Prevention and the treatment of oral mucositis: the efficacy of sodium bicarbonate vs other agents: a systematic review

IntroductionOral mucositis (OM) is a major side effect of cancer therapy, which is associated with significant symptoms, treatment delays and increased costs for the health system. It is an important component of the quality of life of cancer patients and, until now, there has been no gold standard regarding prevention or treatment of this pathology. Notwithstanding the paucity of treatment guidelines (due to limited evidence from high-quality, rigorous studies), sodium bicarbonate (SB) rinses are one of the most used agents for OM management.ObjectivesA systematic review (2000–2022) was performed in order to compare and examine different agents versus sodium bicarbonate (SB) in preventing or treating OM.SourcesEleven randomized controlled trials (RCT) were evaluated: four were conducted for the prevention and seven for the management of OM. The risk of bias of RCTs was assessed using the revised Cochrane risk of bias tool for randomized trials.Study selectionAccording to the RoB2 evaluation for randomized trials, four RCTs were judged to be at a high risk of bias, two were rated as ‘problematic’, while five were deemed to be a low risk of bias.ConclusionsThe results revealed that there was no evidence for supporting SB in OM treatment regarding management and prevention.Clinical significanceResults showed in this review takes on a strategic importance in the use of SB for OM management or prevention; indiscriminate use of SB could be counterproductive because it causes a sudden pH increase and it delays proper OM pharmacological treatment.

A Comparative Study of Sodium Bicarbonate and Hyaluronidase on Pain Perception, Anesthesia, and Akinesia during Peribulbar Anesthesia for Cataract Surgery

Background:An ideal anesthetic solution should provide good anesthesia and akinesia with minimal pain on injection.Aims:The aim of this study is to determine the effect on pain perception and efficacy of sodium bicarbonate over hyaluronidase in the local anesthetic mixture during peribulbar anesthesia.Settings and Design:A prospective, randomized, double-blind study.Materials and Methods:An independent observer labeled two injections as A (hyaluronidase 1500 IU in 30 mL of lignocaine) and B (7.5% sodium bicarbonate 1 mL in 30 mL of lignocaine). Group 1 was injected with injection A while Group 2 was injected with injection B. The visual analog scale (VAS) was used to determine the intensity of pain. Onset and degree of anesthesia and akinesia were recorded.Statistical Analysis:Computer software Microsoft Excel SPSS version 26 (Chicago Inc) for windows was used. The qualitative data and quantitative data were reported as proportions and mean ± (standard deviation), respectively. Chi-square test for proportions was used for the comparison of qualitative variables and unpaired Student's t-test was used to test the significance between quantitative variables. P < 0.05 was considered statistically significant. All P were two-tailed.Results:Out of 123 patients, 23 were excluded from the study. Hundred patients were divided into Group 1 and Group 2. The mean age in Group 1 was 64.92 ± 10.77 years while in Group 2 was 62.86 ± 11.17 years. The mean heart rate and mean systolic blood pressure in both groups were statistically insignificant. Group 2 experienced very less pain (mean pain score VAS = 5.12 ± 1.17) as compared to Group 1 (mean pain score was 7.16 ± 1.09) and the difference between both the groups was found to be statistically significant. There was a significant difference in the onset of anesthesia in both groups (P = 0.001). In the sodium bicarbonate group, the onset was faster. The onset of akinesia was better in Group 1 (4.76 ± 2.06 min). Grading of akinesia was better in Group 1.Conclusion:Sodium bicarbonate reduces pain on injection in peribulbar anesthesia and also results in a quicker onset of anesthesia.

The Effect of Sodium Bicarbonate Alone or in Combination with Reduced Dose of Prochloraz Manganese on the Control of Wet Bubble of Agaricus bisporus

ABSTRACTIn vitro experiments showed that sodium bicarbonate (SBC) was effective in inhibiting the growth of Mycogone perniciosa. The inhibition of radial growth, germination and germ tube elongation of chlamydospores was 100.0%, 73.9%, 79.1%, respectively, at 4% SBC. Complete inhibition of the germination and germ tube elongation of conidia was achieved by 4% SBC. Seven experiments were conducted to test the in vivo efficacy of sodium bicarbonate and reduced dose of prochloraz manganese either alone or in combination. Treatments were applied to casing soil twice by drenching 2 h before casing and 2 days after casing. In seven in vivo experiments, the effect of SBC in inhibiting M. perniciosa was inconsistent. The efficacy of the combination of SBC with reduced dose (10% of label dose) of prochloraz mangenese were not significantly and consistently different from that of the label dose of prochloraz manganese in inhibiting the disease and increasing the yield in three repeated experiments. Similarly, in the bags treated with the combination of sodium bicarbonate and reduced dose of prochloraz manganese, the amount of sclerodermoid masses were reduced to 309 g from 1048 g in the control. Sodium bicarbonate treatment neither impaired quality of sporophores nor reduced the yield.

The effect of sodium bicarbonate alone or in combination with a reduced rate of mancozeb on the control of leaf rust [Puccinia triticina] in wheat

In vitro experiments showed that sodium bicarbonate (SBC) was effective in inhibiting urediniospore germination and germ-tube elongation of Puccinia triticina, the causal agent of leaf rust on wheat. In vitro efficacy of SBC at 0.006 and 0.012 mol/L in inhibiting the spore germination was 64.1% and 100%, respectively. Two field experiments were conducted to investigate the efficacy of SBC alone or in combination with reduced rates of mancozeb in inhibiting leaf rust on wheat (Triticum aestivum). The results showed that the application of SBC at 0.12 or 0.24 mol/L, sprayed twice at 2-week intervals, significantly reduced the disease severity compared with a water-treated control in two experiments. In the first field experiment, the SBC treatment at 0.12 mol/L reduced disease severity from 11.4% to 2.2% that of the water-treated control. The efficacy of SBC was comparable with that of tebuconazole. Combining SBC with a reduced rate of mancozeb (25% of label rate) did not improve the efficacy of the SBC treatment in the first field experiment. In pot experiments conducted under controlled conditions, SBC at 0.12 and 0.24 mol/L significantly reduced disease severity. The efficacy of SBC at 0.24 mol/L was higher than that of the 0.12 mol/L treatment and caused no adverse effect on quantity and quality of wheat.

Control of Postharvest Diseases of Organically Grown Strawberry with Preharvest Applications of some Food Additives and Postharvest Hot Water Dips

AbstractEthanol at 50% (v/v) and sodium bicarbonate at 1% (wt/v), either alone or in combination, were applied to organically grown strawberries 1 h before harvest to control the natural incidence of postharvest diseases. Botrytis cinerea was the major cause of decay in all of the experiments. In three experiments, ethanol significantly reduced the decay incidence after storage for 3 days at 1°C followed by 2 days at 24°C, while the efficacy of sodium bicarbonate was inconsistent. The combination of ethanol and sodium bicarbonate did not increase their efficacy. Postharvest hot water dips at 55 and 60°C for 30 s significantly reduced the decay incidence to 3.4 and 2.7%, respectively, while decay incidence in the control was 28.5% (the first experiment). The efficacy of the hot water treatments at 55 and 60°C for 30 s was consistent in three experiments. In the third experiment, the efficacy of hot water treatment at 60°C was significantly higher than that of hot water treatment at 55°C. All pre‐ and postharvest treatments significantly reduced natural fungal populations on the surfaces of fruits. None of the pre‐ and postharvest treatments caused surface injuries to the fruit or adversely affected weight loss and taste parameters.
