Ultrasound in the Modern Management of the Diabetic Foot Syndrome: A Multipurpose Versatile Toolkit.

Ultrasound (US) is a noninvasive and versatile technology that in recent years found acceptance in almost all the medical specialties, with diagnostic and interventional applications. In the diabetic foot syndrome (DFS), US found specific indications mainly in the screening, quantification, and follow-up of the vascular component of the pathology, but also in the study of the deformities and structural modifications induced by neuropathy and in the diagnosis and surgical management of infections, especially those that induce anatomical changes, like abscesses and fasciitis. This review will summarize all these application of US, giving special attention to the vascular aspects, and on the predominant role that US gained in recent times to guide the indication to revascularization, on the new standardized approach to the study of the arterial tree of the limb and the foot, the so-called duplex ultrasound arterial mapping, which significantly increased the utilization of US to plan the revascularizations in this complex pathology. Outside the vascular fields, the diagnosis of neuropathy and infection and the intraoperative use of US in the surgical management of abscesses and fasciitis will be discussed, leaving the last part to the new and interesting applications of US in the management of DFU, a field that is still in evolution, offering new possibilities to the health care professionals involved in the management of these chronic wounds. The variety of applications both in diagnostic and operative fields makes US a rather versatile technology-a toolkit-that should have a special place among those at reach of the specialists of DFS care.

Ten-year technical and clinical outcomes in TransAtlantic Inter-Society Consensus II infrainguinal C/D lesions using duplex ultrasound arterial mapping as the sole imaging modality for critical lower limb ischemia

The aim of this study was to evaluate duplex ultrasound arterial mapping (DUAM) as the sole imaging modality when planning for bypass surgery (BS) and endovascular revascularization (EvR) in patients with critical limb ischemia for TransAtlantic Inter-Society Consensus (TASC) II C/D infrainguinal lesions. This was a retrospective review evaluating the accuracy of DUAM as the sole imaging tool in determining patient suitability for BS vs EvR. Primary outcomes were the sensitivity and specificity of DUAM compared with intraoperative digital subtraction angiography. Secondary outcomes were procedural, hemodynamic, and clinical outcomes, amputation-free survival, and freedom from major adverse clinical events. From 2002 to 2012, a total of 4783 patients with peripheral arterial disease were referred, of whom 622 critical limb ischemia patients underwent revascularization for TASC C and D lesions (EvR: n = 423; BS: n = 199). Seventy-four percent of EvR and 82% of BS were performed for TASC D (P = .218). The DUAM showed sensitivity of 97% and specificity of 98% in identifying lesions requiring intervention. Of the 520 procedures performed with DUAM alone, there was no difference regarding the number of procedures performed for occlusive or de novo lesions (EvR: 65% and 71%; BS: 87% and 78%; P = .056). Immediate clinical improvement to the Rutherford category ≤3 was 96% for EvR and 97% for BS (P = .78). Hemodynamic success was 79% for EvR and 77% for BS (P = .72). Six-year freedom from binary restenosis was 71.6% for EvR and 67.4% for BS (P = .724). Six-year freedom from target lesion revascularization was 81.1% for EvR and 70.3% for BS (P = .3571). Six-year sustained clinical improvement was 79.5% for EvR and 66.7% for BS (P = .294). Six-year amputation-free survival was 77.2% for EvR and 74.6% for BS (P = .837). There was a significant difference in risk of major adverse clinical events between EvR and BS (51% vs 70%; P = .034). Only 16.4% of patients required magnetic resonance angiography, which tended to overestimate lesions with 84% agreement with intraoperative findings. Six-year binary restenosis was 71% for DUAM procedures compared with 55% for magnetic resonance angiography procedures (P = .001), which was solely based on the prospective modality. The DUAM epitomizes a minimally invasive, economically proficient modality for road mapping procedural outcome in BS and EvR. It allows for high patient turnover with procedural and clinical success without compromising hemodynamic outcome. The DUAM is superior to other available modalities as the sole preoperative imaging tool in a successful limb salvage program.

The Use of Duplex Ultrasound Arterial Mapping (DUAM) and Preoperative Diagnostics in Patients with Atherosclerotic Ischaemia of Lower Extremities

In spite of the fact duplex Doppler ultrasonography is recognized as an scanning technique equivalent to other imaging methods for preoperative diagnostics of the lower extremity arteries, in many centres its use is limited to screening. was to the determine usefulness of arterial duplex mapping for preoperative diagnostics in patients with peripheral artery occlusive disease (PAOD). In 2008-2009, 103 patients with PAOD symptoms (129 limbs) were examined in a prospective, non-randomized study. DUAM was performed in all limbs, DSA in 90, CT-angio in 29, and revascularisation procedures in 116 limbs. Problems with completing DUAM occurred in 10,1% of cases, including 1.5% of examinations of no diagnostic value. Consistency of DUAM and DSA results was found in 94.6% of cases, and consistency of DUAM results and the range of actually performed ranges was found in 97.8% of cases. Consistency of DUAM and CT-angio results was recorded in 68.0% of cases. The comparison between individual examinations and the actually performed procedures showed consistency of 96.5% for DUAM and 72.4% for CT-angio. DUAM usefulness for detecting and defining lesions in lower extremity arteries is similar to that of DSA and CT-angio. This examination may be successfully used in preoperative diagnostics, particularly for intravascular procedures, which makes possible to reduce exposition to radiation and minimalize diagnostics costs.

A Prospective Feasibility Study of Duplex Ultrasound Arterial Mapping, Digital-Subtraction Angiography, and Magnetic Resonance Angiography in Management of Critical Lower Limb Ischemia by Endovascular Revascularization

Duplex ultrasound arterial mapping (DUAM) allows precise evaluation of peripheral vascular disease (PVD). However, magnetic resonance angiography (MRA) and digital-subtraction angiography (DSA) are the diagnostic tools used most frequently prior to intervention. Our aim was to compare clinical pragmatism, hemodynamic outcomes, and cost-effectiveness when using DUAM alone compared to DSA or MRA as preoperative assessment tools for endovascular revascularization (EvR) in critical lower limb ischemia (CLI). From 2002 through 2005, 465 patients were referred with PVD. Of these, 199 had CLI and 137 required EvR. Preoperative diagnostic evaluation included DUAM (n = 41), DSA (n = 50), or MRA (n = 46). EvR was aortoiliac in 27% of cases and infrainguinal in 73%. Patients were assessed at day 1, 6 weeks, 3 months, and 6 months. Composite end points were relief of rest pain, ulcer/gangrene healing, and increase in perfusion pressure, as measured by ankle-brachial index (ABI) and digital pressures. Patency by DUAM, limb salvage, morbidity, mortality, length of stay, and cost-effectiveness were compared between groups using nonparametric t-test, analysis of variance, and Kaplan-Meier analysis. The three groups were comparable in terms of age, sex, comorbidity, and Society for Vascular Surgery/International Society of Cardiovascular Surgery clinical classification. Six-month mean improvement in ABI in the DUAM group was comparable to that in the DSA group (P = 0.25) and significantly better than that in the MRA group (P < 0.05). Six-month patency rates for the DUAM group were comparable to those in the DSA group (P = 0.68, relative risk [RR] = 0.74, 95% confidence interval [CI] 0.18-2.99) and superior to that in the MRA group (P = 0.022, RR = 0.255, 95% CI 0.09-0.71). Length of hospital stay was lower in the DUAM group compared with the DSA group (P < 0.0001) and the MRA group (P = 0.0003). The cost of DUAM is lower than that of both DSA and MRA. DUAM accurately identified the total number of target lesions for revascularization; however, MRA overestimated it. Our results indicate that DUAM is outstanding when compared with other available modalities as a preoperative imaging tool in a successful EvR program. DUAM is a minimally invasive preoperative evaluation for EvR and offers precise consecutive data with patency and limb salvage rates comparable to EvR based on DSA and superior to MRA. We believe that our feasibility study has established DUAM as an economically proficient primary modality for investigating patients with CLI that significantly shortens length of hospital stay.

Acute Lower Limb Ischemia: The Value of Duplex Ultrasound Arterial Mapping (DUAM) as the Sole Preoperative Imaging Technique

Contrast arteriography (CA) is the gold standard preoperative imaging modality for patients with chronic and acute lower limb ischemia. We have previously shown that high-quality DUAM can safely replace CA in patients with chronic ischemia. The goal of this study was to investigate whether DUAM can also be used effectively in the setting of acute ischemia. From January 1998 to February 2001, 68 patients were admitted to our institution with 87 instances of acute lower limb(s) ischemia and underwent 87 operations. There were 34 men and 34 women whose age ranged from 51 to 95 years (mean 72 +/- 12.5). There were 44 cases of acute arterial occlusions and 43 cases of bypass graft thromboses. In the former group the most proximal occluded site based upon duplex was the aorta in 1 case, common iliac in 4 cases, external iliac in 15 cases, and infrainguinal arteries in 24 cases. In the latter group, there were 4 suprainguinal grafts, 24 bypasses to the popliteal artery, and 15 bypasses to infrapopliteal arteries. All patients had DUAM as their initial diagnostic study. The duplex protocol varied according to the pulse exam. In patients with a good femoral pulse but absent popliteal pulse, attempts were made to visualize the ipsilateral femoral-popliteal segment and the proximal third of the infrapopliteal arteries. This was extended to the pedal arteries in cases of proximal occlusion. When the femoral pulse was absent the protocol included visualization of the distal aorta, bilateral iliac, and common femoral arteries. This exam was extended into the deep and superficial femoral-popliteal segments in cases of proximal occlusion. None of these cases had preoperative or prebypass CA. Intraoperative arterial pressures to confirm the adequacy of the inflow tract and completion arteriography to assess the runoff were performed in 78% of the cases at the end of the procedure. This initial experience suggests that high-quality DUAM may replace CA in patients with lower limb ischemia. DUAM provides a reliable assessment of the inflow and outflow arteries even in very low-flow situations. In addition, DUAM can identify the cause of the arterial occlusion, thereby making therapy more effective and less time consuming.

Comparison of Magnetic Resonance Angiography (MRA) and Duplex Ultrasound Arterial Mapping (DUAM) prior to infrainguinal arterial reconstruction

Objective: the purpose of this study is to compare Magnetic Resonance Angiography (MRA) to Duplex Ultrasound Arterial Mapping (DUAM) and intraoperative findings to determine the clinical, accuracy of MRA for planning lower extremity revascularization procedures. Methods: forty-two patients who underwent lower extremity revascularization procedures had both MRA and DUAM evaluations. These data were analyzed retrospectively and two distinct endpoints were considered. First, we compared the MRA and DUAM findings for aorto-iliac, femoral-popliteal and infra-popliteal segments to intraoperative findings and evaluated the degree to which they agreed. Second, if there was a disagreement between imaging modalities, it was established whether a change in operative procedure would have resulted. Results: MRA and DUAM findings agreed in 26 of 31 cases (83%) of aorto-iliac segments, in 25 of 31 cases (81%) of femoral-popliteal segments, and in 16 of 21 cases (76%) of infra-popliteal segments. In total, DUAM agreed with intraoperative findings in 98% of cases while MRA agreed in 82% (p<.001). Disagreement between intraoperative findings and DUAM lead to an alternate surgical procedure in only one case (2%) while disagreement with MRA lead to a different procedure in 38% of cases (p<.001). Conclusions: these data show that MRA is not yet adequate to replace conventional angiography and is less accurate that DUAM. Further improvements are necessary before MRA can be used as the sole modality for formulation of a pre-operative plan for lower extremity revascularization.Eur J Vasc Endovasc Surg 25, 139–146 (2003)

Lower Extremity Revascularization without Preoperative Contrast Arteriography: Experience with Duplex Ultrasound Arterial Mapping in 485 Cases

This study reviews our experience with duplex ultrasound arterial mapping (DUAM) for preoperative evaluation in 466 patients (262 men) who underwent 485 lower extremity revascularization procedures from January 1, 1998 to May 30, 2001. Preoperative imaging consisted of DUAM alone in 449 procedures and DUAM and contrast angiography (CA) in 36. An attempt to image from the distal aorta to the pedal arteries was made in all the patients. The selection of optimal inflow and outflow bypasses anastomotic sites was based on a schematic drawing following DUAM examination. Inflow disease was also assessed by intraoperative pressure gradient (IPG) between the distal anastomosis and radial arteries, and completion arteriography of the runoff vessels was obtained, which was correlated with the preoperative findings. Indications for surgery were severe claudication in 91 (19%) limbs, tissue loss in 197 (40%), rest pain in 113 (23%), acute ischemia in 46 (10%), popliteal aneurysm in 18 (4%), superficial femoral artery aneurysm in 1, abdominal aortic aneurysm with claudication in 1, and failing graft in 18 (4%). Age ranged from 30 to 97 years (mean 72 +/- 12 (SD) years) and risk factors such as diabetes, hypertension, use of tobacco, coronary artery disease, and end-stage renal disease were present in 45%, 45%, 44%, 44%, and 13% of the patients, respectively. One hundred twenty-one (25%) limbs had at least 1 previous ipsilateral revascularization. The mean DUAM time was 66 +/- 20 (SD) min (30-150 min). Additional preoperative imaging was deemed necessary in 36 cases due to extensive ulcers, edema, severe arterial wall calcification, and very poor runoff. The distal anastomosis was to the popliteal artery in 173 cases and to the tibial and pedal arteries in 255. Inflow procedures to the femoral arteries, embolectomy, thrombectomy, balloon angioplasty, and patch angioplasty accounted for the remaining 57 cases. Overall, 6-, 12-, and -24- month secondary patency rates were 86%, 80%, and 66%, respectively. This early experience shows that high-quality arterial ultrasonography performed by a highly skilled vascular technologist may represent an alternative to conventional arteriography for patients in need of lower extremity revascularization. Because of limitations inherent to the technique and very poor runoff observed on ultrasonographic examination, additional preoperative imaging procedure's are needed for certain patients.

Lower-extremity Revascularisation Without Preoperative Contrast Arteriography in 185 Cases: Lessons Learned with Duplex ultrasound Arterial Mapping

Purpose: we have previously reported our experience with lower-extremity duplex-ultrasound arterial mapping (DUAM) compared to contrast arteriography (CA) to predict lower-extremity bypass sites. The present study evaluates arterial revascularisation procedures for chronic limb ischaemia based on DUAM. Materials and methods: from January 1998 to July 1999, 195 patients (128 men, 67 women) underwent 211 lower-extremity revascularisation procedures based on DUAM. Indications for surgery were tissue loss, severe claudication, rest pain and popliteal aneurysm in 57%, 25%, 14% and 4% of the limbs, respectively. The mean age was 72±12 years and risk factors such as diabetes, hypertension, tobacco use, coronary artery and end-stage renal disease were present in 53%, 58%, 53%, 50% and 12% of the patients, respectively. Previous revascularisation procedures had been performed in 23% of the limbs. Preoperative evaluation consisted of DUAM alone (185) or of a combination of DUAM and CA (29 limbs). CA was deemed necessary due to a combination of technical difficulties that jeopardised adequate sonographic imaging and presence of disadvantaged run-off for medico-legal reasons. DUAM consisted of direct imaging of all major arteries from the distal aorta to the pedal circulation. Optimal inflow and outflow bypass anastomotic sites were selected according to a diagram based on DUAM. Adequacy of the inflow was additionally assessed by common-femoral-artery waveform and confirmed by intraoperative pressure measurements. Post-bypass CA was obtained to verify patency of the run-off. Results: DUAM procedure time averaged 75±26 min. For patients who underwent only DUAM, the distal anastomosis was to the popliteal artery in 91 cases and to tibial or pedal arteries in 58 cases. Distal anastomosis was proximal to a significant lesion in two cases that required jump grafts. Cumulative patency rates at 1 and 3 months for popliteal bypasses were 96% and 90%, and for infrapopliteal bypasses 90% and 83%, respectively. Inflow procedures to the femoral artery, patch and balloon angioplasties accounted for the remaining 40 cases. Four primary amputations were performed after CA confirmed DUAM findings. Conclusions: contrary to general belief, these data show that high-quality arterial ultrasonography represents a safe alternative to preoperative CA, even for infrapopliteal bypasses. This non-invasive approach may be especially useful for patients with contrast allergy or impaired renal function.

The use of duplex ultrasound arterial mapping as an alternative to conventional arteriography for primary and secondary infrapopliteal bypasses

Background: For more than 1 decade, duplex ultrasonography has been investigated as a potential replacement for invasive contrast arteriography (CA) prior to lower extremity revascularization. While some authors advocate the reliability of this methodology for infrainguinal imaging, revascularizations based on duplex ultrasonography alone are seldom reported. Our initial experience with duplex ultrasound arterial mapping (DUAM) for primary and secondary infrapopliteal bypasses performed without preoperative CA is reported in this series. Methods: Twenty-eight infrapopliteal bypasses based on DUAM alone (26) and combined DUAM and CA (2) were performed. Indications for surgery were tissue loss in 22 cases and rest pain in 6. Eight patients (29%) had at least 1 previous ipsilateral revascularization. A schematic anatomical drawing based on DUAM helped the selection of optimal inflow and outflow bypass sites. Bypasses originated from the femoral arteries in 17 cases, external iliac in 3, and popliteal in 4, and from a previous graft in 4. The distal anastomosis was to the dorsalis pedis in 12 cases, posterior tibial in 7, anterior tibial in 6, and peroneal in 3. Results: DUAM revealed unobstructed inflow in 24 cases, significant iliac stenosis in 1, and in 3 cases imaging was not possible owing to obesity. A pressure gradient of 15 mm Hg between donor and radial artery confirmed the stenosis diagnosed by DUAM, and was successfully treated with balloon angioplasty and stent placement. A single infrapopliteal runoff artery was identified by DUAM in 22 cases, 2 in 4 cases, and 3 in 2 cases. Preoperative CA was performed in 2 instances because of severe arterial calcification. Completion arteriography matched the run-off status predicted by DUAM in 25 out of 26 cases (96%). DUAM missed a significant anterior tibial artery stenosis that required a distal graft extension. One-, 3-, and 6-month patency rates were 96%, 85%, and 62%, respectively. Limb salvage rate was 86%. Conclusions: This early experience shows that DUAM may be a safe alternative to invasive conventional arteriography for the majority of patients (89%) presenting with critical ischemia and in need of infrapopliteal bypasses.

Values and Limitations of Duplex Ultrasonography as the Sole Imaging Method of Preoperative Evaluation for Popliteal and Infrapopliteal Bypasses

The purpose of this study was to evaluate whether duplex ultrasound arterial mapping (DAM) can reliably replace standard arteriography (SA) in the preoperative assessment of candidates for popliteal and infrapopliteal bypasses. This is the report of a two-phase study undertaken at our institution from July 1997 to May 1998. Carefully performed DAM averted the need for preoperative SA (51 of 58 procedures). The results of this study can be used as an impetus to initiate larger, multicenter protocols to further evaluate this newer noninvasive approach for patients presenting with severe lower-limb ischemia.
