Extra facial esthetic indications of botulinum toxin: A review

Harnessing of the most potent neurotoxin, botulinum toxin (BTX) has been one of the most significant developments in the field of esthetics. Ever since its approval by the Food and Drug Administration in 2002 for the temporary improvement in the appearance of moderate-to-severe glabellar lines, BTX has been used for numerous off-label esthetic indications, the majority being facial indications. In this review, we intend to discuss the extra facial esthetic indications of BTX which include neck rejuvenation, improving shoulder contours, calf slimming, ankle slimming, and androgenetic alopecia. We have included essential tips for each indication.

The TRICK-TIP Rhinoplasty: Tip of the Nose Preservation Using the Combined Synergy of Open and Closed Approaches

BackgroundThis study introduces and assesses the outcomes of a novel rhinoplasty technique, TRICK-TIP (Transcolumellar and Inter Cartilaginous Keystoning with Tip preservation), employing a combined open and closed approach with tip anatomy conservation and structured tip support.MethodsThe procedure involves a low stairstep columellar sectioning, followed by transmembranous and intercartilaginous incisions without skin dissection in the columella or tip. Elevating the entire mobile nose as a three-layered flap provides extensive access to the entire nasal pyramid and septum. Tip modifications, including retrograde cephalic cartilage resection and supratip skin thinning, are performed based on individual cases. A key columellar strut is frequently used, initially sutured in the interdomal space and then turned down for height adjustment and final fixation. Interdomal sutures, supratip sutures, and alar resection are implemented as needed.ResultsOne hundred twenty patients participated, with high satisfaction and a low frequency of adverse effects reported using four FACE-Q™ questionnaires. One hundred and two independent raters evaluated pre and postoperative photographs, scoring “overall nose result” as 3.6 out of 5, with minimal or absent nostril deformities (1.84), soft triangle deformities (1.73), and columellar external scar deformity/visibility (1.35) where 1 is the absence of the deformity and 5 is disfigurement. Complications were absent, and revisions were infrequent.ConclusionsThe combined benefits of the wide-open approach, shortened surgery duration, and nasal tip preservation contribute to outcome optimization. TRICK-TIP rhinoplasty is characterized by simplicity, enabling targeted modifications, preventing soft triangle and rim complications, and facilitating essential tip support while maintaining favorable results.Level of Evidence V This journal requires that authors assign a level of evidence to each article. Twelve tips for Natural Language Processing in medical education program evaluation

With the increasing application of Natural Language Processing (NLP) in Medicine at large, medical educators are urged to gain an understanding and implement NLP techniques within their own education programs to improve the workflow and make significant and rapid improvements in their programs. This paper aims to provide twelve essential tips inclusive of both conceptual and technical factors to facilitate the successful integration of NLP in medical education program evaluation. These twelve tips range from advising on various stages of planning the evaluation process, considerations for data collection, and reflections on preprocessing of data in preparation for analysis and interpretation of results. Using these twelve tips as a framework, medical researchers, educators, and administrators will have an understanding and reference to navigating applications of NLP and be able to unlock its potential for enhancing the evaluation of their own medical education programs.

Linguocultural adaptation in TV series translation

The article considers common challenges of translating culture-bound vocabulary in popular American TV series. The article is aimed to determine basic techniques of linguocultural adaptation in Russian translation of “Friends” TV series. Traditional comparative and statistical analysis methods are applied to examine culture-bound elements of the original text of the above-mentioned sitcom, and therefore, to identify adequate techniques of linguocultural adaptation in its Russian translation. Moreover, “Friends” TV series contains a huge number of phraseological units, puns and jokes adequately conveyed in its Russian translation, and therefore, keeping the target audience interested in the sitcom. Scientific novelty of the research is determined by the author’s attempt to compile some essential tips for linguocultural adaptation while translating TV series from English into Russian. Research findings have proved that achieving success in both linguistic and cultural adaption implies the translator’s ability to take into account the particular nature of both the form of the material presented and its perception by the target audience. Thus, the linguocultural adaptation whiletranslating TV series from English into Russian requires a translator to have a bicultural competence that involves background knowledge (linguistic and extralinguistic) about both the source and target cultures.

Essential tips for enhancing the opto-electrical performance of CdSe (NPs)/Epoxy resin

The current paper presents the advantages of the addition of CdSe nanoparticles (NPs) with different ratio (0, 0.25, 0.5, 0.75, 1.0, 1.25 and 1.5%) on opto-electrical properties of Epoxy resin [ER]. The organometallic method was used for preparing CdSe (NPs) in epoxy matrix solution. The morphology of the obtained nanocomposites (NCs) was characterized in terms of X-Ray diffraction, scanning electron microscope (SEM), Energy -dispersive X-ray spectroscopy (EDX), optical and dielectric properties. The UV–Visible absorption spectra for (NCs) were collected in the wavelength range (250–700 nm) and were analyzed. The absorption coefficient (α), optical energy gap (Eg), and refractive index (n) showed a clear dependence on the CdSe NPs concentration. Increasing the ratio of CdSe NPs led to the increase of the values of the absorption coefficient (α). Moreover, the results also demonstrate that the value of the direct optical band energy for [ER] was decreased from 3.40 eV to 2.69 eV with increasing the CdSe (NPs) concentration from 0.25% to 1.50% in the [ER]. It was also observed that the optical energy gap (Eg) is correlated with the optical electronegativity (Δχopt) and is affected by the increase of CdSe NPs ratio as well. The refractive index (n), the dielectric constant (ε) and optical dielectric constants (εopt) increased with the increase in CdSe NPs content. Furthermore, the optical conductivity (σopt), increases with the increase CdSe NPs content. Finally, the variation of the electronic polarizability (αe), the dielectric susceptibility (χe), transmission coefficient (T) and reflection loss (RL) with the increase of CdSe NPs ratio in [ER] were analyzed. In this work we highlight the significance of the interrelations between the above defined parameters, which were successfully predicting their effect on the performance of [ER] upon the addition of CdSe NPs.

Minimally-invasive anatomical liver resection for hepatocellular carcinoma: a literature overview with technical and anatomical tips and tricks

Since its introduction in 1985, anatomical liver resection (AR) has been performed to treat early-stage hepatocellular carcinoma. The minimally-invasive AR (MIALR) approach can be safely performed at high-volume tertiary referral centers. The resection techniques can vary among surgeons, depending on the center’s experience, patient characteristics, hepatic segment involvement, and tumor characteristics. Profound knowledge of the liver’s surgical anatomy and a standardized inflow control approach is fundamental to performing MIALR safely. This article aims to summarize the applications of the MIALR and its outcomes, focusing on the techniques for vascular inflow control and the essential tips and tricks to standardize these techniques for laparoscopic and robotic approaches.

A Two-Stage Method for Adult Congenital Microtia: The Essentials of 15-Year Experience.

Treatment of congenital microtia in adults remains challenging due to the unique physiological characteristics of the costal cartilages and retroauricular skin, which interfere with obtaining a satisfactory aesthetic result; thus, different perspectives and technical modifications during treatment are warranted. This article aims to present complementary new information and essential tips to refine the surgical technique in adult microtia reconstruction. A total of 346 adult microtia patients underwent surgical intervention at the Auricular Reconstruction Center of Plastic Surgery Hospital (Beijing, China) between 2006 and 2021. Each patient underwent a rigorous preoperative evaluation and stages one and two surgeries. Patients were followed in our clinic for 6 months to 10 years (average: 15.3 months). The postoperative complication rate was 8.1%, which included cartilage exposure, broken helix, local hematoma, infection, poor skin graft survival, and scar hypertrophy. Our results showed that the two-stage treatment strategy for adults is versatile, reliable, and effective for the treatment of congenital microtia. 4 Laryngoscope, 133:2148-2153, 2023.

Navigating Cervical Fibroids during Laparoscopic Hysterectomy: Essential Tips and Tricks

<h3>Study Objective</h3> To review three key strategies to safely navigate cervical fibroids when performing a laparoscopic hysterectomy. <h3>Design</h3> Case series of two patients undergoing total laparoscopic hysterectomy (TLH) for abnormal uterine bleeding in the setting of large cervical fibroids. <h3>Setting</h3> Both patients underwent their planned surgeries at a large-volume county hospital. Video footage was obtained during these procedures. <h3>Patients or Participants</h3> Two women who had completed childbearing and desired definitive surgical management for abnormal uterine bleeding underwent TLH. The first patient's cervical fibroid was 6.5cm in size, and the second patient's cervical fibroid was 8cm in size, confirmed on both pelvic exam and pre-operative pelvic MRI imaging. Neither patient had a significant past medical or surgical history. <h3>Interventions</h3> Total laparoscopic hysterectomy for a patient with a large cervical fibroid can be completed safely using three key strategies: 1. Ureteral identification, including ureterolysis if necessary and consideration of prophylactic ureteral catheter placement 2. Ligation of the uterine artery at its origin to minimize risk of blood loss 3. Delineation of the cervicovaginal junction using a Breisky-Navratil retractor to safely perform the colpotomy <h3>Measurements and Main Results</h3> The two presented cases demonstrate successful incorporation of the above techniques and ultimately safe completion of the laparoscopic hysterectomy for both patients. <h3>Conclusion</h3> Cervical fibroids are rare, however when present, can pose unique challenges to the gynecologic surgeon when performing a laparoscopic hysterectomy. Here we demonstrate three reproducible techniques for minimizing injury to adjacent organs and minimizing intraoperative blood loss, allowing the surgeon to safely navigate the difficulties presented by the cervical fibroid and maintain a minimally invasive approach.

Spanish Cabinet declared on Saturday 14th of March 2020, state of alarm in Span to slow coronavirus COVID-19 spread and the universities were closed. Later, lecturers were forced to turn to remote teaching in a short period of time. The transition required time and patience to learn the technology, i. e. Microsoft Teams, and identify the best teaching tactics for our chemistry and construction materials. Every day, we gave lecture with new contents and exercises by using Microsoft Teams. In our opinion, there is an essential tip: seek continuous feedback from students. Students need questions to know what they have learnt. In addition, we asked for feedback to improve the class and to make sure they understood the knowledge base. We should have into account that the biggest risk is that we could become a talking instrument teaching concepts that students are not following. In this case, students give up.

Transparency in higher education: An investigation of University students' perceptions in Italy and Russia

The primary purpose of this research is to investigate students' perceptions of the presence of transparency in their university teaching/learning processes.Effects of transparency in achievement, motivation, and anxiety were explored from the perspective of teachers', one's own, and peers' behaviour. The secondary objective of the study was to create a valid tool for investigating "transparency" perception in university educational practices, based on the theoretical assumption that transparency reduces performance anxiety, stimulates study motivation, and represents a universal construct across universities, countries, cultures and courses of study. A sample of 439 undergraduates from leading research universities in Russia and Italy completed self-report scales and reported demographic data. The scales showed internal consistency and structural invariance in both countries. Research results confirmed the hypothesis from which the investigation originated. Based on the findings, essential tips for future investigations of transparency perception at universities have been developed.

Management of community-acquired pneumonia: essential tips for the physician on call.

Community-acquired pneumonia is a common clinical problem requiring admission to hospital, with a particularly high incidence in the elderly population and those with significant comorbidities. Diagnosis is made on the combination of a short history of respiratory symptoms and systemic ill-health with new examination and/or radiological features of consolidation. Multiple other infective and non-infective conditions can mimic community-acquired pneumonia, leading to misdiagnosis in 5-17% of cases. The CURB-65 score can identify patients with community-acquired pneumonia with a higher risk of mortality, but is insensitive at identifying patients requiring intensive care support and needs to be combined with clinical markers of potential severity. Both high admission levels of C-reactive protein and the failure of levels of C-reactive protein to decline by >50% by day 4 after admission are associated with higher risk of complications, need for ventilation or inotropic support, and mortality. Empirical antibiotic therapy for most patients admitted to hospital is combination of a ß-lactam and a macrolide. Short courses of antibiotics do not result in significantly different outcomes to longer courses unless the patient has developed complications such as a complex parapneumonic effusion. Implementation of a community-acquired pneumonia care bundle into clinical practice reduces mortality, and should be a high priority for all acute hospitals.
