Improving EEG Forward Modeling Using High-Resolution Five-Layer BEM-FMM Head Models: Effect on Source Reconstruction Accuracy

Electroencephalographic (EEG) source localization is a fundamental tool for clinical diagnoses and brain-computer interfaces. We investigate the impact of model complexity on reconstruction accuracy by comparing the widely used three-layer boundary element method (BEM) as an inverse method against a five-layer BEM accelerated by the fast multipole method (BEM-FMM) and coupled with adaptive mesh refinement (AMR) as forward solver. Modern BEM-FMM with AMR can solve high-resolution multi-tissue models efficiently and accurately. We generated noiseless 256-channel EEG data from 15 subjects in the Connectome Young Adult dataset, using four anatomically relevant dipole positions, three conductivity sets, and two head segmentations; we mapped localization errors across the entire grey matter from 4,000 dipole positions. The average location error among our four selected dipoles is ∼5mm (±2mm) with an orientation error of ∼12∘ (±7∘). The average source localization error across the entire grey matter is ∼9mm (±4mm), with a tendency for smaller errors on the occipital lobe. Our findings indicate that while three-layer models are robust under noiseless conditions, substantial localization errors (10–20mm) are common. Therefore, models of five or more layers may be needed for accurate source reconstruction in critical applications involving noisy EEG data.

Electroencephalographic source imaging of spikes with concurrent high-frequency oscillations is concordant with the clinical ground truth.

Epilepsy raises critical challenges to accurately localize the epileptogenic zone (EZ) to guide presurgical planning. Previous research has suggested that interictal spikes overlapping with high-frequency oscillations, referred to here as pSpikes, serve as a reliable biomarker for EZ estimation, but there remains a question as to whether and to how pSpikes perform as compared to other types of epileptic spikes. This study aims to address this question by investigating the source imaging capabilities of pSpikes alongside other spike types. A total of 2819 interictal spikes from 76-channel scalp electroencephalography (EEG) were analyzed in a cohort of 24 drug-resistant focal epilepsy patients. All patients received surgical resection, and 16 were declared seizure-free based on at least 1 year of postoperative follow-up. A recently developed electrophysiological source imaging algorithm-fast spatiotemporal iteratively reweighted edge sparsity (FAST-IRES)-was used for source imaging of the detected interictal spikes. The performance of 217 pSpikes was compared with 772 nSpikes (spikes with irregular high-frequency activations), 1830 rSpikes (spikes with no high-frequency activity), and all 2819 aSpikes (all interictal spikes). The localization and extent estimation using pSpikes are concordant with the clinical ground truth; using pSpikes yields the best performance compared with nSpikes, rSpikes, and conventional spike imaging (aSpikes). For multiple spike type seizure-free patients, the mean localization error for pSpike imaging was 6.8 mm, compared with 15.0 mm for aSpikes. The sensitivity, precision, and specificity were .41, .67, and .93 for pSpikes compared with .32, .48, and .93 for aSpikes. These results demonstrate the merits of noninvasive EEG source localization, and that (1) pSpike is a superior biomarker, outperforming conventional spike imaging for the localization of epileptic sources, and especially those with multiple irritative zones; and (2) FAST-IRES provides accurate source estimation that is highly concordant with clinical ground truth, even insituations of single spike analysis with low signal-to-noise ratio.

Integrating Electroencephalography Source Localization and Residual Convolutional Neural Network for Advanced Stroke Rehabilitation.

Motor impairments caused by stroke significantly affect daily activities and reduce quality of life, highlighting the need for effective rehabilitation strategies. This study presents a novel approach to classifying motor tasks using EEG data from acute stroke patients, focusing on left-hand motor imagery, right-hand motor imagery, and rest states. By using advanced source localization techniques, such as Minimum Norm Estimation (MNE), dipole fitting, and beamforming, integrated with a customized Residual Convolutional Neural Network (ResNetCNN) architecture, we achieved superior spatial pattern recognition in EEG data. Our approach yielded classification accuracies of 91.03% with dipole fitting, 89.07% with MNE, and 87.17% with beamforming, markedly surpassing the 55.57% to 72.21% range of traditional sensor domain methods. These results highlight the efficacy of transitioning from sensor to source domain in capturing precise brain activity. The enhanced accuracy and reliability of our method hold significant potential for advancing brain-computer interfaces (BCIs) in neurorehabilitation. This study emphasizes the importance of using advanced EEG classification techniques to provide clinicians with precise tools for developing individualized therapy plans, potentially leading to substantial improvements in motor function recovery and overall patient outcomes. Future work will focus on integrating these techniques into practical BCI systems and assessing their long-term impact on stroke rehabilitation.

Modulation of Posterior Default Mode Network Activity During Interoceptive Attention and Relation to Mindfulness

BackgroundInteroceptive attention to internal sensory signals, such as the breath, is fundamental to mindfulness. However, interoceptive attention can be difficult to study, with many studies relying on subjective and retrospective measures. Response consistency is an established method for evaluating variability of attention on exteroceptive attention tasks, but it has rarely been applied to interoceptive attention tasks. MethodsIn this study, we measured consistency of response times on a breath-monitoring task with simultaneous electroencephalography in individuals across the life span (15–91 years of age, N = 324). ResultsWe found that consistency on the breath-monitoring task was positively correlated with attentive performance on an exteroceptive inhibitory control task. Electroencephalography source reconstruction showed that on-task alpha band (8–12 Hz) activity was greater than that measured at rest. Low-consistency/longer breath responses were associated with elevated brain activity compared with high-consistency responses, particularly in posterior default mode network (pDMN) brain regions. pDMN activity was inversely linked with functional connectivity to the frontoparietal network and the cingulo-opercular network on task but not at rest, suggesting a role for these frontal networks in on-task regulation of pDMN activity. pDMN activity within the precuneus region was greater in participants who reported low subjective mindfulness and was adaptively modulated by task difficulty in an independent experiment. ConclusionsElevated pDMN alpha activity serves as an objective neural marker for low-consistency responding during interoceptive breath attention, scales with task difficulty, and is associated with low subjective mindfulness.

Stereo electroencephalography-guided radiofrequency ablation in focal epilepsia partialis continua: illustrative case.

Epilepsia partialis continua (EPC) is a variant of focal motor status epilepticus that can occur as a single or repetitive episode with progressive or nonprogressive characteristics. The authors describe the feasibility of identifying focal EPC in a 33-year-old woman using video electroencephalography (VEEG), electroencephalography source localization, [18F]fluorodeoxyglucose positron emission tomography, magnetic resonance imaging, and psychiatric and neuropsychological assessments and of treating it with stereo electroencephalography-guided radiofrequency (SEEG-RF) ablation. EPC comprised recurrent myoclonus of the right thigh and iliopsoas with a progressive pain syndrome after left anterior-temporo-mesial resection. Switching between VEEG under regular and epidural block helped to define myoclonus as the presenting ictal symptom with a suspected seizure onset zone in the left parietal paramedian lobule. After the epileptic network was identified, SEEG-RF ablation abolished all seizures. No correlation was found between pain and VEEG/SEEG abnormalities. Rehabilitation began 3 days after the SEEG-RF ablation. By 1 year of follow-up, the patient had no EPC and could walk with assistance in rehabilitation; however, due to the abrupt abolishment of EPC and underlying psychological factors, the patient perceived her pain as overriding, which prevented her from walking. The application of SEEG-RF ablation is an efficient therapeutic option for focal EPC with special concerns regarding concurrent nonepileptic pain. https://thejns.org/doi/10.3171/CASE23611.

Transcranial focused ultrasound to V5 enhances human visual motion brain-computer interface by modulating feature-based attention

A brain-computer interface (BCI) enables users to control devices with their minds. Despite advancements, non-invasive BCIs still exhibit high error rates, prompting investigation into the potential reduction through concurrent targeted neuromodulation. Transcranial focused ultrasound (tFUS) is an emerging non-invasive neuromodulation technology with high spatiotemporal precision. This study examines whether tFUS neuromodulation can improve BCI outcomes, and explores the underlying mechanism of action using high-density electroencephalography (EEG) source imaging (ESI). As a result, V5-targeted tFUS significantly reduced the error in a BCI speller task. Source analyses revealed a significantly increase in theta and alpha activities in the tFUS condition at both V5 and downstream in the dorsal visual processing pathway. Correlation analysis indicated that the connection within the dorsal processing pathway was preserved during tFUS stimulation, while the ventral connection was weakened. These findings suggest that V5-targeted tFUS enhances feature-based attention to visual motion.

348 Alexithymia impacts vulnerability for cognitive decline in healthy elders via frontal lobe connectivity during response inhibition, especially in women

OBJECTIVES/GOALS: This project aimed to examine the impacts of biological sex and alexithymia on frontal lobe connectivity in executive functioning (EF)-related neural networks during successful inhibition as a means to index vulnerability for future cognitive decline. METHODS/STUDY POPULATION: Healthy, cognitively intact older adults (n=43, 33 female, Mage=79) completed the 20-item Toronto Alexithymia Scale (TAS-20) and the stop-signal task in this study. We used electroencephalography (EEG) source estimation to investigate EF-related frontal connectivity during successful inhibition in stop-signal task trials. Connectivity was measured in bilateral frontal ROIs relevant to inhibition using time series correlations over the N200 (186-350ms) and P300 (340-616ms) time windows, associated with the inhibitory subprocesses of conflict processing and performance evaluation, respectively. RESULTS/ANTICIPATED RESULTS: Those with higher alexithymia, as measured by the difficulty identifying feelings (DIF) facet of the TAS-20, had lower right anterior cingulate cortex (ACC)-left superior frontal gyrus (SFG) connectivity in the P300 window, suggesting impaired performance evaluation. Further, in females specifically, those with higher DIF had greater right inferior frontal gyrus (rIFG)-bilateral ACC connectivity in the N200 window than those with lower DIF, suggesting greater resources were allocated for conflict processing and inhibition. Right ACC-rIFG connectivity also correlated with better stop accuracy and faster stop-signal reaction time, supporting this network’s role in successful inhibition. DISCUSSION/SIGNIFICANCE: Overall, during successful inhibition, higher DIF was associated with reduced performance monitoring efficiency as well as greater resource allocation for conflict processing during motor stopping in women only. Thus, alexithymia (via DIF) may exacerbate age-related EF dysfunction and risk for future cognitive decline, especially for females.

VSSI-GGD: A Variation Sparse EEG Source Imaging Approach Based on Generalized Gaussian Distribution.

Electroencephalographic (EEG) source imaging (ESI) is a powerful method for studying brain functions and surgical resection of epileptic foci. However, accurately estimating the location and extent of brain sources remains challenging due to noise and background interference in EEG signals. To reconstruct extended brain sources, we propose a new ESI method called Variation Sparse Source Imaging based on Generalized Gaussian Distribution (VSSI-GGD). VSSI-GGD uses the generalized Gaussian prior as a sparse constraint on the spatial variation domain and embeds it into the Bayesian framework for source estimation. Using a variational technique, we approximate the intractable true posterior with a Gaussian density. Through convex analysis, the Bayesian inference problem is transformed entirely into a series of regularized L2p -norm ( ) optimization problems, which are efficiently solved with the ADMM algorithm. Imaging results of numerical simulations and human experimental dataset analysis reveal the superior performance of VSSI-GGD, which provides higher spatial resolution with clear boundaries compared to benchmark algorithms. VSSI-GGD can potentially serve as an effective and robust spatiotemporal EEG source imaging method. The source code of VSSI-GGD is available at https://github.com/Mashirops/VSSI-GGD.git.

Quantifying active brain areas at spatial hearing process using Electroencephalography (EEG) source localization approach

The human ability for spatial hearing encourages the development of spatial audio technology to support many human activities. Spatial audio mimics the real-life sound and provides a more immersive hearing experience as if the listener were present in the environment where the sound source was recorded. Recently, research on how spatial hearing processes are encoded in the brain has begun to be developed. As a modality for brain activity measurement that is non-invasive and has a high temporal resolution, Electroencephalography (EEG) is suitable for studying brain responses to moving sound stimuli. This study compared brain activity in hearing stereo and spatial audio stimulus. Through measurements using 21 EEG electrodes on the scalp, the results showed an increase in the mean PSD for all Theta, Alpha, and Beta waves in the spatial audio stimulus compared to stereo audio. The most significant increase in the mean PSD occurred in the Beta wave of 93.8008 µV2/Hz. Furthermore, the source localization approach with Independent Component Analysis (ICA) and Low-resolution Brain Electromagnetic Tomography (LORETA) method was used to estimate and quantify the active brain area of this process. The results showed an activation of the non-auditory cortex when the subject was hearing spatial audio stimulus. By using Paired T-Test of current density for both hearing processes, the results showed there was no significant difference (p > 0.05) in Brodmann area (BA) 41 (Primary Auditory Cortex) and BA 42 (Secondary Auditory Cortex). Meanwhile, for the spatial audio hearing process, there was a significant difference (p < 0.05) in BA 6 (Premotor Cortex), which is related to spatial orientation, and BA 9 (Dorsolateral Prefrontal Cortex), which is associated with executive functions, including working memory and selective attention. This study offers potential insights into spatial hearing research and immersive audio production.

Solution of the EEG inverse problem by random dipole sampling

Electroencephalography (EEG) source imaging aims to reconstruct brain activity maps from the neuroelectric potential difference measured on the skull. To obtain the brain activity map, we need to solve an ill-posed and ill-conditioned inverse problem that requires regularization techniques to make the solution viable. When dealing with real-time applications, dimensionality reduction techniques can be used to reduce the computational load required to evaluate the numerical solution of the EEG inverse problem. To this end, in this paper we use the random dipole sampling method, in which a Monte Carlo technique is used to reduce the number of neural sources. This is equivalent to reducing the number of the unknowns in the inverse problem and can be seen as a first regularization step. Then, we solve the reduced EEG inverse problem with two popular inversion methods, the weighted Minimum Norm Estimate (wMNE) and the standardized LOw Resolution brain Electromagnetic TomogrAphy (sLORETA). The main result of this paper is the error estimates of the reconstructed activity map obtained with the randomized version of wMNE and sLORETA. Numerical experiments on synthetic EEG data demonstrate the effectiveness of the random dipole sampling method.

EEG electrode localization with 3D iPhone scanning using point-cloud electrode selection (PC-ES)

Objective. Electroencephalography source imaging (ESI) is a valuable tool in clinical evaluation for epilepsy patients but is underutilized in part due to sensitivity to anatomical modeling errors. Accurate localization of scalp electrodes is instrumental to ESI, but existing localization devices are expensive and not portable. As a result, electrode localization challenges further impede access to ESI, particularly in inpatient and intensive care settings. Approach. To address this challenge, we present a portable and affordable electrode digitization method using the 3D scanning feature in modern iPhone models. This technique combines iPhone scanning with semi-automated image processing using point-cloud electrode selection (PC-ES), a custom MATLAB desktop application. We compare iPhone electrode localization to state-of-the-art photogrammetry technology in a human study with over 6000 electrodes labeled using each method. We also characterize the performance of PC-ES with respect to head location and examine the relative impact of different algorithm parameters. Main Results. The median electrode position variation across reviewers was 1.50 mm for PC-ES scanning and 0.53 mm for photogrammetry, and the average median distance between PC-ES and photogrammetry electrodes was 3.4 mm. These metrics demonstrate comparable performance of iPhone/PC-ES scanning to currently available technology and sufficient accuracy for ESI. Significance. Low cost, portable electrode localization using iPhone scanning removes barriers to ESI in inpatient, outpatient, and remote care settings. While PC-ES has current limitations in user bias and processing time, we anticipate these will improve with software automation techniques as well as future developments in iPhone 3D scanning technology.

Multi-compartment head modeling in EEG: Unstructured boundary-fitted tetra meshing with subcortical structures.

This paper introduces an automated approach for generating a finite element (FE) discretization of a multi-compartment human head model for electroencephalographic (EEG) source localization. We aim to provide an adaptable FE mesh generation tool for EEG studies. Our technique relies on recursive solid angle labeling of a surface segmentation coupled with smoothing, refinement, inflation, and optimization procedures to enhance the mesh quality. In this study, we performed numerical meshing experiments with the three-layer Ary sphere and a magnetic resonance imaging (MRI)-based multi-compartment head segmentation which incorporates a comprehensive set of subcortical brain structures. These experiments are motivated, on one hand, by the sensitivity of non-invasive subcortical source localization to modeling errors and, on the other hand, by the present lack of open EEG software pipelines to discretize all these structures. Our approach was found to successfully produce an unstructured and boundary-fitted tetrahedral mesh with a sub-one-millimeter fitting error, providing the desired accuracy for the three-dimensional anatomical details, EEG lead field matrix, and source localization. The mesh generator applied in this study has been implemented in the open MATLAB-based Zeffiro Interface toolbox for forward and inverse processing in EEG and it allows for graphics processing unit acceleration.

Cerebellar EEG source localization reveals age-related compensatory activity moderated by genetic risk for Alzheimer's disease.

The apolipoprotein-E (APOE) ε4 allele is the greatest genetic risk factor for late-onset Alzheimer's disease (AD), but alone it is not sufficiently predictive. Because neuropathological changes associated with AD begin decades before cognitive symptoms, neuroimaging of healthy, cognitively intact ε4 carriers (ε4+) may enable early characterization of patterns associated with risk for future decline. Research in the cerebral cortex highlights a period of compensatory recruitment in elders and ε4+, which serves to maintain cognitive functioning. Yet, AD-related changes may occur even earlier in the cerebellum. Advances in electroencephalography (EEG) source localization now allow effective modeling of cerebellar activity. Importantly, healthy aging and AD are associated with declines in both cerebellar functions and executive functioning (EF). However, it is not known whether cerebellar activity can detect pre-symptomatic AD risk. Thus, the current study analyzed cerebellar EEG source localization during an EF-dependent stop-signal task (i.e., inhibitory control) in healthy, intact older adults (Mage = 80 years; 20 ε4+, 25 ε4-). Task performance was comparable between groups. Older age predicted greater activity in left crus II and lobule VIIb during the P300 window (i.e., performance evaluation), consistent with age-related compensation. Age*ε4 moderations specifically showed that compensatory patterns were evident only in ε4-, suggesting that cerebellar compensatory resources may already be depleted in healthy ε4+ elders. Thus, the posterolateral cerebellum is sensitive to AD-related neural deficits in healthy elders. Characterization of these patterns may be essential for the earliest possible detection of AD risk, which would enable critical early intervention prior to symptom onset.

Electrophysiological correlates of distance and direction processing during cognitive map retrieval: A source analysis.

The cognitive map is an internal representation of the environment and allows us to navigate through familiar environments. It preserves the distances and directions between landmarks which help us orient ourselves in our surroundings. The aim of our task was to understand the role played by theta waves in the cognitive map and especially how the cognitive map is recalled and how the manipulation of distances and directions occurs within the cognitive map. In order to investigate the neural correlates of the cognitive map, we used the Cognitive Map Recall Test, in which 33 participants had to estimate distances and directions between familiar landmarks tailored to their own knowledge. We examined the role of theta waves in the cognitive map, as well as the brain regions that generated them. To that aim, we performed electroencephalographic source imaging while focusing on frequency spectral analysis. We observed increases of theta amplitude in the frontal, temporal, parahippocampal gyri and temporal poles during the recall of the cognitive map. We also found increases of theta amplitude in the temporal pole and retrosplenial cortex during manipulation of directions. Overall, direction processing induces higher theta amplitude than distance processing, especially in the temporal lobe, and higher theta amplitude during recall compared to manipulation, except in the retrosplenial cortex where this pattern was reversed. We reveal the role of theta waves as a marker of directional processing in the retrosplenial cortex and the temporal poles during the manipulation of spatial information. Increases in theta waves in frontal, parahippocampal, temporal and temporal pole regions appear to be markers of working memory and cognitive map recall. Therefore, our Cognitive Map Recall Test could be useful for testing directional difficulties in patients. Our work also shows that there are two distinct parts to the cognitive map test: recall and manipulation of spatial information. This is often considered as two similar processes in the literature, but our work demonstrates that these processes could be different, with theta waves from different brain regions contributing to either recall or manipulation; this should be considered in future studies.

Comparison of EEG Source Localization Using Simplified and Anatomically Accurate Head Models in Younger and Older Adults.

Accuracy of electroencephalography (EEG) source localization relies on the volume conduction head model. A previous analysis of young adults has shown that simplified head models have larger source localization errors when compared with head models based on magnetic resonance images (MRIs). As obtaining individual MRIs may not always be feasible, researchers often use generic head models based on template MRIs. It is unclear how much error would be introduced using template MRI head models in older adults that likely have differences in brain structure compared to young adults. The primary goal of this study was to determine the error caused by using simplified head models without individual-specific MRIs in both younger and older adults. We collected high-density EEG during uneven terrain walking and motor imagery for 15 younger (22±3 years) and 21 older adults (74±5 years) and obtained [Formula: see text]-weighted MRI for each individual. We performed equivalent dipole fitting after independent component analysis to obtain brain source locations using four forward modeling pipelines with increasing complexity. These pipelines included: 1) a generic head model with template electrode positions or 2) digitized electrode positions, 3) individual-specific head models with digitized electrode positions using simplified tissue segmentation, or 4) anatomically accurate segmentation. We found that when compared to the anatomically accurate individual-specific head models, performing dipole fitting with generic head models led to similar source localization discrepancies (up to 2 cm) for younger and older adults. Co-registering digitized electrode locations to the generic head models reduced source localization discrepancies by ∼ 6 mm. Additionally, we found that source depths generally increased with skull conductivity for the representative young adult but not as much for the older adult. Our results can help inform a more accurate interpretation of brain areas in EEG studies when individual MRIs are unavailable.

Electroencephalography source localization.

Electroencephalography (EEG) has been and is still widely used in brain function research. EEG has advantages over other neuroimaging modalities. First, it not only directly images the electrical activity of neurons; it has a higher temporal resolution. Furthermore, current advanced technologies enable accurate mathematical calculations and sophisticated localization from EEG data. Several important factors should be considered for EEG analysis using these advanced technologies. First, raw EEG data contain physiological or nonphysiological artifacts. Therefore, preprocessing methods and algorithms to detect and remove these artifacts have been proposed and developed. In the analysis of preprocessed EEG, forward and inverse problems require solving and several proposed models have been applied. To solve the forward problem, the source information and matrix parameters from which the EEG originates are essential. Therefore, an accurate head model is required. In contrast, the possible combinations of the current sources computed inversely from EEG measured at a limited number of electrodes are infinite, referring to the inverse problem. The inverse problem can be solved by setting limits based on assumptions made of the anatomy and physiology on the generation and propagation of the current sources. Thus, methods such as dipole source models and distributed source models have been proposed. Source localization requires the consideration of many factors such as the preprocessing of raw EEG data, artifact removal, accurate head models and forward problems, and inverse computation problems. This review summarizes the methods and considerations applied to the above EEG source localization process. It also introduces the applications of EEG source localization for epilepsy and other diseases as well as brain function studies and discusses future directions.

Can individually targeted and optimized multi-channel tDCS outperform standard bipolar tDCS in stimulating the primary somatosensory cortex?

BackgroundTranscranial direct current stimulation (tDCS) has emerged as a non-invasive neuro-modulation technique. Most studies show that anodal tDCS increases cortical excitability, however, with variable outcomes. Previously, we have shown in computer simulations that our multi-channel tDCS (mc-tDCS) approach, the distributed constrained maximum intensity (D-CMI) method can potentially lead to better controlled tDCS results due to the improved directionality of the injected current at the target side for individually optimized D-CMI montages. ObjectiveIn this study, we test the application of the D-CMI approach in an experimental study to stimulate the somatosensory P20/N20 target source in Brodmann area 3b and compare it with standard bipolar tDCS and sham conditions. MethodsWe applied anodal D-CMI, the standard bipolar and D-CMI based Sham tDCS for 10 min to target the 20 ms post-stimulus somatosensory P20/N20 target brain source in Brodmann area 3b reconstructed using combined magnetoencephalography (MEG) and electroencephalography (EEG) source analysis in realistic head models with calibrated skull conductivity in a group-study with 13 subjects. Finger-stimulated somatosensory evoked fields (SEF) were recorded and the component at 20 ms post-stimulus (M20) was analyzed before and after the application of the three tDCS conditions in order to read out the stimulation effect on Brodmann area 3b. ResultsAnalysis of the finger stimulated SEF M20 peak before (baseline) and after tDCS shows a significant increase in source amplitude in Brodmann area 3b for D-CMI (6–16 min after tDCS), while no significant effects are found for standard bipolar (6–16 min after tDCS) and sham (6–16 min after tDCS) stimulation conditions. For the later time courses (16–26 and 27–37 min post-stimulation), we found a significant decrease in M20 peak source amplitude for standard bipolar and sham tDCS, while there was no effect for D-CMI. ConclusionOur results indicate that targeted and optimized, and thereby highly individualized, mc-tDCS can outperform standard bipolar stimulation and lead to better control over stimulation outcomes with, however, a considerable amount of additional work compared to standard bipolar tDCS.

From theory to practical fundamentals of electroencephalographic source imaging in localizing the epileptogenic zone.

With continued advancement in computational technologies, the analysis of electroencephalography (EEG) has shifted from pure visual analysis to a noninvasive computational technique called EEG source imaging (ESI), which involves mathematical modeling of dipolar and distributed sources of a given scalp EEG pattern. ESI is a noninvasive phase I test for presurgical localization of the seizure onset zone in focal epilepsy. It is a relatively inexpensive modality, as it leverages scalp EEG and magnetic resonance imaging (MRI) data already collected typically during presurgical evaluation. With an adequate number of electrodes and combined with patient-specific MRI-based head models, ESI has proven to be a valuable and accurate clinical diagnostic tool for localizing the epileptogenic zone. Despite its advantages, however, ESI is routinely used at only a minority of epilepsy centers. This paper reviews the current evidence and practical fundamentals for using ESI of interictal and ictal epileptic activity during the presurgical evaluation of drug-resistant patients. We identify common errors in processing and interpreting ESI studies, describe the differences in approach needed for localizing interictal and ictal EEG discharges through practical examples, and describe best practices for optimizing the diagnostic information available from these studies.
