Efficacy of different Er:YAG laser-activated photoacoustic streaming modes compared to passive ultrasonic irrigation in the retreatment of curved root canals.

To evaluate the efficacy of passive ultrasonic irrigation (PUI) and super short pulse (SSP) and shock wave-enhanced emission photoacoustic streaming (SWEEPS) modes of Er:YAG laser-activated irrigation (LAI) with two different laser tips, in removing filling remnants after conventional retreatment in severely curved root canals. The study sample consisted of 40 extracted molars with curved mesiobuccal root canals. The canals were instrumented with ProTaper Next and filled with an epoxy resin-based sealer and gutta-percha using continuous wave vertical compaction and warm injection back-filling. After retreatment with ProTaper Universal Retreatment system and sodium hypochlorite (NaOCl), all samples were randomly divided into four groups (n = 10) according to the final irrigation technique: PUI, LAI/SSP, SWEEPS/flat-tip, and SWEEPS/radial-tip, using 6mL of 3% NaOCl for an activation time of 3 × 30s. The samples were subjected to micro-CT scans after root canal filling, retreatment, and final irrigation. The filling material volume and percentage reduction were calculated. All tested irrigation techniques were successful in the elimination of the filling remnants after the retreatment (p < 0.001). The LAI/SSP group showed a higher reduction rate than the SWEEPS/flat-tip group (p = 0.032). No significant differences were found between the other groups (p > 0.05). All tested techniques improved the removal of filling material during retreatment in curved canals. LAI/SSP showed slightly better results than other techniques. The study highlights the need for additional activated irrigation after the retreatment of curved root canals. The choice between PUI and LAI is not decisive for success.

SEM study of the effect of passive ultrasonic irrigation on the cleanliness of dentinal tubules in endodontic retreatment

Aim and Objective: Endodontic retreatment is indicated in the event of a failure of initial treatment of the root canal and of an initial treatment that looks to be correctable with retreatment. This in-vitro study was designed to assess the efficacy of passive ultrasonic irrigation in endodontic retreatment, by employing scanning electron microscopes, on the cleanliness of dentine tubules with and without solvent in curved root channels. Material and method: Sixty human maxillary molar teeth were treated endontically. The protaper rotary instrument was used in root canals in the crown down procedure under copious 3% NaOCl (2 ml) and Glyde irrigation. The saline solution (1 ml) was used for final rinsing. The 10 sample root surfaces of each group were evaluated for SEM pictures. Results: Amongst group I and II here were more open dentinal tubules that were statistically significant (p&lt;0.05) in Group II without solvent group. Group IV demonstrated larger, statistically significant open dentinal tubules between group III and IV. The most dentinal open tubules were found in Group IV among the four groups.

Passive ultrasonic irrigation, EndoActivator system and XP-endo Finisher R as additional cleaning techniques to remove residual filling materials from flattened root canals.

Background:The effectiveness of endodontic retreatment essentially depends on the cleaning and/or disinfection processes. In this context, the removal of root canal filling materials plays a crucial role.Aims:To assess the efficacy of passive ultrasonic irrigation (PUI), EndoActivator system (EAS), and XP-endo Finisher R (XPEFR) as additional cleaning techniques to remove the remaining root canal filling materials from flattened root canals.Subjects and Methods:Thirty-six similar flattened distal root canals of extracted human first lower molars were selected by micro-computed tomography (micro-CT) and then instrumented and filled. After the initial retreatment procedures, the residual volume of root canal filling materials was assessed by micro-CT (V1). Then, the specimens were divided into three groups (n. 12), according to the additional cleaning technique and submitted to another micro-CT scan (V2).Statistical Analysis Used:Analysis of variance and Games-Howell tests (P < 0.05).Results:The percentage reduction in the residual volume of root canal filling materials reached by PUI, EAS, and XPEFR was 28.38%, 28.12%, and 43.52%, respectively, considering the total space of the root canal (P > 0.05). In the apical third, these values were 20.05%, 21.54%, and 48.82% (P < 0.05).Conclusions:Additional cleaning techniques enabled removing a greater amount of root canal filling material from flattened distal root canals of extracted human first lower molars. Considering the total space of the root canal, there were no statistically relevant differences among the groups. In the apical third, XPEFR performed better.

Effect of Passive Ultrasonic Irrigation over Organic Tissue of Simulated Internal Root Resorption.

This study aimed to evaluate the efficacy of passive ultrasonic irrigation (PUI) on dissolving the organic tissue inside simulated internal root resorption (IRR) using sodium hypochlorite (NaOCl) or chlorhexidine (CHX). A total of 40 human lower premolars were collected based on dimensional and morphological similarities. The roots were embedded in cylinders (3 cm diameter; 2.5 cm height) of self-cured acrylic resin, and then an IRR was simulated. The specimens were divided into 4 groups (n = 10) according to irrigation protocols: group 1: CHX + PUI; group 2: CHX; group 3: NaOCl + PUI; group 4: NaOCl. The total irrigation time was 150 s at a flow rate of 5 mL/min. A tissue mass of porcine palatine mucosa was used to simulate the organic tissue, it was weighed before and after the irrigation using an analytic balance, and the difference between both readings was calculated and transferred to percentage values. Data were submitted to statistical analysis using two-way ANOVA (factors: irrigant type and with/without PUI) and Tukey's test for multiple comparisons among the experimental groups (α = 0.05). There was a significant difference in both factors (irrigant: p=0.04; PUI: p ≤ 0.001). The groups that used PUI were more effective in dissolving the organic tissue of the IRR simulation than the groups without PUI. PUI is more effective than the syringe and needle irrigation in organic tissue dissolution.

Background: This study aimed to evaluate the efficacy of passive ultrasonic irrigation and XP-Endo Finisher on the cleanliness of canal wall surface after post-space preparation. Methods: Forty endodontically treated mandibular premolar teeth were used. After post-space preparation, the teeth were divided into four groups. Irrigation protocols were performed as follows; distilled water as a control group (Group 1), NaOCl/EDTA without any activation (Group 2), NaOCl/EDTA irrigation with passive ultrasonic activation (Group 3), and NaOCl/EDTA irrigation with XP-Endo Finisher (Group 4). The teeth were split and the canal walls were examined with scanning electron microscope under 1000× magnification. The presence of debris was evaluated with 3-point scoring system. For each sample 10 scores were obtained and averaged. The results were statistically analyzed. Results: None of the techniques completely cleaned the canal walls after post-space preparation. Passive ultrasonic irrigation and XP-Endo Finisher groups demonstrated lower scores than the control and non-activated NaOCl/EDTA groups. The highest results were recorded in the control group. Conclusion: The activation with passive ultrasonic irrigation or XP-Endo Finisher was more effective in cleaning of the root canal walls after post-space preparation.

Comparison of Anti-Bacterial Efficacy between Passive Ultrasonic Irrigation and 980 nm-GaAlAs Laser Application in Two Root Types.

Background and Objectives: The purpose of the present study was to investigate the anti-bacterial efficacy of passive ultrasonic irrigation (PUI) and a 980-nm-gallium-aluminum-arsenide (GaAlAs) laser using a real-time DNA-based quantitative polymerase chain reaction (qPCR) assay and scanning electron microscopy (SEM). Materials and Methods: Eighty-six extracted single- and double-rooted human teeth were used in the experiment. The following four experimental groups were evaluated, as well as a control group: group 1: single root + PUI (n = 20); group 2: single root + laser application (n = 20); group 3: double roots + PUI (n = 20); group 4: double roots + laser application (n =20); control group (n = 6): 3 single roots, 3 double roots. The qPCR assay was performed in three stages to evaluate the efficacy of the adjunctive method against Enterococcus faecalis. SEM analysis was also used to examine the microstructure of root canal surfaces. The data were statistically analyzed using the Friedman test and the Kruskal–Wallis test with Bonferroni adjustment. Results: The decrease in the DNA levels from S1 (before preparation) to S2 (just after preparation) was highly significant in all groups, and decreases in DNA levels from S2 to S3 (after incubation for 1 week) were found in all experimental groups except group 1. An inter-group analysis showed that PUI was significantly more effective in terms of antibacterial efficacy than GaAlAs laser irradiation in single root (p < 0.05). However, in double roots, experimental groups did not show significantly lower DNA levels than the control group (p > 0.05). SEM images showed that cleaning of the root canal surface and reduction of dentin debris were achieved only in single-rooted teeth when using PUI application. Conclusions: Two adjunctive methods were effective in reducing E. faecalis in single rooted tooth.

Efficacy of passive ultrasonic irrigation in the removal of three different intracanal medicaments - An in-vitro study.

A successful root canal treatment involves the elimination of microorganisms, and prevents them from reinfecting the root and/or peri-radicular tissues. This can be mainly achieved by efficient cleaning and shaping of the root canal system. Due to the complexity of the root canal morphology, hand and rotary files cannot clean areas such as the isthmuses and lateral canals. In such cases, effective irrigation and the use of intracanal medicaments are essential. Medicaments that are applied should be removed before obturation. To evaluate the efficacy of passive ultrasonic irrigation in the removal of three different intracanal medicaments - Triple antibiotic paste (TAP), Odontopaste, and Metapex using a stereomicroscope. A total of 45 single-rooted human teeth were selected and decoronated. The root canals were shaped with ProTaper rotary files. The specimens were fixed in modified Eppendorf vials with silicone material. The roots were split longitudinally, and a standardized groove was prepared in the apical part. The teeth were randomly divided into three groups each containing 15 samples based on the intracanal medicament used: TAP, Odontopaste, and Metapex. The root halves were reassembled with sticky wax and remounted in the Eppendorf vial. Each specimen was irrigated with 3% NaOCl and agitated using a passive ultrasonic device. The root segments were disassembled. Digital images were obtained using a stereomicroscope. The data were evaluated statistically using Kruskal-Wallis and Mann-Whitney U tests. There was a statistically significant difference in the remaining intracanal medicament score (P < 0.05). Odontopaste showed the least remaining amount score, whereas TAP and Metapex showed greater retention within the dentin.

The effectiveness of passive ultrasonic irrigation and the easy-clean instrument for removing remnants of filling material.

Objective:This study aimed to evaluate the efficacy of passive ultrasonic irrigation (PUI) and the easy clean instrument by micro-computed tomography (CT) for removing remnant filling materials during endodontic retreatment.Materials and Methods:Forty mesial roots of mandibular molars were divided into four groups (n = 10) according to the agitation system and sealer used: Group 1: PUI/AH Plus; Group 2: PUI/TotalFill; Group 3: Easy Clean/AH Plus; and Group 4: Easy Clean/TotalFill. The groups were compared by micro-CT analysis according to the volumes of the obturation and the remaining material before and after the agitation systems were used.Results:There was no difference between the groups for total filling volume and initial and final remaining filling volume (P > 0.05).Conclusions:Both the PUI and easy-clean instrument are effective for removing remnants of filling material with no difference between the groups.

Irrigant Agitation Techniques versus Passive Ultrasonic Irrigation for Removing Debris from Curved Root Canals: An Environmental Scanning Electron Microscopic Study.

The aim of this study was to compare the efficacy of passive ultrasonic irrigation (PUI) versus mechanical agitation of the irrigant promoted by the EasyClean (EC) and XP-Endo Finisher (XP-F) systems in removing debris from root canal walls, using environmental scanning electron microscopy (ESEM). Twelve curved mesiobuccal canals of mandibular molars were prepared with the ProTaper Next system up to file X2 (25/0.06). The specimens were embedded in flasks containing silicone putty, sectioned lengthwise, and a 4-mm long groove was made on the canal wall of the buccal half of the specimen, extending from 2 mm up to 6 mm short of the apex. Five cross-sectional markings were made along this groove to establish standardized locations for imaging. The same specimens were used to prepare a negative control group (without debris), a positive control group (completely covered by debris), and 3 experimental groups according to the final irrigation protocol employed: PUI, EC or XP-F. ESEM images were obtained and evaluated by 3 examiners. The amount of debris observed on the images was classified according to a 4-category scoring system. The kappa test was used to assess inter-examiner agreement, and the Kruskal-Wallis and Dunn tests were used to compare the scores (P<0.05). The scores attributed to the PUI, EC, and XP-F groups were statistically similar to those attributed to the negative control group (P>0.05). Based on this in-vitro study, the mechanical agitation of the irrigant promoted by EC and XP-F was as effective as using PUI to remove debris from the root canal walls.

Effect of passive ultrasonic irrigation on hard tissue debris removal: a systematic review and meta-analysis.

Accumulated hard tissue debris (AHTD) in root canal irregularities may negatively impact adequate root canal disinfection. In light of this, the efficacy of passive ultrasonic irrigation (PUI) to reduce AHTD has been largely studied in in vitro studies, which have adopted different analytic methods of varying accuracy to determine the extent of AHTD more correctly. Therefore, the aim of this study was to compare how well PUI and non-activated irrigation (NAI) systems perform in reducing AHTD during final irrigation protocols, based exclusively on studies whose analyses used microCT scanning. A systematic search of the studies published up to April 2020 was performed using MeSH terms and free terms, in the following databases: PubMed, Scopus, Web of Science, BVS (Lilacs and BBO) and Embase. The inclusion criteria consisted of laboratory studies that evaluated the amount of AHTD, and compared PUI with NAI protocols using microCT analysis. The risk of bias in the selected studies was assessed critically by two reviewers. A meta-analysis was performed using the RevMan software program (P<0.05), and included studies providing the standardized mean difference (SMD), using a fixed effect model, and adopting a confidence interval of 95%. In all, 3495 studies were identified, three of which met the inclusion criteria. All three were considered as having a low risk of bias. The meta-analysis comparing the ability of PUI and NAI protocols to remove hard tissue debris showed a higher percentage of AHTD reduction (P<0.01) for PUI, with a confidence interval of 1.41 [0.79, 2.02]. The heterogeneity among the studies was 82% (I2). Considering the limitations of the present study, this systematic review and meta-analysis showed that PUI was more effective than NAI in removing hard tissue debris, based exclusively on studies that used microCT scanning to provide a more precise analysis of the two techniques used. The findings presented in the present study reinforce the concept that PUI can increase residue removal and improve the cleanliness of the root canal in endodontic treatments.

Evaluation of Passive Ultrasonic Irrigation and GentleWave System as Adjuvants in Endodontic Retreatment

IntroductionNonsurgical endodontic retreatment continues to be a challenge in endodontics, particularly when dealing with a complex tooth anatomy. This study evaluated the efficacy of passive ultrasonic irrigation (PUI) and the GentleWave system as supplementary techniques to remove remaining filling materials from oval-shaped root canals. MethodsTwenty distal roots of human mandibular molars with single and oval-shaped canals were shaped with R40 (40.06) instrument and filled with gutta-percha and AH Plus sealer using warm vertical obturation. Initial filling material removal was performed with R50 (50.05) instrument, followed by the use of PUI (n = 10) or GentleWave system (n = 10). Micro-computed tomographic images were obtained after obturation, initial material removal, and after the use of PUI and GentleWave. The volume of remaining filling material was calculated for the entire canal as well as for the coronal, middle, and apical thirds. Statistical analyses were performed by using analysis of variance, Kruskal-Wallis and Mann-Whitney tests. P ≤ .05 was considered significant. ResultsThe use of PUI and GentleWave as supplementary techniques significantly reduced the volume of remaining filling material after initial instrumentation (P < .05). However, none of these techniques was able to render canals free from filling materials. PUI showed better performance by removing 18% of the remaining filling material, whereas the GentleWave system was able to remove approximately 10% (P = .02). ConclusionsThe use of supplementary techniques optimized filling material removal after initial instrumentation. PUI enhanced the overall cleaning of the root canal system during endodontic retreatment in oval-shaped canals.

Efficacy of Different Irrigation Systems Used to Remove Calcium Hydroxide from the Root Canal

Introduction: The present study aimed to compare the cleaning efficacy of passive ultrasonic irrigation (PUI), self-adjusting file (SAF), EndoVac, and conventional syringe irrigation (CSI) techniques on Ca(OH)2 removal from the root canal. Methods: This study was conducted on the mesial roots of 36 human mandibular molar teeth. After 1 month, the Ca(OH)2 was removed from the experimental groups using four different irrigation techniques (SAF, PUI, EndoVac, and CSI). The inner surfaces of the canals were examined using a stereomicroscope at 30x magnification. The assessment of remaining Ca(OH)2 medicament in the grooves was performed by three calibrated dentists using a scoring system ranged 0-3. Results: None of the groups showed complete Ca(OH)2 removal from the root canals. The PUI technique removed significantly more Ca(OH)2 from all root canals, compared to EndoVac and CSI techniques (P<0.05). In the apical third, SAF group results were significantly better than the CSI group results (P<0.05). In the middle third, the SAF technique led to significantly better results in the removal of Ca(OH)2, compared to the techniques used in other groups (P<0.05).Conclusion: Currently, there is no irrigation system that can completely remove Ca(OH)2 from the root canal in endodontics. Regarding the removal of Ca(OH)2 from root canals, the PUI technique was significantly better than EndoVac and CSI techniques. In addition, the SAF technique showed significant efficacy in the middle third of the root canals.

Efficacy of passive ultrasonic irrigation, continuous ultrasonic irrigation versus irrigation with reciprocating activation device in penetration into main and simulated lateral canals.

Context:The use of chemicals solutions and means of activation is of utmost importance in endodontic treatment.Aims:This study compared three activation techniques used in the final irrigation of the endodontic treatment.Subjects and Methods:Eighty uniradicular teeth were instrumented with the Protaper Universal system up to F4 file. After decalcification, the teeth had artificial lateral canals created at 2, 4.5 and 6 mm from working length (WL). The groups were randomly divided into four groups (n = 20): control group (C), passive ultrasonic irrigation (PUI) group, continuous ultrasonic irrigation (CUI) group, and easy clean (EC) group. The penetration of the irrigant into the samples was evaluated using image observation using the Image J program.Statistical Analysis Used:The level of agreement among the observers was determined by the Cronbach's alpha test. The likelihood ratio test was used to evaluate possible differences between the groups and the Kendall's W statistic test to verify possible differences between the irrigant penetration levels in the lateral canals. The Fisher's exact test was applied to verify differences by the studied group considering the WL variables and lateral canals.Results:The results showed no statistical difference in the penetration of the irrigator in the main canal when compared to the C, PUI, CUI, and EC groups (P > 0.05).Conclusions:The method using a positive syringe and needle pressure was not able to effectively carry the irrigator to the artificially made lateral canals, whereas PUI, CUI, and EC were equally efficient in this regard (P < 0.01).

Influence of Passive Ultrasonic Irrigation on the Removal of Root Canal Filling Material in Straight Root Canals.

Objective:This study evaluates the efficacy of passive ultrasonic irrigation (PUI) in removing root canal filling material from endodontically treated teeth after using one of two reciprocating systems, Reciproc (VDW, Munich, Germany) or WaveOne (Dentsply Maillefer, Ballaigues, Switzerland), or one nickel-titanium (NiTi) rotary system, ProTaper Universal Retreatment (Dentsply Maillefer).Methods:One hundred and twenty straight root canals of extracted human maxillary incisors were instrumented and then obturated. The specimens were divided into six groups (n=20) as follows: Group R, Reciproc R25 instrument without PUI; Group W, WaveOne Primary instrument without PUI; Group PT, ProTaper Universal Retreatment system without PUI; Group R-PUI, Reciproc R25 with PUI; Group W-PUI, WaveOne Primary with PUI and Group PT-PUI, ProTaper Universal Retreatment system with PUI. After removing the filling material, the teeth were cleaved longitudinally and photographed. The total canal space and remaining material were quantified with the aid of an imaging software tool. The Kruskal-Wallis test was used to identify significant differences between the groups.Results:No statistically significant differences (P>0.05) in residual filling material were observed between the groups.Conclusion:The use of PUI did not improve the removal of filling material from the root canals, regardless of the previously used instrumentation system.

Investigation of the Efficacy of Passive Ultrasonic Irrigation Versus Irrigation with Reciprocating Activation: An Environmental Scanning Electron Microscopic Study

IntroductionThe objective of this ex vivo study was to compare the efficacy of passive ultrasonic irrigation (PUI) versus a new activation system using reciprocating motion (EasyClean [EC]; Easy Equipamentos Odontológicos, Belo Horizonte, Brazil) to remove debris from the root canal walls at 6 predetermined apical levels using environmental scanning electron microscopy. MethodsMesiobuccal root canals of 10 mandibular molars were prepared with a 30/.05 final instrument. The specimens were embedded in flasks containing heavy body silicone, cleaved longitudinally, and 6 round indentations were made into the apical region of the buccal half at 1-mm intervals. The same specimens were used to prepare a blank control group (no debris), a negative control group (completely covered by debris), and 2 experimental groups: PUI and irrigation with reciprocating activation. Standardized images of the indentations were obtained under environmental scanning electron microscopy and assessed by 2 examiners. The amount of debris was then classified using a 4-category scoring system. The kappa test was applied to determine interexaminer agreement, whereas the Kruskal-Wallis, Dunn, and Friedman tests were used to compare scores. ResultsThe EC group had results statistically similar to those of the blank control group for all 6 root levels examined. The PUI group had results statistically similar to those of the negative control group for the 3 most apical levels and similar to those of the blank control group for the 3 most cervical levels. ConclusionsActivating the irrigant with a reciprocating system (EC) promoted more effective debris removal from the more apical regions of the root canal when compared with PUI.

Efficacy of passive ultrasonic irrigation in removing root filling material during endodontic retreatment

Abstract Introduction The aim of endodontic retreatment is to remove the previous root filling materials completely to allow for the subsequent steps of cleaning, canal shaping and filling replacement. Objective To evaluate the efficacy of passive ultrasonic irrigation (PUI) in the removal of residual root filling material after removing the bulk of the root filling with nickel-titanium rotary instruments. Material and method Eighteen mesial isthmus-containing roots of human mandibular molars were prepared and filled. The root canal filling was removed with the ProTaper Retreatment system and K-files. The specimens were randomized into 2 groups (n = 9), both irrigated with 2.5% NaOCl and 17% EDTA. In the conventional group, an irrigation syringe was used. In the PUI group, the PUI technique was used. The roots were sectioned longitudinally and evaluated under stereomicroscopy. Student’s t test was used to compare groups and the general linear model was used to compare the apical, cervical, and middle root thirds within each group, in regard to the percentages of remaining residual filling material. Result There was no statistically significant intergroup difference, irrespective of which root third was evaluated (P &gt; 0.05). In the PUI group, the apical third showed a significantly higher percentage of residual filling material, in comparison with the cervical (P = 0.038) and middle (P = 0.029) thirds. Conclusion The PUI technique was not more effective than the conventional irrigation technique in removing residual root filling material during endodontic retreatment in root canals with a complex anatomy.

The synergistic effect of ultrasonic activation and irrigation on Enterococcus faecalis biofilm

Aim: The aim of this investigation was to compare the efficacy of passive ultrasonic irrigation (PUI) with either 2.5% sodium hypochlorite (NaOCl) or saline, with that of conventional syringe irrigation on intraradicular Enterococcus faecalis biofilm. Materials and Methods: Biofilms of E. faecalis were established over 21 days in 80 single roots that had undergone biomechanical preparation followed by gamma radiation. Biofilms were treated for 1 min with 2.5% NaOCl/PUI (Group 1), 2.5% NaOCl (Group 2), sterile saline/PUI (Group 3), and sterile saline (Group 4). The positive control (n = 4) was used to confirm the presence of biofilm before various treatments. Additional four samples that served as a negative control were used to confirm the sterility of the samples. Biofilm eradication was evaluated by Colony Forming Unit (CFU) quantification and scanning electron microscopy (SEM). Results: The median of CFUs of S1 was significantly higher than that of S2 in all experimental groups. SEM examination showed a significant difference between the positive control and the experimental groups (P < 0.001), with the highest score of biofilm in the positive control group followed by Group 4 and both groups were not statistically significant from each other (P = 0.067). Following various treatments, the highest scores of biofilm were observed in the coronal third and the least were in the apical third. Conclusions: PUI did not increase the effectiveness of NaOCl irrigation on biofilm removal, however, PUI enhanced biofilm disturbance when used with saline. The least mean score of remaining biofilm was in the apical third of all treatment groups compared to other thirds.

Micro–Computed Tomographic Evaluation of Hard Tissue Debris Removal after Different Irrigation Methods and Its Influence on the Filling of Curved Canals

IntroductionThe aim of this study was to compare the efficacy of passive ultrasonic irrigation (PUI) and the EndoVac (EV) System (Discus Dental, Culver City, CA) in hard tissue debris removal and its influence on the quality of the root canal filling with the aid of micro–computed tomographic scanner. MethodsTwenty-four mandibular molars were subjected to 4 microtomographic scannings (ie, before and after instrumentation, after final irrigation, and after obturation) using the SkyScan 1176 X-ray microtomograph (Bruker microCT, Kontich, Belgium) at a resolution of 17.42 μm. Mesial canals were prepared using R25 Reciproc instruments (VDW GmbH, Munich, Germany) and divided into 2 groups according to the final irrigation method: the PUI group (n = 12) and the EV group (n = 12). All specimens were filled with the continuous wave of condensation technique. CTAn and CTvol software (Bruker microCT) were used for volumetric analysis and 3-dimensional model reconstruction of the root canals, hard tissue debris, and the filling material. Data were statistically analyzed using the Student t test. ResultsAnalysis of the micro–computed tomographic scans revealed debris accumulated inside the root canals, occupying an average of 3.4% of the canal's volume. Irrigation with PUI and the EV system reduced the volume of hard tissue debris in 55.55% and 53.65%, respectively, with no statistical difference between them (P > .05). Also, there was no difference among the groups with regard to the volume of filling material and voids (P > .05). ConclusionsPUI and the EV system were equally efficient in the removal of hard tissue debris and the quality of root canal filling was similar in both groups, with no influence from the irrigation method.

Effect of Ultrasonic Activation of Irrigants on Smear Layer Removal

IntroductionThe objective of this study was to evaluate the efficacy of passive ultrasonic irrigation (PUI) with 17% EDTA and 1% NaOCl solutions on smear layer removal. MethodsRoot canal preparations of 32 human teeth were performed with the ProTaper system. Next, they were longitudinally fractured to permit quantitation of smear layer creation from the cervical, middle, and apical thirds of the roots by using scanning electron microscopy. After reassembling the fractured tooth halves, they were divided into 4 groups according to different final irrigation protocols: group1, EDTA + NaOCl; group 2, EDTA with PUI + NaOCl; group 3, EDTA + NaOCl with PUI; and group 4, EDTA + NaOCl, both with PUI. After irrigation, the tooth halves were separated to permit imaging the same areas by scanning electron microscopy, and a percentage of opened dentinal tubules in irrigated areas as a percent of the total area was obtained. The results were submitted to Kruskal-Wallis, analysis of variance, and Bonferroni tests (α = 0.05). ResultsThe cervical third of the samples from all groups showed higher percentage of smear layer removal and open dentinal tubule areas, followed by the middle and apical thirds. Among the irrigation groups, there were statistically significant differences in cervical third between group 2 and group 4 samples, with the highest and lowest percentage of smear layer removal, respectively. ConclusionsPUI by using 1% NaOCl and ultrasonic tip placed within 1 mm of the apical foramen did not show higher efficacy in smear layer removal compared with conventional irrigation.

Radiographic evaluation of root canal cleaning, main and laterals, using different methods of final irrigation

OBJECTIVE: To evaluate the efficacy of passive ultrasonic irrigation (PUI) using intermittent or continuous flushing and conventional manual irrigation (CMI) on the cleaning of main and simulated lateral root canals.MATERIAL AND METHOD: The root canals of 24 artificial teeth were prepared and simulated lateral canals were made in the medium and apical thirds of the root. The specimens were divided into three groups: G1- CMI, G2- PUI 1 (intermittent flushing) and G3- PUI 2 (continuous flushing). Root canals were filled with contrast solution and the roots were radiographed pre- and post-irrigation. The digital images were transferred to Image Tool 3.0 software and the areas of root canal completely filled with contrast, and after irrigation with contrast remnant, were measured to obtain percentage data. Statistical analysis between groups was performed by ANOVA and Tukey tests.RESULT: In the apical third, G2 and G3 (PUI) groups showed higher percentage of cleaning than G1 (CMI) (p&lt;0.05).CONCLUSION: Passive ultrasonic irrigation using intermittent flushing promoted a higher cleaning of simulated lateral canals than conventional manual irrigation in the apical third. There were no differences between groups in the main root canal and the middle third.
