Retired Orthopedic Surgeons' Reflections on Their Lives and Careers: A Cross-Sectional Study.

Background: Retired surgeons often have limited opportunities to disseminate their wisdom and expertise in a structured manner to their younger colleagues. In addition, when asked to reflect on their personal and professional lives, many physicians say they wish they had done something differently. The extent to which this is true of retired orthopedic surgeons is not known. Purpose: We sought to determine the percentage of retired orthopedic surgeons who say that they would like to have changed something in their life/career and delineate the most commonly desired changes. Methods: We conducted a cross-sectional study of retired orthopedic surgeons, by emailing a Qualtrics survey to 5864 emeritus members of the American Academy of Orthopaedic Surgeons (AAOS), with 1 initial email invitation in April 2021 followed by 2 reminders in May 2021. The survey employed a branching logic, with up to 16 questions designed to determine whether they would have done anything differently in their life/career. Results: The survey was completed by 1165 of 5864 emeritus AAOS members, for a response rate of nearly 20%. The 3 most represented surgical subspecialties were general orthopedics, adult reconstruction, and hand and upper extremity surgery. Respondents' average age was 74.9 years and age at retirement was 67.8 years; nearly half worked part-time before retiring. More than 80% of the participants said that they had retired at the appropriate time, and 28.5% said they wished they had done something differently. The wished-for changes most often noted were spending more time with family, spending more time on personal wellness, and selecting better practice partners. Conclusion: The results of our survey of retired orthopedic surgeons show that while most were satisfied with their lives and careers, some had regrets. These findings suggest that there may be factors in the work lives of current surgeons that could be altered to reduce regret. Further study is warranted.

Editorial: Proceedings of the Knee Society: A Knee Society-Journal of Arthroplasty Collaborative Partnership!

It is a distinct privilege for the Journal of Arthroplasty to be awarded the opportunity to publish the Proceedings of The Knee Society going forward. The Knee Society was founded in 1983, with Chitranjan Ranawat as the first president, for education and research in the area of total knee arthroplasty as well as in the pathogenesis and non-arthroplasty treatment of osteoarthritis and other disease processes that lead to end-stage arthritis of the knee. The goal was to have membership based on a proven ability to perform research, teach, and demonstrate exceptional skills as a clinician and surgeon. The membership currently consists of 206 members, including North American, International, and Emeritus members who are internationally recognized thought-leaders, innovators, mentors, researchers, and educators, and who, as was the vision of the Society founders, are committed to promoting outstanding care to patients who have disorders of the knee through innovation, research, and education. Since its conception, the Society has recognized the need for candid exchange of ideas on implant design and surgical techniques. Hence, annually in the fall, the members convene to present and discuss their most current research in the field of arthritis-related knee surgery. Importantly, the Society recognizes the need and desire to educate the global arthroplasty community on current, data-generated implant design concepts and surgical techniques. Specialty Day at the American Academy of Orthopaedic Surgeons annual meeting has been the forum for this exchange since the inception of the Society. As a highlight of that program, three award papers named in honor of three of the founding members: Drs John Insall, Mark Conventry, and Chitranjan Ranawat are presented. The open call for award worthy research papers, in the previous fall, consistently result in a robust number of high-quality submissions from which to choose these prestigious legacy awards. The published Proceedings of The Knee Society have evolved to a publication of papers from the Fall members meeting and the award papers from the following American Academy of Orthopaedic Surgeons Specialty Day open meeting of the Knee Society which is presently co-sponsored with the American Association of Hip and Knee Surgeons. With the ensuing collaborative partnership between the Journal of Arthroplasty and the Knee Society, the intent will be to produce a robust Proceedings of The Knee Society that archives and memorializes the fall members meeting as well as the outstanding competitive award papers presented at the following annual open meeting of the Knee Society. To publish a meaningful (to the international arthroplasty community [ie, clinicians, researchers, and patients]) Proceedings that truly archives the members’ meeting and publishes the following year’s award papers in a timely manner is a daunting challenge. It can only be done with a truly collaborative relationship and effort with a shared purpose. The production of this Proceedings began with the President of the Society, Robert Trousdale, his Program Chairman and Editor of the Proceedings, Rafael J. Sierra, and the Society staff, led by Olga Foley, committing to, and finalizing a program months in advance and engaging the membership to contribute their work to the Proceedings by the required deadline. Dedicated, experienced, and punctual reviewers from the Society and the Journal of Arthroplasty to assure the requirement for high quality relevant scientific research and an experienced production and Editorial Staff were critical. Regular organized communication among Knee Society and Journal of Arthroplasty staff, editors, reviewers, and authors was paramount. This inaugural Proceedings of The Knee Society in Journal of Arthroplasty is the product of this Knee Society-Journal of Arthroplasty collaboration and team effort intended to provide the most current scientific and innovative work of our Knee Society members to the clinicians, surgeons, researchers, and patients in the arthroplasty community. It is the hope that the Knee Society-Journal of Arthroplasty collaborative product is eagerly anticipated annually by Journal of Arthroplasty readership. Expect the unexpected: presenting research leads to unforeseen gains

Dan Lane is a first year PhD student at the University of Leicester, developing biochemical methods in non-adherence testing alongside Leicester Diabetes Centre. A pilot study he helped to conduct prior to the start of his postgraduate degree was accepted by Diabetes UK and so, with the help of a travel grant from the Biochemical Society, he travelled to Liverpool to present his work. The Biochemical Society provides General Travel Grants to support attendance at scientific meetings, training events or any other event relevant to your career. Postgraduate, Early Career, Full and Emeritus Members are encouraged to apply to further their lifelong learning.

Reporting from outside the lab: attending the ASH conference

Postgraduate, Early Career, Full and Emeritus Members of the Biochemical Society can apply for a General Travel Grant. These grants support attendance at scientific meetings, training courses, lab visits or any other event relevant to your career and encourage lifelong learning. James Hillier is currently investigating the role of the aminotransferase BCAT1 in acute myeloid leukaemia (AML) development. BCAT1 contains a CXXC motif, a feature of many antioxidant enzymes such as thioredoxin that are able to metabolize reactive oxygen species (ROS). His research focuses how altered expression of BCAT1 affects redox homeostasis and how this impacts processes dysregulated in cancer such as proliferation, differentiation and apoptosis. Cancer cells are known to favour pro-oxidative states that induce proliferative signalling yet maintain ROS at sub-lethal levels, therefore targeting the antioxidant capacity of BCAT1 to induce cell death, which may open a promising treatment option for patients with aberrant BCAT1 expression.

Members for life

As a chemistry major nearing the end of his days as an undergraduate, Zory Glaser joined the American Chemical Society hoping it might help him land a job. Not only did it do that, but “membership in ACS was one of the major factors that helped me decide to continue my education into graduate school, where I earned a Ph.D. in chemistry,” Glaser says. That was 57 years ago, and he has remained a member ever since. It may be hard to imagine joining a professional organization these days and expecting to remain a member for life. But for ACS’s more than 15,000 emeritus members, who have been in the society for at least 35 years, it’s hard to imagine going through their careers without their involvement with ACS. From the Business Office

Be sure that you have renewed your membership in ASLO for 2016 in order to keep your benefits and support of our society going. ASLO had 4267 members for the 2015 year and this included many new members joining to attend the meeting in Granada, Spain. We want you to remain a member so please renew. The 2015 membership represented 75 countries and included 1042 students, 935 early career professionals, and 165 emeritus members. Special thanks to our emeritus members for all their years of service! ASLO was one of the three societies sponsoring the 2016 Ocean Sciences Meeting in New Orleans. While AGU was the lead, ASLO was well represented with Adina Payton, Susanne Neuer, and Amy Burgess serving on the scientific committee and over 1000 ASLO members attending. Special thanks for all the hard work. At the meeting, ASLO recognized our inaugural class of Fellows with a champagne toasts. A special reception will be held at the summer meeting. This means that the ASLO meeting in Santa Fe, New Mexico, 05–10 June will be our “ASLO only” meeting for 2016. We will get people up and moving following the wonderful southwestern cuisine with yoga and runs early mornings. The meeting will also have some free time built in on Wednesday afternoon. Other special events include the ASLO annual business meeting, a tapas night complete with a live band and closing reception Friday following the last oral sessions. Construction and validation of a surgical skills assessment tool for general surgery residency program.

To develop and validate an instrument for measuring the acquisition of technical skills in conducting operations of increasing difficulty for use in General Surgery Residency (GSR) programs. we built a surgical skills assessment tool containing 11 operations in increasing levels of difficulty. For instrument validation we used the face validity method. Through an electronic survey tool (Survey MonKey®) we sent a questionnaire to Full and Emeritus members of the Brazilian College of Surgeons--CBC--all bearers of the CBC Specialist Title. Of the 307 questionnaires sent we received 100 responses. For the analysis of the data collected we used the Cronbach's alpha test. We observed that, in general, the overall alpha presented with values near or greater than 0.70, meaning good consistency to assess their points of interest. The evaluation instrument built was validated and can be used as a method of assessment of technical skill acquisition in the General Surgery Residency programs in Brazil.

Of Elections and Emeritus Memberships

The balloting for the 2013 RRA elections has now been completed. Your voting yielded the following outcomes: Jack Marcum was elected President for a term that begins in 2015. He became Vice-President at the end of this year’s annual meeting. Mary Gautier and Melinda Lundquist Denton were elected to 4-year terms on the RRA Board of Directors. They also took office at the end of this year’s annual meeting. Conrad Hackett and Robert Woodberry were elected to the Nominating Committee and will begin their work with the election of 2014. On behalf of the RRA membership I want to congratulate all of these individuals and extend our appreciation to everyone who agreed to stand for election, as well as to Nominations Chair Elaine Howard Ecklund and those who worked with her. Finding well-qualified persons who are willing to accept nomination is not the easiest job in RRA. On another front, the Executive Office had quite a surprising event as dues rolled in this year, inasmuch as a number of people declared themselves Emeritus members and paid Emeritus dues. Some also included generous contributions, for which we are grateful. Others did not. This matter apparently needs to be set straight formally, so that it is included in that part of our institutional printed record that goes to all members: The RRA By-Laws revision of 1995 ended the category of Emeritus membership, but the Board also recognized at that time that this honor had been bestowed on a number of individuals and stated that those persons who held this status prior to 1995 would retain it for the remainder of their active lives in the RRA. The only RRA members who now hold this status are David O. Moberg and Everett L. Perry. We wish them both long and productive lives, but once they have gone to their rewards, that is the end of Emeritus membership in the RRA. In the future, these Emeritus members will be sent separate dues notices to avoid this confusion.

A Brief History of the American College of Veterinary Pathologists, 1988–2008

s have been published in the September issue of Veterinary Pathology. From the early 1990s, a Town Hall meeting has occurred at annual meetings to give members an opportunity to air their views, and for the Council to listen. Subjects of meetings included recertification, the importance of toxicologic pathology and the possibility of subspecialty certification, credentialing, shortage of veterinary pathologists, a qualifying examination, the status of training programs, and improving the examination. It is still possible to indulge in a mystery slide session at an annual meeting, but now one can download Imagescope viewing software to examine the tissue sections on a computer screen. Because I qualify by virtue of having been an AVCP member for more than 25 years, I was very glad to learn of the Very Important Pathologists program at annual meetings, which allows emeritus and senior members to take in some relaxed socializing as well as the science. Communication: The Newsletter, the Membership Directory, the Website The newsletter has been online since 2002. The newsletter seems much more fun to read than it was twenty years ago. Contemporary Musculoskeletal Tumor Research: Selected Papers Presented at the 2008 Meeting of the Musculoskeletal Tumor Society: Editorial Comment

The purpose and focus of the Musculoskeletal Tumor Society is stated on the organization’s website. “The mission of the Musculoskeletal Tumor Society is to advance the science of orthopaedic oncology and to promote high standards of patient care. It does this through excellence in education and research” [1]. The 2008 MSTS meeting in Phoenix, AZ, November 6–8, 2008, and hence this symposium, follows part of the prescription, providing an excellent forum for education. The second portion of the mission statement encourages advancing orthopaedic oncology through research. The organization has enjoyed limited success in conducting or promoting research despite valiant efforts by the leaders of the Research Committee. Why? Answers come from the membership, the discipline, and the organization. The membership of the MSTS has changed dramatically over the years. The original “Piranha Club” had 18 domestic orthopaedic surgeons, of whom only one remains actively practicing surgical oncology. The roster has ballooned to over 220 members, including Candidate, Affiliate, and Emeritus Members. While this reflects successful maturation of orthopaedic oncology, it has a dilutive effect on research and possibly even clinical progress. Case volume is correlated with outcome in all other cancer systems studied to date, for disease of organs ranging from prostate to pancreas. Why shouldn’t it also be true for bone and soft tissue sarcomas? As knowledge and training disseminate, experience per surgeon inevitably declines. Smaller case loads prevent concentrated expertise. Paradoxically, the numerical expansion of our membership may have improved our ability to deliver more convenient, good quality care to more people, but it may prevent us from making substantive advances in the field. Single centers usually lack the volume to ask or answer important questions. Collaboration is essential to maintain patient numbers required to study rare diseases. Standardization of treatment protocols is essential if the cases from different centers may be combined. This requires strong-willed commitment on the part of musculoskeletal oncologists to suppress some of their individuality and cooperate. This is necessary to advance the discipline and improve patient care. Large cooperative groups (eg, Children’s Oncology Group or COG, the Southwest Oncology Group or SWOG) have failed due to their cumbersome structure and an inability to channel resources to orthopaedic problems. The greatest success has been in ad hoc collaborations such as that coordinated by the MSTS to review the outcome of patients treated by limb salvage or amputation for distal femoral osteosarcoma [2], or the Toronto Group’s prospective evaluation of prognostic genetic markers (p53 and RB) in osteosarcoma patients from six centers [3]. This symposium includes a variety of novel and informative clinical papers dealing with resection and reconstruction techniques. However, most are single center studies that should be viewed as hypothesis generating ideas. They require confirmation in larger trials involving other centers to show the results can be generalized. The levels of evidence are typically IV. There is also a deficiency in basic and translational research needed to advance the field. Clearly, more effort needs to be expended to improve the quality and generalizability of our research. Investigators and reviewers need to support cluster- and association-based comparative research efforts, clinical trials, and basic investigations. There are national initiatives in this direction. Responding to the challenge proposed by the American Academy of Orthopaedic Surgeons, the Musculoskeletal Tumor Society is trying to develop evidence-based assessments of practice. This symposium includes the Society’s first effort along those lines, summarizing the best available opinion regarding the biopsy of musculoskeletal tumors. More recently, the AAOS/ORS/NIH sponsored a workshop in Albuquerque to define the research agenda for prospective outcome studies in each orthopaedic subspecialty. Two tumor projects made the final cut of recommendations. The first was a prospective study of proximal femoral metastases, comparing internal fixation (rodding) with joint arthroplasty. The second was a long-term followup of reconstructions performed for patients randomized to receive, or not receive, muramyl tripeptide phosphoethanolamine (MTP-PE) in the national Intergroup-133 study. These projects should receive the support of the oncology community, and would be good topics on which the MSTS could focus and include in future CORR Symposia.

WHEN MEMBERS OF THE American Chemical Society retire from their chemistry vocations, they can feel detached from the society. There is no more research to present, no pressing need for professional development, and no desire to get ahead. But the society needs these people more than ever because they come with a wealth of expertise and a rare commodity: time. For years, some local sections have been mobilizing their senior members, offering them opportunities to socialize, network, and give back to the community. Now, ACS as a whole has taken notice, and a fledgling program called Silver Circle is heading a broader effort to tap into this unique and valuable resource. Senior chemists make up a significant proportion of ACS membership. In 2005, retired and emeritus members made up nearly 15% of ACS's membership. This percentage is only expected to grow in the coming years as the baby-boom generation enters retirement. ...
