Long-term performance monitoring of a-Si 1200 electronic portal imaging device for dosimetric applications.

Recently, dosimetri applications of the electronic portal imaging device (EPID) in radiotherapy have gained popularity. Confidence in the robust and reliable dosimetric performance of EPID detectors is essential for their clinical use. This study aimed to evaluate the dosimetric performance of the a-Si 1200 EPID and assess the long-term stability of its response. Weekly measurements were performed on two clinically used TrueBeam linear accelerators (linacs) equipped with a-Si 1200 EPID detectors over a 2-year period. They included dark and flood calibration fields, and EPID response to an open field corrected for the long-term machine output drift measured with the secondary absolute dosimeters: an ion chamber and an ion chamber array. All measurements were performed using five photon beam energies and two imaging modes: continuous and dosimetry. The measurements were analyzed for constancy and the presence of long-term trends. Comparisons were made between the two linacs for each beam energy. Pixel sensitivity matrices (PSM) were determined semi-annually and analyzed for long-term constancy for both treatment machines. The long-term variation of the dark and flood field signals, integrated across the EPID plane, over the entire observation period did not exceed 0.17% and 0.79%, respectively. The output-corrected EPID response showed long-term variation from 0.28% to 0.36%, depending on beam energy, while the short-term variation was 0.04%-0.07% for EPID and 0.02%-0.06% for secondary dosimeters. The long-term variation of secondary dosimeters was 0.2%-0.3%. PSMs were found to be stable to within 1% for 97.8% of pixels and 2% for 100% of pixels. Techniques to monitor and assess the long-term performance of the a-Si 1200 EPID as a dosimeter were developed and implemented using two TrueBeam linacs. The long-term variation of the EPID response was within clinical tolerance indicated in AAPM TG-142 report, and the detector was shown to be stable and reproducible for routine clinical dosimetry.

Deep learning methods for 2D in-vivo dose reconstruction with EPID detector

Over the past two decades, radiotherapy has seen a steep increase in technological complexity of treatment preparation and treatment execution, calling for the development of more and better quality assurance tools and procedures. In-vivo dosimetry has emerged as a very powerful tool for treatment verification in conjunction with Electronic Portal Imaging Devices (EPIDs). However, EPIDs present several drawbacks like non-water equivalence and cumbersome calibration procedures. Artificial intelligence, specifically Deep Learning (DL), plays a crucial role in this context. A critical step is modeling the EPID response to estimate 2D dose distributions (Portal Dose, PD). This work introduces a DL-based methodology aimed at converting EPID responses into PD images. The proposed procedure is fully data driven, being based on advanced DL methods. This approach will allow bypassing the complex calibration steps needed to overcome the non-water equivalent panel.

A feasibility study for in vivo treatment verification of IMRT using Monte Carlo dose calculation and deep learning-based modelling of EPID detector response

BackgroundThis paper describes the development of a predicted electronic portal imaging device (EPID) transmission image (TI) using Monte Carlo (MC) and deep learning (DL). The measured and predicted TI were compared for two-dimensional in vivo radiotherapy treatment verification.MethodsThe plan CT was pre-processed and combined with solid water and then imported into PRIMO. The MC method was used to calculate the dose distribution of the combined CT. The U-net neural network-based deep learning model was trained to predict EPID TI based on the dose distribution of solid water calculated by PRIMO. The predicted TI was compared with the measured TI for two-dimensional in vivo treatment verification.ResultsThe EPID TI of 1500 IMRT fields were acquired, among which 1200, 150, and 150 fields were used as the training set, the validation set, and the test set, respectively. A comparison of the predicted and measured TI was carried out using global gamma analyses of 3%/3 mm and 2%/2 mm (5% threshold) to validate the model's accuracy. The gamma pass rates were greater than 96.7% and 92.3%, and the mean gamma values were 0.21 and 0.32, respectively.ConclusionsOur method facilitates the modelling process more easily and increases the calculation accuracy when using the MC algorithm to simulate the EPID response, and has potential to be used for in vivo treatment verification in the clinic.

Assessing the accuracy of electronic portal imaging device (EPID)-based dosimetry: I. Quantities influencing long-term stability.

The aim of this study is to reduce the uncertainty associated with determining dose-to-water using an amorphous silicon electronic portal imaging detector (EPID) under reference conditions by identifying and accounting for operational and environmental factors influencing the long-term stability of EPID response. Measurements of the EPID relative response, corrected for variations in linear accelerator (linac) output, were performed regularly over a period of 12 months. For every acquired image set, measurements of detector supply voltages, internal operating temperature, and ambient environmental conditions were obtained. Pearson r correlation coefficients were then calculated for each pair of variables, a subset of which were fitted using multiple linear regression to develop a predictive model of EPID response. Principal component analysis was performed on the dataset to reveal the internal structure of the data in a way that best accounts for the observed variations. The +5.5V power supply voltage, internal operating temperature, and the accumulated dose absorbed in EPID were identified as having the greatest influence on the long-term stability of EPID response. By correcting for the combined effect of these variables, the mean difference in linac output as measured by the EPID relative to a reference class chamber improved from -0.46% to 0.23% over the period of the study. This work suggests that the stability of an EPID over a period of a year can be improved by a factor of two by monitoring and accounting for the effects of variations in power supply voltage, internal temperature of the detector, and accumulated absorbed dose.

Assessing the accuracy of electronic portal imaging device (EPID)‐based dosimetry: II. Evaluation of a dosimetric uncertainty budget and development of a new film‐in‐EPID absorbed dose calibration methodology

The aim of this study is to reduce the uncertainty associated with determining dose-to-water using an amorphous silicon electronic portal imaging detector (EPID) under reference conditions by developing a direct calibration formalism based on radiochromic film measurements made within the EPID panel and detailed Monte Carlo simulations. To our knowledge, this is the first EPID-based dosimetry study reporting an uncertain budget METHODS: Pixel sensitivity and relative off-axis response were mapped by simultaneously irradiating film contained within the imager panel and acquiring an EPID image set. The detector panel was disassembled for the purpose of modeling the EPID in detail using the EGSnrc DOSXYZnrc usercode, which was in turn used to calculate dose-to-film in the EPID and dose-to-water in water conversion factors RESULTS: A direct comparison of the two correction methodologies investigated in this work, the previously established empirical method and the proposed simultaneous measurement approach involving in-EPID film dosimetry, produced an agreement with an RMS deviation of 1.4% overall. A combined standard relative uncertainty of 3.3% (k=1) was estimated for the determination of absorbed dose to water at the position of the EPID using the proposed calibration methodology CONCLUSIONS: This work describes a direct method of calibrating EPID response in terms of absorbed dose to water requiring fewer measurements than other empirical approaches, and without 2D spatial interpolation of correction factors.

A Large Area Pixelated Silicon Array Detector for Independent Transit In Vivo Dosimetry

A large area pixelated silicon array detector named “MP987” has been developed for in vivo dosimetry. The detector was developed to overcome the non-water equivalent response of EPID (Electronic Portal Imaging Device) dosimetry systems, due to the shortfalls of the extensive corrections required. The detector, readout system and software have all been custom designed to be operated independently from the linac with the array secured directly above the EPID, to be used in combination with the 6 MV imaging system. Dosimetry characterisation measurements of percentage depth dose (PDD), dose rate dependence, radiation damage, output factors (OF), profile measurements, linearity and uniformity were performed. Additionally, the first pre-clinical tests with this novel detector of a transit dosimetry characterization and a collapsed IMRT (intensity-modulated radiation therapy) study are presented. Both PDD and OF measurements had a percentage difference of less than 2.5% to the reference detector. A maximum change in sensitivity of 4.3 ± 0.3% was observed after 30 kGy of gamma accumulated dose. Transit dosimetry measurements through a homogeneous Solid Water phantom had a measured dose within error of the TPS calculations, for field sizes between 3 × 3 cm2 and 10 × 10 cm2. A four-fraction collapsed IMRT plan on a lung phantom had absolute dose pass fractions between the MP987 and TPS (treatment planning system) from 94.2% to 97.4%, with a 5%/5 mm criteria. The ability to accurately measure dose at a transit level, without the need for correction factors derived from extensive commissioning data collection procedures, makes the MP987 a viable alternative to the EPID for in vivo dosimetry. This MP987 is this first of its kind to be successfully developed specifically for a dual detector application.

15 Characterization of an aSi-1000 EPID response in integrated and continuous acquisition modes for SBRT dosimetry applications

Introduction The amorphous silicon (a-Si) electronic portal imaging devices (EPIDs) mounted on linacs have been extensively studied and have shown dose-response characteristics suitable for dose verification. Their use for dosimetric purposes is already in clinical routine. However, the response of EPID in continuous acquisition mode was not so studied in literature, and in less extent when considering it for dosimetric applications in stereotactic body radiotherapy (SBRT) conditions (i.e. small fields, and high dose rates using flattening filter free (FFF) beams). The purpose of this work was therefore to characterize the EPID response in terms of relevant dosimetric properties, mainly for the potential use of continuous mode for further SBRT dosimetry with dynamic techniques. Methods The EPID used in this study was the aSi-1000 model installed on a Varian TrueBeamSTx. Its response was characterized in FFF photon beams with nominal energies of 6 and 10 MV and maximum available dose rates (1400 and 2400 MU/min, for 6 and 10 MV respectively). Different sets of images were acquired in both integrated and continuous modes, with EPID at 150 cm from the source, to study dose linearity, repeatability and reproducibility of response, field-size dependence, ghosting effect and response to energy spectrum variations. Before data acquisition, dark field and flood filed images were acquired for offset and gain corrections. As fluctuations in dose rate can potentially influence the response of EPID when using dynamic techniques with continuous acquisition mode, the linearity of EPID response was further evaluated by irradiating a 10 × 10 cm2 field in 20° and 90° arcs focusing on low MU range (3–100 MU), and was also compared to static irradiations. Results Conclusions The characterization of the aSi-1000 EPID response to relevant dosimetric properties was investigated, showing the feasibility of its use for dosimetry with FFF beams and high dose rates. Evaluation of the Portal Imaging System Performance for an Elekta Precise Linac in Radiotherapy

Introduction: Electronic portal imaging devices (EPIDs) provide two- and three-dimensional planar and volumetric cone beam images to improve the accuracy of radiation treatment delivery. Periodic quality assurance (QA) of EPIDs is essential for dosimetric verification in radiotherapy. In this study, a QA program was implemented to evaluate the function of the EPID to be confident in applying corrections for the uncertainty of patient set-up. Material and Methods: Firstly, the safety features were verified, and the uniformity of EPID response was evaluated using flat panel detector. Additionally, the contrast and spatial resolutions of the EPID were assessed using detail counting of the Los Vegas phantom images by visualization method and measuring the modulation transfer function using edge technique, respectively. Moreover, a combination of smoothing methods was used for optimal use of edge detection algorithm for the noisy portal images. Finally, the location of the central ray on the EPID surface at different gantry angles was determined to evaluate the mechanical stability of the supporting arm. Results: The safety interlocks were found to be functional.The EPID response variation was less than 3% according to the results obtained from the detector. The contrast resolution met the recommended tolerance; however, the visualization method was widely observer-dependent. The value of f50 for spatial resolution was 0.401±0.005 lp/mm for the photon energy of 6 MV. The supporting arm deviation was within ±1 mm. Conclusion: The periodic QA of image guidance system gave confidence to apply the corrections for set-up in clinic.

Development and implementation of EPID-based quality assurance tests for the small animal radiation research platform (SARRP).

Although small animal image-guided radiotherapy (SA-IGRT) systems are used increasingly in preclinical research, tools for performing routine quality assurance (QA) have not been optimized and are not readily available. Robust, efficient, and reliable QA tools are needed to ensure the accuracy and reproducibility of SA-IGRT systems. Several investigators have reported custom-made phantoms and protocols for SA-IGRT systems QA. These are typically time and resource intensive and are therefore not well suited to the preclinical radiotherapy environment, in which physics support is limited and routine QA is performed by technical staff. We investigated the use of the inbuilt electronic portal imaging device (EPID) to develop and validate routine QA tests and procedures. In this work, we focus on the Xstrahl Small Animal Radiation Research Platform (SARRP) EPID. However, the methodology and tests developed here are applicable to any SA-IGRT system that incorporates an EPID. We performed a comprehensive characterization of the dosimetric properties of the camera-based EPID at kilovoltage energies over a 11-month period, including detector warm-up time, radiation dose history effect, stability and short- and long-term reproducibility, gantry angle dependency, output factor, and linearity of the EPID response. We developed a test to measure the constancy of beam quality in terms of half-value layer and tube peak potential using the EPID. We verified the SARRP daily output and beam profile constancy using the imager. We investigated the use of the imager to monitor beam-targeting accuracy at various gantry and couch angles. The EPID response was stable and reproducible, exhibiting maximum variations of ≤0.3% and ≤1.9% for short and long terms, respectively. The detector showed no dependence on response at different gantry angles, with a maximum variation ≤0.5%. We found close agreement in output factor measurement between the portal imager and reference dosimeters, with maximum differences ≤3% for ionization chamber and ≤1.7% for Gafchromic EBT3 dosimetry film, respectively. We have shown that the EPID response is linear with tube current (mA) for the entire range of tube kilovoltage peak. Notably, a close relationship was seen between the detector response vs mA slope, and the kilovoltage peak, allowing an independent verification of kilovoltage peak stability based solely on EPID response. In addition to dosimetry tests, according to the beam-targeting measurement using portal images, maximum displacement of the central axis of the x-ray beam (due to sag) was 0.76±0.09mm at gantry 135°/couch 0° and 0.89±0.06mm at gantry 0°/couch -135°. We performed the first comprehensive analysis on the dosimetric properties of an EPID operating at kilovoltage x-ray energies. We characterized the detector performance over a 11-month period. Our results indicate that the imager is a stable and convenient tool for SARRP routine QA tests. We then developed EPID-based tests to perform routine SA-IGRT systems QA tasks, such as verifying constancy of beam quality, energy, output, and profile measurements, relative output factors, and beam targeting.

Normalize the response of EPID in pursuit of linear accelerator dosimetry standardization.

Normalize the response of electronic portal imaging device (EPID) is the first step toward an EPID‐based standardization of Linear Accelerator (linac) dosimetry quality assurance. In this study, we described an approach to generate two‐dimensional (2D) pixel sensitivity maps (PSM) for EPIDs response normalization utilizing an alternative beam and dark‐field (ABDF) image acquisition technique and large overlapping field irradiations. The automated image acquisition was performed by XML‐controlled machine operation and the PSM was generated based on a recursive calculation algorithm for Varian linacs equipped with aS1000 and aS1200 imager panels. Cross‐comparisons of normalized beam profiles and 1.5%/1.5 mm 1D Gamma analysis was adopted to quantify the improvement of beam profile matching before and after PSM corrections. PSMs were derived for both photon (6, 10, 15 MV) and electron (6, 20 MeV) beams via proposed method. The PSM‐corrected images reproduced a horn‐shaped profile for photon beams and a relative uniform profiles for electrons. For dosimetrically matched linacs equipped with aS1000 panels, PSM‐corrected images showed increased 1D‐Gamma passing rates for all energies, with an average 10.5% improvement for crossline and 37% for inline beam profiles. Similar improvements in the phantom study were observed with a maximum improvement of 32% for 15 MV and 22% for 20 MeV. The PSM value showed no significant change for all energies over a 3‐month period. In conclusion, the proposed approach correct EPID response for both aS1000 and aS1200 panels. This strategy enables the possibility to standardize linac dosimetry QA and to benchmark linac performance utilizing EPID as the common detector.

Abstract ID: 130 Energy response correction for EPID dosimetry in photon radiation therapy

Electronic portal imaging device (EPID) is a popular tool for a verification of the patient setup for radiation therapy. Recently, it is grabbing attention as a tool for in vivo dosimetry. It detects therapeutic photons which penetrated the patient body to acquire an image. Conversion to absorbed dose is required from the digital pixel value. However, the response of the EPID is susceptible to the energy spectrum changes through the body [1] , [2] . The purpose of this study was to perform the energy response correction of the EPID image using Monte Carlo simulation (MC) to improve the accuracy of the EPID dosimetry. Geant4.9.6 patch04 was used for the MC with a combination of a HTCondor 7.6 parallel computing software. The MC simulated the 6 and 10 MV photon beams from Trilogy (Varian Medical Systems) linear accelerator. The EPID response to the energy spectrum was evaluated. CT DICOM data set of a chest phantom was imported to the MC, and the simulated EPID images were acquired after irradiating 6 and 10 MV photon beams. The measurement was performed with the same setup as the MC and obtained the EPID images. The energy response was corrected to obtain the absorbed dose and compared with that measured with 2D array detector (PTW) as a reference. Multi-parametric Improvements in the CCD Camera-based EPID for Portal Dosimetry

Dosimetric verification of radiation treatment has recently been extended by the introduction of electronic portal imaging devices (EPIDs). Detailed dose response specifications of EPID should be addressed prior to any dosimetric application. The present study evaluates improvements of dosimetric properties of the low elbow camera-based EPID Theraview (Cablon Medical, Leusden, The Netherlands) equipped with a cooled charge coupled device (CCD) for portal dosimetry. The dose response, warm-up behavior, stability over long- and short-term scales (throughout a day) were studied. The field size dependency of the EPID response was also investigated and compared with ion chamber measurements under the same conditions. The EPID response without saturation for doses up to 2 Gy was linear for both beam qualities (6 and 15 MV). There was no evident warm-up characteristic. The detector sensitivity showed excellent stability in short term [standard deviation (SD) 0.38%]. In long-term stability (over a period of approximately 3 months), a negligible linear decline of 0.01% per day was observed. It was concluded that the cooled CCD camera-based EPID could be used for portal dosimetry, after accurate corrections for the field size dependency and sensitivity loss.

MO‐AB‐BRA‐03: Development of Novel Real Time in Vivo EPID Treatment Verification for Brachytherapy

Purpose:High Dose Rate (HDR) brachytherapy treatments are employed worldwide to treat a wide variety of cancers. However, in vivo dose verification remains a challenge with no commercial dosimetry system available to verify the treatment dose delivered to the patient. We propose a novel dosimetry system that couples an independent Monte Carlo (MC) simulation platform and an amorphous silicon Electronic Portal Imaging Device (EPID) to provide real time treatment verification.Methods:MC calculations predict the EPID response to the photon fluence emitted by the HDR source by simulating the patient, the source dwell positions and times, and treatment complexities such as tissue compositions/densities and different applicators. Simulated results are then compared against EPID measurements acquired with ∼0.14s time resolution which allows dose measurements for each dwell position. The EPID has been calibrated using an Ir‐192 HDR source and experiments were performed using different phantoms, including tissue equivalent materials (PMMA, lung and bone). A source positioning accuracy of 0.2 mm, without including the afterloader uncertainty, was ensured using a robotic arm moving the source.Results:An EPID can acquire 3D Cartesian source positions and its response varies significantly due to differences in the material composition/density of the irradiated object, allowing detection of changes in patient geometry. The panel time resolution allows dose rate and dwell time measurements. Moreover, predicted EPID images obtained from clinical treatment plans provide anatomical information that can be related to the patient anatomy, mostly bone and air cavities, localizing the source inside of the patient using its anatomy as reference.Conclusion:Results obtained show the feasibility of the proposed dose verification system that is capable to verify all the brachytherapy treatment steps in real time providing data about treatment delivery quality and also applicator/structure motion during or between treatments.

SU‐F‐T‐259: GPR Tables for the Estimation of Mid‐Plane Dose Using EPID

Purpose:To develop a simple method for estimating the mid‐plane dose (MPD) of a patient using Electronic Portal imaging Device (EPID).Methods:A Varian TrueBeam with aSi100 EPID was used in this study. The EPID images were acquired for a 30 cm × 30 cm homogeneous slab phantom and a 30 cm diameter 20 cm thick cylindrical phantom in the continuous dosimetry mode. The acquired EPID images in XIM format were imported into in‐house MATLAB program for the data analysis. First, the dosimetric characteristics of EPID were studied for dose‐response linearity, dose‐rate dependence, and field size dependence. Next, the average pixels values of the EPID images were correlated with the MPD measured by an ionisation chamber for various thicknesses of the slab phantom (8 cm – 30 cm) and for various square field sizes (3×3 cm2 – 25×25 cm2 at the isocenter). Look‐up tables called as GPR tables were then generated for both SSD and SAD setup by taking the ratio of MPD measured by the ionisation chamber and the corresponding EPID pixel values. The accuracy of the GPR tables was evaluated by varying the field size, phantom thickness, and wedge angles with the slab and cylindrical phantoms.Results:The dose response of EPID was linear from 20 MU to 300 MU. The EPID response for different dose rates from 40 MU/min to 600 MU/min was within ±1%. The difference in the doses from the GPR tables and the doses measured by the ionization chambers were within 2% for slab phantoms, and 3% for the cylindrical phantom for various field sizes, phantom thickness, and wedge angles.Conclusion:GPR tables are a ready reckoner for in‐vivo dosimetry and it can be used to quickly estimate the MPD value from the EPID images with an accuracy of ±3% for common clinical treatment. project work funded by Union for International cancer control(UICC) under ICRETT fellowship

Dose response characteristics of a novel CCD camera-based electronic portal imaging device comparison with OCTAVIUS detector.

Dosimetric properties of a CCD camera-based Electronic Portal Imaging Device (EPID) for clinical dosimetric application have been evaluated. Characteristics obtained by EPID also compared with commercial 2D array of ion chambers. Portal images acquired in dosimetry mode then exported raw fluence or uncorrected images were investigated. Integration time of image acquisition mode has adjusted on 1 s per frame. As saturation of camera of the EPID, dose response does not have linear behavior. The slight nonlinearity of the camera response can be corrected by a logarithmic expression. A fourth order polynomial regression model with coefficient of determination of 0.998 predicts a response to absolute dose values at less than 50 cGy. A field size dependent response of up to 7% (0.99-1.06) relative OCTAVIUS detector measurement was found. The EPID response can be fitted by a cubic regression for field size changes, yielded coefficient of determination of 0.999. These results indicate that the EPID is well suited for accurate dosimetric purposes, the major limitation currently being due to integration time and dead-time in frame acquisition.

Transit dosimetry in dynamic IMRT with an a-Si EPID.

Using an amorphous silicon (a-Si) EPID for transit dosimetry requires detailed characterization of its dosimetric response in a variety of conditions. In this study, a measurement-based model was developed to calibrate an a-Si EPID response to dose for transit dosimetry by comparison with a reference ionization chamber. The ionization chamber reference depth and the required additional buildup thickness for electronic portal imaging devices (EPID) transit dosimetry were determined. The combined effects of changes in radiation field size, phantom thickness, and the off-axis distance on EPID transit dosimetry were characterized. The effect of scattered radiation on out-of-field response was investigated for different field sizes and phantom thicknesses by evaluation of the differences in image profiles and in-water measured profiles. An algorithm was developed to automatically apply these corrections to EPID images based on the user-specified field size and phantom thickness. The average phantom thickness and an effective field size were used for IMRT fields, and images were acquired in cine mode in the presence of an anthropomorphic phantom. The effective field size was defined as the percentage of the jaw-defined field that was involved during the delivery. Nine head and neck dynamic IMRT fields were tested by comparison with a MatriXX two-dimensional array dosimeter using the Gamma (3%, 3 mm) evaluation. A depth of 1.5 cm was selected as the ionization chamber reference depth. An additional 2.2 mm of copper buildup was added to the EPID. Comparison of EPID and MatriXX dose images for the tested fields showed that using a 10% threshold, the average number of points with Gamma index <1 was 96.5%. The agreement in the out-of field area was shown by selection of a 2% threshold which on average resulted in 94.8% of points with a Gamma index <1. The suggested method is less complicated than previously reported techniques and can be used for all a-Si EPIDs regardless of the manufacturer.

TU‐F‐BRE‐04: Development of a High‐Resolution EPID Based Dosimetry Strategy for Radiosurgery QA

Purpose:To systematically investigate a high spatial‐resolution (0.2mm) electronic portal imaging device (EPID) for CyberKnife (CK) based radiosurgery system quality assurance (QA).Methods:An EPID‐based dosimetric measurement technique is applied to CK output measurement and field size verification. A Monte Carlo (MC) simulated pixel‐to‐pixel EPID response specific to CK is used to convert a raw EPID‐measured image of a radiosurgery field into water‐based dose distribution. The output factors are measured using EPID for radiosurgery fields formed by fixed and variable aperture (Iris) cones. Circular fields of 5, 7.5, 10, 15, 30 and 60mm diameters are measured and compared with diode measurements. The equivalent diameters are determined by analyzing the area received dose greater than half maximum.Results:For both fixed and Iris cones, the EPID measured output factors of circular fields of 5mm to 60mm diameters are in good agreement with the radiosurgery diode measurements. The mean output differences are 1.0% and 1.5% for fixed and Iris cone respectively. The max differences are 2.2% for the 15mm fixed cone, and 1.8% for the 10mm Iris field. The equivalent diameters derived from the EPID measurements are in good agreement comparing to the water scan result with mean differences of 0.21±0.09mm and 0.02±0.22mm for fixed and Iris cone, respectively. The high detector density EPID is able to measure the whole radiation field and identify the field edge and center. Therefore, there is no need to align the detector center perfectly at field center and the setup time is greatly reduced for QA.Conclusion:The high spatial‐resolution EPID is proved to be an accurate and efficient dosimetric tool for radiosurgery QA and especially useful in Cyberknife QA for variable aperture collimators.

WE-E-18A-08: Towards a Next-Generation Electronic Portal Device for Simultaneous Imaging and Dose Verification in Radiotherapy

Purpose: This work forms part of an ongoing study to develop a next-generation electronic portal imaging device (EPID) for simultaneous imaging and dose verification in radiotherapy. Monte Carlo (MC) simulations were used to characterize the imaging performance of a novel EPID that has previously been demonstrated to exhibit a water-equivalent response. The EPID ' s response was quantified in several configurations and model parameters were empirically validated against experimental measurements. Methods: A MC model of a novel a-Si EPID incorporating an array of plastic scintillating fibers was developed. Square BCF-99-06A scintillator fibers with PMMA cladding (Saint-Gobain Crystals) were modelled in a matrix with total area measuring 150×150 mm{sup 2}. The standard electromagnetic and optical physics Geant4 classes were used to simulate radiation transport from an angled slit source (6 MV energy spectrum) through the EPID and optical photons reaching the photodiodes were scored. The prototype's modulation transfer function (MTF) was simulated and validated against experimental measurements. Several optical transport parameters, fiber lengths and thicknesses of an air gap between the scintillator and photodiodes were investigated to quantify their effects on the prototype's detection efficiency, sensitivity and MTF. Results: Simulated EPID response was more sensitive to variations in geometry than inmore » the optical parameters studied. The MTF was particularly sensitive to the introduction of a 0.5–1.0 mm air gap between the scintillator and photodiodes, which lowered the MTF relative to that simulated without the gap. As expected, increasing the fiber length increased the detector efficiency and sensitivity while decreasing the MTF. Conclusion: A model of a novel water-equivalent EPID has been developed and benchmarked against measurements using a physical prototype. We have demonstrated the feasibility of this new device and are continuing to optimize the design to achieve an imaging response that warrants the development of a next-generation prototype.« less

Monte Carlo simulation of the transit dosimetric response of an a-Si electronic portal imaging device

Amorphous silicon (a-Si) electronic portal imaging devices (EPIDs) are x-ray detectors frequently used in radiotherapy imaging and dosimetry applications. EPIDs employ a copper plate and gadolinium oxysulfide phosphor screen with an array of a-Si photodiodes to indirectly detect incident radiation. In this study, a previously developed Monte Carlo (MC) model of an a-Si EPID has been extended for transit dosimetry. The GEANT4 MC toolkit was used to integrate an a-Si EPID model with two phantoms and a 6 MV x-ray source. A solid water phantom was used to simulate EPID transmission factors, field size output factors and relative dose profiles and results were compared to experimental measurements. An anthropomorphic head phantom was used to qualitatively compare simulated and measured portal images of humanoid anatomy. Calculated transmission factors and field size output factors agreed to within 2.0% and 1.9% of experimental measurements, respectively. A comparison of calculated and measured relative dose profiles yielded >98% of points passing a gamma analysis with 3%/3 mm criterion for all field sizes. The simulated anthropomorphic head phantom image shows macroscopic anatomical features and qualitatively agrees with the measured image. Results validate the suitability of the MC model for predicting EPID response in transit dosimetry.

Experimental validation of a MCNP-based model to predict portal dose images from MLC beams

This work presents a technique to perform an electronic portal imaging device (EPID) signal planar calibration into dose terms using Monte Carlo simulations. To that, a complete Monte Carlo (MC) simulation of a radiotherapy treatment linear accelerator including a multi-leaf collimator system (MLC) and an EPID has been developed. Several solid water blocks were used to obtain calibration measurements and simulations with a large dose range in order to study the relation between the squared EPID response and the MC calculated dose. The calibrated EPID has also been used as a dosimetric tool to validate the MC MLC Linac model. Simulation and measurement comparisons are below 5% root mean square of percentage dose difference. This technique could potentially provide a more accurate verification of dose delivery to heterogeneous anatomical regions in patients receiving complex multi-field conformal radiation therapy treatments.
