Clinical Effect of Integrated Traditional Chinese and Western Medicine Treatment on Early Postoperative Inflammatory Small Bowel Obstruction

Objective: To observe the clinical effect of integrated traditional Chinese and Western medicine treatment on patients with early postoperative inflammatory small bowel obstruction (EPISBO). Methods: 86 EPISBO cases admitted from October 2021 to October 2023 were grouped based on the double-blind method. The control group received conventional comprehensive treatment with Western medicine, and the observation group received integrated treatment with traditional Chinese and Western medicine. The clinical treatment effectiveness, general observation indicators, inflammatory factors, and adverse reactions were compared between the two groups after treatment. Results: The total effective rate of clinical treatment of the observation group was higher than that of the control group. The post-operative bowel sound recovery time, anal exhaust time, first defecation time, and hospitalization time of the observation group were shorter than those of the control group. The post-treatment level of inflammatory factors of the observation group was lower than that of the control group. These results were statistically significant (P < 0.05). There were no serious adverse reactions in either group. Conclusion: Implementing integrated traditional Chinese and Western medicine therapy can help post-appendectomy patients with EPISBO improve clinical treatment effectiveness, accelerate symptom improvement, and reduce inflammation without causing serious adverse reactions. Therefore, it is worthy of promotion and clinical application.

Efficacy of ileus tube combined with meglumine diatrizoate in treating postoperative inflammatory bowel obstruction after surgery.

Early postoperative inflammatory small bowel obstruction (EPISBO) is easy to be complicated after colorectal cancer surgery. Both intestinal obstruction catheter and meglumine can treat EPISBO. To investigate the efficacy of an intestinal obstruction tube combined with meglumine diazo in treating EPISBO of colorectal cancer. Data from 60 patients with colorectal cancer and intestinal obstruction admitted to the Proctology Department of our hospital from April 2018 to May 2022 were collected and analyzed and divided into three cohorts according to different treatment regimens. Cohort A (n = 20) received a transnasal intestinal obstruction catheter with panumglumine, and cohort B (n = 20) received a transnasal intestinal obstruction catheter with liquid paraffin. Cohort C (n = 20) received oral treatment with meglumine. The clinical efficacy, first exhaust/defecation time, length of hospital stay, gastrointestinal decompression time, relief time of abdominal pain, and relief time of abdominal distension were compared among the three cohorts. The levels of C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), monocyte chemotactic protein-1 (MCP-1), serum albumin, and transferrin were compared among the three cohorts before and after treatment. The occurrence of adverse reactions in the three cohorts was compared. Compared with cohort C, the successful treatment rate of cohort A was significantly higher. There were statistically significant variations in the time of first exhaust/defecation, length of hospital stays, gastrointestinal decompression time, relief time of abdominal pain, and relief time of abdominal distention among the three cohorts. Compared with cohort C, cohort A's first exhaust/defecation time, hospitalization time, gastrointestinal decompression time, abdominal pain relief time, and abdominal distension relief time was reduced (P < 0.05). After treatment, serum CRP, TNF-α, IL-6, and MCP-1 expression levels increased, and serum albumin and serum transferrin levels increased in the three cohorts. The serum albumin level in cohort A was higher than in cohort C. Compared with cohort B and cohort C, the serum transferrin level in cohort A increased (P < 0.05). Compared with cohort C, the total incidence of adverse reactions in cohorts A and B was significantly higher (P < 0.05). The incidence of adverse reactions was similar between cohort A and cohort B. Using an ileus tube combined with meglumine diatrizoate can effectively treat postoperative inflammatory ileus obstructions after surgery colorectal cancer and improve prognosis, inflammatory response, and nutritional status.

Clinical efficacy and safety of somatostatin in the treatment of early postoperative inflammatory small bowel obstruction: A protocol for systematic review and meta analysis.

Background:As one of the complications after abdominal operation, early postoperative inflammatory small bowel obstruction (EPISBO) is a great trouble for many patients. The use of somatostatin in treating this disease had been widely reported, but its efficacy and safety were controversial. Therefore, the present research carried out a systematic review of the clinical efficacy and safety of somatostatin in treating EPISBO.Methods:Computer retrieval was conducted in foreign databases (including PubMed, The Cochrane Library, and Embase) and Chinese database (including Sino Med, CNKI, VIP, and WangFang Data), supplementary search for the literatures included was performed, and manual retrieval was performed in abstracts, books, and non-electronic magazines related to the present research to ensure the recall rate. Among all republished relevant experimental studies in Chinese and English from January 1, 1996 to February 1, 2020, randomized controlled trials on the curative efficacy of somatostatin for EPISBO were collected. The evaluation measures included patients’ total effective rate, peristaltic sound recovery time, time of disappearance of abdominal pain, time of first defecation after operation, drainage of gastrointestinal decompression and length of stay after treatment. The literatures were selected by two investigators independently to extract data according to the inclusion and exclusion criteria, Bias Risk Assessment Tool recommended by Cochrane Review Hand book 5.2 was used to assess literature quality, and meta-analysis was carried out by using soft Stata 12.0.Results:This study will scientifically and effectively analyze the clinical efficacy and safety of somatostatin in the treatment of EPISBO through systematic review and meta-analysis.Ethics and dissemination:The results of this study will be published in a peer-reviewed SCI journal to provide evidence-based medical evidence for the clinical treatment of early postoperative inflammatory bowel obstruction.Protocol and registration:Open Science Framework (https://osf.io, OSF), registration number: ryd2g.

The evaluation measures included the total effectiveness rate among patients, peristaltic sound recovery time, time of disappearance of abdominal pain, time until first defecation after operation, drainage of gastrointestinal decompression and length of stay after treatment. The studies were selected by two investigators independently to extract data according to the inclusion and exclusion criteria. The, Bias Risk Assessment Tool recommended of the Cochrane Review Hand book 5.2 was used to assess literature quality, and a meta-analysis was carried out by using Stata 12.0 software. Results: Thirty papers involving 2,257 cases were included. The meta-analysis indicated that for the treatment of EPISBO, somatostatin combined with basic therapy compared with basic treatment alone promotes peristaltic sound recovery [SMD=-1.10, 95% CI (-1.50, -0.69), P =0.001&lt;0.01] and a higher total efficiency rate [RR=1.38, 95% CI (1.27, 1.48), P &lt;0.00001]; and reduces the duration of abdominal pain [SMD=-1.55, 95% CI (-2.01, -1.09), P &lt;0.00001] and time until first defecation after operation [SMD=-1.66, 95% CI (-2.71, -0.60), P &lt;0.00001]. Early use of somatostatin reduces drainage from gastrointestinal decompression [SMD=-6.23, 95% CI (-7.64, -4.82), P &lt;0.00001] and the length of stay [ n =9, SMD=-1.59, 95% CI (-2.11, -1.07), P &lt;0.00001]. Conclusion: The current evidence shows that for EPISBO patients in China, somatostatin combined with basic therapy could clearly shorten the duration of abdominal pain, time until defecation recovery, length of stay and peristaltic sound recovery as well as significantly reduce gastrointestinal decompression., However, the present results indicate that there is no difference between the 2 methods in regard to improving the total effectiveness rate. Therefore, the conclusions of the present research proves that somatostatin combined with basic therapy can be used as an effective drug for treating EPISBO in the clinic, but high-quality RCT research is still needed in the future to validate these findings.

Recognition of the diagnosis and treatment of small intestinal obstruction

Small intestinal obstruction is a common and frequent disease in general surgery. Delayed diagnosis and surgical timing, and inaccurate treatment strategy will lead to life-threatening complications. Therefore, how to diagnose and treat quickly and accurately is still a difficult problem in current gastrointestinal surgery. This paper summarizes and analyzes the diagnosis and treatment strategies of small intestinal obstruction based on current laboratory technology, medical imaging technology, perioperative management and improvement of surgical concepts. At the same time, early postoperative inflammatory small bowel obstruction, radioactive enteritis with intestinal obstruction, primary abdominal cocoon, primary pseudo obstruction are discussed according to the long-term experience of the author's department. The aim is to establish a reasonable and effective strategy for the diagnosis and treatment of common and special small intestinal obstruction.

Role of the small intestinal decompression tube and Gastrografin in the treatment of early postoperative inflammatory small bowel obstruction

To evaluate the role of the small intestinal decompression tube (SIDT) and Gastrografin in the treatment of early postoperative inflammatory small bowel obstruction (EPISBO). Twelve patients presented EPISBO after abdominal surgery in our department from April 2011 to July 2012. Initially, nasogastric tube decompression and other conventional conservative treatment were administrated. After 14 days, obstruction symptom improvement was not obvious, then the SIDT was used. At the same time, Gastrografin was injected into the small bowel through the SIDT in order to demonstrate the site of obstruction of small bowel and its efficacy. In 11 patients after this management, obstruction symptoms disappeared, bowel function recovered within 3 weeks, and oral feeding occurred gradually. Another patient did not pass flatus after 4 weeks and was reoperated. After postoperative follow-up of 6 months, no case relapsed with intestinal obstruction. For severe and long course of early postoperative inflammatory intestinal obstruction, intestinal decompression tube plus Gastrografin is safe and effective, and can avoid unnecessary reoperation.

Diagnosis and treatrment of 68 patients with early postoperative inflammatory small bowel obstruction

Objective To investigate the clinical features of early postoperative inflammtory small bowel obstruction,and discuss its diagnosis,treatment and preventive measures.Methods The clinical data of 68 cases of early postoperative inflammatory small bowel obstruction were analyzed retrospectively.Results Within the 68 cases,65 cases were cured by non-operative treatment including gastrointestinal decompression,anti-inflammatory drugs,somatostatin,total parenteral nutrition(TPN) and traditional Chinese medicine.The mean time from the onset of symptoms to the recovery of bowel function was 14 days.Three cases were cured by transferring to operation.One case of them was transferred to operation for strangulated intestinal obstruction during the period of conservative treatment.One case underwent oper.ion because the patient didn t recover after 4 weeks expectant treatment.One case underwent operation for the impatience with the conservative treatment of both the doctors and the patient.Conclusion Non-operative therapy should be the first choice for the treatment of early postoperative inflammatory small bowel obstruction,and its therapeutical effect was well-accepted. Spiral CT diagnosis of early postoperative inflammatory small bowel obstruction

To discuss the computerized tomography (CT) finding of early postoperative inflammatory small bowel obstruction (EPISBO) so as to enhance its diagnostic level. The CT findings of 35 EPISBO cases proved by surgery were retrospectively analyzed from August 2003 to October 2008. The main CT findings were as follows: (1) Incomplete small bowel obstruction; (2) Thickening and edema of focal or global bowel wall; (3) Local or diffuse effusion mainly around operative incision; (4) Without definite obstruction point or bent adhesive point; (5) Locally ring-shaped or delayed enhancement during an enhancement scan. Spiral CT has an important value in the diagnosis of EPISBO.

Clinical analysis of 22 cases with early postoperative inflammatory small bowel obstruction

Objective To investigate the principles of diagnosis and treatment of early postoperative inflammatory small bowel obstruction(EPISBO).Methods The clinical data of 22 patients admitted to our hospital from January 2000 to June 2009 were analysed for early postoperative inflammatory small bowel obstruction.Results 22 cases were cured in a mean period of 15 days through gastrointestinal decompression,somatostatin,TPN and other medications.There was no reoperations.Conclusion For EPISBO,conservative treatment should be adopted first,somatostatin and TPN has batter results. 目的 探讨术后早期炎性肠梗阻的诊断和治疗措施.方法 回顾性分析我院1996年1月至2006年1月收治的61例术后早期炎性肠梗阻患者的临床资料.结果 54例患者经保守治疗痊愈,平均住院时间10.2 d.7例手术治疗后痊愈,其中4例因保守治疗时出现绞窄性肠梗阻而手术,3例保守治疗40 d无效而手术.结论 术后早期炎性肠梗阻应以保守治疗为主,胃肠外营养及生长抑素的应用具有较佳的疗效。
