The Triple Layer Graft Protocol to Repair Esthetically Compromised Implants: Rationale, Technique, Indications, and Contraindications

ABSTRACTObjectivesThese two clinical case reports show the use of the Triple Layer Graft procedure along with the Decoronation procedure to help restore normal contour and height of tissue The procedure was highly effective at restoring the esthetics that the patients needed on their implants. Short and long term results along with the step by step technique are shown.Clinical ConsiderationsTwo patients of 33 and 25 years of age both had significant reduction in the height over their implants in the #7 and #10 locations. The first patient had the implants placed 15 years ago and the second patient had them done 5 years before our treatment. Current techniques to remedy esthetic issues created by malpositioned implants consist of soft and/or hard tissue grafting but tend to be utilized independently of each other in discrete procedures as opposed to being combined in one surgical protocol. The Triple Layer Graft (TLG) surgical protocol is novel in that it incorporates all three layers of grafting to address both soft and hard tissue deficits in one procedure done at one time after doing a decoronation technique. The TLG surgical and prosthetic protocol has been previously published by these authors, but this article will discuss the rationale along with the technique and indications and contraindications, illustrated via case reports. The authors will also report on the long‐term results of this technique.ConclusionsThe Triple Layer Graft TLG) in combination with the Decoronation technique is an effective method in managing mild as well as severe esthetic defects over the facial of previously placed implants.Clinical SignificanceThe Triple Layer Graft will allow implants that were previously removed in order to place a new implant along with ridge augmentation may be able to be saved. This is a wonderful procedure for the patient since the implant does not have to be removed and replaced.

Integrated RNA Sequencing Analysis Revealed Early Gene Expression Shifts Associated with Cancer Progression in MCF-7 Breast Cancer Cells Cocultured with Adipose-Derived Stem Cells

Breast cancer remains a prevalent global health challenge, with tumor-removal surgeries being among the most common treatments but often leading to aesthetic defects. Adipose-derived stem cell (ADSC)-enriched fat grafting in breast reconstruction offers promising therapeutic benefits. However, concerns about its oncological safety persist, particularly regarding the potential risks of promoting cancer recurrence. This study investigated the effects of ADSCs on breast cancer progression by coculturing ADSCs with the MCF-7 breast cancer cell line for a short cell cultivation period of 3 days. We performed an RNA-seq analysis to identify significant transcriptomic changes in cocultured MCF-7 cells and carried out functional enrichment analyses to uncover key biological pathways influenced by ADSCs. Our findings revealed that transcriptomic alterations in MCF-7 cells are linked to aggressive cancer traits, including the upregulation of epithelial–mesenchymal transition (EMT) and the HIF-1 signaling pathway, which indicate a shift toward aerobic glycolysis. Some of the observed gene expression changes also correlated with relapse risk and mortality. These findings underscore the need for further research to explore the implications of these genes and pathways in driving aggressive cancer phenotypes and assess the safety of ADSCs in clinical settings.

Perda precoce dos elementos 11 e 12 por trauma dental, com intervenção ortodôntica preventiva na odontopediatria: relato de caso

Dental trauma is one that affects the hard tissues embraced, supporting structures or soft tissues in the oral cavity. It is observed more frequently in male children of school age (07 to 12 years old). The most affected teeth in the trauma taken are central and lateral incisors. With regard to tooth avulsions, the protection of children's bone tissue favors a higher frequency in this population than in the adult population. Dental avulsion occurs after displacement of the element out of its alveolus. There is a break in the blood supply to the pulp of the tooth and when this happens the chance of replantation achieved is totally impossible. Thus bringing several harms to the patient such as difficulty in speech, poor aesthetics, or even mesialize or distalize some other acquired element. Space maintainers are used as removable or fixed appliances to treat malocclusion or esthetic defects. They are braces made of acrylics with two extended teeth. Choosing a removable appliance comes with benefits such as better hygiene, satisfactory aesthetics, low cost, and maintenance of the cervico-occlusal space, in addition to the mesio-distal space. The objective of this study was to report a clinical case of avulsion of element 11 and 12 caused by trauma in childhood with the use of a space maintainer. We conclude that it is possible to treat a patient with dentoalveolar trauma with intervention using a space maintainer, since in the present case success was obtained in the selection of therapy, restoring aesthetics and function to the patient. Thus reducing possible orthodontic problems of the child, with follow-up until his/her old age.

Study of the use of La Roche-Posay Dermatological Laboratory skincare products for radiodermatitis prevention and treatment

Risks of adverse effects during therapy increase along with progress in oncology and development of new treatment methods. Thus, radiotherapy, one of the main methods of treatment of malignant tumors of various locations, can cause radiodermatitis: an adverse side effect manifesting on the skin in the irradiated area or near it. Clinical manifestations vary from stage to stage and negatively affect patients’ willingness to undergo or continue treatment due to fear of esthetic defects of the skin. Additionally, development of advanced stage radiodermatitis carries the risk of infectious complications in this area. One of the goals of radiation therapists is management of radiation-induced dermatitis and search for techniques aimed at its treatment and prevention.This study evaluates the effects of several products and their components on clinical manifestations of radiodermatitis caused by radiation therapy.

Effectiveness of results of complex treatment of pathological wear of hard tissues of teeth

Background:Tooth wear is a multifactorial condition that results in the loss of hard dental tissue. Physiological wear does not usually result in any subjective symptoms. Pathological abrasion of hard dental tissues is accompanied by both morphological and functional disorders of the dental system Objective: To evaluate the functional and aesthetic results of complex treatment of pathological abrasion of dental hard tissues. Materials and methods: This is a prospective controlled study that included 82 patients with a clinically confirmed diagnosis of Increased tooth wear of the first and second degree of severity (from 2020 to 2024). The age of the patients ranged from 34 to 68 years (48 men, 34 women). The examination of patients included: collecting complaints and identifying concomitant somatic diseases, X-ray examination, photographs of the patient's face, bite height, and nasolabial folds. Distribution of the group: Group 1 included patients with the first degree of hard tissue loss 47 (29 men, 18 women) Group 2 included patients with the second degree of hard tissue damage 35 (21 men, 14 women). During examination, patients complained of aesthetic defects caused by tooth wear and discoloration, discomfort when closing, chewing and phonetic disorders, typical symptoms of TMJ dysfunction, such as pain and crunching in the joints, fatigue of the masticatory muscles and muscle pain, displacement of the lower jaw to the side during vertical movements, a feeling of distension in the ears, headaches and bruxism. In patients of group 1: tooth wear was restored with orthopedic structures. In patients of group 2: temporary prosthetic structures and occlusal splints were used to normalize occlusal relationships in case of TMJ and masticatory muscle dysfunction. Gradual raising of the bite was carried out by means of a set of kappa-splints for 14 days, 1 month and 3 months from 1.0 to 5.0 mm until complete restoration of the occlusal height depending on the severity of pathological abrasion of teeth, abrasion of teeth restored by orthopedic structures. Dynamic observation of the clinical condition of the prosthesis structure was carried out in the following periods: 1 week, 1, 6, 12, 24 months,assessment of the condition of the performed restorations were recorded in the medical record of the dental patient. Results:Clinical radiological results showed satisfactory results and are encouraging in patients of groups 1 and 2. After treatment in patients of groups 2, according to the analysis of the lateral teleroentgenogram of the head, an increase in the height of the lower third of the face is noted, in 28 patients the nasolabial folds were smoothed out. Patients were satisfied with the aesthetic and functional results of the treatment Conclusions: The results of the study showed that treatment using occlusal splints at the preparatory stage in patients of group 2 with occlusion disorders due to pathological abrasion of hard dental tissues and restoration with orthopedic structures is relevant for normalizing the occlusal relationships of the jaws and this complex treatment method provides a good aesthetic and functional result.

Nonsurgical Rhinoplasty after Rhinoplasty: A Systematic Review of the Technique, Results, and Complications.

Introduction: Surgical rhinoplasty is a complex procedure with a high revision rate. Nonsurgical rhinoplasty (NSR) could avoid secondary rhinoplasty allowing the correction of postsurgical defects. A systematic review has been performed among adult patients who had previously undergone surgical rhinoplasty and now presenting for NSR with filler, demonstrated most common indications, fillers, and complications in this technique. Materials and Methods: A systematic review was performed according to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) system guidelines. Primary outcomes included indications for NSR in patients with prior rhinoplasty and complication rate. Secondary outcomes included filler material and degree of patient satisfaction. Results: Twenty articles met the inclusion criteria, obtaining 2,048 patients analyzed in the review. Hyaluronic acid was the most used filler, found in 67% of patients. Indications were highly variable, considering deformities of the middle third of the nose the most remarkable. A high degree of satisfaction was found in the analyzed studies and the rate of major complications was low. Discussion and Conclusions: NSR in patients with prior rhinoplasty is a useful option for correcting a range from subtle aesthetic defects to severe nasal deformities. However, this technique is not exempt from complications, since an increased risk of skin necrosis has been observed.

An esthetic flap design for surgical endodontics in the anterior zone.

To describe an esthetic flap design for surgical endodontics in the anterior zone and to discuss the treatment outcome. Traditional flaps used in apical surgery in the anterior region are created with horizontal and vertical incisions. These incisions do not completely avoid scarring, and may sometimes cause esthetic defects. We describe an envelope flap with no vertical incision for apical surgery in anterior that mainly consists of oblique incision in interproximal papilla. The approach achieve rapid would healing while avoiding scar formation. The present report describes successfully treated cases of surgical endodontics in the esthetic zone using an envelope flap that the author recommend in order to achieve better esthetic results.

Comparative study of orbital volumes according to multispiral computed tomography data

BACKGROUND: It is believed that accurate reconstruction of the correct anatomy of the orbital bony walls, restoration of symmetry and volume are necessary for a favorable functional prognosis and elimination of an aesthetic defect. The main problem when estimating orbital volume is its complex anatomy. Scientists use various methods to determine orbital volume, and in reviews comparing various methods for calculating orbital volumes, there is no data on which orbital volumetric method is the most accurate, since each of them has both advantages and disadvantages. AIM: The aim of this study is the analysis and the comparison of orbital volumes according to multispiral computed tomography data of healthy patients without bone-traumatic changes of orbital walls. MATERIALS AND METHODS: To measure the volume of orbits, 50 random patients were selected who were examined in the radiology department No. 2 of the University Clinical Hospital No. 1 of the Sechenov University from 2023 to 2024 and who underwent multispiral computed tomography of the facial skeleton for indications unrelated to orbital conditions. Computed tomography data from 25 women and 25 men of different ages (from 18 to 85 years old) were analyzed. A total of 8 groups were formed depending on gender and age: group 1 — women 18–25 years old, group 2 — men 18–25 years old, group 3 — women 26–35 years old, group 4 — men 26–35 years old, group 5 — women 36–50 years old, group 6 — men 36–50 years old, group 7 — women 51 and older, group 8 — men 51 and older. Multispiral computed tomography of the facial skeleton was performed using the Aquilion One 640 CT scanner (Japan) with 0.5 slice thickness in bone and soft tissue windows, to be able to calculate orbital volumes at the workstation, all bone contours of the orbits were marked on each axial slice, starting from the upper wall to the bottom of the orbit with a representation of the orbital volumes in ml. RESULTS: The difference in the volumes of the right and left orbits of 0.5 ml or more was detected in 5 women out of 25 cases, among men the difference was present in 12 cases out of 25; a difference in volumes of more than 1 ml was noted in 1 woman and 2 men; a difference of more than 1.5 ml was not observed in any study group. The orbital asymmetry coefficient in women ranged from 0 ml to 1.21 ml, in men — from 0.08 ml to 1.19 ml, in women, the average volume of both orbits is less than in men. It was also detected that the volume of orbits increases with age, both in men and women. CONCLUSIONS: Considering that the majority of patients had differences in orbital volumes of up to 1.0 ml, we agree with studies that indicate the possibility of using a healthy contralateral orbit as a guideline when planning reconstructive surgery. However, for planning surgical treatment, it is recommended to use manual contouring, as this allows the most accurate reproducibility of the bone boundaries of the orbit, although it requires more time than semi- and automatic segmentation. Also, the advantages of the used method for calculating the volumes of orbits in this study include reproducibility on any workstation from different manufacturers, since the method is carried out on a standard tomograph workstation and does not require any additional software.

Temporary Skeletal Anchorage Devices and Cone Beam Tomography in Orthodontics—Current Application and New Directions of Development

Continuous progress in dentistry and orthodontics is crucial to ensuring high-quality diagnosis and treatment of patients, especially since malocclusions occur in up to half of the population. In addition to limiting the physiological functions of the masticatory system, they are often an aesthetic defect that may directly affect the well-being and even self-esteem of patients, especially in their teenage years and early adulthood. A holistic model of perceiving and treating orthodontic diseases, such as the Biocreative Orthodontic Strategy, focusing not only on the correction of the defect itself but also taking into account the least possible interference in the physiology of the masticatory system, limiting the use of appliances to a minimum, and taking into account the patient’s preferences, is a special alternative to conventional therapeutic models. In this review, we are presenting the current knowledge regarding the applications of temporary skeletal anchorage devices (TSAD) and cone beam computed tomography (CBCT) in orthodontics.

Multidisciplinary clinical decision-making of anterior diastema closure.

Anterior diastema is a common esthetic defect in China. The general treatment for a patient with diastemata, including orthodontics and direct and indirect restorations, is a multidisciplinary clinical procedure covering the orthodontics, operative dentistry, general dentistry, and prosthodontics department. Given the diversity of departments and the complex etiology of this defect, decision-making regarding the closing methods and time selection is undefined and unintegrated, which makes the long-term stability of closure unpredictable. This article proposed an etiology-based decision tree with actual measurement of diastemata width for diastemata closure. The decisional steps include classifying the etiological factors based on patients' medical history and clinical manifestation to evaluate the stability of diastemata. After maintaining the stability of diastemata, contemporary and multidisciplinary treatment plans were selected in accordance with the measured width of diastemata and patients' cosmetic psychology, economics, and available time. These decision trees focus on the challenges of collaboration among dental departments, propose an objective and efficient ways for connections, and promote efficient and effective diastemata closure.

Nasal profile changes after LeFort I osteotomy (maxillary advancement) with and without ANS reduction in class III skeletal patients: a study protocol for a randomized clinical trial

BackgroundDentofacial malformation is a common condition that affects a significant portion of the population, resulting in functional and aesthetic defects. Orthognathic surgeries, such as LeFort I osteotomy, are performed to correct these abnormalities. However, the impact of these surgeries on nasal profile changes remains unclear. Additionally, the role of anterior nasal spine (ANS) reduction in maxillary advancement surgeries of 3–5 mm range is yet to be determined. This study aims to investigate the effect of ANS reduction on soft tissue profile changes following LeFort I osteotomy with a maxillary advancement range of 3–5 mm in class III skeletal patients. The hypothesis is that the changes in nasolabial angle and upper lip length will not significantly differ between patients who undergo LeFort I osteotomy with and without ANS reduction.Method and designThis study is designed as a randomized controlled trial. A total of 26 class III skeletal patients with maxillofacial abnormalities will be recruited from the maxillofacial clinic of Bu-Ali and Farahikhtegan Hospitals in Tehran, Iran. Patients meeting the inclusion criteria will be randomly assigned to two groups: one group will undergo LeFort I osteotomy with ANS reduction, and the other group will undergo LeFort I osteotomy without ANS reduction. The soft tissue profile changes, specifically the nasolabial angle and upper lip length, will be evaluated and compared between the two groups.DiscussionAchieving facial harmony through orthognathic surgery requires careful planning and consideration of the impact on surrounding soft tissue. The primary objective is to predict and plan for the effects on the nasolabial region. LeFort I osteotomy is a common procedure used to correct dentofacial deformities, particularly in class III patients. Maxillary advancement during this surgery can lead to changes in nasal tip position, width, and rotation, potentially due to repositioning of the anterior nasal spine and soft tissue dissection. In this study, soft tissue changes will be assessed in non-growing class III patients using cephalometric radiographs. The impact of reducing the anterior nasal spine (ANS) on nasal profile changes will be investigated for maxillary advancements of 3–5 mm. Objective measurements and patient-reported outcomes will be evaluated to gain insights into the aesthetic outcomes of orthognathic surgery. Trash, Art, and the Comics

Abstract Many comics are aesthetically trashy: They are immediately grasped and easily available. Historically, this trashiness is lobbed as an aesthetic defect of many comics, a defect for both their production and their appreciation. To defend these comics, some point to non-aesthetic values, like sociality. I argue that there is aesthetic value to these comics, and that it lies precisely in their trashy characteristics: their immediacy and availability. Many comics have these characteristics because many comics are cartooned. The immediacy of cartooning is precisely what makes so many ordinary comics beautiful in an ordinary way.

Development and Validation of a Novel Surface Defect Index (SDI) Method for the Effective Quality Evaluation of Concrete Surfaces

Concrete defects have a significant impact on concrete constructions. These defects should be considered not only aesthetic defects but also structural defects. In this study, a novel Surface Defect Index (SDI) method was developed to quantify the defect volume according to liquids’ penetrating properties by applying ready-mixed plaster (RMP). The SDI refers to the volumetric proportion of all apparent and unapparent defects in a given area of concrete, and it is expressed as a percentage of the total volume affected by defects. The proposed SDI method was validated and tested under various controlled defect configurations. Regardless of the specific characteristics of each defect configuration, the SDI method consistently demonstrated a high level of consistency, repeatability, and reproducibility, with coefficients of variation (CVr and CVR) below 5% and with correlation coefficients of R2 = 0.9968. The method succeeded in assessing the surface quality levels through the SDI, demonstrating a significant correlation between this index and the volume of defects. The proposed index was tested on real concrete surfaces, affirming its efficacy in accurately quantifying the volume of surface defects; thus, it can provide an important metric for quality control. Moreover, it provides an excellent evaluation of the quality of concrete surfaces.

Alternative surgical restoration of aesthetic defect during superficial parotidectomy – Case report

Tumors in the parotid gland, which is the largest salivary gland, could appear as a swelling in the cheek area or in the area in front of and below the earlobe. The majority are benign tumors. In all cases, removal of the tumor by parotidectomy is the main option. In the majority of these operations, postoperatively a defect remains in the lateral region of the face after the removal of the tumor. Many solutions have been proposed internationally to treatment this aesthetic defect. In this study, a proposal is presented to use part of the remaining parotid tissue, to restore the defect that appears after the removal of the tumor and the preparation of the facial nerve.

Uveitis complicated by phthisis bulbi: hemodynamic parameters in the prediction of anterior-posterior eye size reduction

Background. Phthisis bulbi is a serious complication of uveitis, causing vision loss and esthetic defects. Hemodynamic disorders can play an important role in the formation of this complication as a result of uveitis. We did not find data on predicting the progression of phthisis bulbi with a reduction in anterior-posterior size in the available literature. Aim. To analyze the possibility of predicting the reduction in the anterior-posterior size of the eye with uveitis, complicated by phthisis bulbi, based on hemodynamic parameters Materials and Methods. 33 patients aged 5–84 years, of them 19 male and 14 female patients, with unilateral endogenous uveitis, complicated by phthisis bulbi, were examined. 15 patients with uveitis complicated by phthisis bulbi, who had not received any treatment during this time, were examined in dynamics (for at least 1 year). All patients underwent Doppler ultrasound examination of the ophthalmic artery and short posterior ciliary arteries. In addition, all patients underwent biomicroscopic and tonometric examinations, ultrasound biomicroscopy, rheoophthalmography of eyeballs, A-scan and B-scan ultrasonography. The results of the examination of eyes with uveitis complicated by phthisis bulbi were compared with the results of the examination of paired (healthy) eyes of the same patients. Results. We have developed a multiple regression model that allows us to predict a reduction in the anterior-posterior size of the eye with phthisis bulbi due to uveitis. The dynamics of the disease in 15 patients not taking any treatment for uveitis, has been monitored for more than a year. The prediction of phthisis bulbi progression was confirmed in 13 patients, which amounts to 86.7% (p<0.05). Conclusions. We have proposed a model that allows predicting the progressive or stationary course of phthisis bulbi due to uveitis based on hemodynamic parameters. A novel technique myomucosal rotational upper lip flap for anterior palatal fistula closure

ABSTRACT Palatal fistulas are a pathological condition where there is a connection between the nasal area and the oral cavity, being one of the most frequent complications in cleft palate surgery. Palatal fistulas can cause esthetic defects and functional alterations in both feeding and speech of patients. Anterior palatal fistulas are the least common but represent a great surgical challenge for the surgeon because of their location and poor amount of tissue for closure. The purpose of this article is to present a novel technique of performing an upper lip myomucosal rotation flap for the closure of an anterior fistula, taking advantage of the labial tissue for a tension-free suture in the area of the fistula. Our technique allowed an adequate management of the fistula with low morbidity and without flap necrosis, proposing it as a useful technique for the closure of anterior fistulas.

Lipotranferences in post neurosurgical esthetic defects

Background: There are numerous procedures in which, beyond adequate manipulation of the temporalis muscle and cranial closure, patients may present bone and muscle defects due to atrophy and consequent facial asymmetry, causing psychological discomfort and functional deterioration. The objective of our work is to combine the knowledge of plastic surgery and apply it to cranial reconstructions with fat transfers in post-neurosurgical patients, analyzing its results. Methods: During the year 2022, 45 fat transfer procedures were performed for the correction of craniofacial defects, of which 29 were female and 16 were male. All had a surgical history of pterional craniotomies and their variants, orbitozygomatic and transzygomatic approaches, with the consequent volume deficit. Results: The procedure was performed on an outpatient basis, with local anesthesia, and in an average time of 30–40 min. The lower hemiabdominal region was used as the donor area, processing the fat using the decantation technique and injecting it into the receptor area at the craniofacial level. The patients tolerated the procedure adequately without intraoperative complications or superadded events. Conclusion: Fat transfer is a minimally invasive, effective, and cost-effective technique that plastic surgery offers us to implement in post-neurosurgical patients, as it achieves natural results that stand the test of time.

Psoriatic onychodystrophy: Clinical course, diagnosis and therapeutic aspects

Psoriatic onychodystrophy is observed in about 80% of patients with psoriasis. Psoriasis can affect two nail structures ― the matrix and the nail bed.
 Involvement of the nail matrix in the pathological process can lead to the formation of leukonychia, thimble symptom, red spots in the area of the hole and destruction of the nail plate. On the other hand, the lesion of the nail bed is characterized by the formation of onycholysis, subcutaneous hyperkeratosis and subcutaneous hemorrhages.
 Nail lesions in psoriasis are accompanied by functional disorders and aesthetic defects, and also indicate a severe course of psoriasis.
 Treatment of psoriatic onychodystrophy includes topical medications, systemic therapy in cases accompanied by extensive skin lesions.
 This article is an overview of the main clinical features, diagnostic issues and evaluation methods, as well as treatment options for nail psoriasis.

Percutaneous laser coagulation of dilated intradermal veins: from theory to practice

Review authors consider the current possibilities of percutaneous laser coagulation for telangiectasias and reticular veins, which are the most common cosmetic defects of vascular genesis, occurring in more than 80% of women of different age groups. This vascular pathology constituting an aesthetic defect and causing local physical discomfort is one of the most common indications for phlebosclerosing therapy and percutaneous laser coagulation. The authors present the most effective and safe guidelines for percutaneous laser coagulation based on analyses of a large volume of specialized literature. Due to the use of state-of-the-art Nd:YAG lasers, percutaneous laser coagulation is increasingly used in patients with various types of intradermal vein lesions and, owing to high efficiency and safety, has advantages over compression sclerotherapy by several criteria. Nd:YAG lasers can be divided into long- and short-pulse ones according to their technical characteristics. Long-pulse lasers provide coagulation of the target vessel due to a single pulse with a high energy density (fluence), while short-pulse lasers, on the contrary, generate a series of pulses to the target vein with a running time of fractions of a millisecond, which ensures the conversion of hemoglobin into methemoglobin with a ten-fold increased ability to absorb laser energy and convert it to heat.
