Co-occurring PTSD in intensive OCD treatment: Impact on treatment trajectory vs. response.

Obsessive-compulsive disorder (OCD) and co-occurring posttraumatic stress disorder (PTSD) is associated with more severe and chronic OCD. However, findings regarding treatment effectiveness of cognitive behavioral treatment (CBT) with exposure and response prevention (ERP) with this comorbidity are mixed. Research aimed at understanding the precise barriers to OCD treatment effectiveness for individuals with co-occurring PTSD may help elucidate unique treatment needs. The current study used linear regression and latent growth curve analysis comparing treatment response and trajectory from patients with OCD (n=3083, 94.2%) and OCD+PTSD (n=191, 5.2%) who received CBT with ERP in two major intensive OCD treatment programs. Although patients with OCD+PTSD evidenced similar trajectories of overall severity change, patients at one site required nearly 11 additional treatment days to achieve comparable reduction in OCD severity. Further, at the dimensional level, those with OCD+PTSD had poorer treatment response for unacceptable thoughts and symmetry symptoms. The moderate effect for unacceptable thoughts, indicating the widest gap in treatment response, suggests these symptoms may be particularly relevant to PTSD. Findings are limited by a naturalistic treatment sample with variation in treatment provision. Findings emphasize caution in using a one-size-fits-all approach for patients with co-occurring OCD+PTSD within intensive OCD treatment programs, as broadly defined outcomes (e.g., reduction in overall severity) may not translate to reduction in the nuanced symptom dimensions likely to intersect with trauma. Unacceptable thoughts and symmetry symptoms, when co-occurring with PTSD, may require a trauma-focused treatment approach within intensive OCD treatment.

Interventions for psychiatric disorders among university students: An umbrella review of systematic reviews and meta-analyses

BackgroundMental disorders are considered to be the main reason for the increase of the disease burden. College students seem to be more vulnerable to the adverse effects of stress, which makes them more at risk of suffering from mental disorders. This umbrella review aimed to evaluate the credibility of published evidence regarding the effects of interventions on mental disorders among university students. MethodsTo identify systematic reviews and meta-analyses investigating the effects of interventions on mental disorders in the university student population, extensive searches were carried out in databases including PubMed, Embase, and the Cochrane Database, spanning from inception to July 21, 2023. Subsequently, a thorough reanalysis of crucial parameters such as summary effect estimates, 95 % confidence intervals, heterogeneity I2 statistic, 95 % prediction intervals, small-study effects, and excess significance bias was performed for each meta-analysis found. ResultsNineteen articles involving 74 meta-analyses were included. Our grading of the current evidence showed that interventions based on exercise, Cognitive-behavioural Intervention (CBI), mindfulness-based interventions (MBI), and other interventions like mood and anxiety interventions (MAI) were effective whereas exercise intervention had the highest effect size for both depression and anxiety among university students. However, the credibility of the evidence was weak for most studies. Besides, suggestive evidence was observed for the positive effects of CBI on sleep disturbance(SMD: -0.603, 95 % CI: -0.916, -0.290; P-random effects<0.01) and MAI on anxiety (Hedges'g = -0.198, 95 % CI: -0.302, -0.094; P-random effects<0.01). ConclusionBased on our findings, it appears that exercise interventions, CBI, and MAI have the potential to alleviate symptoms related to mental disorders. Despite the overall weak credibility of the evidence and the strength of the associations, these interventions offer a promising avenue for further exploration and research in the future. More high-quality randomized controlled trials should be taken into account to verify the effects of these interventions on various mental disorders.

Investigating the Effect of Cognitive-Behavioral, Mindful-Based, Emotional-Based Intervention and Professional Training on Teachers' Job Burnout: A Meta-Analysis.

Teachers are more likely to experience occupational burnout; intervention for their job burnout has been paid more and more attention by the academic community. However, there is not enough evidence to support the interventions' effect. This study adopts the meta-analysis method and makes a statistical analysis of the interventions' effect on teachers' job burnout based on 29 papers in the literature of randomized controlled experiments. It mainly presents the basic external characteristics of the research literature; besides, based on the results of software CMA.V3, this paper also analysed the overall intervention effect and the separate effect of four types of intervention, namely, cognitive-behavioral interventions (CBI), mindfulness-based interventions (MBI), professional training (PT), and emotional-based intervention (EBI). The results showed that CBI had the best effect, and mindfulness-based intervention had the second-best effect. Professional training also showed a good intervention effect, while the intervention effect of emotional-based intervention remains to be verified. In addition, it was found that only the effect of cognitive-behavioral intervention (CBI) was significant and had a strong effect (g = 0.876, 95% CI-1.06, 2.814, p < 0.05) when considering their effects on emotional exhaustion, while the other three interventions had no significant effect. The Effectiveness of Cognitive-Behavioral Intervention on Dental Anxiety during Pulpotomy in 7-10 Year-Old Children: A Clinical Trial.

Objectives: Dental anxiety is a common phenomenon experienced by children in the dental setting. However, there is limited research on the effectiveness of cognitive-behavioral interventions in managing this anxiety. Therefore, the aim of this study was to evaluate the impact of a cognitive-behavioral psychologic intervention on dental anxiety in children. Materials and Methods: A total of thirty children, aged 7-10 years, underwent prophylaxis and fluoride therapy. The baseline anxiety levels of the children were determined using the Modified Child Dental Anxiety Scale (MCDAS). The intervention group (N=15) received two 30-minute sessions of cognitive-behavioral exercises. Additionally, a short film demonstrating a cooperative child undergoing pulpotomy was created and parents were instructed to show it to their children at home. During the treatment session, the exercises were reinforced at each step, and the MCDAS questionnaire was administered again. The children's behavior was also assessed using the Venham Clinical Cooperation Scale. Independent t-test, Mann-Whitney, chi-square, and Pearson's correlation coefficient were used for statistical analysis and P<0.05 was considered statistically significant. Results: Postoperatively, the MCDAS score in the intervention group was significantly lower compared to the control group. Notably, the intervention group demonstrated a downward trend in anxiety scores, while the control group showed an increase (P<0.001). Conclusion: Cognitive-behavioral interventions were found to be successful in decreasing dental anxiety in children undergoing pulpotomy. These interventions can be successfully implemented in clinical settings.

Effectiveness of Cognitive Behavior Therapy on Depression and Quality of Life in Spouse Caregivers of Inpatients With Substance Use Disorders

Background: Substance dependence in families can have a wide range of negative effects on the lives of those close to the dependent person, in addition to causing pain to the dependent person. The present study aims to investigate the effect of cognitive behavioural intervention on depression and quality of life (QoL) in wives who cared for husbands with substance use disorders. Methods: The research was conducted using a quasi-experimental design with pre- and post-tests. Purposive sampling was used to select 60 spouse carers of male inpatients with substance use disorders, who were randomly assigned to the intervention (n=30) and control (n=30) groups. At baseline and post-intervention, all participants completed the Depression Anxiety Stress Scale (DASS-21) and the World Health Organization Quality of Life-Bref (WHO QoL-BREF). The intervention group received eight sessions per week of individual cognitive behavioral therapy (CBT) for 45 to 60 minutes, while the control group did not. Data were analyzed using multivariate analysis of covariance (MANCOVA) in SPSS software, version 26. In statistical tests, the significance level was 0.05. Results: The results showed significant differences between the pre-test and post-test scores of the experimental group; therefore cognitive behavior intervention can significantly reduce depression and improve the QoL of substance dependent’s wife. Conclusion: These results have crucial implications for clinical practice in terms of improving mental health and well-being in the context of male substance use disorder spouse caregivers.

Participant Characteristics as Moderators of the Effects of Cognitive Behavioral Interventions on Concerns About Falling: Secondary Analyses of Two Randomized Controlled Trials.

Effects of interventions may vary among participants. We explored whether participant characteristics were moderators of the effects of two cognitive behavioral interventions on concerns about falling (CaF) in older community-dwelling people. Secondary analyses of two RCTs were performed, concerning the group intervention A Matter of Balance - Netherlands (AMB-NL, n = 540) and individual AMB - Home (n = 389) intervention. Marginal models were used to assess moderation. Analyses included single moderator and multiple moderator models containing multiple moderators at once. A total of 19 characteristics were assessed. Moderating effects were found for living situation, fall history, symptoms of depression, perceived general health, ADL disability, cognitive status, and consequences of falling-loss of independence subscale. Effects varied by intervention, time point, and type of model.

EFFECTS OF DEPRESSIVE SYMPTOMS ON COGNITIVE BEHAVIORAL INTERVENTIONS WITH CUSTODIAL GRANDMOTHERS

Abstract Over 25% of custodial grandmothers report elevated depressive symptoms, which may affect situational appraisals and learning/retaining new information, and thus the effectiveness of cognitive-behavioral interventions. We examined the effect of depressive symptoms on self-appraised stress and reward associated with caregiving to co-residential grandchildren in a sample of 342 grandmothers who participated in a nationwide behavioral RCT testing two methods of stress reduction. Participants completed a baseline questionnaire, one of 2 cognitive-behavioral interventions (guided journaling or journaling with Resourcefulness Skills Training ©), and 3 subsequent questionnaires over 6 months. We analyzed self-appraised stress and reward with RM-ANOVAs to evaluate whether level of depressive symptoms (CES-D below-16, 16-29, 30+) affected the effectiveness of each arm with regard to self-appraised stress and reward. We found differences by depressive symptom category and interaction effects of depressive symptoms by arm in self-appraised stress. The below-16 group had relatively constant appraised stress over time, but the 16-29 and 30+ groups had significant decreases in stress over time. The Resourcefulness Skills Training © group had an average decrease in stress of .68 over time compared to .2 points for the journal-only group; there were no intervention effects in the below-16 group. The highest symptom group showed a stronger effect for the journal-only intervention, perhaps reflecting the challenges in learning and applying new skills. Self-appraised reward increased over time across groups. Participants' level of depressive symptoms have significant effects on the effectiveness of interventions and should be taken into consideration when designing studies and analyzing effectiveness.

Evaluation of the Effect of Cognitive Behavioral Interventions on the Emotional Reactions of Parents of Children with Retinoblastoma

Background: Retinoblastoma is the most common primary intraocular malignancy in childhood. Diagnosis of the disease and treatment decisions put a lot of stress on the family. Excessive anxiety and stress can lead to serious psychological problems. The cognitive behavioral approach focuses on the individuals’ thoughts, behaviors, and emotions and their interaction. This study aimed to investigate the effect of cognitive behavioral interactions on the emotional reactions of parents of children with retinoblastoma.&#x0D; Methods: This study was carried out using a quasi-experimental design on 106 parents of children with retinoblastoma referred to the Farabi Eye Hospital, Tehran, Iran, between 2017 and 2018. Cognitive behavioral therapy was performed through eight sessions of 90-minute training for parents. The data collection method was self-responding using the depression, anxiety, and stress scale-21 Items (DASS-21) questionnaire. Pre-and post-intervention test scores were collected for statistical analysis.&#x0D; Results: The mean anxiety score decreased from 13.65 (moderate anxiety) before the cognitive-behavioral intervention to 10.13 (mild anxiety) after the intervention (p&lt;0.05). The mean depression score decreased from 11.26 (mild depression) before the intervention to 8.32 (no depression) after the intervention (p&lt;0.05). The mean stress score decreased from 10.79 (normal) before the intervention to8.25 (normal) after the intervention (p&lt;0.05).&#x0D; Conclusion: Our study showed that the occurrence of retinoblastoma in children poses a significant risk to the mental health of their parents. Cognitive-behavioral interventions can be effective in improving the level of parent's anxiety, depression, and stress.

Occupational stress among Nigerian construction trade artisans in the building construction sector: An intervention study.

Background:Construction work can negatively affect artisans’ mental health in the form of stress. This research investigated the effect of cognitive behavioral intervention on occupational stress among Nigerian construction trade artisans in the building construction sector.Methods:In this randomized controlled study involving 3 waves of data collection, 140 construction trade artisans who presented with high occupational stress symptoms at the study onset were assigned randomly to either a treatment condition (n = 70) or to a waitlist control condition (n = 70). The study involved pre-treatment and post-treatment assessments and a 3-months follow-up. The artisans’ occupational stress questionnaire and artisans’ dysfunctional thoughts at work scale were the data collection instruments.Results:Results revealed a significant effect of group on artisans’ occupational stress and dysfunctional thoughts at work. Also, significant effects of time on artisans’ occupational stress and dysfunctional thoughts at work were recorded. Finally, group × time interaction effects on artisans’ occupational stress and dysfunctional thoughts at work were significant.Conclusion:Cognitive behavioral intervention effectively reduced occupational stress symptoms and dysfunctional thoughts at work among Nigerian construction trade artisans compared with a waitlist control condition.

Does cognitive behavioral therapy for anxiety disorders improve threat reappraisal?: A meta-analysis

The purpose of this study is to evaluate the evidence that cognitive behavioral therapy improves exaggerated appraisals of threat among people with a primary anxiety disorder. The current meta-analysis of 19 randomized clinical trials (1594 participants) tests whether cognitive behavioral treatments for anxiety disorders improve threat reappraisal to a greater extent than wait-list control conditions, non-cognitive behavioral treatments, and active control conditions. A statistically significant and large cumulative effect ( g = 0.76, 95% CI: [0.51–1.01], p < 0.01) indicated that cognitive behavioral therapy produces greater improvements in threat reappraisal relative to a comparison condition. Heterogeneity of the effect was high ( I 2 = 80.92%). A moderator analyses showed this effect was significant when compared to waitlist control conditions ( g = 1.08) non-cognitive behavioral treatments ( g = 0.37), and active control conditions ( g = 0.29). The effect of cognitive behavioral treatment on threat reappraisal was greater for studies on panic disorder ( g = 1.06) relative to social anxiety disorder ( g = 0.60) and generalized anxiety disorder ( g = 0.35). Findings from this meta-analysis support cognitive models of anxiety, which assert that cognitive behavioral therapy improves people's ability to make more realistic appraisals of threat.

Effect of Cognitive Behavioral Intervention on Stress Response of Nurses with PTSD in Operating Room

Objective: To investigate the effect of cognitive behavioral intervention on nurses with PTSD by using cognitive behavioral intervention. Methods: The subjects were selected by randomized control method, and the PTSD nurses were divided into control group (n = 30) and intervention group (n = 30). The intervention group used cognitive behavioral intervention; the control group did not use cognitive behavioral intervention. Nurses with PTSD in the two groups completed the basic situation survey, medical coping questionnaire (MCMQ) and anxiety self-rating form (SAS) psychological scale one month later. Results: There was no significant difference in MCMQ and SAS scores between the two groups before intervention (P > 0.05). After intervention, the scores of MCMQ and SAS in the intervention group were compared with the scores of MOCQ and SAS in the control group. The scores of the control group after intervention were significantly higher than those of the intervention group, with statistical significance (P 0.05). It shows that cognitive behavioral intervention therapy has the effect and value of improving the psychological status of nurses with PTSD. Conclusion: Cognitive behavioral intervention therapy combined with psychology for nurses with PTSD can effectively relieve their stress level, relieve their mood, and improve their positive coping ability and work efficiency.

Cognitive behavioral intervention for trauma in adolescent girls in child welfare: A randomized controlled trial

This study tested the effectiveness of Cognitive Behavioral Intervention for Trauma in Schools (CBITS) in adolescent girls involved in the child welfare system. Three outcomes were evaluated: symptoms of posttraumatic stress disorder (PTSD), depression, and social problem-solving skills. A randomized controlled trial was utilized to compare the effects of an adapted version of CBITS with usual care (UC) services. Participants were ages 12 to 19 (N = 249), the majority of whom (69.5%) were African American. Participants’ symptoms of PTSD, depression, and social problem-solving skills were evaluated at pre, post (3 months), and follow-up (6 months) assessments. Linear mixed models were used to compare condition by time interactions using all available data. Control variables were demographics, service use, and number of types of traumas. Treatment fidelity, participant acceptability, and satisfaction with the intervention were also examined. Analyses indicated that participants in the CBITS condition showed significantly greater increases in social problem-solving than the UC condition. For both PTSD and depression symptoms, there were no significant differences between the two conditions. Both the CBITS and UC participants showed significant reductions in symptoms. Results also indicated that this intervention is an acceptable model for this population. Despite the growth of trauma-focused, evidence-supported interventions for reducing PTSD and depression, knowledge of effective interventions in child welfare youth lags behind. Because CBITS is more effective than UC in increasing social problem-solving skills, this intervention may be an important treatment option for this population.

Using cognitive behavioral intervention to prevent substance use among university students: comparative study

Purpose This paper aims to evaluate effectiveness of cognitive behavioral intervention (CBI) to prevent substance use among first-year university students at high risk of substance use. Design/methodology/approach Sixty university students at high risk for substance use recruited and assigned to CBI and control groups (30 students per groups). Intervention impact was assessed on measures of attitudes toward substance use at three time points: baseline, postintervention and three-months postintervention. Findings The analysis showed that participants’ negative attitudes toward substance use in the intervention group was increased post intervention. Although the mean score was significantly higher than the baseline (Time I) at Time II and dropped at Time III, the mean scores remained higher than time I, using repeated measure ANOVA (p &lt; 0.05). Practical implications Findings of this study provide evidence that CBI is effective to sustain substance abstinence among this age group. Originality/value The paper is testing effectiveness CBI among a high-risk population of substance use. The study is highlighting the importance of sustaining substance abstinence using psychological preventive methods.
