The association between left atrial appendage emptying velocity and atrial fibrillation recurrence after radiofrequency catheter ablation in patients with early persistent atrial fibrillation

ObjectiveIn the present study, we aimed to explore the association between left atrial appendage emptying velocity (LAAEV) measured by transesophageal echocardiography and atrial fibrillation (AF) recurrence after radiofrequency catheter ablation (RFCA) in patients with early persistent atrial fibrillation (PeAF).MethodsWe retrospectively analyzed patients with early PeAF who underwent their initial ablation procedure. The echocardiographic and clinical data of the enrolled patients were collected and recorded prior to the operation. Following adjustment for confounding factors, we investigated the relationship between the LAAEV and AF recurrence in patients with early PeAF after radiofrequency ablation.ResultsThe proportions of AF recurrence in the low, medium, and high LAAEV groups were 48.8%, 16.0%, and 13.1%, respectively. After adjusting for potential confounding factors, we observed a gradual decrease in the risk of AF recurrence with increasing LAAEV (odds ratio: 0.882, 95% confidence interval: 0.842—0.924, p<0.001). This trend was statistically significant (p<0.001), particularly when comparing the high and low LAAEV groups (odds ratio: 0.033, 95% confidence interval: 0.009―0.116, p<0.001). Curve fitting analysis demonstrated an approximate negative linear association between LAAEV and the probability of AF recurrence.ConclusionsAmong patients with early PeAF who successfully underwent their first RFCA, we found that the LAAEV within 24 h before the procedure was independently correlated with the risk of AF recurrence. Notably, this association was negative, indicating that higher LAAEV was associated with a lower risk of AF recurrence.

Pulsed field ablation versus very high-power short-duration radiofrequency ablation in persistent atrial fibrillation: a single-centre experience

Abstract Background Pulsed Field ablation (PFA) has recently emerged as a treatment strategy for patients with atrial fibrillation (AF) and early studies showed promising procedural data and durable pulmonary vein isolation (PVI). However, Very High-Power Short-Duration ablation (VHPSD) has previously reported good efficiency and mid-term outcomes, and only a few works compared these two procedures, especially in patients with persistent AF. Objective Our aim is to compare these different modalities of transcatheter ablation in patients with persistent AF, focusing mainly on some procedural aspects, safety and efficacy. Methods We performed a retrospective observational study enrolling all consecutive patients with persistent AF from September 2021 to May 2023 undergoing transcatheter ablation with PFA or VHPSD. In the first group, PVI was achieved by 2kV pulses with eight applications to each vein using both catheter configurations, while isolation of the posterior wall of the left atrium was achieved by applications through the only "flower" configuration; additional lesions were then added at the operator's discretion. In the second group, PVI was achieved by delivering 90W radiofrequency pulses for 4 seconds in the posterior portions of the left atrium and 50W pulses in the anterior region of the pulmonary veins; additional lesions were occasionally applied. All patients received general anaesthesia or deep sedation with fentanyl and dexmedetomidine. Results A total of 79 patients were included, n = 24 (30%) in the PFA group and N = 55 (70%) in the VHPSD one: early persistent AF (n= 2 [8,3%]; N = 7 [12,7%]), persistent AF (n= 17 [70,8%]; N = 36 [65,5%]), long-standing persistent AF (n= 5 [20,8%]; N = 12 [21,8%]). The two populations showed similar clinical features: CHA2DS2-VASc (median value 3 [0-6] in PFA vs 2 [0-6] in VHPSD, p = 0,10), indexed left atrial volume (mean value 42,6 ml/m² in PFA vs 42,4 ml/m² in VHPSD, p = 0,07) and EF (mean value 55,4% in PFA vs 54,8% in VHPSD, p = 0,33). PVI was successfully achieved in all patients. Nevertheless, the PFA group was associated with shorter procedural duration (92,1 ± 36,9 vs 134,2 ± 36,1 minutes, p = 0,00001) but longer fluoroscopy time (25,5 ± 9,3 vs 17,4 ± 11,5 minutes, p = 0,001); it also showed a lower rate of minor complications (n = 1 [4,2%] vs N = 3 [5,5%], p = 0,057), even though serious complications were not observed in either group. After a median follow-up of 14 (9-26) months, n = 19 [79.2%] in the PFA and N = 43 [78.2%] in the VHPSD population were free of any atrial arrhythmia (p = 0,92). Conclusion PFA allows faster procedures, representing a major advantage for the operator and the patient, who is at lower risk of peri-procedural complications. On the other hand, VHPSD correlates with shorter fluoroscopy times, reducing exposure to ionizing radiation. Finally, the two procedures show similar mid-term efficacy in terms of recurrence-free time.

Intra-procedural three-dimensional rotational angiography in cryoballoon ablation of atrial fibrillation - results from a randomised clinical trial

Abstract Introduction There are a variety of approaches for pre-procedural imaging to facilitate atrial fibrillation (AF) ablation. Intra-procedural three-dimensional rotational angiography (3DRA) is the least employed method and it can be advantageous regarding efficiency and procedural logistics (1). There is an open question whether LA imaging could improve safety and outcomes of cryoballoon (CB) ablation. Purpose The aim of this study is to evaluate possible benefits of 3DRA for procedural success, safety and long-term outcomes of CB ablation. Methods We designed a single-centre unblinded randomised clinical trial and recruited 134 patients (64.2% male, 59.0±11.6 years) with paroxysmal (93.3%) and early-persistent AF. Patients were randomised to no imaging or 3DRA which was performed intra-procedurally after trans-septal puncture. Angiographic images were segmented and overlaid to the fluoroscopy screen to guide the ablation procedure. Patients were scheduled for follow-up visits and twenty-four hour ECG recordings at 3, 6 and 12 months, and every 6 months thereafter. Results Of all the patients recruited, 66 (49.3%) were randomised to 3DRA and angiography was successfully performed in all but one patient (1.5%). Procedure time was significantly longer when 3DRA was performed (86.7±27.8 min compared to 67.2±22.1 in non-3DRA group, p&amp;lt;0.001). Total radiation dose (447.4±485.0 vs. 133.9±166.2 mGy, p&amp;lt;0.001) and contrast administration (131.8±28.3 vs. 40.8±26.5 ml, p&amp;lt;0.001) were significantly higher in the 3DRA group (Table 1). In patients who received no imaging one pulmonary vein (PV) couldn’t be isolated in 2 patients while this occurred in 3 patients in the 3DRA group (OR 1.57, 95% CI 0.25-9.72). Only minor complications were reported in both groups. Five patients developed large haematoma (4 in the 3DRA group vs. 1 in the control group). In each group there were 3 incidents of impending or transient phrenic nerve palsy. During 12 months of follow-up, 88.2% of patients in the control group and 81.8% in the 3DRA group were free of atrial fibrillation (OR 1.67, 95% CI 0.63-4.38). Two patients from the control group developed atrial flutter and were treated by ablation. In total, 10 patients underwent redo procedures (7 in the control and 3 in the 3DRA group respectively) and PV reconnections were detected in 9 cases. Conclusion 3DRA is a safe and efficient intra-procedural imaging method to guide CB ablation of AF. However, it significantly increases procedure duration, total radiation dose and contrast expenditure. In our trial it did not have any impact on the acute success rate of PV isolation or freedom from atrial fibrillation during 12-month follow-up.

A Predictive Model for Recurrence of Atrial Fibrillation Based on P-Wave Duration in Patients with Early Persistent Atrial Fibrillation Who Underwent Radiofrequency Catheter Ablation

Purpose: To construct a predictive model for the recurrence of atrial fibrillation (AF) based on P-wave duration (PWD) in patients with early persistent AF (PeAF) who underwent radiofrequency catheter ablation (RFCA), with the aim of helping clinicians accurately adjust clinical strategies. Methods: Data from patients with early PeAF, who were admitted to the Department of Cardiology at the authors' hospital were collected. Based on predefined inclusion and exclusion criteria, only those who successfully underwent RFCA for the first time were included in the analysis. Pre- and postoperative clinical, echocardiographic, and electrocardiographic data were collected and recorded. Multivariate logistic regression was used to construct a predictive model for AF recurrence based on PWD. The predictive efficacy of each continuous variable and the predictive model were compared using the area under the receiver operating characteristic (ROC) curve. The corresponding nomogram for the predictive model was constructed. Interaction tests were performed to evaluate the predictive efficacy of the model for AF recurrence. Results: A total of 237 patients were enrolled and divided into two groups: recurrence (n = 59); and sinus rhythm (n = 178). PWD was greater and left atrial appendage emptying velocity (LAAV) was lower in the recurrence group; these differences were statistically significant (p &lt; 0.001). The ROC curve for univariate prediction of AF recurrence revealed that the area under the ROC curve (AUC) for PWD and LAAV were 0.7912 and 0.7713, respectively, which were greater than those of other continuous variables. Compared with PWD alone, the multivariate predictive model containing PWD, left ventricular ejection fraction, and LAAV demonstrated no statistically significant difference in AUC (p = 0.0553) but improved the prediction efficiency of correctly reclassifying recurrence rates, net reclassification improvement 14.13% (95% confidence interval: 0.19–28.07%; p = 0.0469). The interaction effect did not significantly alter the effectiveness of the predictive models. Conclusions: The multivariate model based on PWD measured after RFCA demonstrated better predictive efficacy than the univariate model in patients with early PeAF. These results may contribute to evidence supporting the formulation of personalised treatments for patients with AF.

Baseline left atrial low-voltage area predicts recurrence after pulmonary vein isolation: WAVE-MAP AF results.

Electro-anatomical mapping may be critical to identify atrial fibrillation (AF) subjects who require substrate modification beyond pulmonary vein isolation (PVI). The objective was to determine correlations between pre-ablation mapping characteristics and 12-month outcomes after a single PVI-only catheter ablation of AF. This study enrolled paroxysmal AF (PAF), early persistent AF (PsAF; 7 days-3 months), and non-early PsAF (>3-12 months) subjects undergoing de novo PVI-only radiofrequency catheter ablation. Sinus rhythm (SR) and AF voltage maps were created with the Advisor HD Grid™ Mapping Catheter, Sensor Enabled™ for each subject, and the presence of low-voltage area (LVA) (low-voltage cutoffs: 0.1-1.5 mV) was investigated. Follow-up visits were at 3, 6, and 12 months, with a 24-h Holter monitor at 12 months. A Cox proportional hazards model identified associations between mapping data and 12-month recurrence after a single PVI procedure. The study enrolled 300 subjects (113 PAF, 86 early PsAF, and 101 non-early PsAF) at 18 centres. At 12 months, 75.5% of subjects were free from AF/atrial flutter (AFL)/atrial tachycardia (AT) recurrence. Univariate analysis found that arrhythmia recurrence did not correlate with AF diagnosis, but LVA was significantly correlated. Low-voltage area (<0.5 mV) >28% of the left atrium in SR [hazard ratio (HR): 4.82, 95% confidence interval (CI): 2.08-11.18; P = 0.0003] and >72% in AF (HR: 5.66, 95% CI: 2.34-13.69; P = 0.0001) was associated with a higher risk of AF/AFL/AT recurrence at 12 months. Larger extension of LVA was associated with an increased risk of arrhythmia recurrence. These subjects may benefit from substrate modification beyond PVI.

Variants of development and management of patients with persistent atrial fibrillation

The article discusses controversial aspects of modern management of patients with persistent atrial fibrillation (AF). The appearance of a persistent form of AF is mainly a consequence of the processes of fibrosis and electrical restructuring (remodeling) of the atrial tissue. However, the question of the causes and mechanisms of the formation of fibrosis of the atrial tissue is quite debatable. The suggestions of experts regarding the division of persistent AF depending on the features of the arrhythmological anamnesis are presented: the appearance of persistent episodes of AF as a stage of disease progression in patients who previously had a paroxysmal form of arrhythmia, or «persistent AF from the beginning». From the standpoint of predicting the course of the disease and differentiating the management of patients, there are also grounds for distinguishing «early persistent AF» in cases where the duration of the episode is more than 7 days and less than 3 months, as well as «late persistent AF» lasting from 3 to 12 months. From the point of view of evidence-based medicine, the paradigm of managing patients with persistent AF is shifting towards the fastest and most aggressive fight for sinus rhythm. Changes in favor of the rhythm control strategy have largely occurred due to the rapid development of catheter treatment technologies. Its effectiveness is generally lower than in paroxysmal AF, which is due to more pronounced structural remodeling of the atria, as well as electrophysiological changes in the myocardium against the background of a prolonged episode of arrhythmia. At the same time, research into new techniques using modification of the arrhythmia substrate is ongoing.

Abstract 13614: Why the Pulmonary Veins: Linking Atrial 4D Flow MRI Derived Biomechanics and Pulmonary Vein Electrical Remodeling

Introduction: Pulmonary veins (PV) often trigger atrial fibrillation (AF). However, the underlying mechanism is not completely understood and predicting who benefits from PV isolation (PVI) is imprecise. We propose that arrhythmogenic PV remodeling may be related to myocardial mechanical function. Flow-encoded cardiac magnetic resonance imaging (4D Flow MRI) based computational modeling enables assessment of left atrial (LA) hemodynamics and biomechanics. In this pilot study, 4D Flow MRI derived parameters were compared to invasive electroanatomic mapping (EAM) and hemodynamics to explore potential non-invasive evaluation of PV remodeling and arrhythmogenicity. Methods: Patients (5 female, 4 male, age 64±6.7 years) with paroxysmal or early persistent AF scheduled for PVI were enrolled. All patients underwent 4D Flow MRI prior to ablation (in sinus rhythm) and intraprocedural EAM (Carto) with atrial pacing under baseline conditions and after increasing LA pressure with a fluid bolus. Non-invasive measures included peak and average velocity (m/s) and net flow (ml/cycle) in each vein (n=36) and averaged for each patient. Apparent conduction velocities in the proximal, mid and distal portions of each vein were calculated under baseline and stretch conditions from EAM. Change in LA pressure per volume bolus, a measure of atrial stiffness, was compared to non-invasive parameters. Results: In patients with LA stiffness greater than median, peak velocities (0.77±0.12 vs. 0.62±0.24 m/s, p=0.26) and net flows were higher (17±2.0 vs. 14.0±2.4 ml/cycle, p=0.06). Patients with paroxysms of spontaneous arrhythmias during mapping had higher LA stiffness (11.5±5.7 vs. 7.6±5.3 mmHg/L, p=0.30). There was a significant correlation between non-invasive average velocity and apparent conduction velocity in the proximal portion of the vein (R=-0.36, p=0.03) and the suggestion of a correlation between increasing peak flow velocity and decreased apparent conduction velocity under stretch conditions compared to baseline (R=-0.24, p=0.16). Conclusions: Our findings may indicate a link between atrial hemodynamics, electrical remodeling and PV arrhythmogenicity. Additional studies are warranted to confirm and better define these findings.

Low voltage area as a predictor of recurrence after a single pulmonary vein isolation procedure: results of the WAVE-MAP AF study

Abstract Background/Introduction Pulmonary vein isolation (PVI) is a recommended approach for atrial fibrillation (AF) ablation procedures. Substrate modification beyond PVI has mixed results but identifying and ablating low voltage zones and ablating those targets in addition to PVI may be beneficial. Electroanatomic mapping is critical to identify subjects that may require further substrate modification. Low voltage area may be predictive of optimal treatment approach. Purpose This was a prospective, multicenter, interventional study of a high-density grid-style mapping catheter (HD Grid) to characterize left atrial low voltage substrate during sinus rhythm (SR) and AF and identify associations with 12 month recurrence rates after a single de novo radiofrequency (RF) ablation using a PVI only approach. Methods This study (NCT03882021) enrolled 300 subjects at 18 centers in Europe and Israel. Subjects underwent de novo RF ablation for paroxysmal AF (PAF) (N=113), early persistent AF (PsAF; AF sustained 7 days to 3 months) (N=86) or non-early PsAF (AF sustained &amp;gt;3 months to 12 months) (N=101). High density voltage maps were collected with HD Grid. Two pre-ablation maps, in SR and AF, were created for each subject (N=196) followed by PVI only ablation. Low voltage area (using cutoffs of 0.1 mV to 1.5 mV) was investigated in SR and AF. Follow up visits were at 3, 6 and 12 months post-ablation, with a 24-hour Holter monitor at 12 months. A Cox proportional hazards model was used to identify associations between mapping data and 12 month AF/atrial flutter (AFL)/atrial tachycardia (AT) recurrence after a single PVI procedure. Results At 12 months, 75.5% of subjects were free from AF/AFL/AT recurrence. On average, PsAF subjects had more low voltage area than PAF subjects in SR and AF. However, while univariate analysis found no correlation between recurrence and PAF or PsAF diagnosis (p=0.1261), those with recurrence had a significantly larger percent left atrial low voltage area under 0.5 mV with simultaneous orthogonal bipole wave configuration (HDW) in both AF (p=0.0011) and SR (p=0.0210) than those without recurrence. Using HDW, low voltage area (identified as &amp;lt;0.5 mV) greater than 28% of the left atrium in SR (HR: 4.82, 95% CI: 2.08–11.18, p=0.0003) and greater than 72% in AF (HR: 5.66, 95% CI: 2.34–13.69, p=0.0001) were associated with a higher risk of AF/AFL/AT recurrence at one year. Conclusion(s) Using a standard cutoff of 0.5 mV, a larger percent low voltage area was associated with increased risk of recurrence in both SR and AF. Future analyses will explore optimal low voltage cutoffs and thresholds predictive of recurrence that may necessitate additional substrate modification beyond PVI. Data supporting different practices is still evolving and there is an open question whether LA imaging could improve safety and outcomes of CB ablation. Purpose The aim of this study is to evaluate possible benefits of 3DRA for procedural success, safety and long-term outcomes of CB ablation. Methods We designed a single-centre unblinded randomised clinical trial and recruited 101 patients (65.3% male, 59.3±11.5 years) with paroxysmal (92%) and early-persistent AF. Patients were randomized to no imaging or 3DRA which was performed intra-procedurally after trans-septal puncture. Angiographic images were segmented and overlaid to the fluoroscopy screen to guide the ablation procedure. All patients are followed for 1 year and data concerning procedural characteristics, safety outcomes and AF recurrence are being collected. Results Of all the patients recruited, 48 (47%) underwent 3DRA. In patients who received no imaging (non-3DRA group) one PV couldn’t be isolated while in 3DRA group isolation of one PV could not be achieved in 3 patients (OR 3.47, 95% CI 0.35-34.51). The occurrence of left common PV, an anatomical variation that could have potential to influence results, was similar in both groups (25,4% in non-3DRA and 25.0% in 3DRA group). Procedure time was significantly longer when 3DRA was performed (87.4±27.7 min compared to 69.9±23.0 in non-3DRA group, p&amp;lt;0.001). Total radiation dose (148.2±177.7 vs 387.7±203.8 mGy, p&amp;lt;0.001) and contrast administration (42.5±27.1 vs 133.8±26.8 ml, p&amp;lt;0.001) were significantly lower in the control group. Only minor complications were reported in both groups. Three patients developed large haematoma (2 in 3DRA group vs 1 in control group). In each group there was one incident of transient phrenic nerve palsy. Conclusion 3DRA is a safe and efficient intra-procedural imaging method to guide CB ablation for AF. However, it significantly increases procedure duration, total radiation dose and contrast expenditure. In our trial it did not have any impact on the acute success rate of PV isolation. One-year follow up data is still being collected and will subsequently be presented.

Safety and acute performance of paroxysmal and early persistent atrial fibrillation ablation using temperature-controlled, very high power, short duration catheter with a new radiofrequency generator

Background/Introduction The temperature-controlled contact force radiofrequency (RF) ablation catheter (QDM) has 6 thermocouples to maintain target catheter tip temperature and 3 tip microelectrodes for high local signal resolution. It has an enhanced irrigation profile to reduce char formation. Three prospective multicenter trials demonstrated safety and effectiveness of the QDM catheter using very high-power, short duration (vHPSD; 90W/4s), conventional power (CP; ≤50W) or combined modes. These studies were performed with an older RF generator. A new generator was developed to support the full suite of legacy, temperature controlled and multichannel ablation catheters, but has not yet been formally tested in a clinical setting. Purpose Evaluate safety and acute performance of the QDM catheter with the new 90W-enabled generator. Methods This prospective, multicenter study (4 Canadian sites) used vHPSD as primary mode for Pulmonary Vein (PV) isolation with overlapping 3mm lesion tags for anterior, inferior and roof of left atrium. Adjacent, non-overlapping lesions were used posteriorly to avoid esophageal heating. CP mode was used at operator discretion for thicker atrial tissue, touch ups, and non-PV triggers (Figure). Primary endpoint was confirmed entrance block in all PVs after adenosine and/or isoproterenol challenge. Acute procedural and safety data were assessed. Systematic char evaluation was performed. Results Effectiveness analysis included 38/40 subjects with mean 110-day follow up (mean age, 61.7 ± 9.5 years; 63.2% male). Hypertension (47.4%) and congestive heart failure (15.8%) were the most common comorbidities. Fourteen subjects were treated with version V0 of the generator, and 24 subjects were treated using version V1c which included further software adjustments to the power titration, and current response to optimize performance. Combination vHPSD/CP workflow was used for PV isolation in 28 (75.7%) subjects and vHPSD was used solely in 10. A non-study catheter was used in 1 subject during connectivity troubleshooting. Primary endpoint of entrance block confirmation was achieved in 37/38 subjects and in all 37 subjects treated with study catheter only. No incidences of char or device-related adverse events were reported. There were no noted differences in the endpoints evaluated between the V0 and V1c groups. Median (Q1/Q3) RF application times for ablating PVs was 13.0 min (9.0/16.0) for the entire cohort. Median (Q1/Q3) time to achieve PV isolation in vHPSD-only group was 9 (7/12) min, and 14 (11/18) min, respectively. Conclusion(s) Consistent with data from prior 3 clinical studies, QDM catheter with the new 90W-enabled generator demonstrated the acute safety and effectiveness of temperature-controlled AF ablation with short PV ablation time and no char observed.

Investigating the association between P wave duration and atrial fibrillation recurrence after radiofrequency ablation in early persistent atrial fibrillation patients

BackgroundIn the present study, we aimed to explore the association between P wave duration, as the measured time from the start point of the P wave to the end point, and atrial fibrillation recurrence after transcatheter radiofrequency ablation in patients with early persistent atrial fibrillation. MethodsPatients with early persistent atrial fibrillation who underwent the first radiofrequency ablation procedure were retrospectively analyzed. The electrocardiographic, echocardiographic and clinical data of the enrolled patients before and after operation were collected and recorded. After adjusting confounding factors and performing stratified analysis, the association between the P wave duration and the atrial fibrillation recurrence of patients with early persistent atrial fibrillation after radiofrequency ablation was explored. ResultsThe proportions of atrial fibrillation recurrence of the low, medium, and high P wave duration groups were 6.4%, 19.7%, and 47.0%, respectively. After potential confounding factors were adjusted, the risk of atrial fibrillation recurrence gradually increased with the increase of P wave duration (odds ratio: 1.093, 95% confidence interval: 1.063–1.124, p < 0.001). This trend was statistically significant (odds ratio: 1.099, 95% confidence interval: 1.052–1.149, p < 0.001), especially in comparison of high vs. low (odds ratio: 16.99, 95% confidence interval: 4.75–60.78, p < 0.001). Curve fitting showed that there was a linear and positive association between the P wave duration and the risk of atrial fibrillation recurrence. This association was consistent in different subgroups based on gender, drinking, history of smoking, hypertension, diabetes mellitus, peripheral artery disease, stroke or transient ischemia attack, hyperlipidemia, heart failure, and heart rate, suggesting that there was no significant interaction between different grouping parameters and the association (p for interaction range = 0.217–0.965). ConclusionsIn patients with early persistent atrial fibrillation who underwent radiofrequency ablation procedure for the first time and converted to sinus rhythm, the P wave duration within 72 h after the procedure was independently associated with the risk of atrial fibrillation recurrence, and such association was linear and positive.

Long-term outcome of targeted therapy of underlying conditions in patients with early persistent atrial fibrillation and heart failure: data of the RACE 3 trial.

AimsThe Routine vs. Aggressive risk factor driven upstream rhythm Control for prevention of Early persistent atrial fibrillation (AF) in heart failure (HF) (RACE 3) trial demonstrated that targeted therapy of underlying conditions improved sinus rhythm maintenance at 1 year. We now explored the effects of targeted therapy on the additional co-primary endpoints; sinus rhythm maintenance and cardiovascular outcome at 5 years.Methods and resultsPatients with early persistent AF and mild-to-moderate stable HF were randomized to targeted or conventional therapy. Both groups received rhythm control therapy according to guidelines. The targeted group additionally received four therapies: angiotensin-converting enzyme inhibitors and/or angiotensin receptor blockers (ARBs), statins, mineralocorticoid receptor antagonists (MRAs), and cardiac rehabilitation. The presence of sinus rhythm and cardiovascular morbidity and mortality at 5-year follow-up were assessed. Two hundred and sixteen patients consented for long-term follow-up, 107 were randomized to targeted and 109 to conventional therapy. At 5 years, MRAs [76 (74%) vs. 10 (9%) patients, P < 0.001] and statins [81 (79%) vs. 59 (55%), P < 0.001] were used more in the targeted than conventional group. Angiotensin-converting enzyme inhibitors/ARBs and physical activity were not different between groups. Sinus rhythm was present in 49 (46%) targeted vs. 43 (39%) conventional group patients at 5 years (odds ratio 1.297, lower limit of 95% confidence interval 0.756, P = 0.346). Cardiovascular mortality and morbidity occurred in 20 (19%) in the targeted and 15 (14%) conventional group patients, P = 0.353.ConclusionIn patients with early persistent AF and HF superiority of targeted therapy in sinus rhythm maintenance could not be preserved at 5-year follow-up. Cardiovascular outcome was not different between groups.Trial registration numberClinicaltrials.gov NCT00877643.

Utility of hot-balloon-based pulmonary vein isolation under balloon surface temperature monitoring: First clinical experience.

A new hot balloon system that registers balloon surface temperature (BST) during energy delivery is now available for clinical use in Japan. This study sought to investigate the utility of BST measurement for achievement of pulmonary vein isolation (PVI) by a single-shot energy delivery strategy during hot balloon ablation (HBA). We applied and tested the system in 30 consecutive patients undergoing HBA for paroxysmal or early-persistent atrial fibrillation (AF). We also performed real-time PV potential monitoring using a circular catheter. Acute PVI was achieved with single hot balloon shots in 88% (106/120) of the PVs. Real-time BSTs and PV potentials were recorded in all cases. Mean BST at documentation of PVI was 49.4°C, and acute reconnections were observed in most cases (86%, 12/14) in which the single-shot technique was ineffective. Time-to-isolation (TTI) (23.1 ± 8.7 s vs. 36.3 ± 9.3 s, p < .01) and median BST (59.9 ± 2.6°C vs. 55.7 ± 1.9°C, p < .01) differed significantly between cases in which PVI was achieved (vs. those in which PVI was not achieved). Multivariable analysis revealed strong association between both TTI and median BST and acute PVI. The best median BST cutoff value for achieving PVI with a single shot was >58.7°C (sensitivity 67.0%, specificity 100%). Our data suggest that real-time BST monitoring during energy applications is useful for predicting achievement of acute PVI by a single shot during HBA.

Antiarrhythmic drugs in patients with early persistent atrial fibrillation and heart failure: results of the RACE 3 study.

AimsMaintaining sinus rhythm in patients with persistent atrial fibrillation (AF) is challenging. We explored the efficacy of class I and III antiarrhythmic drugs (AADs) in patients with persistent AF and mild to moderate heart failure (HF).Methods and resultsIn the RACE 3 trial, patients with early persistent symptomatic AF and short history of mild to moderate HF with preserved or reduced left ventricular ejection fraction (LVEF) were randomized to targeted or conventional therapy. Both groups received AF and HF guideline-driven treatment. Additionally, the targeted-group received mineralocorticoid receptor antagonists, statins, angiotensin-converting enzyme inhibitors and/or receptor blockers, and cardiac rehabilitation. Class I and III AADs could be instituted in case of symptomatic recurrent AF. Eventually, pulmonary vein isolation could be performed. Primary endpoint was sinus rhythm on 7-day Holter after 1-year. Included were 245 patients, age 65 ± 9 years, 193 (79%) men, AF history was 3 (2–6) months, HF history 2 (1–4) months, 72 (29.4%) had HF with reduced LVEF. After baseline electrical cardioversion (ECV), 190 (77.6%) had AF recurrences; 108 (56.8%) received class I/III AADs; 19 (17.6%) flecainide, 36 (33.3%) sotalol, 3 (2.8%) dronedarone, 50 (46.3%) amiodarone. At 1-year 73 of 108 (68.0%) patients were in sinus rhythm, 44 (40.7%) without new AF recurrences. Maintenance of sinus rhythm was significantly better with amiodarone [n = 29/50 (58%)] compared with flecainide [n = 6/19 (32%)] and sotalol/dronedarone [n = 9/39 (23%)], P = 0.0064. Adverse events occurred in 27 (25.0%) patients, were all minor and reversible.ConclusionIn stable HF patients with early persistent AF, AAD treatment was effective in nearly half of patients, with no serious adverse effects reported.

Role of the Ambulatory Assessed Apnea-Hypopnea Index for Predicting Recurring Atrial Fibrillation After Ablation Therapy

Catheter ablation is an effective treatment for atrial fibrillation (AF). Obstructive sleep apnea (OSA) is a known risk factor for recurrent AF. The apnea-hypopnea index (AHI) is a measurement tool to screen patients for OSA. We sought to evaluate if the ambulatory assessed AHI is associated with AF recurrence following AF catheter ablation. 187 patients with paroxysmal (n = 155) or early persistent (n = 32) AF presenting for catheter ablation were included in the study. AHI was determined prior to ablation using an ambulatory screening device. All patients underwent pulmonary vein isolation (PVI). In patients with early persistent AF (17%) additional ablation of complex fractionated atrial electrograms (CFAE) was performed. Clinical follow-up was available after 3 and 12 months including 7-day Holter-ECG. All 187 patients (60.3 ± 11.4 years, 64.2% male) completed the 3 months follow-up and 170 patients the 12 months follow-up. A pathological AHI ≥15 was found in 45/187 (24.1%) patients. Additional CFAE did not differ between patients with an AHI ≥15 and AHI <15 (p = 0.663). After 3 months, 12/41 (29.3%) patients with AHI ≥15 showed recurrent AF compared to 24/146 (16.4%) patients with AHI <15 (p = 0.066). After 12 months, AHI ≥15 was associated with a significant higher rate of AF recurrence of 47.4% (18/38) versus 26.5% (35/132) in patients with AHI <15 (p = 0.014). In the logistic regression analysis AHI ≥15 was an independent predictor of recurrent AF at 12 months (p = 0.011). In conclusion, ambulatory assessed AHI ≥15 is associated with increased risk for AF recurrence following catheter ablation. OSA screening should be performed in AF patients as it might influence catheter ablation success.

A predictive model using left atrial function and B-type natriuretic peptide level in predicting the recurrence of early persistent atrial fibrillation after radiofrequency ablation.

AimA predictive model using left atrial function indexes obtained by real‐time three‐dimensional echocardiography (RT‐3DE) and the blood B‐type natriuretic peptide (BNP) level was constructed, and its value in predicting recurrence in patients with early persistent atrial fibrillation (AF) after radiofrequency ablation was explored.MethodsA total of 228 patients with early persistent AF who were scheduled to receive the first circular pulmonary vein ablation (CPVA) were enrolled. Clinical data of patients were collected: (1) The blood BNP level was measured before radiofrequency ablation; (2) RT‐3DE was used to obtain the left atrial (LA) time‐volume curve; (3) The clinical characteristics, BNP level and LA function parameters were compared, and logistic regression was used to construct a prediction model with combined parameters; (4) The receiver operating characteristic (ROC) curve was used to examine the diagnostic efficacy of the model.Results(1) 215 patients with early persistent AF completed CPVA and the follow‐up. After 3–6 months of follow‐up, the patients were divided into sinus rhythm group (160 cases) and recurrence group (55 cases); (2) The recurrence group showed higher minimum LA volume index, diastolic ejection index, and preoperative BNP (all p ≤ .001), while the sinus rhythm group exhibited higher expansion index (PI) and left atrial appendage peak emptying velocity (p ≤ .001); (3) In univariate analysis, BNP level had the best diagnostic performance in predicting the recurrence of AF(AUC = 0.703). We constructed a model based on LA function and BNP level to predict the recurrence of persistent AF after CPVA. This combined model was better than BNP alone in predicting the recurrence of persistent AF after CPVA (AUC: 0.814 vs. 0.703, z = 2.224, p = .026).ConclusionThe combined model of LA function and blood BNP level has good predictive value for the recurrence of early persistent AF after CPVA.

Thoracoscopic surgical ablation or catheter ablation for patients with atrial fibrillation? A systematic review and meta-analysis of randomized controlled trials.

There is an urgent need to understand the difference in the influence of thoracoscopic surgical ablation (TSA) and catheter ablation (CA) on clinical outcomes in patients with atrial fibrillation (AF). This meta-analysis of randomized controlled trials aimed to examine the efficacy and safety of TSA versus CA in patients with AF. Databases including EMBASE, Clinical Trials, PubMed and Cochrane Central Registered Control System were screened for the retrieval of articles. A direct meta-analysis of TSA versus CA was conducted. The I2 test analysis was performed to evaluate heterogeneity. The Begg-Mazumdar test and the Harbord-Egger test were used to detect publication bias. The primary efficacy outcome was freedom from atrial tachyarrhythmia, while the primary safety outcome was severe adverse event (SAE) occurrence. Of the 860 identified articles, 6, comprising 466 participants, were finally included. The rate of freedom from AT was higher in the TSA group (75%) than in the CA group (57.1%) (odds ratio 0.41; 95% confidence interval 0.2-0.85; P = 0.02; I2 = 57%). A larger number of SAEs were observed in the TSA group than in the CA group (odds ratio 0.16; 95% confidence interval 0.006-0.46; P = 0.0006; I2 = 44%). The result of the subgroup analysis of 3 studies that enrolled AF patients without a history of ablation showed that the incidence of AT was comparable in both arms. The ablation procedure and hospitalization durations were longer in the TSA arm. In our study, TSA was associated with better efficacy but a higher rate of SAEs compared to CA. In addition, TSA did not show better efficacy results as the first invasive procedure in the sub-analysis of patients with paroxysmal AF or early persistent AF. Therefore, doctors should recommend either TSA or CA to patients with AF after due consideration of the aforementioned findings.

High-density mapping in patients undergoing ablation of atrial fibrillation with the fourth-generation cryoballoon and the new spiral mapping catheter.

To assess the value of high-density mapping (HDM) in revealing undetected incomplete pulmonary vein isolation (PVI) after the fourth-generation cryoballoon (CB4G) ablation compared to the previous cryoballoon's versions. Consecutive patients with paroxysmal or early-persistent atrial fibrillation (AF) undergoing CB ablation as the index procedure, assisted by HDM, were retrospectively included in this study. A total of 68 patients (52 males; mean age: 60 ± 12 years, 58 paroxysmal AF) were included, and a total of 272 veins were mapped. Fourth-generation cryoballoon with the new spiral mapping catheter (SMC) was used in 35 patients (51%). Time to PVI was determined in 102/132 (77%) and in 112/140 (80%) veins during second-generation cryoballoon/third-generation cryoballoon (CB2G/CB3G) and CB4G ablation, respectively (P = 0.66). There was a statistically significant difference in terms of discrepancy rate between the SMC and the mini-basket catheter in PV detection after CB4G and CB2G/CB3G ablation(1.4% vs. 7.6%; P = 0.01). A total of 57 patients (84%) remained free of symptomatic AF during a mean follow-up of 9.8 ± 4.6 months. High-density mapping after cryoballoon ablation using CB4G and the new SMC identifies incomplete PVI, not detected by the new SMC, in a significantly lower proportion of veins compared to HDM performed after the other generation CB ablation.

Anatomic predictors of recurrence after cryoablation for atrial fibrillation: a computed tomography based composite score.

Effective pulmonary vein isolation (PVI) with cryoablation depends on adequate occlusion of pulmonary veins (PV) by the cryoballoon and is therefore likely to be affected by PV and left atrial (LA) anatomical characteristics and variants. Thus, the objective of this study was to investigate the effect of LA and PV anatomy, evaluated by computed tomography (CT), on acute and long-term outcomes of cryoablation for atrial fibrillation (AF). Fifty-eight patients (64.72+9.44years, 60.3% male) undergoing cryoablation for paroxysmal or early persistent AF were included. Pre-procedural CT images were analyzed to evaluate LA dimensions and PV anatomical characteristics. Predictors of recurrence were identified using regression analysis. 60.3% of patients had two PVs on each side with separate ostia, whereas 29.3% and 10.3% had right middle and left common PVs, respectively. The following anatomic characteristics were found to be independent predictors of recurrence: right superior PV ostial max:min diameter ratio > 1.32, left superior PV ostial max:min diameter ratio > 1.2, right superior PV antral circumference > 69.1mm, right inferior PV antral circumference > 61.38mm, right superior PV angle > 22.7°. Using these factors, LA diameter and right middle PV, a scoring model was created for prediction of "unfavorable" LA-PV anatomy (AUC = 0.867, p = 0.000009, score range = 0-7). Score of ≥ 4 predicted need for longer cryoenergy ablation (p = 0.039) and more frequent switch to radiofrequency energy (p = 0.066) to achieve PVI, and had a sensitivity of 83.3% and specificity of 82.5% to predict clinical recurrence. CT-based scoring system is useful to identify "unfavorable" anatomy prior to cryo-PVI, which can result in procedural difficulty and poor outcomes.

An ablation index operator-independent approach to improve efficacy in atrial fibrillation ablation at 24-month follow-up: a single center experience.

Since the introduction of catheter ablation as a mainstream treatment for atrial fibrillation (AF), several technical improvements have been put forward. In this contest, Ablation Index (AI) is an accurate ablation quality marker by incorporating power, delivery time, contact force (CF), and catheter stability in a weighted formula. The aim of our study is to evaluate the efficacy of AI-guided AF ablation over 24month follow-up. We evaluated 72 consecutive patients with drug-refractory paroxysmal (66.7%) and early-persistent AF (33.3%) undergoing AI-guided ablation, compared to 72 propensity-matched control patients who underwent CF-guided procedure. All procedures were performed by three skilled operators. Data concerning procedural characteristics and long-term freedom from AF recurrence were analyzed. At 24-month follow-up, Kaplan-Meier curves of AF recurrence were significantly lower in AI group than in CF group (15.5% vs. 30.6%; p 0.042). These findings were confirmed in a sub analysis regardless of the continued use of antiarrhythmic drugs in the follow-up (42.2% in AI-guided group and 66.7% in CF-guided group, p 0.004). At 24-month follow-up, a positive trend in the decrease of arrhythmia recurrences was observed in AI-guided ablation for all operators. AI-guided ablation results more effective than CF-guided ablation as demonstrated by a lower incidence of AF recurrences regardless of the use of antiarrhythmic drugs in the follow-up. Each operator seems to improve the long-term success using an AI-guided ablation, thus showing both the efficacy and the reproducibility of this approach.
