Efficacy of non-computerized cognitive rehabilitation in Parkinson’s disease: A one year follow up study

In this study, we explored the effect of non-computerized cognitive rehabilitation in patients with Parkinson’s disease in comparison with an intervention with elements of music therapy after the completion of a three-month program and one year after the end of the intervention. After the initial neuropsychological examination, the respondents were divided into two intervention groups. The experimental group (n = 26) underwent a twelve-week program of cognitive rehabilitation at a frequency of 60 minutes once a week. The control group (n = 27) underwent an intervention program with elements of music therapy at the same frequency. Respondents who underwent the cognitive rehabilitation program improved in the delayed recall from visual memory in the follow-up examination after the end of the cognitive intervention. One year after the end, the effect of cognitive rehabilitation persisted in delayed recall from visual memory and in executive mental flexibility. Cognitive rehabilitation is an effective approach to compensate for cognitive deficits in P D, but other approaches to cognitive stimulation may be equally effective.

Comparative Effects of Cognitive Rehabilitation and Photobiomodulation on Drug Craving in Treatment-Seeking Opioid Addicts.

Background: Drug addiction refers to a maladaptive pattern of drug use that frequently leads to substance abuse problems and accompanying cognitive and behavioral symptoms. Among the crucial criteria of drug addiction, craving stands out as a potent factor contributing to ongoing drug abuse and relapse following period of abstinence. To date, there is no definitive method for eradicating opioid cravings. The introduction of novel neurocognitive interventions, such as cognitive rehabilitation and photobiomodulation (PBM), into the realm of psychiatric treatments holds promise due to the parallels between drug addiction and other psychiatric disorders. These innovative techniques offer potential value in addressing drug addiction. Objective: This study aimed to assess and compare the efficacy of cognitive rehabilitation and PBM in alleviating drug cravings among individuals undergoing addiction treatment in clinical settings. Methods: The research used randomized clinical trial as the chosen research method. The statistical population encompassed all clients receiving treatment for addiction at clinics, selected through the convenience sampling method, with α = 0.05 significance level and an effect size of 85%. Gpower software was utilized to determine three equal groups. Sixty-three participants, each having a mean total score higher than 3 out of 5 on the Desire for Drug Questionnaire (DDQ), were randomly assigned to two experimental groups (n = 21 each) and a control group (n = 21). For the assessment of immediate and periodic opioid craving, the DDQ and the Obsessive Compulsive Drug Use Scale were used. In the low-level laser group, an 810 nm wavelength with continuous irradiation was applied twice a week at a distance of 1 cm by 1 cm to the prefrontal part of the brain for duration of 6 weeks (12 sessions). In the brain rehabilitation group, the stop signal and n-back tasks software were used twice a week for period of 6 weeks (comprising 12 sessions, each lasting 30 min). Results: The results demonstrated that each intervention significantly reduced drug craving in both the post-test and follow-up phases compared to the control group. The Bonferroni post hoc test indicated that PBM was more effective than cognitive rehabilitation in terms of working memory (WM) and inhibitory control for reducing drug craving (p < 0.05). Conclusions: While both PBM and cognitive rehabilitation targeting WM and inhibitory control effectively reduced opioid drug craving, low-level laser therapy proved to be more effective than cognitive rehabilitation in this regard.

AbstractBackgroundMore than 600 million people have been infected by Covid‐19 since the beginning of the pandemic. Common cognitive and psychological symptoms in these patients are memory impairment, attention difficulties, anxiety and depression, and neuropsychological rehabilitation is a possible strategy to improve them. In addition, some studies have shown a decrease in depressive symptoms after starting a cognitive rehabilitation program, although this was not the main objective of rehabilitation.MethodFive patients underwent mood assessment, more specifically, depressive symptoms. For this, the Patient Health Questionnaire‐9 (PHQ‐9) was used. The application was made immediately before and after the cognitive rehabilitation intervention, lasting 20 weekly sessions of about one hour of activity.ResultThe evaluation was performed on five patients with a mean age of 52.5 years (Table 1). These presented measures that indicated the presence of depressive symptoms at the beginning of the study. After 20 sessions of cognitive rehabilitation it was not possible to observe an objective change in depressive symptoms. This can be explained by the reduced number of individuals in the sample, which may have impaired the statistical calculation. But it was observed a clinical improvement of depressive symptoms was observed in 60% of patients and 40% remained stable (Table 2). Clinical improvement was also noted in patients, who reported that they were feeling better and that they resumed activities that they have previously avoided or that they were finding it easier, in addition to perceiving their own evolution throughout the treatment, increasing their sense of self‐efficacy and feeling more hopeful.ConclusionFinally, it is noted that cognitive rehabilitation is a possible strategy to improve not only cognitive impairment, but also the clinical manifestations of depressive symptoms in post‐Covid‐19 patients.

Analysis of the effectiveness of cognitive rehabilitation for patients with chronic mental illness: A meta-analysis

BACKGROUND People suffering from chronic mental illness are sensitive to stressful stimuli, lack coping skills, and have low self-esteem due to problems such as social situations. They also experience depression, isolation, fear, and frustration. Due to cognitive dysfunction, people suffering from chronic mental illness have inadequate cognitive processes that lead to distorted thinking. AIM To confirm the effectiveness of cognitive rehabilitation therapy in improving cognitive function and alleviating behavioral and psychological symptoms in patients with chronic mental illness, and to identify the cognitive function that had the main effect. METHODS The quality of the studies was evaluated using the Assessment of Multiple Systematic Reviews criteria, and data published from 2011 to December 2022 were searched using PubMed, Cochrane, RISS, KISS, and DBpia. The keywords used in the search were “mental illness,” “cognitive rehabilitation,” “cognition,” and “mental.” A meta-analysis was conducted on the 12 selected papers. RESULTS The level of evidence for the 12 documents was that of a randomized experimental study. Intervention types in cognitive rehabilitation can be divided into cognitive behavior, cognitive training, cognitive rehabilitation, and computerized cognitive programs. Most of the studies were on schizophrenia, and the measurement areas were cognitive functions (e.g ., concentration, memory, and executive function) as well as depression, sociability, and quality of life. As a result of the meta-analysis of each variable, the effect size for cognitive rehabilitation treatment was in the following order: Sociability, memory, concentration, executive function, quality of life, and depression. Particularly, sociability and memory exhibited significant effects. CONCLUSION Cognitive rehabilitation aids cognitive function and sociability in patients with chronic mental illness and can be used as evidence for cognitive rehabilitation in mental health and occupational therapy.

Summary Abstract:The explosion of digital technology in the past decade has led to unprecedented possibilities towards improving cognitive assessment and understanding brain health. Digital technology encompasses using a multitude of devices, such as laptops, smartphones, etc, to collect health-related data. The settings can be varied to include in-clinic, remote/virtual, or a combination of hybrid models for data collection. Data can be collected at a single time point or over a continued period of time. Furthermore, the unique combination of devices used, settings, and methods of collecting digital data can become even more exclusive against the backdrop of the 'purpose’ for conducting the digital study. This symposium, consisting of four abstracts, brings together the unique combination of digital studies with exclusive devices, methodologies, settings, and purposes. The topics range from how smartphone-based assessments can be applied to understand the interaction between day-to-day variability in sleep and cognition, to the use of computerized testing to investigate the associations between cognitive performance and markers of brain pathology (e.g. amyloid and tau status), to understanding cognition from an open-source smartphone application to passively and continuously capture sensor data including global positioning system trajectories, to the development and validation of an online simulated money management credit card task, and to determining the effects of cognitive rehabilitation via digital technology on cognition, neuropsychiatric symptoms, and memory strategies.

Alzheimer's disease (AD) and mild cognitive impairment (MCI) have taken their place among the most common neurodegenerative diseases by affecting countless people around the world. Mild cognitive impairment can be defined as the earliest stage of deterioration in the cognitive functions. While impairment in more than one cognitive domain can be seen in MCI, it is seen that individuals can show some level of functionality in daily life. Also, Alzheimer's and MCI cause degeneration in some parts of the brain (parietal lobe, frontal lobe, temporal lobe), and disease is progressed, shrinkage may occur in some brain regions. Thus, a series of intervention techniques have been introduced to improve the daily living activities of individuals diagnosed with Alzheimer's and MCI. Cognitive rehabilitation is an individualized process that focuses on improving daily living activities. Furthermore, the effectiveness of cognitive rehabilitation on Alzheimer's disease and MCI in numerous cognitive domains is proven by different researchers. Different researchers prove that different cognitive rehabilitation programs are effectively improving the cognitive, social, and psychological domains of individuals' lives. Therefore, this literature review aims to examine the effectiveness of cognitive rehabilitation programs applied after 2015 to individuals with Alzheimer's and MCI and to compile the examined findings.

Cognitive rehabilitation and aerobic exercise for cognitive impairment in people with progressive multiple sclerosis (CogEx): a randomised, blinded, sham-controlled trial

Cognitive dysfunction in people with relapsing-remitting multiple sclerosis can improve with cognitive rehabilitation or exercise. Similar effects have not been clearly shown in people with progressive multiple sclerosis. We aimed to investigate the individual and synergistic effects of cognitive rehabilitation and exercise in patients with progressive multiple sclerosis. CogEx was a randomised, sham-controlled trial completed in 11 hospital clinics, universities, and rehabilitation centres in Belgium, Canada, Denmark, Italy, UK, and USA. Patients with progressive multiple sclerosis were eligible for inclusion if they were aged 25-65 years and had an Expanded Disability Status Scale (EDSS) score of less than 7. All had impaired processing speed defined as a performance of 1·282 SD or greater below normative data on the Symbol Digit modalities Tests (SDMT). Participants were randomly assigned (1:1:1:1), using an interactive web-response system accessed online from each centre, to cognitive rehabilitation plus exercise, cognitive rehabilitation plus sham exercise, exercise plus sham cognitive rehabilitation, or sham exercise plus sham cognitive rehabilitation. The study statistician created the randomisation sequence that was stratified by centre. Participants, outcome assessors, and investigators were blinded to group allocation. The study statistician was masked to treatment during analysis only. Interventions were conducted two times per week for 12 weeks: cognitive rehabilitation used an individualised, computer-based, incremental approach to improve processing speed; sham cognitive rehabilitation consisted of internet training provided individually; the exercise intervention involved individualised aerobic training using a recumbent arm-leg stepper; and the sham exercise involved stretching and balance tasks without inducing cardiovascular strain. The primary outcome measure was processing speed measured by SDMT at 12 weeks; least squares mean differences were compared between groups using linear mixed model in all participants who had a 12-week assessment. The trial is registered with ClinicalTrials.gov, NCT03679468, and is completed. Between Dec 14, 2018, and April 2, 2022, 311 people with progressive multiple sclerosis were enrolled and 284 (91%) completed the 12-week assessment (117/311 [38%] male and 194/311 [62%] female). The least squares mean group differences in SDMT at 12 weeks did not differ between groups (p=0·85). Compared with the sham cognitive rehabilitation and sham exercise group (n=67), differences were -1·30 (95% CI -3·75 to 1·16) for the cognitive rehabilitation plus exercise group (n=70); -2·78 (-5·23 to -0·33) for the sham cognitive rehabilitation plus exercise group (n=71); and -0·71 (-3·11 to 1·70) for the cognitive rehabilitation plus sham exercise group (n=76). 11 adverse events possibly related to the interventions occurred, six in the exercise plus sham cognitive rehabilitation group (pain, dizziness, and falls), two in the cognitive rehabilitation plus sham exercise group (headache and pain), two in the cognitive rehabilitation and exercise group (increased fatigue and pain), and one in the dual sham group (fall). Combined cognitive rehabilitation plus exercise does not seem to improve processing speed in people with progressive multiple sclerosis. However, our sham interventions were not inactive. Studies comparing interventions with a non-intervention group are needed to investigate whether clinically meaningful improvements in processing speed might be attainable in people with progressive multiple sclerosis. The effects of cognitive rehabilitation combined with aerobic exercise or stretching-and-toning on new learning and memory in persons with moderate-to-severe TBI: Protocol for a randomized controlled trial

This paper describes the protocol for a Phase I/II, parallel-group, blinded randomized controlled trial that compares the effects of 12-weeks of combined learning and memory rehabilitation with either aerobic cycling exercise or stretching on cognitive, neuroimaging, and everyday life outcomes in 60 persons with moderate-to-severe traumatic brain injury (TBI) who demonstrate impairments in new learning. Briefly, participants will undergo baseline testing consisting of neuropsychological testing, neuroimaging, daily life measures, and cardiorespiratory fitness. Following baseline testing, participants will be randomized to one of 2 conditions (30 participants per condition) using concealed allocation. Participants will be masked as to the intent of the conditions. The conditions will both involve supervised administration of an enhanced, 8-week version of the Kessler Foundation modified Story Memory Technique, embedded within either 12-weeks of supervised and progressive aerobic cycling exercise training (experimental condition) or 12-weeks of supervised stretching-and-toning (active control condition). Following the 12-week intervention period, participants will complete the same measures as at baseline that will be administered by treatment-blinded assessors. The primary study outcome is new learning and memory impairment based on California Verbal Learning Test (CVLT)-III slope, the secondary outcomes include neuroimaging measures of hippocampal volume, activation, and connectivity, and the tertiary outcomes involve measures of daily living along with other cognitive outcomes. We further will collect baseline sociodemographic data for examining predictors of response heterogeneity. If successful, this trial will provide the first Class I evidence supporting combined memory rehabilitation and aerobic cycling exercise training for treating TBI-related new learning and memory impairment.

Understanding who benefits most from cognitive rehabilitation for multiple sclerosis: A secondary data analysis

Background: Up to 70% of people with multiple sclerosis (MS) experience cognitive difficulties. Cognitive rehabilitation is a type of therapy that helps manage cognitive problems. Objective: The Cognitive Rehabilitation for Attention and Memory in MS (CRAMMS) trial showed some evidence of effectiveness of cognitive rehabilitation in improving cognitive function, with some participants benefitting more than others. We therefore conducted a secondary analysis of the CRAMMS data to understand who benefits most. Methods: We grouped baseline data into four categories of possible predictors. We used regression models to identify specific factors/characteristics that could predict the likelihood that an individual will benefit from cognitive rehabilitation. Results: The models predicted whether a participant improved or did not improve in neuropsychological function following cognitive rehabilitation in up to 86% of participants. Results suggest that younger participants with medium to high education, diagnosed with relapsing-remitting multiple sclerosis (RRMS) and primary-progressive multiple sclerosis (PPMS) who have not experienced any recent relapses, with mild to moderate cognitive difficulties were most likely to benefit from cognitive rehabilitation. Conclusion: We can predict which participants are most likely to demonstrate significant improvements in neuropsychological function following group-based cognitive rehabilitation. Clinically, this allows us to optimise limited neuropsychology resources by offering such cognitive rehabilitation to those most likely to benefit.

Investigating the Effect of Cognitive Rehabilitation on the Memory Improvement of Patients With Alzheimer

Objectives: Alzheimer is the most prevalent cognitive disturbance with a high spread rate among the elderly. The current research aims to investigate the impact of cognitive rehabilitation on the memory improvement of Alzheimer disease patients. Methods: This was a semi-experimental design with pre-test and post-test designs. The statistical population in Baghdad City, Iraq, in 2021 included 60 patients with Alzheimer and was divided into an experimental (n=30) and a control group (n=30). The patient’s cognitive abilities were assessed before the intervention (pre-test), promptly after the intervention (post-test), and 2 months later (follow-up). The experimental group had twenty-eight 45-min sessions of training based on the Montessori method (2 sessions per week). The data were analyzed via the SPSS software, version 19, using the independent t-test and repeated measures analysis of variance. The results had a P&lt;0.05. Results: The findings of the independent t-test demonstrated no significant difference between the scores of both groups during the pre-test stage (P&gt;0.05); however, the difference was significant during the post-test and follow-up stages (P&lt;0.001). In addition, the findings of the repeated measure analysis of variance indicated a significant difference between both groups’ mean scores in the post-test and follow-up (P&lt;0.001). Discussion: Cognitive rehabilitation can help patients with memory disorders and positively affect their memory performance.

The Effectiveness of Cognitive Rehabilitation on Improving the Cognitive Abilities of the Elderly

Objectives The stagnation of mental activity in aging is associated with a decline in the function of all body systems, including cardiovascular, respiratory, genital, glandular, immune, and other organs. Cognitive disorders are also one of the most common problems in aging, which in turn exposes them at risk. Aging is associated with significant changes in memory, intelligence, perception, metacognition, recall, problem-solving, and other cognitive abilities. Therefore, this study was conducted to determine the effectiveness of cognitive rehabilitation in improving the cognitive abilities of the elderly. Methods &amp; Materials The present study was a quasi-experimental study with a pre-test-post-test design with a control group. The statistical population included elderly people living in nursing homes in Kermanshah City, Iran, from which 24 people were selected by convenience sampling method and according to the inclusion criteria and were randomly assigned to the experimental and control groups. The experimental group underwent 20 sessions of 35 minutes twice a week with the Captain Log cognitive rehabilitation program, during which time the control group did not receive any intervention. After the intervention, post-test was performed for both groups. Research tools included a cognitive rescue skills questionnaire and Captain Log rehabilitation software. Descriptive statistics and univariate and multivariate analysis of covariance were used to analyze the data using SPSS software, version 24. Results The results of data analysis showed that F statistic for dependent components, including memory (F=193.57, P&lt;0.001), selective attention (F=375.43, P&lt;0.001), decision-making (F=39.20, P&lt;0.001), planning (F=120.003, P&lt;0.001), sustained attention (F=312.07, P&lt;0.001), flexibility (F=91.44, P&lt;0.001), and social cognition (F=4.48, P&lt;0.05) are significant at the level of 0.05 and 0.01. Therefore, it can be concluded that the cognitive rehabilitation program has improved the cognitive abilities of the elderly. Conclusion Considering that cognitive rehabilitation has a significant effect on the neural activities of brain areas related to cognitive abilities and improves their function, it can be used as a suitable method to improve cognitive abilities.

The Effects of Cognitive Rehabilitation on Pragmatic Language in Traumatic Brain Injury: A Meta-Analysis

Purpose:This meta-analysis addressed the research question: In adults with traumatic brain injury (TBI), does cognitive rehabilitation improve pragmatic language?Method:Based on the Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines, a systematic literature search was conducted to identify empirical studies that met inclusion criteria including (a) adults (18 years and older) with TBI, (b) a cognitive-based intervention, and (c) reported quantitative pragmatic language outcome measures data. A meta-analysis was conducted on pragmatic language outcome measures. Cohen'sdand a 95% confidence interval were used to evaluate the intervention effect.Results:After applying the inclusion and exclusion criteria to 640 eligible papers, six studies (N= 76 participants combined across articles) remained. Results showed that cognitive rehabilitation targeting skills used in communication has a large, positive effect (Cohen'sd= 0.89 [0.54,1.24]) on improving pragmatic language abilities in adults with TBI.Conclusions:This meta-analysis provides evidence that cognitive rehabilitative protocols created in collaboration with speech-language pathologists (SLPs) have a significant impact on improving pragmatic language abilities of adults with TBI. This is important for SLPs to consider when determining a treatment approach when working with adults diagnosed with a TBI who have deficits in the area of pragmatic language.
