Comparison of deep learning models for building two-dimensional non-transit EPID Dosimetry on Varian Halcyon.

This study compared the effectiveness of five deep learning models in constructing non-transit dosimetry with an a-Si electronic portal imaging device (EPID) on Varian Halcyon. Deep learning model is increasingly used to support prediction and decision-making in several fields including oncology and radiotherapy. Forty-seven unique plans of data obtained from breast cancer patients were calculated using Eclipse treatment planning system (TPS) and extracted from DICOM format as the ground truth. Varian Halcyon was then used to irradiate the a-Si 1200 EPID detector without an attenuator. The EPID and TPS images were augmented and divided randomly into two groups of equal sizes to distinguish the validation and training-test data. Five different deep learning models were then created and validated using a gamma index of 3%/3 mm. Four models successfully improved the similarity of the EPID images and the TPS-generated planned dose images. Meanwhile, the mismatch of the constituent components and number of parameters could cause the models to produce wrong results. The average gamma pass rates were 90.07 ± 4.96% for A-model, 77.42 ± 7.18% for B-model, 79.60 ± 6.56% for C-model, 80.21 ± 5.88% for D-model, and 80.47 ± 5.98% for E-model. The deep learning model is proven to run fast and can increase the similarity of EPID images with TPS images to build non-transit dosimetry. However, more cases are needed to validate this model before being used in clinical activities.

Tungsten Filled 3-Dimensional Printed Lung Blocks for Total Body Irradiation

Lung blocks for total-body irradiation are commonly used to reduce lung dose and prevent radiation pneumonitis. Currently, molten Cerrobend containing toxic materials, specifically lead and cadmium, is poured into molds to construct blocks. We propose a streamlined method to create 3-dimensional (3D)-printed lung block shells and fill them with tungsten ball bearings to remove lead and improve overall accuracy in the block manufacturing workflow. 3D-printed lung block shells were automatically generated using an inhouse software, printed, and filled with 2 to 3 mm diameter tungsten ball bearings. Clinical Cerrobend blocks were compared with the physician drawn blocks as well as our proposed tungsten filled 3D-printed blocks. Physical and dosimetric comparisons were performed on a linac. Dose transmission through the Cerrobend and 3D-printed blocks were measured using point dosimetry (ion-chamber) and the on-board Electronic-Portal-Imaging-Device (EPID). Dose profiles from the EPID images were used to compute the full-width-half-maximum and to compare with the treatment-planning-system. Additionally, the coefficient-of-variation in the central 80% of full-width-half-maximum was computed and compared between Cerrobend and 3D-printed blocks. The geometric difference between treatment-planning-system and 3D-printed blocks was significantly lower than Cerrobend blocks (3D: -0.88 ± 2.21 mm, Cerrobend: -2.28 ± 2.40 mm, P = .0002). Dosimetrically, transmission measurements through the 3D-printed and Cerrobend blocks for both ion-chamber and EPID dosimetry were between 42% to 48%, compared with the open field. Additionally, coefficient-of-variation was significantly higher in 3D-printed blocks versus Cerrobend blocks (3D: 4.2% ± 0.6%, Cerrobend: 2.6% ± 0.7%, P < .0001). We designed and implemented a tungsten filled 3D-printed workflow for constructing total-body-irradiation lung blocks, which serves as an alternative to the traditional Cerrobend based workflow currently used in clinics. This workflow has the capacity of producing clinically useful lung blocks with minimal effort to facilitate the removal of toxic materials from the clinic.

EPID-based in vivo dosimetry – new developments and applications

In vivo dosimetry has been shown to be a powerful quality assurance method in modern radiation therapy. The most common tool used for in vivo dosimetry is the electronic portal imaging device (EPID) which can quantitatively image the therapeutic beam fluence exiting the patient during treatment delivery. Since the last major literature review on this topic was published five years ago, the radiation oncology community has shown continued strong interest in this subject. Commercial options have become more widely available, with a related increase in validation efforts and sensitivity testing, while new applications continue to be explored. Work has been done to understand and increase the accuracy of the EPID for dosimetric applications, as well as continued efforts to provide practical, quantitative experiences from clinical implementation of in vivo dosimetry systems. This review examines the published literature related to in vivo EPID dosimetry from January 2017 to February 2022. The literature is classified into three main topical areas: (1) new or improved algorithmic developments including validation work, (2) applications of the in vivo EPID dosimetry method, and (3) error identification and error sensitivity analyses.

A Method for &lt;i&gt;In Vivo&lt;/i&gt; Dosimetry Using an Electronic Portal Imaging Device

The implementation of advanced radiotherapy techniques such as Intensity-Modulated RadiationTherapy (IMRT) and Volumetric Modulated Arc Therapy (VMAT) allows much more accuratecontrol of the irradiation area and more conformal dose delivery. Since these treatments are characterizedby high dose gradients and hence small error margins, they also require advanced and highlyeffective treatment verification techniques. Fortunately, an imaging modality, originally intended forset-up verifications, called the Electronic Portal Imaging Device (EPID) was reported to have hugepotential for dosimetry and treatment verification applications. However, methods and algorithmspreviously developed for EPID dosimetry are not yet widely adopted in cancer centers due to complexitiesin the implementation. These motivate more investigation and development of new methodsto encourage cancer centers to practice EPID dosimetry and improve patient safety.This research generally aims to develop an in-vivo dosimetry method that can be used in the future toapproximate the actual dose delivered inside the target by utilizing the transmitted fluence detectedby the EPID. In this study, Monte Carlo simulations are carried out using the Geant4 Application forTomographic Emission (GATE) to implement a virtual set-up composed of a water target irradiatedwith a monoenergetic 2 MeV photon beam and an EPID model to detect the total transmitted fluence.A mathematical model has been proposed to calculate the target absorbed dose by utilizing the transmittedprimary fluence. This involves back-projection of the transmitted fluence to determine fluencevalues at certain depths inside the irradiated homogeneous medium. This is followed by the calculationof the collision kerma using the back-projected fluence and the values of the linear attenuation coefficientand mass energy absorption coefficient in the National Institute of Standards and Technology(NIST) database. The results show accurate prediction of the fluence values inside the target, calculationof the corresponding values of the collision kerma, and determination of the proportionalityconstant β that relates the collision kerma to the target absorbed dose.

Effect of metal implants and metal artifacts on back-projected two-dimensional entrance fluence determined by EPID dosimetry.

To evaluate the errors caused by metal implants and metal artifacts in the two-dimensional entrance fluences reconstructed using the back-projection algorithm based on electronic portal imaging device (EPID) images. The EPID in the Varian VitalBeam accelerator was used to acquire portal dose images (PDIs), and then commercial EPID dosimetry software was employed to reconstruct the two-dimensional entrance fluences based on computed tomography (CT) images of the head phantoms containing interchangeable metal-free/titanium/aluminum round bars. The metal-induced errors in the two-dimensional entrance fluences were evaluated by comparing the γ results and the pixel value errors in the metal-affected regions. We obtained metal-artifact-free CT images by replacing the voxel values of non-metal inserts with those of metal inserts in metal-free CT images to evaluate the metal-artifact-induced errors. The γ passing rates (versus PDIs obtained without a phantom in the beam field (PDIair ), 2%/2mm) for the back-projected two-dimensional entrance fluences of phantoms containing titanium or aluminum (BPTi /BPAl ) were reduced from 92.4% to 90.5% and 90.6%, respectively, relative to the metal-free phantom (BPmetal-free ). Titanium causes more severe metal artifacts in CT images than aluminum, and its removal resulted in a 0.0022 CU (median) reduction in the pixel value of BPTi artifact-free relative to BPTi in the metal-affected region. Moreover, the mean absolute error (MAE) and root mean square error (RMSE) decreased from 0.0050 CU and 0.0063 CU to 0.0034 CU and 0.0040 CU, respectively (vs. BPmetal-free ). Metal implants increase the errors in back-projected two-dimensional entrance fluences, and metals with higher electron densities cause more errors. For high-electron-density metal implants that produce severe metal artifacts (e.g., titanium), removing metal artifacts from the CT images can improve the accuracy of the two-dimensional entrance fluences reconstructed by back-projection algorithms.

AAPM Task Group Report 307: Use of EPIDs for Patient-Specific IMRT and VMAT QA.

Electronic portal imaging devices (EPIDs) have been widely utilized for patient-specific quality assurance (PSQA) and their use for transit dosimetry applications is emerging. Yet there are no specific guidelines on the potential uses, limitations, and correct utilization of EPIDs for these purposes. The American Association of Physicists in Medicine (AAPM) Task Group 307 (TG-307) provides a comprehensive review of the physics, modeling, algorithms and clinical experience with EPID-based pre-treatment and transit dosimetry techniques. This review also includes the limitations and challenges in the clinical implementation of EPIDs, including recommendations for commissioning, calibration and validation, routine QA, tolerance levels for gamma analysis and risk-based analysis. Characteristics of the currently available EPID systems and EPID-based PSQA techniques are reviewed. The details of the physics, modeling, and algorithms for both pre-treatment and transit dosimetry methods are discussed, including clinical experience with different EPID dosimetry systems. Commissioning, calibration, and validation, tolerance levels and recommended tests, are reviewed, and analyzed. Risk-based analysis for EPID dosimetry is also addressed. Clinical experience, commissioning methods and tolerances for EPID-based PSQA system are described for pre-treatment and transit dosimetry applications. The sensitivity, specificity, and clinical results for EPID dosimetry techniques are presented as well as examples of patient-related and machine-related error detection by these dosimetry solutions. Limitations and challenges in clinical implementation of EPIDs for dosimetric purposes are discussed and acceptance and rejection criteria are outlined. Potential causes of and evaluations of pre-treatment and transit dosimetry failures are discussed. Guidelines and recommendations developed in this report are based on the extensive published data on EPID QA along with the clinical experience of the TG-307 members. TG-307 focused on the commercially available EPID-based dosimetric tools and provides guidance for medical physicists in the clinical implementation of EPID-based patient-specific pre-treatment and transit dosimetry QA solutions including intensity modulated radiation therapy (IMRT) and volumetric modulated arc therapy (VMAT) treatments.

Dosimetric Performance Evaluation of MLC-based and Cone-based 3D Spatially Fractionated LATTICE Radiotherapy.

This study aims to dosimetrically compare multi-leaf collimator (MLC)-based and cone-based 3D LATTICE radiotherapy (LRT) plans. Valley-peak ratios were evaluated using seven different 3D LATTICE designs. Target volumes of 8 cm and 12 cm were defined on the RANDO phantom. Valley-peak dose patterns were obtained by creating high-dose vertices in the target volumes. By changing the vertex diameter, vertices separation, and volume ratio, seven different LATTICE designs were generated. Treatment plans were implemented using CyberKnife and Varian RapidArc. Thermoluminescent dosimeter (TLD), EBT3 films, and electronic portal-imaging device (EPID) were employed for dosimetric treatment verification, and measured doses were compared to calculated doses. By changing the vertex diameter and vertices separation, the valley-peak ratio was exhibited little difference between the two systems. By changing the vertex diameter and volume ratio, the valley-peak ratio was observed nearly the same for the two systems. The film, TLD, and EPID dosimetry showed good agreement between the calculated and measured doses. Based on the results, we concluded that although smaller valley-peak ratios were obtained with cone-based plans, the dose-volume histograms were comparable in both systems. Also, when we evaluated the treatment duration, the MLC-based plans were more appropriate to apply the treatment in a single fraction.

Effects of reconstruction methods on dose distribution for lung stereotactic body radiotherapy treatment plans.

The aim of the present study was to investigate the effect of tumour motion on various imaging strategies as well as on treatment plan accuracy for lung stereotactic body radiotherapy treatment (SBRT) cases. The ExacTrac gating phantom and paraffin were used to investigate respiratory motion and represent a lung tumour, respectively. Four-dimensional computed tomography (4DCT) imaging was performed, while the phantom was moving sinusoidally with 4s cycling time with three different amplitudes of 8, 16, and 24mm. Reconstructions were done with maximum (MIP) and average intensity projection (AIP) methods. Comparisons of target density and volume were performed using two reconstruction techniques and references values. Volumetric modulated arc therapy (VMAT) and intensity modulated radiation therapy (IMRT) were planned based on reconstructed computed tomography (CT) sets, and it was examined how density variations affect the dose-volume histogram (DVH) parameters. 4D cone beam computed tomography (CBCT) was performed with the Elekta Versa HD linac imaging system before irradiation and compared with 3D CBCT. Thus, various combinations of 4DCT reconstruction methods and treatment alignment methods have been investigated. Point measurements as well as 2 and 3D dose measurements were done by optically stimulated luminescence (OSL), gafchromic films, and electronic portal imaging devices (EPIDs), respectively. The mean volume reduction was 7.8% for the AIP and 2.6% for the MIP method. The obtained Hounsfield Unit (HU) values were lower for AIP and higher for MIP when compared with the reference volume density. In DVH analysis, there were no statistical differences for D95%, D98%, and Dmean (p > 0.05). However, D2% was significantly affected by HU changes (p < 0.01). A positional variation was obtained up to 2mm in moving direction when 4D CBCT was applied after 3D CBCT. Dosimetric measurements showed that the main part of the observed dose deviation was due to movement. In lung SBRT treatment plans, D2% doses differ significantly according to the reconstruction method. Additionally, it has been observed that setups based on 3D imaging can cause a positional error of up to 2mm compared to setups based on 4D imaging. It is concluded that MIP has advantages over AIP in defining internal target volume (ITV) in lung SBRT applications. In addition, 4D CBCT and 3D EPID dosimetry are recommended for lung SBRT treatments.

Transit dosimetry of stereotactic body radiotherapy treatments with electronic portal dosimetry device in patient with spinal implant.

In recent years, the use of the Electronic Portal Imaging Device (EPID) as an in vivo dosimeter has become widespread. However, reports of EPID for stereotactic body radiotherapy (SBRT) applications is scarce. There is no data on this topic especially when there are high-density materials in the radiation field. In this study, we aimed to investigate the dose distributions of SBRT treatment plans in patients with spinal implants by transit EPID dosimetry. Implants were inserted in phantoms that mimic the vertebrae, and VMAT plans were created on the phantoms to deliver 16Gy radiation doses to the target in 1 fraction. Transit EPID measurements were performed for each irradiation. The results were compared with the treatment planning system using the gamma analysis method. According to the gamma analysis results, while the non-implant model met the acceptance criteria with a rate of 95.4%, the implanted models did not pass the test with results between the rates of 70% to 73%. In addition, while the dose difference in the isocenter was 1.3% for the non-implanted model, this difference was observed to be between 7 and 8% in the implanted models. Our study revealed that EPID can be used as transit dosimetry for the VMAT-SBRT applications. However, unacceptable dose differences were obtained by transit EPID dosimetry in the VMAT-SBRT applications of patients with an implant. In the treatment of such patients, alternative treatment methods should be preferred in which the interaction of the implants with radiation can be prevented.

A Large Area Pixelated Silicon Array Detector for Independent Transit In Vivo Dosimetry

A large area pixelated silicon array detector named “MP987” has been developed for in vivo dosimetry. The detector was developed to overcome the non-water equivalent response of EPID (Electronic Portal Imaging Device) dosimetry systems, due to the shortfalls of the extensive corrections required. The detector, readout system and software have all been custom designed to be operated independently from the linac with the array secured directly above the EPID, to be used in combination with the 6 MV imaging system. Dosimetry characterisation measurements of percentage depth dose (PDD), dose rate dependence, radiation damage, output factors (OF), profile measurements, linearity and uniformity were performed. Additionally, the first pre-clinical tests with this novel detector of a transit dosimetry characterization and a collapsed IMRT (intensity-modulated radiation therapy) study are presented. Both PDD and OF measurements had a percentage difference of less than 2.5% to the reference detector. A maximum change in sensitivity of 4.3 ± 0.3% was observed after 30 kGy of gamma accumulated dose. Transit dosimetry measurements through a homogeneous Solid Water phantom had a measured dose within error of the TPS calculations, for field sizes between 3 × 3 cm2 and 10 × 10 cm2. A four-fraction collapsed IMRT plan on a lung phantom had absolute dose pass fractions between the MP987 and TPS (treatment planning system) from 94.2% to 97.4%, with a 5%/5 mm criteria. The ability to accurately measure dose at a transit level, without the need for correction factors derived from extensive commissioning data collection procedures, makes the MP987 a viable alternative to the EPID for in vivo dosimetry. This MP987 is this first of its kind to be successfully developed specifically for a dual detector application.

Voxel-Level BED Corrected Dosimetric and Radiobiological Assessment of 2 Kinds of Hybrid Radiotherapy Planning Methods for Stage III NSCLC.

Background/purpose: To access the comparative dosimetric and radiobiological advantages of two methods of intensity-modulated radiation therapy (IMRT)-based hybrid radiotherapy planning for stage III nonsmall cell lung cancer (NSCLC). Methods: Two hybrid planning methods were respectively characterized by conventional fraction radiotherapy (CFRT) and stereotactic body radiotherapy (SBRT) and CFRT and simultaneous integrated boost (SIB) planning. All plans were retrospectively completed using the 2 methods for 20 patients with stage III NSCLC. CFRT and SBRT dose regimes 2 Gy × 30 f and 12.5 Gy × 4 f were, respectively, used for planning target volume of lymph node (PTVLN) and planning target volume of the primary tumor (PTVPT), while dose regimes 2 Gy × 26 f for PTVLN and sequential 2 Gy × 4 f for PTVLN combined with 12.5 Gy × 4 f for PTVPT were adopted for CFRT and SIB plans. SBRT and SIB EQD2 dose were calculated voxel by voxel, and then, respectively, superimposed with 30-fraction and 26-fraction CFRT plan dose to achieve biological equivalent dose (BED) dosimetric parameters of CFRT and SBRT and CFRT and SIB plans. Tumor control probability (TCP)/normal tissue complication probability (NTCP) was, respectively, calculated by equivalent uniform dose/Lyman–Kutcher–Burman models. BED plan parameters and TCP/NTCP were analyzed between 2 methods of hybrid planning. Primary tumor/lymph node (LN)/total TCP values were, respectively, evaluated as a function of the radiation dose needed to control 50% of tumor (TCD50) for 20 patients. Dosimetric errors were analyzed by nontransit electronic portal imaging device dosimetry measurement during hybrid plan delivery. Results: Statistically lower BED plan parameters of PTVLN D2 and homogeneity index resulted in slightly lower averaged LN/total TCP curves by CFRT and SIB planning. The gaps between Max and Min LN/total TCP curves were significantly closer for CFRT and SIB planning, which indicated better robustness of LN/total TCPs. A lower esophagus dose resulted in a lower esophagus NTCP by CFRT and SIB planning, which may be compromised by 1 week shorter overall treatment time by CFRT and SIB irradiation. Spinal cord Dmax was significantly reduced by CFRT and SIB plans. The dose verification results of the subplans involved in hybrid plans were acceptable, which showed that the 2 methods of hybrid planning could be delivered accurately in our center. Conclusion: CFRT and SIB plannings have more advantages on BED plan parameters and TCP/NTCP than CFRT and SBRT planning, and both methods of IMRT-based hybrid planning could be executed accurately for stage III NSCLC. The effectiveness of the results needs to be validated in the hybrid trial.

External validation of a hidden Markov model for gamma-based classification of anatomical changes in lung cancer patients using EPID dosimetry.

To externally validate a hidden Markov model (HMM) for classifying gamma analysis results of in vivo electronic portal imaging device (EPID) measurements into different categories of anatomical change for lung cancer patients. Additionally, the relationship between HMM classification and deviations in dose-volume histogram (DVH) metrics was evaluated. The HMM was developed at CHU de Québec (CHUQ), and trained on features extracted from gamma analysis maps of in vivo EPID measurements from 483 fractions (24 patients, treated with three-dimensional 3D-CRT or intensity modulated radiotherapy), using the EPID measurement of the first treatment fraction as reference. The model inputs were the average gamma value, standard deviation, and average value of the highest 1% of gamma values, all averaged over all beams in a fraction. The HMM classified each fraction into one of three categories: no anatomical change (Category 1), some anatomical change (no clinical action needed, Category 2) and severe anatomical change (clinical action needed, Category 3). The external validation dataset consisted of EPID measurements from 263 fractions of 30 patients treated at Maastro with volumetric modulated arc therapy (VMAT) or hybrid plans (containing both static beams and VMAT arcs). Gamma analysis features were extracted in the same way as in the CHUQ dataset, by using the EPID measurement of the first fraction as reference (γQ), and additionally by using an EPID dose prediction as reference (γM). For Maastro patients, cone beam computed tomography (CBCT) scans and image-guided radiotherapy (IGRT) classification of these images were available for each fraction. Contours were propagated from the planning CT to the CBCTs, and the dose was recalculated using a Monte Carlo dose engine. Dose-volume histogram metrics for targets and organs-at-risk (OARs: lungs, heart, mediastinum, spinal cord, brachial plexus) were extracted for each fraction, and compared to the planned dose. HMM classification of the external validation set was compared to threshold classification based on the average gamma value alone (a surrogate for clinical classification at CHUQ), IGRT classification as performed at Maastro, and differences in DVH metrics extracted from 3D dose recalculations on the CBCTs. The HMM achieved 65.4%/65.0% accuracy for γQ and γM, respectively, compared to average gamma threshold classification. When comparing HMM classification with IGRT classification, the overall accuracy was 29.7% for γQ and 23.2% for γM. Hence, HMM classification and IGRT classification of anatomical changes did not correspond. However, there is a trend towards higher deviations in DVH metrics with classification into higher categories by the HMM for large OARs (lungs, heart, mediastinum), but not for the targets and small OARs (spinal cord, brachial plexus). The external validation shows that transferring the HMM for anatomical change classification to a different center is challenging, but can still be valuable. The HMM trained at CHUQ cannot be used directly to classify anatomical changes in the Maastro data. However, it may be possible to use the model in a different capacity, as an indicator for changes in the 3D dose based on two-dimensional EPID measurements.

In vivo dosimetry in external beam photon radiotherapy: Requirements and future directions for research, development, and clinical practice.

External beam radiotherapy with photon beams is a highly accurate treatment modality, but requires extensive quality assurance programs to confirm that radiation therapy will be or was administered appropriately. In vivo dosimetry (IVD) is an essential element of modern radiation therapy because it provides the ability to catch treatment delivery errors, assist in treatment adaptation, and record the actual dose delivered to the patient. However, for various reasons, its clinical implementation has been slow and limited. The purpose of this report is to stimulate the wider use of IVD for external beam radiotherapy, and in particular of systems using electronic portal imaging devices (EPIDs). After documenting the current IVD methods, this report provides detailed software, hardware and system requirements for in vivo EPID dosimetry systems in order to help in bridging the current vendor-user gap. The report also outlines directions for further development and research. In vivo EPID dosimetry vendors, in collaboration with users across multiple institutions, are requested to improve the understanding and reduce the uncertainties of the system and to help in the determination of optimal action limits for error detection. Finally, the report recommends that automation of all aspects of IVD is needed to help facilitate clinical adoption, including automation of image acquisition, analysis, result interpretation, and reporting/documentation. With the guidance of this report, it is hoped that widespread clinical use of IVD will be significantly accelerated.

An Empirical Transmitted EPID Dosimetry Method using a Back-Projection Algorithm.

Background:The present study aimed to introduce a rapid transmission dosimetry through an electronic portal-imaging device (EPID) to achieve two-dimensional (2D) dose distribution for homogenous environments. Material and Methods: In this Phantom study, first, the EPID calibration curve and correction coefficients for field size were obtained from EPID and ionization chamber. Second, the EPID off-axis pixel response was measured, and the grey-scale image of the EPID was converted into portal dose image using the calibration curve. Next, the scattering contribution was calculated to obtain the primary dose. Then, by means of a verified back-projection algorithm and the Scatter-to-Primary dose ratio, a 2D dose distribution at the mid-plane was obtained. Results: The results obtained from comparing the transmitted EPID dosimetry to the calculated dose, using commercial treatment planning system with gamma function while there is 3% dose difference and 3mm distance to agreement criteria, were in a good agreement. In addition, the pass rates of γ < 1 was 94.89% for the homogeneous volumes. Conclusion:Based on the results, the method proposed can be used in EPID dosimetry.

SBRT combined with concurrent chemoradiation in stage III NSCLC: Feasibility study of the phase I Hybrid trial

PurposeTo assess the technical and clinical feasibility of the phase I Hybrid trial (NCT01933568), combining SBRT of the primary tumor (PT) and fractionated radiotherapy (FRT) to the lymph nodes (LN). Materials and methodsTen patients with stage III NSCLC with a peripheral PT < 5 cm were prospectively selected. The EQD2 corrected normal tissue dose parameters of the FRT plan of 24×2.75 Gy to PT and 24×2.42 Gy to LN (IMRT) was compared with 3×18 Gy on the PT and 24×2.42 Gy on the LN (VMAT) using a Wilcoxon signed-rank test. To anticipate differential motion between PT and LN, worst-case scenarios for OAR were calculated. Electronic portal imaging device (EPID) dosimetry analysis was performed to rule out dosimetric errors during delivery. ResultsThe Hybrid plans revealed a significant decrease of esophagus EUD n = 0.13, lung V5 and V20 and a significant increase in Dmax of the PRV of the mediastinal envelope. Plans were robust against differential motion of 5 mm between PT and LN in 8 patients and failed in 2 patients due to spinal cord constraints. Average pass rates were ≥87% for EPID dosimetry. ConclusionsSBRT and FRT could be combined within the given OAR constraints. Safety will be assessed in the Hybrid trial.

Two-dimensional EPID dosimetry for an MR-linac: Proof of concept.

At our institute, in vivo patient dose distributions are reconstructed for all treatments delivered using conventional linacs from electronic portal imaging device (EPID) transit images acquired during treatment using a simple back-projection model. Currently, the clinical implementation of MRI-guided radiotherapy systems, which aims for online and real-time adaptation of the treatment plan, is progressing. In our department, the MR-linac (Unity, Elekta AB, Stockholm, Sweden) is now in clinical use. The aim of this work is to demonstrate the feasibility of two-dimensional (2D) EPID dosimetric verification for the magnetic resonance (MR)-linac by comparing back-projected EPID doses to ionization chamber (IC) array dose distributions. Our conventional back-projection algorithm was adapted for the MR-linac. The most important changes involve modeling of the attenuation by and scatter from the cryostat. The commissioning process involved the acquisition of square field EPID measurements using various phantom setups (varying SSD, phantom thickness, and field size). Commissioning models were created for gantry 0°, 90°, and 180° and verified by comparing EPID-reconstructed 2D dose distributions to measurements made with the OCTAVIUS 1500 IC array (PTW, Freiburg, Germany) for two prostate and one rectum IMRT plans (25 beams total). The average of the γ parameters (y-mean and y-pass rate) and the dose difference at a reference point were reported. Due to their construction, the attenuation of couch, bridge, and cryostat shows a much stronger dependence on gantry angle in the MR-linac compared to conventional linacs. We present a method to correct for these effects. This method is validated by dose reconstruction of the 25 intensity-modulated radiation therapy beams recorded at a certain gantry angle using the model of another gantry angle, combined with the correction method. For dose verification performed at a gantry angle identical to the commissioned model, the average y-mean and y-pass rate values (3% global dose, 2mm, 10% isodose) were 0.37±0.07 and 98.1, 95% CI [98.1±2.4], respectively. The average dose difference at the reference point was -0.5%±1.8%. Verification at gantry angles different from the commissioned model (i.e., using the gantry angle dependent correction) reported 0.39±0.08 and 97.6, 95% CI [96.9, 98.3] average y-mean and y-pass rate values. The average dose difference at the reference point was -0.1%±1.8%. The EPID dosimetry back-projection model was successfully adapted for the MR-linac at gantry 0°, 90°, and 180°, accounting for the presence of the MRI housing between phantom (or patient) and the EPID. A method to account for the gantry angle dependence was also tested reporting similar results.

Transit and non-transit 3D EPID dosimetry versus detector arrays for patient specific QA.

PurposeDespite their availability and simplicity of use, Electronic Portal Imaging Devices (EPIDs) have not yet replaced detector arrays for patient specific QA in 3D. The purpose of this study is to perform a large scale dosimetric evaluation of transit and non‐transit EPID dosimetry against absolute dose measurements in 3D.MethodsAfter evaluating basic dosimetric characteristics of the EPID and two detector arrays (Octavius 1500 and Octavius 1000SRS), 3D dose distributions for 68 VMAT arcs, and 10 IMRT plans were reconstructed within the same phantom geometry using transit EPID dosimetry, non‐transit EPID dosimetry, and the Octavius 4D system. The reconstructed 3D dose distributions were directly compared by γ‐analysis (2L2 = 2% local/2 mm and 3G2 = 3% global/2 mm, 50% isodose) and by the percentage difference in median dose to the high dose volume (%∆HDVD 50).ResultsRegarding dose rate dependency, dose linearity, and field size dependence, the agreement between EPID dosimetry and the two detector arrays was found to be within 1.0%. In the 2L2 γ‐comparison with Octavius 4D dose distributions, the average γ‐pass rate value was 92.2 ± 5.2%(1SD) and 94.1 ± 4.3%(1SD) for transit and non‐transit EPID dosimetry, respectively. 3G2 γ‐pass rate values were higher than 95% in 150/156 cases. %∆HDVD 50 values were within 2% in 134/156 cases and within 3% in 155/156 cases. With regard to the clinical classification of alerts, 97.5% of the treatments were equally classified by EPID dosimetry and Octavius 4D.ConclusionTransit and non‐transit EPID dosimetry are equivalent in dosimetric terms to conventional detector arrays for patient specific QA. Non‐transit 3D EPID dosimetry can be readily used for pre‐treatment patient specific QA of IMRT and VMAT, eliminating the need of phantom positioning.

The effect of respiratory motion on electronic portal imaging device dosimetry.

There is an increasing need to develop methods for in vivo verification of the delivery of radiotherapy treatments. Electronic portal imaging devices (EPID's) have been demonstrated to be of use for this application. The basic principle is relatively straightforward, the EPID is used to measure a two‐dimensional (2D) planar exit or portal dose map behind the patient during the treatment delivery that can provide information on any errors in linear accelerator output or changes in the patient anatomy. In this paper we focused on the effect of intra‐fraction motion, particularly respiratory motion, on the measured 2D EPID dose–response. Measurements were made with a breast phantom undergoing one‐dimensional (1D) sinusoidal motion with a range of amplitudes (0.5, 1.0, and 1.5 cm) and frequencies (12, 15, and 20 cycles/min). Further measurements were made with the phantom undergoing breathing sequences measured during patient planning computed tomography simulation. We made use of the quadratic calibration method that converts the EPID images to a surrogate for dose, equivalent thickness of Plastic Water®. Comparisons were made of the 2D thickness maps derived for the different motions compared to the static phantom case and the resulting dose difference analyzed over the “breast” region of interest. A 2D gamma analysis within the same region of interest was performed of the motion images compared to static reference image. Comparisons were made of 1D thickness profiles for the moving and static phantom. The 1D and 2D analyses show the method to be sensitive to the smallest motion amplitude of 0.5 cm tested in the phantom measurements. The results using the phantom demonstrate the method to be a potentially useful tool for monitoring intra‐fraction motion during the delivery of patient radiotherapy treatments as well as more generally providing information on the effects of motion on EPID based in vivo dosimetric verification.

Simple Electronic Portal Imager-Based Pretreatment Quality Assurance using Acuros XB: A Feasibility Study.

Objective:This study demonstrates a novel electronic portal imaging device (EPID)-based forward dosimetry approach for pretreatment quality assurance aided by a treatment planning system (TPS).Materials and Methods:Dynamic multileaf collimator intensity-modulated radiation therapy (IMRT) plans were delivered in EPID and fluence was captured on a beam-by-beam basis (FEPID). An open field having dimensions equal to those of the largest IMRT field was used in the TPS to obtain the transmitted fluence. This represented the patient-specific heterogeneity correction (Fhet). To obtain the resultant heterogeneity-corrected fluence, EPID measured fluence was corrected for the TPS generated heterogeneity (FResultant = FEPID × Fhet). Next, the calculated fluence per beam basis was collected from TPS (FTPS). Finally, FResultant and FTPS were compared using a 3% percentage dose difference (% DD)-3 mm distance to agreement [DTA] gamma analysis in an isocentric plane (two-dimensional [2D]) and multiple planes (quasi three-dimensional [3D]) orthogonal to the beam axis.Results:The 2D heterogeneity-corrected dose reconstruction revealed a mean γ passing of the pelvis, thorax, and head-and-neck cases of 96.3% ±2.0%, 96.3% ±1.8%, and 96.1% ±2.2%, respectively. Quasi-3D γ passing for the pelvis, thorax, and head-and-neck cases were 97.5% ±1.4%, 96.3% ±2.4%, and 97.5% ±1.0%, respectively.Conclusion:EPID dosimetry produced an inferior result due to no heterogeneity corrections for sites such as the lung and esophagus. Incorporating TPS-based heterogeneity correction improved the EPID dosimetry result for those sites with large heterogeneity.
