoblastic leukemia (ALL) in 15 countries on three continents. The aim of this trial was to explore the impact of differential delayed intensification (DI) on outcome in all risk groups. For this trial, 5,060 eligible patients were divided into three risk groups according to age, WBC, early treatment response, and unfavorable genetic aberrations. DI was randomized as follows: standard risk (SR), two 4-week intensive elements (protocol III) versus one 7-week protocol II; intermediate risk (IR), protocol III × 3 versus protocol II × 1; high risk (HR), protocol III × 3 versus either protocol II × 2 (Associazione Italiana Ematologia Oncologia Pediatrica [AIEOP] option), or 3 HR blocks plus single protocol II (Berlin-Frankfurt-Münster [BFM] option). At 5 years, the probabilities of event-free survival and survival were 74% (± 1%) and 82% (± 1%) for all 5,060 eligible patients, 81% and 90% for the SR (n = 1,564), 75% and 83% for the IR (n = 2,650), and 55% and 62% for the HR (n = 846) groups, respectively. No improvement was accomplished by more intense and/or prolonged DI. The ALL IC-BFM 2002 trial is a good example of international collaboration in pediatric oncology. A wide platform of countries able to run randomized studies in ALL has been established. Although the alternative DI did not improve outcome compared with standard treatment and the overall results are worse than those achieved by longer established leukemia groups, the national results have generally improved.

Emergency Response Planning: An Example of International Collaboration in CCS Community Outreach and Project Development

When addressing community engagement and outreach, North American carbon capture and storage (CCS) projects have parameters unique to the continent, including the history of CO2 enhanced oil recovery (EOR), which goes back over 40 years in some jurisdictions and, aligned with this, the use of landmen and one-on-one dialogue with landowners and community residents that are well versed in oilfield technologies.These variables alone are in marked contrast to the CCS experiences of many global projects, which do not have the tradition of engaging in one-on-one discussion. Even where CCS projects have conducted extensive public consultation and education, significant opposition has shut down some, and put in jeopardy others, in a manner that contradicts the North American hydrocarbon experience. With the increase in North America of integrated CCS projects that go beyond CO2-EOR, a change in community engagement strategies has taken place under the unique auspices of the United States Department of Energy's (US DOE) Regional Carbon Sequestration Partnerships Initiative (RCSP). Part of the planning in each of these seven geographic regions includes significant public education, outreach, and communications programs, particularly in areas unfamiliar with injection and storage technologies (i.e., outside of traditional oil producing areas). The bringing together of different demonstration projects’ participants – not just nationally within the US but including projects in Western Canada – has allowed for the sharing of best practices between projects and across international jurisdictions.Such sharing is particularly true where the development of community engagement guidelines and strategies are concerned. The publication in 2010 of the US DOE's Best Practices for Public Outreach and Education for Carbon Storage Projects is one example where the experiences of several United States demonstration projects were brought to bear on developing communications guidelines, which in turn were used to help develop public outreach strategies for such projects as Aquistore in the province of Saskatchewan, Canada [1]. Another example of such international information sharing is the World Resources Institute's Guidelines for Community Engagement in Carbon Dioxide Capture, Transport, and Storage Projects where, over a period of a year and a half, international experts were brought together for round table discussions to form the basis of the guidelines and provide an international peer review [2].More recently, the development of an emergency response plan for the Illinois Basin – Decatur Project, led by the Midwest Geological Sequestration Consortium, one of the RCSP partnerships, drew upon this international collaborative structure, employing the experience of communicators from Schlumberger Carbon Services, the Petroleum Technology Research Centre (managers of the IEAGHG Weyburn- Midale CO2 Monitoring and Storage Project) and the Illinois State Geological Survey to develop a map of potential crisis points. This planning process brought together the lessons learned from various projects, risk assessments, media experiences, and best practices to help identify potential risks for the project (a list of events and scenarios) with the goal of creating response paths and directions for the management of risks and the mitigation of potential threats. These scenarios involved not only potential external issues – such as leakage or pipeline failure – but also addressed management issues internal to a project such as loss of key personnel or loss of funding.The development of this emergency response plan is an example to other projects of the value of interconnecting communications experiences between projects, and of identifying common high-risk scenarios that require advanced response planning.

Damage prevention methods in Europe: experiences from LIFE nature projects

Management of damage caused by wolf to domestic livestock is a crucial measure that must be part of an integrated management strategy. Despite the existence of responsible authorities for tackling such aspects, resources are often insufficient for addressing the complex issues. LIFE Nature projects represent a valid tool for the implementation of measures for wolf conservation as the species is included in the Annexes of the Habitat Directive as a priority species. In the last ten years, over 30 LIFE Nature projects targeting wolf conservation were financed by the EU. Measures adopted in the projects were largely consistent and coherent with the Action plan for the Conservation of Wolf in Europe published by the Council of Europe in 2000. The LIFE COEX project was implemented in Portugal, Spain, France, Italy and Croatia from 2004 to 2008, and represented an excellent example of international collaboration and amplification of knowledge and experiences of management measures adopted at different levels. Adapted to local conditions, the measures implemented achieved extremely positive results, particularly in areas where wolves are expanding. As an example, after installation of electric fences, the damage suffered by holdings from wolf attacks decreased by 100\% in Portugal, 99% in Spain and 58% in Italy. In France and Croatia measures were adopted for intersectoral involvement (tourism and agriculture), which have contributed to the development of a participatory approach for wolf management. The experiences acquired during the COEX project are in the process of being transferred to other places through the implementation of the LIFE EX-TRA and LIFE WOLFNET projects. The former involves Italy, Bulgaria, Greece and Romania, while the latter is implemented in three national parks in Italy. The results obtained are encouraging and future LIFE Nature Projects should capitalise on the experiences done, making use of studies and researches that will allow the maximisation of efficacy of adopted management measures.

Establishing and Upgrading Physical Therapist Education in Developing Countries: Four Case Examples of Service by Japan and United States Physical Therapist Programs to Nigeria, Suriname, Mongolia, and Jordan

Background and Purpose. Physical therapist (PT) professional (entry-level) education programs are non-existent in most developing countries. The PT professional education programs that do exist in some developing countries may need to upgrade and strengthen their existing curricula. Many of these programs are in significant need of additional PT faculty and advanced education for their existing PT faculty. Efforts to create or enhance PT education programs in developing countries are frequently limited by inadequate human and material resources. Method/Model Description and Evaluation. This paper describes and discusses 4 models of collaboration and international partnerships in which educators and PT education programs in developed countries with more human and material resources partner with universities in developing countries to help establish or enhance PT education programs. Experiences of PT education programs in the US working with programs in Nigeria, Suriname, and Jordan, and a program in Japan working with a program in Mongolia, are presented. Outcomes. The models of collaboration between service and host countries resulted in provision of online education to practicing physical therapists, consultation on curriculum, short-term teaching/consulting for existing and developing programs, and PhD education of foreign PTs with the intention that they return to their country of origin as faculty members in PT programs. Discussion and Conclusion. These case examples of international collaboration serve as models for other PT programs and educators to form similar international partnerships and to develop new models. BACKGROUND AND PURPOSE Developing countries carry over 55% of the global burden of disease, but have less than 15% of global health care work force.1 In contrast, many developed countries carry a lower global burden of disease, but have a relatively higher ratio of the global health care work force. For instance, the United States with 10% of the global burden of disease has 25% of the global health care work force; whereas Africa, which carries 25% the global burden of disease, has less than 4% of the global health care work force.24 The shortage of health care work force in developing countries is exacerbated by migration of health care professionals from resourcepoor to resource-rich countries. This concept of migration is known as brain drain.2,510 Often, health care professionals in developing countries immigrate either for pecuniary reasons, a poor practice environment and infrastructure, or political instability.7,1113 To address this disparity and perennial health care work force shortage, efforts by the World Health Organization (WHO) and governments in developed and developing countries primarily focus on training, retention, and recruitment of health care professionals in medicine, nursing, pharmacy, dentistry, imaging, and medical laboratory sciences.14,15 For instance, education and training specific to medical and nursing health care professionals in sub-Sahara African countries have received a tremendous boost in funding, human and material resources, and partnerships with many US academic institutions.1618 These initiatives, driven by HIV/ AIDS funding, have been funded by the US government through the Medical Education Partnership Initiative (MEPI)19,20 and the Nursing Education Partnership Initiative (NEPI)21,22 programs of the National Institutes of Health (NIH) and the US President's Emergency Plan for AIDS Relief.19,20 Specifically, successful upgrading of nursing education programs through NEPI have been reported for Botswana, Lesotho, Kenya, Malawi, Zambia, Brazil, India, Thailand, and the Philippines.22 The Consortium of Universities for Global Health,23 comprised of over 50 academic institutions in the United States and Canada, and funded by the Bill and Melinda Gates Foundation and the Rockefeller Foundation, currently partner with developing countries and their institutions. …

Progress in soundscape research requires a common agenda

It is commonly believed that progress and success in any field requires competition. This is probably true, but this belief implies that all competitors have a common view on the objectives. There would not be much competition if all parties ran off in opposite directions, striving to achieve different goals. Nor would it lead to much progress. The present session calls for networking and international collaboration in soundscape research. For such collaboration to be successful, it is critical to agree on a common agenda; a mission; an objective. Recent development in soundscape research makes evident that the objective must be practical and applicable. Our minds must be set to implementing soundscape research in practice to avoid exhausting academic debates, which tend to be ends in themselves and do not contribute to progress. Two excellent, recent examples of international collaboration in soundscape research, contributing to progress, are ISO/TC 43/SC 1/WG 54 and the European COST Action TD0804 “Soundscape of European Cities and Landscapes.” Both illustrate the need for international and interdisciplinary collaboration among acousticians, architects, and urban planners to accelerate progress in soundscape research. The present paper presents possible topics for a common agenda in soundscape research.

International, collaborative nursing

It is a truism that nursing has become more and more international in its scope and inter-relationships. This is one of the great benefits of the globalisation of the world economy, together with electronic communication and ease of air travel. Every time I put together a new collection of articles for publication in INR, I am amazed at the range of countries represented. In this issue for example there are authors from Belgium, Cyprus, Finland, Israel, Jordan, Nigeria, Portugal, Republic of Ireland, South Korea, Sweden, Taiwan, Turkey, Uganda and the USA. Not only are authors from individual countries represented, but also at least five of the articles in the current issue represent collaborations between nurses from different countries. If we were also to include the nationalities of the reviewers of all these articles, the list would be even longer. We should not underestimate the obstacles that may have been overcome in order to achieve this collaboration. Differences in language (written, possibly using different alphabets, and spoken), resource availability, time zones and, above all, subtle differences in cultural interpretations of nursing, health care and life in general. It is also a truism that engaging in international collaborative research and publication demands huge personal, voluntary commitment on the part of all participants. ‘Day jobs’ do not disappear whilst all this activity is undertaken. Caring, teaching, and personal lives cannot simply be ‘put on hold’. That so much has been, and is being, achieved internationally is a matter for celebration amongst nurses everywhere. This collaboration is the positive ‘other side of the coin’ to the conflict that I wrote about in the September issue (57.3). A briefing (RCN 2010) from the International Department of the Royal College of Nursing, England (http://rcn.org.uk) gives an update on the European Year of Volunteering 2011. The RCN gives as an example of international collaboration in nurse education the case study on a Canadian-Bangladeshi partnership published in INR (Berland et al. 2010) (currently on the front page of the ICN website: http://www.icn.ch). The RCN cites amongst the mutual benefits derived from the partnership: the material resources made available; the sharing of expertise between high and low income countries; professional development; and interaction between faculty and students. This seems to be a classic example of what the Europe for Citizens programme, which includes the European Year of Volunteering within its brief, aims to achieve ‘in bringing people closer together’. The International Council of Nurses (ICN), of course, extends its brief internationally to nurses in more than 130 countries. In promoting participation and shared values, achieving intercultural dialogue, employment, social cohesion and sustainable development (European Commission – Citizenship 2010), it appears that nursing, once again, leads the way!

International organizations and NGOs: An example of international collaboration to improve women's health by preventing unsafe abortion

International collaboration with organizations and agencies is a basic requirement for the success of the FIGO Initiative for the Prevention of Unsafe Abortion and its Consequences. Many activities being carried out by the organizations form a part of the plans of action of all countries participating in the Initiative. It was, therefore, not difficult to obtain their collaboration in implementing the plans of action. The many ways in which they have collaborated and continue to do so are described in this article. This collaboration has saved time, avoided duplication of effort, and has also satisfied the Accra Agenda of Action by reducing fragmentation of funding. It has already contributed toward preventing unsafe abortion and reducing abortion-related maternal deaths and morbidities, and is expected to contribute even more significantly in the coming months and years.

The design and construction codes for nuclear power plants worldwide have been established based on ASME Sec. III. Since the requirements of fatigue evaluation were first introduced in the ASME codes in 1963, the fatigue evaluation methods have been used with few changes. In 2006, the world's first consensus codes for the environmental fatigue evaluation method have been established in JSME. The codes incorporate the results of international research projects mainly led by Japan on the environmental fatigue, that is, the reduced fatigue life of components in a high-temperature reactor coolant environment. This is a good example of international collaboration involving code engineers with expertise in the field of codes and standards. In these projects, the Japanese team contributed to the establishment of the world-leading codes for environmental fatigue evaluation. This paper describes the activities leading to the establishment of the codes for environmental fatigue evaluation, development of the evaluation method, roles of code engineers and remaining technical issues to be addressed.

African languages — is the writing on the screen?

The trends emerging in the natural language processing (NLP) of African languages spoken in South Africa, are explored in order to determine whether research in and development of such NLP is keeping abreast of international developments. This is done by investigating the past, present and future of NLP of African languages, keeping especially the multidisciplinary nature of the field and the role of the linguist in mind. A Human Sciences Research Council (HSRC) report of 1986, expressed concern about the backlog in South Africa regarding NLP, and called for dynamic action. As computational power increased and became less expensive, more interest began to be shown in NLP in South Africa. Pockets of expertise that have developed at various institutions over the past 20 years are discussed and the importance of cooperation in the field, across disciplines, is illustrated in this paper. In order to facilitate coordinated action and prevent the duplication of language resources and the development of basic enabling technologies, the implementation of the concept of the Basic Language Resource Kit (BLARK) is recommended, while a new project, which aims to create a platform for WordNet development for African languages, is cited as prime example of international collaboration.

ACROPOLIS: An example of international collaboration in the field of energy modelling to support greenhouse gases mitigation policies

Energy models are considered as valuable tools to assess the impact of various energy and environment policies. The ACROPOLIS initiative, supported by the European Commission and the International Energy Agency, used up to 15 energy models to simulate and evaluate selected policy measures and instruments and then compare their impacts on energy systems essentially in terms of costs of greenhouse gas emissions (GHG) reduction and energy technology choice. Four case studies are formulated considering policies and measures on renewable portfolio schemes and internationally tradable green certificates, emissions trading and global GHG abatement target, energy efficiency standards and internalisation of external costs. The main focus of the project is on the electricity sector. From a large set of quantified results, ACROPOLIS provides an international scientific consensus, on some key issues, which could be useful in assessing and designing energy and environment policies at the world, European and national/regional levels. It concludes that the Kyoto targets (and their continuation beyond 2010 in specific scenarios) could be achieved at a cost around 1% of GDP through global emissions trading, indicating also that this flexibility mechanism is a more cost-effective instrument for GHG mitigation than meeting the goal domestically without trade. It demonstrates that internalising external costs through a price increase reduces local pollutants (SO x , NO x , and others) and it produces other benefits such as triggering the penetration of clean technologies in addition to the curbing of CO 2 emissions.

Reliability-Based Design and Construction Issues for a Micropile Foundation in Costa Rica

The paper describes the geotechnical and structural, reliability-based design of micropile foundations for a factory in Costa Rica, along with its construction issues. The chaotic nature of the very loose soil led the designers to substitute micropiles for the driven piles originally called for in the final design. The same chaotic nature of the soil made any deterministic prediction of the pile lengths impracticable, and the designers developed an original and \Iad hoc\N reliability-based procedure to cope with the problem. The area was seismically active, with a maximum considered earthquake spectral response acceleration at short periods (0.2 s) equal to 1.04 g. In order to lower costs and to improve the surrounding ground, the micropiles were specially engineered by the designers, who acted as consultants for the foundation specialist subcontractor. The field test gave excellent results, and negligible elastic deformations were recorded up to 1.5 times the design load. This is a positive example of international collaboration among a Costa Rican client, a Spanish general contractor, and Italian subcontractors and consultants. The mutual understanding, knowledgeability, and open-mindedness of all parties involved (client, general contractor, subcontractors, designers) resulted in a reliable and cost-effective design and construction.

Factors involved in international scientific collaborations in Ethiopia, using a research project on schistosomiasis as an example

Ethiopia is host to a large number of international collaborative research projects in the areas of science, medicine and health. This paper attempts to (1) Give a brief introduction to Ethiopia and its health care system, including its primary and referral health care institutions and its medical education; (2) Briefly discuss four examples of international collaborations in Ethiopia, on very different scales: National HIV/AIDS Programme, Gondar Chronic Illness Project, Dabat and Butajira Health Surveillance Projects, a local schistosomiasis project and related laboratory studies; (3) Summarize some of the lessons learned from these projects and to suggest some common characteristics of successful international projects.

The big ears of radio astronomy

The special value of radio astronomy lies in the probing of extreme conditions in the universe, including the highest energies and the lowest temperatures. Radio waves can penetrate clouds of gas and dust to reveal objects in the universe and, in particular, in our Galaxy that cannot be seen by visible light. To achieve the highest resolution, radio telescopes in widely separate parts of our globe combine their reception to produce a synthesized image. This is a splendid example of international collaboration. Among the images visualized are pulsars, derived from the remnants of supernovae explosions, and quasar sources powered by black holes.
