The politics and economics of AIDS

During the first years of the Reagan administration, and the first years of the AIDS epidemic, research was the only thing that the Congress and the administration ever talked about. There have been 24 Congressional hearings in my subcommittee on AIDS. I would say probably the first seven or eight were on the minimum adequacy of the administration's budget for biomedical research and drug development. There were threats of vetoes of appropriations bills by Mr. Reagan because there was too much AIDS research in the bills. It is no longer avant-garde to support AIDS research funding; it is now mainstream politicsright, left and center all support it. And I think it is a great step forward from the years when Mr. Reagan threatened to veto appropriations bills to the years when Mr. Bush campaigns that AIDS research will not be part of the flexible freeze; that AIDS research money will grow during the years even if other health programs are constrained. Mr. Bush has gone quite far. The criticisms I voice ofthe Public Health Service and of the administration are of the Reagan administration years. There are still many questions left for the Congress and for the Public Health Service in AIDS research. There is the continued limitation of prophylactic and preventive trials, there is the continued limitation ofpediatric drug trials, and there are some very hard legal, ethical, and moral questions about the ethics of vaccine development. These will continue but will not be the full-scale battles, that the Congress had over budgets and simple funding. I think now we can safely say that AIDS research is mainstream politics and on its way.

Studying the Cost of HIV-Related Illnesses: Reflections on the Moving Target

Although medical care costs of the HIV epidemic by 1991 may reach $6 billion, or 1.2 percent of all estimated personal health care expenditures in the United States, costs per patient of treating AIDS appear to be declining. Calculating the epidemic's costs is difficult, however, in that data are lacking on health care expenditures for HIV-infected persons other than those with AIDS, intravenous drug users, women, and children. Shifts in demographic segments affected, changes in medical treatments, and diffusion beyond initial urban centers will alter the economics of AIDS. Prospective studies at both national and local levels are needed to gauge the epidemic's costs and demands on health services.

The Public Health Challenge

A mere six years after the first cases were officially diagnosed, AIDS has reached epidemic—some would say plague—proportions. And the number of cases and deaths is expected to mount. Once dismissed as a disease of homosexuals, AIDS is moving slowly but steadily into the heterosexual population. According to Centers for Disease Control tallies, nearly 45,000 have AIDS and about 26,000 Americans have died from the disease. CDC believes that another 1.5 million Americans are infected with the virus; many are symptomless. CDC estimates that, by 1991, 179,000 Americans will have died from the disease—three times the number killed in the Vietnam war. Efforts to control AIDS are raising countless legal and ethical issues. Health care, disability, and life insurance costs are expected to rise sharply as the number of AIDS cases increases.
