The Effects of Malocclusion Among Priests in Enugu, Nigeria: An Anglican Church-Based Study.

Malocclusion refers to the departure from the normal relationship of teeth to other teeth in the same and opposing arch. It can negatively impact on facial appearance and the quality of life of a person. The present study aimed to investigate the influence of malocclusion on the role of priests as counsellors and preachers in Nigeria as perceived by church members. This was a cross-sectional survey of 112 church members who completed an electronic questionnaire. Thirty-five (31.3%) participants said unattractive dental appearance (malocclusion) will impact negatively on their acceptance of the priest's gospel message. Twenty-seven (24.1%) said dental appearance will influence the choice of priest that they go to for counselling while 71 (63.4%) said it will not. The study found that though unattractive teeth arrangement did not significantly affect the priest's duties as it pertains to sermons and counselling, awareness on the importance of orthodontic care among the clergy is necessary and should be encouraged among all spiritual leaders.

Effect of Malocclusion on Bully Victim School Students

Background: Bullying or peer victimization in schools is a specific sort of aggressive behavior and can be described as a circumstance in which a student is subjected, frequently and over time, to unpleasant acts on the part of one or more classmates. The study’s objective was to evaluate Malocclusion’s effect on bully-victim school students, specifically self-esteem and oral health-related quality of life. Material & Methods: This study was a prospective cross-sectional investigation carried out at the Baridhara Scholar’s International School & College. The study was carried out between October 2021 and October 2022 which included 310 students between the ages of 10 and 14 years. Results: Most of the students, 91(29.4%), were aged 13 years. whereas 143(46.1%) were boys and 167(53.9%) were girls. The majority of the participants, 177(57.1%), were bullied, and 133(42.9%) were not bullied. Those who had experienced bullying were significantly more likely to have a Class II Division 1 incisor relationship (P value, 0.034), an increased overjet >4 mm (P value, 0.002), and an increased overbite (P value, 0.041). A negative impact on total Oral health-related quality of life (OHRQoL) (P value, 0.001) is caused by bullied participants reporting higher degrees of oral symptoms (P value, 0.042), functional limits (P value, 0.001), emotional impact (P value, 0.001), and social impact (P value, 0.001) from their oral condition. In comparison to non-bullied participants, those who had experienced bullying reported lower levels of social competence (P-value, 0.001), athletic competence (P-value, 0.001), physical appearance-related self-esteem (P-value, 0.001), and general self-esteem (P-value, 0.001). Conclusion: According to this study, bullying in schools is the most prevalent offense children commit. Significant associations exist between the self-esteem of bullying victims, the existence of malocclusion, and their OHRQoL. Orthodontic characteristics such as a Class II Div1, Div2, class III incisor relationship, enhanced overjet, and overbite are linked to bullying.

A Classification Tree to Identify Factors Affecting Temporomandibular Disorders

To examine the effects of stress, bruxism, malocclusion, and oral habits on the incidence of temporomandibular disorders (TMDs) using a classification tree method (CTM). We obtained data from a total of 1.338 patients: 669 who had TMDs and 669 who but haven’t TMDs. We recorded interview data on stress and oral habits, and during oral examination, noted malocclusions, bruxism, and any sign of poor oral habits. All data were subjected to CTM analyses. The TMD rate was highest in stressed patients (96.8%), and significantly lower in stress-free patients (25.3%) (P < 0.001). TMD was evident in all patients exhibiting both stress and bruxism (100.0%). In stressed patients lacking bruxism, the effects of malocclusion on TMD were significant (P < 0.001). CTM analyses showed that the most significant factor affecting TMD was stress, followed by bruxism and malocclusion. This is the first report to use CTM to define factors affecting TMD.

Translation and validation of the Swedish version of the Psychosocial Impact of Dental Aesthetics Questionnaire (PIDAQ) for adolescents

Objective The Psychosocial Impact of Dental Aesthetics Questionnaire (PIDAQ) is an instrument developed for assessment of orthodontic aspects of oral health-related quality of life (OHRQoL). This study aimed to translate and validate the Swedish version of PIDAQ for use in Swedish adolescents 12–19 years of age. Material and methods The translation was made according to the process described in ‘Guidelines for Establishing Cultural Equivalency of Instruments’ by Ohrbach et al. Field testing was performed in a group of consecutive patients who were about to start orthodontic treatment (untreated group) and a group of consecutive patients who had recently finished orthodontic treatment (treated group). Results During the translation procedure, a Swedish version of PIDAQ was formed. In the field testing, a total of 144 questionnaires, 72 from untreated patients (mean age 14.6 years) and 72 from treated patients (mean age 17.3 years) were collected. The exploratory factor analysis detected two factors, which differs from the factor structure in the original questionnaire. The mean total PIDAQ score was significantly higher (p < .001) in the untreated group, 40.6 (20.3), than in the treated group, 9.4 (10.5), implying a high discriminant validity. Reliability was excellent, with internal consistency Cronbach’s alpha values varying from 0.94 to 0.97. Conclusion The Swedish version of PIDAQ demonstrates good validity and excellent reliability and is suitable for future research on the effect of malocclusion on OHRQoL in Swedish 12- to 19-year-olds.

The relationship between malocclusion and oral health-related quality of life among adolescents: a systematic literature review and meta-analysis.

Malocclusion is a deviation from an anatomical norm that occurs in various populations. Evidence shows that it has physical and psychological implications as well as an influence on the patient's oral health-related quality of life (OHRQoL). Self-perception of oral health plays an important role in the understanding of malocclusion's influence on the quality of life. Malocclusion has been reported to impair a patient's function, appearance, interpersonal relationships and psychological well-being. The aim of this study was to investigate whether adolescents aged between 11 and 18 years with malocclusion are at increased risk for having a high level of OHRQoL compared with those without malocclusion. The Ovid Medline, Embase and Scopus databases were used to identify studies measuring the association between malocclusion and HRQoL. Only cross-sectional studies were included, and the following inclusion criteria were used: subjects were systemically healthy; data were available for untreated malocclusion and OHRQoL; malocclusion was measured by Dental Aesthetic Index and Index of Orthodontic Treatment Needs; OHRQoL was assessed by Child Perception Questionnaire, Child Oral Health Impact Profile and Child Oral Health Impact Profile; and the paper was published in English. Data were extracted by two independent reviewers. The Appraisal tool for Cross-Sectional Studies (AXIS) was used to establish the risk of bias, and the Grading of Recommendations Assessment, Development and Evaluation (GRADE) evidence quality assessment tool was used to assess the impact of malocclusion on adolescents across studies. The initial search yielded 530 papers. Eleven cross-sectional studies were included in the meta-analyses, and all data describing the relationship between malocclusion and OHRQoL were gathered via a structured questionnaire. Our results showed that malocclusion is linked to decreased OHRQoL in children and adolescents aged between 11 and 18 years in continuously analysed studies. However, in dichotomously analysed studies, it was shown that young people without malocclusion are linked to decreased OHRQoL. Adolescents with severe levels of malocclusion might have among the worst levels of OHRQoL. The effects of malocclusion on OHRQoL were influenced by the age of the adolescents as well as by their culture and environment. PROSPERO (CRD42020178657).

Effect of Malocclusion on Quality of Life among Persons Aged 7 - 25 Years: A Cross-Sectional Study

Background: The theory of Oral Health Related Quality Of Life (OHRQOL) has manifold qualities. It endeavors to address social and/or emotional concerns as well as any symptoms or functional problems related to the teeth and jaws from the patients view point. The difficulty of utilizing any one standard evaluation tool to determine how the patient feels about problems with malocclusions or arising from them, led to the development of this construct/theory. Aim: To evaluate the impact of malocclusion on the socio-functional and psychological wellbeing of persons 7 - 25 years old. Method: 100 patients within the ages of 7 - 25 years who visited the Orthodontics Unit of the Oral Health Directorate of Komfo Anokye Teaching Hospital (KATH), Kumasi, were conveniently sampled and surveyed. Participants were given questionnaires to assess the effects of malocclusion on their Oral Health Related Quality of Life and also to determine if their educational status had any influence on the impact of malocclusion on their oral health related quality of life. Results: The modal age of the participants was 10 - 15 years with 40% as it frequency followed by 7 - 9 years while the age with least frequency was 16 - 25 years which stood at 22.0%. 28 respondents reported negative effects on their functional domain. 71 of the participants reported negative effects on psychological well-being and 49 of them reported negative effects on social well-being. Conclusion: Malocclusion has an overall negative impact on Oral Health Related Quality of Life and its related purviews. It was observed to be highest for the psychological discomfort domain and the lowest in the functional well-being domain.

The effect of malocclusion on dating prospects.

To establish whether the appearance of malocclusion affected dating prospects. The secondary objective was to identify any significant predictors relating to likelihood of dating. Cross-sectional study. King's College London. A total of 232 undergraduate students. Participants were shown images of either one man or one woman. The images had been manipulated using computer software to represent one of three differing dental appearances: post-orthodontic aligned teeth; pre-orthodontic crowded teeth; or pre-orthodontic missing teeth (developmentally absent maxillary lateral incisors). Sealed envelopes were randomly assigned to participants, who rated the individual in the image in relation to: attractiveness; intelligence; happiness; nervousness; and how much they would want to go on a date with them. The response format was a 100-mm visual analogue scale. The aligned dental appearance received statistically significant higher ratings of attractiveness (P < 0.001), intelligence (P = 0.031), likelihood of dating (P = 0.017) as well as lower ratings of nervousness (P = 0.020). There was no statistically significant difference in relation to happiness (P = 0.43), neither were there any statistically significant differences between the crowded and missing dental appearances. Regression analysis revealed that rated attractiveness was the most significant predictor in relation to likelihood of dating (P < 0.001). Malocclusion did influence dating prospects. Photographs of individuals with aligned teeth were more likely to be rated highly in relation to attractiveness, intelligence and likelihood of dating, when compared with images of malocclusions. The most significant predictor of likelihood of dating was rated attractiveness.

Mandibular asymmetry in Angle malocclusion samples

Purpose: The purpose of this study was to investigate the effect of different types of occlusion on mandibular asymmetry by using panoramic radiography in various anatomical points.Materials and Methods: This retrospective study included 100 patients whose panoramic images and malocclusions were registered in a patient database. Asymmetry indices were evaluated according to the Habets asymmetry index formula using linear and angular measurements of images. The effects of malocclusions, age and gender on the asymmetry indices were investigated and ˂0.05 p value was considered significant Results: Of the total 100 patients 51 (51%) were female and 49 (49%) were male. Associations were found between gender and the right corpus length, right and left gonial angle values, and between malocclusion and the right condylar height and right and left corpus length. Conclusion: Although values of some parameters varied with gender and malocclusion, there was no statistically significant relationship found between age, gender and malocclusion and asymmetry index values for any parameter.

New insights in the link between malocclusion and periodontal disease

We aimed to investigate associations between malocclusions and periodontal disease by comparing it to that of smoking in subjects recruited from the population-based cross-sectional study "Study of Health in Pomerania." Sagittal intermaxillary relationship, variables of malocclusion and socio-demographic parameters of 1,202 dentate subjects, 20-39years of age, were selected. Probing depth (PD) and attachment loss (AL) were assessed at four sites by tooth in a half-mouth design. Analyses were performed with multilevel models on subject, jaw and tooth level. Distal occlusion determined in the canine region, ectopic position of canines, anterior spacing, deep anterior overbite and increased sagittal overjet were associated with AL (p-value <0.05). Associations between malocclusions and PD: deep anterior overbite with gingival contact (odds ratio [OR]=1.40, 95% CI: 1.08-1.82; p-value=0.0101) and anterior crossbite (OR=1.75, 95% CI: 1.29-2.38; p-value=0.0003). Regarding crowding, only severe anterior crowding was compatible with a moderate to large association with PD (OR=1.93, 95% CI: 0.89-4.20). Compared to smoking, the overall effect of malocclusions was about one half for AL and one-third for PD. Malocclusions or morphologic parameters were associated with periodontal disease.

Patient-reported outcome measures in orthodontics

Psychological perspective of orthodontic patients due to malocclusion and treatment is less explored in orthodontics until recently. Use of psychometric tools is the method to measure this intangible dimension of malocclusion and treatment. However, many generic and condition-specific tools are available now for this purpose. Patient-reported outcome measures (PROMs) will supplement normative outcome measures in the future and will remain an important aspect in patient-centered orthodontic care. In general, with the use of these tools, there has been a conflicting report regarding the effect of malocclusion on a person's Oral Health-Related Quality of Life (OHRQoL). However, orthodontic treatment has shown consistently to improve OHRQoL though there is a transient worsening in the initial phases of treatment. The effect of self-esteem and psychosocial well-being as mediators influencing OHRQoL and contextual factors such as socioeconomic status and family support mechanism on QoL need further evidence from long-term studies.

Evaluation of mandibular asymmetry in angle malocclusion samples by posterioanterior cephalometric radiography: A preliminary study.

The aim of this study was to evaluate the effect of different occlusion types on mandibular asymmetry in different anatomical points using posteroanterior cephalometric radiography. This study was retrospectively conducted on 100 patients whose posteroanterior images and malocclusions were registered in a patient database. Asymmetry indices were determined using four linear measurements on images, and the effect of malocclusions, age, and gender on these asymmetry indices was investigated. P <0.05 was considered statistically significant. Right and left horizontal plane gonion distance values varied according to gender, right vertical plane condylar distance and left horizontal plane gonion distance values varied according to age, and right vertical plane condylar distance and left horizontal plane gonion distance values varied according to malocclusions. The measured values of some parameters varied according to age, gender, and malocclusion. Although vertical plane gonion asymmetry index and horizontal plane condylar asymmetry index values varied according to gender, there was no relationship between asymmetry index values of all parameters with age and malocclusion.

Awareness survey about the effects of malocclusion among young adults.

Malocclusion has been a problem since ancient times. Diverse factors influence malocclusion such as oral habits, anomalies in the dentition like changes in shape, position and number of teeth. But awareness and attitude about the problem differs within the neighborhood, depending upon their understanding and education. Hence, observation is essential to diminish and avoid the deleterious effects of malocclusion. Hence the objective of this study is to perceive the knowledge of youngsters and enlighten their perception on misaligned teeth. A cross sectional study was conducted among 1000 young adults between ages 18-22 in Sathyabama University, Chennai, India. A questionnaire consisting of 24 multiple choice questions were given to the participants to assess their awareness, attitude towards malocclusion and orthodontic treatment. Descriptive statistics and chi square test was done using SPSS software version 20. The level of significance was set at 0.05. More than 70% of the participants show concern and interest on self-image, 39.9% of the total study population were unhappy with the arrangement of their teeth. There was an overall lack of knowledge about the etiology and the effects of malocclusion among the participants. 60.2% of the population have not visited a dentist in the past 3 years. Many had false impressions towards orthodontic treatment. Awareness about the effects of malocclusion among young adults seems to be lacking. Most of the time, the treatment is neglected until the need for it arises.

Çapraz kapanış, açık kapanış, Sınıf II kapanış, Sınıf III kapanış, dental kontaktlara bağlı oluşan maloklüzyonlar oral fonksiyonları negatif yönde etkileyebilmektedirler. Ortodontik tedavi ile dental ve iskeletsel yapılara ilişkin problemler giderilerek; hastaların normal fonksiyon, fonasyon ve estetik değerlere kavuşması sağlanmaktadır. Ancak, ortodontik tedavi planlamaları yapılır iken, kraniyomandibüler fonksiyonlar ve bu fonksiyonları etkileyen faktörler göz önünde tutulmalıdır. Bu çalışmada, maloklüzyonların ağız fonksiyonlarını ne şekilde değiştirdiği incelenmiştir. Kraniyomandibüler hareketleri etkileyen maloklüzyonlar hakkında ileri araştırmaların yapılması, tedavi yöntemlerinin çeşitliliğinin ve verimlerinin artırılması için gereklidir.

Introduction: In a socially competitive world, an improved dental appearance is important to patients. Malocclusion has a wellknown detrimental effect on the overall facial esthetics, confidence and self-esteem, especially in girls. Patients seeking orthodontic treatment have the assumption of having pleasant facial and dental appearance at the end of treatment. Adolescence is known as a period of developmental & psychological instability, it is also the stage of self-esteem formation.

Development of a conceptual model of oral health for malocclusion patients.

To provide an empirical test of the applicability of Locker's conceptual model of oral health for malocclusion patients, and to suggest alternative models of the effect of malocclusion on well-being. Data from a survey of 323 adolescents attending for orthodontic treatment were analyzed to develop a new oral health model for malocclusion patients. Oral health-related quality of life (OHRQoL) was measured using the 14-item Oral Health Impact Profile; malocclusion was measured using the Dental Health Component (DHC) of the Index of Orthodontic Treatment Need (IOTN). Using structural equation modeling, the relationship between conceptual domains in Locker's model was explored and three models of their interrelationship tested for goodness of fit. Fit indexes for Locker's model indicated that it did not fit the data well. Therefore, a modified model was developed to incorporate additional paths between other levels to better fit the data. The best fit was provided by a model in which the direct effects of malocclusion on pain, discomfort, and handicapping-and the direct effect of pain on disability-were removed. A direct effect of functional limitation on disability was allowed. The modified Oral Health Impact Profile model proved to be a good fit to the data (root mean square error of approximation = 0.069). The pathways identified in Locker's (1988) conceptual model of oral health may not be appropriate for describing the relationships between OHRQoL constructs in individuals with malocclusion. An alternative model is proposed.

Digital lateral cephalometric assessment of maxillary sinus dimensions in different skeletal classes

Maxillary sinus is the largest of the four paranasl sinus, among the paranasal sinus its plays an important role in the formation of facial contours. Therefore, knowledge of the development and size of the maxillary sinus may be crucial for diagnosing and treating various cases of malocclusion. The purpose of this study is to evaluate the effect of malocclusion in three skeletal classes on maxillary sinus dimensions. Material and methods: Total of 120 lateral cephalograms were derived from subjects with skeletal Class I, Class II, and Class III malocclusions, classified on the basis of the A-N-B angle. Each malocclusion group consisted of 20 males and 20 females ranging in age from 12 to 16 years. Two linear measurements and three area measurements were made to evaluate maxillary sinus size. The procedure was accomplished by mean of computer and Auto Cad program. Results: it had been found that maxillary sinus dimensions were significantly Larger in male than in females in different skeletal malocclusion classes, skeletal malocclusion classes has no effect on dimensions of maxillary sinus except in male class II skeletal malocclusion. Conclusion: Skeletal classes malocclusion has no effect on dimension of maxillary sinus except in male class II skeletal malocclusion, male showed significantly greater maxillary sinus length, maxillary sinus height, and maxillary sinus area than that of female in all skeletal classes.

Relationship between Malocclusion and Heart Rate Variability Indices in Young Adults

Heart rate variability (HRV) has been used to assess sympathetic and parasympathetic modulation of heart rate. Chronic stress relates to reduced HRV. Malocclusion has effects on quality of life, which can lead to chronic stress. Therefore, we hypothesized that malocclusion, as chronic stress, may contribute to reduced HRV. The aim of this study was to investigate the relationship between malocclusion and HRV indices in healthy young adults. Thirty-seven non-smoking healthy subjects, aged 22 to 25 years, were examined. Malocclusion was defined by Angle classification. HRV indices included root mean square of successive differences, low frequency (LF), high frequency (HF) and ratio of LF to HF. The effects of malocclusion on quality of life and mental health were assessed using self-reported questionnaires, the condition-specific Oral Impacts on Daily Performances index (CS-OIDP) and the Hopkins Symptoms Checklist (HSCL), respectively. Significantly lower score of HF and higher heart rate (HR) level and CS-OIDP score were observed in subjects with malocclusion (n = 17) compared to those in the control subjects (n = 20) ( P <0.05). There was a positive correlation between HR and score of "anxiety" in HSCL ( P <0.05). The data showed an association between malocclusion and lower HRV. Based on our results, orthodontic treatment might contribute not only to improvement of oral esthetic and functional problems but also to improvement of stress and HRV indices.
