Economic impact of rheumatoid arthritis (RA) biotherapies in France

Objective Determining the economic impact of rheumatoid arthritis (RA) biotherapies in France. Method The number of patients on RA biotherapy in France was estimated from the French national medical information system program (PMSI) database using the 2007 hospital data. The cost of each biotherapy was calculated on a theoretical basis (French national health authority (HAS) recommendations) and on real-life setting, using ‘real-life’ setting data. In order to calculate the economic impact of the biotherapies, the cost of management with each biotherapy was applied to the RA patient population taking into account the market share of each biotherapy. Results The number of patients with RA estimated was 15,873. Management costs ranged from 11,576 to 21,128 € for the theoretical management scenario and from 6,451 to 19,618 € for the real-life scenario. The overall cost was 222 million € (real-life setting). TNF antagonists (adalimumab, etanercept, infliximab) were prescribed for 82% of the patient population and accounted for 80% of the annual overall cost of theoretical management and 84% of the cost of the real-life setting, respectively. Other biotherapies (abatacept, rituximab) were prescribed for 18% of the patients and accounted for 20 and 16% of the annual overall cost of the theoretical setting and real-life setting. Outpatient biotherapy was prescribed for 61% of the patient population and generated 68 and 71% of the total costs. Conclusion The data constitutes an initial inventory of the economic impact of RA biotherapies in France.

Annual costs of rheumatoid arthritis in Turkey

Objective of the present study is aimed to determine costs of rheumatoid arthritis (RA) based on reimbursement agencies perspective [Social Security Institution (SSI)] in Turkey. The international clinical guidelines for RA are followed for analysing the direct costs. Data were collected from hospital bills, social security institution price lists, and Ministry of Health drug price list. Direct costs of RA patients were estimated as euro 2,669.14 patient/year. Outpatient costs were found to be euro 240.40. Routine tests during the year were calculated as euro 98.85. Ten percent of patients are hospitalized per year, and 0.62% of these patients received arthroplasty and/or other interventions. The cost during hospital stay was euro 87.76. euro 2,238 was determined as being paid per year for medication alone (including anti-TNF) and euro 4 is spent on auxiliary materials annually. Our data show a remarkable economic impact of RA over society. We hope that the cost of RA studies will help package price practices for reimbursement agencies.

Economic burden of rheumatoid arthritis: a systematic review.

To summarize the state of knowledge with regard to the economic impact of rheumatoid arthritis (RA) and to highlight any weaknesses in the work conducted to date, so as to inform future RA cost-of-illness studies. Four computerized literature databases were searched to identify all the literature relevant to this review. Seven elements indicating a quality cost-of-illness study were established and used to appraise the literature identified critically. Where possible, costs reported by the different studies were converted to 1996 US dollars using the consumer price index for medical care. Total average medical costs were reported to range from US$5720 (UK pound3575) to US$5822 (UK pound3638). Medication constituted between 8 and 24% of total medical costs, physician visits between 8 and 21%, and in-patient stays between 17 and 88%. The average number of days absent from work due to a person's RA was reported to range from 2.7 to 30 days/year. The economic impact of RA in terms of cost was reported to be substantial by all studies reviewed. However, methodological problems meant that discrepancies in the average (per person) annual cost of RA existed across studies.
