Fetal erythropoietin and endothelin-1: relation to hypoxia and intrauterine growth retardation

We have examined whether endothelin-1 (ET-1) and erythropoietin (EPO) in amniotic fluid, and EPO in fetal serum obtained by cordocentesis from fetuses with signs of intrauterine growth retardation (IUGR), were correlated to fetal growth and/or chronic fetal hypoxia. Amniotic fluid and fetal serum were obtained by cordocentesis from 28 fetuses suspected to have IUGR and subsequently analyzed for EPO and ET-1 by ELISA. These data were correlated to blood gas results and fetal/maternal parameters at delivery. A novel finding was that ET-1 correlated to PO2 in amniotic fluid. The average level of ET-1 in amniotic fluid was 48.3+/-4.7 pmol/L. The results also showed a correlation between EPO levels in amniotic fluid and EPO in fetal serum. Furthermore, EPO correlated weakly to birth weight at delivery. Children with the lowest birth weights had the highest EPO levels. High EPO values, similarly to ET-1, correlated to low pO2 values. The level of EPO in amniotic fluid was 8.0+/-1.6 mIU/ml and in cord blood 29.5+/-9.6 mIU/ml. The results indicate that ET-1 levels may be a marker for short-term hypoxia, but not for fetal growth, since ET-1 in amniotic fluid was correlated to PO2 at the time of cordocentesis, but not to birth weight. The results also indicate that EPO levels in amniotic fluid and in fetal cord serum are highly correlated, and thus both can be used as markers for fetal growth and chronic hypoxia before the onset of labor.

Endothelin receptor mRNAs in human fetal membranes, chorionic vessels, and decidua parietalis

Endothelin-1 (ET-1) is synthesized in avascular human amnion; and, immunoreactive ET-1 is present in amniotic fluid in concentrations 10 to 100 times those in blood. ET-1 acts, most commonly, in a local or paracrine manner; therefore, it is possible that amnion/amniotic fluid ET-1 acts on contiguous tissues, namely chorion laeve or placental surface (chorionic) vessels, or in an autocrine fashion on amnion cells. To address these possibilities, the levels of ET A and ET B receptor mRNAs were evaluated in amnion, chorion laeve, decidua parietalis, placenta, and chorionic vessel tissues. By Northern analysis of total RNA (20 μg), ET A and ET B receptor mRNAs were detected in decidua ( n=18), placenta ( n=14), and chorionic vessels ( n=13). In chorion laeve ( n=24), ET B receptor mRNA but not ET A receptor mRNA was detected by Northern analysis of total RNA. Northern analysis of chorion laeve poly(A) + mRNA (1.5–2.5 μg) revealed ET A receptor mRNA at low levels. Neither ET A nor ET B receptor mRNAs were detected in amnion tissue by Northern analysis of total RNA ( n=30; placental and reflected amnion from 15 pregnancies) or by Northern analysis of poly(A) + mRNA (1.5–2.5 μg). Moreover, there was no demonstrable dose-dependent effect of ET-1 on prostaglandin E 2 production or DNA synthesis in amnion epithelial cells in primary culture. The findings of this investigation are indicative that ET-1 in amniotic fluid or secreted from amnion may act in a paracrine fashion on chorion laeve by way of the ET B receptor and on chorionic vessels by way of ET A and ET B receptors.

Big Endothelin in Plasma and Amniotic Fluid

We previously demonstrated that endothelin-1 (ET-1) exists both in the peripheral circulation and the amniotic fluid (AF). The objects of the present study were (a) to measure the concentrations of ET-1 and big endothelin (big ET) in plasma and AF by radioimmunoassay (RIA) and (b) to characterize the molecular forms of endothelin in human AF by high-performance liquid chromatography (HPLC) coupled with RIAs. Plasma samples from healthy male and female volunteers, and AF samples from full-term and mid-trimester pregnant women were extracted by C18 cartridges. Big ET, like ET-1, exists both in plasma and in AF. Significant increases (p less than 0.01) of big ET and ET-1 in AF from mid-trimester to full-term pregnancy were observed. The reverse-phase HPLC elution profile of immunoreactive (ir-) ET-1 presented a single major peak at a position corresponding to that of the standard ET-1 in the extracted AF. Two major components of ir-big ET were revealed: one corresponded to the elution position of standard big ET; the other one to that of the standard endothelin fragment [22-38] (EF). Increased maternal plasma concentration of endothelin-1 during labor pain or on delivery and the existence of a large amount of endothelin-1 in amniotic fluid

The concentration of endothelin-1 (ET-1) in plasma and amniotic fluid from normal pregnant women was determined by a sensitive sandwich-enzyme immunoassay system, established recently. The plasma ET-1 level increased gradually during normal pregnancy as the pregnancy advanced, the levels (0.40 +/- 0.02 pmol/l, n = 45) being significantly (p less than 0.05) higher after 29 weeks of gestation than those (0.32 +/- 0.01 pmol/l, n = 30) before 28 weeks of gestation. The plasma ET-1 level during labor pain was significantly higher (0.59 +/- 0.06 pmol/l, n = 10) than that (0.40 +/- 0.02 pmol/l, n = 45) in the 3rd trimester of pregnancy without labor pain (p less than 0.02). Moreover, a high level of ET-1 (17.38 +/- 0.25 pmol/l, n = 18) was detected in amniotic fluid on term delivery. The ET-1 level in amniotic fluid was significantly higher than the levels in maternal and umbilical cord plasma (p less than 0.001 and p less than 0.001, respectively). After delivery the maternal ET-1 level decreased gradually and 2 day postpartum ET-1 levels reached the normal non-pregnant level.
