The curative effect of early use of enteral immunonutrition in postoperative gastric cancer: a meta-analysis.

The clinical efficacy of early enteral immunonutrition in postoperative gastric cancer patients is uncertain. The aim of this study is to assess its clinical efficacy. Use of the search engine database incuding the PubMed, the Cochrane library, Web of Science, Medline, and Embase, which present comparative studies of early efficacy of enteral immune nutrition and standard enteral nutrition after gastric cancer operation. We used the RevMan 5.3 software to meta-analyze the CD4, CD8, CD4/CD8, the length of hospitalization, postoperative complications, and postoperative weight loss. A total of 6 randomized control articles were included in this study, which comprised 606 patients. Among them, there were 308 cases of early postoperative enteral nutrition in patients with gastric cancer, and 298 cases were in standard enteral nutrition. Early postoperative enteral immune nutrition on seventh day of the CD4, CD8, and CD4/CD8 value greatly compared with standard enteral nutrition, CD4 (WMD=2.71, CI: 2.71~7.79, P<0.01), CD8 (WMD=2.71, CI: 2.71~7.79, P<0.01), CD4/CD8 (WMD=0.29, CI: 0.29~0.65, P<0.01). Postoperative complications occurred in patients with gastric cancer after operation (OR=0.46, CI 0.46 to 0.46, P<0.01), the postoperative weight loss (WMD=1.00, CI 1.00 to 2.82, P<0.01), length of hospitalization, and the value of early postoperative enteral immune nutrition group were lower than the group pf early standard enteral nutrition. The treatment which could improve the clinical efficacy of patients should be considered for future management of postoperative gastric cancer.

Is early feeding after major gastrointestinal surgery a fashion or an advance? evidence-based review of literature

Early enteral nutrition (EN) after major digestive surgery has been receiving increasing attention. Supporting evidence has not been clear. This evidence-based review traces the development of early EN and analyses whether it is indeed an advance. We performed a PubMed search in October 2009 with the key words enteral nutrition, early feeding, and gastrointestinal surgery. Our emphasis was on earliest studies documenting the benefits or adverse effects of EN, comparative studies, documenting the benefits or adverse effects of EN, comparative studies, and randomized controlled trials. Thirty-one results were returned from which 17 were included for evaluation (1979-2009). Fifteen papers concluded that early EN was beneficial. In general, patients put on early EN and immunonutrition postoperatively seemed to have decreased hospital stay, decreased complication rates, decreased treatment and hospital costs, and even decreased morbidity and mortality; however, judicious use has been suggested. One study did not recommend early enteral feeding in well-nourished patients at low risk of nutrition-related complications and another suggested that immunonutrition is not beneficial and should not be used routinely. Early EN has been safely given after major digestive surgery since 1979. It benefits patients undergoing major gastrointestinal (GI) surgeries, with reduction in perioperative infection, better maintainance of nitrogen balance, and shorter hospital stay. Early EN may be superior to total parenteral nutrition (TPN). However, TPN is perhaps better tolerated in the immediate postoperative period. Early enteral immunonutrition should be used only in malnourished and in transfused patients. Early EN after major digestive surgery is an old advance that is now in fashion.

We compared early parenteral nutrition (PN) and early enteral immunonutrition (iEN) in critically ill patients, distinguishing those with and without severe sepsis or septic shock (SS) on admission to intensive care units (ICUs). Multicenter, randomized, unblinded clinical trial in 33 Italian general ICUs. The study included 326 patients, 287 of whom did not have SS on ICU admission. Eligibility criteria excluded the two tails in the spectrum of critical conditions, i.e., patients either too well or too ill. Of the patients recruited 160 were randomized to iEN (142 without SS) and 166 to PN (145 without SS). Patients were randomized to two arms: early iEN or early PN. Primary endpoint was 28-day mortality for all patients and the occurrence of SS during ICU stay for patients admitted without such condition. While 28-day mortality did not differ between iEN and PN (15.6% vs. 15.1%), patients without SS who received iEN had fewer episodes of severe sepsis or septic shock (4.9% vs. 13.1%). ICU length of stay was 4 days shorter in patients given iEN. Compared to parenteral nutrition iEN appears to be beneficial in critical patients without severe sepsis or septic shock. Parenteral nutrition in these patients should be abandoned, at least when enteral nutrition can be administered, even at an initial low caloric content.
