Two-year clinical performance of indirect resin composite restorations in endodontically treated teeth with different cavity preparation designs: a randomized clinical trial

Trial designThis is a randomized, controlled, superiority, double-blinded, parallel-group, two-arms trial with an allocation ratio of 1:1. This study aimed to assess whether the cavity design could affect the clinical performance of the CAD/CAM generated indirect resin composite restoration in endodontically treated teeth (ETT) evaluated using the Modified USPHS criteria after a two-year follow up.MethodsA total of 30 participants who underwent endodontic treatment for MOD cavities in permanent molars were divided randomly into two parallel groups (n = 30 restorations) according to the performed cavity design to group 1 in which there was no cuspal reduction (inlay) and group 2 in which cuspal reduction was performed (overlay). All pulp chambers were filled with bulk fill flowable composite, and the cavities were prepared following the criteria of the cavities for indirect restorations and restored using nano-hybrid composite resin blocks (Brilliant, Coltene, Switzerland). The restorations were evaluated using the modified USPHS criteria at baseline, six months, one-year and two years follow-up visits. For qualitative data, frequencies (n) and percentages (%) were used to display the data, while mean and standard deviation (SD) were used for quantitative data. The normality of the data was evaluated using the Shapiro-Wilk and Kolmogorov-Smirnov tests. For every test, P ≤ 0.05 was used as the significance threshold.ResultsTwenty-six individuals completed the follow-up period after receiving the assigned intervention.The inter-group comparison showed that, at the 6- months and 12- months observation points, the overlay design had significantly better marginal adaptation, less incidence of discoloration or tooth/restoration fracture, and similar marginal integrity and caries incidence to the inlay design. After 24- months, the overlay design still had better marginal adaptation, less incidence of discoloration or tooth/restoration fracture and less caries incidence in comparison to the inlay design, while there was no difference in the marginal integrity between either design.Conclusions and clinical relevanceCuspal reduction in endodontically treated teeth showed better clinical performance than the cusp preservation thus, the former is more reliable.

Knowledge and Practice of Iranian Dentists Regarding Post Space Preparation and Coronal Reconstruction of Endodontically Treated Teeth

Background: Due to severe coronal destruction in endodontically treated teeth (ETT), the canal space is often utilized to provide retention for the core material. Objectives: This study aimed to assess the knowledge level and practices of Iranian dentists regarding post space preparation and coronal reconstruction of ETT. Methods: This cross-sectional study was conducted with 117 dentists at Kurdistan University of Medical Sciences in 2021. Data were collected using a two-part questionnaire. The first part included 6 questions regarding the demographic information of the participants. The second part comprised 10 questions regarding the methods of post space preparation and coronal reconstruction of ETT. SPSS-20 software was used to analyze the data, calculating frequency (%), mean, and standard deviation. Results: 44.4% of the participants used amalgam along with a prefabricated post, 59% correctly identified the definition of the ferrule effect, and 87.2% believed that the ferrule effect increases fracture resistance. Additionally, 39.3% reported preparing the post space immediately after obturation; 91.5% used rotary instruments for removing root filling material, 93.2% reported cleaning the post space prior to cementation of the intracanal post, and 35.9% used saline for this purpose. Moreover, 72.9% believed that teeth with post and core necessarily require a prosthetic crown, and 94% used full-coverage crowns for ETT. Coronal fracture was reported by 62.4% as the most common cause of failure of ETT. Conclusions: The knowledge level of dental clinicians regarding post space preparation and coronal reconstruction of ETT was moderate, while their practice level was poor. The most common variables that had a significant relationship were age and academic degree.

Non-surgical endodontic treatment of a type III dens invaginatus with large perirradicular radiolucency using bioceramic materials: Case report

Dens invaginatus (DI), also known as dens in dente, is an uncommon dental anomaly in humans, characterized by irregular morphological development during odontogenesis, which can manifest in various coronal and radicular anatomies. Endodontic treatment of teeth with this variation demands anatomical knowledge, detailed planning, and the use of specialized techniques and materials. The objective of this research is to detail, through a case report, the non-surgical endodontic treatment of type III dens invaginatus in the upper lateral incisor with extensive periapical lesion, using bioceramic materials. An 18-year-old female patient was referred to the referral center by an orthodontist after radiographic examination revealed an extensive periapical lesion on tooth 12. Consequently, cone beam computed tomography was requested to assess the lesion and internal/external anatomy. The endodontic treatment was performed entirely under operative microscopy, utilizing mechanized instrumentation and thermoplasticized obturation. Disinfection was enhanced through activated irrigation technique and intracanal placement of bioceramic medication, with periapical repair achieved using bioceramic sealer cement. Long-term follow-up demonstrated that conservative endodontic treatment, employing modern techniques alongside bioceramic materials, proved effective in resolving complex cases.

Evaluating the Proximal Contact Tightness in Direct or In-direct Restoration of Endodontically Treated Teeth: Randomized Clinical Trial.

This study aimed to compare the efficacy of resin composite for establishing a proper proximal contact in comparison to digital work flow Zirconia for restoration of endodontically treated teeth (ETT). Forty patients with posterior root canal treated teeth considering the inclusion and exclusion criteria were divided into two groups: half were restored by resin composite while the other received zirconia crown. Then, the proximal contact tightness (PCT) was measured via two methods: 1: VAS: the magnitude of felt PCT was recorded as a number between 0 to 10 (Visual Analogue Scale (VAS)). VAS direct and indirect as VAS-D, and VAS-I respectively. 2: Quantitative: using a custom-made force gage device to record the amount of force needed to pass a mounted dental floss through the proximal contact (Quantitative direct and indirect as Qn-D, and Qn-I respectively) those were compared with the PCT of natural teeth (NT). Data was compared with each other using Chi-square, Shapiro-Wilk, One Way ANOVA, Tukey Post Hoc, Linear regression, and Pearson tests (α= 0.05 in all tests). There were no significant difference between direct and in-direct groups regarding either sex of the patients (P= 0.10), type of teeth (P= 0.32), or jaw side (P= 0.36). The VAS-D and VAS-I showed similar results in pairwise comparison (P= 0.21). Moreover, both the Qn-D and Qn-I showed significantly higher PCT comparing to NT (P= 0.45 and 0.0.0001 respectively) while the Qn-D and Qn-I were not distinguishable statistically (P= 0.23). Furthermore, significant correlation was observed between VAS and quantitative methods for evaluation of PCT (Pearson P value= 0.005). Both the direct and in-direct restorations lead to clinically acceptable PCT, whilst indirect restorations showed slightly better results which was not statistically noticeable. Advanced Material Strategy for Restoring Damaged Endodontically Treated Teeth: A Comprehensive Review.

The restoration of endodontically treated teeth (ETT) remains a significant challenge in modern dentistry. These teeth often suffer from substantial structural damage due to both the original pathology and the invasive nature of endodontic procedures. Consequently, ETT are more susceptible to fractures compared to vital teeth, necessitating restorative strategies that can effectively restore both function and aesthetics while minimizing the risk of failure. In recent years, advances in adhesive dentistry and the development of high-strength ceramics have further expanded the restorative options for ETT. Bonded restorations have gained popularity as they preserve more tooth structure and enhance the overall strenght of the tooth-restoration complex. The choice of restorative material and technique is influenced by numerous factors, including the amount of remaining tooth structure, the functional requirements of the tooth, and the aesthetic demands of the patient. Despite the plethora of available materials and techniques, the optimal approach to restoring ETT remains a topic of ongoing research and debate. In this comprehensive review, the current state of and recent advances in restoring damaged endodontically treated teeth are explored. Numerous therapeutic options exist, involving a wide range of materials. This article aims to present the biomaterial advancements of the past decade and their applications, offering alternative approaches to treating damaged ETT with the goal of prolonging their retention on the dental arch and serving as a valuable resource for dental practitioners who face this issue daily.

Ameloblastic Fibroma of Mandibular Anterior Segment

Abstract: Ameloblastic fibroma is a rare odontogenic tumor occurring in first two decades of life. Various case reports have shown that conservative treatment is successful in such cases. This case report highlights a case of ameloblastic fibroma occurring in 11-year female which was successfully treated with endodontic treatment of teeth and surgical removal of the lesion.

Performance of Bonded Lithium Disilicate Partial-coverage Crowns in the Restoration of Endodontically Treated Posterior Teeth: An Up to Seven-Year Retrospective Study.

To evaluate the clinical performance of adhesively bonded lithium disilicate glass-ceramic (LDG) partial-coverage crowns in restoring posterior endodontically treated teeth (ETT). A total of 121 morphologically compromised posterior ETT were restored with LDG partial-coverage crowns between October 2015 and January 2018. The restorations were fabricated in the laboratory or at the chairside. Two adhesive systems and resin cements were used to cement the restorations. Tooth and restoration survival rates were calculated. The restorations were evaluated clinically using the modified United States Public Health Service (USPHS) criteria for an observation period of 5-7 years. The Cox proportional hazards model was used to estimate relative failure risks such as tooth type, resin cements, gender, and sleep bruxism. The standard chi-squared test was used to compare the survival of different tooth types for significant differences (α=0.05). In addition, survival probability was calculated using the Kaplan-Meier algorithm. Among seven failed cases, one was a tooth fracture, and six were restoration fractures. According to the Kaplan-Meier analysis, the estimated survival rate of the teeth was 99% for seven years, while the estimated survival rate of the restorations was 94.8% for 5 years and 92.8% for 7 years. Tooth type and resin cements did not influence restoration survival rates (p>0.05), while sleep bruxism and male patients might increase the risk of failure (p<0.05). The indirect adhesively bonded LDG partial-coverage crowns of posterior ETT exhibited favorable clinical outcomes. Ceramic fracture was the most common failure pattern.

Effect of Orthodontic Treatment on Endodontically Treated Teeth

ABSTRACT Background: This research has been conducted to assess the effect of orthodontic treatment on endodontically treated tooth. Materials and Methods: Overall, fifty subjects had been recruited. The participants received root canal therapy within 3 months as well as had an appointment for fixed orthodontic therapy. All candidates underwent an intraoral investigation. Radiographic evaluation as well as therapy planning were completed. Fixed orthodontic therapy had been initiated. All of the participants had been recalled. Endodontically restored teeth had been evaluated. All outcomes were assessed via Statistical Package for the Social Sciences (SPSS) software. Results: In this study, there were 25 males and 25 females. Pulp stones were seen in 15 subjects. Apical periodontitis was seen in nine subjects. Apical root resorption was seen in seven subjects, and enamel decalcification was seen in four subjects. Overall, 35 out of 50 subjects showed negative effects of orthodontic treatment on endodontically treated teeth. Conclusion: Excessive orthodontic force may result in increased pulp and peri-apical inflammation.

Experimental study of the antibacterial effect of anodic dissolution of a copper electrode used in endodontic treatment of teeth

BACKGROUND: One of the possible ways to improve the quality of dental treatment with obliterated root canals is the use of transcanal direct current exposure. AIM: The aim of the study was to investigate the comparative antibacterial activity of anodic dissolution of copper and silver-copper electrodes used in the endodontic treatment of teeth in the experiment. MATERIALS AND METHODS: An experimental study was performed by implementing a technique for the automatic cultivation of microorganisms in liquid culture media. We used for the study clinical isolates of individual strains of bacteria and yeasts, namely: S. constellatus, P. intermedia, C. albicans, as well as mixed cultures: 1) S. constellatus + F. nucleatum; 2) Streptococcus sanguis + Enterococcus faecium obtained from the root canals of the teeth during treatment of chronic pulpitis. RESULTS: The results of the study showed that anodic dissolution of both silver-copper and copper electrodes had a significant and, in general, unidirectional antibacterial effect. At the same time, it was found that, if for clinical isolate of P. intermedia the use of a silver-copper electrode was more effective, then for S. constellatus and C. Albicans strains, as well as for mixed cultures of pathogenic microorganisms S. constellatus + F. nucleatum and Streptococcus sanguis + Enterococcus faecium anodic dissolution of a copper electrode showed a more pronounced antibacterial effect. CONCLUSION: In endodontic treatment of teeth with partially obliterated root canals, along with anodic dissolution of silver-copper electrodes, it is possible to use anodic dissolution of copper electrodes as a means capable of having a pronounced antibacterial effect.

Comparative evaluation of three types of fiber orientation techniques on fracture resistance of endodontically treated premolars restored with composite resin: An in vitro study

ABSTRACT Introduction: A new technique of fiber reinforcement (horizontal + base fiber [HBF]) in the restoration of endodontically treated teeth (ETT) with composite resin was proposed. Aim: This study aims to compare three different types of fiber orientation techniques on fracture resistance of ETT. Materials and Methodology: Sixty intact extracted human maxillary premolars were assigned to five groups (n = 12). Group 1 intact teeth (control). The teeth in Groups 2–5 received endodontic treatment and a MOD cavity preparation and restored with composite resin with polyethylene fiber at different positions. Group 2: No fiber group (NF), Group 3: Occlusal fiber group (OF), Group 4: Horizontal fiber group (HF), and Group 5: HBF group. After thermocycling, the specimens were loaded using a universal testing machine until fracture and the fracture patterns were analyzed. Statistical Analysis: The data were analyzed statistically using analysis of variance with Tukey’s post hoc test (fracture load) and Chi-square test (fracture patterns). Results: Fiber-reinforced groups (OF, HF, HBF) showed significantly higher fracture resistance compared to NF group (P &lt; 0.05). No significant differences in fracture resistance were observed between (a) fiber-reinforced groups (OF, HF, HBF) and intact teeth (P &gt; 0.05) (b) amongst the fiber-reinforced groups (OF, HF, HBF) (P &gt; 0.05). With regard to fracture patterns, no significant differences were observed between the study groups (P &gt; 0.05). Conclusion: Restoring ETT with different techniques of fiber reinforcement resulted in the improvement of the fracture resistance comparable to that of intact teeth. The newly proposed technique of fiber insertion might be considered as an alternative to earlier techniques for the reinforcement of ETT.

Stress Analysis on Mesiolingual Cavity of Endodontically Treated Molar Restored Using Bidirectional Fiber-Reinforced Composite (Wallpapering Technique).

Endodontically treated teeth (ETT) undergo extensive structure change and experience high stress during biomechanical function. Stress distribution is influenced by the restoration material and the type of bond between material and tooth structure. The selection of materials that can distribute stress will affect the resistance and retention of ETT to mastication forces, thus biomechanical functions were achieved. Composite has mechanical properties similar to dentin, it can transmit and distribute stresses throughout the tooth surface. The disadvantage of composites in large cavities is their lack of toughness. The addition of fiber to composites can increase their toughness. This research is to determine the stress distribution of a fiber-reinforced composite made of polyethylene and e-glass on the mesiolingual cavity of ETT. A three-dimensional model of the mandibular molar was prepared for cavity preparation and the formation of restorations using SolidWorks 2021. The models were analyzed with Abaqus 2020 to determine stress concentrations after given vertical and oblique loading. The maximum and minimum principal stress data were obtained to assess material resistance and interfacial damage criterion. Polyethylene fiber shows a more homogeneous stress distribution because the modulus of elasticity is close to the dentin and has a thickness that can reduce the volume of the composite. The E-glass shows the stress concentration on the circumferential fiber and cavity floor. The stress distribution of fiber-reinforced composite on the buccolingual cavity of ETT using the finite element method did not show structural failure in the polyethylene group because the maximum and minimum principal stresses were lower than the strength of the material. Interfacial bond failure occurs at the enamel portion. The maximum and minimum principal values of e-glass indicate structural failure in the circumferential fiber and the base fiber because the stress exceeds the strength of the material. Interfacial bond failure occurred on the circumferential and the cavity floor.

Apical periodontitis in endodontically-treated teeth: association between missed canals and quality of endodontic treatment in a Colombian sub-population. A cross-sectional study.

The aim of this study was to evalúate the quality of endodontic treatment and the frequency of missed canals associated with teeth with apicalperiodontitis (AP) through CBCTin a Colombian sub-population. This was a cross-sectional study assessing 318 cone beam computed tomography (CBCT) scans of endodontically treated teeth from Colombian individuals. The scans were taken using J Morita X550 (J Morita Corporation, Osaka, Japan), with voxel size 0.125 to 0.20 mm. All endodontically treated teeth were assessed for quality of treatment, presence of missed canals and AP. Allsamples were analyzed bytwo endodontics specialists and an radiology specialist. Chi-square or Fisher 's test and odds ratio were calculated to identify the association and risk relationship between the presence of AP and the study variables. Missed canals were found in 18.61% (86/462), and 95.3% were associated with AP. The frequency of AP was 62.34% (288/462) for all the evaluated teeth. AP was found in 27.43 % (79/288) of the teeth with adequate endodontic treatment, in contrast to 72.57% (209/288) of the teeth with inadequate treatment (P<0.01). The frequency of missed canals was highest in maxillary molars, with 55.23% (58/105), with 96.55% presenting AP. The second mesiobuccal canal wasthe most frequently missed canal, 88.52%o(54/61), with AP in 90.74% (49/54) of the cases. There was a high frequency of teeth with missed canals and PA. More than half of the teeth with missed canals were maxillary molars, with MB2 being the most common canal, commonly presenting apical periodontitis.

Comparative evaluation of dental integrity using three computer-aided design/computer-aided manufacturing customized post systems

Background Endodontically treated teeth (ETT) are considered structurally compromised due to significant changes in their biomechanics.The purpose of this in vitro study was to evaluate fracture resistance using three computer-aided design/computer-aided manufacturing (CAD/CAM) customized post systems. Methods Forty extracted intact maxillary central incisors were used in the study, and the teeth were endodontically treated and randomly divided into four equal groups (n = 10): control group without any post and accessed cavity was restored with direct composite restoration. Test groups were prepared for a post and restored according to the material type. The posts and cores were fabricated with computer-aided design/computer-aided manufacturing technology using titanium, vita enamic, and biological dentin materials. The posts were luted using self-adhesive resin cement, and then, zirconia crowns were fabricated, cemented, teeth in all groups were mounted to universal testing machine and subjected to compressive load in Newton at crosshead speed 0.5 mm/min. Results Control group exhibited the highest fracture resistance (727 ± 172.05) followed by biological dentin posts with mean value (712.29 ± 298.58) then group titanium posts with mean value (678.59 ± 132.83) and the least fracture resistance was for group vita enamic with mean value 331.84 ± 74.27. Regarding to fracture favorable it was significantly higher in vita post group than titanium post group (70% vs. 30%) and control group (70% vs. 10%) but it was significantly higher in biological dentin group than titanium group (80% vs. 70%). Conclusion According to type of material, teeth restored with the biological dentin posts showed highest fracture resistance followed by titanium posts, the least fracture resistance was vita enami posts.

Effective Treatment of Recurrent Periapical Lesion Involving Multiple Teeth in a Pediatric Patient: A Rare Case Report with Review of Literature

Nonsurgical endodontic therapy is usually advised as primary approach to treat the nonvital teeth with pulpal or periapical infection. However, in certain clinical scenarios and also owing to various factors, surgical modality of endodontics is eventually adopted to manage the recurrent or persistent pulpal or periapical infection and also to treat the failed cases of previously endodontically-treated teeth with conventional approach. In existing literature, there is deficiency of published case report articles regarding successful surgical endodontic management of recurrent pulpal/priapical signs and symptoms and also of failed cases of nonsurgical modality of endodontic therapy in growing pediatric patients. Hence, the present article describes the case report of the effective and successful clinical management of recurrent and failed case of conventional nonsurgical orthograde endodontic therapy by employing the surgical endodontic modality in an adolescent pediatric patient aged 13 years. The whole clinical procedure in step-wise manner has been described, and furthermore, the relevant contemporary endodontic techniques have been elucidated in context of the present case.

Effectiveness of Chitosan Oligosaccharide as Chelating agent in Smear Layer Removal and The effect on Fracture Resistance of Endodontically Treated Teeth

One factor affecting root canal treatment's success is the cleanliness of root canal systems. Smear layer removal is needed to ensure the cleanliness of root canals. Fracture resistance is also essential things in endodontic-treated teeth. Chitosan is one of the irrigant alternatives in dentistry that is biocompatible. The study aimed to investigate the effect of chitosan oligosaccharide as a root canal chelating alternative in the cleaning smear layer and its impact on fracture resistance of endodontic treated teeth.Experimental research with 56 mandibular premolar teeth with a single canal was used. For the smear layer cleanliness test, 24 teeth were used and divided into four treatment groups. Statistical analysis used was the one‑way analysis of variance and LSD Post Hoc test. Thirty-two teeth were divided into two treatment groups for the fracture resistance test. The statistical analysis used was the Independent T-test. There was no significant difference between smear layer removal in chitosan oligosaccharide 2% and EDTA 17% groups. There was a considerable difference between fracture resistance of endodontic treated teeth in chitosan oligosaccharide 2% and EDTA 17% groups. Chitosan oligosaccharide 2% can be used as an alternative in the final irrigation of root canal treatment.

Orthodontics and Endodontics Clinical Practice Correlation: A Narrative Review.

Research on the connection between endodontic therapy and orthodontics is lacking. This overview of the literature synthesizes the findings from the fields of orthodontics and endodontics and explains how they are related. Beginning with the diagnosis, treating the patient at the appropriate time, moving endodontically treated teeth and traumatized teeth, resorbing roots by orthodontic mechanics, and managing traumatized teeth with orthodontic therapy. Multiple electronic databases were utilized including(PubMed, Scopus, Science Direct, and Web of Science) to perform manual literature searches. A total of 31 articles were reviewed and summarized in this paper in keywords like "Endodontically Treated Teeth and Orthodontic Treatment," "Endodontically Treated Tooth and Orthodontic Movement," "Orthodontic Treatment in RCT Teeth," "Root Canal Treatment with Orthodontic Movement," "Trauma with Orthodontic Movement," "Orthodontic and Endodontic." Orthodontic treatment of endodontically treated and traumatized teeth is a subject of controversy. The lack of research on the topic makes it a hard decision to make when to treat these teeth. Especially given that both orthodontic and endodontic treatments have multiple consequences on each other's outcomes. Thus, it is crucial for clinicians to understand how they integrate and have a guideline to refer to during decision-making. Successful orthodontic tooth movement could be carried out immediately after endodontic treatment. However, traumatized teeth need a follow-up period before initiating orthodontic movement, which ranges from three months to 12 months depending on the type of trauma and severity. Careful radiographic and clinical follow-up should be done during the healing period. Collaborative teamwork is important between orthodontists and endodontists for the success of treatment, and to achieve satisfactory outcomes.

Evaluation of mechanical properties of anatomically customized fiber posts using E-glass short fiber-reinforced composite to restore weakened endodontically treated premolars

ObjectiveThis study was conducted to assess the influence of combining different forms of fiber-reinforced composites (FRC) on the mechanical behavior and bond strength of compromised endodontically treated teeth (ETT).Materials and methodsEighty extracted human premolar teeth were randomly divided into five experimental groups according to the type of intra-radicular restoration and the canal preparation design which was either non-flared (Group 1), flared (Groups 2–5), closed-apex (Groups 1,3,5) or open-apex (Groups 2,4). Standard prefabricated fiber posts were used as intra-radicular restoration for Groups 1–3 while Groups 4–5 were restored with anatomically customized relined fiber posts. After composite core fabrication, all samples were sent for an artificial aging process. Fracture resistance and push-out bond strength tests were then carried out through a universal testing machine followed by mode of failure analysis via a stereomicroscope and scanning electron microscope.ResultsPairwise Log-Rank comparisons revealed that the survival rate of Group 2 and Group 3 was significantly lower than all other groups after artificial aging. The highest fracture resistance value (1796 N) was recorded in Group 5 and was significantly higher than that of the other groups (p < 0.05), while Group 2 exhibited the lowest fracture resistance (758 N), which was significantly lower compared to the other groups. Group 5 and Group 4 demonstrated a significantly higher push-out bond strength, at all root thirds, than Group 3, Group 2, and Group 1 (p < 0.05). The most frequently observed failure mode in the tested groups occurred between the resin cement and radicular dentin.ConclusionThe use of short fiber-reinforced composite (SFRC) to reline the prefabricated FRC post has been proven to have superior fracture resistance with favorable failure patterns and increased push-out bond strength values compared to standard prefabricated FRC posts.

NEW CONCEPT OF FRONTAL TEETH RESTAURATION FOR EXCELLENT FUNCTION AND AESTHETIC

This clinical case report aimed to investigate impact of a novel inner ferrule (IF) preparation around root canal on the subsequent esthetic restoration. Material and Methods: This clinical research involved a case report featuring endodontically treated teeth (ETT) restored using a newly developed ceramic post. Special drills (Edenta AG GmbH, Switzerland) were utilized for root canal preparation, involving horizontal preparation around the cervical portion of the root canals and post spaces. Experimental samples (ETT) were prepared with and without a 2mm IF preparation. The newly designed zirconia ceramic posts (TZ-3Y-SBE, Tosoh, Japan) were cemented with Multilink Automix (Ivoclar), upgraded with MultiCore (Ivoclar), and crowned with an all-ceramic zirconia crown (Zirkonzahn, Italy). Results: Our clinical case provided insights into a new type of all-ceramic aesthetic restoration of prepared teeth (ETT) with a new IF preparation design and a new zirconia post-core design. Conclusion: The novel horizontal IF preparation around the root canal was developed to accommodate the initial retentive coronal ring of the newly designed zirconia posts. The incorporation of IF preparation and coronal retentive rings within the zirconia post structure effectively enhances the fracture resistance of ETT. This new modern aesthetic rehabilitation in all restored teeth significantly reduces the severity of root fractures for excellent function and aesthetic restoration of ETT.

Biological response to endodontic treatment in one versus two-visit: a systematic review and meta-analysis of animal studies.

To perform a systematic review of animal studies that compared the histopathological characteristics between teeth with apical periodontitis after endodontic treatment in one or two visits. This systematic review was registered on the International Prospective Register of Systematic Reviews (PROSPERO) - CRD42022340849. Studies were collected from PubMed/MEDLINE, LILACS, EMBASE, Livivo, SciELO, Web of Science, Scopus, and Cochrane Library and manual and gray literature searches. Animal studies that evaluated histological characteristics after endodontic treatment of teeth with apical periodontitis in one or two visits were included. Risk of bias analysis of the included studies was performed using the Systematic Review Centre for Laboratory Animal Experimentation (SYRCLE) tool. Data synthesis of the included studies with quantitative data was performed, and meta-analysis was conducted with the Comprehensive Meta-Analysis software, using the random effects model and odds ratio (OR). Eighteen studies met the inclusion criteria (Kappa = 0.891). Meta-analyses indicated values in inflammatory infiltrate intensity with effect size of 5.5% (95% CI: 0.020-0.148; p < 0.001), periodontal ligament thickness: 25.6% (95% CI: 0.134-0.487; p < 0.001), dentin resorption: 13% (95% CI: 0.015-1.141; p = 0.066), cementum resorption: 7.1% (95% CI: 0.015-0.325; p = 0.001), bone resorption: 1.4% (95% CI: 0.002-0.130; p < 0.001), mineralized tissue resorption: 42.8% (95% CI: 0.110-1.671; p = 0.222), biological apical sealing: 13.1% (95% CI: 0.055-0.314; p < 0.001), and presence of microorganisms: 10.3% (95% CI: 0.014-0.747; p = 0.025). When considering animal studies, the two-visit endodontic treatment, using calcium hydroxide-based intracanal medication, resulted in better biological repair characteristics. A two-visit endodontic treatment with calcium hydroxide-based intracanal medication yields superior histopathological outcomes.

Radiographic Assessment of 121 Glass Fiber Post Procedures in 32 Patients Aged 21-60 Years Performed by 6th-Year Dental Students During the 2022-2023 Academic Year at the College of Dentistry, Jazan University, Saudi Arabia.

BACKGROUND The restoration of endodontically treated teeth (ETT) and severely damaged teeth has been a concern of clinicians. Glass Fiber Post (GFPs) combine the strength of carbon fiber posts with the esthetic appearance of glass to resemble natural dentin during dental restoration procedures. This radiographical study assessed the GFP carried out by students enrolled in the Clinical Comprehensive Course at the College of Dentistry, Jazan University. MATERIAL AND METHODS A total of 32 patients treated by 18 6th-year dental students with 121 GFPs were assessed in this cross-sectional radiographic study. The assessment covered tooth type, arch, post-to-root width, length of post in relation to the crown and root lengths, amount of remaining gutta percha (GP), and gap between GP and post. Data were analyzed using SPSS, and associations between variables were determined using the chi-square test. RESULTS Maxillary teeth were the most frequently restored with posts (88.4%) with most being (58.7%) anterior teeth, and 50.4% of posts had widths that were one-third that of the root. The percentage of posts was twice (71.1%) or equal to (26.4%) the crown length, whereas two-thirds of the tested GFPs were >5 mm of the remaining GP. Significant differences were observed in location and position of teeth with post width, post length in relation to crown or root length, and amount of remaining GP, with P values of 0.018, 0.000, and 0.001, respectively. CONCLUSIONS The assessed radiographs revealed that the performance of sixth year students in accomplishment GFP radiographically was satisfactory and within the values recommended in the literature.
