The effect of anthelmintic treatment and efficacy on the faecal microbiota of healthy adult horses

Changes to the faecal microbiota of horses associated with administration of anthelmintic drugs is poorly defined. This study included horses with cyathostomin infection where susceptibility and resistance to oxfendazole and abamectin was known. This study assessed the changes to the faecal microbiota associated with administration of two different anthelmintics in this population. Twenty-four adult horses were included. Faecal egg counts were performed on all horses prior to random allocation into abamectin (n=8), oxfendazole (n=8) or Control groups (n=8) and at Day 14 post treatment. Faecal samples were collected for microbiota analysis prior to anthelmintic administration and on Day 3 and Day 14. From each faecal sample, DNA was extracted prior to PCR amplification, next generation sequencing and analysis using QIIME2. Anthelmintic treatment was associated with changes in alpha diversity (p <0.05), with increased evenness and diversity at Day 14 and increased richness at Day 3 within the abamectin group. Differences in relative abundance of bacteria at the phyla, family and genus taxonomic levels occurred after treatment; indicating that the microbiota was altered with anthelmintic administration. The results support that anthelmintic administration and removal of cyathostomins from the large intestine of horses is associated with changes in the faecal microbiota. The results suggest that removal of cyathostomins is associated with greater differences in microbiota, compared to anthelmintic drug administration that is ineffective in reducing cyathostomin infection. Cyathostomin removal was supported by adequate reduction of faecal egg counts, determined by faecal egg count reduction testing.

Prevalence and associated factors of gastrointestinal helminthiasis of lactating cow and effect of strategic deworming on milk quantity, fat, and protein in Kucha, Ethiopia

BackgroundGastrointestinal helminthiasis poses economic impacts on the dairy sector by reducing milk production. This study aimed at estimating the prevalence of gastrointestinal helminthiasis, the burden of helminths, and appraising potential factors associated with the prevalence in lactating cows. The study was also designed to elucidate the effects of anthelmintic treatment on milk yield, milk fat, and protein content in the Kucha district.MethodsA cross-sectional and field clinical trial study designs were used. Standard parasitological techniques (floatation and sedimentation) were employed to detect cows’ infection status. McMaster and Stoll’s egg counting methods were used to estimate helminths' burden. All putative factors that might have been linked with infection were recorded by field observations and farmers' interviews. Sixty cows tested positive for the parasitic infection in the cross-sectional study design were randomly assigned into one of the two groups (dewormed Vs control). Milk yield, milk fat and protein contents were recorded in both groups on day zero and then on weekly basis. Descriptive statistics, binary logistic regression, and repeated measure ANOVA were used to analyze the data.ResultsOverall, of 422 examined cows, 150 (35.5%, 95% CI; 30.9-40.3%) were infected with at least one of the gastrointestinal helminth parasites. Strongyle nematode was the predominant type accounting for 52% of the total record. Analysis of fecal egg count (FEC) in infected cows depicts the highest record of Parampistomum egg, accounting 457.14 ± 275.45 Egg per gram (EPG) of feces. The prevalence of gastrointestinal helminthiasis in cow that had an average body condition score of 1.7 was over two fold higher (OR = 2.19, 95% CI = 1.17–4.17, P = 0.016) than in cows with 6.3 body condition score. A significant improvement in milk yield, milk fat, and milk protein was observed in dewormed cows over 28 days period.ConclusionGastrointestinal helminthiasis is threatening the welfare of lactating cows in Kucha, Ethiopia given its negative association with the body condition score. Gastrointestinal helminths are responsible for the reduction of milk yield and loss of milk fat and protein. However, an improvement in milk yield, milk fat, and milk protein after deworming using Tetrox® (Tetramisole and Oxyclozanide combination), a new drug to the area/ a drug used by relatively few farmers in the study area, proves the effectiveness of strategic deworming.

Association of anthelmintic treatment with malaria prevalence, incidence, and parasitemia: A systematic review and meta-analysis

A chronic helminth infection can alter host immune response and affect malaria infection. We conducted a systematic review and meta-analysis to find the impact of anthelmintic treatment on malaria prevalence, incidence, and parasitemia. Nine and 12 electronic databases were searched on 28th July 2015 and 26th June 2020 for relevant studies. We performed meta-analysis for malaria prevalence, incidence, parasitemia, and a qualitative synthesis for other effects of anthelmintic treatment. Seventeen relevant papers were included. There was no association between anthelmintic treatment and malaria prevalence or change of parasitemia at the end of follow up period (pooled OR 0.93, 95% CI: 0.62, 1.38, p-value=0.71 and SMD -0.08, 95%CI: -0.24, 0.07, p-value=0.30 respectively) or at any defined time points in analysis. Pooled analysis of three studies demonstrated no association between malaria incidence and anthelmintic treatment (rate ratio 0.93, 95%CI: 0.80, 1.08, p-value=0.33). Our study encourages anthelmintic treatment in countries with high burden of co-infections as anthelmintic treatment is not associated with change in malaria prevalence, incidence, or parasitemia.

Previous studies have conflicting results on the macrocyclic lactones (ML) efficacy against internal parasitism. The objective of this study was to evaluate the efficacy of milbeymycin and avermectin sub groups of ML on cow performance. Multiparous fall calving, crossbred beef cows (n = 106) were allocated randomly to 1 of 3 anthelmintic treatments: 1) Negative control (CON), in which cows did not receive an anthelmintic, 2) Injectable moxidectin (MOX) and 3) Injectable extended release eprinomectin (ERE). Body weights (BW), body condition scores (BCS), and fecal egg counts (FEC) were taken throughout the duration of the calving season to weaning on d0, d80, d162, and d217, with weaning occurring on d217. Performance data were analyzed using the MIXED procedures of SAS, and pregnancy data were analyzed using the GENMOD procedures of SAS. Significance was fixed at P &amp;lt; 0.05 and tendencies were established from 0.05 ≤ P ≤ 0.10. There was no effect of anthelmintic treatment on cow BW (P ≥ 0.57) or cow BCS (P ≥ 0.22) during the 217 d study; however, CON cows tended to have lower BCS (P = 0.08) throughout the duration of the study. Cows treated with ERE had fewer FEC compared to MOX and CON groups (P ≤ 0.001), and tended to improve pregnancy rates (c2 = 0.0546). Calf weaning weight was similar among treatments averaging 216, 225, and 223 kg regarding CON, MOX, and ERE cow treatments, respectively.

Effect of anthelmintic treatment on serum free IGF-1 and IGFBP-3: a cluster-randomized-controlled trial in Indonesia

In children, soil-transmitted helminth infections have been linked to poor nutritional status and growth retardation in association with lower levels of IGF-1. In adults, IGF-1 has an anabolic and metabolic function and is related to nutritional status. Here, we assessed the impact of helminth infection on free IGF-1 and its major binding protein, IGFBP-3, in adults. The levels of IGF-1 and IGFBP3 were measured in 1669 subjects aged ≥ 16 years, before and after receiving four rounds of albendazole 400 mg/day or matching placebo for three consecutive days. Helminth infection status was assessed by microscopy (Kato-Katz) and PCR. Serum free IGF-1 level was significantly lower in helminth-infected subjects [mean difference and 95% CI − 0.068 (− 0.103; − 0.033), P < 0.001 after adjustment for age, sex, body mass index, and fasting insulin level]. There was no difference in IGFBP-3 level between helminth infected versus non-infected subjects. In the whole study population, albendazole treatment significantly increased serum free IGF-1 level [estimate and 95% CI 0.031 (0.004; − 0.057), P = 0.024] whereas no effect was found on the IGFBP-3 level. Our study showed that helminth infection in adults is associated with lower free IGF-1 levels but not with IGFBP-3 and albendazole treatment significantly increases free IGF-1 levels in the study population.Clinical Trial Registration: https://www.isrctn.com/ISRCTN75636394.

Effects of anthelmintic treatment on production performance, carcass quality, and predominant nematode species in feedlot cattle from western Canada

The objective of this study was to determine the effects of currently used anthelmintics on production performance, carcass quality characteristics, and species diversity in western Canadian feedlot calves. Changes in climate and anthelmintic susceptibility are changing the epidemiology of gastrointestinal nematodes in livestock. However, the production impact of gastrointestinal nematodes in beef cattle from western Canada has not been studied in decades.

Exploring the species diversity of gastrointestinal nematodes in western Canadian beef cattle

The objectives of this study were to explore herd level species diversity of gastrointestinal nematode (GIN) communities in calves from western Canada and the effects of anthelmintic treatment on their relative species abundance in feedlot steers. Coinfection with multiple GIN species is common in cattle. Exploring the GIN diversity is important to understand parasite epidemiology and particularly the response to control programs; however, research is limited in Canadian beef cattle.

Little impact of over‐winter parasitism on a free‐ranging ungulate in the high Arctic

Abstract Macroparasites have a central place in wildlife ecology because they have the potential to regulate host populations through effects on reproduction and/or survival. However, there remains a paucity of studies that have demonstrated the regulatory role of these parasites in free‐ranging animals. Previous work on Svalbard reindeer demonstrated that the experimental removal of the parasitic gastrointestinal nematode Ostertagia gruehneri transmitted in summer, improved reindeer fecundity, and that the species was capable of mediating a density‐dependent reduction in fecundity. The main coexisting parasitic nematode in the system, Marshallagia marshalli, exhibits a contrasting life history with low abundances in summer and an increase during the winter. Here, we examine the impact of this over‐winter infection on fitness of female reindeer in the same population. Worm burdens were removed at the start of the winter in October using a novel delayed‐release anthelmintic bolus. Body mass and pregnancy rates were recorded for animals recaptured five (February) and seven (April) months later. The sub‐set of treated animals recaptured in February was treated again with anthelmintic to remove newly acquired nematodes. The experiment was replicated over six winters (2006/2007‐2011/2012) with a total of 343 recaptures, comprising 145 individual animals. We found little effect of anthelmintic treatment on reindeer body mass or pregnancy rates during the winter, suggesting that nematode infection during this time does not significantly affect fitness of Svalbard reindeer. Our studies are consistent with marked virulence nutritional trade‐offs in this system and highlights the need to integrate studies of parasite co‐infection, interspecific competition and community ecology to understand parasite impacts on hosts. Effect of anthelmintic treatment on leptin, adiponectin and leptin to adiponectin ratio: a randomized-controlled trial

Emerging evidence suggests that helminths might confer protection against the development of type 2 diabetes. We aimed to assess the role of adipokines in mediating the effect of helminths on insulin resistance. Serum samples were obtained from a randomized-controlled trial of anthelmintic treatment in an area endemic for soil-transmitted helminths (STH), Flores Island, Indonesia. In STH-infected subjects, anthelmintic treatment significantly increased the ratio of leptin to adiponectin (treatment effect factor (95% confidence interval (CI)), P-value for interaction: 1.20 (1.06–1.35), P=0.010), which largely stemmed from a significant reduction in adiponectin (0.91 (0.85–0.98), P=0.020) and a trend for an increase in leptin level (1.10 (1.00–1.21), P=0.119). No significant effect on resistin level was observed. This increase in leptin to adiponectin ratio seemed to contribute to the observed effect of deworming on increased insulin resistance (IR) as adjustment for leptin to adiponectin ratio attenuated the effect on IR from 1.07 (1.01–1.14, P=0.023) to 1.05 (0.99–1.11, P=0.075). Anthelmintic treatment in STH-infected subjects increases leptin to adiponectin ratio which may in small part contribute to the modest increase in IR. Further studies will be needed to assess the effect of the changes in adipokine levels on the host immune response and metabolism.

Monepantel in the control of Haemonchus spp. and Trichostrongylus spp. and possible side effects of treatment in naturally infected sheep

Sheep breeding represents an important economic source in many countries; however, anthelmintic resistance is an important limiting factor for sheep breeding development, due to its effects in animal performance. Monepantel is an active principle derived from an amino-acetonitrile class, with proven efficacy in the control of gastrointestinal parasites. However, the side effects associated to this treatment remain unknown. Thus, the aim of this study was to evaluate the effect of anthelmintic treatment with monepantel as well as evaluate the hepatic function, proteic and lipid metabolism, and antioxidant/oxidant levels in serum of naturally infected sheep with helminths. Sixteen naturally infected animals were used; all subjects had similar parasitic degree (determined by McMaster technique) on day 0. The animals were divided in two groups (A and B) with eight animals each. Group A did not receive any treatment (control group), while group B received a single dose of monepantel (2.5 mg/kg) on day 0 of the experiment. Fecal exams performed on days 4 and 8 post-treatment (PT) revealed a significant reduction (p < 0.05) in the number of eggs per gram of feces (EPG) in the group B where animals were treated with monepantel. On days 4 and 8 PT, seric reactive oxygen species (ROS) and superoxide dismutase (SOD) activity increased (p < 0.05), while the catalase (CAT) activity in serum decreased (p < 0.05) within the group B. Based on these results, we can conclude that monepantel is very likely to be a safe drug, effective in the control of helminths, and possesses direct effect on SOD activation in sheep.

Effects of anthelmintic treatment and feed supplementation on parasite infections and morbidity parameters in Cambodian cattle

Helminth infections are the cause of morbidity in Cambodian cattle but other factors such as nutritional deficiencies and concurrent diseases may enhance the effects of parasites. The present study aimed to investigate the impact of anthelmintic treatment, feed supplementation, or both on gastrointestinal strongyle (GIS) and trematode infections as well as on morbidity parameters in Cambodian village cattle. At the beginning of the dry season, cattle populations in six villages were randomly assigned to a group: (A) receiving anthelmintic treatment (ivermectin+clorsulon) at week 0; (P) feed pellet supplementation during week 0–13 or both (AP). On five visits (week 0–29), faecal and blood samples were obtained for parasitological examination and haematocrit determination, respectively. Body condition (BCS), hind quarter fouling (HQFS), diarrhoea (DS), and conjunctiva colour (FAMACHA©) were scored and heart girth circumference was determined. To investigate the impact of treatment over time (week 0–29), a mixed model was used with treatment, time, and their interaction as fixed effects, and animal and village as random factors. At baseline, the proportion of GIS positive animals was high (67.9%), whereas trematode infections were low (Paramphistomum: 8.8%; Fasciola: 2.6%). Very thin to emaciated cattle (BCS 1–2) were more prevalent (11.4%) and FAMACHA© scores of ≤3 or below (65.8%) less prevalent than in an earlier study in the region. A Time ⨯ Treatment interaction was present for faecal egg counts (FEC) of GIS, GIS prevalence (both p<0.0001), PCV (p=0.0034), DS (p=0.0086) and HQFS (p=0.0241). For GIS FEC, treatment groups differed at a specific time point, with levels of treatment group P being higher than in A at week 6 (p=0.0054). For Paramphistomum prevalence as well as FAMACHA© scoring, heart girth and BCS, the interaction between treatment and time was not significant, yet, time in itself had a significant impact on all (p<0.0001). The beneficial effects of protein supplementation were unclear from the current study.

Effects of anthelmintic treatment on ewe feed intake, digestion, milk production and lamb growth

SUMMARYTwenty Small Tailed Han (STH) and 20 Ujumqin (UJU) ewes naturally infected with gastrointestinal nematodes were randomly assigned to one of four treatments arranged in a 2 × 2 factorial design, receiving anthelmintic treatment (AT) or non-anthelmintic treatment (NonAT) prior to lambing. After lambing, the effects of AT on feed intake, digestion and milk yield in ewes, and the growth rates of lambs fed their mother's milk were assessed for 28 days. Faecal samples were collected to determine faecal egg counts (FECs), milk was collected to measure milk yield and ewes and lambs were weighed to quantify daily body weight change. The results showed that AT significantly increased ewe dry matter intake (2411 g/d for AT and 2209 g/d for NonAT) and decreased FECs (50 eggs/g for AT and 2655 eggs/g for NonAT). All ewes lost weight after lambing, but body weight loss in the AT (43 g/d) was significantly less than in NonAT (84 g/d), and STH ewes (70 g/d) lost more weight than UJU ewes (58 g/d). Anthelmintic-treated ewes produced more milk for their lambs to consume. However, the extent of these positive effects of AT differed between STH and UJU ewes. The average daily body weight gain of lambs in AT was higher than those in NonAT. In conclusion, effective AT in ewes before lambing benefits subsequent lactation in ewes and growth rate in lambs.

The impact of albendazole treatment on the incidence of viral- and bacterial-induced diarrhea in school children in southern Vietnam: study protocol for a randomized controlled trial.

BackgroundAnthelmintics are one of the more commonly available classes of drugs to treat infections by parasitic helminths (especially nematodes) in the human intestinal tract. As a result of their cost-effectiveness, mass school-based deworming programs are becoming routine practice in developing countries. However, experimental and clinical evidence suggests that anthelmintic treatments may increase susceptibility to other gastrointestinal infections caused by bacteria, viruses, or protozoa. Hypothesizing that anthelmintics may increase diarrheal infections in treated children, we aim to evaluate the impact of anthelmintics on the incidence of diarrheal disease caused by viral and bacterial pathogens in school children in southern Vietnam.Methods/designThis is a randomized, double-blinded, placebo-controlled trial to investigate the effects of albendazole treatment versus placebo on the incidence of viral- and bacterial-induced diarrhea in 350 helminth-infected and 350 helminth-uninfected Vietnamese school children aged 6–15 years. Four hundred milligrams of albendazole, or placebo treatment will be administered once every 3 months for 12 months. At the end of 12 months, all participants will receive albendazole treatment. The primary endpoint of this study is the incidence of diarrheal disease assessed by 12 months of weekly active and passive case surveillance. Secondary endpoints include the prevalence and intensities of helminth, viral, and bacterial infections, alterations in host immunity and the gut microbiota with helminth and pathogen clearance, changes in mean z scores of body weight indices over time, and the number and severity of adverse events.DiscussionIn order to reduce helminth burdens, anthelmintics are being routinely administered to children in developing countries. However, the effects of anthelmintic treatment on susceptibility to other diseases, including diarrheal pathogens, remain unknown. It is important to monitor for unintended consequences of drug treatments in co-infected populations. In this trial, we will examine how anthelmintic treatment impacts host susceptibility to diarrheal infections, with the aim of informing deworming programs of any indirect effects of mass anthelmintic administrations on co-infecting enteric pathogens.Trial registrationClinicalTrials.gov: NCT02597556. The effect of anthelmintic treatment on coccidia oocyst shedding in a wild mammal host with intermittent cestode infection.

While hosts are routinely exploited by a community of parasite species, the principles governing host responses towards parasites are unclear. Identifying the health outcomes of coinfections involving helminth macroparasites and microparasites is one area of importance for public and domestic animal health. For instance, it is controversial how deworming programmes affect incidence and severity of such important microparasite diseases as malaria. One problem is that most study systems involve domestic and laboratory animals with conditions hardly comparable to those of free-living animals. Here, we study the effect of anthelmintic treatment on coccidia infection intensity in wild Alpine marmots, M. marmota. Our results lend support to the hypothesis that helminth infection has a positive effect on concurrent microparasite infection. However, our work also points to the fact that within-host interactions between helminths and microparasites are context-dependent and can turn to negative ones once helminth burdens increase. Our study suggests that coccidia benefit from intermittent helminth infection in marmots due to the protective effects of helminth infection only during the early phase of the host's active season. Also, the marmot's response towards coccidia infection appears optimal only under no helminth infection when the host immune response towards coccidia would not be compromised, thereby pointing to the importance of regular intestinal helminth elimination by marmots just before hibernation.

Integrating Fasciolosis Control in the Dry Cow Management: The Effect of Closantel Treatment on Milk Production

The liver fluke Fasciola hepatica is a parasite of ruminants with a worldwide distribution and an apparent increasing incidence in EU member states. Effective control in dairy cattle is hampered by the lack of flukicides with a zero-withdrawal time for milk, leaving the dry period as the only time that preventive treatment can be applied. Here, we present the results of a blinded, randomized and placebo-controlled trial on 11 dairy herds (402 animals) exposed to F. hepatica to 1) assess the effect of closantel treatment at dry-off (or 80–42 days before calving in first-calving heifers) on milk production parameters and 2) evaluate if a number of easy-to-use animal parameters is related to the milk production response after treatment. Closantel treatment resulted in a noticeable decrease of anti-F. hepatica antibody levels from 3–6 months after treatment onwards, a higher peak production (1.06 kg) and a slightly higher persistence (9%) of the lactation, resulting in a 305-day milk production increase of 303 kg. No effects of anthelmintic treatment were found on the average protein and fat content of the milk. Milk production responses after treatment were poor in meagre animals and clinically relevant higher milk production responses were observed in first-lactation animals and in cows with a high (0.3–0.5 optical density ratio (ODR)), but not a very high (≥0.5 ODR) F. hepatica ELISA result on a milk sample from the previous lactation. We conclude that in dairy herds exposed to F. hepatica, flukicide treatment at dry-off is a useful strategy to reduce levels of exposure and increase milk production in the subsequent lactation. Moreover, the results suggest that treatment approaches that only target selected animals within a herd can be developed based on easy-to-use parameters.

The effect of anthelmintic treatment during pregnancy on HIV plasma viral load: results from a randomized, double-blind, placebo-controlled trial in Uganda.

To investigate the effect of helminth infections and their treatment during pregnancy on HIV load, we conducted a 2 × 2 factorial randomized controlled trial of albendazole versus placebo and praziquantel versus placebo in pregnant women in Entebbe, Uganda. Two hundred sixty-four HIV-infected pregnant women from the Entebbe Mother and Baby Study (ISRCTN 32849447) were included in this analysis. Women were tested for helminth infections at enrollment, and mean HIV load was compared between infected and uninfected groups. The effect of anthelmintic treatment on HIV load was evaluated at 6 weeks after treatment and at delivery using linear regression and adjusting for enrollment viral load. Hookworm and Trichuris infections were associated with higher mean viral load at enrollment [adjusted mean difference 0.24 log10 copies/mL, 95% confidence interval (CI): 0.01 to 0.47, P = 0.03, and 0.37 log(10) copies/mL, 95% CI: 0.00 to 0.74, P = 0.05, respectively]. There were no associations between viral load and other helminth species. There was some evidence that albendazole reduced viral load at 6 weeks after treatment (adjusted mean difference -0.17, 95% CI: -0.36 to 0.01, P = 0.07); however, this effect did not differ according to mother's hookworm infection status and had diminished at delivery (adjusted mean difference -0.11, 95% CI: -0.28 to 0.07, P = 0.23). There was no effect of praziquantel treatment on HIV load at any time point. Infection with some soil-transmitted helminth species is associated with increased HIV load in pregnancy. Treatment with albendazole causes a small decrease in HIV load; however, this may not represent a direct effect of worm removal.

The effect of repeated, four-weekly eprinomectin treatment on milk production in pasture-based, seasonally-calving dairy cattle

A randomised clinical trial from the North Island of New Zealand was conducted to assess the effect of repeated anthelmintic treatment on milk production, and to assess factors that affect treatment response. Nine hundred and twenty three multiparous, lactating dairy cattle from three pasture-based, spring-calving dairy herds were enrolled in this trial. Within each herd, cattle were stratified on age and calving date, and were randomly allocated to treatment (n=319) or control (n=604) groups. The treatment group received ≥0.05mg/kg of topical eprinomectin every 28 days for eight treatments during lactation. Pooled-milk from treated cows and bulk-milk samples were obtained at each treatment and analysed with an Ostertagia antibody ELISA, expressed as optical density ratios (ODR). Bi-monthly milk data were collected and expressed as energy-corrected milk (kg/day; ECM). A linear mixed model was used to analyse ECM, with cow as the random effect. The effect of anthelmintic treatment on days from calving, and start-of-mating, to conception were analysed with Cox-proportional hazard models.ODR values ranged from 0.6 to 1.3; there were no differences in ODR between herds (p=0.12), or between pooled-milk from treated cows and bulk-milk (p=0.26). Repeated treatments had no effect on daily ECM yields (p=0.74). However, there was a significant treatment×herd interaction (p=0.03); treatment increased ECM in one herd by 0.781kg/cow/day (p=0.015), but resulted in a non-significant decrease in the other two herds. A curvilinear interaction existed between days-in-milk and treatment response (p=0.039); the greatest treatment effect occurred during mid-lactation. Previous year milk production (p=0.46) and age (p=0.11) did not influence the effect of treatment on ECM. Treatment had no effect on any reproductive parameter. In conclusion, under New Zealand pastoral conditions, anthelmintic treatment increased milk production in one herd, but had no effect in two other herds. Further work is needed to identify why this variation in gastro-intestinal parasitism occurs.

The effects of anthelmintic treatments against gastrointestinal nematodes on the performance of breeding ewes and lambs on pasture in semi-arid Kenya

The effects of anthelmintics treatments in controlling gastrointestinal nematodes in breeding ewes in a semi-arid area of Kenya were determined. The study carried out during two breeding seasons, between June 2000 and December 2001 where albendazole was administered to groups of ewes, 2 weeks before mating, 3 weeks to lambing and mid lactation indicated significantly lower nematode egg counts in treated than untreated groups of ewes. In the first breeding season, reduced rainfall resulted in pasture scarcity and weight loss in both groups of ewes through out the gestation period, but losses were higher for the untreated group. In the second season, both groups of ewes showed a steady increase in weight gain during the gestation period and post-partum, but weight gains were higher in the treated group. In lambs, weight gains at 6 weeks were higher for treated ewes than control groups, in both breeding seasons. The results of this trial indicate that anthelmintic treatments in breeding ewes in the study area are beneficial in reducing gastrointestinal nematode infections and improving performance of the ewes and their lambs. In addition to the treatments, breeding ewes should be given feed supplementation particularly during periods of pasture scarcity.

Environmental determinants of total IgE among school children living in the rural Tropics: importance of geohelminth infections and effect of anthelmintic treatment

BackgroundThe environmental factors that determine the elevated levels of polyclonal IgE observed in populations living in the Tropics are poorly understood but may include geohelminth infections. We investigated the association between geohelminth infections and total IgE levels in school children in rural tropical Ecuador, and assessed the effect on IgE of repeated anthelmintic treatments over a period of 12 months. The study was nested within a cluster-randomized study that randomized 68 schools to receive either 400 mg of albendazole every 2 months over a year or no treatment. We studied random samples of children completing follow-up and representing four groups stratified by the presence of geohelminth infection at baseline and treatment allocation. We measured levels of total IgE and anti-A. lumbricoides IgG (used as a measure of past and current geohelminth infectious exposure) in blood samples collected at the start of the study and after 12 months.ResultsWe observed elevated levels of total IgE (compared to standard reference values) at the start of the study in this population of school children (geometric mean, 1,004 IU/mL, range 12 to 22,608 IU/mL)) and baseline IgE levels were strongly associated with parameters of geohelminth infection but not with age, nutritional and socioeconomic status. After 12 months, levels of IgE fell significantly in the treatment (by 35.1%) and no treatment (by 10.4%) groups, respectively, but the fall was significantly greater in the treatment group. Falls in IgE were independently associated with albendazole treatment, having a baseline geohelminth infection and with high baseline levels of anti-A. lumbricoides IgG. Increases in IgE at 12 months were associated with the presence of geohelminth infections and increasing levels of anti-A. lumbricoides IgG at 12 months independent of treatment allocation.ConclusionThe data provide evidence that geohelminth infections are an important determinant of total IgE in school children in the rural Tropics and that periodic anthelmintic treatments over 12 months are associated with reductions in IgE. The failure of anthelmintic treatment to reduce IgE levels to that considered normal in industrialized countries may be attributed to continued exposure of children to geohelminths or to the effects of infections in early life in programming a long-lasting Th2-biassed immunity.
